ABSTRACT The primates that inhabit the rainforest surrounding the city of Manaus (Amazonas, Brazil) have long been recognised as potentially important reservoirs of emerging and re-emerging infectious diseases (ERIDs). PCR amplification of filarial sequences from wild-caught Simulium oyapockense has been used to incriminate potentially important Amazon-region ERID bridge vectors by showing they had previously fed on non-human primates. The broader use of filarial parasite sequences for the incrimination of biting insects as potentially important zoonotic disease vectors is limited by a paucity of primate-derived filarial parasite reference sequences which can be matched to the PCR amplified sequences obtained from insect-vector vectors. Here we have used shotgun sequencing to obtain reference data from an adult Dipetalonema gracile parasite which was found infecting a wild pied tamarin (Saguinus bicolor) in a peripheral region of Manaus. We report the parasite´s complete mitochondrial genome (which is 13,647 base pairs in length), 894,846 base pairs of its Wolbachia genome and 6,426 base pairs of its ribosomal DNA locus (spanning from the start of its 18S subunit to the end of its 28S subunit). Despite being critically endangered, S. bicolor is commonly encountered around the periphery of Manaus and in urban forest fragments. The reported sequences may be a useful reference tool for identifying ERID bridge vectors and potentially provide some insights into the amount and the nature of contact between primate pathogen reservoirs and the residents of Manaus.

RESUMO Os primatas que habitam a floresta tropical ao redor da cidade de Manaus (Amazonas, Brasil) há muito são reconhecidos como reservatórios potencialmente importantes de doenças infecciosas emergentes e reemergentes. Sequências de DNA de parasitas filariais detectadas por PCR em amostras de Simulium oyapockense foram usadas para demonstrar que eles haviam se alimentado anteriormente de primatas não humanos e, dessa maneira, incriminar vetores-ponte da região amazônica. O uso mais amplo de detecção de parasitas filariais para a incriminação de vetores-ponte é limitado por uma escassez de sequências referência de parasitas filarias obtidas de hospedeiros. Aqui nós usamos o sequenciamento tipo shotgun para obter dados de referência de um parasita adulto Dipetalonema gracile encontrado infectando um sauim-de-coleira, Saguinus bicolor no entorno de Manaus. Relatamos o genoma mitocondrial completo do parasita (que tem 13.647 pares de bases de comprimento), 894.846 pares de bases de seu genoma de Wolbachia e 6.426 pares de bases de seu locus de DNA ribossômico (desde o início de sua subunidade 18S até o final de sua subunidade 28S). Apesar de criticamente ameaçado, S. bicolor é comumente encontrado no entorno de Manaus e em fragmentos florestais urbanos. As sequências relatadas podem ser uma ferramenta de referência útil para identificar vetores ponte e potencialmente fornecer algumas informações sobre o contato entre reservatórios de patógenos de primatas e os moradores de Manaus.

The identification and analysis of documentary fraud is always a challenge for forensic science. Document analysis has proven to be an important branch of forensics in elucidating the authenticity of documents. The development and incorporation of luminescent inks in authentic documents have proved to be an excellent security feature. This paper purposes the use of a possible luminescent ink marker for anti-counterfeiting applications, aiming to create a document encoding process that is simple, robust, sensitive, and non-destructive. Since luminescent inks markers provide a visual, chemical, and spectral signature, and can be easily detected by using a UV lamp, the aid of unsupervised chemometric tools makes it possible to differentiate the luminescent markers inserted in the ink. Unsupervised models of principal component analysis (PCA) and K-mean were successful in correctly associating marked inks with their respective pure markers, while a supervised classification model based on partial least squares discriminant analysis (PLS-DA) correctly classified all samples from the prediction set and the blind test samples. ABSTRACT Objective: This study aims to investigate the ability of the six-minute walk distance (6MWD) as a prognostic marker for midterm clinical outcomes three months after coronary artery bypass grafting (CABG), to identify possible predictors of fall in 6MWD in the early postoperative period, and to establish the percentage fall in early postoperative 6MWD, considering the preoperative baseline as 100%. Methods: A prospective cohort of patients undergoing elective CABG were included. The percentage fall in 6MWD was assessed by the difference between preoperative and postoperative day (POD) five. Clinical outcomes were evaluated three months after hospital discharge. Results: There was a significant decrease in 6MWD on POD5 compared with preoperative baseline values (percentage fall of 32.5±16.5%, P<0.0001). Linear regression analysis showed an independent association of the percentage fall of 6MWD with cardiopulmonary bypass (CPB) and preoperative inspiratory muscle strength. Receiver operating characteristic curve analysis revealed that the best cutoff value of percentage fall in 6MWD to predict poorer clinical outcomes at three months was 34.6% (area under the curve = 0.82, sensitivity = 78.95%, specificity = 76.19%, P=0.0001). Conclusion: This study indicates that a cutoff value of 34.6% in percentage fall of 6MWD on POD5 was able to predict poorer clinical outcomes at three months of follow-up after CABG. Abstract Cnesterodon hypselurus is a small fish that has a restricted distribution in southern Brazil, including headwaters of the Tibagi and Itararé river basins (Upper Paraná River). This study reported C. hypselurus in a headwater of Cinzas River basin, where there were no previous records of this species, and employed microsatellite loci and mitochondrial haplotypes in a population genetic analysis. A total of 57 specimens was analyzed, including 30 from Cinzas River basin, 25 from Itararé River basin and two from Tibagi River basin. Results indicated low genetic diversity levels (HE = 0.334 and h = 0.246) for the sample from Cinzas River, suggesting reflections of a founder effect after the species had dispersed from one watershed to another, possibly by headwater captures. Since different populations were detected between the Cinzas and Itararé rivers (DEST = 0.248, P-value < 0.05) and other occurrence sites are still unknown in the Cinzas River basin, the data herein have great relevance and should be taken into account in future management and conservation actions, as well as in evolutionary studies of C. hypselurus.

Resumo Cnesterodon hypselurus é um pequeno peixe que possui distribuição restrita no sul do Brasil, incluindo cabeceiras das bacias dos rios Tibagi e Itararé (alto rio Paraná). Este estudo reportou C. hypselurus na cabeceira da bacia do rio das Cinzas, onde não havia registros prévios desta espécie, e empregou locos microssatélites e haplótipos mitocondriais em uma análise genética de populações. Um total de 57 espécimes foi analisado, incluindo 30 do rio das Cinzas, 25 da bacia do rio Itararé e dois da bacia do rio Tibagi. Os resultados indicaram baixos níveis de diversidade genética (HE = 0,334 e h = 0,246) para a amostra do rio das Cinzas, sugerindo reflexos de um efeito fundador após a espécie ter dispersado de uma bacia para a outra, possivelmente a partir de captura de cabeceiras. Uma vez que diferentes populações foram detectadas entre os rios das Cinzas e Itararé (DEST = 0,248, valor de P < 0,05) e que outros pontos de ocorrência ainda são desconhecidos na bacia do rio das Cinzas, os dados do presente estudo mostram grande relevância e deveriam ser considerados em futuras ações de manejo e conservação, bem como em estudos evolutivos de C. hypselurus.

Resumo Barragens são umas das principais ameaças para peixes potamódromos Neotropicais, causando bloqueios de rotas migratórias e perdas de áreas de reprodução em um número crescente de sistemas de drenagem. Neste cenário, dados atuais defendem que a manutenção de tributários de fluxo livre em bacias hidrográficas fragmentadas seria uma importante estratégia na conservação de peixes migradores, tanto em situações de panmixia quanto em cenários com diferentes estoques coexistindo, tal como já relatado para Prochilodus lineatus. Este estudo analisou locos microssatélites e haplótipos mitocondriais em amostras de P. lineatus obtidas em diferentes anos ao longo da bacia do rio das Cinzas, no reservatório de Capivara (maior reservatório na calha principal do rio Paranapanema, alto rio Paraná) e no rio Tibagi, todos conectados por trechos livres, visando investigar as contribuições da bacia do rio das Cinzas na dinâmica populacional de peixes migradores na região do reservatório de Capivara. Ambos os marcadores detectaram altos níveis de diversidade genética e indicaram diferentes estoques de P. lineatus ao longo da bacia do rio das Cinzas, sugerindo alguma estruturação genética temporal, bem como a mistura destes estoques na amostra do reservatório de Capivara e de outros pontos, corroborando as relações de estoques neste reservatório e seus tributários. Abstract Dams are one of the main threats to Neotropical potamodromous fish, causing blockages of migratory routes and loss of breeding areas in an increasing number of drainage systems. In this scenario, current data argue that the maintenance of free-flowing tributaries along fragmented watersheds would be an important strategy to the migratory fish conservation, both in panmixia situations and in scenarios with different coexisting stocks, as already reported for Prochilodus lineatus. This study analyzed microsatellite loci and mitochondrial haplotypes in samples of P. lineatus obtained in different years along the Cinzas River basin, as well as in the Capivara Reservoir (largest Reservoir in the Paranapanema main channel, upper Paraná River) and in the Tibagi River, all connected by free stretches, aiming to investigate the contributions of the Cinzas River basin to the population dynamics of migratory fish in the Capivara reservoir region. The cariogenic processes are mainly caused by the bacterium Streptococcus mutans (S. mutans) and consist of the demineralization of the tooth that occurs when the acid production overcomes the natural repair or if a problem occurs in the last one. In this work, we performed the synthesis of twenty-one 1,4-naphthoquinones tethered to 1,2,3-1H-triazoles (8a-8k and 9a-9j), antibacterial evaluation against the S. mutans in vitro and the acute toxicity of the better ones in vivo. We observed strong inhibition results in the disc diffusion test ranging, the halos of inhibitions, from 18.66 (± 0.57) to 29 (± 2.64) mm, and good values in the minimum inhibitory concentration (5 to 50 μg), for the compounds 9e, 9h, 9i and 9j: Furthermore, they do not have a cytotoxic effect at the concentrations tested. Besides that, in the in vivo test, they show some slight alteration in the histopathological analyses and the biochemistry. Thus, we found four potential candidates to become instruments for the treatment of cavities.

RESUMO O objetivo deste trabalho foi prospectar fontes de resistência ao nematoide-das-galhas Meloidogyne enterolobii em espécies silvestres de Solanum e híbridos com potencial para serem utilizados como porta-enxertos para solanáceas cultivadas. Vinte e três acessos de sete espécies de Solanum, bem como 35 híbridos de dois cruzamentos interespecíficos, foram caracterizados quanto à resistência a M. enterolobii. Os experimentos foram conduzidos em casa de vegetação em Brasília-DF, Brasil, em delineamento de blocos inteiramente casualizados, com quatro repetições de uma planta por vaso inoculada com ovos do nematoide, avaliadas quanto às variáveis nematológicas: índice de massa de ovos, índice de galhas, número de ovos por grama de raiz e fator de reprodução. Todos os acessos de Solanum torvum, S. paludosum e S. paniculatum apresentaram alta resistência. No caso de S. crinitum, S. macrocarpon e S. sisymbriifolium, as reações foram variadas, com seis de sete, dois de cinco e um de três acessos resistentes, respectivamente. Vinte e três dos 24 híbridos interespecíficos entre S. stramonifolium var. inerme e S. stramonifolium, e cinco híbridos interespecíficos de S. stramonifolium e S. aethiopicum gr. gilo, entre onze, também foram resistentes. Esses resultados são de interesse prático para o controle do nematoide-das-galhas, uma vez que solanáceas cultivadas resistentes a M. enterolobii são de difícil obtenção por meio de melhoramento convencional. Por isso, a identificação de resistência em espécies de Solanum compatíveis com as espécies cultivadas pode ser importante para permitir o uso como porta-enxertos.

ABSTRACT The objective of this work was to prospect sources of resistance to the root-knot nematode Meloidogyne enterolobii in wild Solanum species and hybrids to be used as potential rootstocks for cultivated Solanaceae. Twenty-three accessions of seven Solanum species, as well as 35 interspecific hybrids of two crosses, were characterized for their resistance to M. enterolobii. The experiments were conducted in a greenhouse in Brasília-DF, Brazil, in a completely randomized block design with four replications of one plant per pot. Plants were inoculated with egg masses and evaluated for the nematological variables egg mass index, galls index, number of eggs per root gram and reproduction factor. Solanum torvum, S. paludosum, and S. paniculatum presented high levels of resistance. Six among seven accessions of S. crinitum were resistant, as well as two accessions among five of S. macrocarpon, and one among three of S. sisymbriifolium. Except one, the 24 interspecific hybrids among S. stramonifolium var. inerme and S. stramonifolium, as well as five interspecific hybrids of S. stramonifolium and S. aethiopicum gr. gilo, among eleven were resistant. These results are relevant, since cultivated Solanaceae species of economic importance resistant to M. enterolobii are difficult to obtain by conventional breeding. Therefore, the identification of resistance in Solanum species compatible for grafting with cultivated species can be important to enable their use as rootstocks for disease control.

Chikungunya virus (CHIKV) is a re-emerging disease caused by an alphavirus of the Togaviridae family. Since its first description in 1952, the disease has spread worldwide, affecting populations in both tropical and temperate countries. To date, there is no licensed vaccine or specific pharmacological treatment. Therefore, there is an increasing urgency in developing new antiviral drugs capable of specifically inhibiting viral replication. In the present work, we report the synthesis and antiviral activity evaluation of nineteen naphthoquinone derivatives, containing a sulfonamide or sulfonate group. Cell viability assays indicated a low toxic potential for all tested compounds and inhibitory assays against CHIKV identified five compounds with potent activity. The compounds were also evaluated for their virucidal potential, and the results demonstrated that compound 11a exhibited a virucidal effect higher than 70% in the treatment with 20 µM. Furthermore, in silico studies were performed to predict the antiviral drug targets.

Resumo Serrasalmus maculatus é uma espécie de piranha que, a despeito de ser abundante em ambiente de reservatório, possui poucas informações sobre seus aspectos parasitológicos e dieta. Assim, o presente estudo objetivou documentar a fauna parasitária e a dieta de S. maculatus em um reservatório brasileiro. Além disso, foram realizadas duas revisões literárias para a região Neotropical, registrando a fauna parasitária, já associada a S. maculatus e a ocorrência dos gêneros parasitários identificados neste estudo, registrados em outros peixes Characiformes em outros sistemas aquáticos. Foram coletados 31 hospedeiros com redes de espera entre agosto de 2014 e setembro de 2016. Serrasalmus maculatus apresentou hábito alimentar piscívoro e comunidade componente parasitária com baixa riqueza, incluindo dois táxons de monogenéticos, Anacanthorus lepyrophallus e Mymarothecium sp.; não foram observados endohelmintos. Dados da revisão da literatura, juntamente com os achados deste estudo, mostraram que S. maculatus, na região Neotropical, abriga 25 táxons de helmintos, sendo monogenéticos o grupo de parasitos mais prevalente, e o Brasil o país com mais relatos de parasitos. Estes resultados fornecem informações sobre as relações entre dieta, o comportamento social e a fauna parasitária de S. maculatus e sobre os padrões de distribuição e infecção das taxas de parasitos observadas.

Abstract Serrasalmus maculatus is a species of piranha which, despite being abundant in a reservoir environment, has few studies related to its parasitological and diet aspects. Thus, we aimed to document the parasitic fauna and diet of the S. maculatus in a hydroelectric reservoir in Brazil. In addition, we perform two literature reviews for the Neotropical region, recording the parasitic fauna already associated with S. maculatus and the occurrence of parasite genera identified in this study parasitizing Characiformes from other aquatic systems. Thirty-one hosts were collected with gillnets, from August 2014 to September 2016. Serrasalmus maculatus had a piscivorous feeding habit and a low richness parasitic component community, including two taxa of monogeneans, Anacanthorus lepyrophallus and Mymarothecium sp.; no endohelminths were observed. Data from the literature review, together with the findings of the study, showed that S. maculatus in the Neotropical region harbors 25 helminth taxa, with the monogenean being the most prevalent parasitic group and Brazil is the country with the most reports of the parasitic genera. These findings provide information on the relationships between diet, social behavior, and parasitic fauna of S. maculatus and on the patterns of distribution and infection of the observed parasite rates.

RESUMO Os estuários hipersalinos são comumente encontrados na região semiárida da costa nordestina brasileira. No entanto, a utilização desses estuários pelos primeiros estágios de vida dos peixes é pouco documentada. O objetivo deste estudo foi avaliar os efeitos de fatores ambientais sobre o ictioplâncton e descrever sua variação espacial e temporal em dois estuários (Tubarão e Casqueira). As amostragens foram diurnas em quatro excursões realizadas em dois meses da estação seca (novembro e dezembro de 2017) e dois meses da estação chuvosa (maio e julho de 2018). Um total de 423 larvas representando 30 espécies (14 famílias) e 2.762 ovos (cinco famílias) foram coletadas. Embora as estruturas das comunidades fossem semelhantes em ambos os estuários, houve fortes diferenças de variação sazonal, com maiores abundâncias observadas durante a estação seca. A baixa precipitação foi o principal fator ambiental que afetou o ictioplâncton em ambos os estuários. Outros fatores, como profundidade, temperatura da água e largura do canal, também foram preditoras para a distribuição de ovos e larvas. O ictioplâncton foi dominado por espécies marinhas dependentes em ambos os estuários, representando 76% dos indivíduos. A presença de estágios iniciais de espécies tipicamente marinhas sugere que ambientes hipersalinos fornecem áreas de berçário importantes e desempenham um papel essencial para algumas espécies de peixes costeiros.

ABSTRACT Hypersaline estuaries are commonly found in the Semiarid Northeast Brazilian coast. However, the utilization of these estuaries by early life stages of fishes is less well documented. This study aimed to evaluate the effects of environmental factors on the ichthyoplankton, and to describe their spatial and temporal variation in two estuaries (Tubarão and Casqueira). The sampling were diurnally in four excursions carried out during two dry-season months (November and December 2017) and two rainy-season months (May and July 2018). A total of 423 larvae representing 30 species (14 families) and 2,762 eggs (five families) were collected. Although the community structures were similar in both estuaries, there was strong seasonal variation, with higher abundances observed during the dry season. Low rainfall was the primary environmental driver affecting ichthyoplankton in both estuaries. Other environmental factors, such as depth, water temperature, and channel width, were also predictor variables for the distribution of eggs and larvae. The ichthyoplankton was dominated by marine estuarine-dependent species in both estuaries, accounting for 76% of individuals. The presence of the early life stages of typically marine species suggests that hypersaline environments provide important nursery areas and play an essential role for some coastal fish species.

Abstract Regular exercise is beneficial to health. This study evaluated the effects of moderate and intense physical exercise modalities on intradermal infection by Staphylococcus aureus in a murine model. Mice that practiced moderate exercise had lower bacterial load on lymph nodes and less inflammatory infiltrate in dermis. They presented greater weight, however, less amount of epididymal fat: the weight was increased while they had fat diminished. A positive correlation was observed between lipid content and bacterial load in mice trained at moderate intensity. Animals that were under high intensity exercises presented superior bacterial load on the lymph nodes, increased neutrophil count and circulating lymphocytes, and had leukocyte recruitment to the dermis augmented, when compared to the ones in moderate exercise. These findings suggest that moderate physical activity modulates the immune response in dermal infection caused by S. aureus in a murine model.

RESUMO: Dirofilaria immitis é um parasito de coração e de grandes vasos que acomete principalmente cães domésticos, também causador de uma zoonose reemergente nos últimos anos. O objetivo deste trabalho foi determinar a prevalência, fatores associados e áreas geográficas de D. immitis em cães na cidade de Sousa, Paraíba, Nordeste do Brasil. Para isso, foram selecionados e avaliados 320 cães, sendo 160 domiciliados e 160 errantes, provenientes dos 32 bairros da cidade. Foi realizado o exame clínico, coleta de sangue e dados epidemiológicos de cada animal. O ambiente foi observado visualmente quanto as condições de saneamento no momento de avaliação. Para pesquisa de microfilárias, foram realizados três métodos: esfregaço sanguíneo capilar (ESC), periférico (ESP) e teste de Knott-modificado, associado ao diagnóstico morfométrico das microfilárias. Os dados foram submetidos a estatísticas uni e multivariadas para observação de fatores de risco e avaliação qualitativa dos exames. Obteve-se prevalência de 17,5% (56/320) de animais positivos para D. immitis, sendo 25% (40/160) errantes e 10% (16/160) domiciliados. Dos 32 bairros avaliados, em 24 foram encontrados cães positivos, sendo Angelim e Doutor Zezé, os bairros com maior percentual 1,56% (5/320) cada. Somente as categorias de alterações cardíacas (OR 6,231 [1.539-25.236]) e cães errantes (OR 2,463 [1.281-4.735]) demonstraram potencial fator de risco para infecção. Dos 56 animais positivos, 28 apresentaram positividade nos três testes, e outros 28 demonstraram variância entre métodos e/ou entre lâminas confeccionadas. Não foram observados outros filarídeos. Considerou-se que a cidade de Sousa apresenta prevalência significativa de infecção por D. immitis, sendo necessário estabelecer formas de controle e profilaxia, para reduzir os riscos da transmissão para animais, como também para humanos.

ABSTRACT: Dirofilaria immitis is a heart and large vessel parasite that mainly affects domestic dogs and has shown a re-emerging zoonosis in recent years. The objective of this study was to determine the prevalence, associated factors, and geographic areas of D. immitis in dogs in the city of Sousa, Paraíba, Northeast Brazil. A total of 320 dogs were selected and evaluated, 160 domiciled and 160 wandering, from the 32 districts of the city. Clinical examination, blood collection, and epidemiological data retrieval were performed for each animal. The sanitation conditions of the environment were visually observed at the time of evaluation. Three methods were used to investigate the morphometric diagnosis of microfilariae: capillary blood smear (ESC), peripheral blood smear (ESP), and modified Knott test. The data were subjected to univariate and multivariate statistics for the observation of risk factors and qualitative assessment of the examinations. Of the 17.5% (56/320) of animals testing positive for D. immitis, 25% (40/160) were wandering and 10% (16/160) were domiciled dogs. Positive dogs were found in 24 of the 32 neighborhoods evaluated, with Angelim and Doutor Zezé having the highest percentages of 1.56% (5/320) each. Only the categories of cardiac alterations (OR 6.231 [1,539-25,236]) and stray dogs (OR 2.463 [1,281-4,735]) demonstrated potential risk factors for infection. Of the 56 positive animals, 28 were positive in the three tests, and another 28 showed variance between methods and/or between prepared slides. No other filaridae were observed. The city of Sousa is considered to have a significant prevalence of infection by D. immitis, and forms of control and prophylaxis are required to reduce the risks of transmission to animals, as well as to humans.

ABSTRACT Objectives: To evaluate the impact of COVID-19 on clinical and surgical practice, educational activities, health and lifestyle behavior of Brazilian urology residents. Materials and Methods: A web-based survey was sent to 468 Brazilian urology residents from postgraduate years (PGY) 3 to 5 to collect data on clinical practice and training after 4 months of COVID-19. We also assessed health-related and behavior changes, rate of infection by SARS-CoV-2, deployment to the front line of COVID-19, residents’ concerns, and access to personal protective equipment (PPE). Results: Massive reductions in elective and emergency patient consultations, diagnostic procedures and surgeries were reported across the country, affecting PGY 3 to 5 alike. Most in-person educational activities were abolished. The median damage to the urological training expected for 2020 was 6.0 [3.4 – 7.7], on a scale from 0 to 10, with senior residents estimating a greater damage (P< 0.001). Educational interventions developed included online case-based discussions, subspeciality conferences and lectures, and grand rounds. Most senior residents favored extending residency to compensate for training loss and most younger residents favored no additional training (p< 0.001). Modifications in health and lifestyle included weight gain (43.8%), reduced physical activity (68.6%), increased alcoholic intake (44.9%) and cigarette consumption (53.6%), worsening of sexual life (25.2%) and feelings of sadness or depression (48,2%). Almost half were summoned to work on the COVID-19 front-line and 24.4% had COVID-19. Most residents had inadequate training to deal with COVID-19 patients and most reported a shortage of PPE. Residents’ concerns included the risk of contaminating family members, being away from residency program, developing severe COVID-19 and overloading colleagues. Conclusions: COVID-19 had a massive impact in Brazilian urology residents´ training, health and lifestyle behavior, which may reflect what happened in other medical specialties. Studies should confirm these findings to help developing strategies to mitigate residents’ losses.

Resumo Fundamento: A incidência de reestenose da artéria coronária após o implante de um stent não farmacológico é mais baixa que na angioplastia com balão; no entanto, ainda apresenta altas taxas. Objetivo: O objetivo deste estudo foi identificar novos indicadores de risco para reestenose de stent usando ultrassonografia das carótidas que, em conjunto com indicadores já existentes, ajudariam na escolha do stent. Métodos: Realizamos um estudo prospectivo transversal incluindo 121 pacientes consecutivos com doença arterial coronariana que foram submetidos à intervenção coronária percutânea com angiografia nos 12 meses anteriores. Após os casos de reestenose de stent serem identificados, os pacientes foram submetidos à ultrassonografia de carótidas para avaliar a espessura da camada íntima média e placas ateroscleróticas. Os dados foram analisados por regressão múltipla de Cox. O nível de significância foi p<0,05. Resultados: A idade mediana dos pacientes foi de 60 anos (1º quartil = 55, 3º quartil = 68), e 64,5% dos pacientes eram do sexo masculino. A angiografia coronária mostrou que 57 pacientes (47,1%) apresentaram reestenose de stent. Cinquenta e cinco pacientes (45,5%) apresentaram placas ateroscleróticas ecolucentes nas artérias carótidas e 54,5% apresentaram placas ecogênicas ou nenhuma placa. Dos pacientes que apresentaram placas ecolucentes, 90,9% apresentaram reestenose do stent coronário, e daqueles com placas ecogênicas ou nenhuma placa, 10,6% apresentaram reestenose de stent. A presença de placas ecolucentes nas artérias carótidas aumentou o risco de reestenose de stent coronário em 8,21 vezes (RR=8,21;IC95%: 3,58-18,82; p<0,001). Conclusões: A presença de placas ateroscleróticas ecolucentes na artéria carótida constitui um preditor de risco de reestenose de stent coronário e deve ser considerada na escolha do tipo de stenta ser usado na angioplastia coronária.

Abstract Background: The incidence of restenosis of the coronary artery after a bare-metal stent implant has been lower than in simple balloon angioplasty; however, it still shows relatively high rates. Objective: The aim of this study was to find new risk indicators for in-stent restenosis using carotid ultrasonography, that, in addition to the already existing indicators, would help in decision-making for stent selection. Methods: We carried out a cross-sectional prospective study including 121 consecutive patients with chronic coronary artery disease who had undergone percutaneous coronary intervention with repeat angiography in the previous 12 months. After all cases of in-stent restenosis were identified, patients underwent carotid ultrasonography to evaluate carotid intima-media thickness and atherosclerosis plaques. The data were analyzed by Cox multiple regression. The significance level was set a p<0.05. Results: Median age of patients was 60 years (1st quartile = 55, 3rd quartile = 68), and 64.5% of patients were male. Coronary angiography showed that 57 patients (47.1%) presented in-stent restenosis. Fifty-five patients (45.5%) had echolucent atherosclerotic plaques in carotid arteries and 54.5% had echogenic plaques or no plaques. Of patients with who had echolucent plaques, 90.9% presented coronary in-stent restenosis. Of those who had echogenic plaques or no plaques, 10.6% presented in-stent restenosis. The presence of echolucent plaques in carotid arteries increased the risk of coronary in-stent restenosis by 8.21 times (RR=8.21; 95%CI: 3.58-18.82; p<0.001). Conclusions: The presence of echolucent atherosclerotic plaques in carotid artery constitutes a risk predictor of coronary instent restenosis and should be considered in the selection of the type of stent to be used in coronary angioplasty.
