OBJECTIVE: This study aimed to analyze the physical and pulmonary capacities of hospitalized patients with severe coronavirus disease and its correlation with the time of hospitalization and complications involved. METHODS: A total of 54 patients, aged ≥18 years of both sexes, were evaluated 2-4 months after hospital discharge in São Paulo, Brazil. The physical characteristics analyzed were muscle strength, balance, flexibility, and pulmonary function. The K-means cluster algorithm was used to identify patients with similar physical and pulmonary capacities, related to the time of hospitalization. RESULTS: Two clusters were derived using the K-means algorithm. Patients allocated in cluster 1 had fewer days of hospitalization, intensive care, and intubation than those in cluster 2, which reflected a better physical performance, strength, balance, and pulmonary condition, even 2-4 months after discharge. Days of hospitalization were inversely related to muscle strength, physical performance, and lung function: hand grip D (r=−0.28, p=0.04), Short Physical Performance Battery score (r=−0.28, p=0.03), and forced vital capacity (r=−0.29, p=0.03). CONCLUSION: Patients with a longer hospitalization time and complications progressed with greater loss of physical and pulmonary capacities.

Resumo Objetivo: Avaliar em uma amostra de pacientes com distúrbios da diferenciação do sexo (DDS), dados relacionados à idade, ao encaminhamento e sua correlação com as queixas iniciais, ao sexo ao encaminhamento e ao sexo final e diagnóstico etiológico. Métodos: Revisão retrospectiva da idade por ocasião da primeira consulta e motivo dela, sexo social inicial e após definição do diagnóstico, cariótipo e diagnóstico etiológico de todos os casos atendidos em um ambulatório especializado em DDS entre 1989 e 2016. Foram excluídos casos que não compreendiam DDS e diagnósticos de DDS que não cursam comumente com ambiguidade genital, não necessitam de acompanhamento especializado. Resultados: Dos 1.793 casos atendidos, 1.139 foram diagnosticados com algum DDS. Excluíram-se 430 (272 síndrome de Turner, 66 síndrome de Klinefelter e 92 disgenesia gonadal pura), totalizando 709. Desses, 82,9% foram encaminhados por ambiguidade genital, somente um quarto ainda no primeiro mês de vida e 6,6% por atraso puberal, a maioria com 10 anos ou mais; 68,6% tiveram diagnóstico de DDS XY; 22,4% DDS XX e 9% de anomalias dos cromossomos sexuais. Conclusões: Este estudo apresenta a maior casuística na literatura de pacientes com DDS atendidos em um único serviço. O momento de encaminhamento da maioria dos pacientes com ambiguidade genital foi aquém do ideal e casos mais leves de ambiguidade e muitos com manifestações puberais foram encaminhados ainda mais tardiamente. Os resultados reforçam a importância do ensino continuado a profissionais que terão o primeiro contato com esses pacientes, principalmente pediatras e neonatologistas.

Abstract Objective: To evaluate, in a sample of patients with disorders of sex development (DSD), data related to the age at referral and their correlation with the initial complaints, gender at referral, defined gender after diagnosis and etiological diagnosis. Methods: Retrospective review of the age at the first consultation and the reason for it, initial social gender and gender after the diagnosis, karyotype and etiological diagnosis of all cases treated at a DSD outpatient clinic between 1989 and 2016. Cases that did not involve DSD and DSD diagnoses that do not usually involve ambiguous genitalia, thus not requiring specialized monitoring, were excluded. Results: Of the 1793 treated cases, 1139 were diagnosed with some type of DSD. This study excluded 430 cases (272 with Turner's syndrome, 66 with Klinefelter syndrome, and 92 with pure gonadal dysgenesis), thus a total 709 individuals were included. Of these, 82.9% were referred due to ambiguous genitalia; only one-quarter were still in the first month of life, and 6.6% were referred due to pubertal delay, with most of them aged 10 years or older. Of these patients, 68.6% had a diagnosis of XY DSD, 22.4% of XX DSD, and 9% of sex chromosome abnormalities. Conclusions: This study presents the largest series in the literature of patients with DSD treated in a single center. The time of referral of the majority of patients with ambiguous genitalia fell short of the ideal, and milder cases of ambiguous genitalia and many with pubertal manifestations were referred even later. The results reinforce the importance of continuing education for professionals who will have the first contact with these patients, mainly pediatricians and neonatologists.

Este estudo analisa como se articulam os elementos que contribuem para a formação política de professores de Educação Física, engajados em diferentes instituições onde esta formação acontece. Partiu-se da seguinte questão investigativa: como se dá a formação política de professores de Educação Física engajados em diferentes lutas sociais? Realizamos seis entrevistas e as analisamos à luz da produção do conhecimento sobre o objeto de estudo da formação política de professores e de uma avaliação sobre aspectos conjunturais da sociedade, da escola, da Educação e da Educação Física. Interessou-nos explorar a questão de como o sentido político da Educação se expressa na teoria e na prática entre esses professores.

This study aimed to ascertain how is the articulate of the elements that contribute to the political formation of Physical Education teachers engaged in various institutions where this happens. We start with the following investigative question: how is the political formation of Physical Education teachers engaged in various social struggles? We conducted six interviews and analyzed them in light of the knowledge production about the object of study teachers' political formation and an evaluation of conjunctural aspects of society, school, education and Physical Education. Interested in exploring the question of how the political meaning of education is reflected in theory and practice among these teachers.

Este estudio analiza como se articulan los elementos que contribuyen para la formación política de profesores de Educación Física empleados en diferentes instituciones adonde esta formación se sucede. La investigación inició con la siguiente cuestión: ¿cómo se da la formación política de profesores de Educación Física que participan en distintas luchas sociales? Realizamos seis entrevistas y las analizamos bajo la luz de la producción del conocimiento acerca del objecto de estudio formación política de profesores y de una evaluación sobre aspectos conjunturales de la sociedad, de la escuela, de la educación y de la Educación Física. Interesó explotar la cuestión de cómo el sentido político de la educación se expresa en la teoría y la práctica entre esos profesores.

A enzima 17β-hidroxiesteroide desidrogenase tipo 3 (17-β-HSD3) catalisa a conversão de androstenediona a testosterona nos testículos, e sua deficiência é uma forma rara de distúrbio do desenvolvimento do sexo em indivíduos 46,XY. A desordem apresenta um amplo espectro de características fenotípicas e de resultados de dosagens laboratoriais. Neste trabalho, são relatados quatro casos de deficiência da 17-β-HSD3 com cariótipo 46,XY, ambiguidade genital em diversos graus, androstenediona aumentada, testosterona diminuída, e relação testosterona e androstenediona < 0,8. Em três das pacientes, o diagnóstico foi suspeitado devido à presença de sinais de virilização na puberdade. Todos os pacientes foram criados como mulheres, e a identidade de gênero feminino foi mantida em todas elas. A heterozigose composta da mutação nova c.277+2T>G e da mutação c.277+4A>T, ambas localizadas no sítio doador de splicing do íntron 3 do gene HSD17B3, foi identificada no caso 3. Além dessas, as mutações missense p.Ala203Val, p.Gly289Ser, p.Arg80Gln foram identificadas em homozigose pelo sequenciamento do gene HSD17B3 dos casos 1, 2 e 4, respectivamente. The enzyme 17β-hydroxysteroid dehydrogenase type 3 (17-β-HSD3) catalyzes the conversion of androstenedione to testosterone in the testes, and its deficiency is a rare disorder of sex development in 46,XY individuals. It can lead to a wide range of phenotypic features, with variable hormonal profiles. We report four patients with the 46,XY karyotype and 17-β-HSD3 deficiency, showing different degrees of genital ambiguity, increased androstenedione and decreased testosterone levels, and testosterone to androstenedione ratio < 0.8. In three of the patients, diagnosis was only determined due to the presence of signs of virilization at puberty. All patients had been raised as females, and female gender identity was maintained in all of them. Compound heterozygosis for c.277+2T>G novel mutation, and c.277+4A>T mutation, both located within the intron 3 splice donor site of the HSD17B3 gene, were identified in case 3. In addition, homozygosis for the missense p.Ala203Val, p.Gly289Ser, p.Arg80Gln mutations were found upon HSD17B3 gene sequencing in cases 1, 2, and 4, respectively. Avaliou-se o efeito da utilização de duas fontes de carboidratos solúveis, amido ou fibra solúvel em detergente neutro, associadas a dois níveis de FDN, sobre o desempenho de novilhos de corte em confinamento na fase de terminação. Foram confinados 24 novilhos ½ Angus x ½ Nelore, com idade inicial de 18 meses e peso de 329 ± 24 kg. As quatro dietas foram calculadas para fornecer nível de PB suficiente para ganho diário de 1,4 kg. As dietas foram constituídas de 40% de silagem de milho e 60% de concentrado (grão de milho, polpa cítrica e casca de soja), além de minerais e uréia, arranjados de forma a promover nível alto de amido associado a dois níveis de FDN (48 e 39%) ou nível alto de fibra solúvel associado aos dois níveis de FDN. Observou-se que os animais alimentados com a dieta com maior nível de polpa cítrica (45%), com baixos teores de fibra solúvel e FDN, apresentaram consumo reduzido e, conseqüentemente, baixo ganho de peso e pior conversão alimentar. Entre as dietas com maior nível de FDN, verificou-se que os animais alimentados com fibra solúvel apresentaram o melhor ganho de peso (1,435 vs 1,262 kg/dia) e a melhor conversão alimentar (7,494 vs 8,651), mas não diferiram daqueles do tratamento com amido e baixo teor de FDN. Os resultados demonstraram que é possível alterar o desempenho animal manipulando a fonte e a concentração dos carboidratos solúveis.

The effects of starch or neutral detergent soluble fiber associated with two dietary levels of NDF on performance of finishing feedlot beef steers were evaluated in this trial. Twenty-four crossbred ½ Angus x ½ Nellore steers averaging 18 months of age and 329 ± 24 kg of body weight at the beginning of the trial were used. All four diets were balanced to supply the crude protein requirements for a daily weight gain of approximately 1.4 kg. Diets were composed (dry matter basis) by 40% of corn silage and 60% of concentrate containing corn grain, citrus pulp, soybean hulls, salt, and urea. Animals fed the diet containing high soluble fiber and low NDF content had the lowest dry matter intake (DMI), which compromised weight gain and feed conversion. This decrease in DMI can be explained by the greater level of citrus pulp (45%) compared to the other three diets. Between diets with higher NDF levels, that with soluble fiber resulted in greater daily weight gain (1.435 vs. 1.262 kg) and better feed conversion ratio (7.494 vs. 8.651) than the diet with starch. However, no significant difference was observed between the diet with high NDF and soluble fiber compared to that with low NDF and starch for these same variables. In conclusion, animal performance can be improved by changing the source and content of dietary soluble carbohydrates.
