This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Objetivou-se, com o estudo, avaliar a adição do estéril resultante da extração da gipsita, como condicionante do solo, sobre a disponibilidade de água em um Neossolo Quartzarênico coletado no Município de Petrolina, PE. A capacidade de campo (CC) foi obtida pela saturação e subseqüente drenagem de um perfil simulado de solo em uma coluna de cilindros de PVC com 100 cm de altura, nas proporções de 0, 20, 40, 60 e 80 g kg-1 do estéril, incorporadas nos 40 cm superiores. O ponto de murcha permanente (PMP) foi determinado pelo método fisiológico; adicionalmente, a CC e o PMP o foram pela metodologia usual utilizando-se o extrator de Richards com uma pressão de 10 e 1500 kPa, respectivamente. A incorporação de doses crescentes do estéril aumentou a água disponível do solo, de 4,8 para 8,67%, quando determinado pelo método tradicional, nas doses 0 e 80 g kg-1, respectivamente, e de 14,76 para 15,08%, nas mesmas doses, quando a água disponível foi determinada a partir da CC e o PMP obtidas pelo método da coluna de drenagem e método fisiológico, respectivamente. Inserindo-se a umidade gravimétrica da capacidade de campo obtida pelo método da coluna de solo na curva de retenção de umidade, vê-se que este parâmetro foi melhor representado por um potencial matricial próximo -6 kPa.
This work aimed at evaluating the addition of the sterile material resulting from gypsite mining as a soil conditioner on the water availability of a Quartzsandy Neosol collected in the municipality of Petrolina, PE. Field capacity (FC) was obtained by the saturation and subsequent drainage of a simulated soil profile in a column made up of PVC cylinders 100 cm high where doses of 0, 20, 40, 60 and 80 g kg-1 of the sterile material were incorporated to the upper 40 cm. The permanent wilting point (PWP) was determined by the physiological method. In addition, the FC and the PWP were determined by the usual methodology using Richard's pressure plate with the pressures of 10 and 1500 kPa, respectively. The incorporation of increasing doses of sterile material increased the available soil water from 4.8 to 8.67%, determined by the traditional method, and from 14.76 to 15.08% in the same doses when available water was defined with the FC and the PWP determined by the column and physiological methods, respectively. By inserting the gravimetric water content of FC obtained by the column method in the soil water release curve, this parameter was best represented by a matric potencial close to -6 kPa.

The objective of this research was to improve Glucose-6-phosphate dehydrogenase (G6PD) production by Saccharomyces cerevisiae W303-181, which carry the plasmid YEpPGK-G6PD, by varying the following cultivation conditions: pH value (4.8, 5.7 and 6.6); inoculum concentration (0.1, 0.6 and 1.1 g/L) and initial glucose concentration (20.0, 30.0 and 40.0 g/L). The effect of those variables on G6PD production capability was studied by the application of response surface statistical analysis. The results showed that the highest G6PD production (1594.2 U/L), specific activity (1189.7 U/g cell) and productivity (45.6 U/L.h) occurred at pH 4.8, inoculum concentration of 0.1 g/L and initial glucose concentration of 20.0 g/L, under agitation of 150 rpm at 30ºC after 36 h. In this work, the strain expressed about 21 fold more activity than the wild S. cerevisiae strain, being an attractive and promising new source of this enzyme.

Convolutamydine A is a member of a family of oxindole alkaloids isolated from the Floridian marine bryozan Amathia convoluta in 8.6x10-6 % yield. This compound is interesting as it has been described in the literature to have significant pharmacological activity. When bioactive substances are isolated in low yields, such as in the case of convolutamydine A, the use of synthetic organic chemistry to prepare larger quantities of these substances is necessary. This paper describes the isolation, structural characterization and synthesis of convolutamydine A, using readily available reagents and reaction conditions that may be applied in any organic chemistry undergraduate laboratory course.

Evaluation of the activated charcoals and adsorption conditions used in the treatment of sugarcane bagasse hydrolysate for xylitol production

Xylitol has sweetening, anticariogenic and clinical properties that have attracted the attention of the food and pharmaceutical industries. The conversion of sugars from lignocellulosic biomass into xylitol by D-xylose-fermenting yeast represents an alternative to the chemical process for producing this polyol. A good source of D-xylose is sugarcane bagasse, which can be hydrolyzed with dilute acid. However, acetic acid, which is toxic to the yeast, also appears in the hydrolysate, inhibiting microbe metabolism. Xylitol production depends on the initial D-xylose concentration, which can be increased by concentrating the hydrolysate by vacuum evaporation. However, with this procedure the amount of acetic acid is also increased, aggravating the problem of cell inhibition. Hydrolysate treatment with powdered activated charcoal is used to remove or decrease the concentration of this inhibitor, improving xylitol productivity as a consequence. Our work was an attempt to improve the fermentation of Candida guilliermondii yeast in sugarcane bagasse hydrolysate by treating the medium with seven types of commercial powdered activated charcoals (Synth, Carbon Delta A, Carbon Delta G, Carbon 117, Carbon 118L, Carbon 147 and Carvorite), each with its own unique physicochemical properties. Various adsorption conditions were established for the variables temperature, contact time, shaking, pH and charcoal concentration. The experiments were based on multivariate statistical concepts, with the application of fractional factorial design techniques to identify the variables that are important in the process. Subsequently, the levels of these variables were quantified by overlaying the level curves, which permitted the establishment of the best adsorption conditions for attaining high levels of xylitol volumetric productivity and D-xylose-to-xylitol conversion. This procedure consisted in increasing the original pH of the hydrolysate to 7.0 with CaO and reducing it to 5.5 with H3PO4. Next, the hydrolysate was treated under adsorption conditions employing CDA powdered activated charcoal (1%) for 30 min at 60ºC, 100 rpm and pH 2.5. The optimized xylitol volumetric productivity (0.50 g/L h) corresponded to a D-xylose-to-xylitol conversion of 0.66 g/g.

A number of factors can influence the behavior of yeast during fermentation. Some of these factors (initial wort concentration, initial pH and percentage of corn syrup in the composition of the wort) were studied in order to determine their influence on the productivity of fermentation. Fermentations were carried out at 25ºC utilizing a 2³ factorial design of these factors. The results showed that the percentage of corn syrup had no influence on process productivity, whereas initial pH and especially initial wort concentration did. It can be concluded that using pH and initial wort concentration values higher than those utilized in this work (5.5 and 20ºP, respectively) will result in a higher productivity.

Onze pacientes do sexo masculino com epilepsia e queixa de alteração sexual foram submetidos a avaliação multidisciplinar. A média de idade foi 27 anos (20-34), a duração média da epilepsia foi 19 anos (0,5-32) e a frequência média das crises foi duas por semana (0-7). Dez pacientes apresentavam crises parciais e um, mioclônicas. Dez pacientes recebiam drogas antiepilépticas (difenil-hidantoína - 1, carbamazepina - 8, clonazepam - 3, clobazam - 2, valproato - 3, vigabatrina - 1). Segundo os critérios do DSM III -- R, as queixas foram disfunção erétil (9), redução da libido (4), froteurismo (4), inibição do orgasmo (3), ejaculação precoce (3), fetichismo (2), voyeurismo (2), exibicionismo (2), pedofilia (1) e aversão sexual (1). A avaliação endocrinológica mostrou hipogonadismo hipogonadotrófico em dois pacientes. A avaliação urológica revelou disfunção erétil orgânica em outros dois. Em um paciente a alteração sexual foi considerada psicogênica. Em seis pacientes não foi possível estabelecer diagnóstico etiológico definitivo. Este estudo mostra que a alteração da sexualidade na epilepsia é multifatorial e necessita de abordagem multidisciplinar.
Eleven epileptic men who complained of epilepsy and sexual dysfunction were submitted to a multidisciplinary evaluation. Mean age was 27 years (20-34), mean epilepsy duration was 19 years (0,5-32) and the mean seizure frequency was two by week (0-7). Ten patients had partial seizures and one other had myoclonic epilepsy. Ten patients were treated with antiepileptic drugs (phenytoin - 1, carbamazepine - 8, clonazepam - 3, clobazam - 2, valproic acid - 3, vigabatrin - 1). As defined in the DSM III-R, the complaints were: erectile disorder (9), hypoactive sexual desire disorder (4), frotteurism (4), inhibited orgasm (3), premature ejaculation (3), fetishism (2), voyeurism (2), exhibitionism (2), pedophilia (1) and sexual aversion disorder (1). Two patients showed hypogonadotropic hypogonadism on endocrinologic screening. Urological evaluation disclosed organic erectile dysfunction in other two. One patient had a diagnosis of psychogenic sexual disorder. In six patients a conclusive etiologic diagnosis was not reached This report shows the multifactorial nature of sexual disorder in epilepsy and underlies the need of a multidisciplinar evaluation.

Foram investigados todos os óbitos de crianças menores de um ano residentes no Distrito Sanitário de Pau da Lima, Município de Salvador, Brasil, no ano de 1991, através de consultas aos respectivos prontuários hospitalares e visitas domiciliares. O estudo visava a aferir a efetividade dos serviços de saúde por meio da verificação de óbitos potencialmente evitáveis e o grau de adequação dos cuidados médicos prestados às crianças. De um total de 47 óbitos, observou-se que mais da metade dos mesmos ocorreu por causas vulneráveis à ação oportuna dos serviços de saúde, e que 21,3% dos óbitos ocorreram em via pública e domicílios. Houve 76,6% de perdas de informação nas entrevistas com as mães, atribuídas a problemas de preenchimento incompleto ou incorreto da Declaração de óbito, e por mudanças de endereço. A pesquisa dos prontuários nas unidades de saúde registrou perda de 58,3%, revelando o baixo grau de organização das estatísticas hospitalares; prevaleceram atendimentos classificados como pouco adequados e inadequados, principalmente nos itens exame físico e tratamento. Apesar das limitações operacionais do estudo, os resultados apontam problemas de acesso, efetividade e adequação do processo de assistência à saúde.
All deaths of children under one year of age and residing in the Pau da Lima Health District, in the city of Salvador, Bahia, Brazil, during the year 1991 were investigated through home visits and analysis of patient files. The study measured effectiveness of health services by verifying potentially avoidable deaths and the level of medical care these children received. More than half of the total of 47 deaths were related to causes considered amenable to prompt action by health services; 10 (21.3%) of the deaths occurred at home or in public (outside of health care facilities). Loss of information occurred in 36 (76.6%) of the mother's interviews, related to incomplete information on the death certificate and to changes of address. Analysis of patient files in the health clinics showed a 58.3% loss of information, indicating low-quality organization of hospital statistics; there was a prevalence of consultations classified as inadequate or barely adequate, due mainly to problems with physical examination and treatment. Despite this study's operational limitations, the results point to problems of access, effectiveness, and adequacy in the health care process.
