ABSTRACT This study aimed to determine the antibiotic profile of microorganisms isolated from urine samples of patients with community urine tract infections (UTI) admitted to the University Hospital of the Federal University of Sao Carlos to support an appropriate local empirical treatment. A retrospective cross-sectional study was conducted from October 2018 to October 2020. Data from 1,528 positive urine cultures for bacterial pathogens and antibiograms were tabulated. Bacterial species prevalence and their resistance profile were analyzed and compared by sex and age. For Gram-negative fermenting bacteria, resistance rates were compared between patients with previous hospitalization and the total of infections caused by this group. For comparisons, the Chi-square test was performed, using Fisher’s exact test when necessary (BioEstat program, adopting p ≤ 0.05). A multivariate analysis was applied to assess the effect of the studied variables in predicting multidrug resistance. Infections were more prevalent in women and older adults. Gram-negative bacteria represented 90.44% of total cultures. In both sexes, E. coli prevalence was significantly higher in adults compared with older adults (p < 0.0001). For several antibiotics, resistance rates were higher in the older adults compared with other ages and in patients with Gram-negative fermenting infections and previous hospitalization compared with the total of infections by this group of bacteria. The closer to the hospitalization, the higher the number of antibiotics with superior resistance rates. Resistance rates for aminoglycosides, carbapenems, ceftazidime, nitrofurantoin, piperacillin+tazobactam, and fosfomycin were less than 20%, considered adequate for empirical treatment. Only hospitalization in the previous 90 days was statistically significant in predicting infections by multidrug-resistant bacteria.

Abstract Tea is identified as the second most consumed drink in the world, and its frequent intake is related to several benefits to human health, considering its antimutagenic, anticarcinogenic, antimicrobial, anti-inflammatory, antihistamine, diuretic, calming, and above all, antioxidant effects. These effects are often associated with the action of the phenolic compounds contained in these infusions. In Brazil, among the most consumed infusions are the teas of chamomile (Matricaria chamomilla L.), lemongrass (Cymbopogon citratus (D.C.) Stapf), lemon balm (Melissa officinalis L.), anise (Pimpinella anisum L.), yerba mate (Ilex paraguariensis A. St.-Hil), peppermint (Mentha piperita L.), and green/black tea (Camellia sinensis L.). Therefore, said popularity is the reason why the mentioned species were chosen to be evaluated on inflammatory enzymes. The activity of phospholipases A2 was reduced by more than 25% after treatment with black tea and yerba mate. The most significant inhibition of protease activity was observed after incubation with black tea (40.74%), green tea (31.48%) and yerba mate (25.93%). Infusions of black and green tea reduced hemolysis in semisolid and liquid media, and for the latter, reductions of up to 50% of hemolytic activity were observed, indicating an anti-inflammatory potential of the samples. Plasma incubations with green tea, black tea and lemon balm and subsequent addition of venom (1:10 ratio; tea:PBS, v:v) prolonged the coagulation time of citrated plasma by approximately twice compared to the positive control. All controls with pure tea had a thrombolytic character, in higher proportions than the venom control, especially chamomile (273.55% dissolution). Phenolic compounds derived from phenolic acids, flavonoids, and tannins are identified as the main agents that promote the biological effects observed in this study. This is mainly due to their anti-catalytic properties exerted on inflammatory enzymes and as chelating agents of enzymatic co-factors. The evaluated teas showed potential for nutraceutical use, thus pointing to the possibility of use as an adjuvant in the treatment of diseases linked to hemostasis.

Abstract Objective To assess the functional disability level and identify associated factors in people after Ischemic Cerebral Vascular Accident. Methods A prospective cohort, carried out in a reference hospital in neurology with 224 people with ischemic stroke. Data collection took place between March and October 2019. Participants were followed up during hospitalization, when sociodemographic and clinical variables were collected using specific instruments and contacted after 90 days, by telephone call, to apply the Modified Barthel Index. In the analysis, descriptive statistics and Pearson’s chi-square test were applied. Statistical significance of 5% was adopted. Results Most had some degree of functional disability (58.5%), with 29.5% having moderate dependence and 29.0% having severe dependence. The variables being female (p=0.011), time of arrival at the reference hospital greater than or equal to 4.5 hours (p=0.017), previous stroke (p=0.031), not having undergone thrombolysis (p=0.023), having high blood pressure (p=0.032) and greater severity estimated by the National Institute of Health Stroke Scale (p=0.000) were associated with a higher disability level. Conclusion Moderate to severe dependence predominated. The severity of the event, previous event, hypertension, non-submission to thrombolysis, delay in arriving at the hospital and female gender were associated with a higher functional disability level.

Resumen Objetivo Evaluar el nivel de incapacidad funcional e identificar los factores asociados en las personas después de un accidente cerebrovascular isquémico. Métodos Cohorte prospectiva, realizada en un hospital de referencia en neurología, con 224 personas con accidente cerebrovascular isquémico. La recopilación de datos se llevó a cabo entre marzo y octubre de 2019. Se acompañó a los participantes durante la internación, momento en que se recopilaron las variables sociodemográficas y clínicas mediante la utilización de instrumentos específicos, y se los contactó 90 días después, por teléfono, para aplicar el Índice de Barthel modificado. En el análisis se aplicó estadística descriptiva y la prueba χ2 de Pearson. Se adoptó significación estadística de 5 %. Resultados La mayoría presentó algún nivel de incapacidad funcional (58,5 %), de los cuales el 29,5 % presentó dependencia moderada y el 29,0 % dependencia severa o grave. Las siguientes variables fueron asociadas a un mayor nivel de incapacidad: sexo femenino (p=0,011), tiempo de llegada al hospital de referencia mayor o igual a 4,5 h (p=0,017), accidente cerebrovascular previo (p=0,031), no haber realizado trombólisis (p=0,023), tener hipertensión arterial (p=0,032) y mayor gravedad estimada por la National Institute of Health Stroke Scale (p=0,000). Conclusión Predominó la dependencia de moderada a grave. La gravedad del evento, evento previo, hipertensión, no realización de trombólisis, retraso de la llegada al hospital y sexo femenino fueron las variables asociadas a un mayor nivel de incapacidad funcional.

Resumo Objetivo Avaliar o nível de incapacidade funcional e identificar os fatores associados em pessoas após Acidente Vascular Cerebral Isquêmico. Métodos Coorte prospectiva, realizada em hospital de referência em neurologia, com 224 pessoas com acidente vascular cerebral isquêmico. A coleta de dados ocorreu entre março a outubro de 2019. Os participantes foram acompanhados durante a internação, quando as variáveis sociodemográficas e clínicas foram levantadas empregando-se instrumentos específicos e contatados após 90 dias, por ligação telefônica, para aplicação do Índice de Barthel modificado. Na análise, aplicou-se estatística descritiva e o teste Qui-quadrado de Pearson. Adotou-se significância estatística de 5%. Resultados A maioria apresentou algum grau de incapacidade funcional (58,5%), sendo que 29,5% apresentaram dependência moderada e 29,0% severa ou grave. As variáveis sexo feminino (p=0,011), tempo de chegada ao hospital de referência maior ou igual a 4,5h (p=0,017), Acidente vascular cerebral prévio (p=0,031), não ter realizado trombólise (p=0,023), ter hipertensão arterial (p=0,032) e maior gravidade estimada pela National Institute of Health Stroke Scale (p=0,000) foram associadas a maior nível de incapacidade. Conclusão Predominou a dependência de moderada a grave. A gravidade do evento, evento prévio, hipertensão, não submissão à trombólise, retardo à chegada ao hospital e sexo feminino foram associados a maior nível de incapacidade funcional.

ABSTRACT Objective: To assess the impact of different vertical positions on lung aeration in patients receiving invasive mechanical ventilation. Methods: An open-label randomized crossover clinical trial was conducted between January and July 2020. Adults receiving invasive mechanical ventilation for > 24 hours and < 7 days with hemodynamic, respiratory and neurological stability were randomly assigned at a 1:1 ratio to the sitting position followed by passive orthostasis condition or the passive orthostasis followed by the sitting position condition. The primary outcome was lung aeration assessed using the lung ultrasound score (score ranges from 0 [better] to 36 [worse]). Results: A total of 186 subjects were screened; of these subjects, 19 were enrolled (57.8% male; mean age, 73.2 years). All participants were assigned to receive at least one verticalization protocol. Passive orthostasis resulted in mean lung ultrasound scores that did not differ significantly from the sitting position (11.0 versus 13.7; mean difference, -2.7; [95%CI -6.1 to 0.71; p = 0.11). Adverse events occurred in three subjects in the passive orthostasis group and in one in the sitting position group (p = 0.99). Conclusion: This analysis did not find significant differences in lung aeration between the sitting and passive orthostasis groups. A randomized crossover clinical trial assessing the impact of vertical positioning on lung aeration in patients receiving invasive mechanical ventilation is feasible. RESUMO Objetivo: Avaliar o impacto de diferentes posicionamentos verticais na aeração pulmonar em pacientes em ventilação mecânica invasiva. Métodos: Trata-se de ensaio clínico aberto, randomizado e transversal, realizado entre janeiro e julho de 2020. Adultos em ventilação mecânica invasiva por mais de 24 horas e menos de 7 dias com estabilidade hemodinâmica, respiratória e neurológica foram distribuídos aleatoriamente em uma proporção de 1:1 à postura sentada seguida da condição de ortostatismo passivo ou o ortostatismo passivo seguido de postura sentada. O desfecho primário foi a aeração pulmonar avaliada pelo lung ultrasound score. O escore varia de zero (melhor) a 36 (pior). Resultados: Foram selecionados 186 indivíduos; destes, 19 foram incluídos (57,8% do sexo masculino; média idade de 73,2 anos). Todos os participantes foram selecionados para receber pelo menos um protocolo de verticalização. O ortostatismo passivo resultou em escores médios de aeração pulmonar por ultrassonografia que não diferiram significativamente da postura sentada (11,0 versus 13,7; diferença média, -2,7; IC95% -6,1 a 0,71; p = 0,11). Ocorreram eventos adversos em três indivíduos no grupo ortostatismo passivo e em um no grupo postura sentada (p = 0,99). Conclusão: Esta análise não encontrou diferenças significativas na aeração pulmonar entre os grupos ortostatismo passivo e postura sentada. É factível conduzir um estudo clínico transversal randomizado para avaliar o impacto do posicionamento vertical na aeração pulmonar em pacientes em ventilação mecânica invasiva. Infelizmente, o estudo foi interrompido devido à necessidade de tratar pacientes com COVID-19. Registro ClinicalTrials.gov: NCT04176445

Abstract This study aimed to develop a scale to evaluate coping strategies in cancer patients. Initially, 112 items were created and subjected to theoretical analysis by expert judges, followed by a trial with the target population. After these steps, 42 items remained and were answered by 392 cancer patients. An exploratory factor analysis was conducted to assess the scale’s Internal structure validity. The resulting Coping Strategies Scale for Cancer Patients (E3PO) consists of 20 items grouped into two factors, explaining 36.05% of the total variance. The first factor, Treatment Engagement, includes 14 items (with a composite reliability of .83), which measure commitment to addressing the stressor. The second factor, Emotional Reactions, includes six items (with a composite reliability of .75) that evaluate coping based on the expression of negative emotions coming from the stressor. In the future, E3PO may help in the development of therapeutic plans and the evaluation of behavioral interventions aimed at enhancing coping strategies among cancer patients.

Resumen El objetivo de este estudio fue desarrollar una herramienta para evaluar el afrontamiento de pacientes oncológicos. Se construyeron 112 ítems que fueron sometidos a análisis teórico por parte de jueces especialistas y prueba piloto con la población objetivo. Los 42 ítems restantes tras esta etapa fueron contestados por 392 pacientes oncológicos. El análisis de la validez de la estructura interna se hizo por análisis factorial exploratorio. Denominad Escala de Estrategias de Afrontamiento para Pacientes Oncológicos (E3PO), los resultados señalaron una estructura de 20 ítems divididos en dos factores que explican 36.05% de la varianza total. Compromiso con el Tratamiento con 14 ítems (fiabilidad compuesta .83) evalúa el compromiso orientado hacia el cambio del estresor. Reacciones Emocionales con seis ítems (fiabilidad compuesta .75) evalúa el afrontamiento basado en la expresión de sentimientos negativos derivados del estresor. En el futuro, E3PO puede ser útil en la elaboración de planes terapéuticos y en la evaluación de intervenciones conductuales que se centren en el desarrollo o la mejora del afrontamiento del cáncer.

Resumo O objetivo desse estudo foi desenvolver um instrumento capaz de avaliar o enfrentamento de pacientes oncológicos. Foram construídos 112 itens que passaram por análise teórica de juízes especialistas e pré-teste com o público-alvo. Os 42 que restaram após essas etapas foram respondidos por 392 pacientes oncológicos. A análise de validade de estrutura interna foi realizada por análise fatorial exploratória. Denominada como Escala de Estratégias de Enfrentamento para Pacientes Oncológicos (E3PO), os resultados indicaram uma estrutura de 20 itens divididos em dois fatores que explicam 36,05% da variância total. Engajamento no Tratamento, 14 itens (confiabilidade composta de 0,83), avalia compromisso orientado para mudança do estressor. Reações Emocionais, seis itens (confiabilidade composta de 0,75), avalia enfrentamento baseado na expressão de sentimentos negativos advindos do estressor. A E3PO pode futuramente auxiliar na elaboração de planos terapêuticos e avaliação de intervenções comportamentais que tenham como foco o desenvolvimento ou melhoramento do enfrentamento ao câncer.

ABSTRACT Background: Critical illness is a major ongoing health care burden worldwide and is associated with high mortality rates. Sodium-glucose cotransporter-2 inhibitors have consistently shown benefits in cardiovascular and renal outcomes. The effects of sodium-glucose cotransporter-2 inhibitors in acute illness have not been properly investigated. Methods: DEFENDER is an investigator-initiated, multicenter, randomized, open-label trial designed to evaluate the efficacy and safety of dapagliflozin in 500 adult participants with acute organ dysfunction who are hospitalized in the intensive care unit. Eligible participants will be randomized 1:1 to receive dapagliflozin 10mg plus standard of care for up to 14 days or standard of care alone. The primary outcome is a hierarchical composite of hospital mortality, initiation of kidney replacement therapy, and intensive care unit length of stay, up to 28 days. Safety will be strictly monitored throughout the study. Conclusion: DEFENDER is the first study designed to investigate the use of a sodium-glucose cotransporter-2 inhibitor in general intensive care unit patients with acute organ dysfunction. It will provide relevant information on the use of drugs of this promising class in critically ill patients. ClinicalTrials.gov registry: NCT05558098

RESUMO Antecedentes: A doença crítica é um importante ônus permanente da assistência médica em todo o mundo e está associada a altas taxas de mortalidade. Os inibidores do cotransportador de sódio-glicose do tipo 2 têm demonstrado consistentemente benefícios nos desfechos cardiovasculares e renais. Os efeitos dos inibidores do cotransportador de sódio-glicose do tipo 2 em doenças agudas ainda não foram devidamente investigados. Métodos: O DEFENDER é um estudo de iniciativa do investigador, multicêntrico, randomizado, aberto, desenhado para avaliar a eficácia e a segurança da dapagliflozina em 500 participantes adultos com disfunção orgânica aguda hospitalizados na unidade de terapia intensiva. Os participantes aptos serão randomizados 1:1 para receber 10mg de dapagliflozina e o tratamento padrão por até 14 dias ou apenas o tratamento padrão. O desfecho primário é um composto hierárquico de mortalidade hospitalar, início de terapia renal substitutiva e tempo de internação na unidade de terapia intensiva, até 28 dias. O monitoramento da segurança será rigoroso durante todo o estudo. Conclusão: O DEFENDER é o primeiro estudo desenvolvido para investigar o uso de um inibidor do cotransportador de sódio-glicose do tipo 2 em pacientes de unidade de terapia intensiva geral com disfunção orgânica aguda. RESUMO: O presente estudo teve como objetivo avaliar se os índices obtidos com Canopeo e GreenSeeker (NDVI) em Megathyrsus maximum são capazes de estimar altura de plantas, massa de matéria seca, componentes morfológicos e teor de proteína bruta e fibra em detergente neutro em diferentes dias de crescimento. Cinco parcelas de 5 × 25 m de capim M.maximum foram definidas: sub parcelas de 1 × 1 m foram avaliadas a cada três dias, nas quais a pastagem apresenta 3, 6, 9, 12, 15, 18, 21, 24, 27 e 30 dias de rebrota. A sub parcela foi avaliada quanto à altura do dossel e acúmulo de massa. O índice do Canopeo (IC) obtido em maior altura foi menor do que o obtido em menor altura. A maior altura de medição aumentou o NDVI em relação a 0,10 m. Os maiores índices foram observados desde 18 dias de rebrota. Exceto para o IC avaliado a 0,10 m de altura, os índices não foram correlacionados com a composição química da forragem. IC e NDVI correlacionaram-se positivamente com a altura da planta, massa de MS e índice foliar, e negativamente com o índice de colmos. Assim, menores alturas de avaliação para IC e NDVI podem ser um bom preditor da altura da forragem. Valores de 0,83 e 85,8 para NDVI e IC, respectivamente, indicam um momento adequado para o início do pastejo de Megathyrsus maximum.

ABSTRACT: This study determined whether Canopeo and GreenSeeker measurements in Megathyrsus maximus can estimate plant height, dry matter mass, morphological components, and content of crude protein and neutral detergent fiber at different days of growth. Five plots of 5 × 25m M. maximus grass were defined: subplots of 1×1m were evaluated every three days, in which the pasture shows 3, 6, 9, 12, 15, 18, 21, 24, 27, and 30 days of regrowth. The subplot was evaluated for canopy height and mass accumulation. The canopeo index (CI) obtained at a higher height was lower than those obtained at a smaller height. Higher measurement height increased the normalized difference vegetation index (NDVI) relative to 0.10 m. The highest indexes were observed since 18 d of regrowth. Except for the CI evaluated at 0.10 m of height, the indexes were not correlated to the chemical composition of the forage. The CI and NDVI were positively correlated to plant height, dry matter mass, and leaf index, whereas both were negatively correlated with stalk index. Thus, lower evaluation heights for CI and NDVI can be a good predictor of forage height. Values of 0.83 and 85.8 for NDVI and CI, respectively, indicated an appropriate time to start the grazing of M. maximus.

O objetivo deste estudo é descrever o mix contraceptivo e analisar os fatores associados ao tipo de contraceptivo usado pelas mulheres brasileiras em idade reprodutiva. Trata-se de estudo transversal, de base populacional, com dados de 19.962 mulheres de 15 a 49 anos. Os desfechos foram uso e tipo de contraceptivo, classificados em: contraceptivos reversíveis de curta duração (SARC), longa duração (LARC) e permanentes. As variáveis explicativas foram: características da história reprodutiva, sociodemográficas e de acesso aos serviços de saúde. Utilizou-se a regressão logística multinomial para estimativas da odds ratio (OR), tendo os SARC como categoria de referência. As análises foram realizadas no módulo survey do software Stata, que considerou o efeito do plano amostral complexo da Pesquisa Nacional de Saúde de 2019. A prevalência do uso de contraceptivos foi de 83,7%. Do total de usuárias, 72% usavam SARC, 23,2% métodos permanentes e 4,8%, LARC. Mulheres com maior escolaridade, plano de saúde, que tiveram partos e participaram de grupos de planejamento reprodutivo tiveram maior chance de usar LARC na comparação com o uso de SARC, enquanto o cadastro na unidade básica de saúde se associou a menor chance de uso. Ainda, quanto maior a idade e paridade, além de viver com o companheiro, maior a chance de usar métodos permanentes em relação ao uso de SARC. Apesar da elevada cobertura de contracepção, o mix contraceptivo permanece obsoleto, com predomínio do uso de SARC. Além disso, observou-se importante desigualdade de acesso, sendo os LARC acessíveis apenas por mulheres com melhores condições socioeconômicas, enquanto os métodos permanentes foram associados a um perfil de maior vulnerabilidade social.

El objetivo fue describir la combinación anticonceptiva y analizar los factores asociados al tipo de anticonceptivo usado por las mujeres brasileñas en edad reproductiva. Estudio transversal, de base poblacional, con datos de 19.962 mujeres de 15 a 49 años. Los resultados fueron el uso y el tipo de anticonceptivo, clasificados en: anticonceptivos reversibles de corta duración (SARC), de larga duración (LARC) y permanentes. Las variables explicativas fueron características de la historia reproductiva, sociodemográficas y de acceso a los servicios de salud. Se utilizó la regresión logística multinomial para las estimaciones de la odds ratio (OR), siendo los SARC la categoría de referencia. Los análisis fueron realizados en el módulo survey, del software Stata, que consideró el efecto del sistema de muestreo complejo de la Encuesta Nacional de Salud de 2019. La prevalencia del uso de anticonceptivos fue del 83,7%. Del total de usuarias, 72% usaban SARC, el 23,2% métodos permanentes y el 4,8% LARC. Las mujeres con mayor educación, plan de salud, que tuvieron partos y participaron de grupos de planificación reproductiva tuvieron mayor posibilidad de usar LARC cuando comparados al uso de SARC, mientras que el registro en la unidad básica de salud se asoció con una menor posibilidad de uso. Además, cuanto mayor sea la edad y la paridad, además de vivir con el compañero, mayor será la posibilidad de utilizar métodos permanentes en relación con el uso de SARC. A pesar de la alta cobertura de anticoncepción, la combinación anticonceptiva sigue siendo obsoleta, con un uso predominante de SARC. Además, se observaron importantes desigualdades en el acceso, siendo los LARC accesibles solo para mujeres con mejores condiciones socioeconómicas, mientras que los métodos permanentes se asociaron con un perfil de mayor vulnerabilidad social.

This study aims to describe the contraceptive mix and analyze the factors associated with the type of contraceptive used by Brazilian women of reproductive age. This is a cross-sectional, population-based study with data from 19,962 women aged 15 to 49 years. The outcomes were use and type of contraceptive, classified as: short-acting reversible contraceptives (SARC), long-acting (LARC), and permanent. The explanatory variables were characteristics of reproductive history, sociodemographic history, and access to health services. Multinomial logistic regression was used for odds ratio (OR) estimates, with SARC being the reference category. The analyses were performed in the Survey module of the Stata software, which considered the effect of the complex sampling plan of the 2019 Brazilian National Health Survey. The prevalence of contraceptive use was 83.7%. Of the total number of users, 72% used SARC, 23.2% permanent methods, and 4.8% LARC. Women with higher education, health insurance, who had deliveries, and who participated in reproductive planning groups had a higher chance of using LARC when compared with the use of SARC, while registration at the basic health unit was associated with a lower chance of use. Still, the higher the age and parity, in addition to living with the partner, the greater the chance of using permanent methods in relation to the use of SARC. Despite the high coverage of contraception, the contraceptive mix remains obsolete, with a predominance of the use of SARC. In addition, important inequalities in access were observed, with LARC being accessible only to women with better socioeconomic conditions, while permanent methods were associated with a profile of greater social vulnerability.

Resumo A avaliação de perguntas de questionário de inquérito de saúde no Brasil limita-se, na maioria das vezes, aos testes-piloto e debriefing das entrevistas. O objetivo do presente artigo é contribuir para preencher essa lacuna ao apresentar uma metodologia de avaliação cognitiva para identificação de perguntas potencialmente “problemáticas” quanto à sua formulação e o que ocorre, na interação entrevistador-entrevistado, ao aplicá-las em entrevistas com o uso de questionário padronizado em um inquérito regional de saúde. O estudo utilizou dois roteiros padronizados para identificação de dificuldades de redação e/ou compreensão e avaliação da interação entrevistador-entrevistado. Dentre as 513 perguntas do questionário individual de pesquisa, 68 questões foram indicadas como sendo potencialmente problemáticas à compreensão e comunicação pelos entrevistados. As perguntas que apresentaram problemas na sua formulação, com maior pontuação (>40), foram aquelas relacionadas à quantificação de tempo; ao grau de dificuldade para o autocuidado; à situação no trabalho e à estimação do rendimento. Dentre as perguntas apontadas como problemáticas na avaliação pelo Sistema de Apreciação de Perguntas (SAP), o autocuidado e horas trabalhadas estão entre aquelas que também tiveram pontuação alta na avaliação da interação.

Abstract The evaluation of health survey questions in Brazil is limited, in most cases, to pilot tests and interview debriefing. The purpose of this article is to contribute to filling this gap by presenting a cognitive assessment methodology for identifying potentially “problematic” questions regarding their formulation and what happens in the interviewer-interviewee interaction, when applying them in interviews using the standardized questionnaire in a regional health survey. In this study, two standardized scripts were used to identify difficulties in writing and/or understanding and evaluating the interviewer-interviewee interaction. Among the 513 questions in the individual research questionnaire, 68 questions were indicated as being potentially problematic for the interviewees to understand and communicate. The questions that presented problems in their formulation, with the highest score (>40), were those related to the quantification of time; the degree of difficulty for self-care; work situation and income estimation. Among the questions identified as problematic in the evaluation by the Question Appraisal System (SAP), self-care and hours worked are among those that also scored high in the interaction evaluation.

Resumo O objetivo deste artigo é analisar indicadores de uso de serviços de saúde bucal da atenção primária à saúde no Maranhão antes e durante a pandemia de COVID-19. Estudo ecológico de série temporal interrompida cuja unidade de análise foi o Maranhão. A exposição foi a pandemia de COVID-19, dicotomizada em pré e pandemia. Os desfechos foram calculados pela razão da quantidade de procedimentos preventivos (RPP), de urgência (RPU) e procedimentos curativos (RPC), e a projeção censitária da população do estado, multiplicada por mil, mensalmente. Os dados foram coletados do Sistema de Informação em Saúde para a Atenção Básica e do Instituto Brasileiro de Geografia e Estatística, no período de 2015.1 a 2022.2. As análises foram realizadas por meio do modelo autorregressivo, integrado e de médias móveis com sazonalidade. Houve tendência crescente dos três indicadores até jan./2019. A pandemia causou redução significativa na RPP (Xreg = -6,55; p-valor = 0,0008) e na RPC (Xreg = -4,74; p-valor = 0,0005), mas não influenciou a RPU (Xreg = -0,03; p-valor = 0,12) desde o primeiro semestre de 2020, persistindo até o segundo semestre de 2022. A pandemia de COVID-19 provocou redução nos serviços preventivos e curativos de saúde bucal no Maranhão.

Abstract This article aims to analyze indicators of the utilization of oral health services (UOHS) in primary health care in the state of Maranhão, Brazil, before and during the COVID-19 pandemic. We conducted an ecological time series study. The unit of analysis was the state of Maranhão, and the exposure variable was the COVID-19 pandemic, dichotomized into pre-pandemic and pandemic. The outcome variables were the monthly rates per 1,000 population of three UOHS indicators: rate of preventive procedures (RPP-PHC); rate of urgent procedures (RUP-PHC), and rate of curative procedures (RCP-PHC). The data were collected from the Primary Health Care Information System (SISAB) and the Brazilian Institute of Geography and Statistics (IBGE) for the period from 2015-2022. The analyses were performed using the Seasonal Autoregressive Integrated Moving Average (SARIMA) model. The three indicators showed an upward trend up to January 2019. The pandemic caused a significant reduction in the RPP (Xreg = -6.55; p-value = 0.0008) and RCP (Xreg = -4.74; p-value = 0.0005), starting in the first semester of 2020 and continuing into the second semester of 2022, but did not influence the RUP (Xreg = -0.03; p-value = 0.12). The COVID-19 pandemic caused a reduction in preventive and curative oral health services in Maranhão.

ABSTRACT Objective: To determine whether polymorphisms of the IL10 and IL17 genes are associated with severe asthma control and bronchodilator reversibility in children and adolescents with severe asthma. Methods: This was a cross-sectional study, nested within a prospective cohort study of patients with severe asthma. Two outcomes were evaluated: asthma control and bronchodilator reversibility. We extracted DNA from peripheral blood and genotyped three single nucleotide polymorphisms: rs3819024 and rs2275913 in the IL17A gene; and rs3024498 in the IL10 gene. For the association analyses, we performed logistic regression in three genetic models (allelic, additive, and dominant). Results: The rs3024498 C allele in the IL10 gene was associated with failure to achieve asthma control despite regular treatment (p = 0.02). However, the G allele of the IL17A rs3819024 polymorphism was associated with failure to respond to stimulation with a b2 agonist. The rs2275913 polymorphism of the IL17A gene showed no relationship with asthma control or bronchodilator reversibility. Conclusions: In pediatric patients with severe asthma, the IL10 polymorphism appears to be associated with failure to achieve clinical control, whereas the IL17A polymorphism appears to be associated with a worse bronchodilator response. Knowledge of the involvement of these polymorphisms opens future directions for pharmacogenetic studies and for the implementation of individualized therapeutic management of severe asthma in pediatric patients.

RESUMO Objetivo: Determinar se existe relação entre polimorfismos dos genes IL10 e IL17 e controle da asma grave e reversibilidade com broncodilatador em crianças e adolescentes com asma grave. Métodos: Estudo transversal, aninhado em um estudo prospectivo de coorte com pacientes com asma grave. Foram avaliados dois desfechos: controle da asma e reversibilidade com broncodilatador. Extraímos DNA do sangue periférico e genotipamos três polimorfismos de nucleotídeo único: rs3819024 e rs2275913 no gene IL17A e rs3024498 no gene IL10. Para as análises de associação, realizamos regressão logística em três modelos genéticos (alélico, aditivo e dominante). Resultados: O alelo C do polimorfismo rs3024498 do gene IL10 apresentou relação com asma que permaneceu descontrolada mesmo com tratamento regular (p = 0,02). No entanto, o alelo G do polimorfismo rs3819024 do gene IL17A apresentou relação com ausência de resposta ao estímulo com b2-agonista. O polimorfismo rs2275913 do gene IL17A não apresentou relação com controle da asma ou reversibilidade com broncodilatador. Conclusões: Em pacientes pediátricos com asma grave, o polimorfismo do gene IL10 parece estar relacionado com ausência de controle clínico, ao passo que o polimorfismo do gene IL17A parece estar relacionado com pior resposta ao broncodilatador. O conhecimento a respeito do envolvimento desses polimorfismos abre perspectivas futuras para estudos farmacogenéticos e para a implantação de manejo terapêutico individualizado da asma grave em pacientes pediátricos.

ABSTRACT Objective To analyze the karyotype test and myeloid panel with next-generation sequencing findings in patients with myelofibrosis, and to compare transplant characteristics in patients referred for bone marrow transplantation. Methods Retrospective, single-center study with patients diagnosed with myelofibrosis treated at Hospital Israelita Albert Einstein between 2010 and 2020. Results A total of 104 patients with myelofibrosis were examined. Patients who had not been submitted to tests in our service were excluded. The final sample comprised 69 patients. Of these 69, 56 were submitted to karyotyping and 22 to myeloid panel with next-generation sequencing. Karyotype was normal in 60% of the patients and altered in 40%. The prevalence of high-risk molecular mutations was higher in patients referred for bone marrow transplantation (100% versus 50%). The median follow-up of transplant patients was 2.4 years and the overall survival at 2 years was 80% (95%CI: 62-100%). Conclusion The molecular analysis enables estimating the patient’s risk and thus instituting more aggressive treatment such as bone marrow transplant for patients at higher risk, being a relevant tool to guide therapy. Given the significance of molecular analysis for therapeutic decision-making in myelofibrosis, collection and disclosure of data on the prevalence of cytogenetic changes and findings of next-generation sequencing in affected patients is important.

ABSTRACT BACKGROUND: Stroke is a major cause of death and functional disability worldwide. Knowledge of the associated factors is essential for defining education, management, and healthcare strategies. OBJECTIVE: To analyze the association between arrival time at a neurology referral hospital (ATRH) and functional disability in patients with ischemic stroke 90 days after the event. DESIGN AND SETTING: Prospective cohort study conducted at a public institution of higher education in Brazil. METHODS: This study included 241 people aged ≥ 18 years who presented ischemic stroke. The exclusion criteria were death, inability to communicate without companions who could answer the research questions, and > 10 days since ictus. Disability was assessed using the Rankin score (mR). Variables for which associations showed a P value ≤ 0.20 in bivariate analysis were tested as modifiers between ATRH and disability. Significant interaction terms were used for multivariate analysis. Multivariate logistic regression analysis was performed with all variables, arriving at the complete model and adjusted beta measures. The confounding variables were included in the robust logistic regression model, and Akaike’s Information Criterion was adopted to choose the final model. The Poisson model assumes a statistical significance of 5% and risk correction. RESULTS: Most participants (56.0%) arrived at the hospital within 4.5 hours of symptom onset, and 51.7% presented with mRs of 3 to 5 after 90 days of ictus. In the multivariate model, ATRH ≥ 4.5 hours and females were associated with more significant disability. CONCLUSIONS: Arrival at the referral hospital 4.5 hours after the onset of symptoms or wake-up stroke was an independent predictor of a high degree of functional disability.

Abstract Objectives to verify the factors related to the level of use of the right to health of users with type 1 and 2 diabetes mellitus and understand the knowledge and the use of rights to health of these users. Methods mixed methods study, convergent type, conducted with 206 participants between November 2020 and April 2021. Data analysis was supported using SPSS® and IRAMUTEQ® software, as well as the adoption of Bardin's theoretical-methodological reference guidelines. Results a relationship was found between the score of the level of use of the right to health with monthly family income and self-reported skin color; and it was verified that the more economically vulnerable a population, the more they seek for rights, besides being evident the disparity related to ethnic groups and access to the right to health. Conclusion and implications for practice the combination of quantitative and qualitative data allowed a broader view of the knowledge and use of the right to health by individuals with diabetes. Thus, it is expected that the data obtained can contribute to a deeper reflection on the subject, favoring a qualified, informative and humanitarian assistance as a way to guarantee the right to health.

Resumen Objetivo verificar los factores relacionados con el nivel de utilización de los derechos a la salud de los usuarios con diabetes mellitus tipo 1 y 2 y comprender el conocimiento y la utilización de los derechos a la salud de estos usuarios. Método estudio de métodos mixtos, de tipo convergente, realizado con 206 participantes entre noviembre de 2020 y abril de 2021. El análisis de los datos se apoyó en el uso de los softwares SPSS® e IRAMUTEQ®, así como en la adopción de los referenciales teórico-metodológicos de Bardin. Resultados hubo relación entre el puntaje del nivel de uso de los derechos a la salud con el ingreso familiar mensual y el color de piel autoinformado; y se encontró que cuanto más vulnerable económicamente es una población, mayor es la búsqueda de derechos, además de ser evidente la disparidad relacionada con las etnias y el acceso a los derechos de salud. Conclusión e implicaciones para la práctica La combinación de datos cuantitativos y cualitativos permitió una visión más amplia sobre el conocimiento y el uso de los derechos a la salud por parte de las personas con diabetes. Por lo tanto, se espera que los datos obtenidos puedan contribuir a una reflexión más profunda sobre el tema, promoviendo una asistencia cualificada, informativa y humanitaria como forma de garantizar los derechos a la salud.

Resumo Objetivos verificar os fatores relacionados ao nível de utilização de direitos à saúde de usuários com diabetes mellitus tipo 1 e 2 e compreender o conhecimento e a utilização de direito à saúde desses usuários. Métodos estudo de métodos mistos, do tipo convergente, realizado com 206 participantes entre novembro de 2020 a abril de 2021. A análise dos dados teve como suporte a utilização dos softwares SPSS® e IRAMUTEQ® bem como a adoção das orientações do referencial teórico-metodológico de Bardin. Resultados constatou-se relação entre o escore do nível de utilização de direitos à saúde com renda familiar mensal e cor da pele autorreferida; e verificou-se que, quanto mais vulnerável economicamente uma população, mais se busca pelos direitos, além de ficar evidente a disparidade relacionada aos grupos étnicos e ao acesso aos direitos em saúde. Conclusão e implicações para a prática a combinação entre os dados quantitativos e qualitativos possibilitaram uma visão ampliada sobre o conhecimento e utilização dos direitos à saúde pelos indivíduos com diabetes. Logo, espera-se que os dados obtidos possam contribuir para uma reflexão mais aprofundada sobre a temática, favorecendo uma assistência qualificada, informativa e humanitária como forma de garantia dos direitos à saúde.

Abstract Background Studies show that among the drugs most commonly used in judicial litigation in Brazil, are those used to treat diabetes mellitus, especially insulin analogues. Objective Evaluate the use of the Unified Health System (SUS) by patients with type 1 diabetes mellitus (T1DM), who receive insulin analogues through judicial action, before and after this process. Method In a retrospective longitudinal observational study, secondary data was used from these patients in Minas Gerais, Brazil, in 2018. Socio-demographic information was collected and related to the follow-up of these patients in the SUS. The McNemar χ2 test was used to compare the proportions of the variables. Results Of the 89 patients analyzed, women (53.9%) were predominant. Most patients were aged between 20 and 39 years (52.8%), and more than half, 55.1%, use only a private health system. After the judicial action, there was a significant increase (p <0.05) in the number of patients who had consultations in primary health care (from 19.1% to 30.3%) and emergency medical appointments (from 1.1% to 9.0%). Conclusion It is observed that the majority of patients with T1DM via judicial action in the SUS are not monitored by this health system through examinations, consultations, and hospitalizations.

Resumo Introdução Estudos mostram que, dentre os medicamentos mais adquiridos via ação judicial, estão os utilizados para o tratamento do Diabetes Mellitus, especialmente os análogos de insulina. Objetivo Avaliar a utilização do Sistema Único de Saúde (SUS) pelos pacientes com Diabetes Mellitus tipo 1 (DM1), que recebem insulina por meio de judicialização, antes e após este processo. Método Em um estudo observacional longitudinal retrospectivo, foram utilizados dados secundários de pacientes com DM1, que adquiriram insulinas por processos judiciais em Divinópolis-MG, Brasil, em 2018. Foram coletadas informações sociodemográficas e referentes ao acompanhamento destes pacientes no SUS Realizou-se o teste χ2 de McNemar para a comparação das proporções das variáveis utilizadas para a avaliação do acompanhamento antes e após a judicialização. Resultados Dos 89 pacientes analisados, predominou-se o sexo feminino (53,9%), com idade entre 20 e 39 anos (52,8%). 55,1% destes utilizam apenas o sistema privado de saúde. Após a judicialização, houve um aumento significativo (p< 0,05) no número de pacientes que realizaram consultas na atenção primária à saúde (de 19,1% para 30,3%) e consultas médicas de emergência (de 1,1% para 9,0%). Conclusão A maioria dos pacientes com DM1 que judicializam medicamentos no SUS não são acompanhados por este sistema de saúde através de realização de exames, consultas e hospitalizações.
