Abstract Background Myocardial bridges (MB) are anatomical anomalies with possible clinical repercussions; hence, their understanding deserves attention. Objective To determinate the prevalence and characterize MB in human hearts from the state of Ceará. Methods: Fifty hearts of adult human cadavers from the Medicine School of Federal University of Ceará, Brazil. The hearts were dissected to identify MBs that pass over part of the coronary artery. The segment of the artery (proximal, middle, and distal) with a bridge was identified. The external diameter of the artery at the proximal and distal points of the MB was measured. The length and thickness of the MB were also measured with an electronic caliper. The muscle index (MMI) of the MB was calculated as the product of length and thickness expressed in millimeters. The significance level adopted in the statistical analysis was 5%. Results MB was confirmed in 40% of sample. Approximately one third of the sample had only 1 MB. MB was most frequently found over the anterior interventricular branch of the left coronary artery (59.25%, p=0.02), and its prevalence in other branches was much lower (22.23%). The most affected segments of arteries were the superior (44.44%) and medium (40.74%). The mean diameter of arteries proximal to the MB was 2.38±0.97mm (range=0.78-5.15mm), and the diameter distal to the MB was 1.71±0.75mm (range=0.42-3.58mm). The length was measured as mean=8.55±5.27mm, while the mean thickness was 0.89±0.33mm. Conclusion A high prevalence of MB is more likely to affect the left coronary artery system with larger MMI than other affected branches.

Resumo Fundamento As pontes miocárdicas (PM) são anomalias anatômicas com possíveis repercussões clínicas, e, portanto, seu entendimento merece atenção. Objetivo Para determinar a prevalência e caracterizar a PM em corações humanos do estado do Ceará. Métodos: Foram usados cinquenta corações de cadáveres humanos adultos da Faculdade de Medicina da Universidade Federal do Ceará, Brasil. Os corações foram dissecados para identificar PMs que passam sobre parte da artéria coronária. O segmento da artéria (proximal, médio e distal) com a ponte foi identificado. O diâmetro externo da artéria nos pontos proximal e distal da PM foi medido. O comprimento e a espessura da PM também foram medidos com um calibre eletrônico. O índice de massa muscular (IMM) da PM foi calculado como o produto do comprimento pela espessura expresso em milímetros. O nível de significância adotado para a análise estatística foi 5%. Resultados A PM foi confirmada em 40% da amostra. Aproximadamente um terço da amostra tinha apenas 1 PM. A PM foi encontrada mais frequentemente sobre o ramo interventricular anterior da artéria coronária esquerda (59,25%, p = 0,02), e sua prevalência em outros ramos foi muito mais baixa (22,23%). Os segmentos das artérias mais afetados foram o superior (44,44%) e o médio (40,74%). O diâmetro médio das artérias proximais em relação à PM foi de 2,38 ± 0,97 mm (intervalo = 0,78 - 5,15 mm), e o diâmetro distal da PM foi de 1,71 ± 0,75 mm (intervalo = 0,42 - 3,58 mm). O comprimento foi medido como média = 8,55 ± 5,27 mm, e a espessura média foi de 0,89 ± 0,33 mm. Conclusão A alta prevalência de PM tem mais probabilidade de afetar o sistema da artéria coronária esquerda com IMM maior do que outros ramos afetados.

Resumo Objetivos: descrever o perfil de utilização de medicamentos e de adesão, e a associação com as características clínicas e sociodemográficas de gestantes de alto risco atendidas em um hospital universitário. Métodos: trata-se de um estudo transversal com dados coletados mediante um questionário estruturado aplicado à 386 gestantes. Resultados: a maior parte das participantes era acompanhada apenas pelo ginecologista (75,1%), iniciou o pré-natal no primeiro trimestre gestacional (86,8%), não planejou a gravidez (61,9%) e realizou em média 8,2 (DP=4,4) consultas de pré-natal. Os diagnósticos mais frequentes foram hipertensão arterial (20,5%) e diabetes mellitus (19,7%). A prevalência de uso de medicamentos foi 99,7%, com média de 5,1 (DP=2,1) medicamentos por mulher e 12,7% de automedicação. Os antianêmicos (88,9%) e analgésicos (63,2%) foram as classes farmacológicas mais prevalentes e 17,9% das gestantes referiram uso de fármacos com risco gestacional relevante. Apenas 36,5% das gestantes foram consideradas aderentes ao tratamento, 32,9% declararam desconhecer a indicação dos medicamentos em uso e 42% não receberam orientações sobre o uso de medicamentos durante a gestação. Não há evidências de associação entre o número de medicamentos utilizados e os aspectos clínicos e sociodemográficos. Conclusão: é necessário desenvolver estratégias para melhorar o atendimento desta população e o uso racional de medicamentos, com ênfase no fortalecimento do cuidado multiprofssional.

Abstract Objectives: describe the profile of medication use and adherence, and the association with clinical and sociodemographic characteristics of high-risk pregnant women attended at a university hospital. Methods: cross-sectional study with data collected through a questionnaire applied on 386 pregnant women. Results: most participants were seen only by the gynecologist (75.1%), started prenatal in the first gestational trimester (86.8%), did not plan the pregnancy (61.9%), and performed an average of 8.2 (SD=4.4) prenatal consultations. The most frequent diagnoses were arterial hypertension (20.5%) and diabetes mellitus (19.7%). Prevalence of medication use was 99.7%, with an average of 5.1 (SD=2.1) medication per woman and 12.7% self-medication. Antianemics (88.9%) and analgesics (63.2%) were the most prevalent classes and 17.9% of the women reported the use of medication with significant gestational risk. Only 36.5% were considered adherent, 32.9% declared they were unaware of the indication of the medication in use and 42% did not receive guidance on the use of the medication during pregnancy. There is no evidence of association between the number of the medication used and clinical and sociodemographic aspects. Conclusions: there is a need to develop strategies to improve the care of this population, with emphasis on strengthening multi-professional care.

ABSTRACT Objective To evaluate the incidence of variant hemoglobins of newborn samples from the Neonatal Screening Center in the state of Mato Grosso do Sul, Brazil, and to analyze the distribution and spatial autocorrelation of newborns with sickle cell trait. Methods Samples from 35,858 newborns screened by the Neonatal Screening Center. The samples with inconclusive diagnosis were submitted to electrophoretic, chromatographic, cytological and molecular analyses. The spatial distribution analysis of newborns with sickle cell trait was performed by spatial autocorrelation. Results A total of 919 newborns showed an abnormal hemoglobin profile; in that, ten genotypes had significant clinical impacts identified. Among the asymptomatic newborns, the sickle cell trait was the most frequent (incidence of 1.885 cases/100 newborns). The highest incidence rates were registered in the municipalities of Terenos, Figueirão, Corguinho and Selvíria. There was positive spatial autocorrelation between the proportion of declared individuals of black race/color and the incidence of newborns with sickle cell trait. Conclusion The early diagnosis by neonatal screening and laboratory tests was very important to identify abnormal hemoglobin profiles and guide the spatial autocorrelation analysis of sickle cell trait newborns in Mato Grosso do Sul, serving as a support to anticipate health measures aimed to discuss efficient therapeutic behaviors and effective planning of municipalities with the greatest need for care, monitoring and orientations for affected families.

OBJETIVO: quantificar a ocorrência de movimentos primários de língua em 130 crianças respiradores oronasais e em 89 respiradoras nasais, com idades de cinco a 12 anos, avaliadas no Setor de Respiração Oral do Ambulatório de Otorrinolaringologia do Hospital de Clínicas da Universidade Estadual de Campinas. MÉTODOS: no momento da avaliação fonoaudiológica, foi solicitado à criança deglutir saliva, fechar os olhos, colocar a língua para fora da boca e permanecer assim por, no mínimo, um minuto, tempo necessário para os movimentos primários serem observados. Duas fonoaudiólogas observaram juntas cada criança, a fim de constatar a presença dos movimentos ou não. RESULTADOS: sobre a ocorrência de movimentos primários de língua, foi encontrada em 44,75% da amostra (n=98), enquanto 55,25% (n=121) não apresentaram tais movimentos. Foi observado que as categorias, nasal e oronasal, apresentaram diferenças estatisticamente para a ocorrência de movimentos primários de língua. CONCLUSÃO: em respiração oronasal há uma maior freqüência de manutenção dos movimentos primários de língua e há diferença estatisticamente significante entre o grupo de respiradores oronasais e respiradores nasais para a variável movimento primário de língua.

PURPOSE: to quantify the occurrence of primary tongue movements in 130 children with oronasal breathing and in 89 with nasal breathing with ages between 5 and 12 years old, evaluated in the Section of Mouth Breathing, of the Clinical Hospital of Unicamp, within the Otorrinolaringology Clinic. METHODS: during the evaluation, the children were requested to swallow saliva, close their eyes and put their tongue outside the mouth. They were supposed to stay like this for, at least, one minute, which is enough time to monitor their primary movements. Two language speech pathologists monitored each child, in order to verify the presence of movements. RESULTS: the occurrence of the primary tongue movements was found in 44.75% of the sample (n=98), while 55.25% (n=121) did not show any such movements. It was observed that both nasal and oronasal categories showed statistically significant difference as for the occurrences of primary tongue movements. CONCLUSION: oronasal breathing has higher frequency for maintenance of primary tongue movements, and there is statistically significant difference between oronasal and nasal breathing groups as for the primary tongue movement's variable.

OBJETIVO: propor um protocolo específico para documentação fotográfica do paciente na área de Motricidade Oral. MÉTODO: foi utilizada uma câmera digital fixada em um tripé e foram realizadas fotografias padronizadas corporais e de face. Realizou-se ainda, a teleradiografia lateral contrastada com bário sobre a língua. RESULTADOS: as fotografias facilitaram a visualização dos nossos resultados terapêuticos. CONCLUSÃO: concluiu-se que a documentação proposta auxilia no diagnóstico e no estudo do prognóstico do paciente, bem como, pode ser utilizado como material auxiliar nas orientações aos responsáveis e ao paciente.

PURPOSE: to propose a specific protocol for photographic register of Oral Motricity patients. METHODS: a digital camera mounted on a tripod was used. Standardized photos of body and face were taken. A tele-radiography modified by using contrast with barium which was spread over the patient's tongue was taken. RESULT: the pictures made easy the visualization of our therapeutic results. CONCLUSION: so far it is possible to conclude that the proposed documentation helps in the diagnosis process and prognosis analysis, and it may be used as supplementary material for guiding patients and parents on the best treatment to follow.
