Knowledge and attitudes of rural healthcare providers regarding domestic violence against women: a systematic review

ABSTRACT BACKGROUND: Specific types of violence such as intimate partner sexual violence and intimate partner homicide occur more frequently in rural areas. OBJECTIVE: This study aimed to systematically review the literature on the knowledge and attitudes of rural healthcare providers regarding cases of domestic violence against women. DESIGN AND SETTING: Systematic review developed at Universidade Federal de Uberlândia. METHODS: We conducted an electronic search of six databases, which only included observational studies, regardless of the year, language, or country of publication, except for studies that used secondary data and were exclusively qualitative. Two reviewers performed the selection, data extraction, and risk of bias assessment using a specific Joanna Briggs Institute tool. RESULTS: Six studies met the inclusion criteria. All the studies had a low risk of bias. Approximately 38% of these professionals identified injuries caused by violence in patients. When asked about knowing the correct attitude to take in cases of confirmed violence, between 12% and 64% of rural healthcare providers answered positively; most of them would refer to specialized institutions and promote victim empowerment and counseling. The number of professionals with an educational background in the field ranged from 16% to 98%. CONCLUSIONS: The evident disparity across studies shows that some professionals have suboptimal knowledge and require training to adopt the correct attitude when identifying female victims of domestic violence in clinical practice. ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Abstract The aim of this study was to evaluate the distribution of chemical and physical fractions of soil organic matter (SOM) in anthropic horizons of soil profiles in the west of Pará. The highest total organic carbon values were observed in the superficial horizons, especially in the antrópico LAd and antrópico CHd (45.0g kg-1). Were observed predominance of recalcitrant organic material. It appears that most of the C found in the SOM fractions is associated with C stabilization mechanisms such as the recalcitrance provided by the presence of pyrogenic coal, in addition to its ability to present carboxylic groups that increase its interaction with the mineral fraction of the soil, characterizing the mechanism of chemical protection. It is observed that the SOM fractions can function as indicators that contribute to better understanding of the soil carbon dynamics in soils with antrópico horizons.

Resumo O objetivo deste trabalho foi definir o tamanho ótimo de parcela e o número de repetições para a avaliação da massa de matéria fresca de híbridos de sorgo (Sorghum bicolor). Trinta e dois ensaios de uniformidade foram realizados com dois híbridos, em duas datas de semeadura e quatro períodos de avaliação. Cada ensaio foi dividido em 48 unidades experimentais básicas (UEBs) de 0,5 m2, e a massa de matéria fresca foi determinada para cada UEB. Calcularam-se a média, a variância, o coeficiente de variação, o coeficiente de autocorrelação espacial de primeira ordem, o tamanho ótimo de parcela e o coeficiente de variação do tamanho ótimo de parcela. O número de repetições foi determinado com base no maior tamanho de parcela, por meio de um processo iterativo, para as combinações de número de tratamentos e de diferenças entre médias a serem detectadas como significativas pelo teste de Tukey, a 5% de probabilidade. O tamanho ótimo de parcela oscilou entre 1,79 e 2,58 m2, e o número de repetições entre 2,6 (~3) e 49,2 (~50). O t a m a n ho óti mo de pa rcela é de 2, 58 m 2, e cinco repetições são suficientes para identificar como significativas as diferenças entre médias de tratamentos de 35%.
Abstract The objective of this work was to define the optimal plot size and number of replicates for the evaluation of the fresh weight of sorghum (Sorghum bicolor) hybrids. Thirty-two uniformity trials were carried out with two hybrids, in two sowing dates and four evaluation periods. Each trial was divided into 48 basic experimental units (BEUs) of 0.5 m2, and fresh weight was determined for each BEU. The mean, variance, coefficient of variation, first-order spatial autocorrelation coefficient, optimal plot size, and coefficient of variation of the optimal plot size were calculated. The number of replicates was determined on the basis of the largest calculated plot size, through an iterative process, for the combinations of number of treatments and differences among means to be detected as significant by Tukey’s test, at 5% probability. The optimal plot size ranged from 1.79 to 2.58 m2, and the number of replicates from 2.6 (~3) to 49.2 (~50). The optimal plot size is 2.58 m2, and five replicates are sufficient to identify as significant the differences between treatment means of 35%.

ABSTRACT: The objective of this study was to determine the enamel mass variation after a prolonged bleaching treatment using a calcium-containing 4% hydrogen peroxide gel Twenty sound bovine incisors were randomly assigned to two groups (n=10) stored in G1) distilled water and G2) artificial saliva. An electronic analytic scale (.000 grams measurements) was used to determine the enamel mass variation before and after the bleaching procedures at the following evaluation times: T0) before the bleaching procedures; T1) 14 days of treatment, as instructed by the manufacturer; T2) 21 days of treatment, 50 % beyond what is instructed by the manufacturer; and T3) 28 days of treatment, 100 % beyond what is instructed by the manufacturer. The highest mean was observed at T2/G2 (0.3259 g) and the lowest at T2/G1 (0.3265 g). The specimens stored in distilled water (G1) showed 6 % mass reduction when T0 (0.3277 g) was compared to T3 (0.3277 g). On the other hand, the specimens stored in artificial saliva exhibited a significant mass increase of 19 % when T0 (0.3521 g) was compared to T3 (0.3528 g). Prolonged bleaching therapy using 4 % hydrogen peroxide with calcium resulted in a massive reduction when water was used as a storage medium. When the specimens were stored in artificial saliva, an increase in mass was observed, probably due to the mineralizing properties of the artificial saliva.
RESUMEN: El objetivo de este estudio fue determinar la variación de la masa del esmalte después de un tratamiento de blanqueamiento prolongado utilizando calcio conteniendo un gel de peróxido de hidrógeno al 4 %. Veinte incisivos bovinos intactos fueron asignados aleatoriamente a dos grupos (n-10) almacenados en G1 - agua destilada y G2- saliva artificial. Se utilizó una escala analítica electrónica (.000 gramos) para determinar la variación de la masa de esmalte antes y después de los procedimientos de blanqueamiento en los siguientes tiempos de evaluación: T0) antes de los procedimientos blanqueadores; T1) 14 días de tratamiento, según las instrucciones del fabricante; T2) 21 días de tratamiento, 50 % más allá del tiempo indicado por el fabricante; y T3) 28 días de tratamiento, 100 % más allá del tiempo indicado por el fabricante. La media más alta se observó en T2/G2 (0,3259 g) y la más baja en T2/G1 (0,3265 g). Los especímenes almacenados en agua destilada (G1) mostraron una reducción de masa del 6 % cuando se comparó T0 (0,3277 g) con T3 (0,3277 g). Por otro lado, los dientes almacenados en saliva artificial mostraron un aumento significativo de masa del 19 % cuando se comparó T0 (0,3521 g) con T3 (0,3528 g). La terapia de blanqueamiemto prolongado con calcio conteniendo un gel de peróxido de hidrógeno al 4 % condujo a una reducción masiva cuando se utilizó agua como medio de almacenamiento, mientras que los dientes almacenados en saliva artificial mostraron un aumento en la masa, probablemente debido a las propiedades remineralizadoras de la saliva artificial.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract The use of polymer blends and composites based on fossil-based and bio-based polymers has become an important environmentally protective alternative for common use and disposable plastics applications such as packaging, bottles and trays. The disposal of these more degradable products, however, may also harm the environment and, therefore, recycling these systems becomes relevant. Recycling involves reprocessing which can significantly change the morphology and properties of polymeric products. Therefore, this study deals with the effects of reprocessing cycles on the properties and morphology of blends and nanocomposites based on fossil and bio-based polymers. The systems investigated were: a) neat polypropylene (PP), b) a polypropylene/poly(3-hydroxybutyrate) (PP/PHB) blend and c) PP/PHB/organoclay nanocomposite compatibilized with polypropylene-g-maleic anhydride (PP-g-MA) and erucamide. These materials were submitted to up to seven extrusion cycles in a single screw extruder operating at 60 rpm. Samples were taken after the first, third, fifth and seventh extrusion cycles and their tensile properties and morphology were determined. Scanning electron microscopy indicated that two phases were observed in the blend which showed spherical PHB domains. The addition of clay, PP-g-MA and erucamide improved the adhesion between the nanocomposites components. X-ray diffraction analysis showed that crystallinity tended to increase with the number of reprocessing cycles for all systems investigated up to the fifth cycle and then tended to decrease. A 10% crystallinity increase was observed for neat PP in the fifth cycle. In general, the tensile properties of all systems decreased with reprocessing and the highest losses were observed for the PP/PHB blend after seven processing cycles with 50% and 37% decreases in stress at break and elastic modulus, respectively. Impact strength of the PP matrix and of the PP/PHB blend tended to decrease with reprocessing, except for the nanocomposite which showed a slight increase especially after the seventh processing cycle in which an 18% increase in impact strength was observed.

Abstract The ultrastructural and mechanical properties of enamel surface were evaluated after prolonged bleaching treatments with 10% carbamide peroxide in the presence or absence of orange juice (erosive challenge) and toothbrushing (abrasive challenge). In total, 145 incisor bovine teeth were used in this study. Twenty-five samples were prepared for the ultrastructural evaluations, and 120 samples were prepared for microhardness and roughness tests. These 120 samples were divided into eight experimental groups (n = 15): G1- artificial saliva; G2- abrasion; G3- erosion; G4- dental bleaching; G5- erosion + abrasion; G6- bleaching + abrasion; G7- bleaching + erosion; and G8- bleaching + erosion + abrasion. All groups were tested at T0 (before treatment), T1 (14 days), T2 (21 days), and T3 (28 days). Two-way analysis of variance for repeated measures and the post hoc Sidak tests (p ≤ 0.05) were used. The roughness evaluation demonstrated an increase in damage for all experimental groups with an increase in the time period. For microhardness, the groups exposed to artificial saliva (AS) and abrasive challenge did not show any differences at any time points, while the other groups showed a decrease in microhardness from T0 to T3. Ultrastructural evaluation showed different surface alterations in response to the treatments. Despite prolonged bleaching periods, the procedure caused lesser enamel surface alterations than exposure to orange juice alone or in combination with brushing.

Abstract Coronavirus disease 2019 (COVID-19), caused by the Severe Acute Respiratory Syndrome Coronavirus type 2 (SARS-CoV-2), is the largest pandemic in modern history with very high infection rates and considerable mortality. The disease, which emerged in China’s Wuhan province, had its first reported case on December 29, 2019, and spread rapidly worldwide. On March 11, 2020, the World Health Organization (WHO) declared the COVID-19 outbreak a pandemic and global health emergency. Since the outbreak, efforts to develop COVID-19 vaccines, engineer new drugs, and evaluate existing ones for drug repurposing have been intensively undertaken to find ways to control this pandemic. COVID-19 therapeutic strategies aim to impair molecular pathways involved in the virus entrance and replication or interfere in the patients’ overreaction and immunopathology. Moreover, nanotechnology could be an approach to boost the activity of new drugs. Several COVID-19 vaccine candidates have received emergency-use or full authorization in one or more countries, and others are being developed and tested. This review assesses the different strategies currently proposed to control COVID-19 and the issues or limitations imposed on some approaches by the human and viral genetic variability.

Além disso, doenças neurológicas prévias podem exigir tratamento especial e estar associadas a piores desfechos. Não obstante, o papel dos neurologistas na COVID-19 é provavelmente pouco reconhecido. Objetivo: O objetivo deste estudo foi relatar os motivos para solicitar consultas neurológicas por clínicos e intensivistas em um hospital dedicado à COVID-19. Métodos: Estudo retrospectivo realizado no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, Brasil, um centro dedicado à COVID-19 com 900 leitos (incluindo 300 leitos para unidades de terapia intensiva). O diagnóstico de COVID-19 foi confirmado por SARS-CoV-2-RT-PCR em swabs nasais. Todas as interconsultas de neurologia hospitalar entre 23 de março e 23 de maio de 2020 foram analisadas. Os neurologistas realizaram o exame neurológico, avaliaram todos os dados disponíveis para diagnosticar a patologia neurológica e solicitaram exames adicionais conforme necessidade. Diagnósticos difíceis foram estabelecidos em reuniões de consenso. Após o diagnóstico, os neurologistas participaram da condução dos casos. Resultados: Foram solicitadas consultas neurológicas para 89 de 1.208 (7,4%) em pacientes internados por COVID-19 durante o período. Os principais diagnósticos neurológicos incluíram: encefalopatia (44,4%), acidente vascular cerebral (16,7%), doenças neurológicas prévias (9,0%), crises epilépticas (9,0%), transtornos neuromusculares (5,6%), outras lesões encefálicas agudas (3,4%) e outros sintomas leves inespecíficos (11,2%). Conclusões: A maioria das consultas neurológicas em um hospital dedicado à COVID-19 foi solicitada para condições graves que poderiam afetar o desfecho clínico. Os médicos na linha de frente devem ser capazes de reconhecer sintomas neurológicos. Os neurologistas são membros importantes da equipe médica no atendimento hospitalar à COVID-19.

ABSTRACT Hexapods, commonly known as insects, are a neglected taxonomic group in the Fernando de Noronha archipelago, with unanswered questions about their species richness and the ecological processes in which they are involved (e.g., colonization, introduction, establishment, and extinction). Herein, we provide an updated Hexapod checklist with current nomenclatural combinations. The entomofauna of the Fernando de Noronha archipelago is currently composed of 453 species in 21 orders. The orders, and their respective number of species, are: Blattaria (9), Coleoptera (118), Collembola (29), Dermaptera (3), Diplura (1), Diptera (134), Embioptera (1), Hemiptera (29), Hymenoptera (59), Isoptera (2), Lepidoptera (25), Mantodea (1), Neuroptera (3), Odonata (5), Orthoptera (11), Phasmatodea (1), Phthiraptera (6), Psocoptera (3), Siphonaptera (1), Thysanoptera (10), and Zygentoma (2). The archipelago has 263 new taxon records (family + genera + species). Thirty-eight species (3.39%) were described from local specimens and most of them are likely endemic species. This study more than doubles our knowledge (from the previous 190 records) of the entomofauna in this large Brazilian archipelago. This study also provides a baseline for studies on its conservation status and for implementing future environmental management programs.

RESUMO: O cruzamento das culturas de trigo e centeio resultou na obtenção da cultura do triticale, que apresenta rusticidade, versatilidade na alimentação animal e humana e possibilidade de uso como planta de cobertura. O objetivo deste trabalho foi determinar o tamanho ótimo de parcela e o número de repetições para avaliar a massa verde de triticale em dois momentos de avaliação. Foi conduzido um experimento com a cultivar de triticale IPR111, sendo a área experimental dividida em 48 ensaios de uniformidade, cada ensaio contendo 36 unidades experimentais básicas de 0,51 m2. A massa verde foi avaliada em 24 ensaios aos 99 dias após a semeadura (DAS ) e em 24 ensaios aos 127 DAS. O tamanho ótimo de parcela foi determinado pelo método da máxima curvatura do coeficiente de variação e o número de repetições foi determinado em combinações de número de tratamentos e de diferenças entre médias a serem detectadas como significativas pelo teste de Tukey. Para determinar a massa verde de triticale, o tamanho ótimo de parcela é de 3,12 m2 com coeficiente de variação de 13,69%. Seis repetições são suficientes para identificar como significativas, diferenças entre médias de tratamentos de 25% para experimentos com até sete tratamentos e de 30% para experimentos com até 28 tratamentos, independentemente do delineamento experimental.
ABSTRACT: The hybridization between wheat and rye crops resulted in the triticale crop, which presents rusticity, versatility in animal and human food and possibility of use as a cover plant. The objective of this research was to determine the optimal plot size and the replications number to evaluate the fresh weight of triticale in two evaluation moments. An experiment was carried out with the triticale cultivar IPR111. The experimental area was divided into 48 uniformity trials, each containing 36 basic experimental units of 0.51 m2. The fresh weight was evaluated in 24 uniformity trials at 99 days after sowing (DAS) and in 24 uniformity trials at 127 DAS. The optimal plot size was determined by the method of the maximum curvature of the coefficient of variation and the replications number was determined in scenarios of treatments number and differences between means to be detected as significant by Tukey test. To determine the fresh weight of triticale, the optimal plot size is 3.12 m2, with coefficient of variation of 13.69%. Six replications are sufficient to identify as significant, differences between treatment means of 25% for experiments with up to seven treatments and of 30% for experiments with up to 28 treatments, regardless of the experimental design.

BACKGROUND Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was confirmed in Brazil in February 2020, the first cases were followed by an increase in the number of cases throughout the country, resulting in an important public health crisis that requires fast and coordinated responses. OBJECTIVES The objective of this work is to describe the isolation and propagation properties of SARS-CoV-2 isolates from the first confirmed cases of coronavirus disease 2019 (COVID-19) in Brazil. METHODS After diagnosis in patients that returned from Italy to the São Paulo city in late February by RT-PCR, SARS-CoV-2 isolates were obtained in cell cultures and characterised by full genome sequencing, electron microscopy and in vitro replication properties. FINDINGS The virus isolate was recovered from nasopharyngeal specimen, propagated in Vero cells (E6, CCL-81 and hSLAM), with clear cytopathic effects, and characterised by full genome sequencing, electron microscopy and in vitro replication properties. Virus stocks - viable (titre 2.11 × 106 TCID50/mL, titre 1.5 × 106 PFUs/mL) and inactivated from isolate SARS.CoV2/SP02.2020.HIAE.Br were prepared and set available to the public health authorities and the scientific community in Brazil and abroad. MAIN CONCLUSION We believe that the protocols for virus growth and studies here described and the distribution initiative may constitute a viable model for other developing countries, not only to help a rapid effective pandemic response, but also to facilitate and support basic scientific research.

Resumo As leishmanioses são um grupo de doenças de importância zoonótica causadas por mais de 20 espécies de protozoários do gênero Leishmania, sendo o cão doméstico considerado o principal reservatório da doença. No entanto, diversas pesquisas têm investigado o envolvimento de outros vertebrados como reservatórios do parasita. Portanto, o objetivo do presente estudo foi realizar uma revisão sistemática com meta-análise da ocorrência de leishmaniose em equídeos. Os relatos de caso descreviam animais com sintomas cutâneos de leishmaniose (pápulas, nódulos, úlceras, crostas) que regrediam espontaneamente, localizadas principalmente na cabeça e membros, sendo identificadas três espécies do protozoário nas lesões: Leishmania braziliensis, Leishmania infantum e Leishmania siamensis. Por sua vez, a meta-análise evidenciou uma prevalência combinada de 25%, porém com alta heterogeneidade entre os estudos, atribuída às diferenças nos métodos utilizados no diagnóstico da doença. A leishmaniose em equídeos é uma doença benigna, porém deve ser incluída no diagnóstico diferencial de doenças cutâneas nessas espécies. Os estudos soroepidemiológicos são importantes para investigar e monitorar a suspeita de exposição desses hospedeiros ao parasita, principalmente em áreas endêmicas, porém há necessidade de padronização dos métodos de diagnóstico.
Abstract Leishmaniases are a group of diseases of zoonotic importance caused by over 20 species of protozoa of the genus Leishmania, in which domestic dogs are considered to be the main reservoir for the disease. However, the involvement of other vertebrates as reservoirs for these parasites has also been investigated. Therefore, the objective of the present study was to carry out a systematic review with meta-analysis on occurrences of leishmaniasis in equids. The case reports described animals with cutaneous symptoms of leishmaniasis (papules, nodules, ulcers or crusts) that regressed spontaneously, located mainly on the head and limbs, from which three species of protozoa were identified in the lesions: Leishmania braziliensis, Leishmania infantum and Leishmania siamensis. In turn, the meta-analysis showed a combined prevalence of 25%, although with high heterogeneity among the studies, which was attributed to the use of different methods for diagnosing the disease. Leishmaniasis in equids is a benign disease but it should be included in the differential diagnosis of cutaneous diseases among these species. Seroepidemiological studies are important in investigating and monitoring suspected exposure of these hosts to the parasite, especially in endemic areas. However, there is also a need to standardize diagnostic methods.
