This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.
nbsp;
Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Abstract INTRODUCTION: Viral hepatitis is a major public health problem. It is necessary to understand the epidemic, verifying the combination of biological and demographic characteristics. METHODS: This is an analytical ecological and epidemiological study. Confirmed case data from the Notification Disease Information System (SINAN) were used. RESULTS: From 2009-2018, SINAN confirmed 404,003 viral hepatitis cases in Brazil, with 12.49%, 37.06%, and 48.28% cases of hepatitis A, B, and C, respectively. CONCLUSIONS: In Brazil, 4,296 deaths were associated with viral hepatitis, of which 36.66% were associated with acute hepatitis B. The proportional distribution of cases varied among the five Brazilian regions.

Abstract Guava (Psidium guajava L.), is adapted to tropical and subtropical climates, and, in addition to its nutritional value, has great medicinal potential. One of the medicinal effects is antibacterial, and this can be identified from the phytochemicals present in its various parts, especially the leaf, which contains flavonoids, phenols, and tannins, as well as phytocomposites with antibacterial action. Therefore, the interaction of this plant with arbuscular mycorrhizal fungi and Meloidogyne enterolobii is a biotechnological resource that can increase the production of secondary metabolites so that the guava ethanolic extract is effective against multidrug-resistant bacterial strains. Therefore, the objective of this study was to test the inhibitory action of mycorrhizal guava leaf extract and Meloidogyne enterolobii on strains of Klebsiella pneumoniae carbapenemase. Guava seedlings from cuttings were inoculated with Acaulospora longula, and later with Meloidogyne enterolobii; the leaves were harvested at two maturation times of the plant and placed in an oven. Next, a leaf extract was prepared using ethanol as a solvent. The extract was tested in multidrug-resistant strains of K. pneumoniae carbapenemase from operative wounds using disc diffusion methodology. The plant-AMF-phytonematode interaction positively potentialize the inhibitory action of guava leaf ethanolic extract on multidrug-resistant bacterial strains.

Abstract INTRODUCTION: Schistosomiasis, caused by infection from Schistosoma mansoni, is a disease that represents an important public health problem for Brazil, especially for states in the Northeast region. Thus, the aim of this study is to present a new epidemiological profile for the disease in a municipality with low prevalence in the state of Alagoas, Brazil. METHODS: A cross-sectional study was conducted through a coproparasitological and malacological survey. A structured questionnaire was applied to the study participants to survey possible risk factors and a spatial analysis (kernel density) was used to measure the risk of infection. RESULTS: Of the 347 participants, 106 (30.5%) were infected by Schistosoma mansoni, most of them from the urban area of the municipality (68.9%; 73/106). A 3-fold risk of infection was found for individuals living in the urban area and a risk of 2.15 times for self-declared farmers. Biomphalaria glabrata and B. straminea were the species found in the municipality, but no animals were diagnosed as infected by the parasite. Spatial analysis showed a random distribution of vectors and human cases of the disease, and the formation of two clusters of human cases in the urban area was seen. CONCLUSIONS: A new epidemiological profile for schistosomiasis from S. mansoni infection was presented in a municipality of low endemicity: a high proportion of positive individuals in the urban area; presence of snails without positive diagnosis for S. mansoni infection; random distribution of vectors and human cases; and absence of association between classical risk factors and human infection.

Abstract: Since its inception in 2007, the Colloidal Materials Group, one of the research groups in nanotechnology of the Institute of Chemistry of São Carlos (IQSC) at University of São Paulo (USP) carries out studies related to the development of syntheses methods of nanoparticles and multifunctional nanostructured systems. Our works search new synthesis methodologies that allow high size and shape control of the individual nanoparticles or that compose the nanostructured systems and has as a principle the synthetic approaches based on the modified polyol and thermal decomposition processes. Systems involving nanoparticles have received extensive attention both in the fundamental research and in several technological applications exploring the unique properties presented in nanomaterials. These properties are strongly size- and shape-dependent of the nanoparticles and a large distribution of size or shape implies in a high response dispersion, justifying the intense research for the so-called nanoparticle monodisperse systems. In this review, we present the main aspects to obtain monodisperse nanoparticulate systems, correlating with the synthesis processes used in our group and some of our results in systems involving nanoparticles with magnetic, optical, and electronic properties, as well as some obtained composite materials for different applications.

Four new epimeric spirostanol and furostanol-type steroidal saponins, (25R,S)-5α-spirostan-2α,3β-diol 3-O-β-D-glucopyranosyl-(1→4)-β-D-galactopyranosyl-(1→4)-β-D-galactopyranoside, (25R,S)-5α-spirostan-2α,3β-diol 3-O-β-D-glucopyranosyl-(1→2)-α-L-rhamnopyranosyl-(1→4)-β-D-galactopyranoside, 26-O-β-D-glucopyranosyl-(25R,S)-5α-furost-20-ene,2α,3β-diol 3-O-β-D-galactopyranoside and 26-O-β-D-glucopyranosyl-(25R,S)-5α-furost-20-ene,2α,3β-diol 3-O-β-D-galactopyranosyl-(1→4)-β-D-galactopyranoside, in addition to the known (25R,S)-5α-spirostan-2α,3β-diol 3-O-β-D-galactopyranoside, were isolated from Cestrum laevigatum. Compounds were submitted to cytotoxic activity assays using colorectal adenocarcinoma (HCT-116), human promyelocytic leukemia (HL-60), ovarian carcinoma (OVCAR-8), glioma (SF-295) human cancer cell lines, and the antimicrobial activity was evaluated against Candida parapsilosis, C. albicans, C. krusei, Pseudomonas aeruginosa, Staphylococcus aureus and Bacillus subtilis.

RESUMO A distribuição de calda proporcionada por pulverizadores hidropneumáticos, dentre outros fatores, deve ser analisada com critério, sob pena de elevar custos de produção e causar contaminação ambiental. Objetivou-se avaliar o efeito da distribuição volumétrica vertical de pulverizador hidropneumático na eficácia de controle de Brevipalpus phoenicis. Primeiramente, realizou-se ensaio visando a avaliar a uniformidade de distribuição volumétrica vertical do pulverizador, considerando-se diferentes configurações de pontas nos arcos de pulverização, lados e altura de pulverização. Em seguida, realizou-se outro experimento, com o intuito de analisar a relação entre a distribuição do acaricida Fenpiroximato, aplicado a lavoura de Coffea arabica L., e o controle de B. phoenicis. Os tratamentos foram dispostos em esquema fatorial 4 x 2 + 1 testemunha adicional, sendo eles: quatro volumes de calda (200 L ha-1, 400 L ha-1, 600 L ha-1 e 800 L ha-1) e duas configurações de pontas (1: 100 % das pontas MAG1,5; 2: parte superior com 5 pontas MAG3,0, parte mediana com 9 pontas MAG1,5 e parte inferior com 4 pontas MAG3,0). A configuração 2 proporcionou o menor coeficiente de variação (28 %). As configurações de pontas não apresentaram efeito na incidência de B. phoenicis. A utilização de pontas com diferentes vazões no arco de pulverização melhora a uniformidade de distribuição volumétrica vertical dos pulverizadores hidropneumáticos, mas não influencia na eficácia de controle de B. phoenicis.
ABSTRACT Spray distribution generated by axial fan sprayer, among other factors, should be critically analyzed, in order to prevent unnecessary costs and environmental contamination. This study aimed at evaluating the effect of the vertical volumetric distribution of an axial fan sprayer on the Brevipalpus phoenicis control efficiency. First, an assay was performed to evaluate the vertical volume distribution uniformity of the sprayer, assessing different spray nozzles configurations on the spray arcs, sides of the sprayer and spray heights. Afterwards, another experiment was carried out to analyze the relationship between the distribution of the Fenpyroximate acaricide on Coffea arabica L. and the B. phoenicis control efficiency. The treatments were arranged in a 4 x 2 factorial + 1 additional control, consisting of four spray volumes (200 L ha-1, 400 L ha-1, 600 L ha-1 and 800 L ha-1) and two spray nozzles configurations (1: 100 % of MAG1.5 nozzles; 2: upper extension with 5 MAG3.0 nozzles, middle extension with 9 MAG1.5 nozzles and lower extension with 4 MAG3.0 nozzles). Configuration 2 provided the smallest coefficient of variation (28 %). Nozzle configurations had no effect on the incidence of B. phoenicis. The use of nozzles with different flow rates on the spraying arc improves the vertical volumetric distribution uniformity of axial fan sprayers, but does not affect the B. phoenicis control efficiency.

Application of a Quantitative HPLC-ESI-MS/MS Method for Flavonoids in Different Vegetables Matrices

Plant flavonoids have been increasingly studied for its importance in plant metabolism, inter-species interactions and human health-promoting effects. Each botanical species has a distinct flavonoid profile, and as such, this work aimed to develop and apply a high-performance liquid chromatography coupled to electrospray ionization tandem mass spectrometry (HPLC-ESI-MS/MS) method to quantify specific flavonoids in different plants and parts. Six flavonoids (isoquercitrin, quercetin, naringin, naringenin, hesperidin and rutin), two isoflavonoids (daidzin and daidzein) and one coumarin (umbelliferone) were targeted. Calibration curves showed good correlations (R2 > 0.994) and limits of quantification (≤ 500 ng mL-1). The method was applied for detection of analytes in common bean seeds (Phaseolus vulgaris), soybean leaves (Glycine max), and sour orange (Citrus aurantium), sweet orange (Citrus sinensis), and Tahiti lime (Citrus latifolia) flavedo after ultrasound assisted acidic hydroalcoholic extraction. The method was tuned for quick and efficient extraction. Principal component analysis was helpful for the discrimination of matrices based on their flavonoid profiles.

Resumo Até 2010, o conhecimento sobre a diversidade de fungos do Brasil estava registrado em publicações esparsas de taxonomia e ecologia e em algumas poucas listas de espécies. Com a publicação do Catálogo de Plantas e Fungos do Brasil, e a disponibilização da lista online, tem sido possível agregar o conhecimento disperso. A versão ora apresentada acrescenta 2.111 nomes de espécies aos 3.608 listados em 2010. São citadas 5.719 espécies de fungos distribuídas em 1.246 gêneros, 102 ordens e 13 divisões, consistindo em considerável aumento em relação a 2010, quando estavam registrados 924 gêneros e 78 ordens. Predominam os Basidiomycota (2.741 espécies, em 22 ordens) e Ascomycota (1.881 espécies, em 41 ordens). A Mata Atlântica possui a maior quantidade de registros, com 3.017 espécies, seguido pela Amazonia (1.050), Caatinga (999), Cerrado (638) e Pampa e Pantanal com 84 e 35 espécies, respectivamente. A região Nordeste tem a maior riqueza (2.617 especies), seguida pelo Sudeste (2.252), Sul (1.995), Norte (1.301) e Centro Oeste (488 espécies). Em relação aos Estados da Federação, São Paulo (1.846 espécies), Pernambuco (1.611) e Rio Grande do Sul (1.377) são os mais diversos.
Abstract Knowledge about the Brazilian fungal diversity was, until 2010, recorded in few taxonomy and ecology publications, as well as in a handful of species lists. With the publication of the Catálogo de Plantas e Fungos do Brasil and the continued availability of an online list, it has been possible to aggregate this dispersed knowledge. The version presented here adds 2,111 species names to the 3,608 listed in 2010. A total of 5,719 species of fungi distributed in 1,246 genera, 102 orders and 13 phyla represents a considerable increase over the last five years, when only 924 genera and 78 orders were registered. Basidiomycota (2,741 species in 22 orders) and Ascomycota (1,881 species in 41 orders) predominate over other groups. The Atlantic Rainforest has the largest number of records, with 3,017 species, followed by Amazon Rainforest (1,050), Caatinga (999), Cerrado (638) and Pampa and Pantanal with 84 and 35 species, respectively. The Northeast region has the greatest richness (2,617 species), followed by Southeast (2,252), South (1,995), North (1,301) and Central-West (488 species). Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

O manejo de irrigação por gotejamento e a colheita mecanizada na cultura da cana-de-açúcar estão sugerindo espaçamento entrelinhas de plantio do tipo combinado. Neste contexto, o objetivo do estudo foi avaliar o crescimento da variedade RB98710 irrigada por gotejamento em dois tipos de espaçamentos entrelinhas de plantio; para isto foram realizados dois plantios em agosto de 2011, sendo um com espaçamento entrelinhas de plantio único (1,0 m) e outro combinado (0,6 e 1,4 m). Os dados analisados foram variáveis de crescimento (índice de área foliar, altura do dossel, comprimento dos colmos e plantas por metro quadrado), variáveis de produção e elementos meteorológicos. O valor do fator de forma para a área foliar foi igual a 0,65; em ambos os espaçamentos entrelinhas de plantio as variáveis de crescimento foram relacionadas com os graus-dia acumulados e tiveram equações ajustadas com aplicações satisfatórias. As variáveis de crescimento avaliadas não diferiram entre os espaçamentos de plantio utilizados. As produtividades de colmos frescos (100,9 t ha-1) e de açúcar (17,8 t ha-1) da cana-de-açúcar irrigada cultivada em Rio Largo, Alagoas, não foram influenciadas pelo uso dos diferentes tipos de espaçamentos entrelinhas de plantio.
The management of drip irrigation and mechanized harvesting in the sugarcane crop are suggesting combined row spacing. In this context the objective of this study was to evaluate the growth of RB98710 variety, with drip irrigation in two row spacing. For this, two plantings were performed in August 2011, with single (1.0 m) and combined row spacing (0.6 and 1.4 m). The analysed data were variables of plant growth (leaf area index, canopy height, length of stems and plants per square meter), production variables and weather elements. The form factor value for leaf area was equal to 0.65. In both row spacing, the variables of plant growth were related with accumulated degree-days and had adjusted equations with satisfactory applications. The evaluated variables of plant growth were not different by the used row spacing configurations. The fresh stalk yield (100.9 t ha-1) and sugar yield (17.8 t ha-1) of irrigated sugarcane crop cultivated in Rio Largo, Alagoas state, Brazil, were not influenced by the use of different row spacing configurations.
