RESUMO Encontrar espécies florestais adaptadas a diversas condições edafoclimáticas, e que forneçam atributos favoráveis ao uso comercial é um desafio. A Azadirachta indica é uma espécie potencial para amenizar este problema. Assim, o objetivo deste trabalho foi realizar o resgate vegetativo de diferentes árvores adultas de Azadirachta indica Juss por estaquia. Para tanto, indivíduos de Azadirachta indica foram selecionados aleatoriamente em um plantio da espécie com 10 anos de implantação e realizada a decepa à altura de 30 cm. Em sequência, verificamos: a capacidade de rebrota das cepas (coletas e medições do comprimento das brotações aos 15, 30, 45 e 60 dias), a necessidade de usar auxina exógena para indução do enraizamento (testando 0 (controle), 2000, 4000 e 6000 mg.l-1 de AIB), o tamanho ideal do propágulo (propágulos com 8 cm e 13 cm de altura) e a dinâmica de enraizamento (cada sete dias, em um período de 35 dias, os propágulos foram avaliados quanto a: formação de calos, oxidação e enraizamento). A espécie demostrou alta capacidade de rebrota, bem como facilidade de enraizamento com ou sem a utilização de AIB; verificou-se que o tamanho ideal para o propágulo é 13 cm e que o processo de enraizamento e alongamento das raízes se completa após 35 dias do estaqueamento. Concluímos que o uso da estaquia para o resgate vegetativo da espécie é interessante, recomendamos que 13cm é adequado para o tamanho do propágulo e o auxílio do AIB na dose de 2.000 mg L-1 é ideal para uniformizar o enraizamento.

ABSTRACT Finding forest species adapted to different soil and climatic conditions and, that provide favorable attributes to commercial use is a challenge. Azadirachta indica is a potential species to attend to this goal. Thus, this paper aims to carry out the vegetative rescue of different Azadirachta indica Juss adult trees by cuttings. For that, Azadirachta indica individuals were randomly selected in a plantation of species set ten years ago and the cut was carried out at 30 cm in height. In sequence, verification was carried out on: (a) the stem cut ability to regrowth (collections and measurements of the shoot length on the 15th, 30th, 45th, and 60th days); (b) the need to use exogenous auxin for rooting induction [testing 0 (control), 2000, 4000, and 6000 mg.l-1 of Indole-3-butyric acid (IBA)]; (c) the ideal of the propagule size (with 8 cm and 13 cm in height); and (c) the rooting dynamics (every seven days for thirty-five days; the propagules were evaluated for callus formation, oxidation, and rooting). The species showed high regrowth ability, as well as easy rooting with or without the use of IBA; it was found that the ideal size for the propagule is 13 cm and that the process of rooting and root elongation is completed after thirty-five days of staking. The conclusion is: (a) using cuttings for the species vegetative rescue is interesting; (b) 13 cm is considered appropriate for the propagule size, and; (c) the aid of IBA at a dose of 2,000 mg L-1 is ideal to standardize rooting.

ABSTRACT Introduction: There is a strong commitment in pursuing reconstructive solutions for areas of skin or muscular loss that require covering or stable cavity filling. The anterolateral thigh flap, described in China by Song et al. (1984), is considered by many as the optimal flap in large reconstructions. Objective: To report the applicability of the anterolateral thigh flap, as a versatile resource in reconstructions of the abdominal wall. Methods: Two patients were submitted to tumoral resection of the right colon with invasion of the abdominal wall by a team of oncologic surgeons in conjunction with the plastic surgery team that was responsible for the reconstruction of the abdominal wall. In both cases, the pedicled anterolateral thigh flap was used, which was transposed to the defect after partial resection of the abdominal wall at the same surgical time. A PROCEEDTM surgical mesh was fixed between the abdominal viscera and the flap to reinforce the abdominal wall. Results: Both patients had good postoperative evolution and are in follow-up, with no signs of tumor recurrence and with a good quality of life. Conclusion: The anterolateral thigh flap is a useful resource for the reconstruction of the abdominal wall due to large tumor resections in oncology.

RESUMO Introdução: Há um grande empenho na busca por soluções reconstrutivas para as áreas de perda cutânea ou muscular que exijam cobertura ou preenchimento cavitário estável. O retalho anterolateral da coxa, descrito na China por Song et al. (1984), é considerado por muitos como o retalho ideal nas grandes reconstruções. Objetivo: Relatar a aplicabilidade do retalho anterolateral da coxa como recurso versátil nas reconstruções da parede abdominal. Métodos: Dois pacientes foram selecionados para serem submetidos à ressecção tumoral de cólon direito com invasão da parede abdominal pela equipe de cirurgia oncológica em conjunto com a equipe de cirurgia plástica, que foi a responsável pela reconstrução da parede abdominal. Em ambos os casos, utilizou-se o retalho anterolateral da coxa pediculado, que foi transposto para o defeito após a ressecção parcial da parede abdominal no mesmo tempo cirúrgico. Entre as vísceras abdominais e o retalho, foi fixada tela de PROCEEDTM para reforço da parede abdominal. Resultados: Ambos os pacientes tiveram boa evolução pós-operatória e encontram-se em acompanhamento, sem sinais de recidiva tumoral e com boa qualidade de vida. Conclusão: O retalho anterolateral da coxa mostrou-se recurso útil dentro do arsenal terapêutico reconstrutivo da parede abdominal devido a grandes ressecções tumorais em oncologia.

Scar formation is a consequence of the wound healing process that occurs when body tissues are damaged by a physical injury. Hypertrophic scars and keloids are pathological scars resulting from abnormal responses to trauma and can be itchy and painful, causing serious functional and cosmetic disability. The current review will focus on the definition of hypertrophic scars, distinguishing them from keloids and on the various methods for treating hypertrophic scarring that have been described in the literature, including treatments with clearly proven efficiency and therapies with doubtful benefits. Numerous methods have been described for the treatment of abnormal scars, but to date, the optimal treatment method has not been established. This review will explore the differences between different types of nonsurgical management of hypertrophic scars, focusing on the indications, uses, mechanisms of action, associations and efficacies of the following therapies: silicone, pressure garments, onion extract, intralesional corticoid injections and bleomycin.

INTRODUÇÃO: Neoplasias malignas extensas em couro cabeludo e fronte tornam-se um desafio para o cirurgião plástico, em decorrência das particularidades anatômicas da região. Apesar da existência de muitas técnicas para o reparo dos defeitos, a reconstrução ideal depende da avaliação criteriosa de cada caso clínico e tem por finalidade alcançar o melhor resultado, tanto funcional como estético, com mínima morbidade no sítio doador. O objetivo deste estudo é avaliar uma série de pacientes submetidos a reconstrução imediata após ressecção oncológica em couro cabeludo e fronte, demonstrando tática pessoal e experiência do autor. MÉTODO: Trata-se de análise retrospectiva de 25 pacientes operados no período de junho de 2009 a junho de 2011, submetidos a reconstrução de couro cabeludo e da fronte após tratamento de câncer de pele avançado dessa região. Foram estudados os seguintes parâmetros: sexo, idade, diagnóstico, estadiamento clínico, localização e dimensão do defeito, tática de reparo, complicações e estado clínico atual. RESULTADOS: Amostra composta de 25 pacientes, sendo 60% homens, com média de idade de 64,8 anos. O estádio clínico III foi o mais frequente (88%), o diagnóstico de maior incidência foi o de carcinoma espinocelular (84%) e a região biparietal foi a localização mais atingida (20%). O tamanho dos defeitos variou de 3,8 cm x 3,5 cm a 22,9 cm x 15 cm. A técnica de reparo mais utilizada (80%) foi a confecção de retalho local. Ocorreram 2 (8%) casos de necrose parcial do retalho e 1 (4%) caso de perda parcial do enxerto. Todos os pacientes estão vivos, dos quais apenas um apresenta sinais de neoplasia maligna em atividade (recidiva tumoral e fora de possibilidade terapêutica). Quanto aos aspectos funcional e estético, tanto o autor como os pacientes consideram o resultado bom. CONCLUSÕES: Na literatura internacional, são descritas várias técnicas de reconstrução do couro cabeludo. Na maioria dos casos com grande perda de tecidos moles, os retalhos microcirúrgicos são as opções mais aceitas. A reconstrução com retalhos locais de avanço constitui opção bastante segura e apresenta resultados favoráveis, com realização técnica mais simples e taticamente ideal para aqueles casos limítrofes de operabilidade.

BACKGROUND: The anatomical peculiarities of extensive malignant neoplasms of the scalp and forehead remain a challenge for plastic surgeons. Although several procedures for repair of these defects have been described, the ideal reconstruction relies on careful assessment of each clinical case and aims to achieve the best functional and aesthetic outcome with minimal morbidity of the donor area. The aim of this study is to evaluate a series of patients who underwent immediate reconstruction following scalp and forehead tumor resection, in order to demonstrate the personal technique and experience of the author. METHODS: This is a retrospective analysis of 25 patients who underwent scalp and forehead reconstruction between June 2009 and June 2011 following treatment for advanced skin cancer. The following parameters were studied: gender, age, diagnosis, clinical staging, location and size of the defect, repair strategy, complications, and current clinical status. RESULTS: Of the 25 patients, 60% were men; the average age was 64.8 years. Patients most frequently presented in clinical stage III (88%). Squamous cell carcinoma was the most common tumor (84%) and the biparietal region was the most commonly affected area (20%). The size of the defects varied from 3.8 cm × 3.5 cm to 22.9 cm × 15 cm. The reconstructive procedure used most often (80%) relies on the production of a local flap. Two (8%) cases of partial flap necrosis and 1 case (4%) of partial graft loss were noted. All patients are still alive and only one shows signs of proliferative malignant neoplasm (tumor recurrence without treatment options). With regard to functional and aesthetic aspects, both the author and the patients considered the results obtained to be satisfactory. CONCLUSIONS: In the literature, a variety of procedures for scalp reconstruction have already been described. In patients with extensive loss of soft tissues, microsurgical flaps are the most commonly used option. Reconstruction with a local advancement flap is a safe technique, provides satisfactory results, and is performed with a simple technical procedure. It therefore may be ideal for borderline-operable patients.

JUSTIFICATIVA E OBJETIVOS: Analisar a qualidade e satisfação de vida (QSV) dos pacientes antes da internação na UTI e após a alta hospitalar. O objetivo deste estudo foi verificar se as características demográficas, clínicas e terapêuticas interferiram nessa QSV. MÉTODO: Foi realizado um estudo do tipo coorte prospectivo, com abordagem quanti-qualitativa. Foram analisados os pacientes internados na UTI do Hospital Universitário da Universidade Federal de Santa Catarina, por mais de 24 horas, entre os meses de abril e julho de 2005. Coletaram-se dados referentes a QSV dos pacientes antes da internação na UTI, assim como suas características demográficas, clínicas e terapêuticas. Posteriormente, aplicou-se, por telefone, 7, 90 e 180 dias após a alta hospitalar, um questionário que avaliou os dados sobre a QSV dos pacientes; subdivididos em 2 grupos: QSV melhor/inalterada e piora da QSV. Para a análise estatística foram utilizados os testes t de Student e Qui-quadrado. Foi considerado significativo < 0,05. RESULTADOS: Sessenta e oito pacientes preencheram os critérios de admissão, 21 completaram o estudo. A comparação entre a QSV de 7, 90 e 180 dias após a alta hospitalar revelou que a qualidade de vida (QV) dos pacientes melhorou e que 90,5% estavam satisfeitos. Os pacientes cuja QV foi melhor/inalterada foram os mais idosos. Os grupos foram semelhantes, em relação ao sexo, escolaridade, escore APACHE II médio, tempo de internação, uso de ventilação mecânica e fármaco vasoativo. Sessenta por cento dos pacientes havia retornado ao trabalho após 180 dias. CONCLUSÕES: Houve uma tendência de os pacientes sentirem-se satisfeitos após a alta hospitalar, bem como a melhora da qualidade de vida com o decorrer do tempo. Mesmo aqueles que referiram piora, encontravam-se bem para retomar suas atividades laborativas.

BACKGROUND AND OBJETIVES: To evaluate the quality and satisfaction of life (QSL) of patients before admission in ICU, and after hospital discharge. To verify the influence of the patient's demographic/clinic/therapeutic factors in the QSL. METHODS: Prospective cohort study with quali-quantitative approach. All patients admitted in ICU/HU/UFSC from April-July 2005, who's stayed more than 24 hours were included. Initially, the data of QSL before ICU admission, patient's demographics/clinics/therapeutics features were recorded. Afterwards, by telephone, 7, 90 and 180 days after hospital discharge, the patients answered the questionnaires about QSL. In the sequence, all patients were subdivided into 2 main groups: unchanged or better, and worse QSL. Data were analyzed using t Student and Chi-square tests (p-value < 0.05). RESULTS: Sixty eight patients were enrolled into the study. Completed questionnaires were obtained from 21 of them. A comparison of 7, 90 and 180 days after hospital discharge showed that QSL of patients was unchanged or better at 90 and 180 days. The majority of patients expressed more satisfaction in that moment. Unchanged or better QSL was associated with advanced age. However, there were no statistical significant differences in sex, schooling, APACHE II score, length of stay, mechanical ventilation and used drugs. Sixty percent returned to their previous work. CONCLUSIONS: There was a tendency for patients who felt themselves satisfied after hospital discharge to have their QSL improved as time went bye. Better QSL was associated with advanced age. Even when patients reported worse QSL they returned to their previous work.
