Abstract The Brazilian Practice Guidelines for Stroke Rehabilitation – Part II, developed by the Scientific Department of Neurological Rehabilitation of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, in Portuguese), focuses on specific rehabilitation techniques to aid recovery from impairment and disability after stroke. As in Part I, Part II is also based on recently available evidence from randomized controlled trials, systematic reviews, meta-analyses, and other guidelines. Part II covers disorders of communication, dysphagia, postural control and balance, ataxias, spasticity, upper limb rehabilitation, gait, cognition, unilateral spatial neglect, sensory impairments, home rehabilitation, medication adherence, palliative care, cerebrovascular events related to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the future of stroke rehabilitation, and stroke websites to support patients and caregivers. Our goal is to provide health professionals with more recent knowledge and recommendations for better rehabilitation care after stroke.
Resumo As Diretrizes Brasileiras de Reabilitação do Acidente Vascular Cerebral (AVC) - Parte II, desenvolvida pelo Departamento Científico de Reabilitação Neurológica da Academia Brasileira de Neurologia é voltada para intervenções específicas de técnicas de reabilitação de déficits neurológicos e incapacidades. Seguindo o mesmo modelo da Parte I, a Parte II também se baseia em estudos randomizados, revisões sistemáticas, metanálises e outras diretrizes sobre o mesmo tema. A segunda parte aborda os distúrbios da comunicação, disfagia, controle postural e equilíbrio, ataxias, espasticidade, reabilitação do membro superior, marcha, cognição, negligência espacial unilateral, déficits sensoriais, reabilitação domiciliar, aderênciaao usode medicamentos, cuidados paliativos,ofuturodareabilitação no AVC, e websites de orientação sobre AVC para pacientes e cuidadores. Nosso objetivo é fornecer aos profissionais envolvidos na reabilitação conhecimento atualizado e recomendações para um melhor cuidado no pós-AVC.

Resumen Realizamos cambios en la formulación original del enriquecimiento del medio Ellinghausen-McCullough-Johnson-Harris y evaluamos algunos aspectos, como el tiempo de crecimiento de Leptospira y la utilización en la prueba de aglutinación microscópica (MAT), comparándolos con el enriquecimiento original y un medio de enriquecimiento comercialmente disponible (DIFCOÔ). Se usaron muestras de Leptospira (24 antígenos) que componen nuestro panel de antígenos utilizados en la MAT, entre ellos, cepas autóctonas y de referencia aisladas en Brasil. Las muestras se cultivaron individualmente en medio Ellinghausen-McCullough-Johnson-Harris en las tres condiciones mencionadas (enriquecimiento adaptado, enriquecimiento original y enriquecimiento comercial). Adicionalmente, 89 sueros de sangre de animales domésticos y salvajes fueron analizados por MAT usando los antígenos cultivados en estos medios. Todas las muestras analizadas crecieron eficientemente con el enriquecimiento adaptado y los resultados de la MAT fueron satisfactorios. Por lo tanto, otros laboratorios también podrían beneficiarse del uso de este enriquecimiento adaptado al cultivar las cepas de Leptospira aplicadas en sus paneles MAT.
Abstract Changes were made to the original formulation of the EMJH medium (Ellinghausen-McCullough-Johnson-Harris) enrichment and some aspects such as growth time of Leptospira and utilization in the microscopic agglutination test (MAT) were evaluated and compared to the original enrichment and to a commercially available enrichment (DIFCOÔ). Leptospira samples (24 antigens) that make up our panel of antigens used in MAT were used, among them, reference and autochthonous strains isolated in Brazil. The samples were grown individually in the EMJH medium under the three previously mentioned conditions (adapted enrichment, original enrichment and commercial enrichment). In addition, 89 blood serums from domestic and wild animals were analyzed by MAT using the antigens grown in these media. All samples tested grew efficiently with the adapted enrichment, and the MAT results were satisfactory. Therefore, other laboratories could also benefit from the use of this adapted enrichment when culturing the Leptospira strains applied in their MAT panels.

Passion fruit waste in diets for quail in the laying phase

ABSTRACT. It was aimed to evaluate the effect of including the waste from passion fruit pulp extraction in diets for Japanese quail in the laying phase on their production performance and egg quality. A total of 280 female quail were distributed into five treatments with eight replicates and seven birds per cage. Treatments consisted of 0, 2, 4, 6, or 8% inclusion of passion fruit waste in the diet. The experiment began with quail at 70 days of age and lasted 63 days, with performance evaluated every 21 days and egg quality in the last two days of each cycle. There was no difference between the treatments for feed intake, body weight, specific gravity, or eggshell thickness. However, a decreasing linear regression was obtained for feed intake with the inclusion of the waste, while a quadratic response was show by egg production, weight and parts of the egg. Inclusion of up to 6% passion fruit waste in diets for quail is recommended to maintain the indices in relation to control treatment. However, the best values for egg-laying rate and for the weights of egg, albumen, yolk, and shell are obtained with 2.5 to 3.5% inclusion of passion fruit waste.

Success of promotion strategies for a stroke rehabilitation protocol

RESUMO OBJETIVO Avaliar prospectivamente o sucesso de estratégias de divulgação de um protocolo de reabilitação motora para indivíduos com doenças cerebrovasculares no Hospital Israelita Albert Einstein (HIAE). MÉTODOS Um ensaio clínico de neuromodulação e reabilitação para indivíduos com acidente vascular cerebral (AVC) e paresia do membro superior utilizou meios de divulgação digitais e meios tradicionais, não digitais. Foram calculadas frequências das modalidades de divulgação que alcançaram indivíduos potencialmente elegíveis para o protocolo. RESULTADOS Foram analisados dados de 80 indivíduos que manifestaram interesse em participar da pesquisa. As formas de divulgação mais frequentes que motivaram o contato foram redes sociais (38,8%) e informações fornecidas a médicos externos ao HIAE (23,8%). As frequências de contatos motivados por publicações na internet (53%) foram semelhantes às de contatos motivados por divulgações convencionais (47%) (p=0,288). Em relação às publicações sobre a pesquisa na internet, a divulgação pelo Facebook foi a mais eficiente, seguida pelo site do HIAE. CONCLUSÃO A divulgação de um protocolo de pesquisa em reabilitação por meio de publicações em redes sociais e sites pode ser tão eficaz quanto estratégias convencionais de comunicação. Estes resultados podem ter impacto no planejamento de ensaios clínicos, incluindo os que têm por objetivo avaliar efeitos de intervenções de reabilitação em indivíduos com AVC.
SUMMARY OBJECTIVE To prospectively evaluate the success of promotion strategies for a protocol of motor rehabilitation strategies for patients with stroke at Albert Einstein Hospital. METHODS In a clinical trial of neuromodulation and rehabilitation for patients with stroke, conventional methods of dissemination and publications about the research protocol in social networks or on the hospital’s website were performed. Frequencies of types of advertisements that reached potentially eligible subjects were calculated. RESULTS Data from 80 potentially eligible patients were analyzed. The types of ads that motivated contacts more frequently were social media (38.8%) and information provided to physicians from other hospitals (23.8%) (p=0,288). The frequencies of contacts motivated by publications on the internet (53%) and conventional strategies (47%) were similar. Facebook was the digital strategy associated with the higher number of contacts, followed by the hospital’s website. CONCLUSION Social networks and websites can be as effective as traditional methods of advertisement, in order to reach patients for stroke rehabilitation protocols. These results may have an impact on the planning of clinical trials, including studies that evaluate effects of rehabilitation interventions in patients with stroke.

Abstract: Considering that environmental risk factors substantially contribute to the etiology of orofacial clefts and that knowledge about the characteristics and comorbidities associated with oral clefts is fundamental to promoting better quality of life, this study aimed to describe the risk factors, main characteristics, and comorbidities of a group of patients with cleft lip and/or cleft palate (CL/P) from Rio Grande do Norte (RN), Brazil. Data were obtained from 173 patients with CL/P using a form from the Brazilian database on Orofacial Clefts. Most patients were male with cleft lip and palate and had a normal size and weight at birth; presented few neonatal intercurrent events; and had anemia and respiratory and cardiovascular diseases as main associated comorbidities. They also required timely surgical rehabilitation and multidisciplinary care to stimulate their neuropsychomotor development. In addition, a high frequency of familial recurrence and of parental consanguinity was evidenced in the studied population, especially for the cleft lip and cleft palate type. Other relevant findings were the considerable maternal exposure to alcohol, infections, smoking, and hypertension, as well as low supplementation with vitamins and minerals and deliberate consumption of analgesics, antibiotics, and antihypertensives during pregnancy. Characterization of the CL/P patient profile is essential for the planning of health services and integration among the health professionals involved in the diagnosis and treatment of these malformations. Our results reinforce the need for additional research to confirm the association between environmental factors and the development of orofacial clefts.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Alterações de funções orais na presença de aparelhos ortodônticos fixos com recursos intraorais

OBJETIVO: investigar as possíveis alterações de fala, mastigação e deglutição em usuários de aparelhos ortodônticos com recursos intraorais fixos no palato. MÉTODO: foram avaliados 28 pacientes de ambos os sexos, na faixa etária de 10 a 24 anos, em tratamento no Centro de Reabilitação Estética Orofacial em Campinas. Tais pacientes foram avaliados por fonoaudiólogas antes da colocação do aparelho e após trinta dias de uso de aparelho ortodôntico com recurso fixo. Todos os sujeitos foram avaliados previamente por ortodontistas e apresentaram indicação de colocação do recurso intraoral. O exame fonoaudiológico foi composto pela avaliação da mastigação, deglutição e fala, segundo protocolo especifico de motricidade orofacial, e de teste de fala na área de fonética. Os testes estatísticos aplicados foram: Teste de ANOVA, teste T-Student Pareado, Teste de Igualdade de duas proporções e Intervalo de Confiança para a média. Foram considerados significantes associações com p-valor < 0,05 . RESULTADOS: para a amostra analisada, não houve alterações de mastigação e deglutição estatisticamente significantes após um mês de uso dos aparelhos selecionados. Na fala, 64,3% da amostra não apresentou alterações, mas em 25% dos pacientes observou-se a presença de distorção de grupos consonantais na presença de aparelhos do tipo disjuntor. CONCLUSÃO: o uso de aparelhos fixos com recursos intraorais, provoca modificações estatisticamente significantes somente na fala.
PURPOSE: to investigate possible changes in speech, chewing and swallowing in users orthodontic appliances with features (intraoral) fixed on the palate. METHOD: a total of 28 patients of both sexes, in the 10-24 age-group, were evaluated. These patients were being treated at the CEREO - Centro de Reabilitação Estética Orofacial (Aesthetic and Orofacial Rehabilitation Center) in Campinas, São Paulo. They were evaluated by speech therapists before they had their orthodontic appliances installed and also 30 days after they were using these appliances with feature. All subjects were previously evaluated by orthodontists from the CEREO and were candidates to the installation of intraoral features. The speech therapist's test was composed of two parts: the first was the evaluation of chewing, swallowing and speech, according to the MBGR protocol and the second was a phonological evaluation of language. The statistical tests applied were: Two Proportions Equality Test, Chi-Square Test for Independence. We considered significant the associations with p-value <0.10. RESULTS: for the sample analyzed, there were no statistically significant changes in chewing and swallowing after one month of use of intraoral features. In speech, 64.3% of the sample did not change, but in 25% of the patients it was observed a distortion of consonant clusters in the presence of circuit breaker type appliances. CONCLUSION: the use of fixed orthodontic appliances with intraoral features causes statistically significant changes only in speech.

Após a detecção de morte de macacos Bugios (gênero Alouatta) e isolamento do vírus da Febre Amarela (YFV) no município de Buri, Estado de São Paulo, Brasil, foi iniciada uma investigação entomológica em campo. Uma cepa de YFV foi isolada em camundongos recém-nascidos e cultura de células de Aedes albopictus - C6/36, a partir de um lote de seis mosquitos Haemagogus (Conopostegus) leucocelaenus (Hg leucocelaenus) Dyar & Shannon coletados no local de estudo. RNA do vírus foi amplificado por RT-PCR e seqüenciado. A topologia gerada indica que a cepa isolada está relacionada ao genótipo South American I, em clado monofilético englobando isolados recentes de epidemias e epizootias entre 2008 e 2009. Análises estatísticas geralmente usadas caracterizaram a amostra em relação à diversidade e dominância, indicando dominância relativa de uma ou poucas espécies. Hg. leucocelaenus foi detectado infectado também no Rio Grande do Sul. No Estado de São Paulo trata-se da primeira detecção do YFV em Hg leucocelaenus.
After detecting the death of Howlers monkeys (genus Alouatta) and isolation of yellow fever virus (YFV) in Buri county, São Paulo, Brazil, an entomological research study in the field was started. A YFV strain was isolated from newborn Swiss mice and cultured cells of Aedes albopictus - C6/36, from a pool of six Haemagogus (Conopostegus) leucocelaenus (Hg. leucocelaenus) mosquitoes (Dyar & Shannon) collected at the study site. Virus RNA fragment was amplified by RT-PCR and sequenced. The MCC Tree generated showed that the isolated strain is related to the South American I genotype, in a monophyletic clade containing isolates from recent 2008-2010 epidemics and epizootics in Brazil. Statistical analysis commonly used were calculated to characterize the sample in relation to diversity and dominance and indicated a pattern of dominance of one or a few species. Hg. leucocelaenus was found infected in Rio Grande do Sul State as well. In São Paulo State, this is the first detection of YFV in Hg. leucocelaenus.

O transplante de células-tronco hematopoéticas (TCTH) é o tratamento de escolha para leucemias agudas de alto risco. Apesar da melhora na sobrevida destes pacientes, a recidiva continua sendo a maior causa de óbito pós-transplante de células-tronco hematopoéticas. O objetivo deste trabalho foi analisar os resultados dos transplantes realizados em crianças com leucemia aguda em duas instituições brasileiras. Realizou-se estudo retrospectivo de 208 pacientes transplantados entre 1990-2007. Mediana de idade: 9 anos; 119 pacientes com leucemia linfoide aguda (LLA) e 89 com leucemia mieloide aguda (LMA). Doença precoce: CR1 e CR2. Doença avançada: >CR3, doença refratária ou recidivada. Noventa pacientes vivos entre 258-6.068 dias (M:1.438), com sobrevida global (SG) de 45% (3 anos) e a sobrevida livre de recaída (SLR) 39% (três anos). 14/195 pacientes tiveram falha primária de pega (8%). Não houve diferença na sobrevida global e sobrevida livre de recaída entre pacientes com leucemia linfoide aguda e leucemia mieloide aguda, entre transplantes aparentados e não aparentados, tampouco entre as fontes de células utilizadas. O desenvolvimento da doença do enxerto contra hospedeiro (DECH) aguda ou crônica também não influenciou a sobrevida global e sobrevida livre de recaída. Pacientes com leucemia linfoide aguda condicionados com irradiação corporal total (TBI) apresentaram melhor sobrevida global e sobrevida livre de recaída (p<0,001). Cento e dezoito pacientes morreram entre 1-1.654 dias pós-transplante de células-tronco hematopoéticas (M:160). Mortalidade relacionada a transplante (MRT) (dia+100): 16%. Incidência cumulativa de recaída: 40% (3 anos). Pacientes com doença avançada tiveram menor sobrevida global e sobrevida livre de recaída (três anos)(p<0,001). Na análise multivariada, o status da doença foi o principal fator associado ao aumento da sobrevida global e sobrevida livre de recaída. Nossos resultados mostram que é possível se atingir uma boa sobrevida para pacientes com doença precoce e também mostram a baixa eficácia naqueles com doença avançada.
Hematopoietic Stem Cell transplantation (HSCT) is the treatment of choice for patients with high-risk leukemia. In spite of this, relapse remains a major cause of death of these patients. Our objective was to analyze the outcomes of patients with acute leukemia submitted to hematopoietic stem cell transplantation in two Brazilian institutions. A retrospective study of 208 patients transplanted between 1990 and 2007 with a median age of 9 years (range: 1-18 years) was made. One hundred and nineteen patients had acute lymphocytic leukemia (ALL) and 89 had acute myeloid leukemia (AML). Early disease was considered for CR1 and CR2 cases and advanced disease >CR3 and refractory and relapse disease. Ninety patients are alive between 258 and 6068 days after hematopoietic stem cell transplantation (M: 1438 days). The overall survival (OS) was 45% (3 years) and event free survival (EFS) was 39% (3 years). Primary graft failure occurred in 14/195 patients (8%). There were no differences in the overall survival and event free survival between patients with acute lymphocytic leukemia and acute myeloid leukemia, between sources of cells used or between those who developed acute or chronic graft-versus-host disease (GVHD). When comparing transplants from related and unrelated donors, there was no difference in the overall survival. Patients with acute lymphocytic leukemia receiving the total body irradiation (TBI) conditioning regimen had better overall survival and event free survival (p<0.001). One hundred and eighteen patients died between 0 and 1654 days after hematopoietic stem cell transplantation (M: 160 days). Transplantation-related-mortality (TRM) at D+100 was 16% and cumulative incidence of relapse was 40% (3 years). Patients with advanced disease had lower 3-year overall survival and event free survival (p<0.001). Multivariate analysis showed that disease status was the most significant factor associated with higher event free survival and overall survival . Our results show that children and adolescents transplanted with early disease can achieve considerable overall survival and also highlights the inefficacy of hematopoietic stem cell transplantation for patients with advanced disease.

Foram entrevistados 150 hipertensos (10,33% da população hipertensa) e 30 diabéticos (15% da população diabética), havendo uma predominância de indivíduos idosos (idade média=63,59 + 13,12 anos), do sexo feminino (74,4%), de baixa escolaridade (40,9% de analfabetismo) e de baixa renda (mediana de 0,5 salário mínimo). O tratamento destas morbidades foi basicamente medicamentoso (96,6%); houve uma elevada prevalência do sedentarismo (67,4%). Além disso, houve um grande consumo per capita diário de açúcar (153g + 110,66g), sal (18g + 21,26g) e óleo (60g + 43,23g). Os resultados obtidos apontam para a importância de intervenções multiprofissionais, por meio do Programa de Saúde da Família, com o objetivo de promover a adoção de hábitos e estilos de vida saudáveis, prevenir complicações dessas doenças e proporcionar melhoria da qualidade de vida.

A total of 150 hypertensive (10% of the hypertensive population) and 30 diabetics (12.98% of the diabetic population) people were interviewed. There were a predominance of elder people (average age = 63.59 + 13.12 years old), mostly women (74.4%), with low literacy (40.9% of illiterates) and low income (average = 0.5 minimum wage). The treatment was basically the use of medication (96.6%), and there was a high prevalence of sedentary people (67.4%). Besides that, there was a daily high intake per capita of sugar (153g + 110.66g), salt (18 g + 21.26g) and oil (60 g + 43.23g) . The results of this study showed the importance of a multi-professional intervention by means of the Family Health Program with the objective of promoting the adoption of healthy habits and lifestyle in order to prevent complications of these morbidities, and to provide a better life quality to these people.
