Abstract Data on clinical management options for sleep bruxism in the primary dentition are inconclusive. This umbrella review aimed to synthesize the available evidence from systematic reviews (SRs) on the associated factors and treatment approaches for clinical management of sleep bruxism in children. A search was conducted in the MEDLINE/PubMed, Web of Science, Embase, and OpenGrey databases up to March 2022. SRs published on sleep bruxism in children containing data on associated factors or treatment outcomes were included. The AMSTAR-2 tool was used to assess the methodological quality of SRs. The search identified 444 articles, of which six were included. Sleep conditions, respiratory changes, personality traits, and psychosocial factors were the associated factors commonly identified. Treatments included psychological and pharmacological therapies, occlusal devices, physical therapy, and surgical therapy. All SRs included presented a high risk of bias. Overlapping of the included studies was considered very high. The best evidence available to date for the management of sleep bruxism in children is based on associated factors, with sleep duration and conditions, respiratory changes, as well as personality traits and psychosocial factors being the most important factors commonly reported by studies. However, there is currently insufficient evidence to make recommendations for specific treatment options.

ABSTRACT Aedes (Stegomyia) albopictus (Skuse, 1854) was reported in Brazil for the first time in 1986 and has shown marked expansion throughout the Brazilian territory. During a routine activity to control dengue fever conducted by the Division of Entomology of the Municipal Health Department in Rio Branco city, adults and immatures of Culicidae were collected in a peri-urban area. The identified Culicidae forms indicated that they belonged to the species Ae. albopictus. This is the first official record of the presence of Ae. albopictus in the Acre State, confirming its current presence in all Brazilian states.

Abstract The aim of the present study was to analyze the effect of two different types of resistance training programming (linear and daily undulating) on submaximal strength and functional capacity in elderly women. Twenty-two participants (64 ± 3 years) were randomly assigned to 2 training groups: linear programming (LP; n=12) and daily undulating programming (DUP; n =10). Functional capacity and submaximal strength (10RM) were analyzed before and after 12 weeks of resistance training. The results demonstrated improvement on strength and functional capacity after the resistance training period (p ≤ 0.05), except for the Bench Press (p = 0.30), for both groups DUP and LP. The Effect Size was, respectively, high for DUP (timed up and go test = -2.07, and timed sit test= 4.69), and high for LP (horizontal leg press = 2.35). For all other results, the effect size was trivial or small. No statistical difference was observed between programming models. The LP and DUP trainnings have similar results in increasing submaximal muscle strength in elderly women inexperienced in RT after 12 weeks of intervention (p ≤ 0,05). However, DUP appears to be more effective in increasing functional capacity. In practice, the professional can use both the LP and the DUP to improve the level of fitness in the early stages of training in this population. However, when the goal of programming is to increase functional capacity, DUP can be prioritized.

Resumo O objetivo do presente estudo foi analisar o efeito de dois tipos diferentes de programação do treinamento de força (linear e ondulatório diário) na força submáxima e na capacidade funcional de mulheres idosas. Vinte e duas participantes (64 ± 3 anos) foram randomizados em 2 grupos experimentais: programação linear (PL; n=12) e programação ondulatória diária (POD; n=10). A capacidade funcional e a força submáxima (10RM) foram avaliadas antes e depois das 12 semanas de treinamento de força. Os resultados demonstraram melhora da força submáxima e da capacidade funcional após o período de treinamento (p ≤ 0,05), exceto para o exercício supino (p = 0,30), para ambos os grupos POD e PL. Foi encontrado um tamanho de efeito grande para a POD nos testes de sentar e caminhar = -2,07 e teste de sentar e levantar = 4,69, bem como na PL para o exercício leg press horizontal = 2,35. Não foi observada diferença estatística entre os modelos de programação. As programações LP e DUP têm resultados semelhantes no aumento da força muscular submáxima em mulheres idosas inexperientes em TR após 12 semanas de intervenção (p ≤ 0,05). No entanto, o DUP parece ser mais eficaz para aumentar a capacidade funcional. Na prática, o profissional pode usar tanto o LP quanto o DUP para melhorar o nível de condicionamento físico nos estágios iniciais do treinamento nessa população. Porém, quando o objetivo da programação é aumentar a capacidade funcional, o DUP pode ser priorizado.

Abstract The objective of this study was to compare the protein profile of the acquired enamel pellicle (AEP) formed in vivo in patients with or without gastroesophageal reflux disease (GERD), and with or without erosive tooth wear (ETW). Twenty-four volunteers were divided into 3 groups: 1) GERD and ETW; 2) GERD without ETW; and 3) control (without GERD). The AEP formed 120 min after prophylaxis was collected from the lingual/palatal surfaces. The samples were subjected to mass spectrometry (nLC-ESI-MS/MS) and label-free quantification by Protein Lynx Global Service software. A total of 213 proteins were identified, or 119, 92 and 106 from each group, respectively. Group 2 showed a high number of phosphorylated and calcium-binding proteins. Twenty-three proteins were found in all the groups, including 14-3-3 protein zeta/delta and 1-phosphatidylinositol. Several intracellular proteins that join saliva after the exfoliation of oral mucosa cells might have the potential to bind hydroxyapatite, or participate in forming supramolecular aggregates that bind to precursor proteins in the AEP. Proteins might play a central role in protecting the dental surface against acid dissolution.

RESUMO Objetivo: Adaptar e validar o conteúdo do Inventario para la Evaluación de Competencias en Enfermeras de Práctica Avanzada para a cultura brasileira. Métodos: Estudo metodológico que seguiu os estágios de tradução, síntese, retrotradução, avaliação por um comitê de cinco especialistas, pré-teste com 31 enfermeiros e avaliação pelo autor do instrumento original. Para avaliar o conteúdo, foram calculados o Índice de Validade de Conteúdo (mínimo 0,90) e o Kappa modificado (mínimo 0,74). Resultado: Na primeira rodada de avaliação do conteúdo, 18 itens foram alterados, pois não alcançaram os valores mínimos estabelecidos. Na segunda rodada, três itens não obtiveram consenso e foram encaminhados para o autor da versão original. No pré-teste, 13 itens retornaram aos especialistas, pois sofreram alteração de conteúdo. Conclusão: O Instrumento para Avaliação de Competências do Enfermeiro de Prática Avançada – versão brasileira foi adaptado transculturalmente e teve seu conteúdo validado.

ABSTRACT Objective: To adapt and validate the content of the Advanced Practice Nursing Competency Assessment Instrument (APNCAI) to Brazilian culture. Methods: This is a methodological study that followed the stages of translation, synthesis, back translation, evaluation by a committee of five specialists, pre-test with 31 nurses, and evaluation by the author of the original instrument. The Content Validity Index (minimum 0.90) and the modified Kappa (minimum 0.74) were calculated to evaluate the content. Result: In the first round of content evaluation, 18 items had to be altered because they did not reach the minimum values established. Three items have not reached a consensus in the second round and had to be sent to the author of the original version. In the pre-test, 13 items were returned to the specialists because they suffered content changes. Conclusion: The Advanced Practice Nursing Competency Assessment Instrument (APNCAI) - Brazilian version was cross-culturally adapted, and its content was validated.

ABSTRACT The objective of the present study was to evaluate the stability of candidate reference genes and select the genes that can be used for normalizing real-time polymerase chain reaction (PCR) in the liver, skeletal muscle, and jejunum tissues of Nellore or Nellore × Angus steers fed different diets. Fourteen purebred and 14 crossbred steers were used, in which half of the animals of each genetic group received a diet containing whole shelled corn (WSC) and the other half whole shelled corn and sugarcane bagasse (WSCB). Stability was analyzed by the RefFinder program. To validate the selection of candidate reference genes, the expression of target genes was evaluated using the different groups of reference genes. The most stable genes were 18S, ACTB, and CASC3 for skeletal muscle; HMBS, ACTB, and 18S for the liver; and GAPDH, ACTB, and CASC3 for the jejunum, regardless of breed and diet provided. Possible errors caused in data analyses were clarified comparing the more and less stable genes as reference for normalization of the target genes FASN, ACOX, SCD1, MGAM, and SLC2A1. The use of the more stable and less stable sets of reference genes may lead to different conclusions in respect to the expression profile of the target studied gene. The selection of more suitable reference genes for each experiment is of utmost importance to ensure the reliability of gene expression studies so that they can be applied in practice.

ABSTRACT Lignocellulosic residues are used to produce mushrooms, but they generate large amounts of spent mushroom substrate (SMS). The objective of this study was to evaluate they effect of SMS of Pleurotus ostreatus, combined with organic fertilization, on biomass production and essential oil yield of basil (Ocimum basilicum L.) plants. The fertilizer was formulated using combinations of organic compounds: SMS, organic compost (OC), and earthworm humus (EH). The treatments were applied using 35 g of the formulations, corresponding to an application of 20 Mg ha-1. The treatments used were: 100% OC; 100% EH; 100% SMS; 75% SMS + 25% OC (C1); 75% SMS + 25% EH (C2); 50% SMS + 50% OC (C3); 50% SMS + 50% EH (C4); 50% SMS + 25% OC + 25% EH (C5), and a control with no fertilizer application (CT). Plants grown with the soil fertilizers containing SMS, single or combined with OC and EH, presented higher average plant height (55.6 to 62.2 cm) and leaf area per plant (696.5 to 836.4 cm2). The treatment C3 resulted in plants with higher mean dry weight (10.9 g plant-1), and C2 resulted in the highest oil yield (5.0 kg ha-1), representing a gain of 324.8% in oil yield when compared to the control. Thus, SMS increases biomass production and essential oil yield of basil plants.

RESUMO Resíduos lignocelulósicos são usados na produção de cogumelos, mas geram grandes quantidades de substrato pós-cultivo de cogumelo (SPC). Neste estudo avaliou-se o SPC de Pleurotus ostreatus, aliado a adubação orgânica na produção de biomassa e rendimento de óleo essencial de manjericão (Ocimum basilicum L.). As formulações das adubações foram preparadas com o SPC, composto orgânico (CO) e húmus de minhoca (HM) e suas combinações, todos com aplicação de 35 g no solo, equivalente a 20 t ha-1. Os tratamentos foram: CO = 100 %; HM = 100 %; SPC = 100 %; C1 = 75 % SPC + 25 % CO; C2 = 75 % SPC + 25 % HM; C3 = 50 % SPC + 50 % CO; C4 =50 % SPC + 50 % HM; C5 = 50 % SPC + 25 % CO + 25 % HM e CT = sem adubação. O comprimento médio e a área foliar foram superiores nas plantas cultivadas no solo contendo o SPC, individualmente ou combinado com as fontes orgânicas, com 55,6 a 62,2 cm de comprimento e 696,5 a 836,4 cm2 de área foliar por planta. O tratamento C3 proporcionou plantas com maior acumulo de matéria seca (10,9 g planta-1) e o C2 proporcionou maior rendimento de óleo (5,0 kg ha-1), com ganho de 324,8 % no rendimento, em relação ao controle. O SPC promove um aumento significativo na biomassa e no rendimento de óleo essencial em plantas de manjericão (Ocimum basilicum).

Resumo Objetivos: avaliar o efeito de uma intervenção na incidência de traumas mamilares e na qualidade da técnica de amamentar no primeiro mês pós-parto. Métodos: trata-se de um estudo de intervenção quasi randomizado com 180 puérperas distribuídas entre os grupos experimental e controle. A intervenção foi realizada na mater-nidade e consistiu de sessão educativa sobre a técnica de amamentar. Foi realizada análise descritiva das características dos grupos, comparadas as frequências dos parâmetros desfa-voráveis da técnica de amamentar. O teste do qui-quadrado de Pearson e teste de Fisher foram utilizados, sendo adotado p≤ 0,05 como nível crítico de significância. Resultados: aos 30 dias, 64% e 15%, das mães apresentaram técnica correta, respectiva-mente, nos grupos experimental e controle com RR= 4,87 (IC95%=2,93-8,34); NNT= 1,96 (IC95%=1,61-2,72); p<0,001.No grupo experimental percebeu-se que houve diminuição dos parâmetros desfavoráveis da técnica de amamentar (p≤0,05). A incidência de trauma mamilar foi de 30% no grupo experimental e 38,9% no grupo controle (p=0,21). Conclusão: no grupo experimental a intervenção realizada não foi suficiente para prevenir a ocorrência de traumas mamilares, porém melhorou significantemente a qualidade da técnica de amamentar.

Abstract Objectives: to evaluate the effect of an intervention on the incidence of nipple trauma and the quality of breastfeeding technique in the first month of postpartum. Methods: this is a quasi-randomized intervention study with 180 puerperal women equally distributed between experimental and control groups. The intervention was performed at a maternity and consisted of an educational session on breastfeeding technique. A descrip-tive analysis of the groups’ characteristics was performed, comparing the frequencies of unfa-vorable parameters related to breastfeeding technique between groups. Pearson’s chi-square test and Fisher’s test were used, and p≤0.05 was adopted as the critical level of significance. Results: at 30 days, 64% and 15% of the mothers used the technique correctly, respec-tively, in the experimental and control groups with RR=4.87 (CI95%=2.93-8.34); NNT=1.96 (CI95% =1.61-2.72); p<0.001. In the experimental group, a decrease was observed in the unfavorable parameters of the breastfeeding technique (p≤0.05). The incidence of nipple trauma was 30% in the experimental group and 38.9% in the control group (p=0.21). Conclusions: the intervention was insufficient to prevent nipple trauma in the experi-mental group, but significantly improved the quality in the breastfeeding technique.

Resumo Objetivos: avaliar o efeito de uma intervenção direcionada à técnica de amamentação na prevalência de aleitamento materno exclusivo no primeiro mês de vida. Métodos: foi realizado um estudo de intervenção quasi randomizado em uma maternidade pública, credenciada como Amigo da Criança. A intervenção consistiu em orientações individuais sobre a técnica de amamentação nas primeiras 48 horas após o parto, com ajuda de seio cobaia, boneca e filme. O grupo controle foi orientado sobre a importância do aleitamento materno pelas pesquisadoras, além das orientações fornecidas pelo hospital. A coleta de dados ocorreu na maternidade e ao final do primeiro mês em serviço ambulatorial do hospital, mediante a aplicação de um questionário, observação da mamada e exame das mamas. Resultados: a prevalência do aleitamento materno exclusivo, no primeiro mês de vida do lactente, foi de 76,6% no grupo experimental e 52,2% no grupo controle. RR=1,46 (IC95%=1,16-1,84); NNT=4,09; p=0,001. As mulheres que sofreram a intervenção, apresentaram técnica correta de amamentação em proporção significativamente maior que os controles: 64,04% vs 15,11%, RR= 4,87 [IC95%= 2,93-8,34] NNT= 1,96; p<0,001. Conclusões: a utilização de recursos audiovisuais, uso de instrumentos (seio cobaia e boneca) e tipo de abordagem individualizada contribuíram para o aumento da prevalência do aleitamento materno exclusivo até 30 dias pós parto.

Abstract Objectives: to evaluate the effect of an intervention directed to the breastfeeding technique in the prevalence of exclusive breastfeeding in the first month of life. Methods: this is a quasi-randomized intervention study conducted in a public maternity ward accredited in the Baby-Friendly Hospital Initiative. The intervention consisted of individual orientations on the breastfeeding technique in the first 48 hours after delivery, with the help of a breast model, doll, and movie. The control group was oriented by researchers about the importance of breastfeeding, and received hospital orientations. Data were collected in the maternity ward, and at the end of the first month in the hospital’s outpatient service by implementing a questionnaire, observing the baby’s breastfeeding technique and examining breasts. Results: the prevalence of exclusive breastfeeding in the first month of life of the infant was 76.6% in the experimental group, and 52.2% in the control group. RR=1.46 (CI95%=1.16-1.84); NNT=4.09; p=0.001. The trained women presented a correct breast-feeding technique in 64.04%, much higher when compared to those who did not receive training (15.11%;RR=4.87[CI95%=2.93-8.34]; NNT=1.96;p<0.001). Conclusions: the use of audiovisual resources, use of instruments (model breast and doll), and type of individualized approach contributed to an increased prevalence of exclusive breastfeeding up to 30 days after delivery.

ABSTRACT Objective to describe cultural practices of health care for quilombola women in puerperium. Method this qualitative, descriptive study was framed by Madeleine Leininger’s Theory of Culture Care Diversity and Universality. Fourteen quilombola women who had already experienced the puerperal period participated in the study. Information was collected from June to September 2018. The project was approved by the research ethics committee. Results beliefs and values were observed to be maintained in the local culture as regards practices of care for puerperal quilombola women. Final considerations the postpartum care practices applied by quilombola women were characteristically influenced by the beliefs, values and ways of life transmitted orally by other women who had already experienced puerperium.

RESUMEN Objetivo describir las prácticas culturales de atención a la salud de las mujeres quilombolas en el puerperio. Método este estudio cualitativo y descriptivo fue enmarcado por la Teoría de la Cultura Cuidado de la Diversidad y Universalidad de Madeleine Leininger. Participaron del estudio catorce mujeres quilombolas que ya habían pasado por el puerperio. La información se recopiló de junio a septiembre de 2018. El proyecto fue aprobado por el comité de ética en investigación. Resultados se observó el mantenimiento de creencias y valores en la cultura local en cuanto a prácticas de cuidado de las puérperas quilombolas. Consideraciones finales las prácticas de atención posparto aplicadas por las mujeres quilombolas estuvieron característicamente influenciadas por las creencias, valores y formas de vida transmitidas oralmente por otras mujeres que ya habían experimentado el puerperio.

RESUMO Objetivo descrever as práticas culturais de cuidado no puerpério de mulheres quilombolas. Método estudo descritivo, de abordagem qualitativa e delineado por meio da Teoria da Diversidade e Universalidade do Cuidado de Madeleine Leininger. Participaram do estudo quatorze mulheres quilombolas que já tinham vivenciado o período puerperal. A coleta das informações ocorreu de junho a setembro de 2018. Projeto aprovado por Comitê de Ética em Pesquisa. Resultados observou-se a manutenção das crenças e valores, considerando a cultura local relacionada às práticas de cuidado de puérperas quilombolas. Considerações finais as práticas de cuidado desenvolvidas por mulheres quilombolas no pós-parto se caracterizam por receber influência das crenças, valores e modos de vida que foram transmitidas por meio da oralidade por outras mulheres que já tinham vivenciado esse período.

RESUMO A ferrugem asiática da soja (FAS), causada pelo fungo Phakopsora pachyrhizi, é uma das doenças mais severas na cultura. Dentre as estratégias para seu manejo propõe-se a utilização de genótipos com resistência horizontal ou parcial, ainda restritos no mercado, mas promissores pela durabilidade da resistência. O objetivo deste trabalho foi avaliar a resistência de diferentes genótipos de soja à ferrugem asiática para futura utilização em programas de melhoramento. Na safra 2015/2016, foram avaliados 11 genótipos de soja provenientes do LAGER-UFU e a cultivar Desafio 8473 (padrão de suscetibilidade) frente à infecção artificial e natural de P. pachyrhizi no campo. Com os dados de severidade da FAS, calculou-se a área abaixo da curva de progresso da doença (AACPD). O percentual de área foliar afetada e os valores da AACPD dos genótipos avaliados oscilaram de 1,56 a 80,75 e de 380,62 a 975,62, respectivamente. Os genótipos L-210, L-166, L-268, L-203, L-279, L-224, L-216 e L-218 apresentam menores valores de AACPD, indicando resistência parcial, enquanto os genótipos L-144, L-104, L-266 e a cv. Desafio 8473 apresentaram maiores valores de AACPD, portanto, com menor resistência parcial ou suscetíveis. Ainda assim, os genótipos L-144, L-104 e L-266 apresentaram coeficientes de progresso da severidade da doença de 1,8 a 2,0 vezes menores comparativamente à cv. Desafio 8473. Conclui-se que os genótipos do Programa de Melhoramento Genético da UFU (LAGER-UFU), apresentam potencial de utilização no desenvolvimento de cultivares de soja resistentes à P. pachyrhizi.

ABSTRACT Asian soybean rust (ASR), caused by the fungus Phakopsora pachyrhizi, is one of the most severe diseases that affect the crop. Among management strategies is the use of genotypes with horizontal or partial resistance, which are still restricted in the market but promising due to their resistance durability. The objective of this study was to evaluate the resistance of different genotypes to Asian soybean rust for future use in breeding programs. In the 2015/2016 crop season, 11 soybean genotypes from LAGER-UFU and cv. Desafio8473 (susceptibility pattern) were evaluated for artificial and natural infection by P. pachyrhizi in the field. ASR severity data were used to calculate the area under the disease progress curve (AUDPC). The percentage of affected leaf area and the AUDPC values of the evaluated genotypes ranged from 1.56 to 80.75% and from 380.62 to 975.62, respectively. The genotypes L-210, L-166, L-268, L-203, L-279, L-224, L-216 and L-218 have lower AUDPC values, indicating partial resistance, while the genotypes L-144, L-104, L-266 and cv. Desafio 8473 showed higher AUDPC values and therefore lower partial resistance or susceptibility. Even so, the genotypes L-144, L-104 and L-266 showed coefficients of disease severity progress 1.8 to 2.0-fold lower than those of cv. Desafio 8473. In conclusion, the genotypes from UFU Breeding Program (LAGER-UFU) showed potential to be used in the development of soybean cultivars resistant to P. pachyrhizi.

Resumo Com base na crítica de Spivak a Foucault, discute-se a questão do sujeito na obra de Foucault, a fim de mostrar sua conexão com a perspectiva estruturalista que visava a superar o primado do sujeito autoconsciente nas Ciências humanas recorrendo a estruturas inconscientes como fundamento da explicação. Sustenta-se que essa perspectiva “antiantropocêntrica” permanece presente na obra do autor no período de Vigiar e punir e A vontade de saber. Por fim, propõe-se que as alterações na abordagem de Foucault no início dos anos de 1980 decorriam de uma tentativa de escapar de impasses que a recusa à perspectiva do sujeito soberano e autoconsciente teria acarretado.

Abstract Based on a critique of Spivak to Foucault, the article discusses the question of the subject in Foucault’s work showing its connection to the structuralist perspective aimed to overcome the primacy of the individual self in the human sciences using unconscious structures as the foundation of explanation. It is argued that this perspective not “anthropocentric” remains present in the work of the author in the Discipline and punish period and The history of sexuality, vol. 1: An introduction and, finally, it is proposed that the changes in Foucault’s approach in the early 1980s resulted from an attempt to escape impasses that refusing the prospect of sovereign and self-conscious subject would caused.

BACKGROUND Knowledge on synanthropic phlebotomines and their natural infection by Leishmania is necessary for the identification of potential areas for leishmaniasis occurrence. OBJECTIVE To analyse the occurrence of Phlebotominae in gallery forests and household units (HUs) in the city of Palmas and to determine the rate of natural infection by trypanosomatids. METHODS Gallery forests and adjacent household areas were sampled on July (dry season) and November (rainy season) in 2014. The total sampling effort was 960 HP light traps and eight Shannon traps. Trypanosomatids were detected in Phlebotominae females through the amplification of the SSU rDNA region, and the positive samples were used in ITS1-PCR. Trypanosomatid species were identified using sequencing. FINDINGS A total of 1,527 sand flies representing 30 species were captured in which 949 (28 spp.) and 578 (22 spp.) were registered in July and November, respectively. In July, more specimens were captured in the gallery forests than in the HUs, and Nyssomyia whitmani was particularly frequent. In November, most of the specimens were found in the HUs, and again, Ny. whitmani was the predominant species. Lutzomyia longipalpis was commonly found in domestic areas, while Bichromomyia flaviscutellata was most frequent in gallery forests. Molecular analysis of 154 pools of females (752 specimens) identified Leishmania amazonensis, L. infantum, and Crithidia fasciculata in Ny. whitmani, as well as L. amazonensis in Lu. longipalpis, Trypanosoma sp. and L. amazonensis in Pintomyia christenseni, and L. amazonensis in both Psathyromyia hermanlenti and Evandromyia walkeri. MAIN CONCLUSIONS These results show the importance of gallery forests in maintaining Phlebotominae populations in the dry month, as well as their frequent occurrence in household units in the rainy month. This is the first study to identify Leishmania, Trypanosoma, and Crithidia species in Phlebotominae collected in Palmas, Tocantins, Brazil.

RESUMO OBJETIVO Determinar a prevalência de síndrome metabólica e de seus componentes em adolescentes brasileiros. MÉTODOS Foram avaliados 37.504 adolescentes, participantes do Estudo de Riscos Cardiovasculares em Adolescentes (ERICA), estudo transversal de âmbito nacional e de base escolar. Os adolescentes, de 12 a 17 anos de idade, residiam em municípios com mais de 100 mil habitantes. A amostra foi estratificada e conglomerada em escolas e turmas. Os critérios da International Diabetes Federation foram utilizados para definir síndrome metabólica. Prevalências de síndrome metabólica foram estimadas segundo sexo, faixa etária, tipo de escola e estado nutricional. RESULTADOS Dos 37.504 adolescentes, 50,2% eram do sexo feminino; 54,3% tinham de 15 a 17 anos e 73,3% estudavam em escolas públicas. A prevalência nacional de síndrome metabólica foi 2,6% (IC95% 2,3-2,9), discretamente maior no sexo masculino e naqueles de 15 a 17 anos na maioria das macrorregiões. A prevalência foi a maior nos residentes na macrorregião Sul, nas adolescentes mais jovens e nos adolescentes mais velhos. A prevalência foi maior nas escolas públicas (2,8% [IC95% 2,4-3,2]) que nas escolas privadas (1,9% [IC95% 1,4-2,4]) e nos adolescentes obesos em comparação aos não obesos. As combinações de componentes mais frequentes, respondendo por 3/4 das combinações, foram: circunferência de cintura elevada, HDL-colesterol baixo e pressão arterial elevada, seguida de circunferência da cintura elevada, lipoproteína de alta densidade (HDL-c) baixo e triglicerídeos elevados, e wpor circunferência da cintura elevada, HDL baixa e triacilgliceróis e pressão arterial elevados. A HDL baixa foi o segundo componente mais frequente, mas a maior prevalência de síndrome metabólica (26,8%) foi observada na presença de triglicerídeos elevado. CONCLUSÕES O ERICA é o primeiro estudo nacional a apresentar prevalências de síndrome metabólica e descrever a participação dos seus componentes. Apesar de a prevalência da síndrome metabólica ter sido baixa, as altas prevalências de alguns componentes e de participação de outros na composição da síndrome torna importante o diagnóstico precoce de tais alterações, mesmo que não agrupadas na síndrome metabólica.

ABSTRACT OBJECTIVE To determine the prevalence of metabolic syndrome and its components in Brazilian adolescents. METHODS We evaluated 37,504 adolescents who were participants in the Study of Cardiovascular Risks in Adolescents (ERICA), a cross-sectional, school-based, national study. The adolescents, aged from 12 to 17 years, lived in cities with populations greater than 100,000 inhabitants. The sample was stratified and clustered into schools and classes. The criteria set out by the International Diabetes Federation were used to define metabolic syndrome. Prevalences of metabolic syndrome were estimated according to sex, age group, school type and nutritional status. RESULTS Of the 37,504 adolescents who were evaluated: 50.2% were female; 54.3% were aged from 15 to 17 years, and 73.3% were from public schools. The prevalence of metabolic syndrome was 2.6% (95%CI 2.3-2.9), slightly higher in males and in those aged from 15 to 17 years in most macro-regions. The prevalence was the highest in residents from the South macro-region, in the younger female adolescents and in the older male adolescents. The prevalence was higher in public schools (2.8% [95%CI 2.4-3.2]), when compared with private schools (1.9% [95%CI 1.4-2.4]) and higher in obese adolescents when compared with nonobese ones. The most common combinations of components, referring to 3/4 of combinations, were: enlarged waist circumference (WC), low HDL-cholesterol (HDL-c) and high blood pressure; followed by enlarged WC, low HDL-c and high triglycerides; and enlarged WC, low HDL-c, high triglycerides and blood pressure. Low HDL was the second most frequent component, but the highest prevalence of metabolic syndrome (26.8%) was observed in the presence of high triglycerides. CONCLUSIONS ERICA is the first Brazilian nation-wide study to present the prevalence of metabolic syndrome and describe the role of its components. Despite the prevalence of Metabolic Syndrome being low, the high prevalences of some components and participation of others in the syndrome composition shows the importance of early diagnosis of this changes, even if not grouped within the metabolic syndrome.

RESUMO OBJETIVO Estimar as prevalências de hipertensão arterial e obesidade e a fração atribuível populacional de hipertensão arterial devida à obesidade em adolescentes brasileiros. MÉTODOS Foram avaliados dados dos participantes do Estudo de Riscos Cardiovasculares em Adolescentes, estudo seccional l nacional de base escolar. A amostra foi dividida em 32 estratos geográficos e conglomerados de escolas e turmas, com representatividade nacional, macrorregional e de capitais. Obesidade foi classificada pelo índice de massa corporal segundo idade e sexo. Considerou-se hipertensão arterial a média da pressão arterial sistólica ou diastólica maior ou igual ao percentil 95 da curva de referência. Foram estimadas prevalências e intervalos de confiança de 95% (IC95%) de hipertensão arterial e de obesidade, nacionais e nas macrorregiões do País, por sexo e grupo etário, assim como as frações de hipertensão atribuíveis à obesidade na população. RESULTADOS Foram avaliados 73.399 estudantes, 55,4% do sexo feminino, com média de idade 14,7 anos (DP = 1,6). A prevalência de hipertensão arterial foi 9,6% (IC95% 9,0-10,3); sendo as mais baixas observadas nas regiões Norte, 8,4% (IC95% 7,7-9,2) e Nordeste, 8,4% (IC95% 7,6-9,2) e a mais alta na região Sul, 12,5% (IC95% 11,0-14,2). A prevalência de obesidade foi 8,4% (IC95% 7,9-8,9), mais baixa na região Norte e mais alta na Sul. As prevalências de hipertensão arterial e obesidade foram maiores no sexo masculino. Adolescentes com obesidade tiveram prevalência de hipertensão arterial mais elevada, 28,4% (IC95% 25,5-31,2), do que aqueles com sobrepeso, 15,4% (IC95% 13,8-17,0), ou eutróficos, 6,3% (IC95% 5,6-7,0). A fração de hipertensão arterial atribuível à obesidade foi de 17,8%. CONCLUSÕES O ERICA foi o primeiro estudo brasileiro com representatividade nacional a estimar a prevalência de hipertensão arterial aferida em adolescentes. A fração da prevalência de hipertensão arterial atribuível à obesidade mostrou que cerca de 1/5 dos hipertensos poderiam não ser hipertensos se não fossem obesos.

ABSTRACT OBJECTIVE To estimate the prevalence of arterial hypertension and obesity and the population attributable fraction of hypertension that is due to obesity in Brazilian adolescents. METHODS Data from participants in the Brazilian Study of Cardiovascular Risks in Adolescents (ERICA), which was the first national school-based, cross-section study performed in Brazil were evaluated. The sample was divided into 32 geographical strata and clusters from 32 schools and classes, with regional and national representation. Obesity was classified using the body mass index according to age and sex. Arterial hypertension was defined when the average systolic or diastolic blood pressure was greater than or equal to the 95th percentile of the reference curve. Prevalences and 95% confidence intervals (95%CI) of arterial hypertension and obesity, both on a national basis and in the macro-regions of Brazil, were estimated by sex and age group, as were the fractions of hypertension attributable to obesity in the population. RESULTS We evaluated 73,399 students, 55.4% female, with an average age of 14.7 years (SD = 1.6). The prevalence of hypertension was 9.6% (95%CI 9.0-10.3); with the lowest being in the North, 8.4% (95%CI 7.7-9.2) and Northeast regions, 8.4% (95%CI 7.6-9.2), and the highest being in the South, 12.5% (95%CI 11.0-14.2). The prevalence of obesity was 8.4% (95%CI 7.9-8.9), which was lower in the North region and higher in the South region. The prevalences of arterial hypertension and obesity were higher in males. Obese adolescents presented a higher prevalence of hypertension, 28.4% (95%CI 25.5-31.2), than overweight adolescents, 15.4% (95%CI 17.0-13.8), or eutrophic adolescents, 6.3% (95%CI 5.6-7.0). The fraction of hypertension attributable to obesity was 17.8%. CONCLUSIONS ERICA was the first nationally representative Brazilian study providing prevalence estimates of hypertension in adolescents. Regional and sex differences were observed. The study indicates that the control of obesity would lower the prevalence of hypertension among Brazilian adolescents by 1/5.
