ABSTRACT Agriculture, urbanization, and industrialization are some of the anthropogenic activities that constantly generate negative impacts on natural environments. Part of this degradation directly affects aquatic systems. This study aimed to evaluate the visual characteristics of the Araras Stream, located in the municipality of Araras, São Paulo state, Brazil. Data was collected at six different assessment sites along the river in both rural and urban areas. The evaluation used a Rapid Habitat Diversity Assessment (RHDA) protocol composed of 22 parameters, which define levels of preservation of ecological conditions. According to this protocol, the 32 Km-long study transect along the Araras Stream was rated as impacted (39.6 points). A separate assessment by transects showed that only Transect 1 presented a natural level of preservation (71.8 points). Transects located in the urban area contributed the most to impacts on the aquatic environment. The RHDA protocol proved to be an important tool to evaluate and monitor aquatic environments.

OBJETIVO: Avaliar as características dos usuários de álcool e drogas que procuram tratamento no Sistema Único de Saúde.MÉTODO:Estudo transversal multicêntrico envolvendo cinco centros clínicos/de pesquisa localizados em quatro capitais brasileiras foi realizado com 740 pacientes hospitalizados e ambulatoriais. O único critério de exclusão foi a presença de sintomas neurológicos ou psiquiátricos graves no momento da entrevista. A Escala de Gravidade de Dependência (Addiction Severity Index, ASI-6) e o teste ASSIST (Alcohol, Smoking and Substance Involvement Screening Test) foram utilizados para avaliar a severidade do uso de substância e problemas relacionados.RESULTADOS:Houve significativamente mais homens do que mulheres na amostra; a idade média foi de 36 anos. A droga mais usada em todos os centros foi o álcool (78%), seguido de cocaína/crack (51%). O álcool foi a droga que mais motivou procura por tratamento, em todos os centros. Os Escores Resumidos da ASI-6 para Funcionamento Recente (SS-Rs) foram bastante similares nos centros. SS-Rs foram comparados entre usuários que nunca haviam recebido tratamento para abuso de substâncias (n = 265, 36,1%) e aqueles que já haviam sido tratados uma ou mais vezes (n = 470, 63,9%). Foram observadas diferenças significativas entre os grupos nas áreas de droga, sintomas psiquiátricos e problemas legais e familiares/sociais (p < 0,05).CONCLUSÕES: Nossos dados confirmam evidências prévias de que o manejo de pacientes que procuram tratamento para abuso de substância deve levar em conta vários aspectos, como educação, emprego e relações familiares, que normalmente geram preocupação nesses indivíduos.

OBJECTIVE: To assess the characteristics of alcohol and drug users who seek treatment at the Brazilian Unified Health System in Brazil.METHOD:A multicenter cross-sectional study involving five clinical and research centers located in four Brazilian state capitals was conducted with 740 in- and outpatients. The only exclusion criterion was the presence of neurological or severe psychiatric symptoms at the moment of the interview. The Addiction Severity Index (ASI-6) and the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) were used to assess the severity of substance use and the problems related.RESULTS: There were significantly more men than women in the sample; mean age was 36 years. The drug most frequently used at all sites was alcohol (78%), followed by cocaine/crack (51%). Alcohol was the drug that most commonly motivated treatment seeking, at all centers. ASI-6 Summary Scores for Recent Functioning (SS-Rs) were quite similar among centers. SS-Rs were compared between users who had never received treatment for psychoactive substance abuse (n = 265, 36.1%) and those who had already been treated at one or more occasions (n = 470, 63.9%). This analysis revealed significant differences between the groups in the drug, psychiatric symptoms, legal, and family/social problems areas (p < 0.05).CONCLUSION: Our findings confirm previous evidence suggesting that the management of patients seeking drug abuse treatment should take several different aspects into consideration, e.g., education, employment, and family relationships, which often appear as areas of concern for these individuals.

INTRODUÇÃO: O método mais comumente utilizado para a proteção miocárdica é o de administrar-se solução cardioplégica na circulação coronária. Entretanto, a proteção pode ser alcançada através da perfusão intermitente do sistema coronariano com sangue do próprio paciente, que é realizada por meio de múltiplas sequências de pinçamento e abertura do clamp aórtico ou por meio do pinçamento único e canulação acessória da raiz aórtica. Objetivo: Avaliar o desfecho clínico e a ocorrência de eventos neurológicos no período intra-hospitalar dos pacientes submetidos à cirurgia de revascularização do miocárdio com a técnica proposta aqui neste estudo. Métodos: Descreve-se uma técnica de proteção miocárdica no uso do pinçamento único de aorta que consiste na canulação acessória da raiz aórtica com sistema aperfeiçoado para perfusão coronária intermitente, foi realizado estudo observacional transversal prospectivo onde foram estudados 50 pacientes (idade média 58,5±7.19 anos) submetidos à cirurgia de revascularização do miocárdio sob a técnica proposta. Foram avaliadas variáveis clínicas e laboratoriais pré e pós-operatórias. Resultados: O nível médio de pico da CKMB pós-operatória foi de 51,64±27,10 U/L no segundo pós-operatório e da troponina I foi de 3,35±4,39 ng/ml no quarto pós-operatório, e estiveram dentro do limite da normalidade. Não foi observado nenhum óbito e um paciente evoluiu com alteração neurológica leve. A monitorização hemodinâmica não revelou alterações. Conclusão: A cirurgia de revascularização do miocárdio com a técnica aperfeiçoada de perfusão intermitente da raiz da aorta com pinçamento único mostrou-se como método seguro, possibilitando resultados clínicos satisfatórios nos pacientes estudados.

Introduction: The most common method used for myocardial protection is administering cardioplegic solution in the coronary circulation. Nevertheless, protection may be achieved by intermittent perfusion of the coronary system with patient's own blood. The intermittent perfusion may be performed by multiple sequences of clamping and opening of the aortic clamp or due single clamping and accessory cannulation of the aortic root as in the improved technique proposed in this study, reperfusion without the need for multiple clamping of the aorta. Objective: To evaluate the clinical outcome and the occurrence of neurological events in in-hospital patients submitted to myocardial revascularization surgery with the "improved technique" of intermittent perfusion of the aortic root with single clamping. Methods: This is a prospective, cross-sectional, observational study that describes a myocardial management technique that consists of intermittent perfusion of the aortic root with single clamping in which 50 patients (mean age 58.5±7.19 years old) have been submitted to the myocardial revasculrization surgery under the proposed technique. Clinical and laboratory variables, pre- and post-surgery, have been assessed. Results: The mean peak level of post-surgery CKMB was 51.64±27.10 U/L in the second post-surgery and of troponin I was 3.35±4.39 ng/ml in the fourth post-surgery, within normal limits. No deaths have occurred and one patient presented mild neurological disorder. Hemodynamic monitoring has not indicated any changes. Conclusion: The myocardial revascularization surgery by perfusion with the improved technique with intermittent aortic root with single clamping proved to be safe, enabling satisfactory clinical results.

OBJETIVO: Este estudo avaliou o desempenho hemodinâmico e as alterações miocárdicas decorrentes do emprego de dispositivos de assistência ventricular esquerda (DAVE), associado ou não à descompressão do ventrículo direito por meio de derivação cavo-pulmonar, sendo esses achados comparados ao emprego de assistência circulatória biventricular. MÉTODOS: Vinte e um suínos foram submetidos à indução de insuficiência cardíaca através de fibrilação ventricular, sendo a atividade circulatória mantida por DAVE durante 180 minutos. No grupo controle, foi apenas implantado o DAVE. No grupo derivação, além do DAVE foi realizada cirurgia de derivação cavo-pulmonar. No grupo biventricular, foi instituída assistência biventricular. Foram monitoradas as pressões intracavitárias por 3 horas de assistência e amostras do endocárdio dos dois ventrículos foram coletadas e analisadas à microscopia óptica e eletrônica. RESULTADOS: O lactato sérico foi significativamente menor no grupo biventricular (P=0,014). A diferença observada entre o fluxo do DAVE nos grupos derivação e controle (+55±14 ml/kg/min, P=0,072) não foi significativa, enquanto que o fluxo no grupo biventricular foi significativamente maior (+93±17 ml/kg/min, P=0,012) e se manteve estável durante o experimento. A pressão arterial média (PAM) se manteve constante apenas no grupo biventricular (P<0,001), que também apresentou diminuição significativa das pressões em câmaras direitas. Na análise ultraestrutural, notou-se menor presença edema miocárdico no ventrículo direito no grupo biventricular (P=0,017). CONCLUSÃO: Os resultados apresentados demonstram que o desempenho hemodinâmico da assistência ventricular esquerda associada à derivação cavo-pulmonar, neste modelo experimental, não foi superior ao observado com a assistência de ventrículo esquerdo isolada e não substituiu a assistência biventricular de maneira efetiva.

OBJECTIVE: Right ventricular (RV) failure during left ventricular assist device (LVAD) support can result in severe hemodynamic compromise with high mortality. This study investigated the acute effects of cavo-pulmonary anastomosis on LVAD performance and RV myocardial compromise in comparison with biventricular circulatory support, in a model of biventricular failure. METHODS: LVAD support was performed by centrifugal pump in 21 pigs with severe biventricular failure obtained by FV induction. Animals were randomized to be submitted to cavo-pulmonary anastomosis, to biventricular circulatory support or to control group. They were maintained under circulatory support and hemodynamic monitoring for 3h. Venous lactate and cytokines serum levels were also obtained. Endocardium samples were analyzed by electronic microscopy. RESULTS: FV maintenance was responsible for acute LVAD impairment after 180 min in the control group. cavo-pulmonary anastomosis resulted in non-significant improvement of LVAD pump flow in relation to control group (+55±14 ml/kg/min, P=0.072), while animals under biventricular support maintained higher LVAD flow (+93±17 ml/kg/min, P=0.012). Mean arterial pressure remained constant only in biventricular group (P<0.001), which also presented decrease of right atrial and ventricular pressures. Similar increases in lactate and cytokines levels were observed in the three groups. Ultra-structural analysis documented low levels of myocardial swelling in the biventricular group (P=0.017). CONCLUSION: The concomitant use of cavo-pulmonary anastomosis during LVAD support in a pig model of severe biventricular failure resulted in non-significant improvement of hemodynamic performance and it did not effectively replace the use of biventricular support.

INTRODUÇÃO: Antes de iniciar um tratamento para abuso de substâncias psicoativas (SPA), é primordial avaliar os prejuízos que estas acarretam na vida dos usuários. Considerando a complexidade dessa avaliação, o instrumento atualmente mais utilizado no mundo para a abordagem multidimensional de problemas relacionados ao abuso de SPA é a Escala de Gravidade de Dependência (Addiction Severity Index, ASI). OBJETIVO: Através da revisão da literatura sobre o tema, da apresentação de um caso clínico, de aspectos da ASI e da discussão de tópicos contemplados na avaliação de usuários de drogas, este trabalho tem como finalidade auxiliar o profissional de saúde a sistematizar a avaliação desses pacientes. MÉTODO: A revisão da literatura foi realizada nas bases de dados MEDLINE, LILACS e PsycINFO, utilizando os descritores assessment e evaluation, cruzados com o termo substance-related disorders dos Medical Subject Headings e subexpressões nele incluídas. RESULTADOS E CONCLUSÃO: Na literatura, há uma carência de artigos sobre o tema, e, no Brasil, ainda não existem instrumentos que se proponham a avaliar os usuários de drogas de forma tão ampla como a ASI. A utilização de uma escala com essas características pode ser útil para o sistema de saúde brasileiro, favorecendo a identificação precoce de problemas e propiciando melhora na qualidade da assistência prestada.

INTRODUCTION: Before initiating any treatment for substance abuse, the assessment of problems related to the consumption of those substances is of paramount importance. Considering the complexity of this evaluation, the instrument most widely used in the present days for a multidimensional approach to these patients is the Addiction Severity Index (ASI). OBJECTIVE: The present study presents a literature review, a clinical case vignette, comments on different aspects of the scale, and the discussion of topics covered in the evaluation process, and aims at providing support to health professionals in the sense of systematizing the assessment of these patients. METHOD: The literature review was conducted on MEDLINE, LILACS, and PsycINFO databases, using the keywords assessment and evaluation together with substance-related disorders and other terms presented as synonyms in the Medical Subject Headings. RESULTS AND CONCLUSION: There is a paucity of articles in the literature focusing on the topic, and no other instruments designed to provide a general overview of substance users (as is the case with the ASI) are available in Brazil. The use of a scale with these characteristics may be useful for the Brazilian public health system, allowing for the early identification of problems and promoting an improvement in the quality of treatment provided to these patients.

FUNDAMENTO: O transplante cardíaco enfrenta o grave problema da escassez de doadores. Estima-se que entre 20% e 40% dos pacientes falecem na fila de espera. Para esses pacientes, a utilização de dispositivos de assistência circulatória é, muitas vezes, a única possibilidade de sobrevivência durante a espera do doador. No Brasil, não existe nenhum programa regular de utilização desses dispositivos como ponte para transplante. OBJETIVO: Avaliar o desempenho hemodinâmico e a resposta inflamatória durante a utilização do DAV-InCor como ponte para transplante. MÉTODOS: Entre outubro de 2003 e abril de 2006, 11 pacientes, indicados em caráter de prioridade para o transplante cardíaco, evoluíram em choque cardiogênico refratário. O implante do DAV-InCor foi realizado em sete pacientes. O diagnóstico etiológico foi cardiopatia chagásica em cinco pacientes e cardiomiopatia dilatada idiopática em dois. RESULTADOS: A assistência mecânica ao ventrículo esquerdo foi mantida nos sete pacientes por períodos entre 14 e 42 dias (média 26,2). O desempenho hemodinâmico foi adequado, com a normalização do índice cardíaco, dos níveis de saturação venosa de O2 e do lactato. O transplante foi realizado em dois pacientes, os outros cinco faleceram por infecção sistêmica ou falência de múltiplos órgãos. CONCLUSÃO: O desempenho do DAV-Incor, no comportamento hemodinâmico dos pacientes estudados, foi adequado para a manutenção de uma condição circulatória satisfatória durante o período estudado. Houve melhora dos parâmetros de perfusão tecidual e manutenção de sinais de resposta inflamatória sistêmica. Houve alta incidência de complicações; contudo, não foram demonstradas complicações relacionadas ao dispositivo que comprometam a segurança da utilização do mesmo.

BACKGROUND: Cardiac transplantation faces the serious problem of lack of donors and it is estimated that 20 to 40% of the patients die while waiting for heart transplantation. For these patients, the use of mechanical circulatory assist devices is the only choice of survival while waiting for a donor. In Brazil, the experience with mechanical circulatory support is limited and there is no regular program regarding the use of these devices as a bridge to heart transplantation. OBJECTIVE: To evaluate the hemodynamic performance and the systemic inflammatory response during the clinical use of the InCor-type ventricular assist device (VAD-InCor) as a bridge to heart transplantation. METHODS: Between October 2003 and April 2006, 11 patients in the waiting list for heart transplantation presented hemodynamic deterioration due to refractory cardiogenic shock. Seven of these patients were submitted to VAD-InCor implantation for left ventricular assistance. The etiologic diagnosis was Chagas' disease in 5 patients and idiopathic dilated cardiomyopathy in 2. RESULTS: The duration of left ventricular assistance ranged from 14 to 42 days (mean 26.2 days). During this period, the hemodynamic performance of the DAV-InCor was adequate to support a normal hemodynamic state. There was normalization of central venous oxygen saturation and serum lactate. Two patients were submitted to heart transplantation, while the other 5 patients died under assistance due to infection and multiple organ failure. CONCLUSION: The performance of the VAD-InCor, in the hemodynamic behavior of the studied patients, was adequate for the maintenance of a satisfactory circulatory state during the studied period. There was improvement in the tissue perfusion parameters and maintenance of systemic inflammatory response signs. There was a high incidence of complications; however, complications related to the device, which could compromise the safety of its use, were not demonstrated.

OBJETIVO: Este estudo compara os efeitos do pré-condicionamento isquêmico imediato, baseado na monitorização do potencial evocado somatossensitivo (PESS), com aqueles da drenagem do líquido cefalorraquidiano, em um modelo de oclusão da aorta torácica descendente em cães. MÉTODO: Dezoito cães foram submetidos à isquemia medular induzida pela oclusão da aorta torácica descendente por 60 minutos. O Grupo Controle foi submetido à oclusão da aorta (n=6), o Grupo Pré-Condicionamento Isquêmico (PCI), ao pré-condicionamento isquêmico (n=6) e o grupo drenagem, à drenagem do líquido cefalorraquidiano (n=6), imediatamente antes da oclusão da aorta. A condição neurológica foi acessada por um observador independente, de acordo com a escala de Tarlov. Os animais foram sacrificados e as medulas retiradas para exame histopatológico. RESULTADOS: Pressões da aorta proximal e distal à oclusão foram semelhantes nos três grupos. Sete dias após o procedimento, o índice de Tarlov foi significativamente maior em comparação ao Grupo Controle, somente no Grupo PCI (p<0,05). Foram observados valores menores no tempo de recuperação do PESS com o uso da drenagem liquórica durante a fase final de reperfusão (p<0,01). Exame histopatológico evidenciou necrose menos grave na substância cinzenta torácica e lombar, nos animais submetidos aos dois métodos de proteção medular, sendo mais pronunciada no Grupo PCI (p<0,001). CONCLUSÃO: A drenagem do líquor e o pré-condicionamento isquêmico parecem proteger a medula espinhal, durante a oclusão da aorta torácica descendente. Entretanto, o nível de proteção medular obtido parece ser mais significativo com a drenagem do líquido cefalorraquidiano.

OBJECTIVE: This study compares the effects of immediate ischemic preconditioning based on somatosensory evoked potential (SSEP) monitoring with those of cerebrospinal fluid drainage in a model of descending thoracic aorta occlusion in dogs. METHOD: Eighteen dogs were submitted to spinal cord ischemia induced by descending thoracic aortic cross-clamping for 60 minutes. The Control Group underwent only aortic cross-clamping (n=6). The Ischemic Preconditioning Group (IPC) underwent ischemic preconditioning (n=6) and the Drainage Group underwent cerebrospinal fluid drainage (n=6), immediately before aortic cross-clamping. An independent observer assessed neurological status according to the Tarlov score. The animals were sacrificed and spinal cord harvested for histopathologic study. RESULTS: Aortic pressure before and after the occluded segment was similar in the three groups. Seven days after the procedure, Tarlov scores were significantly higher only in the Drainage Group when compared to the Control Group (p<0.05). Lower SSEP recovery times were also observed with cerebrospinal fluid drainage during the final reperfusion period (p<0.01). In the histopathologic study, stain showed less significant neuronal necrosis in the thoracic and lumbar gray matter in animals submitted to both methods of spinal cord protection, with it being more pronounced in the Ischemic Preconditioning Group (p<0.001). CONCLUSION: Cerebrospinal fluid drainage and immediate ischemic preconditioning seems to protect the spinal cord during descending thoracic aorta cross-clamping. Nevertheless, the obtained level of spinal cord protection seems to be more significant with cerebrospinal fluid drainage.

É relatado o caso de um paciente do sexo masculino com idade de 71 anos, dando entrada no pronto-atendimento com palidez cutaneomucosa, acompanhada de hipertensão arterial sistêmica e dor torácica. Na investigação diagnóstica não foi evidenciada alteração compatível com isquemia miocárdica aguda. A radiografia de tórax evidenciava alargamento importante do mediastino. Ao ecocardiograma, a aorta ascendente media 47 mm, no nível do tronco pulmonar. Um dia após o eco, o paciente foi submetido a exame de ressonância magnética (RNM), quando se evidenciou aorta ascendente de 62 mm, sem evidenciar fluxo em falsa luz ou "flap" intimal, mas mostrando hematoma intramural da aorta ascendente, estendendo-se da raiz da aorta até um terço proximal do arco aórtico. Procedeu-se a correção cirúrgica, sendo realizada substituição da aorta ascendente e parte do arco aórtico (hemiarco), com preservação da valva aórtica pela suspensão das comissuras. Paciente evolui bem sem intercorrência, recebendo alta no nono dia de pós-operatório. Enfatizamos nesse relato de caso a semelhança do quadro clínico do hematoma intramural da aorta com o quadro de dissecção da aorta, a importância de se estabelecer diagnóstico correto e o melhor tratamento.

It is reported the case of a 71 year old male patient admitted to the emergency service pale and with systemic arterial hypertension and thoracic pain. In the diagnostic investigation, there was no evidence of compatible with acute myocardial ischemia. The thorax x-ray showed important enlargement of the mediastinum. In the echocardiogram the ascending aorta measured 47mm, at the level of the pulmonary artery. One day after the echo, submitted to exam of magnetic resonance (RNM), the ascending aorta had a diameter of 62mm, without false lumen flow or intimal "flap", but showing intramural hematoma envolving the ascending aorta and the proximal portion of the aorta. It was submitted to the surgical correction, being accomplished by resection of the ascending aorta and part of the aortic arch (hemiarch), with preservation of the aortic valve with suspension of the comissures. The patient had ununventfull recovery, being discharged in the 9th postoperative day. We emphasized the similarity of the clinical presentation of the intramural hematoma of the aorta with that of aortic dissection, the importance of establishing correct diagnosis and the best treatment.

INTRODUÇÃO: O choque cardiogênico é responsável por elevados índices de mortalidade na fila de espera para o transplante cardíaco. Na cardiomiopatia chagásica, a alta incidência de disfunção biventricular pode contribuir com a gravidade desta complicação. MÉTODO: Foram estudados 141 pacientes indicados em caráter de prioridade para o transplante. Destes pacientes, 46 eram portadores de cardiomiopatia chagásica e 95 de outras cardiomiopatias. O choque cardiogênico foi tratado farmacologicamente e com o implante ocasional do balão intra-aórtico. Em cinco pacientes chagásicos, foi realizado o implante de dispositivo paracorpóreo de assistência ventricular esquerda. RESULTADOS: Num período médio de 2,8 meses, 58 (41,1%) dos 141 pacientes foram transplantados, 73 (51,7%) faleceram e 10 foram retirados da fila. A mortalidade entre os pacientes chagásicos e não chagásicos foi de 45,6% e 54,7%, respectivamente. No entanto, a expectativa média de vida, sem a realização do transplante cardíaco, dos pacientes chagásicos foi de apenas 1,5 meses, sendo observado risco relativo de mortalidade de 1,6 para estes pacientes em relação aos não chagásicos (p<0,05). Os cinco pacientes chagásicos submetidos ao implante do dispositivo de assistência circulatória foram mantidos por um período médio de 22 dias, sendo que dois foram transplantados, dois faleceram por falência de múltiplos órgãos e um ainda está sob assistência. Nenhum destes pacientes apresentou disfunção do ventrículo direito, não tendo ocorrido qualquer complicação relacionada ao dispositivo. CONCLUSÃO: A evolução do choque cardiogênico parece ser mais rápida na cardiomiopatia chagásica, sendo importante a indicação precoce de dispositivos de assistência mecânica como ponte para a realização do transplante cardíaco.

INTRODUCTION: Heart failure is responsible for high mortality rates of patients on heart transplantation waiting lists. In Chagas cardiomyopathy, the presence of biventricular dysfunction increases the severity of this situation. METHOD: One hundred and forty-one patients suffering from cardiogenic shock, listed as high priority for heart transplantation, were studied. Forty-six patients presented with Chagas cardiomyopathy and 95 with other cardiomyopathies. Heart failure was treated using intravenous inotropic drugs and intra-aortic balloon pump implantation. Five patients with Chagas disease underwent paracorporeal left ventricular assist device implantation. RESULTS: During a mean follow-up of 2.8 months, 58 (41.1%) of the 141 patients were transplanted, while 73 (53.7%) died and 10 were removed from the waiting list. The mortality rates in chagasic and non-chagasic patients were 45.6% and 54.7%, respectively. The mean expected survival of patients with Chagas disease, who did not undergo heart transplantation, was only 1.5 months, with these patients presenting a relative risk for death of 1.6 compared to patients with other heart diseases (p<0.05). The five chagasic patients submitted to left ventricular assist device implantation were maintained on support for a mean of 22 days, with two of them undergoing transplantation, two died due to multiple organ failure and one remains on circulatory support. None of these patients presented right ventricular dysfunction and there were no device related complications. CONCLUSION: The evolution of heart failure seems to be rapid in patients with Chagas cardiomyopathy. Therefore, an early indication of mechanical circulatory support is important as a bridge to heart transplantation in these patients.

OBJETIVO: Analisar, retrospectivamente, a sobrevivência dos pacientes submetidos a cardiomioplastia dinâmica, determinando a influência de fatores pré, intra e pós-operatório e o comportamento da fração de ejeção conforme o modo de estimulação. MÉTODO: Foram analisados 43 pacientes submetidos a cardiomioplastia dinâmica, no período de maio de 1988 a setembro de 1997. A classe funcional III foi predominante (81,4%). A fração de ejeção média do VE foi de 19,37±3,48%. A mortalidade hospitalar foi de 2,2% e 39 pacientes, que completaram o período de condicionamento, tiveram seguimento médio de 46±26 meses. Vinte e oito pacientes foram mantidos predominantemente sob estimulação em modo 1:1 e 11 pacientes foram mantidos predominantemente sob estimulação em modo 1:2. RESULTADOS: A sobrevivência foi de 9% em nove anos. As causas de óbito foram progressão da insuficiência cardíaca e morte súbita. A classe funcional, o índice de resistência vascular pulmonar e o modo de estimulação foram identificados como fatores independentes de prognóstico. A manutenção da melhora da fração de ejeção do VE em relação ao pré-operatório foi mais consistente ao longo do período estudado, quando o enxerto muscular foi estimulado em modo 1:2. CONCLUSÃO: Os resultados tardios da cardiomioplastia dinâmica são limitados pela alta incidência de morte súbita e de óbitos decorrentes da progressão da doença de base. A sobrevivência é influenciada pela condição clínica pré-operatória e pelo modo de estimulação do enxerto. O desempenho do enxerto muscular é mais satisfatório a longo prazo quando submetido à estimulação modo 1:2.

PURPOSE: To retrospectively evaluate the survival rate of patients that underwent dynamic cardiomyoplasty, determining the influence of pre-, intra- and post-operative factors and the evolution of left ventricle ejection fraction according to the stimulation mode. METHOD: Forty-three patients that underwent dynamic cardiomyoplasty between May 1988 and September 1997 were analyzed. Functional class III was predominant (81.4%). The mean left ventricle ejection fraction was 19.37 ± 3.48%. Hospital death was 2.2% and 39 patients who completed the conditioning period had a mean follow up of 46 ± 26 months. Twenty-eight patients were predominantly maintained under stimulation mode 1:1 and 11 under 1:2 stimulation mode. RESULTS: Survival rate at nine years of follow up was 9%. The causes of death were progression of the heart failure and sudden cardiac death. Functional class, the pulmonary vascular-resistance index and stimulation mode were identified as risk factors. The maintenance of the increase of the left ventricle ejection fraction when compared to the preoperative values was more consistent during the studied period when the graft was stimulated in the 1:2 mode. CONCLUSION: Late results of dynamic cardiomyoplasty are limited by the high incidence of deaths by progression of the heart failure and sudden cardiac death. Survival rate is influenced by the preoperative clinical condition and by the stimulation mode. Muscle graft performance is higher at late follow up with the 1:2 stimulation mode.

O tratamento com injeção intranodular de etanol vem sendo utilizado há uma década como modalidade terapêutica eficaz no manejo de pacientes com nódulos da tireóide. Diversos estudos validaram o papel desta forma de tratamento em nódulos autônomos, císticos e, mais recentemente, nódulos frios benignos. OBJETIVO: Avaliar a efetividade da injeção intranodular de etanol no tratamento de nódulos císticos e autônomos da tireóide. MÉTODOS: 42 pacientes (26 pacientes com nódulos císticos e 16 com nódulos autônomos da tireóide) foram submetidos a tratamento com aplicação intranodular de etanol a 99%, guiada por ultra-sonografia e acompanhados por um período mínimo de seis meses. RESULTADOS: Não observamos a ocorrência de complicações maiores durante ou após o tratamento, porém, na maioria dos casos, houve queixa de dor de leve a moderada intensidade e/ou desconforto após a aplicação do etanol. A maioria dos nódulos apresentou redução volumétrica significativa. A média de redução obtida para os nódulos autônomos foi de 50,3% e para os nódulos císticos, 69,3%. Não houve diferença significativa entre os valores basais de T3 total, T4 total e TSH em comparação aos valores obtidos seis meses após o tratamento para o grupo de pacientes com nódulos císticos. O grupo de pacientes com nódulos autônomos apresentou redução nos valores séricos de T3 total e T4 total, assim como elevação dos níveis de TSH, confirmando a efetividade do tratamento. CONCLUSÃO: A injeção intranodular de etanol é uma alternativa segura e eficaz no tratamento de nódulos autônomos e císticos da tireóide.

Intranodular ethanol injection has been used for the past 10 years as an efficient modality for treating patients with thyroid nodules. Several studies have reported the success of this therapy in autonomous and cystic nodules and, more recently, in cold benign nodules. PURPOSE: To evaluate the efficacy of this therapeutic modality on the treatment of autonomous and cystic thyroid nodules. METHODS: 42 patients (26 with cystic and 16 with autonomous nodules) were treated with ultrasound guided intranodular 99% ethanol injection and followed for 6 months. RESULTS: No major complications were observed during or after treatment, however, most of the patients reported slight to moderate pain and/or discomfort after the injection. Most of the nodules showed reduction after the treatment. Autonomous nodules had a mean reduction of 50.3% and cystic nodules of 69.3%. No significant differences in pretreatment serum total T3, total T4 or TSH were observed among the patients in the cystic group. Patients in the autonomous group with hyperfunctioning nodules showed a decrease in serum total T3, total T4 and an increase in serum TSH levels, hence, proving the effectiveness of this therapy. CONCLUSIONS: Intranodular ethanol injection is a safe and efficient treatment for autonomous and cystic nodules of the thyroid.

OBJETIVO: Nesta investigação, os resultados tardios da ventriculectomia parcial esquerda, associada à correção da insuficiência das valvas atrioventriculares, foram estudados em 43 pacientes portadores de cardiomiopatia dilatada. CASUÍSTICA E MÉTODOS: Os pacientes estavam em classe funcional III (18) ou IV (25) no pré-operatório, sendo que 7 pacientes foram operados na vigência de choque cardiogênico. A redução cirúrgica do volume do ventrículo esquerdo (VE) foi associada à anuloplastia mitral em 32 pacientes e à substituição daquela valva em 3. Em 10 pacientes, também foi realizada plastia de valva tricúspide. Doze pacientes foram submetidos ao implante de desfibriladores automáticos. RESULTADOS: Ocorreram 9 (20,9%) óbitos hospitalares. O tempo de seguimento pós-operatório variou entre dois e 68 meses, com média de 34,2 meses. Aos seis meses de seguimento, 8 pacientes estavam em classe funcional I, 13 em classe II, 3 em classe III e 1 em classe IV (p<0,001). Por outro lado, outros 19 pacientes faleceram no seguimento tardio, sendo observados índices de sobrevida de 58 (?7% em um ano, de 48 (?7% aos dois anos e de 32?(?8% aos cinco anos. A regressão logística mostrou que a sobrevida nos primeiros seis meses foi influenciada pelo diâmetro das fibras miocárdicas. A análise da sobrevida tardia através do modelo proporcional de Cox mostrou que a classe funcional IV e o nível elevado de nor-adrenalina sérica no pré-operatório estavam relacionados a maior mortalidade. Quando a análise também incluiu variáveis anatomopatológicas, a existência de apoptose das células miocárdicas e o maior diâmetro das fibras miocárdicas no pré-operatório foram identificados como únicos fatores independentes de pior prognóstico tardio. Pacientes operados em classe funcional III ou IV apresentaram índices de sobrevida de 60,6?(?11,6% e de 14,4?(?8,2% em cinco anos, respectivamente. Na presença de apoptose das células miocárdicas, a sobrevida dos pacientes foi de 8,3?(?7,9% no mesmo período, contra 63,1?(?11% na ausência dessa alteração. Em relação à função ventricular, os benefícios observados após a ventriculectomia parcial não se mantiveram tardiamente. Houve redilatação progressiva do ventrículo esquerdo, associada à queda concomitante dos valores de sua fração de ejeção. CONCLUSÃO: A ventriculectomia parcial esquerda, associada, quando necessário, à correção da insuficiência das valvas atrioventriculares, melhora a condição clínica e a função ventricular de pacientes com cardiomiopatia dilatada idiopática. Por outro lado, os resultados tardios deste procedimento são limitados pelo grau de comprometimento das fibras miocárdicas e pela severidade do comprometimento funcional dos pacientes no pré-operatório. Além disso, os seus benefícios a função ventricular são comprometidos pela possibilidade de redilatação do ventrículo esquerdo.

OBJECTIVE: Partial left ventriculectomy has been performed in patients with severe cardiomyopathies. The purpose of this investigation is to document the clinical effects of this procedure, associated with mitral insufficiency correction, in 43 patients with idiopathic dilated cardiomyopathy. METHODS: Eighteen patients were in New York Heart Association class III and 25 were in persistent class IV. Seven of these patients were operated on in cardiogenic shock. The procedure was associated with mitral anuloplasty in 32 patients and with mitral replacement in three. Ten patients were also submitted to De Vega tricuspid valve anuloplasty. Automatic cardioverter-defibrillators were implanted in 12 patients. RESULTS: Nine (20.9%) patients died during the hospital period. The follow-up time ranged from two to 57 months, with a mean of 28.3 months. At six months of follow-up, eight patients were in functional class I, 13 patients in class II, three patients in class III e one patient in class IV (p<0.001). On the other hand, nine patients died during the first six months and other ten in the later postoperative period. Actuarial survival rates were 58?(?7% at one year, 48?(?7% at two years and 32 (?8% at five years of follow-up. Stepwise logistic regression demonstrated that six-month survival was significantly affected by the degree of myocytes hypertrophy. The Cox proportional-hazards analysis showed that preoperative functional class and nor-epinephrine levels were significantly associated with a long-term unfavorable outcome. When that analysis also included anatomicopathological variables, the existence of apoptotic myocardial cells and of more important myocytes hypertrophy were identified as the unique independent predictors of poor outcome. Patients operated in functional class III or IV presented survival rates of 60 ±?11% and of 14 (?8% at five years, respectively. In the presence of myocardial cells apoptosis, the survival was 8?(?7% at the same period, where as it was 63 (11% in the absence of that alteration. CONCLUSION: Partial left ventriculectomy, associated, when necessary, with mitral insufficiency correction, improves LV function and ameliorates congestive heart failure in patients with idiopathic dilated cardiomyopathy. However, this procedure clinical application is limited by the high mortality observed in the first postoperative months and by the possibility of heart failure progression and of arrhythmia related events at the late follow-up. These facts seem to be influenced by myocardial cells compromise and by patients' preoperative condition.

Os mixomas são os tumores cardíacos primários mais freqüentes. Sua localização habitual é o átrio esquerdo, sendo encontrados também em outros locais. É relatado o caso de paciente de 71 anos que, com diagnóstico de tumor em átrio direito, foi submetido a operação para retirada do tumor. A operação foi realizada, sendo confirmado o diagnóstico e procedida a ressecção do tumor com sucesso e sem intercorrência. Em estudo ecocardiográfico de controle no 4º mês de pós-operatório, evidenciou-se presença de massa residual que parecia originar-se na veia cava inferior. Foi submetida a nova operação, em que foi realizada a ressecção do mixoma, que se originava na veia cava inferior e se projetava para o interior do átrio direito. Com o tumor, foi ressecada, na sua implantação, uma porção da veia cava inferior. O presente relato mostra uma localização rara de origem do mixoma, bem como as complicações quanto ao diagnóstico e à abordagem no tratamento cirúrgico.

The myxomas are the most frequent primary cardiac tumors. They are usually located in the left atrium but can be found in other places. This is a case report of a 71 year old patient with diagnosis of a tumor arising from the right atrium, submitted to a surgical resection of the tumor. The operation was realized and the diagnosis confirmed. Resection was successful and the procedure uneventful. Four months postoperatively a standard two-dimensional echocardiogram revealed a residual mass that seemed to arise from the inferior vena cava. He was reoperated and the myxoma originating from the inferior vena cava and extending to the interior of the right atrium was resected. At the basis of the tumor implantation, a portion of the inferior vena cave was resected. The present report shows an unusual location of the myxoma as well as the complications regarding the diagnosis and the approach to surgical treatment.
