The Efficacy of Porcine Dermal Collagen to Treat Complex Anal Fistulae. Results of a Multicenter Cohort

Abstract Introduction The management of complex anal fistulae remains a topical surgical problem. The choice and success of surgical management are based on the balance between healing and continence. Although porcine dermal collagen (Permacol Collagen Paste [PCP]- Covidien plc, Gosport, Hampshire, UK) represents a new generation of non-solid biomaterials, its results in anal fistulae are mixed. Methods A multicenter observational retrospective analysis of consecutive patients with cryptoglandular anal fistula treated in four colorectal surgery units was performed between 2015 and 2020. Clinical cure of the fistula was the main outcome measure. Adverse events and alterations in anal continence were secondary outcomes. Results The study included 119 patients (87 males, 71.1%), with a mean age of 53 years (IR 44-65). Most patients had complex (80.6%) and recurrent (91.6%) fistulae. With the first PCP treatment, the overall cure rate was 41.2% (49 patients) and 45.4% with the second treatment (5 out of 17 patients). The mean follow-up period was 17 months (IR 5-25). Healing was not affected by the location and type of fistula, the existence or not of a cavity, the number of tracts, or the administration of prophylactic antibiotics. After the PCP treatment, no patient in the series had worsening of continence. Morbidity affected 22.7% of the patients (27), with postoperative abscesses being the most frequent adverse event. There were no statistical differences between the four hospitals studied. Conclusions Permacol collagen paste is a safe and easily reproducible therapy for complicated anal fistulae that has moderate efficacy. The overall success rate is slightly over 40%, with no detriment to fecal continence.

ABSTRACT Introduction: Chagas disease is a neglected tropical disease of interest to public health because of its social and economic burden. Identifying infected and sick people with Chagas disease constitutes the first step towards achieving World Health Ooganization’s goals for 2020. Objective: To evaluate the reproducibility with gold standard of a rapid diagnostic test for detection of antibodies to T. cruzi; and to propose a diagnostic algorithm for Chagas disease under the point-of-care concept in an area with limited access to health care coverage. Material and Methods: A cross-sectional study was performed to detect antibodies to T. cruzi in 151 indigenous volunteers belonging to three ethnic groups of the Sierra Nevada de Santa Marta, Colombia. Rapid tests-PDR SD BIOLINE Chagas Ab were implemented in the field versus confirmation in the laboratory using two standardized serological methods (ELISAs). Results: The results show that 19,2 % seroreactivity for T. cruzi was found among the entire population screening. The highest rate of human infection with T. cruzi was detected in the Wiwa community. No significant differences between rapid diagnostic test and the standard techniques (ELISAs) were found. Sensitivity, specificity and concordance for RDT were 100 % (Kappa: 1,0). Conclusions: The Sierra Nevada de Santa Marta continues to be a hyperendemic area for Chagas disease. The area is difficult to access and has low or no primary health care coverage, making the assessed rapid diagnostic test a useful tool for screening programs and defining treatment and control plans, which represents the first approach at establishing a point-of-care testing strategy for endemic countries for Chagas disease.
RESUMEN Introducción: La enfermedad de Chagas es una enfermedad desatendida de interés en salud pública por su carga social y económica. Identificar personas infectadas y enfermas con el mal de Chagas constituye el primer paso para alcanzar los objetivos de la Organización Mundial de la Salud para el 2020. Objetivo: Evaluar la reproducibilidad de una prueba de diagnóstico rápida para la detección de anticuerpos contra T. cruzi; y proponer un algoritmo de diagnóstico para enfermedad de Chagas bajo el concepto de uso de tecnologías en el lugar de atención en áreas de acceso limitados a los servicios de salud. Materiales y métodos: Se realizó un estudio de corte transversal para la detección de anticuerpos para T. cruzi a 151 indígenas voluntarios pertenecientes a tres grupos étnicos de la Sierra Nevada de Santa Marta, Colombia. Se implementó pruebas rápidas-PDR SD BIOLINE Chagas Ab en campo versus la confirmación en el laboratorio mediante dos métodos serológicos estandarizados (ELISAs). Resultados: Se encontró el 19,2 % de seroreactividad para T. cruzi entre toda la población estudiada. La tasa más alta de infección humana por T. cruzi se detectó en la comunidad Wiwa. No hubo diferencias significativas entre la prueba de diagnóstico rápida y las técnicas estandarizadas (ELISAS). La sensibilidad, especificidad y concordancia para la PDR fue del 100 % (Kappa: 1,0). Conclusiones: La Sierra Nevada de Santa Marta continúa siendo un área hiperendémica para la enfermedad de Chagas. Dado que es un área de difícil acceso, con baja o nula cobertura en atención primaria en salud, la prueba de diagnóstico rápida evaluada se convierte en una herramienta útil como prueba de elección para programas de tamización y definir planes de acción de tratamiento y control, y representa el primer acercamiento de uso de tecnologías en el sitio de atención para el diagnóstico rápido en países endémicos para la enfermedad.

Abstract Species of the genus Cinchona are recognized for their effectiveness in treating malaria. For Peru, this genus is of greater importance regarding forestry and civic aspects, because it represents the richness of the plant resource. Nevertheless, nowadays Cinchona populations are strongly threatened by various anthropogenic activities. Cutervo National Park (PNC) is the Peruvian's first Protected Natural Area (ANP) and is considered as a conservation unit for the genus Cinchona. Objective: Determine the genetic diversity and phylogenetic relationships of the species of the genus Cinchona that inhabit the PNC. Methods: We tested 13 samples using the matK marker and a phylogenetic tree was generated using the maximum likelihood method and bootstrap measures of phylogenetic support. Results: 8 number of haplotypes (h) were recorded, with haplotype diversity (Hd) 0.859 and nucleotide diversity (π) 0.35%, also identifying: C. calisaya, C. pubescens and C. pitayensis for the PNC; the phylogenetic tree showed C. pitayensis as conserved specie, meanwhile C. pubsecens is present in many groups. Conclusions: The high genetic diversity of Cinchona and haplotype record of C. calisaya, species with a national conservation category of "Vulnerable", increases the importance for biodiversity conservation of the PNC.
Resumen Las especies del género Cinchona son reconocidas mundialmente por su eficacia en el tratamiento contra la malaria. Para el Perú, este género reviste mayor importancia a los aspectos forestal y cívico, por representar la riqueza del recurso vegetal. Sin embargo, en la actualidad sus poblaciones se encuentran fuertemente amenazadas por diversas actividades antropogénicas. El Parque Nacional de Cutervo (PNC) es la primer Área Natural Protegida (ANP) del Perú y es considerado como unidad de conservación para el género Cinchona. Objetivo: Determinar la diversidad genética y las relaciones filogenéticas de las especies del género Cinchona presentes en el PNC. Métodos: Nosotros evaluamos 13 muestras empleando el marcador matK y se generó un árbol filogenético empleando el método de máxima verosimilitud con soporte de filogenia bootstrap. Resultados: 8 haplotipos fueron registrados, con una diversidad haplotípica (Hd) 0.859 y nucleotídica (π) 0.35 %, identificándose además a: C. calisaya, C. pubescens y C. pitayensis para el PNC; el árbol filogenético mostró que la especie C. pitayensis se encuentra bastante conservada, mientras que C. pubsecens se encuentra presente en distintos grupos con similitud variable. Conclusiones: La elevada diversidad genética de Cinchona y el registro del haplotipo de C. calisaya, especie con categoría de conservación nacional “Vulnerable”, incrementa la importancia para la conservación de la biodiversidad del PNC.

Abstract Recent studies have reported the occurrence of thrombotic phenomena or coagulopathy in patients with COVID-19. There are divergent positions regarding the prevention, diagnosis, and treatment of these phenomena, and current clinical practice is based solely on deductions by extension from retrospective studies, case series, observational studies, and international guidelines developed prior to the pandemic. In this context, the aim was to generate a group of recommendations on the prevention, diagnosis and management of thrombotic complications associated with COVID-19. Methods: A rapid guidance was carried out applying the GRADE Evidence to Decision (EtD) frameworks and an iterative participation system, with statistical and qualitative analysis. Results: 31 clinical recommendations were generated focused on: a) Coagulation tests in symptomatic adults with suspected infection or confirmed SARS CoV-2 infection; b) Thromboprophylaxis in adults diagnosed with COVID-19 (Risk scales, thromboprophylaxis for outpatient, in-hospital management, and duration of thromboprophylaxis after discharge from hospitalization), c) Diagnosis and treatment of thrombotic complications, and d) Management of people with previous indication of anticoagulant agents. Conclusions: Recommendations of this consensus guide clinical decision-making regarding the prevention, diagnosis, and treatment of thrombotic phenomena in patients with COVID-19, and represent an agreement that will help decrease the dispersion in clinical practices according to the challenge imposed by the pandemic.

Abstract Introduction Multiple incisions have been described for the surgical approach of cervical neck nodes. All of these descriptions are associated with better or worse exposure of the surgical field as well as with different functional and aesthetic results, which are not always satisfactory. Objective Compare the transverse cervical incision with the classic incision in J or U. Methods This is a retrospective study of 47 patients who required cervical neck dissection between June 15, 2016 and June 15, 2017.A transversal incision was made in these surgeries, and their results were then compared with those of a group of 57 patients treated between January 1, 2010 and January 1, 2012, in whose cases an incision in J or U was made. Results Regarding the incision type, complications were present in 4 (8.5 %) cases in the transversal incision group, and in 7 (12.2 %) patients of the group of traditional incisions in J or U, without statistical differences (p = 0.078). The only variables associated with complications of healing in the two groups was body mass index (BMI) < 18.5. The patients showed subjective satisfaction with the aesthetic result of the transverse incision, with an average of 7.51 vs 6.20 in the J or U incision. Conclusion The transverse incision represents a safe, aesthetic, and oncologically adequate option, associated with a lower cicatricial retraction rate, without significant complication rate and allowing adequate exposure of the surgical field, similar to the obtained with the classic incision in J or U.

Calidad nutrimental y nutracéutica en ejotes de caupí (Vigna unguiculata [L] walp.) de la península de Yucatán

ABSTRACT. Cowpea beans are an important legume whose primary consumption is in dry grains, although immature pods rich in protein, vitamins, fiber, minerals and antioxidants are sometimes included. The objective was to determine the nutritional and nutraceutical quality of 13 accessions of cowpea beans (Vigna unguiculata [L.] Walp.) grown in the Yucatan Peninsula. The study was carried out under greenhouse conditions, under a completely randomized design. The moisture content, ash, fat, crude protein (CP), fiber, carbohydrates, length and weight of pods, mineral content, total phenols (TP) and antioxidant capacity (AC) were determined by the ABTS method and DPPH. The accessions HEC02, OXC05, JMM09, JMM10 and JAL14 stood out for presenting the longest length and weight of the pods. Accessions OXC04 and CHE06 excelled in ash content (8.48%), moisture (18.24%), CP (23.32%) and fiber (20.11%). In the mineral content, the HEC02 accession obtained the highest levels of K, Ca and Mg. Regarding the content of nutraceuticals, the HAL14 collection presented the highest values of TP (18.02 mg g-1 EAG ps) and CA for DPPH (6.19 mg Trolox 100 g-1 ps) and ABTS (4.95 mg Trolox 100 g-1 ps). The variability between accessions suggests that CHE06 due to its levels of protein and crude fiber, and HEC02 due to its high mineral content could offer nutritional advantages, while the HAL14 accession could be considered the accession with the best nutraceutical quality.
RESUMEN. El frijol caupí es una importante leguminosa cuyo consumo primario es en granos secos, aunque en ocasiones se incluyen las vainas inmaduras ricas en proteínas, vitaminas, fibra, minerales y antioxidantes. El objetivo fue determinar la calidad nutrimental y nutracéutica de 13 accesiones de ejotes de caupí (Vigna unguiculata [L.] Walp.) cultivados en la Península de Yucatán. El estudio se llevó a cabo en condiciones de invernadero, bajo un diseño completamente al azar. Se determinó el contenido de humedad, cenizas, grasa, proteína, fibra cruda (PC), carbohidratos, longitud y peso de las vainas, contenido de minerales, fenoles totales (FT) y la capacidad antioxidante (CA) por el método de ABTS y DPPH. Las accesiones HEC02, OXC05, JMM09, JMM10 y JAL14 destacaron por presentar la mayor longitud y peso de las vainas. Las accesiones OXC04 y CHE06 sobresalieron en contenido de cenizas (8.48%), humedad (18.24%), PC (23.32%) y fibra (20.11%). Para el contenido de minerales, la accesión HEC02 tuvo los mayores niveles de K, Ca y Mg. Respecto al contenido de nutracéuticos, la colecta HAL14 presentó los mayores valores de FT (18.02 mg g-1 EAG ps) y CA para DPPH (6.19 mg Trolox 100 g-1 ps) y ABTS (4.95 mg Trolox 100 g-1 ps). La variabilidad entre accesiones, sugiere que CHE06 por sus niveles de proteína y fibra cruda y HEC02 por su alto contenido de minerales podrían ofrecer ventajas nutricionales, mientras que la accesión HAL14 podría considerarse la accesión con la mejor calidad nutracéutica.

The cowpea (Vigna unguiculata (L) Walp) is an important crop in several regions of the world, but in Mexico it is little used. The characterization of agricultural varieties has been based on morphological descriptors, including seed shape, color and size, as well as quantitative attributes such as mineral content. The aim of this research was to evaluate the morphological and nutritional characteristics of cowpea genotypes from the Yucatan Peninsula, Mexico. Fifteen collections were made from October 2017 to January 2018 and then evaluated for three qualitative morphological variables and 10 quantitative ones. The content of potassium, calcium, manganese, iron and zinc was determined by means of micro X-ray fluorescence. An analysis of variance and principal components were performed. Eighty percent of the collections have an ovoid shape, 73% a smooth to rough texture and 80% are black. In the quantitative morphological variables, 53% of the collections were considered to have small seeds, 40% medium and 7% large. The length, width and thickness of the genotypes were from 6.26 to 9.56, 3.64 to 7.48 and 2.86 to 5.56 mm, respectively, and the PET15 collection was the smallest. For mineral content, the OXC04 collection had the highest concentration of Ca, Mn, Fe and Zn, while the PET07 collection had the highest K content. There are high levels of phenotypic diversity in the collections studied and they are grouped based on their physical characteristics and not on their place of origin.
RESUMEN El frijol caupí (Vigna unguiculata (L) Walp) es un cultivo importante en varias regiones del mundo, pero en México no es muy explotado. La caracterización de variedades agrícolas se ha basado en descriptores morfológicos, en los que se incluyen: forma, color y tamaño de semilla, además de atributos cuantitativos como el contenido mineral. El objetivo del trabajo fue evaluar las características morfológicas y nutricionales de genotipos de frijol caupí de la Península de Yucatán, México. Se realizaron 15 colectas de octubre 2017 a enero 2018, a las que se evaluaron tres variables morfológicas cualitativas, 10 variables cuantitativas y se determinó el contenido de potasio, calcio, manganeso, hierro y zinc, mediante µ-fluorescencia de rayos-X. A los datos se les realizó un análisis de varianza y de componentes principales. El 80% de las colectas tienen forma ovoide, 73% textura suave a áspera y el 80% son de color negro. En las variables morfológicas cuantitativas, 53% de las colectas se consideran pequeñas, 40% medianas y 7% semillas grandes. La longitud, ancho y espesor de los genotipos fue de 6.26 a 9.56, 3.64 a 7.48 y de 2.86 a 5.56 mm, respectivamente, la colecta PET15 fue la de menor dimensión. Para el contenido mineral, la colecta OXC04 tuvo la mayor concentración de Ca, Mn, Fe y Zn, mientras que la colecta PET07 presentó mayor contenido de K. Hay altos niveles de diversidad fenotípica en las colectas estudiadas y se agrupan con base en sus características físicas y no al lugar de origen.

Abstract Introduction: Achalasia is an esophageal motor disorder characterized by failure in esophageal peristalsis as well as relaxation of the lower esophageal sphincter (LES) in response to swallowing (1). It is a rare pathology, with an incidence ranging from 0.3-1.63 per 100,000 adults/year and a prevalence of 8.7-10.8 per 100,000 inhabitants (2). The treatment of Achalasia is not curative, but the objective is to reduce the resistance to flow caused by the lack of relaxation in the LES (3). In 2010, Inoue et al described the technique of per oral endoscopic myotomy (POEM) performed in 17 patients with excellent results in terms of resolution of dysphagia (7). Nine years after the description of the POEM technique for endoscopic treatment, nowadays in Mexico, there are few centers that perform it. Objective: To present the initial experience of the Per-Oral Endoscopic Myotomy (POEM) procedure performed for the treatment of Achalasia in the General Hospital “Dr. Manuel Gea González”. Materials and methods: A review of the Gastrointestinal Endoscopy Service database of the General Hospital “Dr. Manuel Gea González” in search of patients who underwent a POEM procedure for the treatment of Achalasia in the last 3 years. We analyzed demographic characteristics of the patients, classification of the type Achalasia, Eckardt scale before and after the procedure, manometry before and after the procedure, length of the myotomy, surgical time, days of hospital stay, 24hr pH testing before and after the procedure and complications. Results: During the 3-year period, 11 patients underwent POEM technique for the treatment of Achalasia. Eight men and 3 women. Average age was 63 years (48-83). Predominant type of Achalasia was type I. Average Eckardt score prior to the POEM was 8 (7-10) in contrast to the average in the Eckardt score after the POEM was 1 (1-2). Mean surgical time was 2.6 hours (2-4) and the average number of days of hospital stay was 4.4 days (4-8). The mean length of the myotomy was 8.6 cm (7-10). The manometry after procedure showed improvement with respect to the previous one in 100% of the cases, contrary to the esophageal pH testing that showed abnormalities after the POEM in 27% of the patients. There were no major complications during and after the procedure. Conclusions: The POEM technique was performed effectively and safely in our patients, presenting an improvement in the resolution of dysphagia in 100% of cases. A significant percentage presented reflux after the POEM procedure that remitted with the use of proton pump inhibitors.
Resumen Introducción: La acalasia es un trastorno esofágico motor caracterizado por falla en la peristalsis esofágica así como la relajación del esfínter esofágico inferior (EEI) en respuesta a la deglución (1). Es una patología rara, con una incidencia que va de 0.3-1.63 por cada 100,000 adultos/año y una prevalencia de 8.7-10.8 por cada 100,000 habitantes (2). El tratamiento de la acalasia no es curativo, pero el objetivo es reducir la resistencia al flujo causada por la falta de relajación en el EEI (3). En el 2010, Inoue et al describieron la técnica de la miotomía endoscópica por vía oral (POEM) realizada en 17 pacientes con excelentes resultados en cuanto a resolución de la disfagia (7). A 9 años de la descripción de la técnica de POEM para el tratamiento endoscópico, hoy en día en México, existen pocos centros que la realizan. Objetivo: Presentar la experiencia inicial de la técnica de Miotomía Endoscópica Por vía Oral para el tratamiento de acalasia en el Hospital General “Dr. Manuel Gea González”. Materiales y métodos: Se realizó una revisión de la base de datos del Servicio de Endoscopía Gastrointestinal del Hospital General “Dr. Manuel Gea González” en búsqueda de pacientes que se sometieron a procedimiento de POEM para el tratamiento de acalasia en los últimos 3 años. Se analizaron características demográficas de los pacientes, clasificación del tipo acalasia, escala de Eckardt previa y posterior al procedimiento, manometría previa y posterior al procedimiento, longitud de la miotomía, tiempo quirúrgico, días de estancia hospitalaria, phmetría previa y posterior al procedimiento y complicaciones. Resultados: Durante el período de 3 años se encontraron 11 pacientes sometidos a técnica de POEM para el tratamiento de Acalasia. Ocho hombres y 3 mujeres. La edad promedio fue de 63 años (48-83). El tipo de Acalasia predominante fue el tipo I. La media en escala de Eckardt previa al POEM fue de 8 (7-10) en contraste con la media en el score de Eckardt posterior al POEM fue de 1 (1-2). Tiempo quirúrgico medio fue de 2.6 horas (2-4) y la media de días de estancia hospitalaria fue de 4.4 días (4-8). La media de longitud de la miotomía fue de 8.6cm (7-10). La manometría posterior demostró una presión del esfínter esofágico inferior en reposo menor con respecto a la previa en el 100% de los casos, al contrario de la pHmetría que se encontró anormal posterior al POEM en 27% de los pacientes. No se presentaron complicaciones mayores durante y posterior al procedimiento. Conclusiones: La técnica de POEM se realizó de manera efectiva y segura en nuestros pacientes presentando una mejoría en cuanto a la resolución de la disfagia en el 100% de los casos. Un porcentaje importante presentó reflujo posterior al procedimiento de POEM que remitió con el manejo de inhibidores de bomba de protones.

Abstract Introduction: Achalasia is a primary motor disorder of the esophagus infrequent in the population, affects one in 100,000 people year and is characterized by the absence of esophageal peristalsis and incomplete relaxation of the lower esophageal sphincter in relation to swallowing or frequently in association with a sphincter lower esophageal hypertensive. The most common presentation symptoms are dysphagia, regurgitation of undigested food, weight loss, chest pain and respiratory symptoms such as night cough, aspiration and pneumonia.The treatment of achalasia has evolved in relation to the better understanding of the pathophysiology of the disease with tools such as high resolution manometry, as well as the development of new surgical techniques of minimally invasive surgery in addition to the recent emergence of third-space surgery. The development and diffusion of minimally invasive surgery techniques in the last 30 years has radically changed the algorithm of achalasia management. In 2010 the history of third-space surgery begins with the presentation of Inoue & colls with 17 consecutive cases of endoscopic myotomy per oral POEM for the treatment of achalasia. Objective: In the present work we present a historical review of the management of achalasia in the general hospital “Dr. Manuel Gea González “until now with the retrospective review of the results of the CMI surgery and the comparison with the results obtained so far with third space surgery. Materials and methods: We performed a retrospective review of all patients diagnosed with achalasia for 10 years (2007-2017), treated in the endoscopic surgery unit using CMI. The inclusion criteria were: confirmed diagnosis of achalasia and subsequent surgical management. The 1-month follow-up was the next inclusion criterion. The next group analyzed was 11 patients in a period of 3 years (2016-1019) with a diagnosis of achalasia and subsequently treated by third-space surgery (POEM). The comparative results are presented. Results: Of the 58 patients in the CMI group, the average age was 44, with an average of Eckardt of 7 being the most prevalent symptoms of dysphagia and regurgitation. Through the standard technique of Heller with antireflux procedure, an important improvement was achieved in the Eckardt scale of 2 (1-4). Complications occurred in 8.6% of cases. Reflux symptoms occurred in 10% of patients. In the POEM group, the average age of the patients was 63 years. The Eckardt scale improved from 8 to 1 after the procedure and there were no complications inherent to the procedure. Subcutaneous emphysema was the most common finding after the procedure. Conclusion: To date, with around 6,000 procedures performed worldwide, third-space surgery has become the least invasive method with results similar to those obtained with MIS (Minimally invasive surgery). Even with controversies to be defined in terms of technique, endoscopic myotomy is emerging to become the treatment of choice in achalasia.
Resumen Introducción: La acalasia es un trastorno motor primario del esófago infrecuente en la población, afecta a una de cada 100,000 personas año y se caracteriza por la ausencia de peristalsis esofágica y relajación incompleta del esfínter esofágico inferior en relación a la deglución o frecuentemente en asociación con un esfínter esofágico inferior hipertenso.1 Los síntomas de presentación más comunes son la disfagia, regurgitación de alimento no digerido, pérdida de peso, dolor torácico y síntomas respiratorios como tos nocturna, aspiración y neumonía. El tratamiento de la acalasia ha evolucionado en relación al mejor entendimiento de la fisiopatología de la enfermedad con herramientas como la manometría de alta resolución, así como al desarrollo de nuevas técnicas quirúrgicas de cirugía de mínima invasión además del recientemente surgimiento de la cirugía de tercer espacio2. El desarrollo y la difusión de técnicas de cirugía de mínima invasión en los últimos 30 años ha cambiado de forma radical el algoritmo de manejo de la acalasia. En 2010 comienza la historia de la cirugía de tercer espacio con la presentación de Inoue & colls con 17 casos consecutivos de miotomía endoscópica per oral POEM para el tratamiento de acalasia. Objetivo. En el presente trabajo presentamos una reseña histórica del manejo de la acalasia en el hospital general “Dr. Manuel Gea González” hasta la actualidad con la revisión retrospectiva de resultados de la cirugía CMI y la comparativa con los resultados obtenidos hasta el momento con la cirugía de tercer espacio. Materiales y métodos: Se realizó una revisión retrospectiva de todos los pacientes con diagnóstico de acalasia durante 10 años (2007-2017), tratados en la unidad de cirugía endoscópica mediante CMI. Los criterios de inclusión fueron: diagnóstico confirmado de acalasia y posterior manejo quirúrgico. El seguimiento a 1 mes fue el siguiente criterio de inclusión. El siguiente grupo analizado fue de 11 pacientes en un periodo de 3 años (2016-1019) con diagnóstico de acalasia y posteriormente tratados mediante cirugía de tercer espacio (POEM). Se presentan los resultados comparativos. Resultados: De los 58 pacientes del grupo de CMI el promedio de edad fue 44, con un promedio de Eckardt de 7 siendo los síntomas más prevalentes la disfagia y regurgitación. Mediante la técnica estándar de Heller con procedimiento antirreflujo se logro una mejoría importante en la escala de Eckardt de 2 (1-4). Se presentaron complicaciones en 8.6% de los casos. Los síntomas de reflujo se presentaron en 10% de los pacientes. En el grupo de POEM la edad promedio de los pacientes fue mayor 63 años. La escala de Eckardt mejoró de 8 a 1 posterior al procedimiento y no se presentaron complicaciones inherentes al procedimiento. El enfisema subcutáneo fue el hallazgo más común posterior al procedimiento. Conclusión.: Hasta la fecha con al rededor de 6,000 procedimientos realizados a nivel mundial la cirugía de tercer espacio se ha convertido en el método menos invasivo y con resultados que similares a los obtenidos con MIS(Minimally invasive surgery). Aun con controversias a definir en cuanto a la técnica, la miotomía endoscópica se perfila para convertirse el tratamiento de elección en acalasia.

Introduction: When pain associated with hallux valgus deformity is the main symptom and traditional treatment is not effective, then surgery is indicated. The aim of the study was to evaluate our experience with minimally invasive technique and scarf osteotomy using AOFAS scale and radiologically intermetatarsal angle correction, metatarsophalangeal angle, Regnauld scale and Reynolds criteria. Methods: Retrospective cohort observational study. Feet treated with minimally invasive technique and scarf osteotomy were included. Preoperative clinical data and those obtained at 3 and 6 months after surgery were reviewed to evaluate functionality, image findings on three angles, and Reynolds and Regnauld classifications. Results: A total of 123 patients were included, and 150 procedures were performed: minimally invasive surgery (44.7%) and scarf osteotomy (55.3%). Average age was 56 years (IQR 42-61), and 88.6% were women. No differences were observed at the immediate preoperative period, 3 months and 6 months for intermetatarsal angle between both techniques; however metatarsophalangeal angle was corrected (p<0.001) and AOFAS score (p<0.001) was better 3 months after the minimally invasive surgery. Conclusions: Both techniques provide correction to normal values. Minimally invasive technique achieved higher correction levels and a better AOFAS score. Incidence of acute complications was similar. Clinical studies on this issue are necessary to confirm these findings.
Introducción: Cuando el dolor asociado a deformidad en hallux valgus es el principal sintoma y el tratamiento conservador no es eficaz, esta indicada la cirugia. El objetivo de este articulo fue evaluar nuestra experiencia con la tecnica minimamente invasiva y la osteotomia tipo scarf, mediante la escala AOFAS, radiografias con la correccion del angulo intermetatarsiano, el angulo metatarsofalangico, la escala Regnauld y los criterios de Reynolds. Materiales y Métodos: Estudio observacional descriptivo de tipo cohorte retrospectiva. Se incluyeron los pies operados mediante las tecnicas minimamente invasiva y tipo scarf. Se revisaron los datos clinicos preoperatorios, y a los 3 y 6 meses de la cirugia, sobre funcionalidad, hallazgos imagenologicos de tres angulos, y las clasificaciones de Reynolds y Regnauld. Resultados: Se incluyeron 123 pacientes en quienes se realizaron 150 procedimientos: cirugia minimamente invasiva (44,7%) y tipo scarf (55,3%). La mediana de edad era de 56 anos (RIC 42-61), el 88,6% eran mujeres. No hubo diferencias entre el preoperatorio inmediato, y a los 3 y 6 meses para el angulo intermetatarsiano entre las dos tecnicas; sin embargo, el angulo metatarsofalangico se corrigio (p <0,001) y el puntaje AOFAS fue mejor a los 3 meses con la cirugia minimamente invasiva (p <0,001). Conclusiones: Ambas tecnicas corrigen la deformidad a valores normales. Con la tecnica minimamente invasiva, la correccion fue mayor y el puntaje AOFAS resulto mejor. La incidencia de complicaciones agudas fue similar. Se necesitan ensayos clinicos sobre este tema para confirmar los hallazgos de este estudio.

Objective To determine the prevalence of functional teeth (FT) and their distribution using clinical, demographic, and socioeconomic variables in a sample of Mexican elderly. Material and methods This cross-sectional study of 139 adults aged 60 years and older was conducted in two nursing homes and one club in Pachuca, Hidalgo, Mexico. The dependent variable was FT, and participants were operationally categorised as 0 (having less than 21 teeth) or 1 (having 21 or more teeth). Those categorised as 1 comprised the FT group. Sociodemographic, socioeconomic, and behavioural variables were collected using a questionnaire. Clinical variables were collected by a trained examiner using a standardised examination. Analysis was performed using non-parametric tests in Stata. Results The mean age was 79.06 ± 9.78 years. Most (69.1%) of the participants were women. The prevalence of FT was 10.1%. In the bivariate analysis, it was observed that the FT group was younger (p < .05), women displayed a greater prevalence of FT than men (p < .05), as did those involved in a relationship compared to being single (p < .05), and FT prevalence differed by type of isolation (p < .05). Conclusions The prevalence of FT is low among nursing home residents and club members, and it varies with age, sex, relationship status, and type of isolation.
Objetivo Determinar la prevalencia de dentición funcional (DF) y su distribución a través de diversas variables clínicas, sociodemográficas y socioeconómicas de una muestra de ancianos mexicanos. Material y métodos En un estudio transversal se incluyeron 139 adultos de 60 años y más de 2 asilos y un club de ancianos de la ciudad de Pachuca, Hidalgo, México. La variable dependiente fue la dentición funcional; operacionalmente categorizada como: 0 = sujetos con menos de 21 dientes presentes en la boca y 1 = sujetos con 21 o más dientes en la boca, excluyendo prótesis. Las variables sociodemográficas, socioeconómicas y conductuales se recogieron a través de un cuestionario. Las variables clínicas fueron recabadas por un examinador capacitado y estandarizado. El análisis se realizó utilizando pruebas no paramétricas en Stata. Resultados El promedio de edad fue de 79,06 ± 9,78. La mayoría de los sujetos incluidos fueron mujeres (69,1%). La prevalencia de dentición funcional fue de 10,1%. En el análisis bivariado observamos lo siguiente: los sujetos con DF tuvieron menor edad (p < 0,05); entre las mujeres se observó mayor prevalencia de DF (p < 0,05); las personas con pareja presentaron más alto porcentaje de DF (p < 0,05); la prevalencia de DF fue diferente a través de los tipos de asilamiento (p < 0,05). Conclusiones Los datos sugieren una baja prevalencia de dentición funcional en esta muestra de adultos mayores. Este estudio demuestra que la DF varía en función de variables sociodemográficas.

La colestasis es una alteración en el flujo biliar que se presenta por la disminución o el cese de excreción biliar. En la actualidad, son pocos los estudios en Colombia sobre esta patología. Se presentan 21 casos de colestasis neonatal en un hospital infantil de la ciudad de Cartagena (Colombia) entre 2010 y 2013, con el objetivo de caracterizar la etiología y clínica de la enfermedad. Se seleccionaron los pacientes entre 0 y 3 meses de edad con bilirrubina directa >2 mg/dL. En este estudio se encontró que según el género, el 52,4% fueron de sexo masculino y el 47,6%, de sexo femenino. La edad gestacional predominante fue a término en el 76,2% y sin antecedentes perinatales en el 57,1%. Los hallazgos clínicos se presentaron en los primeros 30 días de nacido en un 71% y 4 pacientes fueron remitidos a trasplante hepático. La etiología más frecuente fue de tipo infeccioso en 13 de los pacientes estudiados y 4 pacientes se relacionaron con atresias. La causa más frecuente de colestasis neonatal en este estudio resultó estar asociada con etiologías infecciosas. Sin embargo, las alteraciones obstructivas, como la atresia de vías biliares, siguen ocupando un renglón importante y requieren un estudio y manejo prioritario, dado su mejor pronóstico relacionado con la intervención temprana
Cholestasis is an alteration in the flow of bile resulting from decreases or cessation of biliary excretion. To date, there have been only a few studies on this topic in Colombia. This article presents twenty-one cases of neonatal cholestasis from a Children’s Hospital in Cartagena, Colombia that occurred between 2010 and 2013. The aim of this study is to characterize the etiology and clinical characteristics of the disease. Patients between birth and 3 months old with direct bilirubin levels over 2 mg/dl were selected. By gender, 52.4% of the patients were male, and 47.6% were female. 76.2% of the patients were full term, and 57.1% had no perinatal antecedents. Clinical symptoms presented in the first 30 days after birth in 71% of the patients, and 4 patients were referred for liver transplantation. The most common etiology was infectious (13 patients), and 4 patients had atresia. The most common cause of neonatal cholestasis in this study was infection, but obstructive disorders such as biliary atresia still account for an important proportion of the patients. They require priority study and handling because early intervention results in better prognoses
