The Contribution of Meteorological Parameters and the COVID-19 Partial Lockdown on Air Quality in Rio de Janeiro, Brazil

This study evaluated the pollutant levels (NO2, SO2, CO, and O3), air quality index (AQI) and the influence of meteorological variables and coronavirus disease (COVID-19) pandemic on the air quality in Rio de Janeiro. The data set used comprises periods before (March-April, 2019) and during pandemic (March-April, 2020). According to the AQI results, on most days, the air quality was ranked as “good”. Brazilian air quality standards for SO2, O3, and NO2 were not exceeded in any of the monitoring stations during partial lockdown, while CO exceeded in all periods in one site due to industrial emission. Comparing both periods, descriptive statistics for the meteorological parameters presented no differences, which suggests similar conditions. However, when evaluated week by week in 2020, weather conditions presented some differences that probably affected pollutant concentrations. The correlations between O3 and NO2 and some meteorological parameters indicate that variations in both favored ozone formation, since it is a photochemical process favored by temperature and solar radiation and that, in Rio de Janeiro, low NO2 concentrations lead to increased O3. The improvements on air quality during the partial lockdown may be attributed mainly to a reduction on emission sources rather than weather conditions.

ABSTRACT This cross-sectional seroepidemiological survey presents the seroprevalence of SARS-CoV-2 in a population living in 15 Long-Term Care Facilities (LTCFs), after two intra-institutional outbreaks of COVID-19 in the city of Botucatu, Sao Paulo State, Brazil. Residents were invited to participate in the serological survey performed in June and July 2020. Sociodemographic and clinical characterization of the participants as well as the LTCF profile were recorded. Blood samples were collected, processed and serum samples were tested using the rapid One Step COVID-19 immunochromatography test to detect IgM and IgG anti-SARS-CoV-2. Among 209 residents, the median of age was 81 years old, 135 (64.6%) were female and 171 (81.8%) self-referred as being white. An overall seroprevalence of 11.5% (95% CI: 7.5% – 16.6%) was found. The highest seroprevalences of 100% and 76.9% were observed in LTCFs that had experienced COVID-19 outbreaks. Most residents with positive immunochromatography tests (70.8%) referred previous contact with a confirmed COVID-19 case. Although there was a relatively low seroprevalence of COVID-19 in the total number of elderly people, this population is highly vulnerable and LTCFs are environments at higher risk for COVID-19 dissemination. A well-established test for COVID-19 policies, the adequate characterization of the level of interaction between residents and the healthcare provider team and the level of complexity of care are crucial to monitor and control the transmission of SARS-CoV-2 in these institutions.

Increased platelet distribution width and reduced IL-2 and IL-12 are associated with thrombocytopenia in Plasmodium vivax malaria

BACKGROUND Thrombocytopenia in malaria involves platelet destruction and consumption; however, the cellular response underlying this phenomenon has still not been elucidated. OBJECTIVE To find associations between platelet indices and unbalanced Th1/Th2/Th17 cytokines as a response to thrombocytopenia in Plasmodium vivax infected (Pv-MAL) patients. METHODS Platelet counts and quantification of Th1/Th2/Th17 cytokine levels were compared in 77 patients with uncomplicated P. vivax malaria and 37 healthy donors from the same area (endemic control group - ENCG). FINDINGS Thrombocytopenia was the main manifestation in 55 patients, but was not associated with parasitaemia. The Pv-MAL patients showed increases in the mean platelet volume (MPV), which may be consistent with larger or megaplatelets. Contrary to the findings regarding the endemic control group, MPV and platelet distribution width (PDW) did not show an inverse correlation, due the increase in the heterogeneity of platelet width. In addition, the Pv-MAL patients presented increased IL-1β and reduced IL-12p70 and IL-2 serum concentrations. Furthermore, the reduction of these cytokines was associated with PDW values. MAIN CONCLUSIONS Our data demonstrate that an increase in MPV and the association between reductions of IL-2 and IL-12 and PDW values may be an immune response to thrombocytopenia in uncomplicated P. vivax malaria.

Abstract Paracoccidioidomycosis is a systemic fungal disease occurring in Latin America that is associated with rural environments and agricultural activities. However, the incidence and prevalence of paracoccidiodomycosis is underestimated because of the lack of compulsory notification. If paracoccidiodomycosis is not diagnosed and treated early and adequately, the endemic fungal infection could result in serious sequelae. While the Paracoccidioides brasiliensis ( P. brasiliensis ) complex has been known to be the causal agent of paracoccidiodomycosis, a new species, Paracoccidioides lutzii ( P. lutzii ), has been reported in Rondônia, where the disease has reached epidemic levels, and in the Central West and Pará. Accurate diagnoses and availability of antigens that are reactive with the patients’ sera remain significant challenges. Therefore, the present guidelines aims to update the first Brazilian consensus on paracoccidioidomycosis by providing evidence-based recommendations for bedside patient management. This consensus summarizes etiological, ecoepidemiological, molecular epidemiological, and immunopathological data, with emphasis on clinical, microbiological, and serological diagnosis and management of clinical forms and sequelae, as well as in patients with comorbidities and immunosuppression. The consensus also includes discussion of outpatient treatments, severe disease forms, disease prevalence among special populations and resource-poor settings, a brief review of prevention and control measures, current challenges and recommendations.

RESUMO Introdução: A avaliação da utilização dos exames laboratoriais na atenção primária pode melhorar a qualidade do atendimento prestado à população. Objetivo: Avaliar a utilização de exames laboratoriais pelas unidades de atenção primária à saúde de Botucatu, São Paulo, Brasil. Métodos: Foram realizados grupos tutoriais para avaliação dos laudos de exames laboratoriais solicitados por 15 unidades básicas de saúde do município de Botucatu, no período de 12 meses. As informações obtidas foram avaliadas quanto a quantidade de exames solicitados por consulta médica e proporções de exames com resultado anormal. Resultados: A taxa de solicitação de exames laboratoriais nas consultas médicas encontra-se acima das recomendações do Ministério da Saúde, permitindo inferir que há muitas solicitações desnecessárias. O exame que se apresentou anormal com maior frequência foi hemoglobina glicada, indicando um possível controle inadequado dos pacientes diabéticos. Conclusão: Conhecer e monitorar a atenção laboratorial possibilita aos gestores de unidades de saúde maior efetividade e uso racional dos escassos recursos disponíveis.
ABSTRACT Introduction: The evaluation of the use of laboratory tests in primary care can improve the quality of care provided to the population. Objective: To evaluate the laboratory tests in primary health care. Methods: Tutorial groups were set to evaluate the reports of laboratory tests requested at 15 basic health units in the city of Botucatu during a 12-month period. The obtained information was evaluated as to the amount of tests requested per medical consultation and the proportions of tests with abnormal results. Results: The rate of laboratory tests requested in medical consultations is far above the recommended by the Ministry of Health, allowing us to infer that there are many unnecessary requests. The test that more often showed abnormal results was glycated hemoglobin, what indicates a possible inadequate management of diabetic patients. Conclusion: Knowing and monitoring laboratory attention enables health unit managers to enhance effectiveness and rational use of the scarce available resources.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

DIVERSIDAD Y COMPOSICIÓN FLORÍSTICA EN BOSQUES SUCESIONALES ANDINOS DE LA REGIÓN DEL RÍO PORCE, COLOMBIA

La sucesión secundaria en las regiones tropicales tiende a variar de una localidad a otra debido, entre muchos factores, al tipo e intensidad de la perturbación, la distancia al bosque original, la fauna existente, la topografía y el clima local, los cuales determinan la composición florística y la velocidad con la que la sucesión avanza. En esta investigación se estudió la dinámica del proceso sucesional en suelos dedicados previamente a la ganadería en el cañón del río Porce (Antioquia), Colombia. Se muestrearon en total 25 parcelas, incluyendo 9 (20 x 50 m) en bosques primarios y 16 (20 x 25 m) en bosques secundarios; para éstos últimos la edad fue estimada en estudios previos usando datación por radiocarbono (C14), fotografías aéreas y una imagen de satélite de alta resolución. Se calculó la diversidad y la composición florística en los bosques secundarios, y los resultados se compararon con los obtenidos en bosques primarios cercanos. Se registraron 220 y 197 especies en los bosques primarios y secundarios, respectivamente. Las dos coberturas presentaron composición florística diferente sugiriendo alta diversidad beta. Análisis multivariados mostraron un gradiente sucesional definido y una alta correlación entre la composición florística y las variables ambientales. No obstante, en el área de estudio se presenta un proceso sucesional degradado como consecuencia del uso previo de la tierra. El índice de valor de importancia indica que en los bosques primarios la especie más importante fue Anacardium excelsum (Anacardiaceae) y en los secundarios Vismia baccifera (Hyperiaceae). Además, este índice y la presencia y ausencia de las especies en cada fase de desarrollo, sugieren la existencia de un gradiente sucesional que se ajusta tanto a la teoría clásica clementsiana de relevo florístico, como a la de composición florística original. El ajuste de ciertas especies a dos de las teorías sucesionales existentes, sugieren que el proceso sucesional es diferente y de complejidad variable según las características ecofisiológicas de las especies involucradas y las del ecosistema que se está regenerando.
Some studies indicate that tropical secondary succession is to be affected by factors such as disturbances, distance from original forests, surface configuration, and local climate, determining not only the composition of species but also the time trend of the succession itself. This study seeks to understand the dynamics of successional processes in soils devoted to cattle ranching over various decades in the Porce region of Colombia. A set of 25 permanent plots was measured, including 9 plots (20 x 50 m) in primary forests and 16 (20 x 25m) in secondary forests. The age of secondary forests was estimated in previous studies, using radiocarbondating, aerial photographs and a high-resolution satellite image. Both diversity and composition of species in secondary forests were calculated, and the results were compared against those obtained from natural forests located in the same study area. In total, 220 and 197 species were identified in primary and secondary forests, respectively. The floristic composition for both vegetation types was significantly different, indicating a high Beta diversity. Multivariate analyses suggest the existence of a well-defined successional gradient, and a high correlation between composition of species and some environmental variables as well. However, the study area is characterized by a successional process highly degraded, which seems to be the obvious consequence of the previous land use. The importance value index indicates that Anacardium excelsum (Anacardiaceae) and Vismia baccifera (Hypericaceae) tend to be the dominant species in primary and secondary forests, respectively. The importance value index and the presence or absence of species show that the ecological behavior of various species of the successional gradient may be explained by both the relay and original floristic composition. This finding might be the result of ecophysiological strategies of the species and environmental attributes of the ecosystems, which are experiencing slow but natural recovery.
