RESUMO Objetivo Caracterizar a deglutição das crianças com síndrome congênita do Zika vírus e comparar com crianças típicas. Método Estudo de delineamento transversal, com 45 crianças diagnosticadas com síndrome congênita do Zika vírus e 45 típicas. A deglutição foi avaliada por meio de avaliações clínicas da alimentação através do Protocolo de Avaliação Clínica da Disfagia Pediátrica e dos parâmetros acústicos da deglutição (sonar Doppler). Resultados A idade média das crianças com síndrome congênita do vírus Zika foi de 26,69 ± 4,46 meses e o perímetro cefálico médio foi de 29,20 ± 1,98 cm. Disfagia orofaríngea de moderada a grave foi observada em 32(71,1%) das crianças com síndrome congênita do Zika vírus, ao contrário do grupo comparação que não revelou alterações na deglutição. Nas crianças com síndrome congênita do Zika vírus foram verificados lábios entreabertos 42(93,3%) e tônus alterado em língua 35(77,8%) e bochechas 34(75,6%). Nas crianças do grupo comparação apenas 6(13,3%) apresentaram fechamento labial insuficiente e 1(2,2%) postura de língua inadequada. Alterações durante a deglutição com líquido e alimento pastoso não foram observadas no grupo comparação. Durante a oferta do alimento ocorreram dificuldades no movimento de sorver 14(77,8%), na captação da colher 35(75%), resíduo em cavidade oral 38(86,4%) e sinais clínicos indicativos de penetração/aspiração laringotraqueal como tosse, engasgo e respiração com esforço. Não foram encontradas diferenças nos parâmetros acústicos da avaliação instrumental. Conclusão as crianças com síndrome congênita do Zika vírus têm alterações nas fases oral e faríngea da deglutição, com sinais clínicos indicativos de penetração/aspiração laringotraqueal quando comparadas a crianças típicas.

ABSTRACT Purpose To characterize swallowing in children with congenital Zika virus syndrome in comparison to typical children. Methods This cross-sectional study enrolled 45 children diagnosed with congenital Zika virus syndrome and 45 others with typical development. Swallowing was evaluated through clinical feeding evaluations Protocolo de Avaliação Clínica da Disfagia Pediátrica and using acoustic swallowing parameters (Doppler sonar). Results The mean age of children with congenital Zika virus syndrome was 26.69 ± 4.46 months and the mean head circumference was 29.20 ± 1.98 cm. Moderate/severe oropharyngeal dysphagia was found in 32(71.1%) of the children with congenital Zika virus syndrome. Significant differences were found between the groups on clinical evaluation: Children with congenital Zika virus syndrome presented insufficient lip closure 42(93.3%) and altered tonus of the tongue 35(77.8%) and cheeks 34(75.6%). In the children in the comparison group, only 6(13.3%) presented insufficient lip closure and 1(2.2%) had inadequate tongue posture. Changes during swallowing with liquid and spoonable food were not observed in the comparison group. When liquid/food was offered, affected children presented difficulties in sipping movements 14(77.8%) and lip/spoon contact 35(75%). The presence of residual food in the oral cavity after swallowing 38(86.4%) and clinical signs indicative of laryngotracheal penetration/aspiration, such as coughing, gagging and/or labored breathing, were also notable. No differences were found between the groups with regard to the acoustic parameters evaluated instrumentally. Conclusion Children with congenital Zika virus syndrome present alterations in the oral phase of swallowing, as well as clinical signs indicative of pharyngeal phase impairment.

ABSTRACT: This study evaluated forage-free diets supplemented with sodium bicarbonate (SB) based on ground flint corn on intake, apparent nutrient digestibility, ruminal parameters, and N balance. Four rumen-cannulatedDorper × Santa Inês lambs, with a body weight of 43.4 ± 0.85 kg (mean ± standard deviation) were used. The experimental design was a 4×4 Latin square. Each experimental period lasted 21 days. Experimental diets were: positive control, a diet without SB containing 400 g/kg DM of soybean hulls (CONT), and three diets based on ground flint corn, containing 10 (10SB), 20 (20SB) or 30 (30SB) g/kg DM of SB. Intake of neutral and acid detergent fiber was greater for CONT (P < 0.01) when compared to SB. Intake and apparent nutrient digestibility of dry and organic matter, ether extract, and crude protein did not differ. Intake (P = 0.01) and digestibility (P < 0.01) of non-fiber carbohydrates were greater for SB diets and neutral detergent fiber digestibility was greater (P = 0.03) for CONT. The total digestible nutrients and metabolizable energy were greater for SB (P = 0.04). Rumen concentrations of acetate (P < 0.01), isobutyrate (P < 0.01), butyrate (P < 0.01), isovalerate (P < 0.01), and total short-chain fatty acids (P < 0.01) were greater for CONT. Ruminal pH showed a quadratic (P = 0.05) response to SB levels, with greater values for 20SB. Sodium bicarbonate was efficient in increasing ruminal pH at the level of 20 g/kg DM without interfering with the intake and apparent nutrient digestibility characteristics.

RESUMO: O presente estudo avaliou a suplementação de bicarbonato de sódio (BS) em dietas sem forragem à base de milho moído sobre o consumo e a digestibilidade aparente dos nutrientes, os parâmetros ruminais e o balanço de N. Foram utilizados quatro cordeiros Dorper × Santa Inês, com um peso corporal inicial de 43,4 ± 0,85 kg (média ± desvio padrão). O delineamento experimental foi quadrado latino 4×4. Cada período experimental durou 21 dias. As dietas experimentais foram: controle positivo, sem BS, contendo 400 g/kg de MS de casca de soja (CONT) e três dietas à base de milho moído, sem casca de soja, contendo 10 (10BS), 20 (20BS) ou 30 (30BS) g/kg de MS de BS. O consumo de fibra em detergente neutro e ácido foi maior (P < 0,01)para o CONT quando comparado com BS. O consumo e a digestibilidade aparente da matéria seca, matéria orgânica, extrato etéreo e proteína bruta não diferiram entre os tratamentos. O consumo (P = 0,01) e digestibilidade (P < 0,01) dos carboidratos não fibrosos foram superiores para as dietas contendo BS e a digestibilidade da fibra em detergente neutro foi maior para o CONT. Os teores de nutrientes digestivos totais e energia metabolizável foram superiores para as dietas contendo BS (P = 0,04). A concentração ruminal de acetato (P < 0,01), isobutirato (P < 0,01), butirato (P < 0,01), isovalerato (P < 0,01) e ácidos graxos totais de cadeia curta (P < 0,01) foram mais elevadas para o CONT. O pHruminal apresentou resposta quadrática (P = 0,05) em relação aos níveis de BS, sendo observado maior pH para os animais alimentados com a dieta 20BS. O BS foi eficiente em aumentar o pHruminal no teor de 20 g/kg MS sem interferir no consumo e na digestibilidade aparente dos nutrientes. Portanto, 20 g/kg de MS é a dose indicada de BS em dietas sem forragem para terminação de cordeiros em confinamento.

The study presents CeO2 particles synthesized by the microwave-assisted hydrothermal method using different synthesis times for the analysis of sensitivity to carbon monoxide gas, aiming at its application as a sensor material to prevent poisoning caused by this highly toxic gas. Structural, morphological and spectroscopic and electrical behaviors were analyzed by X-ray diffraction, Rietveld refinement, transmission electronic microscopy, Raman scattering spectroscopy, optoelectronic characterization chamber and FT-IR. The samples presented fluorite-type cubic structures, increase in crystallinity and particle size with the variation synthesis time from 5 to 8 minutes. From the micrographs it was observed that the nanoparticles initially were spherical with a surface domain (200), and the synthesis time made them cubic/polyhedral with a surface (111), showing differences in defects and influencing in its properties. The sample synthesized at 8 minutes showed the best result with response and recovery time of 16 and 2 seconds, respectively, at 390 °C, therefore promising for the fabrication of CO gas selective sensing device.

SUMMARY BACKGROUND: Breast cancer is a public health problem with both high incidence and cure rates. After treatment, patients are monitored for long periods of time due to the risk of recurrence. Thus, staging and follow-up strategies should consider not only the best results for the patient but also its costs for the public health system. OBJECTIVE: The objective of this study was to quantify the waste of resources on breast cancer follow-up and evaluate its impact on the public health system. METHODS: This is a retrospective analysis of consecutive medical records to identify the intervals between consultations and tests used for staging and during the first 2 years of follow-up of patients with breast cancer treated at a public hospital in Brazil. Data were compared with the guidelines of the main international consensus. RESULTS: Medical records of 60 consecutive patients treated in 2018 were selected, of whom 52 had 2 or more years of follow-up, and 8 had only 1 year of complete follow-up. A total of 34 patients (56.67%) underwent excessive examinations for stating. During follow-up, 125 surplus consultations were performed (33.6%). In this phase, 111 surplus exams were also performed, representing an increase of 100.9%. A total of 423 laboratory tests were performed for 18 patients in the first year and 229 tests for 14 patients in the second year. CONCLUSION: Excessive tests and consultations significantly burdened the Unified Health System without any benefit to patients. Better adherence to staging and follow-up recommendations could reduce costs and optimize the limited resources used in the public health system.

Abstract Welding procedures for repairing corroded tubular structures are commonly used; however, composite material repairs are becoming popular since they do not use heat sources that might cause accidents. This paper provides a detailed description of the CFRP patch repair application in steel tubular members with a circular perforation originated from a prolonged corrosion process. The results of experimental tests on mid-scale unrepaired damaged and repaired steel tubes are reported. The predictions of the maximum loads recovered from a proposed numerical model lie close to the experimental observed values, justifying its usage for analysis of repaired tubular structures with circular cutouts when subjected to axial compression. Compared to unrepaired perforated tubes, the experimental results demonstrate a substantial strength recovery for repaired tubes. Abstract: This paper presents the first results of the archaeological investigations carried out in the interfluvium located between the middle courses of the Colorado and Negro rivers (Río Negro province, Argentina). This is an extensive arid plain with many temporary lagoons in which archaeological material is concentrated, mainly on the surface. Our analysis included distributional studies and, to a lesser extent, archaeological excavations, in order to understand the human strategies of space use and occupational intensity. The generated information is discussed in the light of archaeological expectations derived from the space use model proposed by Borrero and colleagues (2008). The results obtained indicate that the archaeological record located around these ephemeral lagoons is mainly the result of a planned and redundant use of space. Furthermore, some sectors of these lagoons were used more intensively and repeatedly, suggesting cases of specific redundancy. Although radiocarbon dates could not be obtained, the presence of pottery relates human occupations at least to the Late Holocene. The evidence discussed reveals the importance of this interfluvial space in the settlement and mobility strategies of the hunter-gatherer societies that inhabited two of the main northern Patagonian rivers.

Resumen: En este trabajo se presentan los resultados de las primeras investigaciones arqueológicas realizadas en el interfluvio ubicado entre los cursos medios de los ríos Colorado y Negro (Provincia de Río Negro, Argentina). En esta extensa planicie árida se encuentran lagunas temporarias en las que se concentra el material arqueológico, principalmente en superficie. Con el fin de comprender las estrategias humanas de uso del espacio y la intensidad de las ocupaciones se llevaron a cabo estudios distribucionales y, en menor medida, excavaciones arqueológicas. La información generada fue discutida a la luz de las expectativas arqueológicas derivadas del modelo de uso del espacio propuesto por Borrero y colaboradores (2008). Los resultados obtenidos indican que el registro arqueológico localizado en torno a estas lagunas efímeras es principalmente el efecto de un uso del espacio planificado y redundante. Asimismo, algunos sectores de estas lagunas fueron utilizados más intensa y repetidamente, sugiriendo casos de redundancia específica. Si bien no se cuenta con fechados radiocarbónicos, la presencia de cerámica permite vincular las ocupaciones humanas al menos al Holoceno Tardío. La evidencia discutida revela la importancia de este espacio interfluvial en los circuitos de asentamiento y movilidad de las sociedades cazadoras-recolectoras en torno a dos de los principales ríos norpatagónicos.

Resumo A retinite por citomegalovírus (CMV) é uma manifestação rara de doença invasiva por CMV e potencialmente ameaçadora para a visão em indivíduos imunocomprometidos. A suspeita clínica é fundamental, uma vez que se trata de uma entidade incomum, com uma instalação progressiva e frequentemente assintomática durante um longo período. Os autores relatam um homem de 70 anos de idade com doença renal do diabetes que foi submetido a um transplante renal (KT) em Agosto de 2014 com boa evolução clínica. Nenhuma infecção anterior por CMV ou episódios de rejeição aguda foram relatados. Cinco anos após o transplante, ele foi internado devido a uma acuidade visual reduzida do olho esquerdo com sete dias de evolução com hiperemia associada, sem exsudato. A avaliação oftalmológica foi compatível com a necrose aguda da retina e presumivelmente associada à infecção por CMV. Ele teve uma melhora progressiva após o início do ganciclovir. A retinite por CMV é uma das complicações oculares mais graves em indivíduos imunossuprimidos e pode levar à cegueira irreversível e, por isso, o diagnóstico e o tratamento precoces continuam sendo cruciais para obter o melhor prognóstico visual em pacientes afetados. A profilaxia secundária com ganciclovir não é consensual, tampouco a segurança de reintroduzir o antimetabólito nestes casos.

Abstract Cytomegalovirus (CMV) retinitis is a rare manifestation of CMV invasive disease and potentially threatening to vision in immunocompromised individuals. Clinical suspicion is fundamental since it is an unusual entity with a progressive and often asymptomatic installation over a long period. The authors report a 70-year-old man with diabetic nephropathy who underwent a kidney transplant (KT) in August 2014 with good clinical evolution. No previous CMV infection or episodes of acute rejection were reported. Five years after transplant, he was admitted due to a reduced visual acuity of the left eye with seven days of evolution with associated hyperemia, without exudate. The ophthalmologic evaluation was compatible with acute necrosis of the retina and presumed associated with CMV infection. He had a progressive improvement after ganciclovir initiation. CMV retinitis is one of the most serious ocular complications in immune-suppressed individuals and can lead to irreversible blindness, and because of that, early diagnosis and treatment remains crucial in obtaining the best visual prognosis in affected patients. Secondary prophylaxis with ganciclovir is not consensual, neither is the safety of reintroducing the antimetabolite in these cases.

Abstract Objective: To analyze patients’ pain perception requiring endodontic treatment referred to a Dental Specialties Center. Material and Methods: Data was collected through a self-administered questionnaire for patients about their experience of pain and another for endodontists about the treatment performed. The results were analyzed descriptively using Pearson’s Chi-square test and Fisher’s Exact test, with Bonferroni correction (p≤0.05). Results: The median age of the patients was 39 years, and 71.1% were female. The median waiting time for treatment was five months. Pain was reported by 75.2% of patients, occurred more than one month earlier (63.6%), with moderate/severe intensity (66.9%), and most patients sought emergency treatment more than once (79.1%). In addition, pain was associated with sex (female; p=0.008); moderate/severe intensity (p<0.001); the number of times that patient had to go to the dentist because of the tooth treatment (twice or more; p=0.002); and type of tooth treated (posterior tooth; p=0.002). Conclusion: Severe pain episodes resulted in a repeated search for emergency services, which may overload the primary care service, especially if the waiting time for endodontic treatment is long.

Abstract Among the 26 Hybosorinae genera, seven are present exclusively in the Neotropical region. However, Hybosorinae from the New World have been less studied. One new genus and new species collected in Brazil is herein described to this subfamily. The genus is recognizable by the following combination of characters: labium with semicircular mentum; mandibles with lateral projection excavated, separated in the middle by longitudinal carina, inner side sheltering the labrum; antennae with nine antenomeres; scutellar shield with strong punctures at the proximal half; elytra with single rows formed by punctures; and protibia without well-defined denticles.

ABSTRACT: Adding ionophores to ruminant diets is a strategy to manipulate ruminal fermentation and improve milk yield. This study evaluates the effects of narasin supply to lactating ewes on dry matter intake (DMI), milk yield and composition, and performance of the lambs. Thirty lactating Santa Inês and Santa Inês × Dorper ewes fed a basal diet containing 50 % coastcross hay and 50 % concentrate were randomly assigned to two treatments: control (CON; without ionophores) or NAR (addition of 13 mg narasin kg–1 DM). From the 2nd to 10th week of lactation, DMI of ewes was determined, and once a week, their milk production and composition was measured over a 3-h interval. At the 10th week of lactation, lambs were weaned and their average daily gain (ADG) and starter DMI continued to be evaluated for two more weeks. Narasin supply did not affect weight and DMI of ewes. Ewes fed NAR had greater feed efficiency for milk production and displayed tendency for higher milk yield. Narasin supply reduced milk protein levels, but it did not affect other milk component levels. Ewes fed NAR had greater production of milk urea nitrogen and showed tendency for higher production of fat and total solids. Starter DMI of lambs was not affected by treatments; however, there was a tendency for greater weaning weight for NAR lambs. At the end of experiment, no differences were observed in the performance of lambs. The supply of 13 mg narasin kg–1 to lactating ewes improved milk yield efficiency and tended to increase the weaning weight of their lambs.

Resumen La pandemia de la covid-19 ha tenido impactos inconmensurables en la economía, la educación y la socialización, además de la pérdida de millones de vidas. Por lo tanto, se ha acelerado el desarrollo de un número sin precedentes de candidatos a vacunas contra la covid-19 para controlar la pandemia. A su vez, la autorización para su uso de emergencia por parte de la Organización Mundial de la Salud permitió el inicio de la inmunización de la población a través de vacunas que aún se encuentran en ensayos clínicos. Aquí presentamos una perspectiva de los preceptos bioéticos de autonomía, no maleficencia, beneficencia y justicia en el contexto del uso de emergencia de vacunas contra la covid-19. Además, se enfatiza la importancia de la vigilancia en todas las etapas del desarrollo de la vacuna con el fin de detectar efectos adversos y asegurar el cumplimiento de los preceptos bioéticos.

Abstract The COVID-19 pandemic produced immeasurable impacts on the economy, education, and socialization, besides the loss of mi llions of lives. Thus, there has been an accelerated development of an unprecedented number of COVID-19 vaccine candidates to control the pandemic. The World Health Organization's emergency use authorization of COVID-19 vaccines still in clinical trial allowed immunizing the population. This paper presents a perspective of the bioethical precepts of autonomy, non-maleficence, beneficence, and justice in the emergency use of COVID-19 vaccines. Furthermore, it emphasizes the importance of surveillance at all stages of vaccine development to detect adverse effects and ensure compliance with bioethical precepts.

Resumo A pandemia ocasionada pela covid-19, além da perda de milhões de vidas, vem trazendo consequências incomensuráveis para a economia, a educação e a socialização. Portanto, vem sendo acelerado o desenvolvimento de um número sem precedentes de candidatos a vacinas contra a covid-19 para controlar a pandemia. Por sua vez, a autorização para seu uso emergencial por parte da Organização Mundial da Saúde permitiu o início da imunização da população por meio de vacinas que ainda se encontram em ensaios clínicos. Aqui, apresentamos uma perspectiva dos princípios bioéticos de autonomia, não maleficencia, beneficência e justiça no contexto do uso emergencial de vacinas contra covid-19. Além disso, é enfatizada a importância da vigilância em todas as etapas do desenvolvimento da vacinação a fim de detectar efeitos adversos e assegurar o cumprimento dos princípios bioéticos.

Resumen: Antecedentes: la nutrición durante el embarazo impacta en la salud del recién nacido, con efectos a nivel epigenético determinando consecuencias neurológicas a largo plazo. Las necesidades de hierro durante el embarazo se estiman en 27 mg/día. El hierro hemo que se absorbe mejor se encuentra en la carne. La determinación de ferritina en sangre de cordón umbilical permite evaluar los depósitos de hierro alcanzados durante la etapa fetal. Sus niveles se asociaron con efectos a largo plazo sobre el desarrollo infantil. Objetivos: el objetivo de este estudio de carácter exploratorio es determinar la relación entre el consumo de carnes rojas durante el tercer trimestre de gestación y el nivel de ferritina en el cordón umbilical. Métodos: se realizó un estudio observacional descriptivo con datos recolectados prospectivamente durante un año en el Departamento de Neonatología del Centro Hospitalario Pereira Rossell (CHPR) en Montevideo, Uruguay. Un total de 188 pacientes cumplieron los criterios de inclusión. Se extrajo sangre del cordón umbilical después de un pinzamiento estricto del cordón pasado un minuto de vida. La ferritina se midió utilizando el método de quimioinmunofluorescencia. Se aplicó una encuesta nutricional materna (cualitativo-cuantitativa) que midió la frecuencia de consumo de alimentos con fuente de hierro y las cantidades aproximadas consumidas durante el último trimestre del embarazo. Esta encuesta se centró en el consumo materno de carne vacuna como principal fuente de hierro hemínico en Uruguay. Se analizó la relación entre estas variables. Resultados: el déficit latente de hierro (ferritina en el cordón umbilical <100 ng/ml) se asoció con un menor consumo de carne vacuna durante el embarazo. Valor p de Fisher: 0,0133, OR: 3,71, IC del 95% (1,25-11,05). Conclusiones: este estudio considera adecuada la evidencia que relaciona que los niveles bajos de consumo total de hierro y de carne vacuna durante el tercer trimestre de gestación determinarán un mayor riesgo de déficit latente de hierro y de ferritina medido en el cordón umbilical. Los niveles descendidos de ferritina en cordón umbilical se asocian con un mayor riesgo de efectos adversos a largo plazo sobre la mielinización y el desarrollo neurocognitivo.

Summary: Background: nutrition during pregnancy impacts the foetus and the newborn health, it has consequences at the epigenetic level and determines long-term neurological consequences. Iron requirements during pregnancy are estimated at 27 mg/day. Iron is blood absorption from is most efficient from beef. Umbilical cord blood ferritin levels can be used to assess iron deposits reached during the foetal stage. Ferritin levels are linked to the child’s long-term development. Objective: this exploratory study’s objective is to determine the relationship between beef consumption during the first quarter of pregnancy and ferritin levels in the umbilical cord. Methods: we carried out a descriptive, observational study with prospectively collected data for one-year at the Neonatology Department of the Pereira Rossell Hospital Center (CHPR) in Montevideo, Uruguay. A total of 188 patients met the inclusion criteria. We extracted umbilical cord blood after a strict cord clamping after one minute of life. Ferritin was measured using the chemoimmunofluorescence method. We carried out a maternal nutritional survey using a qualitative-quantitative method and measured the frequency and approximate quantity of iron source food consumption during the last quarter of pregnancy. This survey was focused on maternal beef consumption as the major heme iron source in Uruguay. We analyzed the relationship between these variables. Results: latent iron deficiency (ferritin in the umbilical cord <100 ng / ml) was associated with lower beef consumption during pregnancy. Fisher p-value: 0.0133, OR: 3.71, 95% CI (1.25 - 11.05). Conclusions: this study agrees with the evidence that shows that low levels of total iron and beef consumption during pregnancy determine an increased risk of latent iron deficiency and lower levels of ferritin in newborns, and therefore, greater risk of long-term adverse effects on myelination and neurocognitive development.

Resumo: Contexto: a nutrição durante a gravidez tem impacto sobre a saúde do recém-nascido, com efeitos no nível epigenético, determinando consequências neurológicas a longo prazo. As necessidades de ferro durante a gravidez são estimadas em 27 mg / dia. O ferro heme de melhor absorção e aquele encontrado na carne vacuna. A determinação da ferritina no sangue do cordão umbilical permite avaliar os depósitos de ferro atingidos na fase fetal. Seus níveis foram associados a efeitos de longo prazo no desenvolvimento das crianças. Objetivos: o objetivo deste estudo exploratório é determinar a relação entre o consumo de carne vermelha durante o terceiro trimestre de gestação e o nível de ferritina no cordão umbilical. Métodos: foi realizado um estudo observacional descritivo com dados coletados prospectivamente durante um ano no Departamento de Neonatologia do Centro Hospitalar Pereira Rossell (CHPR) em Montevidéu, Uruguai. Um total de 188 pacientes cumpriram os critérios de inclusão. O sangue do cordão umbilical foi coletado após clampeamento estrito do cordão após um minuto de vida da criança. A ferritina foi medida pelo método de quimioimunofluorescência. Aplicamos um inquérito nutricional materno (qualitativo-quantitativo) que mediu a frequência de consumo de alimentos com fonte de ferro e as quantidades aproximadas consumidas durante o último trimestre da gravidez. Esta pesquisa enfocou o consumo materno de carne bovina como principal fonte de ferro heme no Uruguai. A relação entre essas variáveis foi analisada. Resultados: a deficiência latente de ferro (ferritina no cordão umbilical <100 mg / ml) foi associada ao menor consumo de carne bovina durante a gestação. Valor de p de Fisher: 0,0133, OR: 3,71, IC 95% (1,25-11,05). Conclusões: este estudo concorda com as evidências que relacionam que os baixos níveis de ferro total e consumo de carne bovina durante o terceiro trimestre de gestação determinam um maior risco de déficit de ferro latente e ferritina mensurado no cordão umbilical. A redução dos níveis de ferritina no cordão umbilical está associada a um risco aumentado de efeitos adversos de longo prazo na mielinização e no desenvolvimento neurocognitivo.

RESUMO INTRODUÇÃO: A rotulagem é uma ferramenta importante para os profissionais de alimentação e nutrição, pois fornece informações sobre segurança alimentar e nutrição. OBJETIVOS: Foi analisar as diferenças nas opiniões dos profissionais de alimentação e nutrição em relação à rotulagem nutricional em alimentos pré-embalados. METODOLOGIA: Um estudo transversal, com uma amostra não probabilística de profissionais, por contacto com diversas entidades portuguesas da alimentação e nutrição, foi realizado. Os critérios de elegibilidade incluíram ter idade igual ou superior a 18 anos, residir e trabalhar em Portugal e exercer uma atividade profissional na área. Foi desenvolvido um questionário disponível online de administração direta, incluindo questões sobre rotulagem (importância, leitura, uso, confiança, satisfação e influência nas escolhas alimentares). Foram analisadas as principais alterações nas regras de rotulagem da União Europeia sobre o conteúdo, apresentação e legibilidade das informações obrigatórias. Neste artigo serão apresentadas as respostas à pergunta aberta sobre sugestões para melhorar a rotulagem nutricional. RESULTADOS: Dos 297 participantes, 33 (11,1%) responderam à questão aberta, tendo sido estas posteriormente agrupadas em seis categorias. Estes profissionais demonstraram uma menor satisfação com a informação presente no rótulo, nomeadamente os termos técnicos específicos, a quantidade de informação, os símbolos utilizados e as alegações nutricionais. Estes apresentaram uma menor concordância com a apresentação e o conteúdo das informações nutricionais implementadas pelo Regulamento (UE) n.º 1169/2011. Foram propostas duas melhorias: a simplificação da informação na rotulagem e a compreensão da sua utilidade, tal como se apresenta atualmente, para fornecer informações percetíveis e úteis ao consumidor. CONCLUSÕES: Ferramentas educacionais de rotulagem nutricional devem ser desenvolvidas, a fim de promover a educação nutricional e o empoderamento do consumidor.

ABSTRACT INTRODUCTION: Labelling is an important tool for food and nutrition professionals since it provides information on food safety and nutrition. OBJECTIVES: The aim was to analyse the differences in food and nutrition professionals’ opinions regarding nutrition labelling on prepacked foods. METHODOLOGY: A cross-sectional study was conducted, using a non-probabilistic sample of professionals, by contacting several Portuguese entities in food and nutrition. The eligibility criteria included being 18 years old and over, living and working in Portugal, and having a professional activity in this area. An online self-administered survey was developed, including questions about labelling (importance, reading, use, trust, satisfaction, and influence in food choices). The main changes in the European Union labelling rules about the content, presentation and legibility of the mandatory information were analysed. The present paper focuses on the answers to the open-ended question on suggestions to improve nutrition labelling. RESULTS: From the 297 participants, 33 (11.1%) provided answers to the open-ended question, which were subsequently grouped into six categories. These professionals demonstrated the least satisfaction with the label information namely the specific technical terms, the quantity of information, the symbols used and the nutritional claims. They showed the least agreement with the presentation and content of the nutrition information, implemented by Regulation (EU) No 1169/2011. Two improvements were proposed: the simplification of the labelling information and the understanding of its usefulness as it is currently presented, to provide perceptible and useful information to the consumer. CONCLUSIONS: Nutrition labelling educational tools should be developed, in order to promote nutrition literacy and lead to consumer’s empowerment.

Resumo Introdução: A veia pequena safena (VPS) in situ pode ser usada como conduto em situações de revascularização do membro inferior em que as artérias alvo estão limitadas à porção distal da perna e nas quais a veia grande safena (VGS) está ausente ou é inadequada. Este cenário ocorre frequentemente em doentes diabéticos ou com doença renal crónica em estadio terminal, cujo padrão de oclusão arterial atinge os vasos tibioperoneais e poupa o segmento femoro-poplíteo. A técnica in situ oferece as potenciais vantagens de diminuir a manipulação e trauma da veia, melhor adaptação de calibre entre os vasos e melhor perfil hemodinâmico. A abordagem posterior simplifica o procedimento cirúrgico e oferece taxas de patência da pontagem e de salvamento do membro comparáveis aos procedimentos habituais. Caso clínico: Apresentamos o caso de um homem de 89 anos, diabético e com tentativas prévias, sem sucesso, de revascularização endovascular das artérias tibial posterior e peroneal; apresenta-se com uma úlcera do hállux do pé direito com evolução desfavorável. O mapeamento ultrassonográfico das veias revela as VGS varicosas em ambos os membros e uma VPS de trajeto linear e com ~3mm de diâmetros na perna direita. O paciente foi submetido a uma pontagem entre a artéria poplítea retrogenicular e artéria tibial posterior distal com VPS in situ, através de uma abordagem posterior. A pontagem trombosou ao sétimo dia pós-operatório; esta complicação foi resolvida com trombectomia cirúrgica, angioplastia da veia e laqueação das fístulas arteriovenosas patentes. No seguimento em ambulatório, a pontagem mantém-se patente e a úlcera do hállux a cicatrizar favoravelmente. Conclusão: A VPS in situ é uma opção segura e viável como conduto para pontagens entre a artéria poplítea e as artérias distais da perna. Os cirurgiões vasculares devem estar cientes da abordagem posterior, que simplifica e expões confortavelmente as estruturas anatómicas necessárias para este procedimento.

Abstract Introduction: In situ lesser saphenous vein (LSV) graft has been advocated in cases of lower limb revascularization where target arteries are confined to the lower leg and the greater saphenous vein (GSV) is neither available nor suitable. This often occurs in diabetic or end-stage renal diseased patients, whose occlusive disease pattern typically affects the tibioperoneal vessels, sparing the femoropopliteal segment. In situ technique offers the potential advantages of decreased surgical trauma to the vein, better size-matching and improved hemodynamics. The posterior approach simplifies the surgical procedure; it achieves similar graft patency and limb salvage rates compared to standard procedures. Case report: We report a case of an 89 years-old male diabetic patient with previous attempts of endovascular revascularization of the posterior tibial and peroneal arteries; he presents with a nonhealing ulcer of the first toe of the right foot. Ultrasonographic vein mapping revealed varicose GSV in both limbs and a linear, ~3mm diameter, LSV in the right leg. The patient underwent right limb retrogeniculate popliteal to distal posterior tibial artery bypass with in situ LSV through a posterior approach. Post-operative bypass thrombosis occurred after seven days; it was resolved with surgical thrombectomy, vein angioplasty and arteriovenous shunt ligation. During follow-up at the outpatient clinic, the bypass remains patent and skin lesions healing without complications. Conclusion: In situ LSV is a safe and viable option conduit for popliteal to distal arteries bypasses. Vascular surgeons should be aware of the posterior approach, which simplifies and comfortably exposes the anatomic structures required for this surgery.

Resumo Introdução: O desenvolvimento de aneurismas em artérias ilíacas excluídas do fluxo de alta-pressão direto da aorta é muito incomum. Reportamos um caso de tratamento eficaz de aneurismas ilíacos metácronos (AIM) bilaterais, diagnosticados 10 anos após cirurgia de bypass aorto-bifemoral, apenas com perfusão retrógrada pelas artérias femorais comuns. Caso Clínico: Um homem de 76 anos foi encaminhado para avaliação após um achado incidental de AIMs bilaterais, 10 anos após uma cirurgia de bypass aorto-bifemoral para tratamento de um aneurisma da aorta abdominal (AAA). O angioTAC revelou um aneurisma de 43-mm de diâmetro da artéria ilíaca interna (AII) direita e um aneurisma de 45-mm de diâmetro da artéria ilíaca comum (AIC) esquerda, que não estavam presentes 10 anos antes. A exclusão do aneurisma da AIC esquerda foi conseguida libertando dois stents recobertos desde a AII esquerda até à artéria ilíaca externa (AIE) de acordo com a técnica da “banana”. O aneurisma da AII direita foi excluído com embolização distal dos ramos da ilíaca interna e do saco aneurismático com coils e oclusão da AIE proximal com um oclusor. Não ocorreram complicações no pós-operatório. O angioTAC dois meses após a cirurgia mostrou total exclusão do aneurisma da AIC esquerda e normal preenchimento da AII esquerda, assim como oclusão completa do aneurisma da AII direita. Conclusão: Este caso realça que a degenerescência aneurismática das artérias ilíacas pode ocorrer mesmo quando estas são perfundidas apenas por via retrógrada após tratamento de AAAs com bypass aorto-bifemoral. Uma abordagem endovascular com a utilização da técnica da “banana” mostrou-se eficaz no tratamento dos AIMs, com preservação do fluxo de uma AII.

Abstract Introduction: The development of aneurysms in iliac arteries excluded from high-pressure direct flow from the aorta is very uncommon. We report a case of successful treatment of bilateral metachronous iliac artery aneurysms (MIA), found 10 years after an aortobifemoral bypass, with perfusion only by retrograde flow from the common femoral arteries (CFA). Case Report: A 76-year-old man came for consultation after incidental finding of bilateral MIAs, 10-years after aortobifemoral bypass for treatment of an aortic abdominal aneurysm (AAA). Computed tomography angiography (CTA) showed a right internal iliac artery (IIA) aneurysm of 43-mm diameter and a left common iliac artery (CIA) aneurysm of 45-mm diameter that were not present 10-years before. Exclusion of the left CIA aneurysm was achieved with a left IIA to external iliac artery (EIA) endograft using two covered stents (“banana” technique). The right IIA aneurysm was excluded with embolization of the distal internal iliac branches and aneurysm sac with coils and occlusion of the proximal EIA with an occluder. There were no postoperative complications. A CTA two months after surgery showed total exclusion of the left CIA aneurysm and normal filling of the left IIA, as well as complete occlusion of the right IIA aneurysm. Conclusion: This case highlights that aneurysmal degeneration of iliac arteries can be generated by retrograde blood flow after treatment of AAAs with aortobifemoral grafts. An endovascular approach using the “banana” technique proved to be effective in treating MIAs while maintaining patency of one IIA.
