Suspected adverse drug reactions reported for Brazilian children: cross-sectional study,

RESUMO Objetivo: Analisar relatos espontâneos de suspeitas de Reação Adversa a Medicamento (RAM) em crianças de 0 a 12 anos notificadas pela Agência Nacional de Vigilância Sanitária entre 2008 e 2013. Métodos: Um estudo transversal a partir de notificações de suspeitas de RAM relacionadas a medicamentos e produtos para a saúde em crianças foi realizado por um período de seis anos (2008-2013). O ano da notificação, a origem do relato por estado brasileiro, sexo, idade, o medicamento suspeito, a descrição da reação adversa e a gravidade foram incluídos na análise, bem como o número de leitos nos serviços de saúde e dados global da VigiBase. Resultados: Um total de 3330 reações adversas foram relatadas em crianças no Brasil no período investigado (54% em meninos). Cerca de 28% dos relatos de suspeitas de RAM envolveram crianças de 0 a 1 ano de idade. Quase 40% dos relatos vieram da região Sudeste. Aproximadamente 60% foram classificados como eventos graves. Houve ocorrência de morte em 75 casos. Quase 30% das mortes envolveram o uso off-label dos medicamentos. Um total de 3875 medicamentos (465 substâncias ativas) foram considerados fármacos suspeitos. Medicamentos anti-infecciosos (vancomicina, ceftriaxona, oxacilina e anfotericina), com ação no sistema nervoso (dipirona) e no trato digestivo foram os mais frequentemente notificados. Conclusões: As notificações de suspeitas de RAM por sexo e faixa etária corresponderam ao perfil de crianças hospitalizadas no Brasil. Os dados sobre gravidade e medicamentos relatados podem ser úteis para encorajar ações reguladoras e melhorar o uso seguro de medicamentos em crianças.
ABSTRACT Objective: To assess spontaneous reports of suspected adverse drug reactions in children aged 0-12 years from the Brazilian Health Regulatory Agency between 2008 and 2013. Methods: A cross-sectional study on suspected adverse drug reactions reports related to medicines and health products in children was carried out for a six-year period (2008-2013). Year of report, origin of report by Brazilian state, gender, age, suspected drug, adverse reaction description and seriousness were included in the analysis. The data obtained was compared to the number of pediatric beds in health services and to global data from the VigiBase (World Health Organization). Results: A total of 3330 adverse drug reactions were reported in children in Brazil in the investigated period (54% were in boys). About 28% of suspected adverse drug reactions reports involved 0 to 1-year-old children. Almost 40% of reports came from the Southeast region. Approximately 60% were classified as serious events. There was death in 75 cases. Nearly 30% of deaths involved off-label use; 3875 medicines (465 active substances) were considered suspected drugs. Anti-infective (vancomycin, ceftriaxone, oxacillin, and amphotericin), nervous system (metamizole) and alimentary tract and metabolism medicines were more frequent in reports. Conclusions: The distribution of suspected adverse drug reactions reports by sex and age group corresponded to the profile of children hospitalized in Brazil. Data about seriousness and medicines reported may be useful to encourage regulatory actions and improve the safe use of medicines in children.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Composição mineral de folhas de Oliveira (Olea europaea L.) em sistema de produção convencional

Objetivou-se com este trabalho quantificar a composição mineral das folhas de Oliveira a fim de diagnosticar o estado nutricional das plantas cultivadas em sistema convencional. O experimento foi conduzido com sete cultivares Arbequina, MGS Asc 315, MGS Mariense, MGS Grap 541, MGS Grap 561, Grappolo 575 e Leccino, em blocos casualizados com 4 repetições e parcelas com cinco plantas. Em maio de 2010 foram coletadas 60 folhas de cada cultivar (12 por planta) e submetidas a analise foliar. As cultivares apresentaram diferenças significativas entre si quanto aos teores foliares médios de nutrientes. Observou-se também que apesar da analise de solo apontar níveis elevados de Fe e Mn, a análise foliar acusou deficiência para esses elementos em todas as cultivares de Oliveira avaliadas. Os resultados alertam para os altos níveis de Cu, como consequência do uso abusivo de fungicidas a base de oxicloreto e calda bordalesa nos pomares. A concentração elevada desse elemento nas folhas pode causar fitotoxidez às plantas. Desta forma, sugere-se o uso racional de fertilizantes e de produtos fitossanitários no cultivo da oliveira a fim de evitar fitotoxidez ou deficiências nutricionais.
This study aimed to quantify the mineral composition of olive leaves to diagnose nutritional status of plants grown in conventional system. The experiment was conducted with seven cultivars: Arbequina, MGS Asc 315, MGS Mariense, MGS Grap 541, MGS Grap 561, Grappolo 575 and Leccino, in a randomized block design with four replications and five plants perplot. In May 2010, we collected 60 leaves for each cultivar (12 per plant),whichwere dried and subsequently analyzed. The results advert to the high levels of Cu, a consequence of the excessive use of fungicides based on copper oxychloride and Bordeaux mixture in the orchards. The high concentration of this element in the leaves can bephytotoxicity to plants and harmful to human health, since the leaves are used popularly as an aid in procedures for weight loss. Thus, we suggest the rational use of fertilizers and pesticides in the cultivation of olive trees to prevent nutritional deficiency or phytotoxicity and, if the research acknowledges the phytotherapic effect of leaves, the adoption of crops in the differentiated system of production, to obtain raw material with good quality and suitable for consumption in natura.

Avaliação do extrato etanólico de Ottonia martiana Miq. para o controle de duas doenças florestais

Metabólitos secundários presentes em plantas medicinais apresentam várias propriedades biológicas incluindo a atividade antifúngica. Esse estudo avaliou o potencial antifúngico da planta medicinal Ottonia martiana no controle da pinta-preta em erva-mate (Ilex paraguariensis) e do mofo-cinzento em eucalipto (Eucalyptus dunnii). Extrato etanólico (EBEtOH) dos órgãos totais (raízes, caules, folhas e frutos) foi preparado e testado na concentração de 1000 μg mL-1 contra os patógenos Cylindrocladium spathulatum (pinta-preta) e Botrytis cinerea (mofo-cinzento). Bioensaios in vitro (germinação de esporos e bioautografia direta) e in vivo (teste de patogenicidade em mudas) mostraram que o EBEtOH reduziu o crescimento micelial dos patógenos testados e a germinação dos esporos de C. spathulatum e estimulou a germinação de esporos de B. cinerea. O teste de patogenicidade mostrou que o controle da pinta-preta em erva-mate e do mofo cinzento em eucalipto não é viável usando-se a concentração testada de EBEtOH de O. martiana. Na bioautografia direta, foram detectadas zonas de inibição de crescimento micelial dos fungos e que foram relacionadas com a presença de piperovatina.
Secondary metabolites from medicinal plants have several biological properties, including antifungal activity. This study evaluated the antifungal potential of the medicinal plant Ottonia martiana to control maté leaf spot (Ilex paraguariensis) and eucalypt gray mould (Eucalyptus dunnii). Ethanol extract (EBEtOH) of the total parts (roots, stems, leaves and fruits) was prepared at the concentration of 1000 μg mL-1 and tested against Cylindrocladium spathulatum (maté leaf spot) and Botrytis cinerea (eucalypt gray mould). In vitro bioassays (spore germination and direct bioautography) and in vivo bioassays (pathogenicity test in seedlings) showed that EBEtOH reduced the mycelial growth of the tested pathogens and the germination of C. spathulatum spores and stimulated the germination of B. cinerea spores. The pathogenicity test showed that the control of maté leaf spot and eucalypt gray mould is not viable using the tested concentration of O. martiana EBEtOH. Zones of mycelial growth inhibition were detected in direct bioautography and were related to the presence of piperovatine.

Pneumonectomy is associated with high rates of morbimortality, with postpneumonectomy pulmonary edema being one of the leading causes. An intrinsic inflammatory process following the operation has been considered in its physiopathology. The use of corticosteroids is related to prevention of this edema, but no experimental data are available to support this hypothesis. We evaluated the effect of methylprednisolone on the remaining lungs of rats submitted to left pneumonectomy concerning edema and inflammatory markers. Forty male Wistar rats weighing 300 g underwent left pneumonectomy and were randomized to receive corticosteroids or not. Methylprednisolone at a dose of 10 mg/kg was given before the surgery. After recovery, the animals were sacrificed at 48 and 72 h, when the pO2/FiO2 ratio was determined. Right lung perivascular edema was measured by the index between perivascular and vascular area and neutrophil density by manual count. Tissue expression of vascular endothelial growth factor (VEGF) and transforming growth factor-beta (TGF-β) were evaluated by immunohistochemistry light microscopy. There was perivascular edema formation after 72 h in both groups (P = 0.0031). No difference was observed between operated animals that received corticosteroids and those that did not concerning the pO2/FiO2 ratio, neutrophil density or TGF-β expression. The tissue expression of VEGF was elevated in the animals that received methylprednisolone both 48 and 72 h after surgery (P = 0.0243). Methylprednisolone was unable to enhance gas exchange and avoid an inflammatory infiltrate and TGF-β expression also showed that the inflammatory process was not correlated with pulmonary edema formation. However, the overexpression of VEGF in this group showed that methylprednisolone is related to this elevation.

Investigaram-se os eventos morfológicos relacionados ao desenvolvimento pré-natal de ovários de 81 embriões e fetos da raça Nelore, coletados em frigoríficos, com idades variando de 26 a 240 dias pós-fecundação. Observou-se formação da crista gonádica e presença de células germinativas em seu interior aos 29 e 34 dias, respectivamente. As oogônias e folículos primordiais, ao contrário dos folículos em crescimento, apresentaram diferenças significativas de diâmetro nos vários períodos estudados. Verificou-se correlação positiva (P<0,05) entre o diâmetro das oogônias e de seus núcleos, bem como entre o diâmetro dos folículos primordiais e em crescimento com seus oócitos e respectivos núcleos. A gônada apresentou-se completamente formada aos 40 dias. Folículos primordiais, em crescimento e antrais surgiram, aproximadamente, aos 95, 140 e 180 dias, respectivamente. Na raça Nelore, a oogênese tem início e duração semelhante à de raças taurinas, mas apresenta como peculiaridade a antecipação do surgimento da foliculogênese.
The morphologic events related to the prenatal development of the ovaries in 81 Nelore breed embryos and fetuses gathered in a local slaughterhouse, with age range from 26 to 240 days following fecundation were studied. The age of fetuses was estimated from measures taken in the cranium-caudal direction. The sex was identified from macroscopic observations and using Polymerase Chain Reaction (PCR) technique. For histology the gonads were fixed in Bouin’s fluid for 24 hours and 5 µm thick section’s were stained with hematoxylin-eosin. Formation of gonadal ridge and presence of germinal cells were found within it at 29 and 34 days, respectively. Oogonia and primordial follicles, unlike the growing follicles, exhibited significant differences in diameter in the various periods studied. Positive correlation (P<0.05) was found between the diameter of oogonia and their nucleus as well as between primordial and growing follicles with their oocytes and respective nuclei. The gonad was fully formed at 40 days. Primordial follicles, in the growing stage, and antral follicles first appeared, approximately at 95, 140, and 180 days, respectively. Despite the onset and duration of oogenesis being similar to that of taurine breeds, folliculogenesis initiates at an early stages in the Nelore breed.

Caracterização física e tecnológica de seis cultivares de soja plantadas no Brasil

O presente trabalho teve como objetivo a avaliação física e tecnológica de seis cultivares de soja dentre as mais plantadas no Brasil: IAS-5, IGUAÇU, DAVIS, BR-16, IAS-4 e EMBRAPA-4, visando identificar a mais adequada para utilização na alimentação humana. Foram avaliados os seguintes parâmetros: densidade aparente, volume do grão, peso de 100 sementes, percentagem das partes do grão, cor da casca e do hilo, tempo de maceração, quantidade de água absorvida e tempo de cozimento. A densidade aparente variou de 1,1592 a 1,1992 g/cm³, o volume do grão de 0,1262 a 0,1711 cm³, o peso de 100 sementes de 15,31 a 19,76 g, as porcentagens de casca, hipocótilo e cotilédone de 6,94 a 8,40%, 2,29 a 3,05% e 88,77 a 90,77%, respectivamente. Nas condições do experimento constatou-se que a cor da casca e do hilo das seis cultivares foram muito semelhantes. O tempo mínimo necessário de maceração variou entre 6 e 9 horas, e o tempo de absorção máxima, de 12 a 15 horas. A absorção de água no tempo mínimo variou entre 108,05 e 124,60 g H2O/100 g de soja, e no tempo de absorção máxima de 125,67 a 132,54 g H2O/100 g de soja. Para o tempo de cozimento, a variação foi de 155 a 219 minutos.
In the present work, six soybean cultivars commonly grown in Brazil (IAS-5, IGUAÇU, DAVIS, BR-16, IAS-4 and EMBRAPA-4) were physically and technologically evaluated, viith the aim of identifying the most adequate one for human consumption. The following determinations were carried out: grain apparent density and volume, weight of one hundred seeds, percentage of hull, cotyledon and germ, seed coat and hilum colour, soaking time, water absorption and finally, cooking time. The apparent density varied from 1.1592 to 1.1992 g/cm³ and the grain volume from 0.1262 to 0.1711 cm³, the weight of one hundred seeds from 15.31 to 19.76 ghull, germ and cotyledon percentages from 6.94 to 8.40%,2.29 to 3.05% and 88.77 to 90.77%, respectively. For the conditions used, it was observed that the seed coat and the hilum colours were very similar for all six cultivars. The minimum soaking time varied from 6 to 9 hours and the maximum soaking time varied from 12 to 15 hours. The water absorption at the minimum soaking time varied from 108.05 to 124.60 g H2O/100 g and the water absorption at the maximum soaking time varied from 125.67 to 132.54 g H2O/100 g. The cooking time varied from 155 to 219 minutes.
