ABSTRACT Purpose To investigate if the narrative score of the ProNOH protocol allows for discriminating age groups, as well as its relation with the global coherence level of the story. The performance of preschool and schoolchildren on the macrostructure aspects. Methods Participants were 97 preschoolers and schoolchildren with typical language development, aged between five and 12 years old, and both sexes who attended public schools. The “Protocolo de Avaliação da Narrativa Oral de História (ProNOH)” (Protocol for the Evaluation of Oral Storytelling) was applied and the narrative score in the macrostructure dimension was calculated with story grammar elements. These same story elements were used to obtain the global coherence level of the story, as proposed by Spinillo and Martins (1997). Results A statistically significant difference was found between age groups, mainly between the borderline ages of 5-6 years, 7-8 years, 9-10 years, and 11-12 years. Positive and statistically significant correlations were found between the narrative score and global coherence and age, as well as between the narrative score and global coherence. Conclusion The protocol proved to be useful for identifying the repertoire of typical story grammar elements as an objective measure that differs in oral narrative across age groups. The results also indicate that the narrative score can provide an idea about the global coherence of the story, although this value does not replace a specific analysis.

RESUMO Objetivo Investigar se o escore narrativo obtido por meio da aplicação do “Protocolo de Avaliação da Narrativa Oral de História (ProNOH)” permite discriminar os grupos etários estudados, bem como sua relação com o nível de coerência global da história. Método Participaram 97 pré-escolares e escolares com desenvolvimento típico de linguagem, entre 5 e 12 anos, ambos os sexos que frequentavam escola pública. Foi aplicado o “Protocolo de Avaliação da Narrativa Oral de História (ProNOH)” e calculado o escore narrativo na dimensão macroestrutural a partir dos elementos estruturais previstos no modelo da gramática de história. Esses mesmos elementos foram usados para obter o nível de coerência global da história, segundo proposta de análise de Spinillo e Martins (1997). Resultados Diferença estatisticamente significante foi encontrada entre os grupos etários, principalmente entre as idades fronteiriças de 5-6 anos, 7-8 anos, 9-10 anos e 11-12 anos. Correlação positiva e estatisticamente significante foi encontrada com a idade e o escore narrativo e a coerência global, bem como entre o escore narrativo e o nível de coerência global. Conclusão O protocolo mostrou ser útil para identificar o repertório de elementos típicos da gramática de história na narrativa oral como uma medida objetiva que diferenciou a narrativa oral de grupos etários. Os resultados indicaram ainda que o escore narrativo pode ser uma medida norteadora do nível de coerência da história, apesar deste valor não substituir uma análise específica.

ABSTRACT Objectives: to analyze the implications of precarious work for the organization of work and for the health of nursing professionals in a surgical center. Methods: qualitative, descriptive study in which the interview technique was applied on 30 nursing professionals from a surgical center in a university hospital located in the Southeast region of Brazil. The project was approved by the research ethics committee. Thematic content analysis was applied in the categorization of speeches. Results: precarious work in the surgical center negatively affects the organization of work due to staff turnover, loss of skilled talent, and the need for continuous training of temporary workers. It also affects the quality of care, leading to risks to patient safety and workers’ health.Final Considerations: it is important to make work conditions less precarious in order to minimize staff turnover and promote the quality of the service offered and the health of the worker.

RESUMEN Objetivos: analizar las implicaciones del trabajo precario para la organización laboral y la salud de los profesionales de enfermería en un centro quirúrgico. Métodos: se trata de un estudio cualitativo, descriptivo, llevado a cabo mediante entrevista con 30 profesionales de enfermería en un centro quirúrgico de un municipio de la región sureste de Brasil. El proyecto fue aprobado por el comité de ética e investigación. En la categorización de las declaraciones, se aplicó el análisis de contenido temático. Resultados: la precarización en centros quirúrgicos interfiere negativamente en la organización del trabajo debido a la rotación de personal, la fuga de capital intelectual y la necesidad de capacitación continua de los trabajadores temporarios. Existe una interferencia en la calidad de la atención con riesgos para la seguridad de los pacientes y la salud de los trabajadores. Consideraciones Finales: se ratifica la importancia de apuntalar el trabajo con el intuito de minimizar la rotación laboral y promover la calidad del servicio brindado y la salud del trabajador.

Métodos: estudo qualitativo, descritivo no qual se utilizou a técnica de entrevista com 30 profissionais de enfermagem de centro cirúrgico em um hospital universitário situado em um município da região sudeste, Brasil. Projeto aprovado por comitê de ética e pesquisa. Na categorização dos depoimentos aplicou-se a análise de conteúdo temática. Resultados: a precarização em centro cirúrgico interfere negativamente na organização do trabalho devido a rotatividade de pessoal, a fuga de capital intelectual e a necessidade de treinamento contínuo dos trabalhadores temporários. Há interferência na qualidade da assistência com riscos para a segurança dos pacientes e a saúde dos trabalhadores. Considerações Finais: ratifica-se a relevância da desprecarização do trabalho no intuito de minimizar a rotatividade dos profissionais, promover a qualidade do serviço ofertado e a saúde do trabalhador.

Resumen Objetivo: Comparar la persistencia, tasa de retención y pauta de prescripción de infliximab original e infliximab CT-P13 en pacientes naive a biológicos con colitis ulcerosa. Método: Estudio ambispectivo de pacientes naive a biológicos en colitis ulcerosa que recibieron tratamiento en primera línea con Remicade® (infliximab) y Remsima® (infliximab CT-P13) de forma no simultánea durante un periodo de estudio de 10 años (2012-2021). Se tomaron datos de su edad, peso, persistencia, tasa de retención y si precisó de intensificación o desintensificación a lo largo del periodo de estudio. Se determinó el coste paciente/año real de Remicade® y Remsima® de forma individualizada en función de las administraciones durante el periodo del estudio. Resultados: Un total de 27 pacientes naive a biológicos fueron tratados con Remicade® y 53 con Remsima®. Ambos grupos de pacientes no presentaron diferencias en cuanto al peso y edad. La persistencia (mediana ± rango intercuartílico) con Remicade® fue de 42,49 ± 57,48 meses frente a 27,50 ± 58,50 meses para Remsima®, sin demostrar diferencias significati vas (p = 0,455). La tasa de retención a los 6, 12 y 24 meses fue del 81%, 63% y 33%, respectivamente, para el grupo de Remicade®, y del 71%, 47% y 37%, respectivamente, para el grupo de Remsima®. En el grupo de pacientes tratados con Remicade®, 9 pacientes fueron intensificados frente a 11 pacientes en el grupo de Remsima®. En cuanto a las desintensificaciones, 5 pacientes que recibieron tratamiento con Remicade® fueron desintensificados frente a 7 pacientes en tratamiento con Remsima®. El ahorro obtenido con el uso de Remsima® fue de 203.649 €, que equivaldría a tratar a 118 pacientes adicionales con infliximab biosimilar durante un año. Conclusiones: No existen diferencias significativas en la persistencia, tasa de retención y número de intensificaciones y desintensificaciones entre los pacientes naive que fueron tratados con Remicade® y aquellos tratados con Remsima®, siendo una alternativa eficaz, segura y económica en el tratamiento biológico de la colitis ulcerosa.

Abstract Objective: To compare the persistence, retention rate and prescription pattern of original infliximab and infliximab CT-P13 in biologic-naïve patients with ulcerative colitis. Method: This was an ambispective study of biologic-naive patients with ulcerative colitis who received non-simultaneous first-line treatment with Remicade ® (infliximab) and Remsima® (infliximab CT-P13) over a 10-year study period (2012-2021). Data on their age, weight, persistence, retention rate and on whether they required intensification or deintensification throughout the study period was collected. The real patient/year cost of Remicade® and Remsima® was determined individually based on the amounts administered during the study period. Results: 27 biologic-naive patients were treated with Remicade® and 53 with Remsima®. Neither patient group presented with differences in terms of weight and age. Persistence (median ± interquartile range) with Remicade® was 42.49 ± 57.48 months, as compared to 27.50 ± 58.50 months for Remsima®, without significant differences (p = 0.455). The retention rate at 6, 12, and 24 months was 81%, 63%, and 33%, respectively, for the Remicade® group and 71%, 47%, and 37%, respectively, for the Remsima® group. Nine subjects in the Remicade® group vs 11 patients in the Remsima® group were intensified. Regarding deintensification, five patients treated with Remicade® were deintensified, as compared with 7 patients on Remsima®. Savings obtained with the use of Remsima® amounted to 203,649 €, which would allow treating an additional 118 patients with biosimilar infliximab for one year. Conclusions: There are no significant differences in persistence, retention, and number of intensifications or deintensifications between biologic-naïve patients treated with Remicade® and those treated with Remsima®, the latter being an effective, safe and economical alternative for the treatment of ulcerative colitis.

ABSTRACT Psychomotor performance is a complex function generated by brain and motor systems integration, measured by accuracy, latency, and movement speed. In sports, looking for ways to improve movements is usual. Also, utilising Transcranial Direct Current Stimulation (tDCS) as a non-invasive stimulation technique may produce alterations in psychomotor sports skills. We conducted a systematic review, including experimental studies with sham or control groups in adults reporting tDCS effects on athletes’ psychomotor performance. Cochrane Manual for Systematic Reviews and the statement on systematic reviews and meta-analysis of PRISMA-P (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Protocols) were followed. PsycINFO, PubMed (central), Scopus, Web of Science, Embase, SPORTDiscus, and Cochrane Library databases were searched. Empirical studies published in English, Spanish, and Portuguese from 2009 onwards and whose primary results presented an effective measure of transcranial direct current stimulation in the psychomotor performance of adult athletes were included. The results list 10 articles, 6 of them entered in the meta-analyses. The articles presented a low risk of bias and low publication bias but great dispersion of stimulation areas.

RESUMO Objetivo: avaliar o desenvolvimento de lesão por pressão e sua associação com a complexidade assistencial em pacientes atendidos em um serviço de emergência. Método: estudo de coorte prospectiva, observacional, realizado de agosto a outubro de 2020, com pacientes atendidos em uma unidade de emergência de um hospital público do Sudoeste da Bahia - BR. Para coleta de dados utilizou-se formulário próprio, Escala de Braden e Instrumento de Classificação de Perroca. Os dados analisados por análise descritiva e teste de qui-quadrado de Pearson. Resultados: a amostra foi constituída de 225 pacientes. A incidência de lesão por pressão foi de 9,3%. Acometendo principalmente mulheres (61,9%), em cuidados intermediários (57,1%) e semi-intensivos (42,9%). Houve associação entre complexidade assistencial e o desenvolvimento de lesão por pressão (p<0,001). Conclusão: destaca-se a importância da avaliação da complexidade assistencial dos pacientes atendidos nos serviços de emergência para proporcionar um cuidado seguro e reduzir eventos adversos.

ABSTRACT Objective: to assess pressure injury development and its association with care complexity in patients treated at an emergency service. Method: a prospective cohort and observational study conducted from August to October 2020 with patients treated in the emergency unit from a public hospital in southwest Bahia, Brazil. A specific form created for the study, the Braden Scale and the Perroca Classification Instrument were used for data collection. The data were analyzed by means of descriptive analysis and the Pearson’s chi-square test. Results: the sample consisted in 225 patients. The incidence of pressure injury was 9.3%. It mainly affected women (61.9%) in the intermediate (57.1%) and semi-intensive (42.9%) care levels. There was an association between care complexity and pressure injury development (p<0.001). Conclusion: the importance of evaluating care complexity in the patients treated at the emergency services is emphasized to provide safe care and reduce adverse events.

RESUMEN Objetivo: evaluar el desarrollo de lesiones por presión y su asociación con la complejidad asistencial en pacientes atendidos en un servicio de emergencia. Método: estudio de cohorte prospectivo y observacional realizado entre agosto y octubre de 2020 con pacientes atendidos en la unidad de emergencias de un hospital público del sudoeste de Bahía, Brasil. Para recolectar los datos se utilizó un instrumento propio, la escala Braden y el Instrumento de Clasificación de Perroca. Los datos se analizaron por medio de análisis descriptivo y de la prueba chi-cuadrado de Pearson. Resultados: la muestra estuvo conformada por 225 pacientes. La incidencia de lesiones por presión fue del 9,3% y afectó principalmente a mujeres (61,9%), en los niveles de cuidados intermedios (57,1%) y semi-intensivos (42,9%). Se registró una asociación entre la complejidad asistencial y el desarrollo de lesiones por presión (p<0,001). Conclusión: se destaca la importancia de evaluar la complejidad asistencial de los pacientes atendidos en los servicios de emergencia para proporcionar una atención segura y reducir la cantidad de eventos adversos.

ABSTRACT Increased planting density, different types of thinning management, and the use of beneficial microorganisms have been adopted as strategic tools in the effort to increase crop yields in a sustainable manner. This investigation evaluated the growth and production of ‘Prata Anã Gorutuba’ banana ( Musa spp., AAB) under different plant densities during two production cycles in the semiarid region of Minas Gerais, Brazil. Micropropagated plantlets were also inoculated with endophytic bacteria and cultivated as part of two mat management strategies. Planting density varied between 1,680 and 3,920 plant ha–1. During the experiment, water was less than optimal because of rationing due to drought in the region. Under field conditions, there was no difference between bacterially inoculated and non-inoculated plants. The dense planting system increased the crop cycle. The highest-density planting treatment (3,920 plant ha–1) resulted in the highest yield in the first cycle, but no net income was observed. However, the high-density planting treatments resulted in the highest yields and gross incomes in the second production cycle. Management with thinning of plants up to 10-months-old and thinning up to harvest associated with high planting density increased the length of the production cycles. Although the water restriction experienced in the first and second crop cycles, the vegetative and productive traits of ‘Prata Anã Gorutuba’ banana did not compromise the objectives of this study.

ABSTRACT The pre-sprouted sugarcane plantlets (PSP) system aims the production of healthy and vigorous plants in reduced time, reducing the number of stalks needed for planting. Irrigation is used in all PSP system stages and water management plays an important role. Stage 1 acclimation follows the budding stage and lasts for approximately 21 days. At this stage the plantlets are grown within an agricultural greenhouse to improve initial development. The objectives of this trial were: to identify the irrigation management which results in highest plantlet growth; to evaluate if responses to irrigation management depends on the cultivar; to evaluate water consumption and water use efficiency at early stage under PSP system; and to assess the water management effect on substrate water matrix potential and stomatal conductance in the cultivar IACSP95-5000. The experimental design was a split-plot randomized block design with four replications. Treatments applied in the plots were different irrigation depths based on daily reference evapotranspiration (ETo): 96, 80, 64 and 48%, estimated by Penman–Monteith method. In the subplots, there were sugarcane cultivars IAC91-1099, IACSP95-5000 and IACSP97-4039. Irrigation management based on 80% ETo resulted in higher growth, dry mass accumulation and greater leaf area. Water use efficiency was not influenced by irrigation management. IAC91-1099 presented higher overall growth, leaf area and dry mass accumulation. Water consumption was cultivar-dependent in irrigation managements using 80 and 96% of ETo. Water use efficiency was higher in IAC91-1099 and lower in IACSP95-5000. Lower substrate water matrix potential reduced leaves stomatal conductance, impairing IACSP95-5000 plantlet growth.

Métodos: Inquérito de saúde, realizado pela internet, com amostragem em cadeia, no período de 24 de abril a 24 de maio de 2020. A intensidade da adesão à restrição de contato físico foi analisada segundo características sociodemográficas, utilizando-se modelos de regressão logística para investigar associações com ‘Nenhuma/pouca adesão’. Resultados: Dos 45.161 participantes, 74,2% (73,8-74,6%) relataram intensa adesão às medidas. O grupo que não aderiu às medidas foi composto homens (31,7%), com idade de 30 a 49 anos (36,4%), baixa escolaridade (33,0%), trabalhando durante a pandemia (81,3%), residentes nas regiões Norte (28,1%) e Centro-Oeste (28,5%) do país. Houve importante redução das taxas de crescimento diário, de 45,4 para 5,0%. Conclusão: Grande parte da população brasileira aderiu às medidas de restrição de contato físico, o que, possivelmente, contribuiu para reduzir a disseminação da COVID-19.

Intensity of adherence to physical contact restriction measures was analyzed according to sociodemographic characteristics, using logistic regression models to investigate associations with ‘No/little adherence’. Results: Of the 45,161 participants, 74.2% (73.8;74.6%) reported intense adherence to the measures. The group that did not adhere to the measures was characterized by men (31.7%), those aged 30 to 49 (36.4%), those with low education levels (33.0%), those who worked during the pandemic (81.3%), those resident in the North (28.1%) and Midwest (28.5%) regions of the country. In Brazil as a whole, there was a decrease in COVID-19 daily growth rates, from 45.4% to 5.0%. Conclusion: A large part of the Brazilian population adhered to physical contact restriction measures, which possibly contributed to decreasing the spread of COVID-19.

Abstract INTRODUCTION Human retroviruses and the hepatitis B and C viruses (HBV and HCV, respectively) share routes of transmission; thus, coinfections occur and could alter subsequent disease outcomes. A preliminary study on human T-lymphotropic virus types 1 and 2 (HTLV-1/2) in serum samples from HBV- and HCV-infected individuals in São Paulo revealed 1.3% and 5.3% rates of coinfection, respectively. These percentages were of concern since they were detected in HTLV-endemic regions and in high-risk individuals in Brazil. The present study was conducted to extend and confirm these data. METHODS HTLV-1/2 and human immunodeficiency virus (HIV) infection status were identified in 1,984 sera for HBV and HCV viral load quantification - 1,290 samples from HBV-infected individuals (53.3% men, mean age: 47.1 years) and 694 samples from HCV-infected individuals (56.3% men, mean age: 50.1 years). HTLV-1/2 antibodies were detected by enzyme immunoassay, followed by western blotting and line immunoassay; HIV infection was detected by enzyme immunoassay. RESULTS HTLV-1/-2 infection was detected in 1.9% HBV-infected individuals (0.7% HTLV-1 and 1.2% HTLV-2) and in 4.0% (2.4% HTLV-1 and 1.6% HTLV-2) HCV-infected individuals; HIV infection was detected in 9.2% and 14.5%, respectively. Strong associations with HTLV and HIV, male sex, and older age were found in HBV/HTLV and HCV/HTLV-coinfected individuals (p<0.05). CONCLUSIONS HTLV-1 and HTLV-2 were confirmed to be prevalent in individuals with HBV and HCV in São Paulo; coinfected individuals deserve further clinical and laboratory investigation.

A utilização imprópria dos recursos naturais e a intensificação do uso agropecuário desrespeitando a aptidão agrícola, sem levar em conta as potencialidades dos agroecossistemas, é uma das principais razões da degradação ambiental. Objetivou-se com esse trabalho estabelecer a aptidão agrícola das terras com o auxílio da análise multicritério no município de Itaberá-SP, discriminando as principais culturas anuais. O método empregado para a classificação das terras para o uso agrícola foi o Sistema de Avaliação da Aptidão Agrícola das Terras e adotou-se a metodologia fundamentada no uso de geotecnologias aplicando planos de informação como o uso da terra, solos, declividade, temperatura e dados pluviométricos. As variáveis foram combinadas utilizando álgebra de mapas por meio de técnica de combinação linear ponderada em que cada um deles foi estimado um fator condicionante à aptidão agrícola das terras. A normalização dos fatores deu-se com base das classes de aptidão que apresentam pesos que variam de um a quatro e a determinação dos pesos pelo método do Processo Hierárquico Analítico (AHP). Esta metodologia mostrou-se eficaz para a classificação da aptidão agrícola. A partir desse método permitiu assegura-se que a área de estudo apresenta em quase sua totalidade aptidão agrícola para as culturas do milho e da soja.

Improper use of natural resources and intensification of agricultural use, disrespecting land use capacity, without taking into account the potential of agroecosystems, is one of the main reasons for environmental degradation. The aim is to establish the land use capacity of lands by using multicriteria analysis at municipality of Itaberá-SP, discriminating main annual crops. The method used to classification of land for agricultural use was the Land Aptitude Assessment System and the methodology based on the use of geotechnologies was adopted, applying information plans such as land use, soil, slope, temperature and pluviometric data. These variables were combined using maps algebra using a weighted linear combination technique in which for each of them a conditioning factor that determines use capacity of the land was estimated. The normalization of the weights was based on the land use capacity classes that present scores ranging from one to four, and the determination of weights by the Analytical Hierarchical Process (AHP). This methodology proved to be effective for classification of land use capacity. By this method, it was possible to assure that the study area presents in almost totality land use capacity for corn and soybean crops.

Abstract: Panniculitis is a rare clinical finding in dermatomyositis, with less than 30 cases reported and there is only one case associated with the amyopathic subtype described in the literature. The present report describes a 49-year-old female patient that one year after being diagnosed with amyopathic dermatomyositis, presented indurated, painful, erythematous to violaceous nodules located on the upper limbs, thighs and gluteal region. Skin biopsy revealed lobular panniculitis with a lymphocytic infiltrate. The patient was treated with prednisone and methotrexate, but remained unresponsive to treatment.
