ABSTRACT Purpose to verify the readiness for oral feeding and breastfeeding in newborns of mothers diagnosed with gestational diabetes mellitus (GDM). Methods observational, analytical, quantitative case-control study. For the evaluation of non-nutritive sucking, the PROFAS protocol was used and for the evaluation of performance at the mother's breast, the protocol of Speech Therapy - Breastfeeding. The sample was stratified into two groups, the experimental group, composed of newborns of mothers diagnosed with GDM, and the control group, with newborns of healthy mothers. For statistical analysis, the Mann-Whitney, Shapiro Wilk and Student's t tests were used. Results the total sample consisted of 46 newborns, 21 from the experimental group and 25 from the control group. P<0.05 was observed in the comparison between the groups in the variables: oscillation in the state of consciousness, global hypotonia, weak search reflex, less than five suctions per pause in the assessment of non-nutritive sucking, holding on to the breast, falling asleep after starting suction and mother-infant positioning. Conclusion newborns of mothers diagnosed with GDM had greater difficulty in readiness for oral feeding and in the practice of breastfeeding in the first 72 hours of life, compared to children of healthy mothers.

RESUMO Objetivo verificar a prontidão para via oral e aleitamento materno em recém-nascidos de mães diagnosticadas com diabetes mellitus gestacional (DMG). Métodos estudo observacional, analítico, quantitativo, do tipo caso-controle. Para avaliação da sucção não nutritiva, foi utilizado o Protocolo de Prontidão do Prematuro para Início da Alimentação por Via Oral - POFRAS e, para avaliação do desempenho em seio materno, o Protocolo de Acompanhamento Fonoaudiológico - Aleitamento Materno. A amostra foi estratificada em dois grupos, sendo o grupo experimental composto por recém-nascidos de mães diagnosticadas com DMG e o grupo-controle, por recém-nascidos de mães hígidas. Para a análise estatística, foram utilizados os testes Mann-Whitney, Shapiro Wilk e t de Student. Resultados a amostra total foi composta por 46 recém-nascidos, sendo 21 do grupo experimental e 25 do grupo-controle. Observou-se p<0,05 na comparação entre os grupos nas seguintes variáveis: oscilação do estado de consciência, hipotonia global, reflexo de procura débil, menos de cinco sucções por pausa na avaliação da sucção não nutritiva, pega em seio, adormecimento após iniciar sucção e posicionamento mãe-bebê. Conclusão recém-nascidos de mães diagnosticadas com DMG apresentaram maior dificuldade na prontidão para via oral e na prática do aleitamento materno nas primeiras 72 horas de vida, comparados aos filhos de mães hígidas.

ABSTRACT Purpose To compare the time of food transition and the form of oral feeding at hospital discharge, between premature newborns diagnosed with bronchopulmonary dysplasia and premature newborns without the diagnosis. Methods Cross-sectional, retrospective study based on data collection from medical records. Data were collected from 78 newborns, in a reference maternity hospital, in which the sample was stratified into two groups according to the presence or absence of the diagnosis of bronchopulmonary dysplasia. The time of food transition and the feeding method at hospital discharge were analyzed for both groups. Results There was a significant difference in the time of food transition and in the feeding method at hospital discharge between the groups. Preterm newborns with bronchopulmonary dysplasia had an average of 18.03 (± 5.5) transition days and left using a bottle. Conclusion The group with bronchopulmonary dysplasia required a longer time of food transition and fewer ( of its ) infants had exclusive breastfeeding compared to the group without the diagnosis.

RESUMO Objetivo Comparar o tempo de transição alimentar e a forma de alimentação por via oral na alta hospitalar, entre recém-nascidos prematuros com diagnóstico de displasia broncopulmonar e prematuros sem o diagnóstico. Métodos Estudo transversal, retrospectivo com base na coleta de dados nos prontuários. Foram coletados dados de 78 recém-nascidos, em uma maternidade de referência. A amostra foi estratificada em dois grupos, de acordo com a presença do diagnóstico de displasia broncopulmonar, sendo um grupo de prematuros com displasia broncopulmonar e outro sem. Foram analisados o tempo de transição alimentar e o método de alimentação na alta hospitalar de ambos os grupos. Resultados Houve diferença significativa no tempo de transição alimentar e no método de alimentação na alta hospitalar entre os grupos. Os recém-nascidos pré-termo com displasia broncopulmonar apresentaram média de 18,03 (± 5,5) dias de transição e saíram em uso de mamadeira. Conclusão O grupo com displasia broncopulmonar necessitou de maior tempo de transição alimentar e teve menor frequência de aleitamento materno exclusivo, em relação ao grupo sem o diagnóstico.

RESUMO Objetivo Caracterizar a transição alimentar da via alternativa para via oral, investigar as técnicas para favorecer a alimentação e a prevalência de aleitamento materno na alta hospitalar de recém-nascidos prematuros. Método Estudo observacional, longitudinal e prospectivo desenvolvido numa maternidade. Amostra foi composta por 52 recém-nascidos prematuros e estratificada entre os grupos de prematuridade tardio, moderado e muito pré-termo. Consideraram-se as variáveis: gênero, idade gestacional e peso de nascimento, classificação da prematuridade, tempo de transição para via oral, idade gestacional corrigida e peso do recém-nascido no início e final da transição alimentar, técnica de transição utilizada, tempo de internação hospitalar e tipo de aleitamento no momento da alta. A análise estatística foi realizada com o Software SPSS, versão 25.0. Considerou-se o nível de significância de 5%. Resultados Foram observados resultados significativos entre os grupos com relação à idade gestacional corrigida no início da transição alimentar, tempo de transição alimentar da via alternativa para via oral e dias de internação. Conclusão O estudo conclui que a técnica sonda-peito foi a mais utilizada. O tempo de internação foi menor para o grupo de recém-nascidos pré-termo tardio e moderado. O tempo de transição alimentar para via oral maior no grupo de prematuros muito pré-termo e a proporção do aleitamento materno exclusivo na alta hospitalar foi semelhante entre os grupos de classificação da prematuridade.

ABSTRACT Purpose To characterize the transition from alternative feeding methods to oral feeding, to investigate techniques to favor feeding used during the transition, and the prevalence of breastfeeding at hospital discharge of premature newborns. Methods The following variables were considered: gender, gestational age and birth weight, classification of prematurity, time of transition to oral use, corrected gestational age and newborn weight at the beginning and end of the food transition, transition technique used, length of hospitalization and type of breastfeeding at discharge. For data analysis, the software SPSS version 25.0 was used considering the significance level of 5%. Results Significant results were observed between the premature groups for corrected gestational age at the beginning of the food transition, transition time, and hospital days. Conclusion The study concludes that the breast-probe technique was the most used. The length of hospital stay was shorter for the group of late and moderate preterm newborns. The transition time to the oral route was longer in the group of very preterm infants. Moreover, the proportion of exclusive breastfeeding at hospital discharge was similar between the prematurity classification groups.

RESUMO Objetivo: verificar a associação entre estado nutricional e ocorrência de disfagia orofaríngea em indivíduos com paralisia cerebral. Métodos: estudo transversal, quantitativo, realizado com indivíduos entre dois e 20 anos. Foi avaliado comprometimento neuromotor, ocorrência e grau de disfagia, antropometria, consumo alimentar, via alimentar, fracionamento e consistência da dieta. A avaliação da deglutição foi realizada por fonoaudióloga capacitada, por meio do Protocolo para Avaliação Clínica da Disfagia Pediátrica. Foi realizada análise estatística pertinente, adotando p<0,05%. Resultados: participaram 40 indivíduos com mediana de 8,7 (5,45-14,5) anos, sendo 65% do sexo masculino. Quanto ao comprometimento neuromotor, 72,5% foram classificados no nível IV e V. A prevalência de disfagia foi de 70%, via alimentar predominantemente oral (77,5%) e consistência da dieta normal (55%). Nas curvas de crescimento, todos foram classificados com estatura adequada para idade. Para peso, índice de massa corporal e dobras cutâneas houve, respectivamente, 82,5%, 85% e 62,5% em eutrofia. Foi encontrada associação estatisticamente significante entre disfagia e índice de massa corporal (p=0,018). A média diária de ingestão calórica e proteica foi de 1427,29±338,62 kcal e 56,86±17,57 gramas, respectivamente. Foi encontrada associação estatisticamente significante entre via alimentar e quantidade de proteína ingerida diariamente (p=0,041). Conclusão: a maioria dos indivíduos apresentou disfagia orofaríngea e comprometimento motor mais grave, alimentavam-se por via oral com consistência de dieta normal. Observou-se que quanto maior a dificuldade de deglutição, maior o comprometimento do estado nutricional.

ABSTRACT Objective: to verify the association between nutritional status and the occurrence of oropharyngeal dysphagia in individuals with cerebral palsy. Methods: a cross-sectional, quantitative study with people aged between two and 20 years. Neuromotor impairment, occurrence and degree of dysphagia, anthropometry, food consumption, feeding route, diet fractionation and consistency were evaluated. Swallowing evaluation was performed by a skilled speech-language-hearing therapist, through the Clinical Evaluation Protocol of Pediatric Dysphagia (PAD-PED, Portuguese acronym).The statistical analysis was performed adopting p<0.05%. Results: 40 subjects with a median of 8.7 (5.45-14.5) years, 65% of whom were males. Concerning neuromotor impairment, 72.5% were classified at levels IV and V. The prevalence of dysphagia was 70%, the feeding route being predominantly oral (77.5%) with a normal diet consistency (55%). In the growth curves, all of them were classified within an adequate height for their age. As for weight, body mass index and skinfolds, 82.5%, 85% and 62.5% were eutrophic, respectively. Statistically significant association was found between dysphagia and body mass index (p=0.018). The mean daily caloric and proteic intake was 1427.29±338.62 kcal and 56.86±17.57 grams, respectively. Statistically significant association was found between the feeding route and the amount of protein ingested daily (p=0.041). Conclusion: most subjects presented oropharyngeal dysphagia and more severe motor impairment; they were fed orally with a normal diet consistency. It was observed that the greater the difficulty in swallowing, the greater the impairment of nutritional status.

RESUMO Objetivo: investigar o Schedule Oral Motor Assessment (SOMA) para utilização com recém-nascidos pré-termo, e comparar seus resultados com o Preterm Oral Feeding Readiness Scale (POFRAS) na liberação da alimentação por via oral. Métodos: trabalho de caráter transversal e quantitativo. Estudo composto por uma amostra de 45 recém-nascidos pré-termo, saudáveis e clinicamente estáveis, avaliados no momento da liberação da alimentação por via oral. Neste momento foi realizada a avaliação do POFRAS e do SOMA. Para a análise dos dados foi utilizado o software Stata 10.0. Resultados: todos 10 recém-nascidos pré-termo que apresentaram prontidão para alimentação por via oral tinham função motora oral normal, e, os 16 que apresentaram disfunção motora oral, esses não apresentavam prontidão para a mamada (p<0,05). O tempo de transição de alimentação por via oral foi de 13,5 (±8,1) dias nos recém-nascidos pré-termo, com melhores resultados nas duas avaliações, e de 17,7 (±10,9) dias aos que não apresentaram prontidão para a alimentação oral e tiveram disfunção motora oral no momento da liberação da via oral, resultando em um dado com relevância clínica mesmo não apresentando significância (p>0,05). Conclusão: estes resultados sugerem que o SOMA pode ser um método complementar para avaliação da função oral-motora no momento da liberação da via oral de recém-nascido pré-termo.

ABSTRACT Objective: this study aimed at investigating the Schedule Oral Motor Assessment (SOMA) tool to be used with preterm infants and to compare its results with the Preterm Oral Feeding Readiness Assessment Scale (POFRAS) to start oral feeding. Methods: a cross-sectional and quantitative study, consisting in a sample of 45 healthy and clinically stable preterm infants, assessed at their first oral feeding with two tools: the Schedule Oral Motor Assessment and Preterm Oral Feeding Readiness Assessment Scale. Stata 10.0 software was used for data analysis. Results: 10 preterm infants with readiness for oral feeding showed normal oral motor function, and 16, presented with oral motor dysfunction, did not show readiness for feeding (p <0.05). The time of transition for full oral feeding was 13.5 (± 8.1) days for preterm infants with better results in both assessment tools, and 17.7 (± 10.9) days for those who did not show readiness for oral feeding and had oral motor dysfunction to initiate oral feeding, resulting in a given clinical relevance, even showing no significance (p> 0.05). Conclusion: these results suggest that the Schedule Oral Motor Assessment can be an adjunctive method for evaluation of the oral motor function at the first oral feeding in preterm infants.

RESUMO Objetivo Correlacionar a saturação periférica de oxigênio com a idade gestacional e com o nível de habilidade de alimentação oral, na introdução da alimentação oral, de recém-nascidos pré-termo. Método Estudo transversal e quantitativo no qual participaram 169 recém-nascidos pré-termo, clinicamente estáveis. A verificação da saturação de oxigênio foi realizada antes e após a introdução da primeira alimentação por via oral. Os recém-nascidos pré-termo foram estratificados em três grupos baseados na idade gestacional ao nascimento: 26-29, 30-33 e 34-36 semanas. A habilidade para alimentação oral foi classificada em quatro níveis com base na habilidade oral e resistência apresentada pelo recém-nascido pré-termo. Resultados Não houve diferença na saturação periférica de oxigênio entre os estratos da idade gestacional e entre os níveis de habilidade oral. Foram observadas diferenças nos grupos com idade gestacional de 30-33 semanas (p=0,04) e 34-36 semanas (p=0,02) e no nível I de habilidade de alimentação oral (p=0,04) quando comparada com a saturação inicial e final. Correlações significativas (p<0,001) foram encontradas entre a idade gestacional e níveis de habilidade oral (r=0,38); no Grupo A, entre a idade gestacional e a saturação inicial (r=0,83); e, no Grupo B, entre o nível de habilidade oral com a saturação inicial, e com a idade gestacional. Conclusão A saturação de oxigênio apresentou correlação quando comparada com a idade gestacional e com o nível de habilidade oral.

ABSTRACT Purpose To correlate the peripheral oxygen saturation with gestational age and the level of oral feeding skills in the introduction of oral feeding in preterm infants. Methods This is a cross-sectional, quantitative study whose sample was composed of 169 clinically stable preterm infants. Peripheral oxygen saturation was assessed before and after introduction of oral feeding. The preterm infants were stratified into three groups based on their gestational age at birth: 26-29, 30-33, and 34-36 weeks. The preterm infants were classified into four levels according to their oral feeding skill and resistance. Results No differences in oxygen saturation were observed between the strata of gestational age and between the levels of oral feeding skill. Differences were observed in the groups of preterm infants aged 30-33 weeks (p=0.04) and 34-36 weeks (p=0.02) and on the level I of oral feeding skills (p=0.04) when oxygen saturation was compared at pre- and post-first oral feeding. Significant correlations (p<0.001) were found between gestational age and the levels of oral feeding skills (r=0.38); in Group A, between gestational age and oxygen saturation before the first oral feeding (r=0.83); in Group B, between the level of oral feeding skill and oxygen saturation before the first oral feeding (r=0.26) and between level of oral feeding skill and gestational age (r=0.26). Conclusion Correlation was found for peripheral oxygen saturation when compared with gestational age and with the level of oral feeding skills.

RESUMO Objetivo Correlacionar os parâmetros cardiorrespiratórios com a idade gestacional e com o nível de habilidade de alimentação oral na introdução da alimentação oral de recém-nascidos pré-termo. Método Participaram desta pesquisa 37 recém-nascidos pré-termo, clinicamente estáveis. A verificação das frequências cardíaca e respiratória foi realizada antes e após a introdução da primeira alimentação por via oral. Os recém-nascidos foram distribuídos em três estratos conforme a idade gestacional do nascimento. A habilidade oral foi classificada em quatro níveis: I - baixa habilidade oral e baixa resistência para alimentação; II - baixa habilidade oral e alta resistência para alimentação; III - alta habilidade oral e baixa resistência para alimentação; IV - alta habilidade oral e alta resistência para alimentação. Resultados Não houve diferença na frequência cardíaca e respiratória entre os estratos da idade gestacional e entre os níveis de habilidade oral. Ao comparar a frequência cardíaca e respiratória inicial e final dentro de cada nível de habilidade oral e estrato da idade gestacional, observou-se diferença entre a frequência cardíaca nos estratos das idades gestacionais entre 30 e 33 semanas e entre 34 e 36 semanas, bem como nos níveis de habilidade oral I, II e IV; com relação à comparação entre a frequência respiratória inicial e final, foi encontrada diferença no nível I de habilidade de alimentação oral. Conclusão As frequências cardíaca e respiratória sofreram alterações quando comparadas com dados pré e pós-prandial na primeira alimentação por via oral, assim como quando comparadas sobre aspectos da idade gestacional e nível de habilidade oral.

ABSTRACT Purpose To correlate cardiorespiratory parameters with gestational age and level of oral feeding skills in the first oral feeding in preterm infants. Methods Study participants were 37 clinically stable preterm infants. Cardiorespiratory rate was assessed before and after introduction of oral feeding. The newborns were divided into three strata according to gestational age at birth. Oral skill was classified into four levels: I - low oral skill and low resistance to feeding; II - low oral skill and high resistance to feeding; III - high oral skill and low resistance to feeding; IV - high oral skill and high resistance to feeding. Results No difference was observed in heart and respiratory rate between the strata of gestational age at birth and between the levels of oral skill. Comparison between pre- and post-cardiorespiratory rates within each level of oral skill and stratum of gestational age showed difference between heart rate in the strata of gestational ages of 30 to 33 weeks and of 34 to 36 weeks, as well as between oral skill of levels I, II, and IV. With regard to the comparison between pre- and post- respiratory rates, difference was found in the oral skill of level I. Conclusion Differences were observed between pre- and post-prandial cardiorespiratory rates regarding the first oral feeding, as well as between strata of gestational age at birth and levels of oral feeding skills.

RESUMO Objetivo avaliar a acurácia do Preterm Oral Feeding Readiness Scale - POFRAS para iniciar a alimentação oral de recém-nascidos pré-termo e verificar a concordância entre este instrumento e o instrumento de avaliação do Nível de Habilidade Oral. Métodos Foram avaliados 82 recém-nascidos pré-termo quanto à prontidão para o início da alimentação oral através do POFRAS e da avaliação do Nível de Habilidade Oral, durante a primeira alimentação oral. A acurácia do POFRAS foi estimada em relação à variável proficiência, por meio da Curva ROC (Receiver Operating Characteristic Curve). Para a análise da concordância entre os instrumentos, foi utilizado o coeficiente Kappa. Resultados A acurácia global do POFRAS foi de 71,29%. O ponto de corte 29 foi o que apresentou melhor equilíbrio entre sensibilidade e especificidade. O coeficiente Kappa mostrou fraca concordância entre os instrumentos na identificação dos RN aptos e inaptos a mamar por via oral (k=0.281). Conclusão a acurácia do POFRAS para o início da alimentação oral, estimada por meio da variável proficiência, foi semelhante à obtida com a técnica de translactação. Observou-se fraca concordância entre os instrumentos avaliados. Sugere-se, portanto, que estes instrumentos de avaliação sejam usados de forma complementar na prática clínica, uma vez que ambos apresentam aspectos importantes do comportamento alimentar do prematuro, que ao serem analisados conjuntamente permitirão orientar a conduta necessária para propiciar uma transição alimentar mais breve e eficaz para essa população.

ABSTRACT Purpose To assess the accuracy of the Preterm Oral Feeding Readiness Scale (POFRAS) on the beginning of oral feeding in preterm infants and to verify the concordance between this tool and the Oral Feeding Skill Level. Methods 82 preterm infants were assessed by POFRAS regarding their readiness to initiate oral feeding and by the oral feeding skill level evaluation during the first oral feeding. POFRAS's accuracy was estimated regarding proficiency by a Receiver Operating Characteristics (ROC) curve. The concordance between the tools was obtained by analysis of the Kappa coefficient. Results POFRAS's global accuracy was of 71.29%. The cut-off value of 29 was the one that presented most optimization of the sensitivity based on specificity. The Kappa coefficient has shown a weak concordance between the instruments to identify infants able and unable to oral feeding (k=0.281). Conclusion POFRAS's accuracy to initiate oral feeding considering the proficiency was similar to that obtained with the technique of translactation. We observed a weak concordance between the instruments. We suggest that, in clinical practice, both instruments should be used in a complementary manner, since both present important aspects of the preterm feeding behavior that together will better guide the necessary conduct to provide an effective and quick transition to full oral feeding in this population.

RESUMO Objetivo Verificar se a presença de prontidão para iniciar a alimentação oral, obtida por meio do Preterm Oral Feeding Readiness Scale (POFRAS) se relaciona com o desempenho alimentar na primeira oferta oral de leite, com o tempo de transição da sonda para a via oral plena e o tempo de internação hospitalar. Métodos Estudo longitudinal, que envolveu 65 recém-nascidos pré-termo. Foi realizada a avaliação da prontidão para início da alimentação oral, por meio do POFRAS, constituindo-se dois grupos: com e sem prontidão para iniciar a alimentação por via oral, em dois estratos de idade gestacional ao nascer: de 28 a 33 semanas e de 34 a 36 semanas. Em cada estrato, os grupos foram comparados entre si, quanto ao desempenho alimentar na primeira oferta oral de leite (por meio das variáveis proficiência, taxa de transferência e desempenho alimentar oral) e quanto ao tempo de transição da sonda para a via oral plena e o tempo de internação hospitalar. O teste t-Student ou o teste de Wilcoxon foram utilizados para comparar as variáveis contínuas e o teste exato de Fisher, para as variáveis categóricas. Resultados A prontidão para iniciar a alimentação por via oral se relacionou com melhores resultados de proficiência e taxa de transferência na primeira alimentação, contribuindo para a transição mais rápida da sonda para a via oral plena, nas crianças nascidas com idade gestacional entre 28 a 33 semanas. Conclusão a avaliação por meio do POFRAS permite estabelecer um prognóstico da alimentação oral em recém-nascidos pré-termo, menores de 34 semanas. No entanto, não exclui a necessidade de avaliação da biomecânica da deglutição.

ABSTRACT Purpose To verify whether readiness for oral feeding as evaluated by the Preterm Oral Feeding Readiness Scale (POFRAS) is associated with performance on the first oral feeding; length of transition from tube to full oral feeding, and duration of hospitalization. Methods This was a longitudinal study involving 65 preterm infants. Participants were divided into groups according to readiness for oral feeding as determined by the POFRAS, and gestational age (29-33 weeks and 34-36 weeks). Performance on the first oral feeding (proficiency, transfer rate and overall transfer), number of days from introduction to full independent oral feeding, and length of hospital stay were compared between groups. Continuous variables were compared using Student’s t-test or Wilcoxon’s signed-rank test. Categorical variables were compared using Fisher’s exact test. Results Readiness for oral feeding was associated with higher proficiency and transfer rates on the first oral feeding, resulting in a shorter transition from tube to independent oral feeding in preterm infants with 29 to 33 weeks of gestational age at birth. Conclusion The POFRAS contributed to the prognosis of oral feeding outcomes in preterm neonates with less than 34 weeks of gestational age at birth. However, its use does not eliminate the need for an assessment of swallowing mechanisms.

OBJETIVO: verificar o desempenho da sucção nutritiva, na mamadeira, em recém-nascidos pré-termo submetidos à estimulação sensório-motora-oral. MÉTODOS: pesquisa do tipo ensaio clínico controlado de intervenção. 20 recém-nascidos pré-termo foram distribuídos em grupo estimulado e grupo controle. Estes grupos foram submetidos à avaliação fonoaudiológica em dois diferentes momentos: na liberação da alimentação por via oral; e quando atingiam alimentação plena por via oral num período de 24 horas. As avaliações foram filmadas, e os resultados foram analisados pela pesquisadora. Para análise dos dados utilizou-se o Teste exato de Fishe r e o Teste "T" de Studen t do software estatístico STATA 10.0. Considerou-se p < 0,05. RESULTADOS: recém-nascidos do grupo estimulado, na segunda avaliação, apresentaram resultados com diferença estatística significante quanto a força de sucção forte (p=0,003), presença dos três reflexos adaptativos (p=0,001), coordenação da sucção/deglutição/respiração (p=0,003), no tempo total de sucções (p=0,01) e na frequência de sucção (p<0,01). O número total de sucções apresentou diferença estatística significante (p<0,05) entre as avaliações do grupo controle. Quanto ao ritmo e a relação entre o volume de leite prescrito e ingerido não apresentou resultados com diferença estatística significante. CONCLUSÃO: a estimulação sensório-motora oral parece favorecer o desempenho nas funções de sucção nutritiva de recém-nascidos pré-termo.

PURPOSE: to check the nutritive sucking performance in preterm infants that underwent sensory-motororal stimulation. METHODS: a controlled clinical trial of intervention was performed in 20 preterm infants divided into experimental and control group. The infants received a speech therapist evaluation in two different times: at the beginning of the oral feeding (first speech therapist evaluation) and when they reached full oral feeding (second evaluation). Statistical data analyses used STATA 10 package and they were analyzed for treatment group differences with Fisher's exact Test and Student's unpaired t-test (p<0.05). RESULTS: infants in the experimental group, in the second evaluation, differed statistically in the pressure of strong sucking (p=0.003), presence of the three adaptive reflexes (p=0.001), coordination among sucking, swallowing and breathing (p=0.003), the total time for sucking (p=0.01) and of number of sucking/time (p<0.01). The total number of sucks showed a statistically significant difference (p<0.05) among the evaluations of the control group. No differences were observed among the groups as for sucking rhythm, and prescribed and ingested milk volume. CONCLUSION: in this study, sensory-motor-oral stimulation contributed for a better sucking performance, during bottle feeding, in preterm infants.

OBJETIVOS: avaliar a influência da estimulação sensório-motora-oral (SMO) sobre o ganho de peso, tempo para realizar a transição da sonda para a via oral e, indiretamente, sobre a alta hospitalar de pré-termos. MÉTODOS: estudo de intervenção, do tipo ensaio clínico controlado, que incluiu 24 pré-termos, internados na UTI Neonatal do Hospital Universitário de Santa Maria, Rio Grande do Sul, Brasil, entre maio de 2007 e março de 2008, alocados, por sorteio, em grupo estimulado (GE) e controle (GC). A análise das diferenças entre os grupos foi testada utilizando o teste t-Student independente e exato de Fisher (valor de p<0,05). RESULTADOS: não houve diferença estatística para as medidas antropométricas, embora o incremento diário de peso tenha sido maior no GE (p=0,16). O GE realizou a transição completa da sonda para a via oral em média 1,6 dias antes que o GC, sendo que 83,3% das crianças do GE fizeram essa transição em até sete dias, enquanto apenas 38,9% do GC alcançaram esse prazo. Não houve diferença no tempo de permanência hospitalar entre os grupos (p=0,48). CONCLUSÕES: a estimulação SMO favoreceu uma transição mais rápida da sonda para a via oral, não comprometendo o ganho de peso de recém-nascidos pré-termo. Não foi possível observar sua influência sobre o tempo de permanência hospitalar.

OBJECTIVES: to evaluate the influence of sensory-motor-oral (SMO) stimulation on weight gain, the time taken to make the transition from tube-to oral feeding and, indirectly, on the duration of hospitalization of preterm infants. METHODS: a controlled clinical intervention study was carried out with 24 preterm infants hospitalized in the Neonatal ICU of the Santa Maria University Hospital in the State of Rio Grande do Sul, Brazil, between May 2007 and March 2008, divided randomly into a treatment (TG) and control group (CG). The analysis of the differences between the groups was tested using Student's independent t-test and Fisher's exact test (with a p value <0.05). RESULTS: there was no statistical difference in terms of anthropometrical measurements, although the daily weight gain was higher in the TG (p=0.16). The TG group made a full transition from tube-to oral feeding on average 1.6 days earlier than the CG, 83.3% of the children in the TG making the transition within seven days, compared to only 38.9% of the control group. There was no difference in terms of the length of stay in hospital between the two groups (p=0.48). CONCLUSIONS: SMO stimulation was likely to lead to a swifter transition from tube-to oral feeding, without any adverse effect on weight gain in preterm newborns. No influence on the length of stay in hospital was observed.

OBJETIVO: Analisar as características da sucção nutritiva na liberação da via oral em recém-nascidos pré-termo (RNPT) de diferentes idades gestacionais. MÉTODOS: A amostra constou de 32 RNPT, avaliados no momento da liberação para alimentação por via oral, dividida em dois grupos: Grupo 1 (G1) com idade gestacional inferior a 34 semanas; e Grupo 2 (G2) com idade gestacional igual ou superior a 34 semanas. O desempenho da sucção nutritiva foi avaliado observando-se presença ou ausência de blocos de sucção, número e tempo das sucções nos três primeiros blocos apresentados, e presença ou ausência de coordenação entre sucção/deglutição/respiração (S/D/R). Nenhum RNPT recebeu estimulação fonoaudiológica prévia. RESULTADOS: O G2 apresentou, na maioria dos RNPT, presença de coordenação entre S/D/R, além de melhor desempenho em relação ao número e ao tempo de sucção nos blocos de sucção quando comparado ao G1. Na correlação do número versus tempo de sucção por bloco, o G1 apresentou resultados aproximados aos do G2. Considerando-se a classificação quanto ao desenvolvimento intra-útero adequado para a idade gestacional, os RNPT do G1 apresentaram desempenho semelhante aos do G2, no primeiro e segundo blocos, observando-se queda, no G1, apenas no terceiro bloco. CONCLUSÃO: O melhor desempenho no padrão de sucção nutritiva foi apresentado pelo G2, sugerindo que a idade gestacional corrigida dos RNPT interfere diretamente nos resultados obtidos na avaliação da sucção nutritiva. Assim sendo, esta também é uma variável a ser considerada para liberação da alimentação por via oral.

PURPOSE: To analyze the characteristics of nutritive sucking in preterm infants of different gestational ages after the medical prescription for oral feeding. METHODS: The sample comprised 32 preterm infants who were evaluated at the time they were released for oral feeding. The subjects were divided into two groups: Group 1 (G1), with gestational ages below 34 weeks; and Group 2 (G2), with gestational ages equal or above 34 weeks. Performance during nutritive sucking was evaluated by observing presence or absence of suction blocks, number and duration of suctions in the first three blocks, and presence or absence of suction/swallowing/breathing (S/S/B) coordination. None of the preterm infants in this sample had previously received oral motor sensory stimulation. RESULTS: Group 2 showed, in most preterm infants, the presence of S/S/B coordination, besides a better performance regarding number and duration of sucking blocks, when compared to Group 1. Regarding correlation between number and duration of sucking by block, G1 and G2 had similar results. Considering the intrauterine development of the subjects suitable to the gestational age, G1 and G2 presented similar performances both in the first and second sucking blocks. A decrease was observed in G1 in the third block. CONCLUSION: Group 2 showed better development of the nutritive sucking pattern, suggesting that the corrected gestational age of preterm infants interfered in the results obtained at the nutritive sucking evaluation. Therefore, this is also a relevant variable to be considered for oral feeding release.
