ABSTRACT Objective: To report outcomes from the largest multicenter series of penile cancer patients undergoing video endoscopic inguinal lymphadenectomy (VEIL). Materials and Methods: Retrospective multicenter analysis. Authors of 21 centers from the Penile Cancer Collaborative Coalition-Latin America (PeC-LA) were included. All centers performed the procedure following the same previously described standardized technique. Inclusion criteria included penile cancer patients with no palpable lymph nodes and intermediate/high-risk disease and those with non-fixed palpable lymph nodes less than 4 cm in diameter. Categorical variables are shown as percentages and frequencies whereas continuous variables as mean and range. Results: From 2006 to 2020, 210 VEIL procedures were performed in 105 patients. Mean age was 58 (45-68) years old. Mean operative time was 90 minutes (60-120). Mean lymph node yield was 10 nodes (6-16). Complication rate was 15.7%, including severe complications in 1.9% of procedures. Lymphatic and skin complications were noted in 8.6 and 4.8% of patients, respectively. Histopathological analysis revealed lymph node involvement in 26.7% of patients with non-palpable nodes. Inguinal recurrence was observed in 2.8% of patients. 10y- overall survival was 74.2% and 10-y cancer specific survival was 84.8%. CSS for pN0, pN1, pN2 and pN3 were 100%, 82.4%, 72.7% and 9.1%, respectively. Conclusion: VEIL seems to offer appropriate long term oncological control with minimal morbidity. In the absence of non-invasive stratification measures such as dynamic sentinel node biopsy, VEIL emerged as the alternative for the management of non-bulky lymph nodes in penile cancer.

Hypertension is one of the primary modifiable risk factors for morbidity and mortality worldwide, being a major risk factor for coronary artery disease, stroke, and kidney failure. Furthermore, it is highly prevalent, affecting more than one-third of the global population.
Blood pressure measurement is a MANDATORY procedure in any medical care setting and is carried out by various healthcare professionals. However, it is still commonly performed without the necessary technical care. Since the diagnosis relies on blood pressure measurement, it is clear how important it is to handle the techniques, methods, and equipment used in its execution with care.
It should be emphasized that once the diagnosis is made, all short-term, medium-term, and long-term investigations and treatments are based on the results of blood pressure measurement. Therefore, improper techniques and/or equipment can lead to incorrect diagnoses, either underestimating or overestimating values, resulting in inappropriate actions and significant health and economic losses for individuals and nations.
Once the correct diagnosis is made, as knowledge of the importance of proper treatment advances, with the adoption of more detailed normal values and careful treatment objectives towards achieving stricter blood pressure goals, the importance of precision in blood pressure measurement is also reinforced.
Blood pressure measurement (described below) is usually performed using the traditional method, the so-called casual or office measurement. Over time, alternatives have been added to it, through the use of semi-automatic or automatic devices by the patients themselves, in waiting rooms or outside the office, in their own homes, or in public spaces. A step further was taken with the use of semi-automatic devices equipped with memory that allow sequential measurements outside the office (ABPM; or HBPM) and other automatic devices that allow programmed measurements over longer periods (HBPM).
Some aspects of blood pressure measurement can interfere with obtaining reliable results and, consequently, cause harm in decision-making. These include the importance of using average values, the variation in blood pressure during the day, and short-term variability. These aspects have encouraged the performance of a greater number of measurements in various situations, and different guidelines have advocated the use of equipment that promotes these actions. Devices that perform HBPM or ABPM, which, in addition to allowing greater precision, when used together, detect white coat hypertension (WCH), masked hypertension (MH), sleep blood pressure alterations, and resistant hypertension (RHT) (defined in Chapter 2 of this guideline), are gaining more and more importance.
Taking these details into account, we must emphasize that information related to diagnosis, classification, and goal setting is still based on office blood pressure measurement, and for this reason, all attention must be given to the proper execution of this procedure.

ABSTRACT The objective of this study was to analyze technical and economic indicators of responses of papaya (Carica papaya L.) to the production factors water and organic compost, in a rural property with characteristic of family farming in the Semiarid region of Brazil. The study was conducted from August 2019 to July 2020, in the first crop cycle, in the municipality of Pentecoste, Ceara, Brazil. A Formosa papaya crop (cultivar Tainung 1) was established with spacing of 2.5 × 2.5 m, and irrigated by a micro-sprinkler localized system. A randomized block design was used, with a split-plot arrangement and four replications. The primary treatments consisted of four irrigation water depths (60%, 80%, 100%, and 120% of the crop evapotranspiration), and the secondary treatments consisted of four organic compost rates (0%, 50%, 100%, and 150% of the required rate for the crop). The irrigation water productivity of 5.38 R$ (BRL) m-3, related to the requirements of the production factors, on average, is five-fold the reference value for papaya crops under conventional production system. Rural credit allows the farmer to reach a social reproduction level with a papaya crop area that can be, on average, half of that needed under conditions without financing.

Abstract Background Controlling blood pressure and glycemic levels is a challenge that requires innovative solutions. Objective To assess the feasibility of implementing a text message intervention among low-income primary care patients, as well as to assess self-reported behavioral change. Methods A set of 200 text messages was developed on healthy eating, physical activity, adherence, and motivation. Participants from Vale do Mucuri, MG, Brazil diagnosed with diabetes or hypertension or undergoing screening for those diseases, received 5 to 8 messages per week for 6 months. They answered a questionnaire to report their satisfaction and behavioral changes. Results Of the 136 patients, 117 (86.0%) answered the questionnaire. Most reported that the messages were very useful (86.3%), easy to understand (90.6%), and were very helpful for behavioral change (65.0%); 84.6% reported that they had started eating healthier. The most frequent reported lifestyle changes were: improved diet quality (85.5%), reduced portions (65.8%), and weight loss (56.4%). The majority of patients shared the messages (60.7%) with family or other acquaintances, considered the number of messages to be adequate (89.7%) and would recommend the program to others (95.7%). Conclusion An intervention based on text messages to promote behavioral change in patients with hypertension or diabetes in primary care is feasible in low-resource settings. Future studies are needed to assess the program’s long-term effects on clinical outcomes.

ABSTRACT Reducing water consumption by crops in semi-arid regions is an important factor for the sustainability of agriculture in these locations. In this sense, this study aims to evaluate the neuro-fuzzy inference method as a support for decision-making in irrigated coriander cultivation. The experiment was performed in two cultivation cycles in Pentecoste-CE, Brazil. The experiment was conducted in randomized blocks arranged in a split-plot design with five primary treatments, consisting of irrigation depths (50, 75, 100, 125, and 150% of the localized evapotranspiration, ETcloc), and five secondary treatments, consisting of different levels of bagana mulch (0, 25, 50, 75, and 100%, equivalent to 16 t ha−1). Neuro-fuzzy models with two input variables and eight output biometric variables were developed to evaluate growth (plant height, number of roots, and root length) and yield variables (productivity and shoot and root fresh and dry mass). In the first cycle, the best results occurred close to 55% ETcloc and between 40 and 50% of mulch; in the second cycle, water consumption returned results between 50 and 80% ETcloc. The fuzzy and multiple regression models showed MAE, MSE, and RMSE errors of 9, 22, and 10% lower, respectively. The neuro-fuzzy model might be a viable option for decision-making in irrigated crops, being able to optimize the use of natural resources and available water in semi-arid regions. The use of 55% of irrigation depth and a range of 40 to 50% of mulch can be a strategy for a higher water use efficiency.

The copper alloy UNS-C27200 is a binary brass, composed of 38wt%Zn, beeing one of the most used in the industry. The union of Cu-Zn alloys by conventional welding by fusion in thin plates presents several challenges. The Friction Stir Welding (FSW) process performs the union of two, or more materials, through a rotating tool that travels throughout the welding region. One the great advantages of FSW is working below the melting point of the material, which reduces the change in the microstructure and properties of the material, when compared to conventional welding processes. Due to the lack of research on FSW process in lap brass joints with thin thicknesses, the present work aims to expand these studies in order to contribute to future research. Eight conditions were evaluated, varying rotation and welding speed. In the best results by visual inspection, analyzes of microstructure, temperature, microhardness and mechanical strength were performed. The better welding results were obtained with ω=1050 rpm and ν=20 mm/min. There was reduction of approximately 50% between Stir Zone and Base metal. The mechanical strength was affected by the pin hole resulting from the process. The microstructure revealed a homogeneous mixture in the stir zone and no defects were detected.

Abstract This study compares the morphology, thermal, and dynamic-mechanical properties of composites based on polybutylene adipate terephthalate/polylactide biocomposites with sponge gourd waste treated code as R, and non-treated sponge gourd, coded as NR, by mechanical disc refining after milled process. Extrusion followed by compression molding was used to produce biocomposites with fiber contents of 0, 2.5, 5, 10, and 15% wt/wt for R and NR sponge gourd fibers. Scanning electron microscopy analysis reveals that NR has the morphology of a rigid tubular shape, whereas R is a thinner, twisted, and fibrillated fiber. Regardless of the type of sponge gourd fiber used, the thermal stability of the composite decreases as the sponge gourd content increases. At 25°C, the biocomposite with 10%wt/wt R fiber has the highest storage modulus value. The comparison of Tangent peak values reveals that the presence of sponge gourd fibers reduces the energy dissipation of the biocomposites. The analysis of the loss modulus at 25°C reveals that R fiber contributes more to the reduction of energy dissipation of the biocomposites than NR. Furthermore, the Cole-Cole plot shows that R and NR fibers are dispersed and do not significantly change the homogeneity of the biopolymer systems.

ABSTRACT The cultivation of onion in the Brazilian Northeast region has been gaining prominence due to the favorable edaphoclimatic conditions and the use of technology. However, the increase in production costs, price fluctuations and the suppression of sulfur in fertilizer formulations constitute a risk for the farmers. Thus, the aim of this study was to evaluate the productive and economic aspects of onion production as a response to sulfur fertilization. Two experiments were carried out from June to November 2018 and 2019, at the Rafael Fernandes Experimental Farm, belonging to the Universidade Federal Rural do Semi-árido, in the municipality of Mossoró, Rio Grande do Norte, Brazil, in an Ultisol. The experimental design was randomized complete blocks with 14 treatments and four replicates, corresponding to the combination of two onion cultivars and seven doses of sulfur. The following characteristics were evaluated: sulfur content in the diagnostic leaf, bulb yield, total operating costs, gross revenue, net revenue, rate of return and yield index. Regardless of the cultivar, input costs add up to higher expenses, reaching approximate values of 60.6 and 48.4% of the total operating cost for the Rio das Antas and IPA 11 cultivars, respectively. Rio das Antas was more sulfur efficient, achieving greater bulb yield and net revenue. The 34 kg ha-1 sulfur dose promoted maximum yield and net revenue for Rio das Antas. The application of sulfur did not influence the yield of the IPA 11 cultivar.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

ABSTRACT Learning is a subject of constant concern for all those dedicated to this activity, where there are different paradigmatic approaches. The objective of this work is to assess the different types of learning associated with current contexts in order to improve the teaching-learning process. The fundamental methods used have been, from the theoretical, the logical historical and the synthetic analytic; and from empyrean, observation and analysis of documents. The main results are associated with the description of the most useful methods in these times, the psychological current that underlies each one of them, their complementary elements and adaptations (whether virtual or face-to-face) to the generation that today is in the classrooms. With the assessment of the different learning concepts, a tool has been provided to support the work of teachers of these times.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Abstract Brazilian spotted fever (BSF) is a common tick-borne disease caused by Rickettsia rickettsii. Horses are the primary hosts of the main vector, Amblyomma sculptum, and are considered efficient sentinels for circulation of Rickettsia. Therefore, the aim of this study was to detect antibodies reactive to R. rickettsii antigens in horses from a non-endemic area in the north-central region of Bahia state, Brazil. Blood samples and ticks were collected from 70 horses from the municipalities of Senhor do Bonfim, Antônio Gonçalves, Pindobaçu, and Campo Formoso. The sera obtained were tested by an indirect immunofluorescence assay to detect anti-Rickettsia antibodies. Overall, 5.7% (4/70) of the horses reacted to R. rickettsii antigens. Ticks were collected from 18.6% (13/70) of the horses and were identified as A. sculptum and Dermacentor nitens. Despite being a non-endemic area, seropositive animals were identified in our study, suggesting circulation of rickettsial agents in the region. This is the first serological survey of this agent in horses from the north-central region of Bahia, and further studies are needed to understand the epidemiology of BSF in these locations.

Abstract The present study provides historical and new records of the introduced millipedes species in Brazil, Oxidus gracilis (C.L. Koch, 1847), Orthomorpha coarctata (Saussure, 1860) (Paradoxosomatidae), Prosopodesmus jacobsoni Silvestri, 1910 (Haplodesmidae), Trachyjulus calvus (Pocock, 1893a), Glyphiulus granulatus (Gervais, 1847) (Cambalopsidae), Trigoniulus corallinus (Gervais, 1842), Leptogoniulus sorornus (Butler, 1876), Epitrigoniulus cruentatus (Brölemann, 1903) (Pachybolidae), Paraspirobolus lucifugus (Gervais, 1837) (Spirobolellidae), Cylindroiulus britannicus (Verhoeff, 1891), Cylindroiulus truncorum (Silvestri, 1896) (Julidae), and Rhinotus purpureus (Pocock, 1894) (Siphonotidae). Among the 27 federative units in Brazil, 21 states present at least one record of a non-native species. Orthomorpha coarctata was the most widely distributed species, occurring in 15 states. Glyphiulus granulatus (state of Rio Grande do Sul), C. truncorum (São Paulo), and R. purpureus (Amazonas) were recorded from only one Brazilian state. The Southeast region concentrates most of the compiled records (42,6%) and richness by grid (5-7 species), mainly in urban areas of the states of Rio de Janeiro and São Paulo.
