Abstract Over the last three decades, stroke care has undergone significant transformations mainly driven by the introduction of reperfusion therapy and the organization of systems of care. Patients receiving treatment through a well-structured stroke service have a much higher chance of favorable outcomes, thereby decreasing both disability and mortality. In this article, we reviewed the scientific evidence for stroke reperfusion therapy, including thrombolysis and thrombectomy, and its implementation in the public health system in Brazil.
Resumo Nas últimas três décadas, o tratamento do AVC sofreu transformações significativas, impulsionadas principalmente pela introdução das terapias de reperfusão e pela organização dos serviços de AVC. Os pacientes que recebem tratamento em um serviço de AVC bem estruturado têm uma probabilidade muito maior de resultados favoráveis, diminuindo assim a incapacidade funcional e a mortalidade. Neste artigo, revisamos as evidências científicas para as terapias de reperfusão do AVC, incluindo trombólise e trombectomia e sua implementação no sistema público de saúde no Brasil.

Selecting a polymer depends on its characteristics, the properties of the drug and of the remaining ingredients in the formulation. The drug, when incorporated into a polymeric matrix, can show several advantages when compared with its conventional form. In this context, this work describes the preparation and characterization of polyethylene glycol (PEG 4000) and its derivative particles loaded with benznidazole, as well as evaluates their trypanocidal activity. In this work, reactions to modify the PEG 4000 polymer and the subsequent incorporation of the benznidazole were made. The nuclear magnetic resonance (NMR) analysis confirmed the efficiency in modifying the PEG chains. The morphology of polymeric films was observed by atomic force microscopy (AFM) and showed considerable changes on the film organization. The acetylation of PEG favored the stability of the system and an increase in the zeta potential from ?14.83 to ?25.54 mV was observed. Although encapsulation efficiency values between 30.14 and 39.48% were found, the enhanced benznidazole dissolution profile by microparticles enables the use of lower drug concentrations. This fact can be proven by the increased trypanocidal effect of benznidazole when encapsulated in BP3 microparticles. Finally, the high selectivity of the formulations for trypanocidal action guarantees their safety as an alternative for the treatment of the Chagas disease.

The oil from the macaúba (Acrocomia aculeata) fruit, a native palm in Brazil, has a real potential for the industrial production of biodiesel. This report describes an experimental work devoted to monitor the chemical behavior of the fatty acid methyl esters from oil of the macaúba fruit kernel and the leaching of metal elements into the biodiesel put in contact with the ASTM A283 grade C and API X65 carbon steels, used to build fuel storage tanks and biodiesel pipelines. The mean values of esters contents points that the analytical precision from atomic absorption measurements was not enough to support a statistically reliable interpretation of the chemical leaching of metals into the biodiesel; a more accurate set of data must be obtained with more sensitive chemical analytical techniques. From the Mössbauer data, the surface of the non - galvanized steels in contact with this biodiesel for up to 105 days tend to be somehow protected against most oxidative reactions even in direct contact with air, although few amount magnetite (Fe3O4) was detected.

Abstract Background: In Brazil, we are facing an alarming epidemic scenario of Yellow fever (YF), which is reaching the most populous areas of the country in unvaccinated people. Vaccination is the only effective tool to prevent YF. In special situations, such as patients with chronic immune-mediated inflammatory diseases (CIMID), undergoing immunosuppressive therapy, as a higher risk of severe adverse events may occur, assessment of the risk-benefit ratio of the yellow fever vaccine (YFV) should be performed on an individual level. Main body of the abstract: Faced with the scarcity of specific orientation on YFV for this special group of patients, the Brazilian Rheumatology Society (BRS) endorsed a project aiming the development of individualized YFV recommendations for patients with CIMID, guided by questions addressed by both medical professionals and patients, followed an internationally validated methodology (GIN-McMaster Guideline Development). Firstly, a systematic review was carried out and an expert panel formed to take part of the decision process, comprising BRS clinical practitioners, as well as individuals from the Brazilian Dermatology Society (BDS), Brazilian Inflammatory Bowel Diseases Study Group (GEDIIB), and specialists on infectious diseases and vaccination (from Tropical Medicine, Infectious Diseases and Immunizations National Societies); in addition, two representatives of patient groups were included as members of the panel. When the quality of the evidence was low or there was a lack of evidence to determine the recommendations, the decisions were based on the expert opinion panel and a Delphi approach was performed. A recommendation was accepted upon achieving ≥80% agreement among the panel, including the patient representatives. As a result, eight recommendations were developed regarding the safety of YFV in patients with CIMID, considering the immunosuppression degree conferred by the treatment used. It was not possible to establish recommendations on the effectiveness of YFV in these patients as there is no consistent evidence to support these recommendations. Conclusion: This paper approaches a real need, assessed by clinicians and patient care groups, to address specific questions on the management of YFV in patients with CIMID living or traveling to YF endemic areas, involving specialists from many areas together with patients, and might have global applicability, contributing to and supporting vaccination practices. We recommended a shared decision-making approach on taking or not the YFV.

ABSTRACT The constant use of chemical insecticides for Aedes aegypti control has caused resistance in the mosquito populations. Thus, the objective of this study was to analyze the larvicidal potential of extracts and fractions of plants on A. aegypti larvae. The analysis included sixty one extracts and twenty five fractions of fifty botanical species at concentrations of 0.25; 0.12; 0.06 to 0.03 mg mL-1; 4 replications and one negative control of dechlorinate water and 1% DMSO; and a positive control with rotenone. The toxicity index in descending order with LC50 for the most active of the extracts selected were ethanol extract of Ormosea arborea (0.111 mg mL-1) seeds and ethanol extracts of leaves such as Piper hispidum (0.169 mg mL-1), Solanum variabile (0.188 mg mL-1), O. arborea (0.238 mg mL-1), Turnera umifolia (0.242 mg mL-1) and Piper hispidum (0.567 mg mL-1). For plant fractions, the most active were chloroform (0.192 mg mL-1) and hexane (0.342 mg mL-1) P. aduncum leaves, hexane fraction (0.415 mg mL-1) and methanol extract (0.625 mg mL-1) of Spermacocea latifolia leaves. Regarding the extract of T. umifolia single species, there is no bibliographic report on their degree of efficiency as an insecticide.

ABSTRACT Freshwater algae are rich sources of structurally biologically active metabolites, such as fatty acids, steroids, carotenoids and polysaccharides. Among these metabolites, lectins stand out. Lectins are proteins or glycoproteins of non-immune origin which bind to carbohydrates or glycoconjugates, without changing ligand structure. Many studies have reported on the use of Spirogyra spp. as effective bioindicators of heavy metals; however, reports on Spirogyra molecular bioprospecting are quite limited. Therefore, this study aimed to detect, isolate, purify and characterize a lectin present in the freshwater green algae Spirogyra. Presence of the lectin protein in the extract was detected by hemagglutination assays. Subsequently, the protein extract was subjected to a sugar inhibition assay to identify the lectin-specific carbohydrate. Following this, the extract was applied to a guar gum column to afford the pure lectin. The lectin was inhibited by N-acetyl-glucosamine and N-acetyl-beta-D-mannose, but more strongly by D-galactose. The apparent molecular mass of the purified lectin was evaluated by Polyacrylamide gel electrophoresis in the presence of sodium dodecyl sulfate (SDS-PAGE). Electrophoretic analysis revealed a single protein band with an apparent molecular mass of 56 kDa. Thus, it could be concluded that a lectin was purified from Spirogyra spp.

OBJETIVO: Determinar a prevalência de sintomas relacionados à asma, à rinite e ao eczema atópico em adolescentes (13-14 anos, AD) residentes em sete cidades brasileiras com o questionário escrito (QE) padronizado do International Study of Asthma and Allergies in Childhood (Isaac) e verificar a tendência temporal passados nove anos da última avaliação do Isaac fase 3 (ISF3). MÉTODOS: O QE Isaac foi respondido por 20.099 AD (13-14 anos) moradores em centros das regiões Norte, Nordeste, Sudeste e Sul. Os índices obtidos foram comparados aos do ISF3 com o teste não paramétrico (qui-quadrado ou Fisher) e foi estabelecida a taxa de incremento/decremento anual para cada um dos centros segundo o sintoma avaliado. RESULTADOS: Em relação ao ISF3, considerando-se os dados nacionais, houve queda da prevalência média de asma ativa (18,5% vs. 17,5%) com elevação da frequência de asma grave (4,5% vs. 4,7%) e de asma diagnosticada por médico (14,3% vs. 17,6%). Aumento da prevalência de rinite e rinoconjuntivite e de eczema flexural também ocorreram. CONCLUSÕES: A prevalência de asma, rinite e eczema atópico no Brasil foi variável. Valores mais altos, sobretudo de asma e eczema, foram observados nos centros localizados mais próximos ao Equador.
OBJECTIVE: To determine the prevalence of symptoms of asthma, rhinitis, and atopic eczema in adolescents (AD; 13-14 years) living in seven Brazilian cities, by applying the standardized written questionnaire (WQ) of the International Study of Asthma and Allergies in Childhood (ISAAC), and to evaluate the time trend nine years after the last assessment of ISAAC phase 3 (ISP3). METHODS: The ISAAC-WQ was answered by 20,099 AD from the Northern, Northeastern, Southeastern, and Southern Brazilian regions. Values obtained were compared to those observed in ISP3 using nonparametric (chi-squared or Fisher) tests, and the ratio of annual increment/decrement was established for each of the centers, according to the symptom assessed. RESULTS: Considering the national data and comparing to values of ISP3, there was a decrease in the mean prevalence of active asthma (18.5% vs. 17.5%) and an increase in the frequency of severe asthma (4.5% vs. 4.7%) and physician-diagnosed asthma (14.3% vs. 17.6%). An increase in prevalence of rhinitis, rhinoconjunctivitis, and atopic eczema was also observed. CONCLUSIONS: The prevalence of asthma, rhinitis, and atopic eczema in Brazil was variable; higher prevalence values, especially of asthma and eczema, were observed in regions located closer to the Equator.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Analysis of Gas Turbine Performance with Inlet Air Cooling Techniques Applied to Brazilian Sites

Abstract: For geographic regions where significant power demand and highest electricity prices occur during the warm months, a gas turbine inlet air cooling technique is a useful option for increasing output. Inlet air cooling increases the power output by taking advantage of the gas turbine's feature of higher mass flow rate, due the compressor inlet temperature decays. Industrial gas turbines that opera te at constant speed are constant-volume-flow combustion machines. As the specific volume of air is directly proportional to the temperature, the increases of the air density results in a higher air mass flow rate, once the volumetric rate is constant. Consequently, the gas turbine power output enhances. Different methods are available for reducing compressor intake air temperature. There are I two basic systems currently available for inlet cooling. The first and most cost-effective system is the evaporative cooling. Evaporative coolers make use of the evaporation of water to reduce the gas turbine inlet air temperature. The second system employs two ways to cool the inlet air: mechanical compression and absorption. In this method, the cooling medium flows through a heat exchanger located in the inlet duct to remove heat from the inlet air. In the present study, a thermodynamic analysis of gas turbine performance is carried out to calculate heat rate, power output and thermal efficiency at different inlet air temperature and relative humidity conditions. The results obtained I with this model are compared with the values of the condition without cooling herein named of Base-Case. Then, the three cooling techniques are computationally implemented and solved for different inlet conditions (inlet temperature and relative humidity). In addition, the gas turbine was tested under different cooling methods for two Brazilian sites, and comparison between chiller systems (mechanical and absorption) showed that the absorption chiller provides the highest increment in annual energy generation with lower unit energy costs. On the other hand, evaporative cooler offered the lowest unit energy cost but associated with a limited cooling potential.

Desenvolvimento de filmes de nananocompósitos polipropileno/argila organofílica para embalagens

Neste trabalho foram preparados filmes de nanocompósitos de polipropileno (PP) contendo 1% em massa de bentonita modificada organicamente com tecnologia nacional (APO). Polipropileno enxertado com anidrido maleico (PP-g-AM) e copolímero de etileno e álcool vinílico (EVOH) foram utilizados como agentes compatibilizantes. Os nanocompósitos foram preparados através da intercalação no estado fundido em extrusora de filme plano. A influência do tipo e do teor de compatibilizante e do tipo de polipropileno, na morfologia, propriedades mecânicas e de barreira ao vapor d'água dos referidos filmes foi investigada. Os dados de difração de raio X evidenciam que apesar da morfologia dos nanocompósitos ter sido afetada pelo tipo de compatibilizante, esta praticamente não variou com o teor de compatibilizante utilizado. As morfologias e as propriedades de barreira ao vapor d'água dos nanocompósitos preparados com os polipropilenos PP H401 e PP H07D-00 são semelhantes. Entretanto, aqueles preparados com PP H07D-00 apresentaram melhores propriedades mecânicas, excetuando a resistência da termossoldagem à tração, do que os preparados com a matriz polimérica PP H401, e a transparência dos nanocompósitos preparados com PP H07D-00 foi idêntica à da matriz polimérica sem argila. Os resultados indicam que os filmes de nanocompósitos PP H07D-00 compatibilizados com 15% de PP-g-AM e contendo 1% de argila organofílica APO podem ser promissores para aplicação em embalagens.
This work deals with polypropylene (PP) nanocomposite films containing 1 wt. (%) bentonite clay organically modified with national technology (APO). Maleic anhydride grafted polypropylene (PP-g-MA) and ethylene vinyl alcohol (EVOH) were used as compatibilizing agents. Nanocomposites were prepared by melt intercalation in a flat film extruder. An investigation was made of the influence from the type and content of compatibilizer and type of polypropylene on the morphology, mechanical properties and barrier properties to water vapor of these films. X-ray diffraction data show that while the nanocomposite morphology was affected by the type of compatibilizer, it was relatively insensitive to its content. The morphologies and barrier properties to water vapor of the nanocomposites prepared with the two polypropylenes (PP H401 and PP H07D-00) were similar. However, except for the heat sealing properties, those prepared with PP H07D-00 showed better mechanical properties. The transparency of P715AMAPO nanocomposite was similar to that of the P715AM matrix. The results indicated that PP H07D-00 nanocomposite films compatibilized with 15% PP-g-AM and containing 1% APO organoclay may be promising for packaging applications.

OBJETIVOS: Analisar as frequências de hospitalização e de atendimentos em serviços de urgência ocorridas em crianças e adolescentes antes e após o ingresso em programa de asma no Brasil. MÉTODOS: Prontuários de 608 pacientes menores de 15 anos e com asma foram avaliados retrospectivamente. As frequências de hospitalização por asma e de atendimentos em serviços de urgência para episódios agudos de broncoespasmo foram avaliadas nos períodos anterior e posterior à admissão no programa que disponibiliza medicamentos, preconiza abordagem integral do paciente e manejo profilático de acordo com a Global Initiative for Asthma (GINA). O período de observação antes do programa (AP) teve a duração de 12 meses enquanto o depois do programa (DP) variou de 12 a 36 meses. RESULTADOS: No período AP, ocorreram 895 hospitalizações e 5.375 atendimentos em serviços de urgência, e no período DP, ocorreram 180 hospitalizações e 713 atendimentos na urgência. O teste t de Student para amostras pareadas e o modelo de regressão para observações dependentes identificaram efeito significativo do programa em relação às variáveis estudadas (p = 0,000). CONCLUSÕES: A adoção das recomendações do GINA levou à redução significativa nas frequências de hospitalização e atendimentos na urgência em crianças e adolescentes com asma.
OBJECTIVES: To assess the frequency of hospitalizations and emergency department visits of children and adolescents before and after the enrolment in an asthma program. METHODS: Medical records of 608 asthmatics younger than 15 years were assessed retrospectively. The frequency of hospitalizations and emergency department visits caused by exacerbations were evaluated before and after enrolment in an asthma program. Patients were treated with medications and a wide prophylactic management program based on the Global Initiative for Asthma (GINA). The before asthma program (BAP) period included 12 months before enrollment, whereas the after asthma program (AAP) period ranged from 12 to 56 months after enrollment. RESULTS: In the BAP period, there were 895 hospitalizations and 5,375 emergency department visits, whereas in the AAP period, there were 180 and 713, respectively. This decrease was significant in all statistical analyses (p = 0.000). CONCLUSIONS: Compliance with the GINA recommendations led to a significant decrease in the frequency of hospitalizations and emergency department visits in children and adolescents with asthma.

OBJETIVOS: Avaliar os valores de cortisol basal em asmáticos persistentes em uso de propionato de fluticasona inalatório na dose de 200 ou 300 mcg/dia. MÉTODOS: O diagnóstico e a classificação da gravidade da asma basearam-se nas recomendações do Global Initiative for Asthma. Pacientes menores de 11 anos receberam fluticasona na dose de 200 mcg/dia, e aqueles com mais de 11 anos receberam 300 mcg/dia. Após 10 semanas de tratamento, a dosagem do cortisol foi realizada para avaliação da função adrenal. RESULTADOS: Foram avaliados 41 pacientes (65,9% do sexo masculino) entre 6 e 18 anos. Não houve diferença significativa entre as médias de cortisol basal nos pacientes que receberam 200 mcg/dia de propionato de fluticasona (n = 13) e naqueles que receberam 300 mcg/dia (n = 28). CONCLUSÕES: Os achados mostram que doses baixas a moderadas de propionato de fluticasona não causam supressão adrenal.
OBJECTIVES: To evaluate basal plasma cortisol in persistent asthmatics on inhaled fluticasone propionate 200 mcg/day and 300 mcg/day. METHODS: Asthma diagnosis and classification was based on Global Initiative for Asthma recommendations. Patients aged 11 years old or less received fluticasone propionate 200 mcg/day and those older than 11 years received 300 mcg/day. After 10 weeks of treatment, plasma cortisol levels were monitored to evaluate the hypothalamic-pituitary-adrenal axis. RESULTS: Forty-one patients (65.9% males) aged 6 to 18 years old were evaluated. No statistical differences were found between plasma cortisol levels in patients who received 200 mcg/day (n = 13) and those who received 300 mcg/day (n = 28). CONCLUSIONS: Our results show that low and moderate doses of fluticasone propionate do not cause adrenal suppression.

Influence of NO2 and metal ions on oxidation of aqueous-phase S(IV) in atmospheric concentrations

A reação de oxidação de S(IV) em fase aquosa foi estudada em laboratório em presença de NO2 dos íons metálicos Fe(III), Mn(II), e Cr(VI) sob condições experimentais controladas (pH, T, concentração dos reagentes, etc.). Na presença de corrente de ar com NO2 (15 ou 130 ppbv) a reação de oxidação de S(IV) ocorreu mais lentamente do que na presença de ar purificado, para uma concentração inicial de S(IV) de 10-4 mol/L. Ao contrário, para concentração inicial de S(IV) dez vezes menor ([S(IV)]° = 10-5 mol/L) a reação ocorreu mais rapidamente na presença de NO2. A explicação está relacionada com o equilíbrio envolvendo a formação de espécies intermediárias de longa vida, que impedem o prosseguimento da reação, porém a depender das concentrações relativas de S(IV) e NO2, essas espécies se decompõem deslocando o equilíbrio no sentido de formação de sulfato. A adição dos íons Fe(III), Mn(II) ou Cr(VI) em presença de corrente de ar com NO2 indicou atividade catalítica para esses íons, em todas as condições nas quais a oxidação de S(IV) foi investigada.
An investigation was made of the influence of atmospheric concentrations (15 or 130 ppbv) of NO2 on the aqueous-phase oxidation rate of S(IV) in the presence and absence of Fe(III), Mn(II) and Cr(VI) metal ions under controlled experimental conditions (pH, T, concentration of reactants, etc.). The reaction rate in the presence of the NO2 flow was slower than the reaction rate using only clean air with an initial S(IV) concentration of 10-4 mol/L. NO2 appears to react with S(IV), producing a kind of inhibitor that slows down the reaction. Conversely, tenfold lower concentrations of S(IV) ([S(IV)]º = 10-5 mol/L) caused a faster reaction in the presence of NO2 than the reaction using purified air. Under these conditions, therefore, the equilibrium shifts to sulfate formation. With the addition of Fe(III), Mn(II) or Cr(VI) in the presence of a NO2 flow, the reaction occurred faster under all the conditions in which S(IV) oxidation was investigated.
