Abstract Background: Recent studies suggest that follow-up paracentesis (FuP) in cases of spontaneous bacterial peritonitis (SBP) should only be performed if there is a clinical and/or analytic worsening. We aimed to evaluate which patients with SBP would benefit from the FuP. Métodos: This retrospective study included consecutive patients diagnosed with SBP between January 2011 and June 2018 in our tertiary center. Clinical and analytical data were obtained at baseline and on the third day of antibiotic therapy. An adequate response on the third day of treatment was defined by a decrease of ≥25% in the neutrophil count of the ascitic fluid (AF). Results: Ninety-six episodes of PBE in 75 patients (79% male sex, mean age 61 ± 11 years old) were included. At admission, a higher serum neutrophil count (p = 0.043), a lower level of serum total proteins (p = 0.040), a positive culture in AF (p < 0.001) and a previous diagnosis of diabetes mellitus (p = 0.035) were related to inadequate response (IR). At day 3, acute kidney injury (p = 0.023), C-reactive protein >100 mg/L (p < 0.001), the presence of fever (p = 0.047) and abdominal pain (p < 0.001) were also associated with IR. In multivariate analysis, the presence of respiratory insufficiency (OR = 16.403; 95% CI: 2.315-116.222; p = 0.005) and abdominal pain (OR = 10.381; 95% CI: 1.807-59.626; p = 0.009) at admission, serum white blood cell count >9 × 109 (OR = 5.832; 95% CI: 1.275-26.669; p = 0.023), and CRP >100 mg/L (OR = 5.043; 95% CI: 1.267-20.076; p = 0.022) at day 3 of antibiotic therapy were predictors of IR. The predictive model presented good accuracy [AUROC of 0.893 (p < 0.001)] - a cutoff of 0.090 had a sensitivity, specificity, positive predictive value, and negative predictive value for IR of 97, 46, 83, and 77%, respectively. Conclusions: The performance of FuP on day 3 after the beginning of empiric therapy should be individualized, according to clinical and analytic variables of this predictive model.

Resumo Introdução: Estudos recentes têm sugerido a realização de paracentese de seguimento ao 3º dia, na peritonite bacteriana espontânea (PBE), apenas em doentes com agravamento clínico e/ou analítico. Este trabalho pretende avaliar quais os doentes em que a paracenteses ao 3º dia se mantém essencial. Methods: Estudo retrospetivo realizado em centro terciário, com inclusão dos doentes com PBE entre janeiro de 2011 e junho de 2018. Dados clínicos e analíticos foram obtidos à data de admissão e ao 3° dia de antibioterapia. A resposta terapêutica foi considerada adequada quando a contagem de neutrófilos no líquido ascítico era ≥25%. Resultados: Foram incluídos 96 episódios de PBE correspondentes a 75 doentes (79% homens, com idade media de 61 ± 11 anos). À admissão, a presença de diabetes mellitus (p = 0.035), uma maior contagem de neurófilos séricos (p = 0.043), nível inferior de proteínas séricas totais (p = 0.040) e positividade nas culturas de líquido ascítico total (p < 0.001) relacionaram-se com inadequada resposta (IR). Ao 3° dia de antibioterapia, a presença de lesão renal aguda (p = 0.023), proteína C reativa >100 mg/L (p < 0.001), febre (p = 0.047) e dor abdominal (p < 0.001) foram também associados a IR. Na análise multivariada, a presença de insuficiência respiratória (OR = 16.403; 95% CI: 2.315-116.222; p = 0.005) e dor abdominal (OR = 10.381; 95% CI: 1.807-59.626; p = 0.009) à admissão, contagem séria de leucócitos >9 ×109 (OR = 5.832; 95% CI: 1.275-26.669; p = 0.023) e PCR >100 mg/L (OR = 5.043; 95% CI: 1.267-20.076; p = 0.022) ao 3° dia de antibioterapia foram preditores de IR. O modelo preditivo apresentado apresenta boa acuidade [AUROC de 0.893 (p < 0.001)] - para um cutoff de 0.090 tem uma sensibilidade, especificidade, valor preditivo positivo e valor preditivo negativo para IR de 97, 46, 83, e 77%, respetivamente. Conclusões: De acordo com o nosso modelo, a realização de paracenteses de seguimento ao 3 dia após início de antibioterapia empírica deverá ser individualizada, de segundo as variáveis clínicas e analíticas apresentadas.

ABSTRACT Currently, there are no biomarkers for Chikungunya fever (CHIK) in clinical practice that can accurately predict the severity or chronification of the disease. The aim of this study is to evaluate the existing literature on biomarkers related to the severity and chronification of CHIK. In this sense, a systematic review was conducted based on the PRISMA Statement guideline. Articles that described the association of biomarkers with the evolution of the disease (severity or chronification), published until August 20th 2019 were considered eligible. The search was carried out in the PubMed, Scopus, Virtual Health Library (VHL) and Science Direct databases. After searching the databases, 17 articles were added to the review, and after analyzing the articles, several biomarkers were associated with severity, such as increased levels of IL-6, IP-10, IL-1b, MIG, MCP-1, and reduced levels of RANTES and IL-8 or chronification, such as increased levels of IL-6, TNF-α, MCP-1, IL-12, INF-α, IL-13, INF-γ, GM-CSF, CRP, IL-1a, IL-15, Factor VII, IP-10, IL-10, IL-4, IL-1RA, IL-8, MIP-1α, MIP-1β, ferritin, MIG, ESR, NO, malondialdehyde, and reduced levels of RANTES, ferritin, eotaxin, HGF, IL-27, IL-17A, IL-29, TGF-β, IL-10, and thiols. IL-6, CRP and TNF-α were included in the meta-analysis to assess the relationship with chronification, although they did not reach statistical significance. It was concluded that several biomarkers showed a relationship with severity and chronification of CHIK; the search for these biomarkers can reveal prognostic factors and important therapeutic targets for the treatment of the disease.

Resumo O presente estudo objetivou analisar diferentes metodologias indiretas para mensuração da erodibilidade do solo, bem como caracterizar a sua variabilidade espacial na Bacia Hidrográfica do Rio Pântano (BHRP), Mato Grosso do Sul. Avaliou-se nove metodologias/adaptações aplicadas aos dados de 103 amostras de solo abrangendo os principais tipos existentes na bacia. Os dados passaram por uma proposta de validação e posteriormente por análise estatística descritiva e de correlação. Realizou-se também a análise da dependência espacial e mapeamento por krigagem. As metodologias que melhor representaram a erodibilidade do solo na BHRP foram dadas pelas propostas de Sharpley e Williams (Latossolos Vermelho-Escuros e Planossolos), Wischmeier e Smith (Latossolos Roxos) e, Renard (Podzólicos Vermelho-Escuros e Vermelho-Amarelos). O mapa final, indicou erodibilidade média (46,4% da BHRP), baixa (45,1%), muito baixa 0,5% e, muito alta 7,9% da área da bacia. Concluiu-se que o uso de uma única metodologia pode subestimar ou superestimar a erodibilidade do solo.

Abstract The present study analyzed different indirect methodologies for measuring soil erodibility and characterized the spatial variability of soil credibility in the Pântano River Hydrographic Basin (PRHB), state of Mato Grosso do Sul, Brazil. Nine methodologies/adaptations were tested in 103 soil samples collected covering the main soil types in the hydrographic basin. The data were submitted to a validation proposal and underwent descriptive and correlation statistical analyses. A spatial dependence analysis and mapping by kriging was also carried out. The methodologies that best represented the erodibility estimates at PRHB were those by Sharpley and Williams (Latossolos Vermelho-Escuros [Oxisols] and Planossolos [Alfisols]), Wischmeier and Smith (Latossolos Roxos [Oxisols]), and Renard (Podzólicos Vermelho-Escuros and Podzólicos Vermelho-Amarelos [Ultisols]). The final map indicated erodibility medium (46.4% of PRHB), low (45.1%), very low (0.5%) and very high (7.9%). The findings indicated that the use of a single indirect methodology may underestimate or overestimate the soil erodibility.

Resumen El presente estudio tuvo como objetivo analizar diferentes metodologías indirectas para medir la erosionabilidad del suelo, así como caracterizar su variabilidad espacial mediante el uso de técnicas geoestadísticas. Para ello, se adoptó como estudio de caso la Cuenca del Río Pântano (CRP) en el estado de Mato Grosso do Sul, Brasil. Las metodologías que mejor representaron las estimaciones de la erosionabilidad em la CRP fueron las propuestas de Sharpley y Williams (Latossolo Rojo Distrófico Álico y Planossolo Álico), Wischmeier y Smith (Latossolo Rojo Distrófico) y Renard (Argissolo Rojo Distrófico y Argissolo Rojo-Amarillo). El mapa final, que se compuso de un mosaico de las metodologías que mejor representaban cada tipo de suelo, indicó las siguientes clases de erosionabilidad: medio (46,4% de la CRP, predominantemente en Argissolos Rojos); bajo (45,1%, predominantemente en Latossolos); muy bajo 0,5% y muy alto 7,9% de la cuenca (predominantemente en Argissolos Rojo-Amarillos).

Introdução: A cápsula de patência (CP) é geralmente realizada para avaliar a patência do intestino delgado, previamente à realização de cápsula endoscópica (VCE), em doentes com doença de Crohn (DC) diagnosticada. Notavelmente, a taxa de retenção da CP nos doentes com DC é significativamente superior à taxa global de retenção de VCE. Os autores pretendem avaliar a patência do intestino delgado e determinar a localização precisa das CP retidas nos doentes com DC, determinar a taxa de falsos positivos da avaliação com RFIT scanner, bem como avaliar possíveis fatores associados. Métodos: Estudo prospetivo de um centro único, incluindo doentes com DC com indicação clínica para VCE. A PillCam® CP foi realizada em todos os doentes para avaliar a patência do intestino delgado. Nos doentes com deteção positiva da CP através de um radiofrequency identification tag (RFID) scanner, 30 horas após a ingestãofoi realizada uma tomografia computadorizada abdominal (TC) de forma a determinar a localização precisa da cápsula. Resultados: Foram incluídos 54 doentes. A taxa de retenção da CP 30h, na avaliação com RFIT scanner, após a ingestão foi de 20% (11). Estes doentes foram então submetidos a TC abdominal, que detetou a CP no intestino delgado apenas em 5 (9%) doentes. Níveis mais elevados de PCR, doença penetrante e história de cirurgia abdominal foram associados com aumento do risco de retenção da CP (p = 0,007, p = 0,011 e p = 0,033, respetivamente). Na análise multivariada, houve uma associação independente entre retenção da cápsula do intestino delgado e níveis de PCR > 5 mg/dL (OR = 15,5; p = 0,03). Conclusões: A taxa de retenção da CP no intestino delgado (9%) foi consideravelmente menor do que a previamente reportada. Estes resultados mostram que, com este protocolo, os casos falso positivos do RFIT scanner ou radiografia abdominal simples podem ser evitados. Isso pode contribuir para uma aplicação mais extensa da VCE sem o risco de retenção do intestino delgado.

Background and Purpose: Patency capsule (PC) examination is usually performed - previously to capsule endoscopy - to evaluate small-bowel patency in patients with established Crohn’s disease (CD). The reported PC retention rate is significantly higher than expected. Our aims were to assess small-bowel patency, to determine the precise location of the retained PC in patients with CD, and to determine the false positive rate of evaluation with a radiofrequency identification tag (RFIT) scanner. Methods: This is a prospective single-center study including CD patients with clinical indication for small-bowel capsule endoscopy. PillCam® PC examination was performed on all patients to assess smallbowel patency. On all patients with a positive identification of the PC using an RFIT scanner, 30 h after ingestion, an abdominal CT was performed in order to identify its precise location. Results: Fifty-four patients were included. The PC retention rate, according to evaluation with the RFIT scanner, was 20% (in 11 patients) 30 h after ingestion. These patients were then submitted to abdominal CT, which revealed that there was small-bowel retention in 5 cases (9%). Higher CRP levels, penetrating disease, and a history of abdominal surgery were associated with an increased risk of PC retention (p = 0.007, p = 0.011, and p = 0.033, respectively). On multivariate analysis, there was an independent association between small-bowel PC retention and CRP levels > 5 mg/dL (OR = 15.5; p = 0.03). Discussion: The small-bowel PC retention rate (9%) was considerably lower than those found in previous reports. Our results show that, with this protocol, the false-positive cases of RFIT scans or plain abdominal Xrays may be avoided. This may contribute to more extensive application of capsule endoscopy without the risk of smallbowel retention.

Introdução: A deficiência de vitamina D é mais comum na doença inflamatória intestinal (DII) que na população geral. Contudo, existem dados controversos sobre fatores preditivos da deficiência de vitamina D e a potencial associação com a atividade da doença. Os objetivos deste estudo foram determinar a prevalência e fatores preditivos da deficiência de vitamina D e aferir possível associação à atividade da doença. Métodos: Desenhou-se um estudo observacional prospetivo incluindo doentes com DII entre janeiro e julho/2016. Foram consideradas as orientações da The Endocrine Society para definir níveis de 25-hidroxivitamina D (25-OH-D) sérica como: deficientes (< 20 ng/mL, sendo <10 ng/mL deficiência grave [DG]), insuficientes (21-29 ng/mL) e adequados (> 30 ng/mL). Resultados: Foram incluídos 152 doentes (52% homens; 47.2 ± 17.3 anos), dos quais 70% com Doença de Crohn (DC). Do total, 37% estavam medicados com immunossupressores e 17% com biológicos. A maioria (88.2%) estava em ambulatório. O nível sérico de 25-OH-D foi 17.1 ± 8 ng/mL (DC: 16.7 ± 8 ng/mL vs. Colite ulcerosa: 17.6 ± 7 ng/mL, p = 0.1). Verificaram-se níveis inadequados em 90.8% (deficiência: 68.4%; insuficiência: 22.4%). Registou-se correlação negativa significativa entre níveis de 25-OH-D e idade (r = -0.2, p = 0.04), proteína C-reativa (PCR) (r = -0.22, p = 0.004) e índice Harvey-Bradshaw (iHB) (r = -0.32, p = 0.001). Doentes com DG apresentaram níveis mais elevados de PCR (0.6 vs. 1.4 mg/dL, p = 0.03), velocidade de sedimentação (VS) (22 vs. 31 mm/h, p = 0.03) e iHB (2 vs. 5, p < 0.001), e mais baixos de hemoglobina (13.6 vs. 12.7 g/dL, p = 0.02). Não se verificou associação entre deficiência de vitamina D e sexo, tipo, extensão e duração da doença, cirurgia, e outras medidas de atividade da doença como VS, hemoglobina (estas duas exceto para DG), calprotectina fecal ou classificação Truelove e Witts. Conclusões: Registou-se prevalência alta de níveis inadequados de vitamina D na DII, particularmente de deficiência (68.4%). Parece existir associação entre níveis mais baixos de vitamina D e maior atividade da doença, nomeadamente na DC.

Background and Aims: Vitamin D deficiency is more common in inflammatory bowel disease (IBD) patients than in the general population. However, there are conflicting data about predictive factors of vitamin D deficiency and its potential association with disease activity. The aims of this study were to determine the prevalence and predictive factors of vitamin D deficiency and to evaluate a possible association with disease activity. Methods: A prospective observational study was conducted, including patients with IBD from January to July 2016. The Endocrine Society guidelines were considered for defining levels of serum 25-hydroxyvitamin D (25-OH-D) as follows: deficient (< 20 ng/mL, < 10 ng/mL being severe deficiency), insufficient (21-29 ng/mL), and adequate (> 30 ng/mL). Results: A total of 152 patients (52% men; 47.2 ± 17.3 years) were included, of whom 70% had Crohn’s disease (CD). Thirty-seven percent of patients were on immunosuppressors and 17% were on biologics. The majority were outpatients (88.2%). Mean 25-OH-D levels were 17.1 ± 8 ng/mL (CD: 16.7 ± 8 ng/mL vs. ulcerative colitis: 17.6± 7 ng/mL, p = 0.1). Inadequate levels were present in 90.8% of patients (deficiency: 68.4%; insufficiency: 22.4%). A significant negative correlation between 25-OH-D levels and age (r = -0.2, p = 0.04), C-reactive protein (CRP) levels (r = -0.22, p = 0.004), and Harvey-Bradshaw index (HBi) (r = -0.32, p = 0.001) was found. Patients with severe deficiency showed a higher CRP (0.6 vs. 1.4 mg/dL, p = 0.03), erythrocyte sedimentation rate (ESR) (22 vs. 31 mm/h, p = 0.03), and HBi (2 vs. 5, p < 0.001) and lower hemoglobin (13.6 vs. 12.7 g/dL, p = 0.02). There was no association between vitamin D deficiency and gender, type, extent, and duration of disease, surgery, and other measures of disease activity, such as ESR, haemoglobin (these 2 items except for severe deficiency), fecal calprotectin, or Truelove and Witts classification. Conclusions: There is a high prevalence of inadequate levels of vitamin D in IBD patients, particularly deficiency (68.4%). There seems to exist an association between lower levels of vitamin D and higher disease activity, especially in CD.

RESUMO Objetivo: Avaliar a efetividade de um programa de fortalecimento muscular progressivo com o uso de uma bola suíça em pacientes com espondilite anquilosante (EA). Métodos: Sessenta pacientes com EA foram randomizados em grupo intervenção (GI) ou grupo controle (GC). Os pacientes com EA fizeram oito exercícios com pesos livres em uma bola suíça duas vezes por semana durante 16 semanas. As avaliações foram feitas por um avaliador cego no início do estudo e após quatro, oito, 12 e 16 semanas com os seguintes instrumentos: teste de uma repetição máxima (1 RM), Basmi, Basfi, HAQ-S, SF-36, teste de caminhada de seis minutos, Timed up and go test, Basdai, Asdas, dosagem de VHS e PCR e escala de Likert. Resultados: Houve uma diferença estatisticamente significativa entre os grupos em relação à força (capacidade no teste de 1 RM) nos seguintes exercícios: abdominal, remada, agachamento, tríceps e crucifixo invertido (p < 0,005); teste de caminhada de seis minutos (p < 0,001); Timed up and go test (p = 0,025); e escala de Likert (p < 0,001), todos com melhores resultados no GI. Não foram observadas diferenças entre os grupos em relação à avaliação da capacidade funcional com Basfi, HAQ-S, Basmi, SF-36, TUG, Asdas, VHS e dosagem de PCR. Conclusões: O fortalecimento muscular progressivo com uma bola suíça é efetivo em melhorar a força muscular e o desempenho na caminhada em pacientes com EA.

ABSTRACT Objective: The purpose was to evaluate the effectiveness of a progressive muscle strengthening program using a Swiss ball for AS patients. Methods: Sixty patients with AS were randomized into the intervention group (IG) or the control group (CG). Eight exercises were performed by the IG patients with free weights on a Swiss ball two times per week for 16 weeks. The evaluations were performed by a blinded evaluator at baseline and after 4, 8, 12 and 16 weeks using the following instruments: the one-repetition maximum test (1 RM), BASMI, BASFI, HAQ-S, SF-36, 6-minute walk test, time up and go test, BASDAI, ASDAS, ESR and CRP dosage and Likert scale. Results: There was a statistical difference between groups for: strength (1 RM capacity) in the following exercises: abdominal, rowing, squat, triceps and reverse fly (p < 0.005); 6-minute walk test (p < 0.001); timed up and go test (p = 0.025) and Likert scale (p < 0.001), all of them with better results for the IG. No differences were observed between the groups with respect to the functional capacity evaluation using the BASFI, HAQ-S, BASMI, SF-36, TUG, ASDAS, ESR and CPR dosage. Conclusions: Progressive muscle strengthening using a Swiss ball is effective for improving muscle strength and walking performance in patients with AS.

Abstract The host status of eighteen progenies (CRP 01-12 to CRP 16-12, CRP 19-12 and CRP 20-12) and two cultivars (FB 200 Yellow Master and FB 300 Araguari) of yellow passion fruit to Meloidogyne incognita race 2 was evaluated in a greenhouse experiment. The height and the biomass of the plants, besides the numbers of galls, eggs and second-stage juveniles in roots and soil were assessed after 70 days of cultivation of the genotypes and tomato (control) in plastic pots with 2 kg of soil infested with 5,000 eggs of the nematode. The reproduction factor of the pathogen (RF = Final population/Initial population) was calculated for classifying the genotypes as immune (RF = 0), resistant (RF < 1) or susceptible (RF ≥ 1). The cultivar FB 200 is resistant to M. incognita race 2 and can be cultivated in areas infested with this nematode.

Resumo O presente estudo investigou os fatores associados à anemia e deficiência de micronutrientes em análise transversal conduzida com 150 crianças de 11 a 14 meses atendidas em unidades básicas de saúde em Rio Branco, Acre. Amostras de sangue venoso foram obtidas para avaliar a ocorrência de anemia, deficiência de ferro (DF) e de vitaminas A (DVA) e B12 (DVB12). Modelos múltiplos de regressão de Poisson foram utilizados para identificar fatores associados à anemia. Anemia, DF, DVA e DVB12 foram observadas em 23%, 76%, 18% e 20% das crianças, respectivamente. Os fatores associados à anemia foram: não ser filho único, residir em domicílio sem acesso a TV a cabo ou internet, déficit de estatura para idade (E/I), introdução tardia da alimentação complementar superior a 240 dias, DVA, DVB12, e evidência de infecção vigente (proteína C reativa plasmática > 5 mg/L). Houve menor ocorrência de anemia entre crianças com peso ao nascer > 3.500g. Do total de crianças, 82% apresentaram pelo menos uma das deficiências de micronutrientes (DF, DVA, DVB12). Ações com ênfase em práticas alimentares saudáveis oportunas, melhor manejo de morbidades e suplementação com outros micronutrientes devem ser priorizadas no aprimoramento do serviço de puericultura da atenção básica à saúde deste município.

Multiple Poisson regression models were used to identify factors associated with anemia. Anemia, ID, VAD and B12D were observed in 23%, 76%, 18% and 20% of children, respectively. The factors associated with anemia were: do not be only child, living in households without access to cable TV or internet, stunting, late introduction of the complementary feeding (more than 240 days), VAD, VB12D, and current evidence of infection (plasma CRP > 5 mg/L). There was a lower prevalence of anemia among children with birth weight > 3,500 g. Overall, 82% of the study children had at least one of the micronutrient deficiencies (ID, VAD and VB12D). Actions with emphasis on timely and healthy feeding practices, better management of morbidities and supplementation with other micronutrients should be focused on the improvement of child care services at primary health care in this county.

There was a lower prevalence of anemia among children with birth weight > 3,500 g. Overall, 82% of the study children had at least one of the micronutrient deficiencies (ID, VAD and VB12D). Actions with emphasis on timely and healthy feeding practices, better management of morbidities and supplementation with other micronutrients should be focused on the improvement of child care services at primary health care in this county.

Introdução A cirurgia de revascularização do miocárdio (CRM) é uma das mais frequentes cirurgias realizadas em todo o mundo, muitos estudos vêm relatando os benefícios do treinamento físico para a melhora da capacidade funcional, porém há poucos estudos sobre os efeitos na função endotelial vascular. Objetivo Analisar os efeitos do treinamento físico sobre a função endotelial vascular em pacientes submetidos à CRM, isoladamente na fase tardia da reabilitação após seis meses de programa de reabilitação cardíaca (PCR). Métodos Foram incluídos pacientes que haviam sido submetidos à CRM no período máximo de um ano após a cirurgia. O PRC foi empregado durante seis meses consecutivos, com três sessões semanais. Todos os pacientes realizaram exames laboratoriais, teste de força muscular de uma repetição máxima (1-RM) para os membros superiores e inferiores, teste de caminhada de 6 min (TC6M) e avaliação da função endotelial através da técnica de vasodilatação mediada pelo fluxo. Resultados Onze pacientes iniciaram o PRC, porém nove pacientes o completaram. A média de idade foi de 66 anos (50 a 82 anos) e o sexo masculino foi predominante (55,6%). Houve mudanças significativas nos exames laboratoriais bioquímicos: aumento do colesterol total (Basal: 162 ± 31mg/dL vs. 195 ± 39mg/dL; P=0,012) e diminuição da hemoglobina glicada (Basal: 6,74 ± 1,64% vs. 6,26 ± 1,62%; P=0,028). A força muscular aumentou significativamente nos membros superiores e inferiores (P=0,030 e P=0,038, respectivamente); no TC6M observou-se um aumento significativo de 20% na distância percorrida (P=0,020) após seis meses consecutivos de treinamento e houve uma melhora na vasodilatação mediada pelo fluxo (Basal: 6,35 ± 3,92% vs. 6 meses: 9,90 ± 4,19%; P=0,026). Conclusão O treinamento combinado realizado em seis meses ajudou a melhorar a função endotelial e a capacidade funcional de pacientes sedentários que foram submetidos à CRM na fase tardia da reabilitação.

Introduction The coronary artery bypass graft surgery (CABG) is one of the most common surgeries performed worldwide, many studies have reported the benefits of physical training to improve functional capacity, however there are few studies about the effect of this training on vascular endothelial function. Objective To analyze the effects of physical training on vascular endothelial function in patients who underwent CABG surgery alone in the last phase of rehabilitation after six months of cardiac rehabilitation program (CRP). Methods Patients who underwent CABG surgery in a maximum period of one year after surgery were included in the study. The CRP was carried out during six consecutive months, with three sessions per week. All patients underwent biochemical blood tests, muscle strength testing of one repetition maximum (1-RM test) for upper and lower limbs, 6-minute walk test (6MWT), and evaluation of endothelial function through the flow-mediated vasodilation technique. Results Eleven patients started CRP, but only nine patients completed it. The mean age was 66 years old (50 to 82 years old), males being predominant (55.6%). There were significant changes in biochemical laboratory tests: increase on total cholesterol (Baseline: 162 ± 31mg/dL vs. 195 ± 39mg/dL; P=0.012), and decrease on glycated hemoglobin (Baseline: 6.74 ± 1.64% vs. 6.26 ± 1.62%; P=0.028). Muscle strength was significantly increased in upper and lower limbs (P=0.030, and P=0.038, respectively) and in 6MWT we observed a significant increase of 20% in distance traveled (P=0.020) after six consecutive months of training. There was also a flow-mediated vasodilation improvement (6.35 ± 3.92% vs. 9.90 ± 4.19%; P=0.026). Conclusion The combined training carried out in six months helped to improve endothelial function and functional capacity in sedentary patients submitted to CABG surgery in the last phase of cardiac rehabilitation.

Introducción La cirugía de revascularización miocárdica (CRM) es una de las cirugías más comunes que se realizan en todo el mundo, muchos estudios han reportado los beneficios del entrenamiento físico para mejorar la capacidad funcional, pero hay pocos estudios sobre el efecto en la función endotelial vascular. Objetivo Analizar los efectos del entrenamiento físico sobre la función endotelial vascular en los pacientes sometidos a CRM solo en la última fase de la rehabilitación después de seis meses del programa de rehabilitación cardíaca (PRC). Métodos Fueron incluidos los pacientes que habían sido sometidos a CRM con el período máximo de un año después de la cirugía. Los pacientes fueron sometidos a lo PRC por seis meses consecutivos, con tres sesiones semanales. Todos los pacientes realizaron pruebas de laboratorio, pruebas de fuerza muscular una repetición máxima (1-RM) para miembros superiores e inferiores, prueba de la marcha de 6 minutos (PM6M) y la evaluación de la función endotelial mediante la técnica de la vasodilatación mediada por flujo. Resultados Once pacientes iniciaron el PRC, pero nueve pacientes lo completaron. La edad promedio fue de 66 años (50-82 años) y los varones predominaron (55,6%). Hubo cambios significativos en las pruebas de laboratorio bioquímico: aumento en el colesterol total (basal: 162 ± 31 mg/dl vs. 195 ± 39 mg/dl; p = 0,012) y disminución de la hemoglobina glucosada (basal: 6,74 ± 1,64% % vs. 6,26 ± 1,62, p = 0,028). La fuerza muscular se incrementó significativamente en los miembros superiores e inferiores (P = 0,030 y P = 0,038, respectivamente) y en PM6M se observó un aumento significativo de 20% en la distancia cubierta (p = 0,020) después de seis meses consecutivos de entrenamiento y también la vasodilatación mediada por flujo ha mejorado (basal: 6,35 ± 3,92% vs. 9,90 ± 4,19%, p = 0,026). Conclusión El entrenamiento combinado realizado en seis meses ha mejorado la función endotelial y la capacidad funcional de los pacientes sedentarios sometidos a CRM en la última fase de la rehabilitación.

Introdução: A proteína-C reativa (CRP) e o Bedside Index for Severity in Acute Pancreatitis (BISAP) têm sido usados na avaliação de risco precoce de doentes com pancreatite aguda (AP). Objectivos: Nós avaliámos o valor prognóstico da CRP às 24 horas após a admissão hospitalar (CRP24) na mortalidade intrahospitalar (IM) na AP, individualmente e com o BISAP. Materiais e Métodos: Este estudo coorte retrospetivo incluiu 134 doentes com AP de um hospital português em 2009-2010. A acuidade prognóstica foi avaliada usando a área debaixo da receiver-operating characteristic curve (AUC), o continuous net reclassification improvement (NRI), e o integrated discrimination improvement (IDI). Resultados: Treze por cento dos doentes tiveram AP grave, 26% desenvolveram necrose pancreática, e 7% morreram durante a hospitalização índice. As AUCs da CRP24 e do BISAP individualmente foram 0,80 (intervalo de confianc¸a (IC) 95%, 0,65-0,95) e 0,77 (IC 95%, 0,59-0,95), respectivamente. Nenhum doente com CRP24 <60 mg/l morreu (P = 0,027; valor predictivo negativo 100% (IC 95%, 92,3-100%)). A AUC para o BISAP mais a CRP24 foi 0,81 (IC 95%, 0,65-0,97). A mudança no NRI-não-eventos (42,4%; IC 95%, 24,9-59,9%) resultou num NRItotal positivo (31,3%; IC 95%, −36,4 a 98,9%), mas num IDI-não-eventos negligenciável (0,004; IC 95%, −0,007 a 0,014). Conclusões: A CRP24 revelou um valor prognóstico bom para a mortalidade intrahospitalar na AP; o seu papel principal poderá ser a selecção dos doentes de menor risco.

Introduction: C-reactive protein (CRP) and Bedside Index for Severity in Acute Pancreatitis (BISAP) have been used in early risk assessment of patients with AP. Objectives: We evaluated prognostic accuracy of CRP at 24 hours after hospital admission (CRP24) for in-hospital mortality (IM) in AP individually and with BISAP. Materials and Methods: This retrospective cohort study included 134 patients with AP from a Portuguese hospital in 2009-2010. Prognostic accuracy assessment used area under receiver-operating characteristic curve (AUC), continuous net reclassification improvement (NRI), and integrated discrimination improvement (IDI). Results: Thirteen percent of patients had severe AP, 26% developed pancreatic necrosis, and 7% died during index hospital stay. AUCs for CRP24 and BISAP individually were 0.80 (95% confidence interval (CI) 0.65-0.95) and 0.77 (95% CI 0.59-0.95), respectively. No patients with CRP24 <60 mg/l died (P = 0.027; negative predictive value 100% (95% CI 92.3-100%)). AUC for BISAP plus CRP24 was 0.81 (95% CI 0.65-0.97). Change in NRInonevents (42.4%; 95% CI, 24.9-59.9%) resulted in positive overall NRI (31.3%; 95% CI, −36.4% to 98.9%), but IDInonevents was negligible (0.004; 95% CI, −0.007 to 0.014). Conclusions: CRP24 revealed good prognostic accuracy for IM in AP; its main role may be the selection of lowest risk patients.

Background: Metabolic syndrome (MS) is suggested to be associated with a low grade inflammation state, but the relationship between inflammation biomarkers and the components of metabolic syndrome in adolescents are still lacking. Objective: To investigate the association between C-reactive protein (CRP) serum concentrations and metabolic syndrome components in adolescents. Methods: A cross-sectional population based study was conducted. Anthropometric, biochemical and clinical data were collected from 524 adolescents (11-15 years old) randomly sampled from school population of Alegre city, Espmto Santo, Brazil. Data were analyzed by STATA version 9.0. Results: Adolescents with higher values for BMI (p = 0.001) and higher body fat percentage (p = 0.003) had higher CRP concentrations than those with lower BMI and body fat percentage. CRP concentrations was directly correlated with BMI (r = 0.17, p = 0.0001), waist circumference (r = 0.15, p = 0.0005), HDL-c (r = 0.13, p = 0.003), fasting insulin (r = 0.12, p = 0.003) and systolic blood pressure (r = 0.11, p with = 0.01). In the multiple linear regression analysis BMI (r = 0.05, p = 0.002), fasting glucose (r = -0.01, p = 0.003) and HDL-c (r = 0.017, p < 0.001) were associated to CRP concentrations after adjusting for the other components of MS. Conclusion: The association found between individual components of MS and CRP concentrations suggests that inflammation might be an early event in the development of metabolic disorders in adolescents.

Antecedentes: El síndrome metabólico (SM) se sugiere que está asociada con un estado de inflamación crónica de bajo grado, pero la relación entre biomarcadores de inflamación crónica de bajo grado, pero la relación entre biomarcadores de inflamación y los componentes del síndrome metabólico en adolescentes son escasos. Objetivo: Investigar la asociación entre las concentraciones séricas de proteína C reactiva (CRP) y los componentes del síndrome metabólico en adolescentes. Metodología: Hemos realizado una población basada en estudio de corte transversal. Los datos antropométricos, bioquímicos y clínicos se obtuvieron de 524 adolescentes (11-15 años de edad) seleccionados al azar de la población escolar de la ciudad Alegre, Espírito Santo, Brasil. Los datos fueron analizados por STATA versión 9.0. Resultados: Los adolescentes, con valores más altos de IMC (p = 0,001) y mayor porcentaje de grasa corporal (p = 0,003) tuvieron mayores concentraciones de PCR que aquellos con menor IMC y porcentaje de grasa corporal. Las concentraciones de PCR se correlacionó directamente con el IMC (r = 0,17, p = 0,0001), la circunferencia de la cintura (r = 0,15, p = 0,0005), HDL-c (r = 0,13, p = 0,003), la insulina en ayunas (r = 0,12, p = 0,009) y la presión arterial sistólica (r = 0,11, p = a 0,01). En el análisis de regresión lineal múltiple IMC (r = 0,05, p = 0,002), la glucosa en ayunas (r = -0,01, p = 0,003) y HDL-c (r = 0,017, p < 0,001) se asociaron a las concentraciones de PCR después de ajustar por los otros componentes de SM. Conclusión: La asociación encontrada entre los componentes individuales de SM y las concentraciones de PCR sugiere que la inflamación podría ser un evento temprano en el desarrollo de trastornos metabólicos en los adolescentes.

OBJETIVO Verificar a associação entre proteína C-reativa (PCR) e pressão arterial (PA) elevada em crianças e adolescentes com sobrepeso ou obesidade. MÉTODOS Estudo transversal, realizado de abril de 2009 a abril de 2010, com 184 crianças e adolescentes de dois a 18 anos, obesos ou com sobrepeso. Realizou-se a classificação do estado nutricional por meio do índice da massa corpórea (IMC). Adotou-se a curva do Centers for Disease Control and Prevention, classificando-se os indivíduos em: sobrepeso (IMC entre percentil 85-95), obesidade (IMC entre percentil 95-97) e obesidade grave (IMC>percentil 97). Consideraram-se alterados valores de: PA sistólica (PAS) e/ou diastótica (PAD) ≥percentil 90, conforme curva de classificação da PA para crianças e adolescentes nas V Diretrizes Brasileiras de Hipertensão Arterial; circunferência abdominal (CA) ≥percentil 90 de acordo com a curva do National Cholesterol Education Program; e PCR ultrassensível (PCR-us) >3mg/dL. Para avaliar a associação de valores inadequados de PCR entre os grupos, utilizaram-se o teste do qui-quadrado e a análise de variância. As análises foram realizadas na versão 17.0 do programa Statistical Package for the Social Sciences, adotando-se nível de significância de 5%. RESULTADOS Dos avaliados, 66,3% eram do sexo feminino, 63,5%, não brancos, 64,1% tinham obesidade grave, 78,3% apresentavam CA alterada e 70,6%, PA elevada. Verificou-se associação significante dos níveis aumentados de PCR com CA alterada e IMC ≥percentil 97. Nos adolescentes, a PCR aumentada associou-se à PAS elevada. Os valores médios da PCR foram superiores nos indivíduos que apresentaram PAS elevada. CONCLUSÕES Valores inadequados da PCR-us associaram-se à obesidade grave e à PAS elevada na população estudada. Tais marcadores podem ser utilizados para identificar crianças e adolescentes com maior risco de desenvolver aterosclerose.

OBJECTIVE To investigate the association between C-reactive protein (CRP) and high blood pressure (BP) in overweight or obese children and adolescents. METHODS Cross-sectional study with 184 overweight or obese children and adolescents aged from two to 18 years old, from April, 2009 to April, 2010. The classification of nutritional status used the body mass index (BMI). Based on the Centers for Disease Control and Prevention curve, individuals were classified as: overweight (BMI between the 85th-95th percentiles), obesity (BMI between 95th-97th percentiles) and severe obesity (BMI >97th percentile). Abnormal values were considered for systolic BP (SBP) and/or diastolic (DBP) if ≥90th percentile of the BP curve recommended for children and adolescents in the V Brazilian Guidelines on Hypertension, for waist circumference (WC) if ≥90th percentile of the curve established by the National Cholesterol Education Program, and for high sensitive CRP (hs-CRP) if >3mg/dL. To evaluate the association of inadequate values of CRP and the studied groups, chi-square test and analysis of variance were applied, using the Statistical Package for the Social Sciences version 17.0 and adopting a significance level of 5%. RESULTS Among the evaluated sample, 66.3% were female, 63.5%, non-white, 64.1% had severe obesity, 78.3% had altered WC and 70.6% presented high BP. There was a significant association of CRP high levels with altered WC and BMI ≥97th percentile. In adolescents, high CRP was related to high SBP. CRP mean values were higher in individuals with elevated SBP. CONCLUSIONS Inadequate values of hs-CRP were associated with severe obesity and high SBP in the studied population. These markers can be used to identify children and adolescents at higher risk for developing atherosclerosis.

OBJETIVO Verificar la asociación entre proteína C-reactiva (PCR) y presión arterial (PA) elevada en niños y adolescentes con sobrepeso u obesidad. MÉTODOS Estudio transversal, realizado de abril de 2009 a abril de 2010, con 184 niños y adolescentes de 2 a 18 años, obesos o con sobrepeso, atendidos en el Centro de Obesidad Infantil. Se realizó la clasificación del estado nutricional por medio del índice de masa corporal (IMC). Se adoptó la curva del Centers for Disease Control and Prevention, clasificándose a los individuos en: sobrepeso (IMC entre percentil 85-95), obesidad (IMC entre percentil 95-97) y obesidad grave (IMC >percentil 97). Se consideraron alterados los valores de PA sistólica (PAS) y/o diastólica (PAD) ≥percentil 90 - conforme curva de clasificación de la PA para niños y adolescentes recomendada en las V Directrices Brasileñas de Hipertensión Arterial, circunferencia abdominal (CA) ≥percentil 90 - conforme a la curva establecida por el National Cholesterol Education Program, y PCR ultrasensible (PCR-us) >3mg/dL. Para evaluar la asociación de valores inadecuados de PCR entre los grupos, se utilizaron la prueba del chi-cuadrado y análisis de variancia. Los análisis fueron realizados en la versión 17.0 del programa Statistical Package for the Social Sciences, adoptándose nivel de significancia de 5%. RESULTADOS De los evaluados, el 66,3% eran del sexo femenino, el 63,5% no blancos, el 64,1% tenían obesidad grave, el 78,3% presentaban CA alterada y el 70,6%, PA elevada. Se verificó la asociación significante de los niveles aumentados de PCR con CA alterada e IMC ≥percentil 97. En los adolescentes, la PCR aumentada se asoció a la PAS elevada. Los valores medianos de la PCR fueron superiores en los individuos que presentaron PAS elevada. CONCLUSIONES Valores inadecuados de PCR-us se asociaron a la obesidad grave y a la PAS elevada en la población estudiada. Es posible utilizar esos marcadores para identificar a niños y adolescentes con mayor riesgo de desarrollar aterosclerosis.

The pathogenesis of alveolitis is not well known and therefore experimental situations that mimic some features of this disease should be developed. OBJECTIVE: In this study, the evolution of the experimentally induced infection in rat sockets is characterized, which leads to clinical signs of suppurative alveolitis with remarkable wound healing disturbs. MATERIAL AND METHODS: Non-infected (Group I) and experimentally infected sockets in Rattus novergicus (Group II) were histometrically evaluated regarding the kinetics of alveolar healing. In addition, the characterization of the present bacteria in inoculation material and the serum levels of C-reactive protein (CRP) were performed. The detected species were Capnocytophaga ochracea, Fusobacterium nucleatum ss nucleatum, Prevotella melaninogenica, Streptococcus anginosus, Treponema socranskii and Streptococcus sanguis. RESULTS: All experimentally infected rats developed suppurative alveolitis, showing higher levels of CRP in comparison to those non-infected ones. Furthermore, infected rats presented a significant delayed wound healing as measured by the histometric analysis (higher persistent polymorphonuclear infiltrate and lower density of newly formed bone). CONCLUSION: These findings indicate that rat sockets with experimentally induced infection produced higher levels of serum CRP, showing the potential of disseminated infection and a disturb in the alveolar repair process in an interesting experimental model for alveolitis studies.

Plathymenia reticulata Benth has an anti-inflammatory effect and is capable of neutralizing the neuromuscular blockade induced by Bothrops jararacussu or Crotalus durissus terrificus venoms, probably by precipitating venom proteins (an effect caused by plant tannins). The present study aimed to evaluate the mutagenic activity of P. reticulata by using the Salmonella mutagenicity assay (Ames test) and the micronucleus test in CHO-K1 cells. P. reticulata extract concentrations of 2.84, 5.68, 11.37, and 19.90 mg/plate were assayed by the Ames test using TA97a, TA98, TA100 and TA102 bacterial strains, with (+S9) and without (-S9) metabolic activation. Concentrations of 5, 1.6 and 0.5 μg/mL of P. reticulata extract were used for the micronucleus test. P. reticulata extract was mutagenic to TA98 (-S9) and showed signs of mutagenic activity in TA97a and TA102 (both -S9) strains. Micronucleus test CBPI values showed that the endogenous metabolic system increased the number of viable cells when compared to the non-activated samples and the micronucleus frequency increased when the cells were treated in the absence of S9. We concluded that P. reticulata extract may present direct mutagenic properties.
