Abstract Given the environmental impacts associated with the production of conventional Portland cement, the study of special cements with reduced environmental impact has been a trend. In this context, cements containing ye'elimite (YCC) stand out due to their low CO 2 emissions. Despite the environmental advantages, the high demand for aluminum economically limits the production of these cements.An alternative to making its production feasible is using residual raw materials as a source of Al 2 O 3 , such as SFCC (spent fluidized catalytic cracking).In view of this, this work aims to evaluate hydration at the initial age of cements. For this, six clinkers were produced with different ye'elimite contents, with and without SFCC. These were characterized by XRD/Rietveld. The pastes were evaluated by TG/DTG and XRD/Rietveld to understand the hydration mechanism on the third day of these cements. It was possible to synthesize the main phases of YCC, in clinkers with and without SFCC. In addition, at the age evaluated, the pastes with and without SFCC presented comparable phase composition, indicating the potential for co-processing this residue.

Resumo Diante dos impactos ambientais associados à produção do cimento Portland convencional, o estudo de cimentos especiais de reduzido impacto ambiental tem sido uma tendência. Neste contexto, os cimentos contendo ye’elimita (YCC) se destacam devido à baixa emissão de CO2. Apesar das vantagens ambientais, a alta demanda por alumínio limita economicamente a produção destes cimentos. Uma alternativa para viabilizar a sua produção é o uso de matérias-primas residuais como fonte de Al2O3, a exemplo do SFCC (catalizador gasto do craqueamento catalítico de leito fluidizado). Diante disso, este trabalho tem como objetivo avaliar a hidratação na idade inicial de cimentos YCC produzidos a partir do coprocessamento do SFCC. Para isso, foram produzidos seis clínqueres com diferentes teores de ye’elimita, com e sem SFCC. Estes foram caracterizados por DRX/Rietveld. As pastas foram avaliadas por TG/DTG e DRX/Rietveld para compreender o mecanismo de hidratação em idade inicial destes cimentos. Foi possível sintetizar as principais fases dos YCC, nos clínqueres com e sem SFCC. Além disso, na idade avaliada, as pastas com e sem SFCC apresentaram composição de fases comparáveis indicando o potencial de coprocessamento do SFCC neste tipo de cimento.

Abstract Objective: to describe mortality and disability-adjusted life years (DALYs) due to laryngeal cancer attributable to occupational and behavioral risk factors in Brazil. Methods: this is an ecological study with data from the 2019 Global Burden of Disease. Mortality and DALY rates for laryngeal cancer attributable to occupational (sulfuric acid and asbestos) and behavioral (tobacco and alcohol) risks were obtained from 1990 and 2019. Results: in 2019, the mortality rate from laryngeal cancer attributable to occupational hazards (sulfuric acid and asbestos) totaled 0.28 (95%UI: 0.17; 0.43) and 0.03 (95%UI: 0.02; 0.04), whereas and DALY rates, 7.33 (95%UI: 4.28; 11.44) and 0.64 (95%UI: 0.35; 0.03) in men and women in Brazil, respectively. Sulfuric acid configured the main occupational risk for the disease. The rates attributable to tobacco (mortality: −45.83%; DALYs: −47.36%) and occupational hazards (mortality: −23.20%; DALYs: −26.31%) decreased in Brazil but increased in some Northern and Northeastern states. Conclusion: laryngeal cancer mortality and burden attributable to occupational factors decreased in the period (although less than that for smoking), reinforcing the importance of actions to reduce the impact of occupational risks, such as the regulatory measures applied to tobacco.

Resumo Objetivo: descrever a mortalidade e os anos de vida ajustados pela incapacidade disability-adjusted life years – DALYs) para câncer de laringe no Brasil atribuíveis a fatores de risco ocupacionais e comportamentais. Métodos: estudo ecológico com dados do estudo Global Burden of Disease 2019. Foram obtidas taxas de mortalidade e de DALYs para o câncer de laringe atribuíveis aos riscos ocupacionais (ácido sulfúrico e amianto) e comportamentais (tabaco e álcool), de 1990 e 2019. Resultados: no Brasil, em 2019, a taxa de mortalidade por câncer de laringe atribuível aos riscos ocupacionais (ácido sulfúrico e amianto) foi 0,28 (II95%: 0,17;0,43) no sexo masculino e 0,03 (II95%: 0,02;0,04) no feminino, e a de DALYs foi 7,33 (II95%: 4,28;11,44) e 0,64 (II95%: 0,35;0,03), respectivamente. O ácido sulfúrico foi o principal risco ocupacional para a doença. Houve redução das taxas atribuíveis ao tabaco (mortalidade:-45,83%; DALYs:-47,36%) e aos riscos ocupacionais (mortalidade:-23,20%; DALYs:-26,31%), no Brasil, com aumento em alguns estados das regiões Norte e Nordeste. Conclusão: houve redução na mortalidade e na carga do câncer de laringe atribuível aos fatores ocupacionais no período, porém menor em comparação ao tabagismo, reforçando a importância de ações para reduzir o impacto dos riscos ocupacionais, como as medidas regulatórias aplicadas ao tabaco.

ABSTRACT: Skin grafting is a simple and very useful surgical technique for wound repair, especially where there is difficulty in performing direct synthesis or other reconstructive techniques. low-intensity laser (LIL) has already been used successfully in cases where the graft was applied to wounds shortly after its creation. However, LIL still lacks standardization due to conflicting studies on its results. Thus, this study compared the effectiveness of continuous wave LIL with pulsed wave LIL in stimulating the healing of skin grafts, applied to newly created wounds in rabbits. For this purpose, 31 rabbits were distributed into: control group (CG, n = 10), continuous wave laser group (GLC; n = 10) and pulsed wave laser group (GLP; n = 11). Postoperative management was similar between all groups, with the exception of laser application, as indicated by the group. The GLC and GLP groups showed similar evolution, with a satisfactory healing process of the graft, unlike the GC group. These results were maintained in the microscopic evaluation, as the groups treated with laser showed less inflammation, better integration with the receptor area, greater re-epithelialization and collagenization. There was no difference in vascular density between the treatment groups on immunohistochemical analysis. The newly created wound was not able to provide nutrition for the skin graft in rabbits, but LIL is an efficient adjuvant therapy in stimulating healing between the recipient area and the graft, promoting full integration. However; although, statistically there was no difference in the form of light emission, pulsed or continuous, patients who received continuous wave LIL showed superior microscopic evolution.

RESUMO: A enxertia cutânea é uma técnica cirúrgica simples e bastante útil para o reparo de feridas, principalmente, quando há dificuldade em realizar a síntese direta ou outras técnicas reconstrutivas. O laser de baixa intensidade (LBI) já foi utilizado com sucesso em casos em que o enxerto foi aplicado em feridas logo após a sua criação. No entanto, a LBI ainda carece de padronizações, mediante estudos conflitantes sobre seus resultados. Desta forma, este trabalho objetivou comparar a eficácia da LBI de onda contínua com onda pulsada na estimulação da cicatrização de enxertos cutâneos, aplicados em feridas recém-criadas em coelhos. Para tanto, foram utilizados 31 coelhos distribuídos em: grupo controle (GC, n = 10), grupo laser onda contínua (GLC; n = 10) e grupo laser onda pulsada (GLP; n = 11). O manejo pós operatório foi semelhante entre todos os grupos, com exceção da aplicação do laser, conforme o grupo indicado. Os grupos GLC e GLP apresentaram evolução semelhante, com processo cicatricial satisfatório do enxerto, ao contrário do grupo GC. Tais resultados se mantiveram na avaliação microscópica, pois os grupos tratados com laser apresentaram menor inflamação, melhor integração à área receptora, maior reepitelização e colagenização. Não houve diferença na densidade vascular entre os grupos de tratamento na análise imuno-histoquímica. A ferida recém-criada não é capaz de fornecer nutrição para o enxerto cutâneo nos coelhos, mas a LBI é uma terapia adjuvante eficiente em estimular a cicatrização entre a área receptora e o enxerto, promovendo integração total. Entretanto, embora não houve diferença estatística na forma de emissão de luz, pulsada ou contínua, os pacientes que receberam LBI em onda contínua apresentam evolução microscópica superior.

ABSTRACT This research aimed to prospect the working conditions of the rescue teams of the Mobile Emergency Care Service (SAMU-192) at the fluvial areas of the Alto Solimoes region, trough the systemic analysis of the activities of the water ambulances service popularly called ‘ambulanchas’ during the peaks of COVID-19 in the state of Amazonas, when the health system functioned under stress caused by the pandemic. Data were obtained from an exploratory cross-sectional design, based on qualitative data collected through interviews and observation of the normal functioning of the system before the pandemic. From there, models of two scenarios were developed showing the functioning of the water ambulances service when coping with the COVID-19 pandemic and the impact on the working conditions of the interprofessional rescue teams. Remote interviews with workers after the pandemic indicated that the prospection of working conditions from the instances of the models corresponded to the real functioning of the system during the COVID-19 pandemic.

RESUMO Esta pesquisa teve como objetivo prospectar as condições de trabalho das equipes de socorro do Serviço de Atendimento Móvel de Urgência (Samu-192) fluvial das áreas ribeirinhas e costeiras da região do Alto-Solimões, a partir da análise sistêmica das atividades no serviço de embarcações popularmente chamadas de ‘ambulanchas’ durante os picos da Covid-19 no estado do Amazonas, quando o sistema de saúde funcionou sob o estresse provocado pela pandemia. Os dados foram obtidos a partir de um desenho transversal exploratório, baseado em dados qualitativos coletados por meio de entrevistas e observação do funcionamento normal do sistema antes da pandemia. A partir daí modelos de dois cenários foram elaborados mostrando o funcionamento do serviço de ambulanchas ao lidar com a pandemia de Covid-19 e o impacto nas condições de trabalho das equipes de socorro interprofissionais de socorro. Entrevistas remotas com trabalhadores das ambulanchas após a pandemia indicaram que a prospecção das condições de trabalho a partir das instâncias dos modelos corresponderam ao funcionamento real do sistema durante a pandemia de Covid-19.

Objective: to analyze the effects of educational technologies in the prevention and treatment of diabetic ulcers. Method: a systematic review conducted in seven databases, a bibliographic index, an electronic library and the Gray Literature. The sample consisted of 11 randomized controlled clinical trials. The synthesis of the results was descriptive and through meta-analysis. Results: the predominant educational technologies were training sessions and verbal guidelines, with soft-hard technologies standing out. When compared to usual care, the educational technologies presented a protective factor to prevent the incidence of diabetic ulcers (RR=0.40; 95% CI=0.18-0.90; p=0.03) and the certainty of the evidence assessment was low. The educational technologies also had a protective factor to prevent the incidence of lower limb amputations (RR=0.53; 95% CI=0.31-0.90; p=0.02) and certainty of the evidence was very low. Conclusion: soft-hard educational technologies such as structured verbal guidelines, educational games, lectures, theoretical-practical training sessions, educational videos, folders, serial albums and playful drawings, and hard technologies such as therapeutic footwear, insoles, infrared digital thermometer, foot care kits, Telemedicine app and mobile phone use, were effective for the prevention and treatment of diabetic ulcers, although more robust studies are required.

Objetivo: analizar los efectos de las tecnologías educativas en la prevención y tratamiento de la úlcera diabética. Método: revisión sistemática realizada en siete bases de datos, un índice bibliográfico, una biblioteca electrónica y literatura gris. La muestra estuvo compuesta por 11 ensayos clínicos controlados aleatorizados. La síntesis de los resultados fue descriptiva y mediante metaanálisis. Resultados: las tecnologías educativas predominantes fueron la capacitación y la orientación verbal, se destacaron las tecnologías blandas-duras. En comparación con la atención habitual, las tecnologías educativas demostraron ser un factor protector para prevenir la incidencia de úlcera diabética (RR=0,40; IC 95%=0,18-0,90; p=0,03) y la evaluación de certeza de evidencia fue baja. Las tecnologías educativas también demostraron ser un factor protector para prevenir la incidencia de amputación en miembros inferiores (RR=0,53; IC 95%=0,31-0,90; p=0,02) y la certeza de evidencia fue muy baja. Conclusión: tecnologías educativas blandas-duras, como orientación verbal estructurada, juegos educativos, clase expositiva, capacitación teórico-práctica, video educativo, folder, rotafolio educativo y dibujos lúdicos, y tecnologías duras, como calzado terapéutico, plantillas, termómetro infrarrojo digital, kits para el cuidado de los pies, aplicación de telemedicina y teléfono móvil, resultaron efectivas para la prevención y el tratamiento de la úlcera diabética, sin embargo, es necesario que se realicen estudios más robustos.

Objetivo: analisar os efeitos das tecnologias educativas na prevenção e tratamento da úlcera diabética. Método: revisão sistemática conduzida em sete bases de dados, um índice bibliográfico, uma biblioteca eletrônica e na literatura cinzenta. A amostra foi constituída de 11 ensaios clínicos controlados randomizados. A síntese dos resultados foi descritiva e por meio de metanálise. Resultados: as tecnologias educativas predominantes foram os treinamentos e as orientações verbais, destacando-se as tecnologias leve-duras. Na comparação com o cuidado usual, as tecnologias educativas apresentaram fator de proteção para prevenção da incidência de úlcera diabética (RR=0,40; IC 95%=0,18-0,90; p=0,03) e a avaliação de certeza da evidência foi baixa. As tecnologias educativas também tiveram fator de proteção para prevenção da incidência de amputação em membros inferiores (RR=0,53; IC 95%=0,31-0,90; p=0,02) e a certeza da evidência foi muito baixa. Conclusão: as tecnologias educativas leve-duras, como orientações verbais estruturadas, jogos educativos, aula expositiva, treinamentos teórico-práticos, vídeo educativo, folder, álbum seriado e desenhos lúdicos, e as tecnologias duras, a exemplo do calçado terapêutico, palmilhas, termômetro digital de infravermelho, kits de cuidados com os pés, aplicativo de telemedicina e telefone móvel, foram efetivas para prevenção e tratamento da úlcera diabética, porém, estudos mais robustos são necessários.

Objective: to evaluate the association between risk classification and door-to-antibiotic time in patients with suspected sepsis. Method: retrospective cohort study, with a sample of 232 patients with suspected sepsis treated at the emergency department. They were divided into 2 groups: with and without risk classification. Once the door-to-antibiotic time was identified, one-way analysis of variance was performed with Bonferroni post hoc test or independent Student’s t-test for continuous quantitative variables; Pearson correlation tests, point-biserial correlation or biserial correlation for association analyses; and bootstrap procedure when there was no normal distribution of variables. For data analysis, the Statistical Package for the Social Sciences software was used. Results: the door-to-antibiotic time did not differ between the group that received risk classification compared to the one that was not classified. Door-to-antibiotic time was significantly shorter in the group that received a high priority risk classification. Conclusion: there was no association between door-to-antibiotic time and whether or not the risk classification was performed, nor with hospitalization in infirmaries and intensive care units, or with the length of hospital stay. It was observed that the higher the priority, the shorter the door-to-antibiotic time.

Objetivo: evaluar la asociación entre la clasificación de riesgo y el tiempo puerta-antibiótico en pacientes con sospecha de sepsis. Método: estudio de cohorte retrospectivo, con una muestra de 232 pacientes con sospecha de sepsis atendidos en el departamento de emergencias. Se dividieron en 2 grupos: con y sin clasificación de riesgo. Una vez identificado el tiempo puerta-antibiótico, se realizó un análisis de varianza de un factor con la prueba post hoc de Bonferroni o la prueba t de Student independiente para variables cuantitativas continuas; pruebas de correlación de Pearson, correlación biserial puntual o correlación biserial para análisis de asociación; y procedimiento de bootstrap cuando no había distribución normal de variables. Para el análisis de los datos se utilizó el software Statistical Package for the Social Sciences. Resultados: el tiempo puerta-antibiótico no difirió entre el grupo que recibió clasificación de riesgo en comparación con el que no fue clasificado. El tiempo puerta-antibiótico fue significativamente más corto en el grupo que recibió una clasificación de riesgo de alta prioridad. Conclusión: no hubo asociación entre el tiempo puerta-antibiótico y si se realizó o no la clasificación de riesgo, ni con la hospitalización en enfermería y en unidad de cuidados intensivos, ni con la duración de la estancia hospitalaria. Se observó que cuanto mayor era la prioridad, más corto era el tiempo puerta-antibiótico.

Objetivo: avaliar a associação entre a realização de classificação de risco e o tempo porta-antibiótico no paciente com suspeita de sepse. Método: estudo de coorte retrospectivo, com amostra de 232 pacientes com suspeita de sepse atendidos no pronto atendimento. Foram distribuídos em 2 grupos: com e sem classificação de risco. Identificado o tempo porta-antibiótico, realizou-se análise de variância de um fator com post hoc de Bonferroni ou teste T-Student independente para variáveis quantitativas contínuas; testes de correlação de Pearson, correlação bisserial por pontos ou correlação bisserial para análises de associação; e procedimento de bootstrap quando não havia distribuição normal de variáveis. Para a análise dos dados foi utilizado o software Statistical Package for the Social Sciences. Resultados: o tempo porta-antibiótico não diferiu entre o grupo que recebeu classificação de risco comparado ao que não foi classificado. O tempo porta-antibiótico foi significativamente menor no grupo que recebeu classificação de risco de alta prioridade. Conclusão: não houve associação entre o tempo porta-antibiótico e a realização ou não da classificação de risco, tampouco com internação em enfermaria e em unidade de terapia intensiva, ou com o tempo de internação hospitalar. Observou-se que quanto maior a prioridade, menor o tempo porta-antibiótico.

Resumo Introdução: A horizontalização do cuidado é uma forma de organização do trabalho em saúde, na qual existe uma equipe multiprofissional de referência que atua diariamente no serviço, em contraposição à forma de organização do trabalho em que profissionais têm uma carga horária distribuída por plantão. Objetivos: Compreender o processo de trabalho relacionado às atribuições do enfermeiro rotineiro na equipe multiprofissional. Métodos: Trata-se de um estudo descritivo, exploratório de cunho qualitativo, realizada com 48 profissionais de um hospital público no Distrito Federal, Brasil. A coleta de dados ocorreu no período de 10 de outubro a 30 de novembro de 2022. Os dados foram analisados por meio da análise de conteúdo de Bardin: pré-análise, exploração do material e tratamento dos resultados, inferência e interpretação. Resultados: Foi possível encontrar três núcleos de sentido que foram divididos em três categorias: 1- Inexistência de sistematização do trabalho do rotineiro; 2- Processo de trabalho na percepção da equipe; e 3- Necessidades de aprendizagem. Conclusões: Verificou-se que o papel do enfermeiro horizontal não está devidamente esclarecido, necessitando, portanto, de elaboração de protocolos institucionais que apresentem as atividades a serem desenvolvidas pela horizontal e pelo plantonista. No intuito de alinhar as condutas no cenário hospitalar, sugere-se ampla capacitação da equipe multiprofissional com abordagem no cuidado horizontal, preconizado pela Política Nacional de Atenção Hospitalar.

Abstract Introduction: The horizontalization of care is a form of organization of work in health, in which there is a multidisciplinary team of reference that works daily in the service, as opposed to the form of organization of work in which professionals have a workload distributed per shift. Objectives: Understanding the work process related to the duties of the horizontal nurse in the multidisciplinary team. Methods: This is a descriptive, exploratory qualitative study, carried out with 48 professionals from a public hospital. Data collection took place from October 10 to November 30, 2022. Data were analyzed using Bardin’s content analysis: pre-analysis, material exploration and treatment of results, inference and interpretation. Results: It was possible to find three nuclei of meaning that were divided into three categories: 1- Lack of routine work systematization; 2- Work process in the team's perception; and 3- Learning needs. Conclusions: It was verified that the role of the horizontal nurse is not properly clarified, therefore requiring the elaboration of institutional protocols that present the activities to be developed by the horizontal nurse and by the on-duty nurse. In order to align conducts in the hospital scenario, it is suggested extensive training of the multidisciplinary team with a horizontal care approach, recommended by the National Policy for Hospital Care.

Abstract Introduction: There is a linear association between pregestational body mass index (BMI) and almost all adverse pregnancy outcomes. Pregnancy is “a window of opportunities” in terms of changing behavior and improving awareness of healthy living. The proper assessment of physical activity levels, during pregnancy, determines trends, health benefits, and their effects over time. This study aims to describe maternal physical activity levels, stratified by pregestational BMI, verify the accomplishment of physical activity recommendations in pregnant women, and correlate pregestational BMI and physical activity accomplishment with maternal, delivery, and neonatal parameters. Methods: A retrospective cohort study was carried out with 103 pregnant women. Physical activity levels were evaluated using a questionnaire and accelerometry. Pregestational BMI was obtained through the Quetelet formula and used the American College of Sports Medicine’s guidelines were used to determine physical activity accomplishment levels. Continuous data were presented as mean and standard deviation and categorical data as numbers and percentages. The F test was used to examine the differences between groups. Results: Pregnant women in the sample spent 42.9% of their time on household activities, and for pregnant women with obesity, 91.5% of the time was spent on sedentary activities. Women with normative BMI had higher levels of moderate-intensity activities. Only 15.8% of participants with obesity reached the international recommendations for physical activity practice and women who accomplished physical activity recommendations gained less weight during pregnancy. A high percentage of pre-obesity and obese pregnant women exceeded the recommendations for gestational weight gain and the gestational diabetes prevalence was higher in the obesity group (p = 0.03 between groups). Regarding delivery and neonatal parameters (Apgar score 1st, Apgar score 5th, birth weight, length and head circumference), no statistical differences were found when adjusted to a gestational week at birth, between BMI (p = 0.58; p = 0.18; p = 0.60; p = 0.34; p = 0.34, respectively) or physical activity (p = 0.12; p = 0.15; p = 0.83; p = 0.70; p = 0.70, respectively) groups. Conclusion: Pregnant women with obesity, exhibit high levels of sedentary behavior, a high prevalence of gestational diabetes, and exceed recommended gestational weight gain. Healthcare professionals have a crucial role in promoting regular physical activity and lifestyle changes before and during pregnancy.

Resumo Introdução: Existe uma associação linear entre o Índice de Massa Corporal (IMC) pré-gestacional e quase todos os desfechos adversos da gravidez. A gravidez é “uma janela de oportunidades” em termos de mudança de comportamento e melhoria da conscientização sobre uma vida saudável. A avaliação adequada dos níveis de Atividade Física, durante a gravidez, determina tendências, benefícios para a saúde e os seus efeitos ao longo do tempo. Este estudo tem como objetivo descrever as atividades e os níveis de atividade física materna, estratificados pelo IMC pré-gestacional, verificar o cumprimento das recomendações de atividade física em grávidas e correlacionar o IMC e a realização de atividade física com parâmetros maternos, de parto e neonatais. Métodos: Foi realizado um estudo de coorte retrospetivo com 103 grávidas. O tipo e a intensidade da atividade física foram avaliados por questionário e acelerometria, o IMC pré-gestacional foi obtido pela fórmula de Quetelet e utilizaram-se as diretrizes do American College of Sport Medicine para determinar os níveis de realização da atividade física. Os dados contínuos foram apresentados em média e desvio padrão, e os dados categóricos em números e percentagens. O F test e foi utilizado para examinar as diferenças entre os grupos. Resultados: As grávidas da amostra despenderam 42.9% do tempo em atividades domésticas e, as grávidas com obesidade, 91.5% do tempo em atividades sedentárias. Grávidas com IMC normativo apresentaram níveis mais elevados de atividades de intensidade moderada, 15.8% das participantes com obesidade atingiram as recomendações internacionais para a prática de atividade física e as mulheres que cumpriram as recomendações de atividade física ganharam menos peso durante a gravidez. Grande percentagem das grávidas com sobrepeso e obesidade não cumpriu com as recomendações para ganho de peso gestacional e a incidência de diabetes gestacional foi maior no grupo obesidade (p = 0.03 entre os grupos). Relativamente aos parâmetros de parto e neonatais (apgar 1º, apgar 5º, peso tamanho e perímetro cefálico), não foram encontradas diferenças estatísticas, quando os scores foram ajustados à semana de gestacional, entre os grupos de IMC (p = 0.58; p = 0.18; p = 0.60; p = 0.34; p = 0.34 respetivamente) ou atividade física (p = 0.12; p = 0.15; p = 0.83; p = 0.70; p = 0.70 respetivamente). Conclusão: Grávidas com obesidade apresentam altos níveis de comportamento sedentário, alta prevalência de diabetes gestacional e excedem o ganho de peso gestacional recomendado. Os profissionais de saúde têm um papel crucial na promoção da atividade física regular e mudanças no estilo de vida antes e durante a gravidez.

ABSTRACT During the state of immune vulnerability in hematopoietic stem cell transplantation (HSCT), the patient has an increased risk of developing a vast number of complications, including severe problems in the oral cavity. These situations require professional oral care to act in the diagnosis and treatment of these conditions, as well as to develop prevention protocols to minimize patient's complications. Oral mucositis, opportunistic infections, bleeding, specific microbiota, taste, and salivary alterations are complications that can occur during HSCT and interfere with various aspects, such as pain control, oral intake, nutrition, bacteremia and sepsis, days of hospitalization and morbidity. Several guidelines have been published to address the role of professional oral care during the HSCT, we describe a consensus regarding these recommendations.

ABSTRACT The oral involvement in the Hematopoietic Stem Cell Transplantation is well described in the literature. The goal of the dental treatment and management of the oral lesions related to the HSCT is to reduce the harm caused by preexisting oral infection or even the worsening of oral acute/chronic GVHD and late effects. The aim of this guideline was to discuss the dental management of patients subjected to HSCT, considering three phases of the HSCT: pre-HSCT, acute phase, and late phase. The literature published from 2010 to 2020 was reviewed in order to identify dental interventions in this patient population. The selected papers were divided into three groups: pre-HSCT, acute and late, and were reviewed by the SBTMO Dental Committee's members. When necessary, an expertise opinion was considered for better translating the guideline recommendations to our population dental characteristics. This manuscript focused on the pre-HSCT dental management. The objective of the pre-HSCT dental management is to identify possible dental situations that On behalf of the Dental Committee of the Brazilian Society of Gene Therapy and Bone Marrow Transplantation (SBTMO) can worsening during the acute phase after the HSCT. Each guideline recommendations were made considering the Dentistry Specialties. The clinical consensus on dental management prior to HSCT provides professional health caregivers with clinical setting-specific information to help with the management of dental problems in patients to be subjected to HSCT.

ABSTRACT Objective: To update the recommendations to support decisions regarding the pharmacological treatment of patients hospitalized with COVID-19 in Brazil. Methods: Experts, including representatives of the Ministry of Health and methodologists, created this guideline. The method used for the rapid development of guidelines was based on the adoption and/or adaptation of existing international guidelines (GRADE ADOLOPMENT) and supported by the e-COVID-19 RecMap platform. The quality of the evidence and the preparation of the recommendations followed the GRADE method. Results: Twenty-one recommendations were generated, including strong recommendations for the use of corticosteroids in patients using supplemental oxygen and conditional recommendations for the use of tocilizumab and baricitinib for patients on supplemental oxygen or on noninvasive ventilation and anticoagulants to prevent thromboembolism. Due to suspension of use authorization, it was not possible to make recommendations regarding the use of casirivimab + imdevimab. Strong recommendations against the use of azithromycin in patients without suspected bacterial infection, hydroxychloroquine, convalescent plasma, colchicine, and lopinavir + ritonavir and conditional recommendations against the use of ivermectin and remdesivir were made. Conclusion: New recommendations for the treatment of hospitalized patients with COVID-19 were generated, such as those for tocilizumab and baricitinib. Corticosteroids and prophylaxis for thromboembolism are still recommended, the latter with conditional recommendation. Several drugs were considered ineffective and should not be used to provide the best treatment according to the principles of evidence-based medicine and to promote resource economy.

RESUMO Objetivo: Atualizar as recomendações para embasar as decisões para o tratamento farmacológico de pacientes hospitalizados com COVID-19 no Brasil. Métodos: A elaboração desta diretriz foi feita por especialistas, incluindo representantes do Ministério da Saúde e metodologistas. O método utilizado para o desenvolvimento rápido de diretrizes baseou-se na adoção e/ou adaptação de diretrizes internacionais existentes (GRADE ADOLOPMENT) e contou com o apoio da plataforma e-COVID-19 RecMap. A qualidade das evidências e a elaboração das recomendações seguiram o método GRADE. Resultados: Chegaram-se a 21 recomendações, incluindo recomendações fortes quanto ao uso de corticosteroides em pacientes em uso de oxigênio suplementar e recomendações condicionais para o uso de tocilizumabe e baricitinibe, em pacientes com oxigênio suplementar ou ventilação não invasiva, e de anticoagulantes, para prevenção de tromboembolismo. Devido à suspensão da autorização de uso, não foi possível fazer recomendações para o tratamento com casirivimabe + imdevimabe. Foram feitas recomendações fortes contra o uso de azitromicina em pacientes sem suspeita de infecção bacteriana, hidroxicloroquina, plasma convalescente, colchicina e lopinavir + ritonavir, além de recomendações condicionais contra o uso de ivermectina e rendesivir. Conclusão: Foram criadas novas recomendações para o tratamento de pacientes hospitalizados com COVID-19, como as recomendações de tocilizumabe e baricitinibe. Ainda são recomendados corticosteroides e profilaxia contra tromboembolismo, esta em caráter condicional. Vários medicamentos foram considerados ineficazes e não devem ser usados, no intuito de proporcionar o melhor tratamento segundo os princípios da medicina baseada em evidências e promover a economia de recursos.

Abstract Using the critique of instrumental rationality, based on Weber, Habermas and Arendt, we analyze, through interviews of the leaders involved in the creation of the Central Bank of Brazil (BCB), the tension between politics and technique in its formation as a bureaucracy. This oral history research was based on interviews conducted by CPDOC/FGV, published in 2019, in 25 volumes of the "Told History of the Central Bank of Brazil Collection," with leaders who participated in the creation of the BCB. As a result, we identified a discourse of rejection of politics in general and aversion to left-wing policies in particular, in addition to the sacredness of technique, giving rise to a technocratic vision of the BCB, an organization that is seen as bureaucratically insulated, but in which only one technique would be possible: the neoclassical liberal economic view. These two dimensions combine to establish a position of subjection of politics to technique in which even democracy can be sacrificed as an opportunity to implement a certain technical-economic agenda. There are indications, therefore, of an instrumentality of reason in the manifestations analyzed, in pitting technique against politics, which can lead to a process of banalization of evil as criticized by Arendt, especially in seeing regimes of exception in the country as mere opportunities to create the BCB and to implement a liberal-economic agenda. This perspective transforms technique into a political obstacle, including with the rejection of the greatest product of democratic political participation in the country, the 1988 Constitution.

Resumo A partir da crítica à racionalidade instrumental, baseada em Weber, Habermas e Arendt, busca-se analisar, por meio de entrevistas das lideranças envolvidas na criação do Banco Central do Brasil (BCB), a tensão entre política e técnica na sua formação como burocracia. A pesquisa, de história oral, foi feita com base em entrevistas realizadas pelo CPDOC/FGV, publicadas em 2019, em 25 volumes da “Coleção História Contada do Banco Central do Brasil”, com lideranças que participaram da criação do BCB. Identificou-se, por meio das entrevistas, um discurso de rejeição à política em geral e de aversão às políticas de esquerda em específico, além da sagração da técnica ensejando uma visão tecnocrática do BCB, órgão que é tido como insulado burocraticamente, mas, no qual apenas uma técnica seria possível: a liberal-econômica de matriz neoclássica. Essas duas dimensões se combinam para estabelecer uma posição de sujeição da política à técnica em que até mesmo a democracia pode ser sacrificada para que se aproveitem oportunidades de implantação de uma determinada agenda técnico-econômica. Há indícios, portanto, de uma instrumentalidade da razão nas manifestações das lideranças do BCB, ao oporem técnica versus política, o que pode acarretar um processo de banalização do mal tal qual criticado por Arendt, sobretudo, ao enxergar regimes de exceção no país como meras oportunidades de criação do BCB e de implantação de uma agenda liberal-econômica. Essa perspectiva transforma a técnica em um entrave político, inclusive, com a rejeição ao maior produto de participação política democrática do país, a Constituição de 1988.

Abstract This article presents a qualitative study that examines hate speech (HS) from the perspectives of the most representative minority communities in Portugal, namely Afro-descendants, Roma, and LGBTQ+. The empirical research employed three focus groups, each with a representation of these communities (n=17), with the primary goal of investigating how members of these groups perceive and experience HS within the Portuguese social and geopolitical context. The results indicate that covert HS can be more detrimental than overt HS since the targeted individuals are often caught off guard and unsure how to respond, contributing to the continuation of systems of oppression and social decline. Additionally, our qualitative data show that covert forms of HS often manifest in the form of compliments and humour, common in various settings, such as the workplace, the media, and the online environment. These devices are communicative strategies anchored in both positive and negative stereotypes and may lead to the normalization of HS in Portuguese society. In this regard, this research draws attention to the significance of identifying covert forms of HS and devising strategies, such as counter-speech, to debunk them. Furthermore, it highlights the need for automatic detection of covert HS since these forms are prevalent in digital platforms.

Resumo Este artigo apresenta um estudo qualitativo acerca do discurso de ódio (DO) a partir das perspetivas de três comunidades minoritárias representativas em Portugal: afrodescendentes, pessoas de etnia Roma (ou cigana) e LGBTQ+. A investigação empírica envolveu a realização de três grupos focais, um com cada comunidade (n=17), tendo como principal objetivo investigar o modo como os membros desses grupos percecionam e experienciam o DO no contexto social e geopolítico português. Os resultados deste estudo mostram que o DO indireto (ou encoberto) pode ser mais prejudicial do que o DO direto (ou explícito), pois inibe os mecanismos de reação por parte dos alvos, o que leva à perpetuação de sistemas de opressão e decadência social. Além disso, os dados revelam que as formas encobertas de DO manifestam-se muitas vezes como elogios e humor, sendo comuns no local de trabalho, nos meios de comunicação e no ambiente online. De facto, o elogio e o humor são estratégias comunicativas, ancoradas em estereótipos positivos ou negativos, que contribuem para a normalização do DO na sociedade portuguesa. Neste sentido, esta investigação alerta para a importância de identificar formas encobertas de DO e de desenvolver estratégias eficazes, como contranarrativas, para combatê-las. Por fim, destaca a necessidade de conceber sistemas de deteção automática capazes de identificar o DO indireto, dada a sua prevalência nas plataformas digitais.

Abstract Isthmocele is a discontinuation of the myometrium at the uterine scar site in a patient with a previous cesarian section (CS). The cause of isthmocele appears to be multifactorial. Poor surgical technique, low incision location, uterine retroflection, obesity, smoking, inadequate healing of scars, and maternal age are possible related factors. Most patients with this condition are asymptomatic. However, women can present with postmenstrual bleeding, pelvic pain, subfertility, dysmenorrhea, infertility, and scar abscess. Brazil has one of the world s highest cesarean section rates. One of the consequences of the rising rate of CS is the isthmocele, an emerging female health problem. Here we report a case of mucinous cystadenoma arising in a uterine isthmocele, a complication, as far as we could investigate, not yet described in the literature.

Resumo Istmocele é a descontinuidade do miométrio no local da cicatriz uterina em paciente com cesariana anterior. A causa da istmocele parece ser multifatorial. Má técnica cirúrgica, baixa localização da incisão, retroflexão uterina, obesidade, tabagismo, cicatrização inadequada de cicatrizes e idade materna são possíveis fatores relacionados. A maioria dos pacientes com esta condição é assintomática. No entanto, as mulheres podem apresentar sangramento pós-menstrual, dor pélvica, subfertilidade, dismenorreia, infertilidade e abscesso cicatricial. O Brasil tem uma das maiores taxas de cesariana do mundo. Uma das consequências da taxa crescente de cesarianas é a istmocele, um problema emergente de saúde feminina. Aqui relatamos um caso de cistoadenoma mucinoso originado em uma istmocele uterina, uma complicação ainda não descrita, até onde pudemos investigar.
