Objectives: To estimate the prevalence of obsessive-compulsive symptoms (OCS) and disorder (OCD) among adolescents and to describe OCD characteristics according to gender. Methods: Participants were selected by cluster sampling at seven high-schools in southern Brazil. In the first stage, 2,323 students were screened for OCS; in the second stage, adolescents scoring ≥ 21 on the OCI-R scale were individually interviewed. OCD diagnosis was established using a semi-structured interview (Schedule for Affective Disorders and Schizophrenia for School Aged Children: Present and Lifetime Version - K-SADS-PL). Results: The past-month estimated prevalence of OCS was 18.3%, and the point estimated prevalence of OCD, 3.3%. Girls showed higher scores (OCS: 24.8 vs. 14.4%; OCD: 4.9 vs. 1.4%; p < 0.001). Only 9.3% of OCD adolescents had been diagnosed and 6.7% received treatment. The most frequent/severe DY-BOCS dimensions were miscellaneous (86.7%; mean score 6.3±3.8) and symmetry (85.3%; 5.9±3.8). Female OCD adolescents predominantly showed depression (p = 0.032), and male adolescents, tic disorders (p = 0.006). Conclusions: OCD is underdiagnosed in adolescents, and few are treated. Future studies should investigate the relationship between OCS and the onset of OCD.

Objective: To compare adolescents with and without obsessive-compulsive disorder (OCD) with regard to quality of life and to investigate the association between quality of life and clinical characteristics. Methods: Participants were recruited from an epidemiological study conducted at high schools in the city of Porto Alegre, southern Brazil. The sample comprised 75 adolescents with OCD and 150 without the disorder, aged between 14 and 18 years. Participants were assessed using the following instruments: Schedule for Affective Disorders and Schizophrenia for School Aged Children - Present and Lifetime Version (K-SADS-PL), Yale-Brown Obsessive-Compulsive Scale (Y-BOCS), World Health Organization Quality of Life Assessment - Abbreviated Version (WHOQOL-BREF), Beck Anxiety Inventory (BAI), and Beck Depressive Inventory - II (BDI-II). Results: The two groups showed significant differences in relation to depression symptoms, anxiety symptoms, and quality of life (all domains), with a poorer performance among adolescents with OCD when compared to those without the disorder. Stepwise regression analysis revealed a significant association between BDI-II scores and quality of life, in all domains. Conclusions: Our findings suggest that adolescents with OCD, especially those with depression symptoms, have a poorer quality of life when compared with adolescents without OCD.

INTRODUÇÃO: O Obsessional Beliefs Questionnaire (OBQ-44) é um instrumento autorrespondido composto por 44 itens que avaliam as crenças de pacientes com transtorno obsessivo-compulsivo (TOC). O objetivo do presente estudo foi descrever o processo de tradução e adaptação da referida escala para o português do Brasil. MÉTODO: Para o processo de tradução e adaptação do OBQ-44, primeiramente foi obtida licença de uso da escala junto aos autores do instrumento original. Em seguida, a escala foi traduzida de inglês para português brasileiro de forma independente por dois profissionais de saúde com fluência em inglês. Comparando-se as duas escalas traduzidas, obteve-se uma versão preliminar, que foi aplicada a uma amostra de 20 pacientes com diagnóstico principal de TOC, visando observar sua compreensão e realizar adaptações de linguagem. A seguir, a escala foi retrotraduzida de forma independente por dois psiquiatras, também fluentes na língua inglesa. Comparadas as duas retrotraduções, gerou-se uma versão final em língua inglesa; esta versão foi avaliada e aprovada pelos autores do instrumento original. RESULTADOS: A versão em português do Brasil do OBQ-44, após sua tradução e adaptação, demonstrou ser de fácil compreensão por parte de pacientes de diferentes níveis educacionais. Portanto, a escala pode ser utilizada em pacientes das diversas classes econômicas e sociais que caracterizam o Brasil. CONCLUSÃO: O OBQ-44, por ser um instrumento autoaplicável e de fácil compreensão, pode ser útil na identificação de crenças disfuncionais em pacientes com TOC, auxiliando na compreensão do papel dessas crenças na origem e manutenção do transtorno.

INTRODUCTION: The Obsessional Beliefs Questionnaire (OBQ-44) is a self-administered instrument comprised of 44 items, designed to assess the beliefs of patients with obsessive compulsive disorder (OCD). The objective of this study was to describe the process of translation and adaption of the questionnaire into Brazilian Portuguese. METHOD: For the translation and adaptation of the OBQ-44, we first obtained authorization from the authors of the original scale to use the instrument. Subsequently, the scale was independently translated from English into Brazilian Portuguese by two health professionals with proficiency in English. Following comparison of the two translations, a preliminary version was obtained and tried out on a sample of 20 patients with a primary diagnosis of OCD. This pretest aimed to assess the patients' understanding of the items and to make any necessary language adaptations. Then, the scale was independently back-translated by two psychiatrists, also with proficiency in English. Following comparison of the two back-translations, a final version in English was developed; this version was evaluated and approved by the authors of the original instrument. RESULTS: The Brazilian Portuguese version of the OBQ-44, after the process of translation and adaptation here described, showed to be of easy interpretation by patients with different educational levels. The instrument can therefore be used to assess patients from different Brazilian socioeconomic contexts. CONCLUSION: OBQ-44 is a self-administered instrument of easy application. Therefore, it can be useful in the identification of dysfunctional beliefs in OCD patients, contributing toward a better understanding of the role played by such beliefs in the onset and maintenance of the disorder.

OBJETIVO: Avaliar se os resultados obtidos com 12 sessões de terapia cognitivo-comportamental em grupo para pacientes com transtorno obsessivo-compulsivo foram mantidos depois de dois anos do final do tratamento e se o grau de remissão dos sintomas esteve associado às recaídas. MÉTODO: Quarenta e dois pacientes foram acompanhados. A gravidade dos sintomas foi avaliada no final da terapia cognitivo-comportamental em grupo, 18 e 24 meses após o término do tratamento. As escalas utilizadas para avaliação foram Yale-Brown Obsessive-Compulsive Scale, Clinical Global Impression, Beck Depression Inventory e Beck Anxiety Inventory. RESULTADOS: A redução da gravidade dos sintomas observada no final do tratamento foi mantida durante o período de dois anos de acompanhamento (F = 57,881; p < 0,001). Ao final do tratamento, 9 (21,4%) pacientes apresentaram remissão completa, 22 (52,4%) remissão parcial e 11 (26,2%) não apresentaram mudança na Yale-Brown Obsessive-Compulsive Scale. Dois anos depois, 13 pacientes (31,0%) apresentaram remissão completa dos sintomas, 20 (47,6%) apresentaram remissão parcial, e 9 (21,4%) não apresentaram mudança na Yale-Brown Obsessive-Compulsive Scale. A remissão completa dos sintomas ao final do tratamento foi protetora contra recaídas (χ2 = 4,962; df = 1; p = 0,026). CONCLUSÃO: Nossos achados apontam para a importância da obtenção da remissão completa dos sintomas obsessivo-compulsivos e para a necessidade de novas estratégias terapêuticas que tenham como meta atingir tal objetivo.

OBJECTIVE: The aim of this study was to assess whether the results obtained with 12 sessions of cognitive-behavioral group therapy with obsessive-compulsive patients were maintained after two years, and whether the degree of symptom remission was associated with relapse. METHOD: Forty-two patients were followed. The severity of symptoms was measured at the end of cognitive-behavioral group therapy and at 18 and 24 months of follow-up. The assessment scales used were the Yale-Brown Obsessive-Compulsive Scale, Clinical Global Impression, Beck Depression Inventory, and Beck Anxiety Inventory. RESULTS: The reduction in symptom severity observed at the end of treatment was maintained during the two-year follow-up period (F = 57.881; p < 0.001). At the end of the treatment, 9 (21.4%) patients presented full remission, 22 (52.4%) presented partial remission, and 11 (26.2%) had unchanged scores in the Yale-Brown Obsessive-Compulsive Scale. After two years, 13 patients (31.0%) presented full remission, 20 (47.6%) had partial remission, and 9 (21.4%) had unchanged Yale-Brown Obsessive-Compulsive Scalescores. The full remission of symptoms at the end of the treatment was a protective factor against relapse (χ2 = 4,962; df = 1; p = 0.026). CONCLUSION: Our findings underscore the importance of attaining full remission of obsessive-compulsive symptoms during treatment and the need for new therapeutic strategies to achieve this.

OBJETIVO: Descrever o processo de tradução e adaptação para o português do Brasil da Family Accommodation Scale for Obsessive-Compulsive Disorder - Interviewer-Rated (FAS-IR). MÉTODO: O processo de tradução e adaptação da escala envolveu quatro profissionais de saúde bilíngues. A escala foi inicialmente traduzida de forma independente para o português do Brasil por dois profissionais. A seguir, as duas versões foram comparadas, resultando em uma versão inicial em português que foi aplicada a 15 familiares de pacientes com transtorno obsessivo-compulsivo (TOC), com diferentes níveis de instrução, deliberadamente escolhidos para coletar as sugestões de ajuste linguístico. Subsequentemente, a escala foi retrotraduzida independentemente por outros dois profissionais da saúde. Depois de comparar as duas retrotraduções, uma nova versão da escala foi gerada em inglês. Essa versão foi revisada e aprovada pelos autores da escala original. RESULTADOS: A versão em português do Brasil da escala FAS-IR mostrou ser facilmente compreendida e pode ser usada em familiares de pacientes com TOC de diferentes níveis socioeconômicos. CONCLUSÃO: A FAS-IR adaptada ao português do Brasil fará com que profissionais de saúde possam avaliar o nível de acomodação em familiares de pacientes com TOC e permitirá a realização de futuros estudos com os objetivos de 1) estudar a influência da acomodação familiar na manutenção e, possivelmente, na facilitação dos sintomas do TOC e 2) examinar o efeito da acomodação familiar sobre os resultados do tratamento em populações de países de língua portuguesa.

OBJECTIVE: To describe the process of translation and adaptation into Brazilian Portuguese of the Family Accommodation Scale for Obsessive-Compulsive Disorder - Interviewer-Rated (FAS-IR). METHOD: The process of translation and adaptation of the scale involved four bilingual health professionals. The scale was initially translated into Brazilian Portuguese independently by two professionals. Then, the two versions were compared, resulting in a preliminary Portuguese version that was administered to 15 relatives of patients diagnosed with obsessive-compulsive disorder (OCD) with different education levels, deliberately chosen to collect language adjustment suggestions. Subsequently, the scale was translated back into English independently by two other professionals. After comparing the two back translations, a new English version was generated. This version was reviewed and approved by the authors of the original scale. RESULTS: The Brazilian Portuguese version of the FAS-IR proved to be easily understood and can be used in relatives of OCD patients from different socioeconomic backgrounds. CONCLUSION: Adaptation of the FAS-IR into Brazilian Portuguese will enable health professionals to assess the level of accommodation in relatives of OCD patients and allows the development of future studies aimed at 1) studying the influence of family accommodation on maintaining and possibly facilitating OCD symptoms, and 2) assessing the effect of family accommodation on treatment outcomes in Portuguese-speaking populations.

OBJETIVO: O transtorno de pânico é uma condição crônica e recorrente que prejudica a qualidade de vida e o funcionamento psicossocial dos portadores. Embora os medicamentos sejam efetivos na redução dos ataques de pânico, muitos pacientes não respondem adequadamente a essas intervenções. A terapia cognitivo-comportamental fornece um método alternativo eficaz para tratar transtorno de pânico e evitação agorafóbica. O objetivo do estudo é o de descrever o uso de técnicas cognitivo-comportamentais no tratamento do transtorno de pânico. MÉTODO: Revisão narrativa a partir dos bancos de dados do Medline, SciELO e PsycInfo e de livros-texto especializados. RESULTADOS: Foram descritos os fundamentos da terapia cognitivo-comportamental no tratamento do transtorno pânico e revisadas as evidências de eficácia em curto e longo prazos. O uso de medicação concomitante a terapia cognitivo-comportamental foi também discutido. CONCLUSÕES: A terapia cognitivo-comportamental individual ou em grupo é eficaz para pacientes com transtorno de pânico, seja como tratamento de primeira linha ou como um próximo passo para pacientes com resposta parcial a outros tratamentos.

OBJECTIVE: Panic disorder is a chronic and recurrent condition that impairs an individual's psychosocial functioning and quality of life. Despite the efficacy of psychopharmacological treatment in reducing panic attacks, many patients fail to respond adequately to these interventions. Cognitive behavioral therapy provides an alternative and efficacious method for treating panic disorder and agoraphobic avoidance. The objective of the study is to describe the use of cognitive behavioral therapy for panic disorder. METHOD: Narrative review of data collected from Medline, SciELO and PsycInfo and specialized textbooks. RESULTS: We describe the cognitive-behavioral model for the treatment of panic disorder, and review both short and long-term efficacy findings. We also discuss the role of combined treatment (cognitive behavioral therapy and psychopharmacology). CONCLUSIONS: Cognitive behavioral therapy, either individual or in group, can be used as first-line therapy for panic disorder. This treatment modality can also be indicated as a next step for patients failing to respond to other treatments.

OBJETIVO: Descrever a terapia cognitivo-comportamental no tratamento dos sintomas do transtorno obsessivo-compulsivo. MÉTODO: Por meio da revisão de artigos e livros-texto, descrever as origens e os fundamentos da terapia cognitivo-comportamental no tratamento dos sintomas do transtorno obsessivo-compulsivo. Por meio da revisão no Medline de ensaios clínicos randomizados e metanálises, apontar as evidências de eficácia dessa modalidade de tratamento. RESULTADOS E CONCLUSÕES: A terapia cognitivo-comportamental é efetiva na redução dos sintomas obsessivo-compulsivos em aproximadamente 70% dos pacientes que aderem ao tratamento. São desafios futuros esclarecer as razões pelas quais muitos portadores não respondem ao tratamento e desenvolver novas estratégias para aumentar sua efetividade.

OBJECTIVE: To describe the cognitive-behavioral therapy in the treatment of the symptoms of obsessive-compulsive disorder. METHOD: Through the revision of text books and articles the origins and fundamentals of cognitive-behavioral therapy in the treatment of the symptoms of obsessive-compulsive disorder are presented. Through the review of randomized clinical trials and meta-analysis in Medline the evidences of effectiveness of cognitive-behavioral therapy in obsessive-compulsive disorder are highlighted. RESULTS AND CONCLUSIONS: The cognitive-behavioral therapy is effective in reducing symptoms of obsessive-compulsive disorder by approximately 70% of patients who adhere to treatment. The challenges ahead are to clarify the reasons why many patients do not respond to this kind of treatment and develop new strategies to increase its effectiveness.

OBJETIVO: Descrever o recrutamento de pacientes, instrumentos de avaliação, métodos para o desenvolvimento de estudos colaborativos multicêntricos e os resultados preliminares do Consórcio Brasileiro de Pesquisa em Transtornos do Espectro Obsessivo-Compulsivo, que inclui sete centros universitários. MÉTODO: Este estudo transversal incluiu entrevistas semi-estruturadas (dados sociodemográficos, histórico médico e psiquiátrico, curso da doença e diagnósticos psiquiátricos comórbidos) e instrumentos que avaliam os sintomas do transtorno obsessivo-compulsivo (Escala para Sintomas Obsessivo-Compulsivos de Yale-Brown e Escala Dimensional para Sintomas Obsessivo-Compulsivos de Yale-Brown), sintomas depressivos (Inventário de Depressão de Beck), sintomas ansiosos (Inventário de Ansiedade de Beck), fenômenos sensoriais (Escala de Fenômenos Sensoriais da Universidade de São Paulo), juízo crítico (Escala de Avaliação de Crenças de Brown), tiques (Escala de Gravidade Global de Tiques de Yale) e qualidade de vida (questionário genérico de avaliação de qualidade de vida, Medical Outcome Quality of Life Scale Short-form-36 e Escala de Avaliação Social). O treinamento dos avaliadores consistiu em assistir cinco entrevistas filmadas e entrevistar cinco pacientes junto com um pesquisador mais experiente, antes de entrevistar pacientes sozinhos. A confiabilidade entre todos os líderes de grupo para os instrumentos mais importantes (Structured Clinical Interview for DSM-IV, Dimensional Yale-Brown Obsessive-Compulsive Scale, Universidade de São Paulo Sensory Phenomena Scale ) foi medida após seis entrevistas completas. RESULTADOS: A confiabilidade entre avaliadores foi de 96%. Até março de 2008, 630 pacientes com transtorno obsessivo-compulsivo tinham sido sistematicamente avaliados. A média de idade (±SE) foi de 34,7 (±0,51), 56,3% eram do sexo feminino e 84,6% caucasianos. Os sintomas obsessivo-compulsivos mais prevalentes foram os de simetria e os de contaminação. As comorbidades psiquiátricas mais comuns foram depressão maior, ansiedade generalizada e transtorno de ansiedade social. O transtorno de controle de impulsos mais comum foi escoriação neurótica. CONCLUSÃO: Este consórcio de pesquisa, pioneiro no Brasil, permitiu delinear o perfil sociodemográfico, clínico e terapêutico do paciente com transtorno obsessivo-compulsivo em uma grande amostra clínica de pacientes. O Consórcio Brasileiro de Pesquisa em Transtornos do Espectro Obsessivo-Compulsivo estabeleceu uma importante rede de colaboração de investigação clínica padronizada sobre o transtorno obsessivo-compulsivo e pode abrir o caminho para projetos semelhantes destinados a integrar outros grupos de pesquisa no Brasil e em todo o mundo.

OBJECTIVE: To describe the recruitment of patients, assessment instruments, implementation, methods and preliminary results of The Brazilian Research Consortium on Obsessive-Compulsive Spectrum Disorders, which includes seven university sites. METHOD: This cross-sectional study included a comprehensive clinical assessment including semi-structured interviews (sociodemographic data, medical and psychiatric history, disease course and comorbid psychiatric diagnoses), and instruments to assess obsessive-compulsive (Yale-Brown Obsessive-Compulsive Scale and Dimensional Yale-Brown Obsessive-Compulsive Scale), depressive (Beck Depression Inventory) and anxious (Beck Anxiety Inventory) symptoms, sensory phenomena (Universidade de São Paulo Sensory Phenomena Scale), insight (Brown Assessment Beliefs Scale), tics (Yale Global Tics Severity Scale) and quality of life (Medical Outcome Quality of Life Scale Short-form-36 and Social Assessment Scale). The raters' training consisted of watching at least five videotaped interviews and interviewing five patients with an expert researcher before interviewing patients alone. The reliability between all leaders for the most important instruments (Structured Clinical Interview for DSM-IV, Dimensional Yale-Brown Obsessive-Compulsive Scale, Universidade de São Paulo Sensory Phenomena Scale) was measured after six complete interviews. RESULTS: Inter-rater reliability was 96%. By March 2008, 630 obsessive-compulsive disorder patients had been systematically evaluated. Mean age (±SE) was 34.7 (±0.51), 56.3% were female, and 84.6% Caucasian. The most prevalent obsessive compulsive symptom dimensions were symmetry and contamination. The most common comorbidities were major depression, generalized anxiety and social anxiety disorder. The most common DSM-IV impulsive control disorder was skin picking. CONCLUSION: The sample was composed mainly by Caucasian individuals, unmarried, with some kind of occupational activity, mean age of 35 years, onset of obsessive-compulsive symptoms at 13 years of age, mild to moderate severity, mostly of symmetry, contamination/cleaning and comorbidity with depressive disorders. The Brazilian Research Consortium on Obsessive-Compulsive Spectrum Disorders has established an important network for standardized collaborative clinical research in obsessive-compulsive disorder and may pave the way to similar projects aimed at integrating other research groups in Brazil and throughout the world.

OBJETIVO: Este artigo apresenta o processo de tradução e adaptação das escalas Obsessive-Compulsive Inventory e do Obsessive-Compulsive Inventory-Revisado versão em português do Brasil. O Obsessive-Compulsive Inventory foi desenvolvido com o objetivo de mensurar os diversos sintomas que caracterizam o transtorno obsessivo-compulsivo e avaliar a freqüência e a ansiedade causada pelos sintomas no último mês, assim como estimar a gravidade geral do transtorno. Desta forma, torna-se possível fazer comparações de gravidade entre as várias obsessões e compulsões. MÉTODO: As escalas foram inicialmente traduzidas para o português por dois psiquiatras bilíngües e retrotraduzidas por outros dois psiquiatras bilíngües de forma independente e em seguida, aplicadas em 15 portadores do transtorno obsessivo-compulsivo com diferente escolaridade e intencionalmente selecionados para efetuar ajustes na linguagem. A versão final das escalas Obsessive-Compulsive Inventory e Obsessive-Compulsive Inventory-Revisado foram aceitas pelo autor após o processo de retrotradução destas. RESULTADOS: As escalas mostraram-se de fácil compreensão e preenchimento pelos pacientes, podendo ser utilizadas em pacientes com transtorno obsessivo-compulsivo de diferentes classes econômicas e sociais. CONCLUSÃO: As escalas Obsessive-Compulsive Inventory e Obsessive-Compulsive Inventory-Revisado, em suas versões adaptadas para o português do Brasil, poderão auxiliar os profissionais de saúde no rastreamento de possíveis portadores de transtorno obsessivo-compulsivo e na avaliação da intensidade dos sintomas obsessivos compulsivos em pesquisa, e sua redução com diferentes métodos de tratamento.

OBJECTIVE: The present study describes the process of translation into Brazilian Portuguese and the cross-cultural adaptation of the Obsessive-Compulsive Inventory and the Obsessive-Compulsive Inventory-Revised scales. The Obsessive-Compulsive Inventory was developed with the purpose of measuring the intensity of the various symptoms that characterize the obsessive-compulsive disorder, assessing their frequency and the distress they caused during the previous month, as well as estimating the overall severity of the disorder. Thus, different levels of severity among different obsessions and compulsions can be assessed and compared. METHOD: The scales were initially translated into Brazilian Portuguese by two bilingual psychiatrists and then independently back-translated by other two bilingual psychiatrists. The scales were then applied to 15 obsessive-compulsive disorder patients, deliberately chosen from different educational levels, to make language adjustments. The author accepted the final version of the Obsessive-Compulsive Inventory and the Obsessive-Compulsive Inventory-Revised scales after their back translation. RESULTS: The scales were easily understood and filled in by individuals and may be used with obsessive-compulsive disorder patients of different socioeconomic levels. CONCLUSION: The Obsessive-Compulsive Inventory and the Obsessive-Compulsive Inventory-Revised scales, in their Brazilian Portuguese version, can help health professionals to screen potential obsessive-compulsive disorder patients, assess the severity of obsessive-compulsive symptoms and reduce these symptoms using different treatments.

INTRODUÇÃO: As terapias comportamental e cognitivo-comportamental reduzem os sintomas do transtorno obsessivo-compulsivo em mais de 70% dos pacientes. Entretanto, cerca de 30% não obtêm nenhuma melhora. Conhecer fatores associados a esses desfechos poderia auxiliar numa melhor indicação do tratamento, incrementando sua eficácia. MÉTODO: Foram revisados trabalhos que investigaram fatores preditivos de resultados nos tratamentos do transtorno obsessivo-compulsivo, nas fontes PubMed, PsycINFO e LILACS. Termos utilizados na busca: predictive factors OR prediction AND obsessive-compulsive disorder AND exposure response prevention OR ritual prevention OR behav* therapy OR cognitive behav* therapy, resultando em 104 artigos. As referências dos artigos foram analisadas, possibilitando maior número de trabalhos revisados. Excluíram-se artigos que utilizaram apenas farmacoterapia e aqueles que não abordavam o tema, resultando em 29 artigos. DISCUSSÃO: As variáveis demográficas parecem exercer papel indireto, destacando-se sexo masculino e ausência de companheiro(a) como fatores de pior prognóstico. Maior gravidade e início precoce dos sintomas indicariam piores resultados. O transtorno esquizotípico comórbido é potencialmente negativo. Sintomas relacionados ao colecionismo e com conteúdo sexual/religioso indicam pior prognóstico. Maior nível de insight, motivação e colaboração com o tratamento são características favoráveis. Maior intensidade da melhora e a remissão completa são preditores para a não-recaída. CONCLUSÃO: A identificação de fatores preditores de resultados está longe de ser esclarecida. É possível que múltiplos fatores, cujas associações são complexas, atuem em conjunto. Os resultados podem depender de fatores não-específicos, ainda não abordados pelos estudos. A heterogeneidade do transtorno obsessivo-compulsivo e das pesquisas e o uso associado de medicamentos dificultam o estabelecimento de conclusões mais definidas.

INTRODUCTION: Behavioral and cognitive-behavioral therapies reduce symptoms of obsessive-compulsive disorder in over 70% of patients. However, about 30% do not show any improvement. The knowledge of factors associated with these outcomes may inform better treatment indications and improve treatment efficacy. METHOD: Review of studies that investigated predictors of obsessive-compulsive disorder treatment results in PubMed, PsychoINFO and LILACS databases. Terms used in the search were predictive factors OR prediction AND obsessive-compulsive disorder AND exposure response prevention OR ritual prevention OR behav* therapy OR cognitive behav* therapy." The search retrieved 104 studies. The references of retrieved studies were also analyzed to ensure that all relevant studies were included. Studies that used only pharmacotherapy or did not discuss the topic under analysis were excluded, and 29 studies met inclusion criteria. DISCUSSION: Demographic variables seem to play an indirect role in treatment results; male sex and not having partner are factors associated with poor prognosis. Greater severity and early symptom onset also indicate poor outcomes. Comorbid schizotypal disorder is potentially negative. Symptoms associated with hoarding and sexual/religious obsessions predict poor prognosis. Greater insight, motivation and collaboration with treatment are favorable characteristics. Greater improvement and complete remission of symptoms are predictors of no relapse. CONCLUSION: The identification of outcome predictors is still far from complete. Multiple factors may contribute to results, and their associations are complex. Results may depend on unspecific factors not yet investigated. Definite conclusions are difficult to reach because of the heterogeneity of obsessive-compulsive disorder and of the studies investigating it and the combined use of drugs.

OBJETIVO: O transtorno obsessivo-compulsivo acomete entre 1,6 a 3,1% da população. Em virtude de uma maior compreensão do sofrimento associado aos sintomas, aumentaram os estudos e o interesse no bem-estar geral e na qualidade de vida dos portadores. MÉTODO: Foi realizada uma revisão bibliográfica de trabalhos que investigaram a qualidade de vida em portadores de transtorno obsessivo-compulsivo. A busca foi feita nas bases de dados Medline, SciELO e PsychoInfo, utilizando como palavras-chave quality of life e obsessive-compulsive disorder. Foram selecionados 27 artigos que abordaram o tema, sendo descritas as áreas mais afetadas pelo transtorno, bem como a sua relação com a qualidade de vida foram descritas. Também foram apresentados efeitos de tratamentos na qualidade de vida dos pacientes. RESULTADOS: Os relacionamentos sociais e familiares, seguidos pelo desempenho ocupacional (capacidade para trabalhar e estudar), foram as áreas mais afetadas pelo transtorno e, apesar de melhorarem com tratamento, permanecem insatisfatórios. As obsessões foram associadas a prejuízos mais significativos na qualidade de vida do que as compulsões (rituais). CONCLUSÕES: Apesar das limitações, os estudos mostram evidências do comprometimento da qualidade de vida de indivíduos com transtorno obsessivo-compulsivo, sendo este comparável ao de pacientes com esquizofrenia. Pesquisas futuras com diferentes delineamentos são necessárias para estabelecer resultados mais consistentes.

OBJECTIVE: Obsessive-compulsive disorder affects between 1.6 and 3.1% of the population. Due to the distress caused by obsessive-compulsive disorder leading to disability as well as the prevalence of the disease, there has been an increase in the number of studies focusing on the general well-being of patients by assessing quality of life. METHOD: A literature review of the studies that investigated the quality of life of patients with obsessive-compulsive disorder was performed. The search was carried out in the following database: Medline, SciELO and PsychoInfo, and the following key-words were used "quality of life" and "obsessive-compulsive disorder". Twenty-seven articles about the topic were selected. The results from this study describe the areas most severely affected by the disorder as well as the relationship between obsessive-compulsive disorder and quality of life. In addition, the effects of the treatment on the patients' quality of life are presented. RESULTS: The social and familial relationships and the occupational performance (capacity to work and study) were the areas most severely affected by the disorder, and, although there was an improvement with the treatment, these areas remain at a poor level of performance. The obsessions were associated with the most significant impairment of the quality of life if compared to the compulsions (rituals). CONCLUSIONS: Results from the selected studies suggest that obsessive-compulsive disorder patients have an impairment of quality of life. The level of impairment is similar to that of schizophrenic patients. Future studies with different designs are necessary so that more consistent results can be established.
