El Mantenimiento en las Relaciones de Pareja: Una Forma de Medirlo

Resumen El presente estudio tuvo como objetivo conocer la estructura factorial de la escala de mantenimiento en las relaciones de pareja en hombres y mujeres. Participaron de manera voluntaria y confidencial 731 personas (49.8% hombres y 50.2% mujeres). Se elaboró una escala con 178 reactivos, para evaluar el mantenimiento de las relaciones de pareja con base en un estudio exploratorio, con un formato de respuesta tipo Likert de 5 puntos. Esta consta de cuatro subescalas: Significado (46 reactivos y 6 factores), Motivos (30 reactivos y cuatro factores), Estrategias (46 reactivos y 6 factores) y Consecuencias (56 reactivos y 4 factores). Para cada subescala se obtuvo el análisis psicométrico. Los resultados indican una escala valida y confiable en sus cuatro subescalas, a través de análisis factoriales exploratorios y análisis de consistencia interna. Los hallazgos permiten contar con un instrumento robusto que permite evaluar en sus diferentes subescalas las áreas que componen el mantenimiento en la pareja, esto con la finalidad de realizar evaluaciones en las relaciones diádicas que fomenten parejas saludables, así como intervenciones, basadas en esta primera aproximación.
Abstract The purpose of this research was to obtain the factor structure of the Couple Relationship Maintenance Scale using a Mexican sample of men and women. A total of 731 voluntary people (49.8% men, 50.2% women) were part of this study and agreed to do so confidentially. Based on results from an exploratory study, the scale was initially developed considering 178 items in order to assess maintenance of couple relationships with a 5-point Likert response style. The scale was divided into four subscales: Meaning (46 items, 6 factors), Motives (30 items, 4 factors), Strategies (46 items, 6 factors), and Consequences (56 items, 4 factors). Each subscale required its own psychometric analysis. Exploratory factor analysis and internal consistency analysis show a valid and reliable scale in all four subscales. These findings allow for a robust scale that allows the evaluation of maintenance throughout several of its components. The development of instruments like the one presented becomes fundamental in the evaluation of dyadic relationships that in turn develop healthy couple relationships, as well as evidence-based interventions.

RESUMO No XXVIII Congresso Brasileiro da SBFa, 24 especialistas reuniram-se e, a partir de um posicionamento condutor sobre pesquisa científica como ferramenta de conexão entre laboratório e clínica, cinco frentes de conhecimento da especialidade de voz foram discutidas: 1. Julgamento perceptivo-auditivo da qualidade vocal; 2. Análise acústica do sinal vocal; 3. Autoavaliação em voz; 4. Técnicas tradicionais de terapia; 5. Técnicas modernas de eletroestimulação e fotobiomodulação em voz. A parte “a” desta publicação é a consolidação das análises dos três primeiros aspectos. A tendência no julgamento perceptivo-auditivo da qualidade vocal é o uso de protocolos padrão. A avaliação acústica do sinal vocal é acessível e pode ser feita de modo descritivo ou por extração de parâmetros, preferindo-se medidas multiparamétricas. Finalmente, a análise do próprio indivíduo fecha essa tríade de documentação fonoaudiológica, que será base para a conclusão da avaliação, referência para monitoramento do progresso e avaliação de resultado de tratamento.
ABSTRACT During the XXVIII Brazilian Congress of SBFa, 24 specialists met and, from a leading position on scientific research as a tool for connecting laboratory and clinic, five fronts of knowledge of the voice specialty were discussed as following: Perceptual-auditory judgment of vocal quality; 2. Acoustic analysis of the vocal signal; 3. Voice self-assessment; 4. Traditional techniques of therapy; 5. Modern techniques of electrostimulation and photobiomodulation (PBMT) in voice. Part “a” of this publication was associated with the consolidation of the analyses of the first three aspects. The trend in the perceptual-auditory judgment of vocal quality was related to the use of standard protocols. The acoustic evaluation of the vocal signal is accessible and can be done descriptively or by extraction of parameters, thus preferring multiparametric measures. Finally, the analysis of the individual himself closes this triad of voice documentation, which will be the basis for the conclusion of the evaluation, reference for monitoring progress, and evaluation of treatment results.

RESUMO O presente texto é a continuação da publicação referente ao XVIII Congresso da SBFa. Na parte “A” apresentamos análises sobre avaliação clínica vocal. O foco da parte “B” são aspectos de reabilitação vocal: 4. Técnicas tradicionais de terapia vocal; 5. Técnicas modernas de eletroestimulação e fotobiomodulação aplicadas à reabilitação. Os inúmeros estudos sobre os diversos programas, métodos ou técnicas tradicionais de reabilitação, muitos de elevada qualidade de evidência, permitem considerar tais procedimentos relativamente bem descritos, seguros e com efeitos conhecidos, dando conta do tratamento de diversos distúrbios vocais. As evidências científicas com as técnicas tradicionais são reconhecidas mundialmente. Novas frentes de evolução, como o uso da eletroestimulação ou fotobiomodulação em voz parecem ser promissoras como abordagens coadjuvantes. Há mais estudos sobre eletroestimulação em voz do que com fotobiomodulação, contudo, evidências científicas para essas duas técnicas modernas são ainda limitadas. Conhecimento e cautela são necessários para a aplicação de quaisquer técnicas.
ABSTRACT This text is the continuation of the XVIII SBFa Congress publication. In part “A” we presented the analyses on clinical vocal evaluation. Part “B” focuses on vocal rehabilitation: 4. Traditional techniques of vocal therapy; 5. Modern techniques of electrostimulation and photobiomodulation applied to vocal rehabilitation. The numerous studies on the various programs, methods, and techniques of traditional rehabilitation techniques, and many with high quality of evidence, allow us to consider such procedures relatively well described, safe, and with known effects, accounting for the treatment of various vocal disorders. The scientific evidence with traditional techniques is recognized worldwide. New fronts of evolution, with electrostimulation or photobiomodulation used to handle voice problems, seem to be promising as coadjutant approaches. There are more studies on electrostimulation in vocal rehabilitation than with photobiomodulation; however, scientific evidence for these two modern techniques is still limited. Knowledge and caution are required for the application of either technique.

Abstract Recent studies have reported the occurrence of thrombotic phenomena or coagulopathy in patients with COVID-19. There are divergent positions regarding the prevention, diagnosis, and treatment of these phenomena, and current clinical practice is based solely on deductions by extension from retrospective studies, case series, observational studies, and international guidelines developed prior to the pandemic. In this context, the aim was to generate a group of recommendations on the prevention, diagnosis and management of thrombotic complications associated with COVID-19. Methods: A rapid guidance was carried out applying the GRADE Evidence to Decision (EtD) frameworks and an iterative participation system, with statistical and qualitative analysis. Results: 31 clinical recommendations were generated focused on: a) Coagulation tests in symptomatic adults with suspected infection or confirmed SARS CoV-2 infection; b) Thromboprophylaxis in adults diagnosed with COVID-19 (Risk scales, thromboprophylaxis for outpatient, in-hospital management, and duration of thromboprophylaxis after discharge from hospitalization), c) Diagnosis and treatment of thrombotic complications, and d) Management of people with previous indication of anticoagulant agents. Conclusions: Recommendations of this consensus guide clinical decision-making regarding the prevention, diagnosis, and treatment of thrombotic phenomena in patients with COVID-19, and represent an agreement that will help decrease the dispersion in clinical practices according to the challenge imposed by the pandemic.

Complete Screening of Exons 2, 3, and 4 of KRAS and NRAS Genes Reveals a Higher Number of Clinically Relevant Mutations than Food and Drug Administration Quantitative Polymerase Chain Reaction-Based Commercial Kits

Abstract Background: The presence of clinically relevant mutations in KRAS and NRAS genes determines the response of anti-epidermal growth factor receptor antibody therapy for metastatic colorectal cancer (mCRC). The only quantitative polymerase chain reaction (qPCR)-based diagnostic tests approved by the Food and Drug Administration (FDA) screen merely for mutations in codons 12 and 13 of KRAS. Objective: The objective of the study was to study the frequency of clinically relevant mutations in KRAS and NRAS genes that are not included in FDA-approved qPCR tests. Methods: Formalin-fixed paraffin-embedded tumor specimens from 1113 mCRC Mexican patients from different health institutions across the country were analyzed by Sanger sequencing for KRAS mutations in exons 2, 3, and 4. Furthermore, 83 were analyzed in exons 2, 3, and 4 of NRAS. Results: From the specimens tested for KRAS, 33.69% harbored a mutation. From these, 71.77% were in codon 12 and 27.69% in codon 13 (both located in exon 2). Codons 59 (exon 3) and 146 (exon 4) accounted for the remaining 0.54%. From the 83 specimens, in which NRAS was analyzed, three mutations were found in codon 12 (3.61%). Approximately 6% of RAS mutated specimens would have been falsely reported as RAS wild type if an FDA-approved qPCR diagnostic test had been used. Conclusions: While these kits based on qPCR can be very practical and highly sensitive, their mutation coverage ignores mutations from poorly genetically characterized populations.

Abstract Qualitative research to elucidate social constructions about dengue, chikungunya and zika in an endemic municipality of Colombia. Sixty-one people with subjective experience of disease participated. Semi-structured and open interviews, informal dialogues and observation notes record were used. The analysis was performed following the inductive method of social research. Two ways of understanding dengue are evident: as a “normal” disease or as a serious disease that can cause death. Chikungunya and zika are conceived as new diseases, difficult to recognize, that fall into social oblivion due to the low occurrence of cases. Two therapeutic itineraries for the three diseases are identified, mediated by the severity of the symptoms and the perception of the attention received by the health services. Drugstores are configured as a key element of therapeutic itineraries. It is concluded that the social construction of these diseases is crossed by a tension between recognition and forgetfulness. Public policies and actions aimed at the prevention and control of these events have the challenge of facing the “forgetfulness” that daily life brings, therefore they should not be limited to approaches that ignore the ways in which diseases are lived and faced.
Resumen Investigación cualitativa para dilucidar construcciones sociales sobre el dengue, chikungunya y zika en un municipio endémico de Colombia. Participaron 61 personas con experiencia subjetiva de enfermedad. Se emplearon entrevistas semiestructuradas y abiertas, diálogos informales y registro de notas de observación. El análisis se realizó siguiendo el método inductivo de la investigación social. Se evidencian dos maneras de entender el dengue: como una enfermedad “normal” o como una enfermedad grave que puede causar la muerte. El chikungunya y el zika son concebidas como enfermedades nuevas, de difícil reconocimiento, que caen en el olvido social ante la baja ocurrencia de casos. Se identifican dos itinerarios terapéuticos frente a las tres enfermedades, mediados por la gravedad de los síntomas y la percepción de la atención recibida por los servicios sanitarios. Resumen Investigación cualitativa para dilucidar construcciones sociales sobre el dengue, chikungunya y zika en un municipio endémico de Colombia. Participaron 61 personas con experiencia subjetiva de enfermedad. Se emplearon entrevistas semiestructuradas y abiertas, diálogos informales y registro de notas de observación. El análisis se realizó siguiendo el método inductivo de la investigación social. Se evidencian dos maneras de entender el dengue: como una enfermedad “normal” o como una enfermedad grave que puede causar la muerte. El chikungunya y el zika son concebidas como enfermedades nuevas, de difícil reconocimiento, que caen en el olvido social ante la baja ocurrencia de casos. Se identifican dos itinerarios terapéuticos frente a las tres enfermedades, mediados por la gravedad de los síntomas y la percepción de la atención recibida por los servicios sanitarios. Abstract Qualitative research to elucidate social constructions about dengue, chikungunya and zika in an endemic municipality of Colombia. Sixty-one people with subjective experience of disease participated. Semi-structured and open interviews, informal dialogues and observation notes record were used. The analysis was performed following the inductive method of social research. Two ways of understanding dengue are evident: as a “normal” disease or as a serious disease that can cause death. Chikungunya and zika are conceived as new diseases, difficult to recognize, that fall into social oblivion due to the low occurrence of cases. Two therapeutic itineraries for the three diseases are identified, mediated by the severity of the symptoms and the perception of the attention received by the health services. Drugstores are configured as a key element of therapeutic itineraries. It is concluded that the social construction of these diseases is crossed by a tension between recognition and forgetfulness. Public policies and actions aimed at the prevention and control of these events have the challenge of facing the “forgetfulness” that daily life brings, therefore they should not be limited to approaches that ignore the ways in which diseases are lived and faced.

Resumen La Candidiasis Invasora (CI) y la candidemia, como su manifestación más frecuente, se ha convertido en la principal causa de micosis oportunista a nivel hospitalario. Este manuscrito realizado por miembros de la Asociación Colombiana de Infectología (ACIN), tuvo como objetivo proporcionar un conjunto de recomendaciones para manejo, seguimiento y prevención de la CI/candidemia y de la infección candidiásica de mucosas, en población adulta, pediátrica y neonatal, en un entorno hospitalario, incluyendo las unidades hemato-oncológicas y unidades de cuidado crítico. Todos los datos obtenidos mediante una búsqueda exhaustiva, fueron revisados y analizados de manera amplia por todos los miembros del grupo, y las recomendaciones emitidas se elaboraron luego de la evaluación de la literatura científica disponible, y el consenso de todos los especialistas involucrados, reconociendo el problema de la emergencia de las infecciones por Candida Spp. y brindando una correcta orientación a los profesionales de la salud sobre el manejo de pacientes con enfermedad candidiásica, de una forma racional y práctica, enfatizando en la evaluación del paciente, estrategias de diagnóstico, profilaxis, tratamiento empírico, tratamiento dirigido y terapia preventiva.
Abstract Invasive Candidiasis (IC) and candidemia (as its most frequent manifestation) have become the main cause of opportunistic mycosis at hospital settings. This study, made by members of the Colombian Association of Infectious Diseases (ACIN), was aimed at providing a set of recommendations for the management, follow-up and prevention of IC / candidemia and mucous membrane candida infection in adult, pediatric and neonatal patients in a hospital setting, including the hemato-oncological and critical care units. All the data obtained through an exhaustive search were reviewed and analyzed in a comprehensive manner by all the members of the group, and the recommendations issued are being made after a careful review of the scientific literature available and the consensus of all specialists involved; the emergence of Candida Spp. problem is highlighted and a correct orientation to health professionals regarding the management of patients with candidiasis is provided in a rational and practical way, emphasizing patient evaluation, diagnostic strategies, prophylaxis, empirical treatment, directed treatment and preventative therapy.

IMPACTOS DO SURTO DE FEBRE AFTOSA DE 2005 SOBRE AS EXPORTAÇÕES DE CARNE BOVINA BRASILEIRA

Resumo A febre aftosa é uma barreira sanitária no comércio internacional e implica elevados investimentos para seu controle e graves prejuízos em casos de surto. Este trabalho investigou os impactos causados pela febre aftosa nas exportações de carne bovina in natura do Brasil após o surto de 2005, além do cumprimento do Princípio da Regionalização do Acordo sobre a Aplicação de Medidas Sanitárias e Fitossanitárias (SPS) pelos países membros da OMC, que compunham a lista dos 10 maiores importadores dessa carne em 2004. Foi realizada análise descritiva das exportações de carne bovina congelada e resfriada (carne bovina in natura), considerando-se os anos de 2004 a 2006, para os 10 maiores importadores desses produtos do Brasil e do mundo em 2004. Verificou-se que o surto não impediu o aumento das exportações de carne bovina in natura do Brasil, mas impactou negativamente as exportações do Mato Grosso do Sul e Paraná. A doença não causou impacto nas exportações para os Estados Unidos, Japão e México. Arábia Saudita, Rússia e Irã não eram Membros da OMC em outubro de 2005 e, portanto, não tinham a obrigação de respeitar o Princípio da Regionalização, ainda que a Rússia o tenha feito. Dentre os demais grandes importadores de 2004, os Países Baixos, Egito, Itália, Reino Unido, França, Alemanha e Espanha respeitaram o Princípio da Regionalização do Acordo SPS. O Chile não respeitou o Princípio da Regionalização e a ocorrência da doença fechou aquele mercado à carne bovina brasileira in natura.
Abstract Foot and Mouth Disease (FMD) can lead to sanitary barriers to international trade and involves high investments for control and great losses in the event of an outbreak. This study investigated the impacts caused by FMD on the exports of fresh beef from Brazil after the 2005 outbreak and the observance of the regionalization principle of the Agreement on the Application of Sanitary and Phytosanitary Measures (SPS) by countries member of the WTO that were listed as the top 10 beef importing countries in 2004. The FMD outbreak that began in 2005 did not limit the increase in exports of fresh beef from Brazil, but impacted negatively on exports from Mato Grosso do Sul and Paraná States. The disease did not impact exports to the United States, Japan or Mexico, since these markets were closed to Brazil. Saudi Arabia, Russia and Iran were not members of the WTO in October 2005 and therefore had no obligation to respect the principle of regionalization, though Russia respected it. Among the other major importers of 2004, the Netherlands, Egypt, Italy, United Kingdom, France, Germany and Spain respected the principle of regionalization of the SPS Agreement. Chile did not respect the principle and the occurrence of the disease closed the market to Brazilian fresh beef.

Use of Antibiotics in Dental Implant Surgery: A Decision Based on Evidence from Systematic Review

Questions remain whether postoperative infections and implant failure can be reduced with the use of antibiotics. Especially, when its routine use can cause adverse effects and may contribute to the development of antibiotic-resistant bacteria. Moreover, there is no consensus regarding appropriate dosage regimen of antibiotics to prevent bacterial infection in implant dentistry. To determine effectiveness of different antibiotics regimens to prevent early infection after implant placement. A systematic review of all relevant studies addressing the use of antibiotic for dental implant surgery was carried-out. Implant groups not using antibiotics, were also analyzed. Primary outcomes were incidence of postoperative infection and frequency of implant failure due to infection. From 164 articles reviewed, 11 fulfilled the selection criteria representing 9472 placed implants. Regimens associated with the use of postoperative antibiotics showed a lower incidence of early infection (postoperative regimens 0%, pre and postoperative regimens 0.22% and 0.53%, long- and short-course, respectively (P= 0.275)). Regarding failure due to infection, no differences between groups were found (P= 0.249). A trend favoring the use of postoperative antibiotic to prevent early infections was observed. Further studies should be carried out in order to provide evidence-based clinical guidelines for use of antibiotics in dental implant placement.
Todavía se mantiene el integorrante acerca de si las infecciones postoperatorias y el fracaso de los implantes se pueden reducir con el uso de antibióticos. Especialmente, cuando su uso rutinario puede causar efectos adversos y puede contribuir al desarrollo de bacterias resistentes a los antibióticos. Por otra parte, no hay consenso en cuanto al régimen de dosificación apropiado de antibióticos para prevenir la infección bacteriana en los implantes dentales. Para determinar la efectividad de los diferentes regímenes de antibióticos destinados a prevenir la infección temprana después de la colocación del implante, se llevó a cabo una revisión sistemática de todos los estudios pertinentes sobre el uso de antibióticos para la cirugía del implante dental. También fueron analizados grupos de pacientes en los que se colocaron implantes sin el uso de antibióticos. Los resultados iniciales demostraron la existencia de infección postoperatoria y el fracaso del implante debido a la infección. De 164 artículos revisados, 11 cumplieron con los criterios de selección, representando un total de 9.472 implantes colocados. Estudios asociados con el uso de antibióticos postoperatorios mostraron una menor incidencia de infección temprana (regímenes postoperatorios 0%, regímenes pre y postoperatoria 0,22% y 0,53%, a largo y corto plazo, respectivamente (P= 0,275)). En cuanto al fracaso debido a la infección, no se encontraron diferencias entre los grupos (p= 0,249). Se observó una tendencia a favor del uso de antibióticos para prevenir infecciones postoperatorias tempranas. Nuevos estudios deben llevarse a cabo con el fin de proporcionar directrices clínicas, basadas en la evidencia, para el uso de antibióticos en la colocación del implante dental.

This article details the experimental and numerical results on the energy absorption performance of square tubular profile with circular discontinuities drilled at lengthwise in the structure. A straight profile pattern was utilized to compare the absorption of energy between the ones with discontinuities under quasi-static loads. The collapse mode and energy absorption conditions were modified by circular holes. The holes were drilled symmetrically in two walls and located in three different positions along of profile length. The results showed a better performance on energy absorption for the circular discontinuities located in middle height. With respect to a profile without holes, a maximum increase of 7% in energy absorption capacity was obtained experimentally. Also, the numerical simulation confirmed that the implementation of circular discontinuities can reduce the peak load (Pmax) by 10%. A present analysis has been conducted to compare numerical results obtained by means of the finite element method with the experimental data captured by using the testing machine. Finally the discrete model of the tube with and without geometrical discontinuities presents very good agreements with the experimental results.

The biological system of a wastewater treatment plant works well with a range of potential of hydrogen (pH) of 6.5 to 8.5. A neutral condition is considered a pH value of 7; however wastewater of industrial processes varies significantly. When the potential of hydrogen is greater than 10 units, the biological treatment may show inhibition, burns to workers of the installation and incompatibility with the physicochemical processes of the treatment plant. Therefore, it is advisable to control the potential of hydrogen from the same plant where it is generated. Or, cover the cost of fees at the discharge of waste water into the drainage system and municipal sewage; i.e., to pay for treatment to the authority in charge of regulating discharges of wastewater. The present research note shows the methodology to determine the non-complaint range of alkaline wastewater, and the cost of fees at the discharge of waste water into the piping system, the latter considering the legal and regulatory framework.
El sistema biológico de una planta de tratamiento de aguas residuales funciona adecuadamente con un rango de potencial de hidrógeno (pH) de 6.5 a 8.5. Una condición neutra se considera un valor de pH de 7; sin embargo, en las aguas residuales de procesos industriales se presentan variaciones importantes. Cuando el potencial de hidrógeno es superior a 10 unidades, el tratamiento biológico puede presentar inhibición, quemaduras a los trabajadores de la instalación e incompatibilidad con los procesos fisicoquímicos de la planta de tratamiento. Por lo tanto, es recomendable controlar el potencial de hidrogeno desde la misma instalación donde se genera. O bien, cubrir los costos por pago de derechos en la descarga de agua residual al sistema de drenaje y alcantarillado municipal; es decir, realizar el pago por tratamiento a la autoridad encargada de regular las descargas de aguas residuales. En la presente nota de investigación se observa la metodología para determinar el rango de incumplimiento de aguas residuales alcalinas, y el costo por pago de derechos en la descarga de agua residual al sistema de tuberías, lo anterior considerando el marco legal y normativo.
