Resumo Objetivo: O objetivo deste estudo prospectivo foi avaliar a dinâmica do estresse oxidativo durante a cirurgia de revascularização miocárdica com circulação extracorpórea. Métodos: Participaram 16 pacientes submetidos à revascularização miocárdica. As amostras de sangue foram coletadas da circulação sistêmica, no momento da indução anestésica (artéria radial - A1), do retorno venoso sistêmico (B1 e B2), quatro minutos após a remoção do pinçamento aórtico, do seio coronariano (SC1 e SC2), quatro minutos após a remoção do pinçamento aórtico, e da circulação sistêmica, quatro minutos após finalização da circulação extracorpórea (artéria radial - A2). O marcador do estresse oxidativo, malondialdeído, foi dosado utilizando espectrofotometria. Resultados: Os valores médios de malondialdeído foram (ng/dl): A1 (265,1), B1 (490,0), SC1 (527,0), B2 (599,6), SC2 (685,0) e A2 (527,2). As comparações entre A1/B1, A1/SC1, A1/B2, A1/SC2, A1/A2 foram significativas, com valores ascendentes (P<0,05). As comparações entre as dosagens do seio coronário e reservatório venoso após os dois momentos de reperfusão (B1/SC1 e B2/SC2) foram mais elevadas no momento SC2 (P<0,05). Apesar dos valores mais elevados após o término da circulação extracorpórea (A2), quando comparadas às amostras da indução anestésica (A1), aqueles apresentam tendência de queda quando comparadas as amostras do segundo momento de reperfusão (SC2) (P<0,05). Conclusão: As dosagens de malondialdeído mostram que a revascularização miocárdica com circulação extracorpórea é acompanhada de aumento de radicais livres com tendência deste diminuir progressivamente após seu término. O pinçamento aórtico exacerba o estresse oxidativo, porém apresenta queda mais acentuada após a reperfusão quando comparadas ao do metabolismo sistêmico. O comportamento das espécies reativas ao ácido tiobarbitúrico indica que o estresse oxidativo é um componente patofisiológico inevitável.

Abstract Objective: The aim of this prospective study was to assess the dynamics of oxidative stress during coronary artery bypass surgery with cardiopulmonary bypass. Methods: Sixteen patients undergoing coronary artery bypass grafting were enrolled. Blood samples were collected from the systemic circulation during anesthesia induction (radial artery - A1), the systemic venous return (B1 and B2) four minutes after removal of the aortic cross-clamping, of the coronary sinus (CS1 and CS2) four minutes after removal of the aortic cross-clamping and the systemic circulation four minutes after completion of cardiopulmonary bypass (radial artery - A2). The marker of oxidative stress, malondialdehyde, was measured using spectrophotometry. Results: The mean values of malondialdehyde were (ng/dl): A1 (265.1), B1 (490.0), CS1 (527.0), B2 (599.6), CS2 (685.0) and A2 (527.2). Comparisons between A1/B1, A1/CS1, A1/B2, A1/CS2, A1/A2 were significant, with ascending values (P<0.05). Comparisons between the measurements of the coronary sinus and venous reservoir after the two moments of reperfusion (B1/B2 and CS1/CS2) were higher when CS2 (P<0.05). Despite higher values after the end of cardiopulmonary bypass (A2), when compared to samples of anesthesia (A1), those show a downward trend when compared to the samples of the second moment of reperfusion (CS2) (P<0.05). Conclusion: The measurement of malondialdehyde shows that coronary artery bypass grafting with cardiopulmonary bypass is accompanied by increase of free radicals and this trend gradually decreases after its completion. Aortic clamping exacerbates oxidative stress but has sharper decline after reperfusion when compared to systemic metabolism. The behavior of thiobarbituric acid species indicates that oxidative stress is an inevitable pathophysiological component.

OBJETIVO: O presente trabalho tem por objetivo demonstrar a experiência inicial e pioneira do implante da prótese Inovare pela via transfemoral ou ilíaca. MÉTODOS: Seis pacientes foram submetidos ao implante valvar aórtico transcateter. A via de acesso foi femoral ou ilíaca, por onde foi inserido o dispositivo de entrega, que consiste em um cateter balão de látex com a prótese "crimpada" sobre o mesmo. Com auxílio de introdutor femoral da marca Gore® DrySeal 24 Fr, posicionava-se uma guia extrarrígida com a ponta atraumática no ventrículo esquerdo, passando-se pelo anel valvar. Após valvuloplastia com cateter balão nos casos de estenose valvar nativa, implante da prótese foi realizado após hipotensão induzida por taquicardia controlada por marcapasso temporário. O posicionamento da valva foi orientado por ecocardiograma transesofágico (ETE) e radioscopia, objetivando posicionar um terço da extensão da prótese para dentro da cavidade ventricular esquerda. RESULTADOS: O implante valvar com sucesso foi possível nos 6 casos. Não houve necessidade de conversão para cirurgia convencional por impossibilidade de acesso ou migração da prótese. Não houve mortalidade intraoperatória ou hospitalar. Houve redução significativa do gradiente médio pré-operatório de 66,84115,46 mmHg para 19,74110,61 mmHg, no pós-operatório (P=0,002), significando redução de 70,46%. CONCLUSÃO: A prótese Inovare, implantada por via femoral ou ilíaca, foi factível do ponto de vista técnico, apresentando adequado desempenho hemodinâmico no seguimento pós-operatório e não apresentando mortalidade nesta pequena casuística.

OBJECTIVE: This paper demonstrates the initial and pioneering experience implant of the Inovare prosthesis implant through transfemoral or iliac artery route. METHODS: Six patients underwent transcatheter aortic valve implantation. The access was femoral or iliac through which the delivery device, a latex balloon catheter with the crimped prosthesis, was inserted. Through the femoral introducer 24 Fr Gore® DrySeal sheath, an extra stiff guide wire with non-traumatic tip was positioned in the left ventricle by passing through the valve ring. After balloon valvuloplasty, in cases of native valve stenosis, the prosthesis implantation was performed after hypotension induced by tachycardia and controlled by temporary pacemaker. The valve positioning was guided by TEE (transesophageal ecocardiography) and fluoroscopy, aiming to position a third of the length of the prosthesis into the left ventricle cavity. RESULTS: The successful valve implantation was possible in six cases. There was no need of conversion to open surgery due to inability to access or graft migration. There were no intraoperative or hospital deaths. We observed a significant reduction in the mean gradient of 66.84115.46 mmHg to 19.74110.61 mm Hg postoperatively (P=0.002), a reduction of 70.46%. CONCLUSION: Inovare prosthesis, implanted by femoral or iliac artery was feasible, and determined adequate hemodynamic performance in the postoperative follow-up, showing no mortality in this small series.

Paciente de 84 anos com dissecção de aorta torácica tipo A de Stanford comprometendo todo o arco aórtico e aorta descendente. Proposto e aceito o tratamento endovascular em função da gravidade do quadro clínico. Procedeu-se à dissecção das artérias femorais comum bilateralmente. A aortografia confirmou a exclusão da falsa luz e a patência dos óstios coronarianos.

Woman, 84 years-old, with Stanford type A thoracic aortic dissection committing aortic arch and descending aorta. Proposed and accepted endovascular treatment according to the severity of the clinical picture. Common femoral artery dissection bilaterally was done. Aortography confirmed the exclusion of the false lumen and patency of the coronary ostia.

FUNDAMENTOS: O tratamento cirúrgico da insuficiência cardíaca padrão-ouro é o transplante cardíaco, porém, em decorrência das dificuldades desse tratamento, outras propostas cirúrgicas têm sido relatadas, entre elas o implante de ressincronizador cardíaco. OBJETIVO: Analisar a função ventricular esquerda, por meio da ecocardiografia, de pacientes portadores de insuficiência cardíaca avançada com dissincronia interventricular submetidos a implante de ressincronizador cardíaco. MÉTODOS: Entre junho de 2006 a junho de 2012, foram avaliados 24 pacientes com idade média de 61,5 ± 11 anos, portadores de insuficiência cardíaca congestiva avançada em classe funcional III e IV (NYHA), dissincronia interventricular e tratamento medicamentoso otimizado. Esses pacientes foram submetidos ao implante de ressincronizador cardíaco e avaliados ecocardiograficamente no pós-operatório de seis meses. RESULTADOS: Houve melhora significativa dos parâmetros ecocardiográficos analisados. A média dos diâmetros diastólicos ventriculares esquerdos reduziu de 69,6 ± 9,8 mm para 66,8 ± 8,8 mm, diâmetros sistólicos de 58,6 ± 8,8 mm para 52,7 ± 8,8 mm e a fração de ejeção, média de 31 ± 8% para 40 ± 7% com nível de significância, respectivamente, de 0,019, 0,0004 e 0,0002, estatisticamente significativos com nível de significância de 0,05. CONCLUSÃO: Houve melhora da função ventricular esquerda analisada por meio da ecocardiografia, em seis meses, de pacientes portadores de insuficiência cardíaca avançada submetidos a implante de ressincronizador cardíaco.

BACKGROUND: The gold standard surgical treatment for heart failure is cardiac transplantation, however, due to difficulties of this treatment, other surgical proposals have been reported, including the implantation of cardiac resynchronizer. OBJECTIVE: To analyze the left ventricular function by echocardiography in patients with advanced heart failure with interventricular dyssynchrony undergone implantation of cardiac resynchronizer. METHODS: Between June 2006 and June 2012, 24 patients with average age of 61.5 ± 11 years were evaluated, carriers of advanced congestive heart failure functional class III and IV (NYHA), interventricular dyssynchrony and optimal drug therapy, and submitted implantation of cardiac resynchronizer and postoperative echocardiographically evaluated in six months. RESULTS: There was significant improvement of the analyzed echocardiography parameters. The average left ventricular diastolic diameter decreased from 69.6 ± 9.8 mm to 66.8 ± 8.8 mm, systolic diameters from 58.6 ± 8.8 mm to 52.7 ± 8.8 mm, and ejection fraction, average of 31 ± 8% to 40 ± 7% with level of significance, respectively, of 0.019, 0.0004 and 0.0002, statistically significant with a significance level of 0.05. CONCLUSION: There was a significant improvement of left ventricular function analyzed by echocardiography at six months, in patients with advanced heart failure undergone implantation of cardiac resynchronizer.

A dissecção de aorta tipo A apresenta grande mortalidade em sua fase aguda, com baixa sobrevida anual sem tratamento cirúrgico. Embora os casos crônicos sejam exceções, as complicações tardias existem e devem ser tratadas.

Aortic dissection type A has a great mortality in its acute phase with low annual survival without surgical treatment. Although the chronic cases are exceptions the late complications exist and should be treated.

Apresentamos o caso de paciente com disfunção de bioprótese valvar aórtica implantada há 11 anos, apresentando quadro de edema agudo pulmonar em decorrência de insuficiência valvar grave. Apresentava disfunção sistólica grave (FE< 30%) e comorbidades que elevavam seu risco operatório (STS score > 10). Realizou-se o implante de valva aórtica transcateter Inovare® - Braile Biomédica, por acesso transfemoral. O implante foi realizado com sucesso e o paciente apresentou boa evolução.

We present a patient with severe aortic valvular bioprosthesis dysfunction implanted for 11 years, presenting with acute pulmonary edema due to severe valvular insufficiency with severe systolic dysfunction (EF <30%) and comorbid conditions that amounted operative risk (STS score > 10). We carried out the transcatheter aortic valve implantation (Inovare® - Braile Biomedica), which was implemented successfully by transfemoral access and good patient outcomes.

INTRODUÇÃO: O ácido épsilon aminocapróico é um antifibrinolítico usado em cirurgia cardiovascular a fim de inibir a fibrinólise e reduzir o sangramento após circulação extracorpórea (CEC). OBJETIVO: Analisar a influência do uso do ácido aminocapróico no sangramento e na necessidade de hemotransfusão nas primeiras 24 horas em pós-operatório de cirurgias valvares mitrais. MÉTODOS: Estudo prospectivo, 42 pacientes, randomizados e divididos em dois grupos, de igual número: grupo I - controle e grupo II - ácido épsilon aminocapróico. No grupo II, foram infundidos 5 gramas de AEAC na indução anestésica, após heparinização plena, no perfusato da CEC, após reversão da heparina e uma hora após o final da cirurgia, totalizando 25 gramas. No grupo I, foi infundido apenas soro fisiológico nestes mesmos momentos. RESULTADOS: O grupo I apresentou volume de sangramento médio de 633,57 ± 305,7 ml e o grupo II média de 308,81 ± 210,1 ml, com diferença estatisticamente significativa (P=0,0003). O volume médio de hemotransfusão nos grupos I e II foi, respectivamente, de 942,86 ± 345,79 ml e de 214,29 ± 330,58 ml, havendo diferença significativa (P<0,0001). CONCLUSÃO: O ácido épsilon aminocapróico foi capaz de reduzir o volume de sangramento e a necessidade de hemoderivados no pós-operatório imediato de pacientes submetidos a cirurgias valvares mitrais.

INTRODUCTION: The epsilon aminocaproic acid is an antifibrinolytic used in cardiovascular surgery to inhibit the fibrinolysis and to reduce the bleeding after CBP. OBJECTIVE: To analyze the influence of the using of epsilon aminocaproic acid in the bleeding and in red-cell transfusion requirement in the first twenty-four hours postoperative of mitral valve surgery. METHODS: Prospective studying, forty-two patients, randomized and divided in two equal groups: group #1 control and group #2 - epsilon aminocaproic acid. In Group II were infused five grams of EACA in the induction of anesthesia, after full heparinization, CPB perfusate after reversal of heparin and one hour after the surgery, totaling 25 grams. In group I, saline solution was infused only in those moments. RESULTS: Group #1 showed average bleeding volume of 633.57 ± 305,7 ml, and Group #2, an average of 308.81 ± 210.1 ml, with significant statistic difference (P=0.0003). Average volume of red-cell transfusion requirement in Groups 1 and 2 was, respectively, 942.86 ± 345.79 ml and 214.29 ± 330.58 ml, with significant difference (P<0.0001). CONCLUSION: The epsilon aminocaproic acid was able to reduce the bleeding volume and the red-cell transfusion requirement in the immediate postoperative of patients submitted to mitral valve surgery.

OBJETIVO: Desenvolver um modelo experimental de miocardiopatia induzida por doxorrubicina em ratos. MÉTODOS: 16 ratos Wistar machos foram randomizados em 2 grupos: Grupo I (placebo) e Grupo II (doxorrubicina 5mg/kg). Após 6 meses, os animais foram submetidos a cardiotomia e seus corações foram pesados e submetidos a cortes transversais. Estes fragmentos foram estudados considerando seus diâmetros externos e internos e a espessura do ventrículo esquerdo. As peças histológicas foram analisadas quanto à presença de fibrose, vacuolização citoplasmática, necrose e variação do tamanho do núcleo. Os resultados foram submetidos a análise estatística pelo teste t de Student. RESULTADOS: Os corações dos animais do grupo II aumentaram 41% em relação ao peso; 33% no diâmetro interno e 14% no diâmetro externo; e 24% na espessura da parede do VE. Fibrose do tecido miocárdico foi observada em 75% dos animais do grupo II; todos os animais apresentaram vacuolização citoplasmática dos miócitos; Houve necrose miocárdica em 75% dos animais e 87% apresentaram variação no tamanho do núcleo. A presença de células polimorfonucleares também foi observada. CONCLUSÃO: A doxorrubicina foi efetiva na promoção de alterações macro e microscópicas no tecido cardíaco de ratos, possivelmente constituindo-se num modelo experimental para estudo da miocardiopatia.

PURPOSE: The development of an experimental model of myocardiopathy induced by Doxorubicin in rats. METHODS: 16 wistar male rats were randomized in two groups: Group I (placebo) and Group II (Doxorubicin - 5mg/kg). After six months, the animals were subjected to cardiotomy and their hearts were weighted and submitted to transversal cuts, from which fragments for a macro and micro study were obtained. These fragments were studied considering their external and internal diameters and the thickness of the left ventricle (LV). The histological pieces were analyzed for the presence of fibrosis, cytoplasmic vacuolization, necrosis and size of nucleus variation. Data obtained was submitted to statistical analysis with Student's t test. RESULTS: The hearts of the animals in Group II increased 41% in relation to their weight; 33% in the internal diameter and 14% in the external diameter of the LV cavity; and 24% in the thickness of the wall. Fibrosis of the myocardial tissue was observed in 75% of the animals of Group II; all the animals presented miocyte cytoplasmatic vacuolization; myocardial necrosis was present in 75% of the animals; and 87/% presented variation in the size of myocite nuclei. The presence of polymorphonuclear cells was also observed. CONCLUSION: Doxorubicin was effective in the promotion of macro and microscopic alterations in the cardiac tissue of rats, possibly constituting a model for the experimental study of myocardiopathy.

O tratamento da Persistência do Canal Arterial (PCA), em adultos, ainda é controverso. A utilização de próteses auto-expansíveis tem-se mostrado como uma alternativa eficaz ao tratamento cirúrgico. Apresentamos um caso de uma paciente de 45 anos submetida ao tratamento endovascular com o uso de stent auto-expansível.

The treatment for closure of persistent ductus arteriosus (PDA) in adults still controversial. The endovascular approach has been shown as an effective alternative to surgical treatment. We report a case of 45 years old pacient submitted to endovascular approach for PDA closure.

OBJETIVO: Avaliar os resultados do tratamento cirúrgico da fibrilação atrial por ablação da parede posterior atrial esquerda utilizando o eletrocautério, em cirurgia valvar mitral. MÉTODOS: De maio de 2004 a dezembro de 2006, 23 pacientes foram submetidos a correção cirúrgica de valvopatia mitral e ao tratamento da fibrilação atrial utilizando o eletrocautério convencional para a realização de linhas de ablação no endocárdio atrial esquerdo. A idade média dos pacientes era de 59 anos, sendo 60,8% do sexo feminino. A média do diâmetro atrial esquerdo era de 50,3 ± 5,09 mm e a fração de ejeção do ventrículo esquerdo de 53,6 ± 11,03%. RESULTADOS: O tempo médio de circulação extracorpórea foi de 52,5 ± 13,3 min; pinçamento aórtico, 35,6 ± 12,9 min; ablação do endocárdio atrial, 3,05 ± 0,16 min. Todos os pacientes estavam livres de fibrilação atrial após o procedimento; na alta hospitalar, 69,5%; no 6º mês, 91,3%; no 12º mês, 76,4%; no 18º e 24º meses, 68,4%. No 12º mês, a média do diâmetro atrial esquerdo era de 42,1 ± 3,5 mm; a fração de ejeção do ventrículo esquerdo era de 59,2 ± 3,48%; e a contração atrial esquerda estava presente em 68,8% de todos os pacientes. CONCLUSÃO: O tratamento cirúrgico da fibrilação atrial com eletrocautério, em cirurgia valvar mitral, foi capaz de determinar a reversão dessa arritmia a um número significativo de pacientes durante um seguimento clínico de curto e médio prazo, sem mortalidade e com baixa morbidade.

OBJECTIVE: To evaluate the results of the surgical treatment of atrial fibrillation for ablation of the posterior left atrial wall using electrocautery in mitral valve surgery. METHODS: From May 2004 to December 2006, 23 patients underwent surgical correction of mitral valve disease and treatment of atrial fibrillation using the conventional electrocautery for the accomplishment of lines of endocardial ablation in the left atrium. The mean age of the patients was 59 years, and 60.8% were female. The left atrium mean diameter was 50.3 ± 5.09 mm and the left ventricular ejection fraction was 53.6 ± 11.03%. RESULTS: The mean time of extracorporeal circulation was 52.5 ± 13.3 min; aortic clamping, 35.6 ± 12.9 min; atrial ablation, 3.05 ± 0.16 min. All the patients were free of atrial fibrillation after the procedure; on hospital discharge, 69.5%; at 6 months, 91.3%; at 12 months, 76.4%; at 18 months, and at 24 months, 68.4%. At 12 months, left atrium mean diameter was 42.1 ± 3.5 mm; left ventricular ejection fraction was 59.2 ± 3.48%; In addition, left atrial contraction was present in 68.8% of the patients. CONCLUSION: The surgical treatment of the atrial fibrillation with electrocautery in mitral valve surgery was capable to determine the reversion of this arrhythmia in a significant number of patients during short- and middle-term clinical follow-up without mortality and fewer complications.

OBJETIVO: Analisar os resultados em curto e médio prazo de pacientes portadores de insuficiência cardíaca e insuficiência mitral moderada/grave submetidos a substituição valvar mitral com a técnica da papilopexia cruzada e constrição anular. MÉTODOS: Treze pacientes em classe funcional III ou IV (NYHA), idade média de 54,1 anos, etiologia idiopática, foram submetidos a substituição valvar mitral com constrição do anel e papilopexia cruzada. Foram analisados os parâmetros ecocardiográficos, classe funcional e curva atuarial de sobrevivência. RESULTADOS: Não houve mortes no período trans e pósoperatório imediato. A média dos diâmetros diastólicos e sistólicos ventricular esquerdo reduziu de 71 ± 8,6 mm para 65,3 ± 8,6 mm (p=0,049) e de 59,1 ± 8,5 mm para 50,4 ± 11,1 mm (p=0,002), respectivamente. Os diâmetros atriais variaram de 49,4 ± 6,4 mm para 44 ± 5,9 mm (p=0,017); o percentual de encurtamento sistólico do ventrículo esquerdo foi de 17 ± 4% para 24 ± 8,3% (p=0,014), a fração de ejeção variou de 34 ± 9% para 45 ± 14% (p=0,008), todos estatisticamente significativos. Onze (84,6%) pacientes se encontram em classe funcional I e II. A sobrevivência foi de 100%, 82,6%, 71,6%, respectivamente para 1, 6 e 12 meses após a cirurgia de seguimento, mantendo-se 71,6% em período superior a 36 meses. CONCLUSÃO: Os resultados obtidos, em pacientes com insuficiência cardíaca e insuficiência mitral moderada/grave submetidos a substituição valvar mitral com papilopexia cruzada e constrição anular, apresentaram evidências de remodelamento cardíaco favorável e significativa recuperação funcional ventricular esquerda.

OBJECTIVE: To analyze the short-term and mid-term follow-ups of patients with heart failure and moderate to severe mitral valve insufficiency and who have undergone mitral valve replacement with crossed papillopexy and annular constriction. METHODS: Thirteen patients in NYHA functional class III or IV, with a mean age of 54.1 years and with idiopathic etiology, underwent mitral valve replacement with ring constriction and crossed papillopexy. Echocardiograph parameters, functional class and survival actuarial curve were analyzed. RESULTS: There were no deaths during surgery or in the postoperative period. The mean left ventricular diastolic diameter was reduced from 71 ± 8.6 mm to 65.3 ± 8.6 mm (p=0.049) and the mean left ventricular systolic diameter was reduced from .1 ± 8.5 mm to 50.4 ± 11.1 mm (p=0.002). The atrial diameters varied from 49.4 ± 6.4 mm to 44 ± 5.9 mm (p=0.017); the percentage of the left ventricular shortening was 17 ± 4 % to 24 ± 8.3% (p=0.014); the ejection fraction varied statistically and significantly from 34 ± 9% to 45 ± 14% (p=0.008). Eleven (84.6%) patients were in FC I and II. At 1, 6 and 12 months after follow-up surgery, the survival rate was 100%, 82.6%, 71.6%, respectively. This rate was maintained at 7.6% for more than 36 months. CONCLUSION: The results obtained from patients with heart failure and moderate to severe mitral valve insufficiency and who underwent mitral valve replacement with crossed papillopexy and annular constriction presented evidence of improved heart remodeling and significant improvement in left ventricular performance.
