The COVID-19 pandemic has challenged the entire world, and patients with diabetes mellitus (DM) have been particularly affected. We aimed to evaluate predictors of mortality during the first 30 days of hospitalization in critically ill patients with COVID-19 and comorbid DM. This prospective study included 110 critically ill patients admitted with COVID-19 infection. Thirty-two (29%) patients had a previous diagnosis of DM. Clinical variables, laboratory tests, and vascular biomarkers, such as VCAM-1, syndecan-1, ICAM-1, angiopoietin-1, and angiopoeitin-2, were evaluated after intensive care unit (ICU) admission. A comparison was made between patients with and without DM. No difference in mortality was observed between the groups (48.7 vs 46.9%, P=0.861). In the multivariate Cox regression analysis, VCAM-1 levels at ICU admission (HR: 1 [1-1.001], P<0.006) were associated with death in patients with DM. Among patients with DM, advanced age (HR 1.063 [1.031-1.096], P<0.001), increased Ang-2/Ang-1 ratio (HR: 4.515 [1.803-11.308] P=0.001), and need for dialysis (HR: 3.489 [1.409-8.642], P=0.007) were independent predictors of death. Higher levels of VCAM-1 in patients with DM was better at predicting death of patients with severe COVID-19 and comorbid DM, and their cut-off values were useful for stratifying patients with a worse prognosis. Vascular biomarkers VCAM-1 and Ang-2/Ang-1 ratio were predictors of death in patients with severe COVID-19 and comorbid DM and those without DM. Additionally, kidney injury was associated with an increased risk of death.

The reproductive cycle of Squamata reptiles is often associated with environmental conditions, such as rainfall. In this respect, seasonal variations may affect the morphology of the ovarian follicles, which are associated with vitellogenesis. The present study describes histological alterations in the ovarian cycle of two lizard species, Tropidurus hispidus (Spix, 1825) and Tropidurus semitaeniatus (Spix, 1825), which inhabit a caatinga region in the state of Rio Grande do Norte. Our goal was to identify morphological differences in the ovarian follicles at each phase of vitelloge nesis and to ascertain if they are associated with rainfall. Three follicular phases were identified in both species: pre-vitellogenesis, vitellogenesis and follicular atresia. An additional phase, the luteal, was found only in T. hispidus. During the development of these phases, vitellus was deposited inside the oocyte and there were identifiable alterations in the granulosa and thecal layers. Rainfall was found to influence the gonadal cycle.

A chimeric yellow fever (YF)-dengue serotype 2 (dengue 2) virus was constructed by replacing the premembrane and envelope genes of the YF 17D virus with those from dengue 2 virus strains of Southeast Asian genotype. The virus grew to high titers in Vero cells and, after passage 2, was used for immunogenicity and attenuation studies in rhesus monkeys. Subcutaneous immunization of naive rhesus monkeys with the 17D-D2 chimeric virus induced a neutralizing antibody response associated with the protection of 6 of 7 monkeys against viremia by wild-type dengue 2 virus. Neutralizing antibody titers to dengue 2 were significantly lower in YF-immune animals than in YF-naive monkeys and protection against challenge with wild-type dengue 2 virus was observed in only 2 of 11 YF-immune monkeys. An anamnestic response to dengue 2, indicated by a sharp increase of neutralizing antibody titers, was observed in the majority of the monkeys after challenge with wild-type virus. Virus attenuation was demonstrated using the standard monkey neurovirulence test. The 17D-D2 chimera caused significantly fewer histological lesions than the YF 17DD virus. The attenuated phenotype could also be inferred from the limited viremias compared to the YF 17DD vaccine. Overall, these results provide further support for the use of chimeric viruses for the development of a new live tetravalent dengue vaccine.

Measles virus is a highly contagious agent which causes a major health problem in developing countries. The viral genomic RNA is single-stranded, nonsegmented and of negative polarity. Many live attenuated vaccines for measles virus have been developed using either the prototype Edmonston strain or other locally isolated measles strains. Despite the diverse geographic origins of the vaccine viruses and the different attenuation methods used, there was remarkable sequence similarity of H, F and N genes among all vaccine strains. CAM-70 is a Japanese measles attenuated vaccine strain widely used in Brazilian children and produced by Bio-Manguinhos since 1982. Previous studies have characterized this vaccine biologically and genomically. Nevertheless, only the F, H and N genes have been sequenced. In the present study we have sequenced the remaining P, M and L genes (approximately 1.6, 1.4 and 6.5 kb, respectively) to complete the genomic characterization of CAM-70 and to assess the extent of genetic relationship between CAM-70 and other current vaccines. These genes were amplified using long-range or standard RT-PCR techniques, and the cDNA was cloned and automatically sequenced using the dideoxy chain-termination method. The sequence analysis comparing previously sequenced genotype A strains with the CAM-70 Bio-Manguinhos strain showed a low divergence among them. However, the CAM-70 strains (CAM-70 Bio-Manguinhos and a recently sequenced CAM-70 submaster seed strain) were assigned to a specific group by phylogenetic analysis using the neighbor-joining method. Information about our product at the genomic level is important for monitoring vaccination campaigns and for future studies of measles virus attenuation.

The aqueous fraction of the ethanolic extract (AFL) of Cissampelos sympodialis Eichl (Menispermaceae), popularly known as milona, has been shown to have both immunosuppressive and anti-inflammatory effects. In the present study we investigated the modulation of macrophage antimicrobicidal activity by in vitro treatment with the extract from C. sympodialis. Normal and thioglycolate-elicited mouse peritoneal macrophages were infected in vitro with the protozoan Trypanosoma cruzi DM28c clone. We observed that the AFL (used at doses ranging from 13 to 100 µg/ml) increased T. cruzi growth and induced a 75% reduction in nitric oxide production. This inhibition could be mediated by the stimulation of macrophage interleukin-10 (IL-10) secretion since the in vitro treatment with the AFL stimulated IL-10 production by T. cruzi-infected macrophages. These results suggest that the anti-inflammatory effect of the AFL from C. sympodialis could be, at least in part, mediated by the inhibition of macrophage functions and that the inhibition of macrophage microbicidal activity induced by the C. sympodialis extract may be mediated by the decrease in macrophage function mediated by interleukin-10 production.

OBJETIVO: verificar a presença de anticorpos contra o sarampo em crianças com infecção perinatal pelo HIV e devidamente imunizadas. MÉTODOS: estudo de coorte retrospectivo realizado em Belo Horizonte, entre 1995 e 1996. Foram incluídas 21 crianças com infecção perinatal pelo HIV e 29 crianças imunocompetentes não-infectadas. Informações acerca da vacina contra o sarampo foram obtidas do cartão de imunizações dos pacientes. A pesquisa de anticorpos contra o sarampo foi realizada pelos testes de neutralização por redução de placa e dosagem de IgM pela técnica de ELISA. Adotou-se nível de significância de 5% em todas as análises estatísticas realizadas. RESULTADOS: a mediana de idade dos pacientes infectados pelo HIV foi de 44,5 meses, e das crianças não-infectadas, de 62,0 meses (P=0,64).Os grupos receberam em média 2 doses da vacina contra o sarampo. Todos os pacientes soronegativos para o HIV apresentaram títulos de anticorpos contra o sarampo superiores a 50 mUI/ml, enquanto 57,1% das crianças infectadas apresentaram títulos acima deste valor (P=0,0001). O título geométrico médio de anticorpos neutralizantes foi significativamente menor no grupo de crianças com infecção pelo HIV (433,5 mUI/ml) do que no grupo de não-infectados(1668,1 mUI/ml), P=0,001. Todos os pacientes dos dois grupos foram negativos para a pesquisa de IgM contra o sarampo. CONCLUSÃO: as crianças infectadas pelo HIV apresentaram menor soroprevalência de anticorpos contra o sarampo após a imunização do que as crianças não-infectadas. Esses resultados alertam para o risco potencial de aquisição do vírus do sarampo, e apontam a necessidade de avaliar alternativas para a imunização das crianças infectadas pelo HIV, no sentido de maximizar a proteção contra o sarampo nesse grupo de pacientes.

OBJECTIVE: to determine the presence of antimeasles antibodies in children perinatally infected with HIV and properly immunized. METHODS: a retrospective cohort study conducted in Belo Horizonte by the Universidade Federal de Minas Gerais, between 1995 and 1996. Twenty one children perinatally infected with HIV and 29 immunocompetent noninfected children were included in the study. Information about measles vaccination was obtained from patients' immunization charts. The presence of neutralizing antibodies against the measles was determined by the plaque reduction neutralization test and IgM was measured by ELISA. The level of significance was set at 5% in all the performed statistical analyses. RESULTS: median age was 44.5 months for HIV-infected patients and 62.0 months for noninfected children (P=0.64). Both groups received on average two doses of antimeasles vaccine. All HIV-seronegative patients presented antimeasles antibody titers greater than 50 mIU/ml, whereas 57.1% of infected children presented titers above this value (P=0.0001). The geometric mean titer of neutralizing antibodies was significantly lower in the group of HIV-infected children (433.5 mIU/ml) than in noninfected children (1,668.1 mIU/ml), P=0.001. All patients in both groups were negative for antimeasles IgM. CONCLUSION: in the present study, HIV-infected children showed a lower seroprevalence of antimeasles antibody after immunization than noninfected children. These results emphasize the risk of acquisition of measles virus and the need to evaluate alternatives to the vaccination of HIV-infected children in an attempt to maximize the protection against the measles in this group of patients.

The yellow fever (YF) virus is the prototype flavivirus. The use of molecular techniques has unraveled the basic mechanisms of viral genome structure and expression. Recent trends in flavivirus research include the use of infectious clone technology with which it is possible to recover virus from cloned cDNA. Using this technique, mutations can be introduced at any point of the viral genome and their resulting effect on virus phenotype can be assessed. This approach has opened new possibilities to study several biological viral features with special emphasis on the issue of virulence/attenuation of the YF virus. The feasibility of using YF virus 17D vaccine strain, for which infectious cDNA is available, as a vector for the expression of heterologous antigens is reviewed
