Elevated serum IL-6 levels predict treatment interruption in patients with moderate to severe psoriasis: a 6-year real-world cohort study

Abstract Background: Real-world, primary data on the treatment of psoriasis are scarce, especially concerning the role of soluble biomarkers as outcome predictors. Objective: The authors evaluated the utility of Th1/Th17 serum cytokines along with clinical characteristics as predictors of drug survival in the treatment of psoriasis. Methods: The authors consecutively included participants with moderate to severe psoriasis who were followed up for 6 years. Baseline interferon-α, tumor necrosis factor-α, and inter-leukin (IL)-2, IL-4, IL-6, IL-10, and IL-17A were measured using a cytometric bead array; clinical data were assessed. The authors calculated hazard ratios (HRs) for drug survival using a Cox proportional hazards model. Results: The authors included 262 patients, most of whom used systemic immunosuppressants or biologics. In the multivariate model, poor quality of life measured by the Dermatology Life Quality Index (HR = 1.04; 95% CI 1.01–1.07; p = 0.012) and elevated baseline IL-6 (HR = 1.99; 95% CI 1.29–3.08; p = 0.002) were associated with treatment interruption. Study limitations: The main limitation of any cohort study is the presence of confounders that could not be detected in clinical evaluation. Conclusions: Poor quality of life and elevated baseline serum IL-6 level predicted treatment interruption in patients with moderate to severe psoriasis. Although IL-6 is not the most important mediator of the inflammatory pathway in the skin environment, it is an interesting biomarker candidate for predicting psoriasis treatment response.

Resumen Este estudio tuvo como objetivo evaluar el efecto de la inoculación con L. buchneri liofilizado y/o activado sobre las poblaciones de bacterias de ácido láctico en ensilajes de maíz. Los tratamientos experimentales consistieron en ensilaje de maíz sin aditivo y ensilaje con los inoculantes L. buchneri 1 × 10 5 ufc/g, aplicados según las recomendaciones del fabricante (1 g/tonelada de forraje) en las formas de inoculante liofilizado e inoculante preactivado. Se identificaron aislados purificados de ensilajes de maíz con y sin inocu lación, y el 93% de los aislados correspondieron a bacterias de ácido láctico de la especie L. plantarum. Entre los aislados no se detectaron bacterias de la especie L. buchneri. La aplicación de L. buchneri liofilizado o activado en la masa ensilada promueve un aumento considerable de la población de bacterias de ácido lácticas en los ensilajes de maíz durante todo el período de fermentación.
Abstract This study aimed to evaluate the effect of inoculation with lyophilised and/or activated Lactobacillus buchneri on lactic acid bacteria populations in corn silage. Experimental treatments consisted of corn silage without additives or silage with the inoculants of L. buchneri (1 x 10 5 cfu/g) applied according to the manufacturer’s recommendations (1 g/tonne fodder) in the forms of the lyophilised inoculant and pre-activated inoculant. Purified isolates from corn silage with and without the inoculant were identified, and 93% of the isolates corresponded to the lactic acid bacteria of the species Lactobacillus plantarum. Among the isolates, no bacteria of the species L. buchneri were detected. The application of lyophilised or activated L. buchneri improved the microbiological profile and reduced ethanol production in corn silage, even without being identified among the isolates captured 70 days after ensilage.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Abstract The skin, by reflecting internal processes, externalizes what happens inside the body in many diseases. Thus, the skin, as an organ, extrapolates its functions of protection, barrier and signals the existence of systemic diseases, expanding the importance of the dermatologist beyond the skin surface. Thus, the dermatologist investigates diagnostic hypotheses for conditions related to all systems and refers patients to the appropriate specialty. Combined with examination by a trained eye, the skin, due to its easy access, is still the ideal place for performing biopsies, which often clarify the diagnosis. This manuscript is the second part of the article on cutaneous manifestations of systemic diseases. In the first part, the cutaneous manifestations of the main rheumatologic and granulomatous diseases were described, and vascular manifestations were also addressed. In the present article, it will be discussed how metabolic, cardiovascular, kidney, and gastrointestinal diseases can manifest themselves in the integumentary system. Malignant diseases and their cutaneous implications, will also be discussed. Pruritus and its clinical cutaneous correspondence will be discussed. Finally, an update on cutaneous signs of SARS-CoV2 coronavirus infection will be presented.

ABSTRACT The use of growth regulators as a technique that can reduce plant height and thus strengthen stalks, which may be an option to eliminate or reduce plant lodging, thereby avoiding crop loss. However, there is a lack of information on the subject. The purpose of this study was to evaluate the effect of the dose (0; 37.5; 75.0; 112.5 e 150.0 g ha-1 active ingredient) and the application times of trinexapac-ethyl (sixth, seventh and eighth leaves), in a randomized complete block, in a 5x3 factorial scheme, with four replicates. The field experiment was performed during the 2016/17 crop season, in the experimental farm located in Selvíria, Mato Grosso do Sul State, Brazil, at Universidade Estadual Paulista- UNESP, Ilha Solteira, São Paulo State, Brazil - Campus. It is not recommended to apply the trinexapac-ethyl at eighth leaf, especially in the doses of 75.0; 112.5 and 150.0 g ha-1, since they generated the lowest values for yield parameters, thus reducing productivity, and providing greater number of unfilled grains. Taking into account the reduction in plant height, the minimization of lodging and grain yield, trinexapac-ethyl should be applied to the seventh leaf of the BRS Esmeralda rice at the dose of 75 g ha-1.

ABSTRACT Purpose To evaluate and compare two types of different scaffolds in critical bone defects in rats. Methods Seventy male Wistar rats (280 ± 20 grams) divided into three groups: control group (CG), untreated animals; biomaterial group 1 (BG1), animals that received the scaffold implanted hydroxyapatite (HA)/poly(lactic-co-glycolic) acid (PLGA); and biomaterial group 2 (BG2), animals that received the scaffolds HA/PLGA/Bleed. The critical bone defect was induced in the medial region of the skull calotte with the aid of an 8-mm-diameter trephine drill. The biomaterial was implanted in the form of 1.5 mm thick scaffolds, and samples were collected after 15, 30 and 60 days. Non-parametric Mann-Whitney test was used, with the significance level of 5% (p ≤ 0.05). Results Histology revealed morphological and structural differences of the neoformed tissue between the experimental groups. Collagen-1 (Col-1) findings are consistent with the histological ones, in which BG2 presented the highest amount of fibers in its tissue matrix in all evaluated periods. In contrast, the results of receptor activator of nuclear factor kappa-Β ligand (Rank-L) immunoexpression were higher in BG2 in the periods of 30 and 60 days, indicating an increase of the degradation of the biomaterial and the remodeling activity of the bone. Conclusions The properties of the HA/PLGA/Bleed scaffold were superior when compared to the scaffold composed only by HA/PLGA.

Abstract Aim: Exercise elicits adaptations in several physiological systems, such as the gastrointestinal tract. We evaluated the effects of an acute strength exercise (acute-SE) on gastric satiety and its correlation with creatinine kinase (CK), lactate, and plasma cytokine levels in humans. Methods: Anthropometric parameters, body composition, muscular strength, and satiety (drink test protocol) at rest and exercise were assessed. Results: In the squat, bench press, and T-bar row exercises, we observed a significant decrease (p<0.05) in muscular strength in the second, third, and fourth sets compared with that in the first set. Compared with rest, we observed a significant increase (p<0.05) in CK and lactate levels after acute-SE. In the drink test, acute-SE significantly increased (p <0.05) the total intake, calories ingested, and a total time of ingestion. Concerning cytokines, there was a significant increase (p<0.05) after acute-SE of IL-1β and IL-6 levels at the beginning of the test and a decrease in IL-6, -10, -13, and TNF-α levels after acute-SE at the end of the test (p<0.05). There was a correlation between CK, lactate, and total intake after acute-SE (p<0.05) as well as between IL-6, 13, TNF-α, and volume ingested in the last score of the drink test after acute-SE (p<0.05). Conclusion: Acute-SE decreases satiety associated with changes in lactate, CK, and plasma cytokine levels in healthy humans.

Abstract Background: Primary Sjögren's syndrome (pSS) is a systemic immune-mediated disease whose main characteristic is exocrine gland inflammation and, subsequent reduction in tear and saliva production. A delayed diagnosis is common due to the nonspecific clinical manifestations of disease. The aim of the present study was to develop recommendations for the diagnosis of glandular manifestations of pSS based on evidence and expert opinion. Main body of the abstract: We conducted a systematic literature review to retrieve the best evidence available on the accuracy of diagnostic tests for pSS. We also held two in-person meetings with experts (rheumatologists, pathologists, ophthalmologists and dentists) to establish their level of agreement using the Delphi method. Ultimately, we generated 18 recommendations that aim to facilitate the diagnosis of the glandular manifestations of pSS. Conclusion: The diagnosis of glandular manifestations of pSS is complex and multidisciplinary. It requires specific knowledge in the field of ophthalmology, immunology, pathology and imaging, making it compulsory for the rheumatologist to work with professionals from these different areas in order to improve accuracy and early diagnosis. Glandular dysfunction tests, ANA, RF, Anti-Ro, protein electrophoresis, urinalysis, blood count, C-Reactive protein, complement, testing for syphilis and viruses (HCV, HIV) and SGUS should be investigated when dryness or systemic manifestation are present. Minor salivary gland biopsy is recommended for all anti-Ro negative or incomplete criteria cases.

Resumen Esta investigación se llevó a cabo para evaluar los efectos de los extractos de Rheedia longifolia (Clusiaceae), Garcinia xanthochymus (Clusiaceae), Plumbago scandens (Plumbaginaceae) Hovenia dulcis (Rhamnaceae), Malpighia glabra (Malpighiaceae), Euphorbia tirucalli (Euphorbiaceae) y Nerium adelfa (Apocynaceae) sobre la tasa incubación de los huevos de Dermacentor (Anocentor) nitens y Ripicephalus sanguineus (Acari: Ixodidae) en concentraciones de 1.000 ppm. Los extractos de R. longifolia, H. dulcis, M. glabra y E. tirucalli retrasaron significativamente la incubación de los huevos de R. sanguineus en tres días, pero los grupos tratados con los extractos de G. xanthochymus y P. scandens eclosionaron después de 14 días. Para las garrapatas D. (A.) nitens, los extractos de H. dulcis y N. adelfa retrasaron la eclosión de los huevos durante tres días, sin embargo, la acción de los extractos de R. longifolia, G. xanthochymus, P. scandens y E. tirucalli causó un retraso mayor de siete días. Para los dos tipos de garrapatas, los huevos del grupo control tardaron un día para eclosionar. Los extractos H. dulcis y M. glabra fueron eficaces in vitro en huevos de R. sanguineus (89 %) y D. (A.) nitens (100 %) y se pueden considerar como candidatos potenciales para el control biológico de estas garrapatas.
Abstract This investigation was carried out to evaluate the effects of the plant extracts of Rheedia longifolia (Clusiaceae), Garcinia xanthochymus (Clusiaceae), Plumbago scandens (Plumbaginaceae), Hovenia dulcis (Rhamnaceae), Malpighia glabra (Malpighiaceae), Euphorbia tirucalli (Euphorbiaceae) and Nerium adelfa (Apocynaceae) on eggs of Dermacentor (Anocentor) nitens and Ripicephalus sanguineus (Acari: Ixodidae) at concentrations of 1,000 ppm. The extracts had a significant effect on the hatching rate of eggs. The extracts of R. longifolia, H. dulcis, M. glabra and E. tirucalli delayed the hatching of R. sanguineus eggs by three days; however, the group treated with the extracts of G. xanthochymus and P. scandens did not hatch until day fourteen. For the tick D. (A.) nitens, extracts of H. dulcis and N. oleander delayed egg hatching by three days; however, the action of extracts from R. longifolia, G. xanthochymus, P. scandens and E. tirucalli caused a delay of seven days. For the control groups of both species, eggs hatched in one day. H. dulcis and M. glabra were effective in vitro on eggs of R. sanguineus and D. (A.) nitens (89 and 100 %, respectively), and can be considered to be potential candidates for the biocontrol of those ticks.

Abstract Steatocystoma multiplex is a rare genetic disorder characterized by the presence of hamartomatous malformations at the junction of the pilosebaceous duct. It consists of encapsulated cystic lesions in the dermis, with adjacent sebaceous gland. When associated with inflammation, resembling hidradenitis, it is called steatocystoma multiplex suppurativa, a condition rarely reported. This is the first case of steatocystoma multiplex suppurativa reported in the Brazilian literature. Female patient, 23 years old, with papular and nodular cystic lesions that started in the armpits and groin, later spreading to the trunk, lower limbs, anticubital fossa, face and scalp. The presence of papular-nodular lesions associated with disseminated hidradenitis-like lesions in flexural areas and the histopathological diagnosis of steatocystoma defined the diagnosis of steatocystoma multiplex suppurativa.

Abstract Gianotti-Crosti syndrome is a rare disease characterized by acral papular eruption with symmetrical distribution. It is a benign and self-limited disease; the symptoms disappear after two to eight weeks, without recurrences or scars. Skin lesions are usually asymptomatic. Prodrome might occur, suggesting upper respiratory infection, or constitutional symptoms. Diagnosis is eminently clinical, and this disease is associated with viral infections. Due to its rarity and low occurrence in adolescents and adults, we report a case of Gianotti-Crosti syndrome of a teenager.

Introducción: Cuando un hermano asesina a otro, hablamos de fratricidio, o sea, una forma de homicidio raramente vista en la práctica psiquiátrica. Dado que aún es un tema poco estudiado, la literatura científica no lo ha comprendido bien. Objetivo: Relatar el caso de un individuo con esquizofrenia paranoide que, habiendo asesinado a su hermano, fue evaluado en peritaje criminalístico para determinar su responsabilidad penal. Métodos: Se realizó una entrevista psiquiátrica, y el diagnóstico psiquiátrico se estableció con base en la entrevista y la observación de los registros periciales y hospitalarios, con los criterios diagnósticos del DSM-IV-TR. También se realizó una revisión bibliográfica sobre el tema. Resultados: Se consideró inimputable al paciente examinado en virtud de que padecía una enfermedad mental que afectaba por entero a su capacidad de comprensión y determinación con respecto al delito cometido. Conclusiones: El estudio de casos como este puede ilustrar e identificar factores motivadores relacionados con el comportamiento homicida de sujetos con trastornos mentales graves.
Abstract Background: Fratricide is the killing of one's own bother. It's a type of homicide rarely seen on psychiatric practice. This is still a theme which is poorly studied, and not well understood by the scientific literature. Objective: To report a case of a men, with paranoid schizophrenia that murdered his own bother and had a psychiatric forensic evaluation to establish his penal responsibility. Methods: A psychiatric interview was carried out and the psychiatric diagnosis was established based on the interview and analysis of forensic and medical records, using the DSM-IV-TR criteria. Literature review was held about the theme. Results: The examinee was considered not guilty by reason of insanity, due to the presence of a mental disorder that affected her entire understanding and volition of the practiced act. Conclusions: The study of such cases may illustrate and identify motivating factors related to homicidal behavior in individuals with severe mental disorders.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Resfriamento rápido de pimentão amarelo com ar forçado

No presente trabalho, determinou-se o tempo de resfriamento de pimentão amarelo 'Zarco HS' submetido ao resfriamento rápido com ar forçado, usando diferentes tipos de embalagens, bem como sua conservação pós-colheita por um período de 30 dias sob refrigeração a 5,1±0,2°C e UR de 98,6±0,3%. A temperatura foi medida através de termopares na câmara, no evaporador, nos frutos e nas embalagens, num total de 48 termopares. Foram realizadas as análises: perda de massa, teor de umidade e índice de firmeza. O trabalho permitiu concluir que o resfriamento rápido com ar forçado mostrou-se adequado quando aplicado como uma tecnologia de conservação pós-colheita para pimentão amarelo 'Zarco HS', com uma redução da temperatura de 7/8 do tempo de resfriamento em no máximo 56 minutos.
In the present research it was determined the cooling time of yellow Bell pepper 'Zarco HS' submitted to forced air cooling, using different types of packaging, as well its postharvest conservation under refrigeration at 5,1±0,2°C and 98,6±0,3% RH during a period of 30 days under. The temperature was measured by thermocouples in the chamber, the evaporator, fruit and packs for a total of 48 thermocouples. The analysis of mass loss, humidity content, and firmness index were realized. It was possible to conclude that the forced air cooling process it's a feasible technology for postharvest conservation of yellow bell pepper 'Zarco HS' reducing the temperature of 7/8 cooling time to the maximum of 56 minutes.
