Abstract Background: Patients using immunosuppressive drugs may have unfavorable results after infections. However, there is a lack of information regarding COVID 19 in these patients, especially in patients with rheumatoid arthritis (RA). Therefore, the aim of this study was to evaluate the risk factors associated with COVID 19 hospitalizations in patients with RA. Methods: This multicenter, prospective cohort study is within the ReumaCoV Brazil registry and included 489 patients with RA. In this context, 269 patients who tested positive for COVID 19 were compared to 220 patients who tested negative for COVID 19 (control group). All patient data were collected from the Research Electronic Data Capture database. Results: The participants were predominantly female (90.6%) with a mean age of 53 ±12 years. Of the patients with COVID 19, 54 (20.1%) required hospitalization. After multiple adjustments, the final regression model showed that heart disease (OR =4.61, 95% CI 1.06–20.02. P < 0.001) and current use of glucocorticoids (OR =20.66, 95% CI 3.09–138. P < 0.002) were the risk factors associated with hospitalization. In addition, anosmia was associated with a lower chance of hospitalization (OR =0.26; 95% CI 0.10–0.67, P < 0.005). Conclusion: Our results demonstrated that heart disease and the use of glucocorticoids were associated with a higher number of hospital admissions for COVID 19 in patients with RA. ABSTRACT This study was conducted to characterize the transmission cycle of the tegumentary leishmaniasis (TL) in an old colonization area at Pernambuco State, Brazil. The aims were to identify autochthonous cases, sandflies fauna, domestic animals as possible reservoir hosts and the Leishmania species involved in this endemic area. A total of 168 suspected human cases of TL and 272 domestic animals (canine, feline, equine, goat, and sheep) were included. The sandflies were captured and identified by species. Patients were predominantly male and the average age was 37+18.1 years old. Of 85 patients who had skin lesions, 25.6% of them had direct positive smears for TL and 34 isolates were identified as Leishmania (Viannia) braziliensis. The confirmation for TL diagnosed by molecular detection (PCR) was almost three times more sensitive than the direct test [p < 0.001; PR = 2.72] associated with clinical examination. The Kappa test on PCR between two different specimens, biopsy, and skin lesion swab was 60.8% (p < 0.001). More than 200 specimens of sandflies (80 males and 159 females) were captured and identified as Lutzomyia whitmani (99.6%) and Lu. evandroi (0.4%). The detection of L. (V.) braziliensis by Real-Time PCR in the blood of a captured fed female was positive in 59.3% of Lu. whitmani. Of the 272 domestic animals included, 61.76% were male (n = 168). Thirty-six animals (13.2%) had lesions compatible with TL (34 dogs, 1 cat and 1 sheep) and 3 of them, all dogs, had lesions on the snout, showing destruction of cartilage and mucosa. The study suggests the participation of domestic animals as possible reservoirs. However, further studies are necessary to better understand the transmission cycle and take recommended measures in order to control the disease.

BACKGROUND Visceral Leishmaniasis (VL) is an infectious disease that is a significant cause of death among infants aged under 1 year and the elderly in Brazil. Serodiagnosis is a mainstay of VL elimination programs; however, it has significant limitations due to low accuracy. OBJECTIVE This study aimed to evaluate three recombinant Leishmania infantum proteins (rFc, rC9, and rA2) selected from previous proteomics and genomics analyses to develop enzyme-linked immunosorbent assay (ELISA) and immunochromatographic tests (ICT) for the serodiagnosis of human VL (HVL) and canine VL (CVL). METHODS A total of 186 human (70 L. infantum-infected symptomatic, 20 other disease-infected, and 96 healthy) and 185 canine (82 L. infantum-infected symptomatic, 27 L. infantum-infected asymptomatic, and 76 healthy) sera samples were used for antibody detection. FINDINGS Of the three proteins, rA2 (91.5% sensitivity and 87% specificity) and rC9 (95.7% sensitivity and 87.5% specificity) displayed the best performance in ELISA-HVL and ELISA-CVL, respectively. ICT-rA2 also displayed the best performance for HVL diagnosis (92.3% sensitivity and 88.0% specificity) and had high concordance with immunofluorescence antibody tests (IFAT), ELISA-rK39, IT-LEISH®, and ELISAEXT. ICT-rFc, ICT-rC9, and ICT-rA2 had sensitivities of 88.6%, 86.5%, and 87.0%, respectively, with specificity values of 84.0%, 92.0%, and 100%, respectively for CVL diagnosis. MAIN CONCLUSIONS The three antigens selected by us are promising candidates for VL diagnosis regardless of the test format, although the antigen combinations and test parameters may warrant further optimisation.

RESUMO Objetivo: Este estudo teve como objetivo investigar os sentidos do trabalho dos artistas circenses, nas três dimensões: individual, organizacional e social, seguindo a adaptação do modelo de Morin (2001), propostas por Oliveira et al. (2004) e Morin, Tonelli e Pliopas (2007). Originalidade/valor: O artigo se destaca por tratar de uma categoria de artistas que está perdendo espaço dentro da sociedade, sendo cada vez mais marginalizada. Diante disso, torna-se relevante entender qual o sentido de uma atividade que está em declínio. Design/metodologia/abordagem: Trata-se de um estudo descritivo e exploratório, de natureza qualitativa, em que, por meio de entrevistas semiestruturadas, foram coletados dados primários com dez artistas circenses. Para coleta de dados, utilizou-se a técnica da análise de conteúdo com o uso do software Atlas.ti. Resultados: Os resultados apontam que há sentido no trabalho para os artistas circenses em todas as dimensões: 1. Na dimensão individual, o fator que predominou foi o prazer, pois em mais de um momento todos os entrevistados enalteceram sua satisfação em pertencer ao circo; 2. Na dimensão organizacional, a utilidade predominou, já que todos consideram contribuir e atender às necessidades do circo; e 3. Na dimensão social, as relações interpessoais demonstraram que, mesmo com o preconceito em evidência, é possível relacionamentos externos ao circo. Os resultados contribuem para a literatura que envolve pesquisas com artistas circenses, levando-se em conta os seus sentidos do trabalho.
ABSTRACT Purpose: This study aimed to investigate the meanings of the work of circus artists in three dimensions, individual, organizational, and social, following the adaptation to the model of Morin (2001) proposed by Oliveira, Piccinini, Fontoura, and Schweig (2004) and Morin, Tonelli, and Pliopas (2007). Originality/value: The article stands out because it deals with a category of artists that is losing space within society, being increasingly marginalized. Given this, it becomes relevant to understand the meaning of an activity that is in decline. Design/methodology/approach: This is a descriptive and exploratory study of a qualitative nature, in which primary data were collected through semi-structured interviews with ten circus artists. For data collection, the content analysis technique was used in Atlas.ti software. Findings: The results point out that there is meaning in work for the circus artists across all dimensions: 1. In the individual dimension, the pleasure was the predominant factor, since in more than one moment, all the interviewees expressed their satisfaction in belonging to the circus; 2. In the organizational dimension, utility prevailed, since everyone considered contributing and meeting the needs of the circus; 3. In the social dimension, interpersonal relations, demonstrating that even with evidence of prejudice, external relationships can be formed in the circus. The results contribute to the literature that involves studies with circus artists, considering their meanings of work.

Resumo A febre chikungunya tem se tornado um importante problema de saúde pública nos países onde ocorrem as epidemias, visto que metade dos casos evolui com artrite crônica, persistente e incapacitante. Os dados na literatura sobre terapêuticas específicas nas diversas fases da artropatia ocasionada pela infecção pelo vírus chikungunya (CHIKV) são limitados, não existem estudos randomizados de qualidade que avaliem a eficácia das diferentes terapias. Há algumas poucas publicações sobre o tratamento das manifestações musculoesqueléticas da febre chikungunya, porém com importantes limitações metodológicas. Os dados atualmente disponíveis não permitem conclusões favoráveis ou contrárias a terapêuticas específicas, bem como uma adequada avaliação quanto à superioridade entre as diferentes medicações empregadas. O objetivo deste trabalho foi elaborar recomendações para o tratamento da febre chikungunya no Brasil. Foi feita uma revisão da literatura com seleção de artigos baseados em evidência, nas bases de dados Medline, SciELO, PubMed e Embase e de resumos de anais de congressos, além da opinião dos especialistas para dar apoio às decisões tomadas para definir as recomendações. Para a definição do grau de concordância foi feita uma metodologia Delphi, em duas reuniões presenciais e várias rodadas de votação on line. Este artigo refere-se à parte 2 das Recomendações da Sociedade Brasileira de Reumatologia para Diagnóstico e Tratamento da Febre Chikungunya, que trata especificamente do tratamento.
Abstract Chikungunya fever has become an important public health problem in countries where epidemics occur because half of the cases progress to chronic, persistent and debilitating arthritis. Literature data on specific therapies at the various phases of arthropathy caused by chikungunya virus (CHIKV) infection are limited, lacking quality randomized trials assessing the efficacies of different therapies. There are a few studies on the treatment of musculoskeletal manifestations of chikungunya fever, but these studies have important methodological limitations. The data currently available preclude conclusions favorable or contrary to specific therapies, or an adequate comparison between the different drugs used. The objective of this study was to develop recommendations for the treatment of chikungunya fever in Brazil. A literature review was performed via evidence-based selection of articles in the databases Medline, SciELO, PubMed and Embase and conference proceedings abstracts, in addition to expert opinions to support decision-making in defining recommendations. The Delphi method was used to define the degrees of agreement in 2 face-to-face meetings and several online voting rounds. This study is part 2 of the Recommendations of the Brazilian Society of Rheumatology (Sociedade Brasileira de Reumatologia - SBR) for the Diagnosis and Treatment of chikungunya fever and specifically addresses treatment.

Resumo A febre chikungunya tem se tornado um importante problema de saúde pública nos países onde ocorrem as epidemias. Até 2013, as Américas haviam registrado apenas casos importados quando, em outubro desse mesmo ano, foram notificados os primeiros casos na Ilha de Saint Martin, no Caribe. No Brasil, os primeiros relatos autóctones foram confirmados em setembro de 2014 e até a semana epidemiológica 37 de 2016 já haviam sido registrados 236.287 casos prováveis de infecção pelo chikungunya vírus (CHIKV), 116.523 confirmados sorologicamente. As mudanças ambientais causadas pelo homem, o crescimento urbano desordenado e o número cada vez maior de viagens internacionais têm sido apontados como os fatores responsáveis pela reemergência de epidemias em grande escala. Caracterizada clinicamente por febre e dor articular na fase aguda, em cerca de metade dos casos existe evolução para a fase crônica (além de três meses), com dor persistente e incapacitante. O objetivo deste trabalho foi elaborar recomendações para diagnóstico e tratamento da febre chikungunya no Brasil. Para isso, foi feita revisão da literatura nas bases de dados Medline, SciELO e PubMed, para dar apoio às decisões tomadas para definir as recomendações. Para a definição do grau de concordância foi feita uma metodologia Delphi, em duas reuniões presenciais e várias rodadas de votação on line. Foram geradas 25 recomendações, divididas em três grupos temáticos: (1) diagnóstico clínico, laboratorial e por imagem; (2) situações especiais e (3) tratamento. Na primeira parte estão os dois primeiros temas e o tratamento na segunda.
Abstract Chikungunya fever has become a relevant public health problem in countries where epidemics occur. Until 2013, only imported cases occurred in the Americas, but in October of that year, the first cases were reported in Saint Marin island in the Caribbean. The first autochthonous cases were confirmed in Brazil in September 2014; until epidemiological week 37 of 2016, 236,287 probable cases of infection with Chikungunya virus had been registered, 116,523 of which had serological confirmation. Environmental changes caused by humans, disorderly urban growth and an ever-increasing number of international travelers were described as the factors responsible for the emergence of large-scale epidemics. Clinically characterized by fever and joint pain in the acute stage, approximately half of patients progress to the chronic stage (beyond 3 months), which is accompanied by persistent and disabling pain. The aim of the present study was to formulate recommendations for the diagnosis and treatment of Chikungunya fever in Brazil. A literature review was performed in the MEDLINE, SciELO and PubMed databases to ground the decisions for recommendations. The degree of concordance among experts was established through the Delphi method, involving 2 in-person meetings and several online voting rounds. In total, 25 recommendations were formulated and divided into 3 thematic groups: (1) clinical, laboratory and imaging diagnosis; (2) special situations; and (3) treatment. The first 2 themes are presented in part 1, and treatment is presented in part 2.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Growth of cowpea plants inoculated with Rhizobium in a saline-sodic soil after application of gypsum

Com o objetivo de avaliar o crescimento do feijão-caupi, cultivado em solos salino-sódicos corrigidos com gesso, realizou-se dois experimentos: um em laboratório, para identificar o melhor nível de gesso para a correção de solos salino-sódico de Pernambuco. E outro em casa de vegetação, depois da correção dos solos, utilizando como planta teste o feijão-caupi, cultivar pele de moça, inoculada com Rizóbio, estirpe BR3267. Os experimentos foram dispostos no delineamento em blocos casualizados, com arranjo fatorial 2 x 5, dois solos e cinco níveis da necessidade de gesso (NG), equivalentes a 50; 100; 150; 200 e 250% da NG do solo, determinado pelo método Schoonover M-1, com cinco repetições. Foram avaliados: condutividade elétrica do extrato de saturação do solo (CE), sódio trocável e percentagem de sódio trocável (PST) do solo, número de nódulos (NN), massa seca dos nódulos (MSN), massa seca da parte aérea (MSPA), altura da parte aérea (APA) e teores de nitrogênio (N) na parte aérea. A aplicação do nível de 100% da NG, seguida de lâmina de lixiviação, foi eficaz para correção da sodicidade dos solos. A aplicação de níveis crescentes da NG do solo resultou em incremento no número de nódulos, massa seca dos nódulos e da parte aérea, altura e teor de N absorvido pelas plantas no solo S2. A utilização dos níveis de 200 e 250% da NG no solo S1 ocasionou decréscimo em todas variáveis estudadas.
Two experiments were carried out with the aim of evaluating the growth of cowpea cultivated in saline-sodic soils corrected with gypsum: one experiment in the laboratory, to identify the best level of gypsum for the correction of the saline-sodic soils of the state of Pernambuco, Brazil; and the other in a greenhouse, after correction of the soils. As the test plant, the cowpea cultivar pele de moça, inoculated with Rhizobium strain BR3267 was used. The experiments were arranged in a randomised block design in a 2 x 5 factorial arrangement, two soils and five levels of the gypsum requirement (GR), equivalent to 50, 100, 150, 200 and 250% of the GR of the soil, as determined by the Schoonover M-1 method, with five replications. The following were evaluated: electrical conductivity of the soil saturation extract (EC), soil exchangeable sodium and percentage of soil exchangeable sodium (ESP), number of nodules (NN), nodule dry weight (NDW), shoot dry weight (SDW), shoot height (PH) and nitrogen concentration (N) in the shoots. Application of 100% of the GR, followed by the enough water for leaching, was effective for the correction of soil sodicity. The application of increasing levels of soil GR resulted in an increase in the number of nodules, dry weight of the nodules and shoots, and the height and levels of N absorbed by the plants in soil S2. In soil S1, the use of levels of 200 and 250% of soil the GR caused a decrease in all the variables under study.

DNA sequencing confirms the involvement of Leishmania (L.) amazonensis in american tegumentary leishmaniasis in the state of São Paulo, Brazil

INTRODUCTION: American tegumentary leishmaniasis (ATL) represents one of the most important public health issues in the world. An increased number of autochthonous cases of ATL in the Northeastern region of São Paulo State has been documented in the last few years, leading to a desire to determine the Leishmania species implicated. METHODS: PCR followed by DNA sequencing was carried out to identify a 120bp fragment from the universal kDNA minicircle of the genus Leishmania in 61 skin or mucosal biopsies from patients with ATL. RESULTS: DNA sequencing permitted the identification of a particular 15bp fragment (5' …GTC TTT GGG GCA AGT... 3') in all samples. Analysis by the neighbor-joining method showed the occurrence of two distinct groups related to the genus Viannia (V) and Leishmania (L), each with two subgroups. Autochthonous cases with identity to a special Leishmania sequence not referenced in Genbank predominated in subgroup V.1, suggesting the possible existence of a subtype or mutation of Leishmania Viannia in this region. In the subgroup L.2, which showed identity with a known sequence of L. (L.) amazonensis, there was a balanced distribution of autochthonous and non-autochthonous cases, including the mucosal and mucocutaneus forms in four patients. The last observation may direct us to new concepts, since the mucosal compromising has commonly been attributed to L. (V.) braziliensis, even though L. (L.) amazonensis is more frequent in the Amazonian region. CONCLUSIONS: These results confirm the pattern of distribution and possible mutations of these species, as well as the change in the clinical form presentation of ATL in the São Paulo State.

FUNDAMENTOS: Métodos moleculares têm-se mostrados mais eficazes para o diagnóstico da LTA. OBJETIVOS: Comparar os resultados da intradermorreação de Montenegro (IRM), presença de leishmania em biópsia (Bc), reação de imunofluorescência indireta (Rifi), seqüenciamento de DNA e PCR-RFLP (-restriction fragment lenght polymorphism) para o diagnóstico da LTA. MÉTODOS: Foram estudados 152 pacientes com LTA. Para a PCR em Bc, utilizaram-se primers específicos para seqüência de 120bp do kDNA do minicírculo comum a todas as espécies de leishmanias. O produto da PCR, utilizado para seqüenciamento e para restrição enzimática com Hae III, foi comparado às culturas L. (L.) amazonensis e L. (V.) braziliensis. RESULTADOS: Houve predomínio do sexo masculino (75%), da cor branca (80%) e da profissão urbana (48%). A idade variou de três a 77 anos, com 56,5% entre 21 e 50 anos. 65,8% eram do Estado de São Paulo, prevalecendo a forma cutânea (79,6%). A IRM foi positiva em 73,4%, e a Rifi em 59,7%, enquanto a Bc evidenciou presença de leishmania em 30,6%. A PCR foi positiva em 81,6%, e a PCR-RFLP identificou L. (V.) braziliensis como espécie predominante (66%), o que também ocorreu com o seqüenciamento. Comparando PCR-RFLP e seqüenciamento, houve 61% de concordância entre os resultados, mostrando significância da PCR-RFLP para L. (V.) braziliensis. CONCLUSÕES: A IRM e a PCR foram estatisticamente equivalentes como métodos subsidiários para o diagnóstico da LTA, a PCR-RFLP e o seqüenciamento também o foram na identificação das espécies de leishmania, o primeiro apresentando menores custo e tempo de execução comparado ao seqüenciamento de DNA.
BACKGROUND: ATL is endemic in Brazil, and molecular methods have been shown more effective for its diagnosis. OBJECTIVES: Our purpose was to compare the results of Montenegro’s skin reaction (MR), presence of leishmania in skin biopsy (Bx), indirect immunofluorescence (IIF) for leishmania in sera samples, PCR, sequencing of DNA and PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) as subsidiary exams for ATL diagnose. METHODS: 152 patients were studied, with accomplishment of MR, Bx, IIF and PCR for leishmania in skin samples. For PCR, a specific pair of primers for a sequence of 120bp from kDNA minicircle, common to all leishmanias species, was used. The product of PCR was used for DNA sequencing and for RFLP with the enzyme Hae III. The analysis of the restriction pattern was compared to the cultures of L. (L.) amazonensis and L. (V.) braziliensis. RESULTS: The predominant sex was male (75%), the white color (80%) and urban professional occupation (48%). The age varied from 3 to 77 years, with prevalence from 21 to 50 years (56.5%). In relation to the origin, 65.8% was from the state of São Paulo, being the cutaneous form the more prevalent (79.6%). MR was positive in 73.4% and there was presence of leishmania in 30.6% of the samples, while IIF presented 59.7% of positivity. PCR was positive in 81.6% of skin samples, and PCR-RFLP identified L. (V.) braziliensis (66%) as predominant species, fact that also happened with DNA sequencing, with 64.4% of the positive results for L. (V.) braziliensis. Comparing PCR-RFLP and DNA sequencing there was 61% of agreement amongst the results, being significant in PCR-RFLP for L. (V.) braziliensis. CONCLUSION: MR and PCR were equivalent as subsidiary methods for the diagnosis of ATL, such as PCR-RFLP and DNA sequencing in the identification of the leishmania species. On the other hand, the method PCR-RFLP presented smaller cost and smaller time of execution compared to the sequencing of DNA.
