Introduction: SARS-CoV-2 began in 2020 and caused important changes in the number of transplants performed in hospitals and in the protocols for admitting candidates to perform the procedure. The Brazilian Association of Organ Transplantation (ABTO) recommends not performing transplants from donors with active COVID-19 infection, positive test results or with Severe Acute Respiratory Syndrome. The hepatic repercussions related to COVID-19 are presented in some reports in the medical literature. Liver changes resulting from other corona viruses such as SARS-CoV and MERS-CoV are well documented. The Hospital de Clínicas of the State University of Campinas is a tertiary center that performs solid organ transplants. Objectives: To carry out a perioperative, retrospective, descriptive analysis of liver transplants in the context of the SARS-CoV-2 pandemic carried out at the Hospital das Clínicas of the State University of Campinas from March 2020 to July 2021. Materials and Methods: Retrospective, descriptive, longitudinal cohort study based on the review of medical records of patients undergoing liver transplantation in the context of the SARS-CoV-2 pandemic from March 2020 to July 2021 at the clinical hospital of the State University of Campinas. Results: Retrospective analysis was performed on 57 patients in the period. Only 1 patient needed to be excluded because he was under 18 years old. Of the 56 patients, 52 underwent RT-PCR laboratory testing and chest tomography (CT). Of these 52 patients, only 2 tested positive, one pre-transplant (TX) and one post-operatively (post-op). Regarding chest CT scans, none of them showed typical changes for COVID pre-TX, in the post-op 4 patients presented typical chest CT scans. The average age was 55.86 years. The mortality rate was 38% and no deaths were attributed to COVID 19. The average MELD-Na scale was 20.94. Conclusion: The present study carried out at the Hospital de Clínicas da Unicamp analyzed the clinical, laboratory and radiological association to better elucidate the variables determined by COVID-19 in its diagnosis and in-hospital management. It is concluded that the SARS-CoV-2 pandemic had an impact on the routine of liver transplantation worldwide and on the service in which the study was carried out.

Introdução: A pandemia causada pelo SARS-CoV-2 teve início no ano de 2020 e ocasionou mudanças importantes no número de transplantes realizados nos hospitais e nos protocolos de admissão de candidatos para realização do procedimento. A Associação Brasileira de Transplante de Orgãos (ABTO) recomendava não realizar transplante de doadores com infecção COVID-19 ativa, teste positivo ou com Síndrome Respiratória Aguda Grave. As repercussões hepáticas relacionadas a COVID-19 são apresentadas em alguns relatos presentes na literatura médica. Estão bem documentadas as alterações hepáticas decorrentes de outros coronavírus tais como SARS-CoV e MERS-CoV. O Hospital de Clínicas da Universidade Estadual de Campinas é um centro terciário que realiza transplantes de órgãos sólidos. Objetivos: Realizar a análise retrospectiva descritiva perioperatória dos transplantes hepáticos no contexto da pandemia por SARS-CoV-2 realizados no hospital das clínicas da Universidade Estadual de Campinas no período de Março de 2020 a Julho de 2021. Materiais e Métodos: Estudo retrospectivo, descritivo de coorte longitudinal baseado na revisão dos prontuários dos pacientes submetidos ao transplante hepático no contexto da pandemia por SARS-CoV-2 no período de Março de 2020 a Julho de 2021 no hospital de clínicas da Universidade Estadual de Campinas. Resultados: A análise retrospectiva foi realizada em 57 pacientes no período. Apenas 1 paciente precisou ser excluído por ter menos de 18 anos. Dos 56 pacientes, 52 realizaram coleta do exame laboratorial RT-PCR e tomografia (TC) de tórax. Desses 52 pacientes apenas 2 positivaram o exame, um pré transplante (TX) e um no pós-operatório (pós-op). Em relação às TC de tórax nenhuma apresentava alterações típicas para COVID pré TX, no pós-op 4 pacientes apresentaram TC típicas. A média de idade foi de 55,86 anos. A taxa de mortalidade foi de 38% e nenhum óbito foi atribuído ao COVID 19. A escala de MELDNa média foi de 20,94. Conclusão: O presente estudo realizado no Hospital de Clínicas da Unicamp analisou a associação clínica, laboratorial e radiológica para melhor elucidar as variáveis determinadas pela COVID-19 no seu diagnóstico e manejo intra-hospitalar. Conclui-se que a pandemia por SARS-CoV-2 teve impacto na rotina de realização do transplante hepático mundialmente e no serviço no qual o estudo foi realizado.

ABSTRACT Patients undergoing hematopoietic stem cell transplantation (HSCT) might present acute and late toxicities and the oral tissues are frequently affected. With the survival increasing, patients show late and long-term morbidities, and there is an important association between the general and the oral health. The first and second parts of this Consensus have showed the importance of the adequacy of oral health in the pre-HSCT, and the main alterations and oral care during the period of admission for HSCT. This third part aims to review specific themes of post-HSCT dental care, such as graft-versus-host disease (GVHD) and the pediatric patient. It also aims to review pertinent subjects, both during the HSCT period and post-HSCT, concerning quality of life, pain, cost-effectiveness, and remote care. Based on this review, it is evident the importance of the work of the dental surgeon (DS) in the follow-up and treatment of the HSCT patient, always collaborating with the whole multidisciplinary team.

ABSTRACT During the state of immune vulnerability in hematopoietic stem cell transplantation (HSCT), the patient has an increased risk of developing a vast number of complications, including severe problems in the oral cavity. These situations require professional oral care to act in the diagnosis and treatment of these conditions, as well as to develop prevention protocols to minimize patient's complications. Oral mucositis, opportunistic infections, bleeding, specific microbiota, taste, and salivary alterations are complications that can occur during HSCT and interfere with various aspects, such as pain control, oral intake, nutrition, bacteremia and sepsis, days of hospitalization and morbidity. Several guidelines have been published to address the role of professional oral care during the HSCT, we describe a consensus regarding these recommendations.

ABSTRACT The oral involvement in the Hematopoietic Stem Cell Transplantation is well described in the literature. The goal of the dental treatment and management of the oral lesions related to the HSCT is to reduce the harm caused by preexisting oral infection or even the worsening of oral acute/chronic GVHD and late effects. The aim of this guideline was to discuss the dental management of patients subjected to HSCT, considering three phases of the HSCT: pre-HSCT, acute phase, and late phase. The literature published from 2010 to 2020 was reviewed in order to identify dental interventions in this patient population. The selected papers were divided into three groups: pre-HSCT, acute and late, and were reviewed by the SBTMO Dental Committee's members. When necessary, an expertise opinion was considered for better translating the guideline recommendations to our population dental characteristics. This manuscript focused on the pre-HSCT dental management. The objective of the pre-HSCT dental management is to identify possible dental situations that On behalf of the Dental Committee of the Brazilian Society of Gene Therapy and Bone Marrow Transplantation (SBTMO) can worsening during the acute phase after the HSCT. Each guideline recommendations were made considering the Dentistry Specialties. The clinical consensus on dental management prior to HSCT provides professional health caregivers with clinical setting-specific information to help with the management of dental problems in patients to be subjected to HSCT.

Abstract Objective To evaluate the impact of the race (Black versus non-Black) on maternal and perinatal outcomes of pregnant women with COVID-19 in Brazil. Methods This is a subanalysis of REBRACO, a Brazilian multicenter cohort study designed to evaluate the impact of COVID-19 on pregnant women. From February2020 until February 2021, 15 maternity hospitals in Brazil collected data on women with respiratory symptoms. We selected all women with a positive test for COVID-19; then, we divided them into two groups: Black and non-Black women. Finally, we compared, between groups, sociodemographic, maternal, and perinatal outcomes. We obtained the frequency of events in each group and compared them using X2 test; p-values < 0.05 were considered significant. We also estimated the odds ratio (OR) and confidence intervals (CI). Results 729 symptomatic women were included in the study; of those, 285 were positive for COVID-19, 120 (42.1%) were Black, and 165 (57.9%) were non-Black. Black women had worse education (p = 0.037). The timing of access to the health system was similar between both groups, with 26.3% being included with seven or more days of symptoms. Severe acute respiratory syndrome (OR 2.22 CI 1.17–4.21), intensive care unit admission (OR 2.00 CI 1.07–3.74), and desaturation at admission (OR 3.72 CI 1.41–9.84) were more likely to occur among Black women. Maternal death was higher among Black women (7.8% vs. 2.6%, p = 0.048). Perinatal outcomes were similar between both groups. Conclusion Brazilian Black women were more likely to die due to the consequences of COVID-19.

Resumo Objetivo Avaliar o impacto da raça (negra versus não negra) nos desfechos maternos e perinatais de gestantes com COVID-19 no Brasil. Métodos Esta é uma subanálise da REBRACO, um estudo de coorte multicêntrico brasileiro desenhado para avaliar o impacto da COVID-19 em mulheres grávidas. De fevereiro de 2020 a fevereiro de 2021, 15 maternidades do Brasil coletaram dados de mulheres com sintomas respiratórios. Selecionamos todas as mulheres com teste positivo para COVID-19; em seguida, as dividimos em dois grupos: mulheres negras e não negras. Finalmente, comparamos, entre os grupos, os resultados sociodemográficos, maternos e perinatais. Obtivemos a frequência dos eventos em cada grupo e comparamos usando o teste X2; Valores de p <0,05 foram considerados significativos. Também estimamos o odds ratio (OR) e os intervalos de confiança (IC). Resultados 729 mulheres sintomáticas foram incluídas no estudo; desses, 285 foram positivos para COVID-19, 120 (42,1%) eram negros e 165 (57,9%) não eram negros. As mulheres negras apresentaram pior escolaridade (p = 0,037). O tempo de acesso ao sistema de saúde foi semelhante entre os dois grupos, com 26,3% incluídos com sete ou mais dias de sintomas. Síndrome respiratória aguda grave (OR 2,22 CI 1,17–4,21), admissão em unidade de terapia intensiva (OR 2,00 CI 1,07–3,74) e dessaturação na admissão (OR 3,72 CI 1,41–9,84) foram mais prováveis de ocorrer entre mulheres negras. A mortalidade materna foi maior entre as negras (7,8% vs. 2,6%, p = 0,048). Os resultados perinatais foram semelhantes entre os dois grupos. Conclusão Mulheres negras brasileiras tiveram maior probabilidade de morrer devido às consequências da COVID-19.

Metonymy is a pervasive linguistic/cognitive mechanism in everyday language. Still, studies assessing its comprehension are rare in the literature. Our goal is to present the elaboration of a verbal metonymy comprehension task in Brazilian Portuguese as well as test it using pilot studies. The task was based on the cognitive linguistics framework and psychometric guidelines were considered. The construction of the task considered psycholinguistic variables and included a conventionality task and an expert analysis. Three studies (N = 290, ages ranging 2 to 54 years old) were conducted until we reached a final version. The study resulted in a task composed of one practice item and six test items. Each item includes a sentence-stimulus and two questions (one open-ended and one closed-ended). Together, the items make up a raw score that represents metonymy comprehension. Data obtained from the pilot studies show that metonymy comprehension improves gradually with age and that there is a significant difference between items in both types of questions. This is in consonance with the literature, and thus points towards the instrument’s construct validity. The study results in a task that presents enough sensitivity to gauge comprehension of this phenomenon at different ages.

Metonímia é um mecanismo linguístico/cognitivo pervasivo na linguagem cotidiana. Entretanto, estudos de avaliação desse fenômeno são escassos. Nosso objetivo é apresentar a elaboração de uma tarefa de compreensão de metonímia em português brasileiro e seus resultados piloto. Referenciais teóricos da linguística cognitiva e da psicometria foram considerados. A construção da tarefa considerou variáveis psicolinguísticas e incluiu uma tarefa de convencionalidade e análise de juízes. Três estudos (N = 290, entre 2 e 54 anos) foram conduzidos até a versão final da tarefa. O estudo resultou em uma tarefa com um item de treino e seis de teste. Os itens incluem uma frase de estímulo e duas perguntas (aberta e fechada). Consideramos que o escore dos itens representa a compreensão de metonímia. Os resultados dos estudos piloto indicam uma melhora gradual na compreensão de metonímia com a idade e uma diferença significativa entre os itens nas questões abertas e fechadas. Isso está em consonância com a literatura, sugerindo evidências de validade de construto ao instrumento. O estudo resulta em um instrumento que apresenta sensibilidade suficiente para aferir a compreensão do fenômeno em diferentes idades.

Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92–2.31)/1.60 (1.27–2.02) for SGA; 0.90 (0.55–1.47)/1.05 (0.55–1.99) for LGA; 1.65 (1.08–2.51)/1.58 (1.28–1.96) for stunting; and 1.48 (1.02–2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes.

Resumo Plinia cauliflora (Mart.) Kausel, popularmente conhecida como jabuticaba, é rica em polifenois. Os compostos fenólicos apresentam diversas propriedades biológicas, que refletem em biomarcadores, como os parâmetros bioquímicos. No presente estudo, avaliamos os níveis plasmáticos de glicose, colesterol total, HDL-colesterol, triglicerídeos e ácido úrico em hamsters chineses alimentados por 45 dias com dieta regular ou dieta enriquecida com colesterol suplementada com extrato líquido obtido de resíduos de frutos de P. cauliflora padronizado em ácido elágico e compostos fenólicos totais. Os resultados mostraram que o extrato concentrado obtido dos resíduos de jabuticaba aumentou a glicemia dos animais alimentados com dieta regular e reduziu os níveis plasmáticos de ácido úrico dos animais alimentados com dieta rica em colesterol. Uma vez que a hiperuricemia é considerada um fator de risco significativo de distúrbios metabólicos e a principal base patológica da gota, o extrato líquido dos resíduos de frutas de P. cauliflora seria um candidato promissor como um novo agente hipouricêmico para investigação posterior.

Abstract Plinia cauliflora (Mart.) Kausel, popularly known as jabuticaba, is rich in polyphenols. Phenolic compounds exhibit several biological properties, which reflect on biomarkers such as biochemical parameters. In the present study, we evaluated the plasmatic levels of glucose, total cholesterol, HDL-cholesterol, triglycerides, and uric acid of Chinese hamsters fed for 45 days with a regular diet or cholesterol-enriched diet supplemented with a liquid extract obtained from P. cauliflora fruits residues standardized in ellagic acid and total phenolic compounds. The results showed that the concentrated extract obtained from jabuticaba residues increased the glycemia of animals fed with a regular diet and reduced the plasmatic uric acid levels of animals fed with a cholesterol-enriched diet. Since hyperuricemia is considered to be a significant risk factor of metabolic disorders and the principal pathological basis of gout, the liquid extract from P. cauliflora fruits residues would be a promising candidate as a novel hypouricaemic agent for further investigation.

ABSTRACT Objective: Arthrodesis techniques such as anterior lumbar interbody fusion (ALIF) and lateral lumbar interbody fusion (LLIF) aim to reestablish physiological lordosis and minimize tissue damage to the paravertebral musculature. Supplementation with percutaneous pedicle screws is indicated in most cases, therefore, intraoperative changes in decubitus are necessary, generating costs and risks for the patient. This study aims to present concepts and results of a series of 100 cases of patients undergoing 360° fusion in lateral single position surgery (LSPS). Methods: retrospective analysis of databases collected between 2016 and 2021. Patients who underwent 360° fusion of the lumbar spine in single lateral decubitus to treat degenerative and infectious diseases were included. Cases with arthrodesis greater than 3 levels were excluded. Data collected include demographics, body mass index (BMI) and scores such as visual analog scale (VAS), EuroQOL 5D (EQ5D) and Oswestry disability index (ODI). Results: 100 patients were included in the study, submitted to LLIF and/or ALIF associated with percutaneous pedicle fixation. The lumbar VAS improved from 6.75 to 2.1 after 12 months, while the sciatica VAS started from 4.55 and reached 0.81 after one year. The EQ5D improved from 66.1 to 81.6 after the first year, while the ODI ranged from 28.54 to 14.18 in the same period. Conclusions: the clinical-functional results of the LSPS procedures are favorable and place the LSPS as an option to be studied, developed and practiced by spine surgery teams. Level of evidence: IV. Resumen: Objetivo: Las técnicas de artrodesis como la fusión intersomática lumbar anterior (ALIF) y la fusión intersomática lumbar lateral (LLIF) tienen como objetivo restablecer la lordosis fisiológica. La suplementación con tornillos pediculares percutáneos está indicada en la mayoría de los casos, por lo que son necesarios cambios en decúbito intraoperatorios, generando costos y riesgos para el paciente. Este estudio tiene como objetivo presentar conceptos y resultados de una serie de 100 casos de pacientes sometidos a fusión de 360° en cirugía de posición única lateral (LSPS). Métodos: análisis retrospectivo de bases de datos recolectadas entre 2016 y 2021. Se incluyeron pacientes que se sometieron a fusión de columna lumbar 360° en decúbito lateral y se excluyeron los casos con artrodesis mayores de 3 niveles. Los datos recopilados incluyen datos demográficos, índice de masa corporal (IMC) y puntajes como la escala analógica visual (VAS), EuroQOL 5D (EQ5D) y el índice de discapacidad de Oswestry (ODI). Resultados: 100 pacientes fueron incluidos en el estudio, sometidos a LLIF y/o ALIF asociados a fijación pedicular percutánea. La EVA lumbar mejoró de 6,75 a 2,1 a los 12 meses, mientras que la EVA de ciática partió de 4,55 y llegó a 0,81 al año. El EQ5D mejoró 66,1 a 81,6 después del primer año, mientras que el ODI varió 28,54 a 14,18 en el mismo período. Conclusiones: los resultados clínico-funcionales de los procedimientos de LSPS son favorables y a sitúan como una opción a ser estudiada, desarrollada y practicada por los equipos de cirugía de columna. Resumo: Objetivo: Técnicas de artrodese como anterior lumbar interbody fusion (ALIF) e lateral lumbar interbody fusion (LLIF) tem como objetivos o reestabelecimento da lordose fisiológica e a mínima lesão tecidual da musculatura paravertebral. A suplementação com parafusos pediculares por via percutânea é indicada na maioria dos casos, sendo, portanto, necessárias mudanças de decúbito intraoperatórias, gerando custos e riscos para o paciente. Este estudo tem como objetivo apresentar conceitos e resultados de uma série de 100 casos de pacientes submetidos a fusão 360° em lateral single position surgery (LSPS). Métodos: análise retrospectiva de banco de dados coletados entre 2016 e 2021. Foram incluídos pacientes submetidos a fusão 360° da coluna lombar em decúbito lateral único para tratamento doenças degenerativas e infecciosas. Foram excluídos casos com artrodeses maiores que 3 níveis. Os dados coletados incluem demografia, índice de massa corpórea (IMC) e scores como visual analog scale (VAS), EuroQOL 5D (EQ5D) e Oswestry disability index (ODI). Resultados: 100 pacientes foram incluídos no estudo, submetidos a LLIF e/ ou ALIF associados a fixação pedicular percutânea. O VAS lombar apresentou melhora de 6,75 para 2,1 após 12 meses, já o VAS ciatalgia partiu de 4,55 e atingiu 0,81 após um ano. O EQ5D apresentou melhora de 66,1 para 81,6 após o primeiro ano, enquanto o ODI variou de 28,54 para 14,18 no mesmo período. Conclusões: os resultados clínico-funcionais dos procedimentos em LSPS se mostram favoráveis e a colocam o LSPS como uma opção a ser estudada, desenvolvida e praticada pelas equipes de cirurgia de coluna. Nível de evidência: IV. Série de casos.

Abstract Aimed to map the instruments used in evaluating Burnout syndrome in public security professionals, indicating the psychometric aspects, samples, and associated variables. Nineteen studies were selected from the searched databases (PsycINFO, PubMED and Google Scholar). Results indicated that the instruments MBI-GS, MBI-HSS, BM-S, CBI and CESQT, but only one of the instruments showed evidence of validity in this context. Burnout was associated with personal, organizational, and specific variables. Comparison of studies and Burnout indices was limited due to the lack of standardization. The absence of evidence of validity and standardization of results leads to problems such as the lack of empirical support for the scores and difficulty in comparing studies. The data reveal limitations in the assessment of burnout levels used in the context of public safety, as the measures do not present validity evidence for this context, pointing to the need for research in the area.

Resumen El objetivo fue mapear los instrumentos utilizados en la evaluación del síndrome de Burnout en profesionales de la seguridad pública, indicando los aspectos psicométricos, muestras y variables asociadas. Se seleccionaron diecinueve estudios de las bases de datos buscadas (PsycINFO, PubMED y Google Scholar). Los resultados indicaron el uso de los instrumentos MBI-GS, MBI-HSS, BM-S, CBI y CESQT, pero solo uno de los instrumentos mostró evidencia de validez en este contexto. El Burnout se encuentra asociado con variables personales, organizacionales y específicas. La comparación de estudios e índices fue limitada debido a la falta de estandarización. La ausencia de evidencia de validez y estandarización de resultados conduce a problemas como la falta de soporte empírico para las puntuaciones y la dificultad para comparar estudios. Los datos revelan limitaciones en la evaluación de los niveles de burnout utilizados en el contexto de la seguridad pública, ya que las medidas no presentan evidencia de validez para este contexto, lo que apunta a la necesidad de realizar investigaciones en el área.

Resumo Objetivou-se mapear os instrumentos utilizados na avaliação da síndrome de Burnout em profissionais da segurança pública, indicando os aspectos psicométricos, amostras e variáveis associadas. Foram selecionados 19 estudos nas bases de dados pesquisadas (PsycINFO, PubMED e Google Scholar). Os resultados indicaram a utilização dos instrumentos MBI-GS, MBI-HSS, BM-S, CBI e CESQT, mas apenas um dos instrumentos apresentou evidência de validade nesse contexto. Constatou-se a associação do Burnout com variáveis pessoais, organizacionais e específicas. A comparação dos estudos e dos índices de Burnout foi limitada devido à falta de padronização. A ausência de evidências de validade e de padronização dos resultados acarreta problemas como a falta de suporte empírico das pontuações e dificuldade na comparação dos estudos. Os dados revelam limitações na avaliação dos níveis de Burnout utilizadas no contexto de segurança pública, uma vez que as medidas não apresentam evidências de validade para esse contexto, indicando a necessidade de pesquisas na área.

ABSTRACT Introduction: many revascularization techniques were designed to reduce the imbalance of ischemia-reperfusion injury. This study’s objective is to evaluate retrograde reperfusion (RR) compared to sequential anterograde reperfusion (AR), with and without the washout technique (WO). Method: this prospective cohort study collected data from 94 deceased donor orthotopic liver transplants and divided it into three groups: RR with WO (RR+WO), AP with WO (AR+WO), and AP without WO (AR). This study did not assign the reperfusion technique to the participants. The primary outcome considered the early graft dysfunction, and secondary outcomes included post-reperfusion syndrome (PRS), post-reperfusion lactate, surgery fluid balance, and vasoactive drug dose during the surgery. Results: 87 patients were submitted to the final analysis-29 in the RR+WO group, 27 in the AR+WO group, and 31 in the AR group. Marginal grafts prevalence was not significantly different between the groups (34% vs. 22% vs. 23%; p=0.49) and early graft dysfunction occurred at the same rate (24% vs. 26% vs. 19%; p=0.72). RR+WO reduced serum post-reperfusion lactate (p=0.034) and the incidence of significant PRS (17% vs. 33% vs. 55%; p=0.051), but norepinephrine dosing >0.5mcg/kg/min were not different during the surgery (20,7% vs. 29,6% vs. 35,5%, p=0.45). Conclusions: primary outcome was not significantly different between the groups; however, intraoperative hemodynamic management was safer using the RR+WO technique. We theorized that the RR+WO technique could reduce the incidence of PRS and benefit marginal graft survival following diseased donor orthotopic liver transplantation.

RESUMO Introdução: várias técnicas de reperfusão foram desenvolvidas a fim de reduzir o dano da lesão induzida por isquemia-reperfusão. Este estudo objetivou avaliar a reperfusão retrograda (RR) comparado com a reperfusão anterógrada (AR), com e sem a realização da técnica de lavagem do enxerto (WO). Métodos: coorte prospectiva com 94 transplantes ortotópicos de fígado de doador falecido divididos em três grupos: RR com WO (RR+WO), reperfusão anterógrada com WO (AR+WO), e AR sem WO (AR). Este estudo não designou a técnica de reperfusão entre os participantes. O desfecho primário considerou a disfunção precoce do enxerto, e os desfechos secundários incluíram a síndrome pós-reperfusão (SPR), lactato pós-reperfusão, balanço hídrico operatório, e uso de drogas vasoativas durante o ato peratório. Resultados: 87 pacientes foram submetidos para consolidação dos dados-29 no RR+WO, 27 no AR+WO, e 31 no AR. A prevalência de enxertos maginais não diferiu entre os grupos (34% vs 22% vs 23%; p=0,49). Disfunção precoce do enxerto ocorreu em uma proporção similar (24% vs 26% vs 19%; p=0,72). RR+WO reduziu o lactato sérico pós-reperfusão (p=0,034) e a incidência de SPR severa (17% vs 33% vs 55%; p=0,051), entretanto a infusão de noradrenalina >0,5mcg/kg/min não foi diferente durante a cirurgia (20,7% vs 29,6% vs 35,5%, p=0,45). Conclusões: o desfecho primário não diferiu significativamente entre os grupos; entretanto, o manejo hemodinâmico intra-operatório foi mais seguro no grupo RR+WO. Nós teorizamos que a técnica RR+WO pode reduzir a SPR e beneficiar enxertos marginais no transplante de fígado.

ABSTRACT Polycystic liver disease, a hereditary pathology, usually manifests as autosomal dominant polycystic kidney disease. The many cysts in the liver cause massive hepatomegaly, majorly affecting the patient’s quality of life. In cases of refractory symptoms, liver transplantation is the only treatment choice. A 43-year-old woman was followed up as a hepatology outpatient in August 2020, with a progressive increase in abdominal volume, lower limb edema, and cachexia. The patient was diagnosed with polycystic renal and liver disease with massive hepatomegaly in March 2021, a combined kidney-liver transplant. Liver size represented 13% of the patient’s corporal composition, weighing 8.6kg. The patient was discharged on the 7th postoperative day with no complications. Only 10-20% of patients with polycystic liver disease have clinical manifestations, most of which result from hepatomegaly. An increase in liver volume deteriorates liver function until the condition becomes end-stage liver disease, as kidney function is already compromised; liver-kidney transplantation remains the only treatment choice. The case described drew significant attention to the massive hepatomegaly presented in the patient, with the liver representing over 10% of the patient’s body weight, approximately five to six times larger than a normal-sized liver.

Abstract This article presents a study performed in early 2022 with 186 students who started higher education at a federal university and in a private college – both in the countryside of Minas Gerais – during the remote teaching period, that is, 2020 and 2021. The research was carried out remotely by emailing a form to all participants and having an online discussion with two of them. The questionnaire consisted of questions about the students’ profile, their experiences during remote teaching, as well as of the reduced version of the Academic Experience Questionnaire – QVA-r, which aims to investigate the students’ experiences and adaptation to higher education. The data showed that students’ interpersonal relationships were affected by remote teaching. As for the personal dimension, measured by the QVA-R and related to mental health and wellbeing, it was severely impaired, which can be an effect of social distancing caused by the Covid-19 pandemic. On the other hand, it was surprising to observe how students and the educational institutions themselves developed ways of becoming close - not only in class but also extra-classroom - by using information and communication technologies to have relationships, even from a distance. The results also show that students considered that their learning had been negatively affected by remote teaching, and that, despite it being easy and convenient, they were eager for better learning with the return of face-to-face classes.

Resumo Este artigo apresenta uma pesquisa desenvolvida no início de 2022, com 186 estudantes que ingressaram no ensino superior durante o período remoto, ou seja, 2020 e 2021, em uma universidade federal e em uma faculdade particular, ambas do interior de Minas Gerais. A pesquisa foi realizada de maneira remota com o envio de um formulário por e-mail e a realização de uma roda de conversa online com duas estudantes. O questionário continha perguntas sobre o perfil dos estudantes, suas experiências no ensino remoto e o Questionário de Vivências Acadêmicas reduzido – QVA-r que tem por objetivo investigar as vivências e a adaptação de estudantes ao ensino superior. Os dados mostraram que as relações interpessoais dos estudantes foram afetadas pelo ensino remoto e a dimensão pessoal, medida pelo QVA-r e relacionada com a saúde mental e bem-estar, estava bastante prejudicada, o que pode ser um efeito do distanciamento social provocado pela pandemia do Covid-19. Por outro lado, surpreendeu o quanto os estudantes e as próprias instituições de ensino criaram meios de aproximação, tanto intra quanto extraclasse, utilizando as tecnologias de informação e comunicação para criar conexões, mesmo à distância. Os resultados mostram, ainda, que os estudantes consideravam que a aprendizagem tinha sido prejudicada pelo ensino remoto e que, apesar de suas facilidades e comodidade, estavam ansiosos por uma melhor aprendizagem com o retorno das aulas presenciais.

ABSTRACT Objective: This study aims to evaluate the correlation between religiosity and spirituality in dental students in the state of Bahia. Methods: Cross-sectional study carried out with 266 dental students from the state of Bahia. To measure religious involvement, the Duke Religious Index (DUREL) was used, while the Spirituality Self Rating Scale (SSRS) was used for spirituality. Results: A total of 266 students were evaluated, with a mean age of 32 years (±11.9), most of them female (62.4%), of self-reported race/color (52.3%), with an income greater than three minimum wages (57.1%) and studying/working from the sixth to the tenth semester (41.0%). The correlations evidenced can be classified as weak for organizational religiosity (OR) (r = 0.406), moderate for non-organizational religiosity (RNO) (r = 0.589) and strong for intrinsic religiosity (RI) (r = 0.677). In the multivariate model, the ORN and RI dimensions proved to be predictors of spirituality, and the elevation of the former implies a consequent increase in spirituality. Conclusion: It is concluded that the RNO and RI dimensions are positively correlated with religiosity.

RESUMO Objetivos: Este estudo objetiva avaliar a correlação entre religiosidade e espiritualidade em acadêmicos de odontologia do estado da Bahia. Métodos: Estudo transversal realizado com 266 acadêmicos do curso de odontologia do estado da Bahia. Para mensuração do envolvimento religioso foi utilizada a escala Duke Religious Index (DUREL) enquanto para a espiritualidade a Spirituality Self Rating Scale (SSRS). Resultados: Foram avaliados 266 acadêmicos, com média de idade de 32 anos (±11,9), sendo em sua maioria do sexo feminino (62,4%), de raça/cor autorreferida (52,3%), com renda maior que três salários mínimos (57,1%) e cursando/atuando do sexto ao décimo semestre (41,0%). As correlações evidenciadas podem ser classificadas como fraca para a religiosidade organizacional (RO) (r = 0,406), moderada para a não organizacional (RNO) (r = 0,589) e forte para e a religiosidade intrínseca (RI) (r = 0,677). No modelo multivariado as dimensões RNO e RI mostraram-se preditores da espiritualidade, sendo que a elevação das primeiras implica um consequente aumento da espiritualidade. Conclusão: Conclui-se que as dimensões RNO e RI estão positivamente correlacionadas à religiosidade.
