BACKGROUND Leishmania RNA virus 1 (LRV1) is commonly found in South American Leishmania parasites belonging to the subgenus Viannia, whereas Leishmania RNA virus 2 (LRV2) was previously thought to be restricted to the Old-World pathogens of the subgenus Leishmania. OBJECTIVES In this study, we investigated the presence of LRV2 in strains of Leishmania (L.) infantum, the causative agent of visceral leishmaniasis (VL), originating from different hosts, clinical forms, and geographical regions. METHODS A total of seventy-one isolates were screened for LRV2 using semi-nested reverse transcription-polymerase chain reaction (RT-PCR) targeting the RNA-dependent RNA polymerase (RdRp) gene. FINDINGS We detected LRV2 in two L. infantum isolates (CUR268 and HP-EMO) from canine and human cases, respectively. MAIN CONCLUSIONS To the best of our knowledge, this is the first detection of LRV2 in the New World.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

BACKGROUND The inappropriate use of antibiotics has led to the accelerated growth of resistance to antibiotics. The search for new therapeutic strategies (i.e., antimicrobial peptides-AMPs) has thus become a pressing need. OBJECTIVE Characterising and evaluating Sarconesiopsis magellanica larval fat body-derived AMPs. METHODS Fat body extracts were analysed by reversed-phase high-performance liquid chromatography (RP-HPLC); mass spectrometry was used for characterising the primary structure of the AMPs so found. ProtParam (Expasy) was used for analysing the AMPs’ physico-chemical properties. Synthetic AMPs’ antibacterial activity was evaluated. FINDINGS Four new AMPs were obtained and called sarconesin III, IV, V and VI. Sarconesin III had an α-helix structure and sarconesins IV, V and VI had linear formations. Oligomer prediction highlighted peptide-peptide interactions, suggesting that sarconesins III, V and VI could form self-aggregations when in contact with the microbial membrane. AMPs synthesised from their native molecules’ sequences had potent activity against Gram-positive bacteria and, to a lesser extent, against Gram-negative and drug-resistant bacteria. Sarconesin VI was the most efficient AMP. None of the four synthetic AMPs had a cytotoxic effect. MAIN CONCLUSIONS S. magellanica larval fat body-derived antimicrobial peptides are an important source of AMPs and could be used in different antimicrobial therapies and overcoming bacterial resistance.

A amostra foi constituída por 24.056 Unidades Básicas de Saúde, nas quais atuavam 29.778 Equipes de Atenção Básica; destas, 16.203 (67,3%) possuíam serviços odontológicos e abrigavam 18.119 Equipes de Saúde Bucal. De maneira geral, as Unidades Básicas de Saúde apresentavam boa estrutura física, porém, baixa acessibilidade para pessoas com deficiência. Os consultórios odontológicos apresentavam estrutura física adequada e equipamentos, instrumentos e insumos odontológicos suficientes. No entanto, exibiam alta proporção na oferta de serviço mutiladores em comparação a procedimentos que evitariam as extrações dentárias, e baixa oferta de referência especializada. Além disso, houve evidenciadas desigualdades regionais, favoráveis às regiões Sul e Sudeste.

The sample consisted of 24,056 Basic Health Units, in which 29,778 Primary Care Teams worked; of these, 16,203 (67.3%) had dental services and housed 18,119 Oral Health Teams. In general, the Basic Health Units had a good physical structure, but low accessibility for disabled persons. The dental offices had adequate physical structure and sufficient dental equipment, instruments and supplies. However, they exhibited a high proportion of mutilating service offerings compared to procedures that would avoid tooth extractions, and a low specialized reference supply. In addition, regional inequalities were observed, favorable to the South and Southeast regions.

Cardiovascular responses to passive static flexibility exercises are influenced by the stretched muscle mass and the Valsalva maneuver

BACKGROUND: The respiratory pattern is often modified or even blocked during flexibility exercises, but little is known about the cardiovascular response to concomitant stretching and the Valsalva maneuver (VM) in healthy subjects. OBJECTIVES: This study evaluated the heart rate (HR), systolic blood pressure (SBP), and rate-pressure product (RPP) during and after large and small muscle group flexibility exercises performed simultaneously with the VM. METHODS: Asymptomatic volunteers (N = 22) with the following characteristics were recruited: age, 22 ± 3 years; weight, 73 ± 6 kg; height, 175 ± 5 cm; HR at rest, 66 ± 9 BPM; and SBP at rest, 113 ± 10 mmHg. They performed two exercises: four sets of passive static stretching for 30 s of the dorsi-flexion (DF) of the gastrocnemius and the hip flexion (HF) of the ischio-tibialis. The exercises were performed with (V+) or without (V-) the VM in a counterbalanced order. The SBP and HR were measured, and the RPP was calculated before the exercise session, at the end of each set, and during a 30-min post-exercise recovery period. RESULTS: The within-group comparisons showed that only the SBP and RPP increased throughout the sets (p<0.05), but no post-exercise hypotension was detected. The between-group comparisons showed that greater SBP increases were related to the VM and to a larger stretched muscle mass. Differences for a given set were identified for the HR (the HFV+ and HFV- values were higher than the DFV+ and DFV- values by approximately 12 BPM), SBP (the HFV+ value was higher than the DFV+ and DFV- values by approximately 12 to 15 mmHg), and RPP (the HFV+ value was higher than the HFV- value by approximately 2000 mmHGxBPM, and the HFV+ value was higher than the DFV+ and DFV- values by approximately 4000 mmHGxBPM). CONCLUSION: Both the stretched muscle mass and the VM influence acute cardiovascular responses to multiple-set passive stretching exercise sessions.

Objetivo: A proposta desse trabalho foi investigar o efeito da rotação do quadril na atividade elétrica dos músculos Vasto Medial Oblíquo (VMO), Vasto Lateral Oblíquo (VLO) e Vasto Lateral Longo (VLL) no agachamento a 45º de flexão do joelho associado à posição neutra, rotação lateral a 45º e rotação medial a 15 e 30º de quadril. Materiais e Método: Foram avaliadas 27 mulheres, sendo 15 mulheres normais do grupo Controle (21,1±2,1 anos) e 12 portadoras de Síndrome da Dor Femoropatelar (SDFP) (21,0±2,3 anos). A atividade elétrica do músculo quadríceps foi captada por eletrodos ativos diferenciais simples de superfície, eletromiógrafo de 8 canais e programa Aqdados 7.2.6. Os dados foram normalizados pela porcentagem da média do agachamento a 75º com o quadril em posição neutra RMS (µV). A análise estatística utilizada foi a ANOVA two way com medidas repetidas e Turkey post hoc (p<0,05). Resultados: Os resultados evidenciaram interação entre grupos e músculos (p=0,00), independente da posição do quadril (p=0,39) e não houve interação entre grupos e posições do quadril (p=0,96). Os músculos VMO e VLL do grupo SDFP apresentaram atividade elétrica significativamente maior que o VMO (p=0,00) e VLL (p=0,04) do grupo controle. Por outro lado, a atividade elétrica do músculo VLO não apresentou diferença significativa entre os grupos (p=0,99). Conclusão: Os resultados dessa pesquisa, nas condições experimentais utilizadas, sugerem que a rotação do quadril não influenciou a atividade elétrica das porções do quadríceps nos dois grupos no agachamento a 45º de flexão do joelho.
Objective: The purpose of this study was to evaluate the effect of hip rotation on the electrical activity of the Vastus Medialis Obliquus (VMO), Vastus Lateralis Obliquus (VLO) and Vastus Lateralis Longus (VLL) muscles when squatting with 45º knee flexion, in association with the neutral position, 45º lateral rotation and 15 and 30º medial hip rotation. Method: 27 women were evaluated: 15 healthy women as a control group (21.1 ± 2.1 years) and 12 subjects with patellofemoral pain syndrome (PFPS) (21.0 ± 2.3 years). The electrical activity of the quadriceps muscle was detected using simple active differential surface electrodes, an eight-channel electromyography system and the Aqdados 7.2.6 software. The data were normalized by the mean squatting percentage at 75º with the hip in the neutral position RMS (µV). Two-way ANOVA with repeated measurements followed by the Tukey post hoc test (p<0.05) was used for statistical analysis. Results: The results revealed interaction between groups and muscles (p=0.00), independent of hip position (p=0.39), and there was no interaction between groups and hip positions (p=0.96). The VMO and VLL muscles of the PFPS group showed significantly greater electrical activity than did VMO (p=0.00) and VLL (p=0.04) muscles from the control group. On the other hand, the electrical activity of the VLO muscle did not present any significant difference between the groups (p=0.99). Conclusion: Under the experimental conditions utilized, the results suggested that hip rotation did not alter the electrical activity of the quadriceps portions in either group when squatting with 45º knee flexion.
