Safety and effectiveness of adding fentanyl or sufentanil to spinal anesthesia: systematic review and meta-analysis of randomized controlled trials

Abstract Introduction Spinal infusions of either fentanyl or sufentanil have been reported in international reports, articles, and scientific events worldwide. This study aimed to determine whether intrathecal fentanyl or sufentanil offers safety in mortality and perioperative adverse events. Methods MEDLINE (via PubMed), EMBASE, CENTRAL (Cochrane library databases), gray literature, hand-searching, and clinicaltrials.gov were systematically searched. Randomized controlled trials with no language, data, or status restrictions were included, comparing the effectiveness and safety of adding spinal lipophilic opioid to local anesthetics (LAs). Data were pooled using the random-effects models or fixed-effect models based on heterogeneity. Results The initial search retrieved 4469 records; 3241 records were eligible, and 3152 articles were excluded after reading titles and abstracts, with a high agreement rate (98.6%). After reading the full texts, 76 articles remained. Spinal fentanyl and sufentanil significantly reduced postoperative pain and opioid consumption, increased analgesia and pruritus. Fentanyl, but not sufentanil, significantly reduced both postoperative nausea and vomiting, and postoperative shivering; compared to LAs alone. The analyzed studies did not report any case of in-hospital mortality related to spinal lipophilic opioids. The rate of respiratory depression was 0.7% and 0.8% when spinal fentanyl or sufentanil was added and when it was not, respectively. Episodes of respiratory depression were rare, uneventful, occurred intraoperatively, and were easily manageable. Conclusion There is moderate to high quality certainty that there is evidence regarding the safety and effectiveness of adding lipophilic opioids to LAs in spinal anesthesia.

ABSTRACT BACKGROUND: Chromosomal abnormalities (CAs) have been described in patients with secondary amenorrhea (SA). However, studies on this association are scarce. OBJECTIVES: To evaluate the frequency and types of CAs detected by karyotyping in patients with SA. DESIGN AND SETTING: This retrospective study was performed in a reference clinical genetic service in South Brazil. METHODS: Data were obtained from the medical records of patients with SA who were evaluated between 1975 and 2022. Fisher’s bicaudate exact test and Student’s t-test were used, and P < 0.05 was considered significant. RESULTS: Among 43 patients with SA, 14 (32.6%) had CAs, namely del (Xq) (n = 3), 45,X (n = 2), 46,X,r(X)/45,X (n = 2), 46,XX/45,X (n = 1), 46,X,i(q10)/45,X (n = 1), 47,XXX (n = 1), 46,XX/47,XXX (n = 1), 46,XX/47,XX,+mar (n = 1), 45,XX,trob(13;14)(q10;q10)/46,XXX,trob(13;14)(q10;q10) (n = 1), and 46,XX,t(2;21)(q23;q11.2) (n = 1). Additional findings were observed mostly among patients with CA compared with those without CA (P = 0.0021). No difference in the mean age was observed between the patients with SA with or without CAs (P = 0.268025). CONCLUSIONS: CAs are common among patients with SA, especially those with short stature and additional findings. They are predominantly structural, involve the X chromosome in a mosaic, and are compatible with the Turner syndrome. Patients with SA, even if isolated, may have CAs, particularly del (Xq) and triple X.

Abstract In our study, we aimed to validate a method based on liquid chromatography-mass spectrometry (LC-MS) to quantify spironolactone (SPI) and its active metabolite canrenone (CAN) simultaneously in plasma samples to support in vivo experiments. Compounds were separated by using a C18 column with the isocratic elution of a mobile phase composed of 0.1% (v/v) formic acid in methanol-water (60:40 v/v) at a flow rate of 0.4 mL min−1. SPI and CAN were detected in na electrospray interface operating in a positive ionization mode and quantified using the selective ion mode monitoring of mass-charge ratios (m/z) of 439.0 for SPI and 363.1 for CAN. After calculating the matrix effect using theoretical equations, we observed the strong interference of plasma in the equipment-generated signal, which required creating analytical curves using the matrix as a solvent. The method was nevertheless linear (r 2 > 0.999) in a concentration range of 0.4-5.0 μg mL−1, as well as precise, with a coefficient of variation less than 5%. SPI’s and CAN’s recovery rates from the plasma ranged from 87.4% to 112.1%, while their limits of detection (i.e., 0.07 μg mL−1 and 0.03 μg mL−1, respectively) and quantification (i.e., 0.20 μg mL−1 and 0.08 μg mL−1, respectively) in the presence of plasma contaminants were low. Therefore, the bioanalytical method seems to be feasible for quantifying SPI and CAN in plasma.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Thiosemicarbazones are a class of compounds of interest for Medicinal Chemistry, as they are structurally diverse and have numerous biological activities reported in the literature. This study describes the synthesis of seventeen thiosemicarbazones, which were investigated as potential therapeutic agents for the treatment of Alzheimer's disease through antioxidant tests and an inhibitory assay of the acetylcholinesterase enzyme. All compounds showed excellent inhibition of acetylcholinesterase and exhibited excellent antioxidant action when compared to the standards. In addition, a quantum study was carried out, in which the HOMO (highest occupied molecular orbital) and LUMO (lowest unoccupied molecular orbital) energy values of each compound were obtained. From these theoretical data, chemical properties were calculated and correlated with the experimental data.

This study describes the synthesis of fourteen thiazinanones from a multicomponent reaction of 2-morpholinoehtylamine (as primary amine), arenealdehydes (as carbonyl compound) and the mercaptopropionic acid using both conventional (thermal heating) and ultrasound methodologies. Through thermal heating methodology, the thiazinanones were obtained in 49 to 97% yields for 16 hours and through sonochemistry methodology, the reaction time was reduced for 25 minutes with yields 41 to 88%. The full identification and characterization of unpublished heterocycles were achieved by proton (1H) and carbon 13 (13C) nuclear magnetic resonance (NMR) spectroscopy, mass spectrometry and infrared. Some of them were also characterized by elemental analysis.

RESUMO A pressão pesqueira afeta o comportamento de peixes recifais, especialmente nas espécies alvo de pesca. Nesse contexto é importante entender se áreas fechadas a pesca ("no-take areas": NTAs), consideradas como melhor instrumento para recuperação dos estoques pesqueiros, garantem a integridade dos comportamentos dos peixes. Comparar distâncias iniciais de fuga ("fligth initiation distance": FIDs) de espécies entre recifes de uso múltiplos ("multiple-use areas": MUAs), onde a pesca é permitida (incluindo a pesca submarina), e as NTAs é uma abordagem útil para testar se as NTAs podem ser efetivas como refúgios para peixes recifais. Nesse trabalho FIDs de duas espécies alvo (Sparisoma axillare e Scarus trispinosus ) e uma não alvo (Halichoeres poeyi ) foram comparadas entre MUAs e NTAs. A possível influência do tamanho do grupo ("group size": GS) e o tamanho do corpo ("body size": BS) sobre FID também foram investigados. Foram amostradas quatro MUAs e quatro áreas NTAs no banco dos Abrolhos (Brasil). Nossos resultados mostraram que apenas FID das espécies alvo de pesca foi menor dentro de NTAs e que o BS teve um efeito positivo no FID de todas as espécies. Dentro da NTA, GS e BS foram maiores do que em recifes abertos à pesca apenas para S. trispinosus . Finalmente, sugerimos que NTAs em Abrolhos não são apenas importantes como recuperação dos estoques pesqueiros, mas estão possivelmente atuando como uma área repositória de peixes "ingênuos" (peixes que permitem aproximação humana) para os recifes onde a pesca é permitida.
ABSTRACT Fishing pressure affects the behavior of reef fish, especially of fishery-targeted species. In this context, it is critical to understand if fish behavior is preserved in no-take areas (NTAs), which are considered the best instrument for the recovery of fish stocks. Comparing the flight initiation distances (FIDs) of fish inhabiting multiple-use areas (MUAs), where fishing is allowed (including spearfishing), and NTAs is a useful approach to test whether NTAs can be effective as fish refuges. Here, we compared whether the FIDs of two target (Sparisoma axillare and Scarus trispinosus ) and one non-target (Halichoeres poeyi ) labrids are greater in the MUAs than in the NTAs. We also investigated whether group size (GS) and body size (BS) exert any effect on the FID. We sampled four MUAs and four NTAs in the Abrolhos Bank (Brazil). We found that only for the targeted species FID was shorter inside the NTAs and that the BS had a positive effect on the FID of all species. The GS and BS of the fish are greater in the NTAs than in the multiple-use areas only for S. trispinosus . Our study shows that fish, especially those species that are fishery targets, display an avoidance behavior against spearfishermen likely as a consequence of fishing pressure. We suggest that the NTAs in Abrolhos are not only important sites for the recovery of fish stocks but also possibly act as a repository area of naïve fish (fish that allow human approach), for reefs open for fishing.

Uma forma alterada da proteína prion celular, a PrPSc ou PrPRes, está envolvida na ocorrência das encefalopatias espongiformes transmissíveis. Nós sintetizamos e caracterizamos previamente compostos aromáticos que inibem o acúmulo da proteína prion resistente a proteases (PrPRes) em células infectadas com prion scrapie. Estes compostos pertencem a diferentes classes químicas, incluindo acilhidrazonas, chalconas e oxadiazóis. Alguns dos compostos ativos não apresentaram toxicidade para células de neuroblastoma em cultura e parecem apresentar propriedades de fármacos, uma vez que todos obedecem aos 5 quesitos da regra de Lipinski e apresentam perfil farmacocinético desejável, como predito in silico. Antes de avaliar a eficácia in vivodos compostos aromáticos em camundongos infectados com prion scrapie, é necessário estimar sua segurança em camundongos saudáveis. Neste trabalho nós utilizamos camundongos Suíços para avaliar o perfil de toxicidade aguda dos seis compostos mais promissores, as 2,4,5-trimetoxichalconas (J1, J8, J20 e J35) e os 1,3,4-oxadiazóis (Y13 e Y17). Administração oral em dose única (300 mg/kg) de J1, J8, J20, J35, Y13 e Y17 ou administração repetida via intraperitoneal (10 mg/kg, 3 vezes por semana, por 4 semanas) de J1, J8 e J35, não geraram toxicidade nos camundongos. Nós acreditamos fortemente que as trimetoxichalconas e os oxadiazóis investigados são compostos interessantes para serem futuramente avaliados in vivo contra as doenças causadas por prions.
An altered form of the cellular prion protein, the PrPScor PrPRes, is implicated in the occurrence of the still untreatable transmissible spongiform encephalopathies. We have previously synthesized and characterized aromatic compounds that inhibit protease-resistant prion protein (PrPRes) accumulation in scrapie-infected cells. These compounds belong to different chemical classes, including acylhydrazones, chalcones and oxadiazoles. Some of the active compounds were non-toxic to neuroblastoma cells in culture and seem to possess drugable properties, since they are in agreement with the Lipinski´s rule of 5 and present desirable pharmacokinetic profiles as predicted in silico. Before the evaluation of the in vivo efficacy of the aromatic compounds in scrapie-infected mice, safety assessment in healthy mice is needed. Here we used Swiss mice to evaluate the acute toxicity profile of the six most promising anti-prionic compounds, the 2,4,5-trimethoxychalcones (J1, J8, J20 and J35) and the 1,3,4-oxadiazoles (Y13 and Y17). One single oral administration (300 mg/kg) of J1, J8, J20, J35, Y13 and Y17 or repeated intraperitoneal administration (10 mg/kg, 3 times a week, for 4 weeks) of J1, J8 and J35, did not elicit toxicity in mice. We strongly believe that the investigated trimethoxychalcones and oxadiazoles are interesting compounds to be further analyzed in vivo against prion diseases.

Resumo Apresenta-se uma síntese atualizada de informações sobre algas no Brasil objetivando refinar os dados reunidos até o presente, bem como avaliar os avanços sobre o conhecimento da diversidade de algas no país desde a publicação do Catálogo de Plantas e Fungos do Brasil. Os resultados de 2015 mostraram um acréscimo de 1.250 espécies (35.7%) a um total de 4.747 em relação a 2010. As classes mais diversas em número de espécies foram Bacillariophyceae, Conjugatophyceae, Florideophyceae, Cyanophyceae, Dinophyceae e Euglenophyceae. Bacillariophyceae e Cyanophyceae tiveram o maior acréscimo de espécies no intervalo de cinco anos. A região Sudeste e Sul foram as mais diversas, porém, as regiões Nordeste com os estados do Piauí e Sergipe e Centro-Oeste com os estados de Mato Grosso, Goiás e Distrito Federal destacaram-se no cenário da biodiversidade nacional. Apesar da escassez de taxonomistas e da infraestrutura limitada, os resultados obtidos evidenciaram um avanço significativo no conhecimento da diversidade de algas no país nesse período de cinco anos, iniciando uma mudança quanto as diferenças geográficas regionais.
Abstract An updated synthesis of cyanobacteria and algae information is presented for Brazil aiming to refine the data gathered to date and evaluate the progress of the biodiversity knowledge about these organisms in the country since the publication of the Catálogo de Plantas e Fungos do Brasil. The results of 2015 showed an increase of 1,250 species (35.7%) when compared to 2010, reaching a total of 4,747 species. The most diverse classes in species number were the Bacillariophyceae, Conjugatophyceae, Florideophyceae, Cyanophyceae, Dinophyceae and Euglenophyceae. Bacillariophyceae and Cyanophyceae had the highest increase in species number in the five-year interval. The Southeast and South regions were the most diverse, however, the Northeast, with the states of Piauí and Sergipe, and the Central-west region, with Mato Grosso, Goiás and Distrito Federal, also stood out in the national algal biodiversity scenario. Despite the shortage of taxonomists and limited infrastructure, the results showed a significant improvement in the knowledge regarding the diversity of cyanobacteria and algae in the country during the study period, starting to even out regional geographical differences caused by subsampling.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

A doença de Chagas, uma infecção parasitária amplamente distribuída na América Latina, é um problema grave de saúde pública com conseqüências devastadoras em termos de morbidade e mortalidade humana. A enzima cruzaína é a principal cisteíno protease do Trypanosoma cruzi, agente etiológico da tripanossomíase Americana ou doença de Chagas, e foi selecionada como alvo atrativo para o desenvolvimento de novos fármacos tripanocidas. No presente trabalho, a síntese e os efeitos inibitórios de uma série de trinta e três chalconas e sete hidrazidas são descritos contra a enzima cruzaína de T.cruzi. A maioria dos compostos mostraram inibição promissora in vitro (valores de IC50 na faixa de 20-60 μM), o que sugere o potencial desses compostos como candidatos a líderes para contínuo desenvolvimento. Doze compostos são inéditos, sendo que quatro destes (7, 13, 16 e 18) estão entre os inibidores mais potentes da série.
Chagas' disease, a parasitic infection widely distributed throughout Latin America, is a major public health problem with devastating consequences in terms of human morbidity and mortality. The enzyme cruzain is the major cysteine protease from Trypanosoma cruzi, the etiologic agent of American trypanosomiasis or Chagas' disease, and has been selected as an attractive target for the development of novel trypanocidal drugs. In the present work, we describe the synthesis and inhibitory effects of a series of thirty-three chalcone and seven hydrazide derivatives against the enzyme cruzain from T. cruzi. Most of the compounds showed promising in vitro inhibition (IC50 values in the range of 20-60 μM), which suggest the potential of these compounds as lead candidates for further development. Twelve compounds have not been reported before, and four of them (7, 13, 16 e 18) are among the most potent inhibitors of the series.

Does the way I see you affect the way I see myself? Associations between interviewers' and interviewees' "color/race" in southern Brazil

Avaliou-se a relação entre "cor/raça" de entrevistadoras, auto e heteroclassificação de "cor/raça" dos entrevistados. Foi realizado um estudo com indivíduos adultos de Pelotas, Rio Grande do Sul, Brasil. As associações foram verificadas com tabelas de contingência e modelos multinomiais, ajustando-se para variáveis sócio-econômicas e demográficas dos entrevistados. Indivíduos com > 40 anos tiveram uma chance 2,1 vezes maior de se classificarem como pardos do que como brancos, quando abordados por entrevistadoras pretas, em relação às brancas. Entrevistados dessa mesma faixa etária apresentaram 2,5 vezes menos chances de se classificarem como pretos do que como brancos, ao serem abordados por entrevistadoras pretas. Essas diferenças foram mais expressivas entre homens de > 40 anos. Entrevistadoras pretas tiveram chance 2,5 vezes menor de classificar homens de > 40 anos como pretos do que como brancos, em comparação com as brancas. Os resultados indicam o caráter complexo da classificação racial, sugerindo a influência de características pessoais das entrevistadoras sobre a "cor/raça" dos entrevistados.
This study assessed the associations between female interviewers' self-classified "color/race" and participants' self- and interviewer-classified "color/race". A cross-sectional study was carried out among adult individuals living in Pelotas, southern Brazil. Associations were examined by means of contingency tables and multinomial regression models, adjusting for interviewees' socioeconomic and demographic factors. Individuals aged > 40 years were 2.1 times more likely to classify themselves as brown (versus white) when interviewed by black (as compared to white) interviewers. Participants in the same age group were 2.5 times less likely to classify themselves as black (versus white), when interviewed by black interviewers. These differences were even greater among men 40 years or older. Compared to white interviewers, black female interviewers were 2.5 times less likely to classify men aged > 40 years as black. These results highlight the complexity of racial classification, indicating the influence of the interviewer's physical characteristics on the interviewee's "color/race".

Avaliou-se a relação entre "cor/raça" de entrevistadoras, auto e heteroclassificação de "cor/raça" dos entrevistados. Foi realizado um estudo com indivíduos adultos de Pelotas, Rio Grande do Sul, Brasil. As associações foram verificadas com tabelas de contingência e modelos multinomiais, ajustando-se para variáveis sócio-econômicas e demográficas dos entrevistados. Indivíduos com > 40 anos tiveram uma chance 2,1 vezes maior de se classificarem como pardos do que como brancos, quando abordados por entrevistadoras pretas, em relação às brancas. Entrevistados dessa mesma faixa etária apresentaram 2,5 vezes menos chances de se classificarem como pretos do que como brancos, ao serem abordados por entrevistadoras pretas. Essas diferenças foram mais expressivas entre homens de > 40 anos. Entrevistadoras pretas tiveram chance 2,5 vezes menor de classificar homens de > 40 anos como pretos do que como brancos, em comparação com as brancas. Os resultados indicam o caráter complexo da classificação racial, sugerindo a influência de características pessoais das entrevistadoras sobre a "cor/raça" dos entrevistados.

A incidência de diabetes mellitus tem aumentado continuamente nos últimos anos. Os pacientes acometidos por esta enfermidade crônica têm maior risco de desenvolver doença aterosclerótica coronária. Infelizmente, devido ao freqüente envolvimento difuso de múltiplos vasos, a intervenção percutânea, quer seja com cateter-balão ou com stent não farmacológico, tem sido associada a pior prognóstico quando comparada à cirurgia de revascularização miocárdica neste grupo de pacientes. Recentemente, a introdução dos stents farmacológicos renovou o entusiasmo para o tratamento percutâneo de pacientes com lesões mais complexas. Neste artigo, reportamos um caso de uma paciente de 63 anos, portadora de diabetes em uso de insulina, tratada percutaneamente com nove stents farmacológicos (TaxusTM, Boston Scientific).
The incidence of diabetes mellitus has continuously risen over recent years. Patients with this chronic condition have a higher risk of developing coronary artery disease. Unfortunately, due to the frequent diffuse involvement of multiple vessels, the long term outcomes after percutaneous intervention, whether by balloon catheter or bare-metal stent, has always been poor and frequently inferior to cardiac surgery for this subset of patients. Recently, the introduction of drug-eluting stents (DES) has brought new hope to treat patients with more complex diseases percutaneously. In this paper we report a successful percutaneous approach of a 63-year-old insulin dependent diabetic patient treated with nine DES (TaxusTM, Boston Scientific).

In this review, emphasis will be given to the chemical and biological aspects of several sub-classes of this family, incluing maleimides, succinimides, glutarimides, naphtalimides, etc. Additionally, will be focused the contribution of our research group in this field.
