ABSTRACT Brazil is a benchmark in sugarcane production, with the state of São Paulo standing out as the largest Brazilian producer. However, for sugarcane suppliers and mills to sustain this activity, there is a need to improve productivity per hectare and reduce production costs. In this regard, this study aimed to propose fuzzy systems to estimate sugarcane productivity based on planted area (Area) and total cost of soil tillage (TCST) for raw material suppliers and mills. To this end, two fuzzy inference systems were constructed for the output variable (productivity) from two input variables (Area and TCST), considering five membership functions (very low, low, medium, high, and very high). Additionally, a survey on 42 sugarcane suppliers and 31 mills in the state of São Paulo was used for model construction. The results showed that the relationship between Area and TCST reflects on the productivity of sugarcane suppliers and mills in distinct ways. For suppliers, an increase in productivity is observed when there is an almost negative relationship between both input variables. For mills, productivity rises when these variables fluctuate in the same direction. Therefore, the proposed method is viable and provides relevant information for conjecturing survival strategies for agents in the sugarcane energy sector.

Resumo Introdução: A Secretaria de Estado da Saúde de Goiás (SES-GO), Brasil, elaborou um programa de reabilitação voltado para a Atenção Primária e tem por finalidade a recuperação dos usuários que apresentam consequências funcionais após a COVID-19. Por conseguinte, as avaliações dos protocolos propostos são importantes para subsidiar ações e políticas públicas de saúde para a oferta dos serviços de reabilitação à população. Objetivo: Compreender as percepções dos usuários do programa de reabilitação da Secretaria de Estado da Saúde de Goiás quanto aos serviços de acompanhamento oferecidos para sua recuperação pós-COVID-19 na Atenção Primária à Saúde. Métodos: Trata-se de um estudo descritivo, exploratório, com abordagem qualitativa. Realizou-se a aplicação de uma pergunta norteadora a 18 usuários, de duas cidades da Região Metropolitana de Goiânia. As respostas foram redigidas pela pesquisadora e tratadas com a análise de conteúdo temática. Resultados: Da análise, surgiram duas categorias temáticas, quais sejam: percepções dos efeitos na funcionalidade e; percepções dos efeitos terapêuticos atribuídas pelos usuários após o programa de reabilitação idealizado pela Secretaria de Saúde do Estado de Goiás. A melhora física potencializa a sua participação no convívio social, o que proporciona benefícios nos seus aspectos emocionais. As diferenças foram perceptíveis pelos usuários em comparação com os momentos em que não tinham realizado nenhum procedimento terapêutico antes da proposta idealizada pela SES-GO. A autoeficácia foi outro elemento latente nas respostas dos usuários referente à implementação do programa. A satisfação dos usuários com a proposta é externada pelos usuários, sendo um fator essencial para evidenciar a sua qualidade. Considerações finais: O programa de reabilitação pode ser um aliado imprescindível ao levar em consideração os aspectos do modelo biopsicossocial na recuperação dos usuários. Esses resultados podem implicar na ampliação do programa de reabilitação proposto pela SES-GO junto às Secretarias Municipais de Saúde.
Abstract Introduction: The State Department of Health of Goiás (SES-GO), Brazil, developed a rehabilitation program aimed at Primary Care and aims at the recovery of users who have functional consequences after COVID-19. Therefore, the evaluation of the proposed protocols is important to support public health actions and policies for the provision of rehabilitation services to the population. Methods: This is a descriptive, exploratory study with a qualitative approach. A guiding question was applied to 18 users, from two cities in the Metropolitan Region of Goiânia. The answers were written by the researcher and treated with thematic content analysis. Results: From the analysis, two thematic categories emerged, namely: perceptions of the effects on functionality and; perceptions of therapeutic effects attributed by users after the rehabilitation program designed by the Health Department of the State of Goiás. Physical improvement enhances their participation in social life, which provides benefits in their emotional aspects. The differences were noticeable by the users in comparison with the moments when they had not performed any therapeutic procedure before the proposal idealized by SES-GO. Self-efficacy was another latent element in users' responses regarding program implementation. The users' satisfaction with the proposal is expressed by the interviewees, being an essential factor to demonstrate its quality. Final Consideration: The rehabilitation program can be an essential ally when considering aspects of the biopsychosocial model in the recovery of users who have had the consequences of COVID-19. These results may imply the expansion of the rehabilitation program proposed by the Health Department of Goiás to other local and regional Health Departments.

RESUMO OBJETIVO Reconhecer elementos que facilitaram ou dificultaram as etapas do processo de implementação do PlanificaSUS. MÉTODOS Foi realizado um estudo de casos múltiplos em quatro regiões de saúde pré-selecionadas – Belo Jardim (PE), Fronteira Oeste (RS), Sul-Mato-Grossense (MT), Valença (BA) –, utilizando como condições traçadoras a hipertensão arterial sistêmica e a atenção materno infantil. Foi realizada observação participante na Comissão Intergestora Regional (CIR), bem como entrevistas em profundidade nas quatro regiões estudadas, com informantes-chave da gestão estadual e municipal e com profissionais dos serviços da atenção primária à saúde e atenção ambulatorial especializada envolvidos no projeto. Construiu-se análise em três dimensões: política, técnico operacional e contexto. RESULTADOS Na dimensão política, ficou evidenciado que, de alguma forma, as regiões identificaram no projeto uma oportunidade de articulação entre estados e municípios e uma aposta política importante para a construção de redes e linhas de cuidado, mas ainda com muitos caminhos a percorrer para enfrentar as barreiras identificadas nas disputas em jogo na construção do Sistema Único de Saúde (SUS). Na dimensão técnico operacional, é importante considerar que foi estimulada uma cultura de planejamento local na APS, além do favorecimento do uso de ferramentas tradicionais, no sentido de organizá-la e qualificá-la, como a organização dos cadastros, da agenda e da demanda. No entanto, os processos centralizados formativos e indutores da planificação nem sempre respondem às necessidades singulares dos territórios e podem produzir barreiras à implementação. CONCLUSÕES Cabe considerar o papel do gestor estadual na aposta relacionada ao projeto, tanto no nível central quanto no nível regional, e o efeito de processos de mobilização da atenção primária à saúde e amplificação de sua potência, mas ainda com muitos caminhos a percorrer para enfrentar as barreiras identificadas nas disputas em jogo na construção do SUS.
ABSTRACT OBJECTIVE To recognize elements that facilitated or hindered the PlanificaSUS implementation stages. METHODS A multiple case study was carried out in four pre-selected health regions in Brazil—Belo Jardim (PE), Fronteira Oeste (RS), Sul-Mato-Grossense (MT) and Valença (BA) using systemic arterial hypertension and maternal and child care as tracer conditions. Participant observation (in regional interagency commissions) and in-depth interviews with key informants from state and municipal management and primary health care and specialized outpatient care service professionals within the project were carried out in these four regions. Analysis was built according to political, technical-operational, and contextual dimensions. RESULTS The political dimension evinced that the regions found the project an opportunity to articulate states and municipalities and an important political bet to build networks and lines of care but that there remained much to be faced in the disputes related to building the Unified Health System (SUS). In the technical operational dimension, it is important to consider that primary health care stimulated a culture of local planning and favored traditional tools to organize and improve it, such as organizing registrations, agendas, and demands. However, centralized training and planning-inducing processes fail to always respond to local needs and can produce barriers to implementation. CONCLUSIONS It is worth considering the central and regional role of state managers in the commitment related to the project and the effect of mobilizing primary health care and expanding its power. There remains much to be faced in the disputes at stake in bullring SUS.

Quality of free-range and commercial eggs subjected to different storage periods and temperatures

Resumo Objetivou-se avaliar a qualidade interna de ovos provenientes de poedeiras comerciais e de galinhas caipiras submetidos a diferentes períodos e temperaturas de armazenamento. Para o experimento, foram utilizados 280 ovos. Os ovos foram distribuídos aleatoriamente nos diferentes tratamentos adotando-se o delineamento experimental inteiramente casualizado, em esquema fatorial 2 x 7, duas temperaturas, sete períodos de armazenamento, totalizando 14 tratamentos com 10 repetições. Os tratamentos consistiram em duas condições de armazenamento: sob refrigeração (6 ± 1,0ºC) e em temperatura ambiente (26,6 ± 1,0ºC). Os ovos foram analisados por um período de 30 dias, com avaliações realizadas em diferentes períodos de armazenamento (0, 5, 10, 15, 20, 25 e 30 dias). Para cada condição de armazenamento, foram separados 140 ovos, sendo 70 ovos comerciais e 70 ovos caipiras. Ocorreu aumento linear na perda de peso dos ovos, peso da gema, pH do albúmen, pH da gema, comprimento e largura do albúmen e da gema dos ovos comerciais e caipiras, à medida que se aumentava o período de armazenamento. Verificou-se redução linear no peso, altura e índice do albúmen e no índice da gema dos ovos comerciais e caipiras conforme se aumentava o período de armazenamento, com respostas mais acentuadas para ovos acondicionados em temperatura ambiente (P<0,05). A porcentagem de albúmen foi reduzida linearmente apenas para ovos comerciais (P<0,05). Ovos mantidos sob temperatura ambiente reduzem a sua qualidade a partir dos 15 dias de armazenamento, sendo o armazenamento sob refrigeração durante o período de 30 dias, o recomendado para preservar a vida de prateleira do ovo para consumo.
Abstract The objective was to evaluate the internal quality of eggs from commercial laying hens and free-range hens subjected to different storage periods and temperatures. For the experiment, 280 eggs were randomly distributed into different treatments, adopting a completely randomized design, in a 2 x 7 factorial arrangement of two temperatures, seven storage periods, totaling 14 treatments with 10 replications. The treatments consisted of two storage conditions: under refrigeration (6 ± 1.0ºC) and at room temperature (26.6 ± 1.0ºC). Eggs were analyzed for 30 days, with evaluations in different storage periods (0, 5, 10, 15, 20, 25 and 30 days). For each storage condition, 140 eggs were separated, 70 commercial eggs and 70 free-range eggs. There was a linear increase in egg weight loss, yolk weight, albumen pH, yolk pH, length and width of albumen and yolk of commercial and free-range eggs, as the storage period increased. There was a linear reduction in weight, height and albumen index and in the yolk index of commercial and free-range eggs as the storage period increased, with more pronounced responses for eggs stored at room temperature (P<0.05). Albumen percentage was linearly reduced only for commercial eggs (P<0.05). Eggs kept at room temperature reduce their quality after 15 days of storage, and the storage under refrigeration for 30 days is recommended to preserve the shelf life of the egg for consumption.

RESUMO Objetivo: Validar um questionário de avaliação do impacto da demência num dos membros do agregado familiar. Métodos: Estudo metodológico. O instrumento foi construído, com base na revisão da literatura, na opinião de peritos e na experiência dos investigadores. Foi aplicado a uma amostra não probabilística de conveniência constituída por 262 familiares de pessoas que coabitam diariamente com a pessoa com demência. A validade de constructo foi estudada pela análise fatorial exploratória, método dos componentes principais, com rotação varimax dos itens. Resultados: Obteve-se um instrumento com 30 itens distribuídos por quatro dimensões “Dimensão emocional”, “Dimensão económica”, “Dimensão relações familiares” e “Dimensão procura de suporte”. A análise fatorial revelou uma variância explicada total de 54,96% e um alfa de Cronbach total de ,899. Conclusões: O instrumento apresenta uma consistência interna elevada agrupando-se em quatro dimensões, todas elas intimamente ligadas à adaptação da família ao aparecimento da demência num dos seus membros.

The instrument was designed based on literature review, expert opinion, and researchers’ experience and then applied to a non-probability convenience sample consisting of 262 family members who live daily with a person with dementia. The construct validity was studied by exploratory factor analysis, principal components method, with varimax rotation of the items. Results: An instrument with 30 items was obtained, distributed in four dimensions: “Emotional dimension,” “Economic dimension,” “Family relations dimension,” and “Support-seeking dimension.” Factor analysis revealed a total explained variance of 54.96% and a total Cronbach’s alpha of .899. Conclusions: The instrument presents high internal consistency, grouped into four dimensions, all closely related to the family’s adaptation to the onset of dementia in one of its members.

El instrumento fue construido basado en la revisión de la literatura, opinión de peritos y experiencia de los investigadores y, en seguida, aplicado a una muestra no probabilística de conveniencia constituida por 262 familiares que cohabitan diariamente con una persona con demencia. La validez de constructo fue estudiada por análisis factorial exploratorio, método de los componentes principales, con rotación varimax de los ítems. Resultados: Obtuvo un instrumento con 30 ítems, distribuidos en cuatro dimensiones: “Emocional”, “Económica”, “Relaciones familiares” y “Búsqueda de soporte”. El análisis factorial reveló variancia explicada total de 54.96% y alfa de Cronbach total de .899. Conclusiones: El instrumento presenta consistencia interna elevada agrupándose en cuatro dimensiones, todas íntimamente relacionadas a la adaptación de la familia delante el aparecimiento de la demencia en uno de sus miembros.

RESUMO Os canabinoides compreendem os endocanabinoides, fitocanabinoides e os canabinoides sintéticos e desempenham ações no sistema nervoso central e periférico. Uma quantidade enorme de publicações tem sido lançada nos últimos anos, embora a cannabis seja conhecida por milênios. Os Departamentos Científicos da Academia Brasileira de Neurologia descreveram as evidências do uso médico em suas áreas. A literatura está em constantes mudanças e possíveis novas evidências podem surgir nos próximos dias ou meses. A prescrição dessas substâncias deve ser discutida com os pacientes e suas famílias, com conhecimento sobre eventos adversos e sua eficácia.
ABSTRACT Cannabinoids comprehend endocannabinoids, phytocannabinoids, and synthetic cannabinoids, with actions both in the central and peripherical nervous systems. A considerable amount of publications have been made in recent years, although cannabis has been known for over a thousand years. Scientific Departments from the Brazilian Academy of Neurology described evidence for medical use in their areas. Literature is constantly changing, and possible new evidence can emerge in the next days or months. Prescription of these substances must be discussed with patients and their families, with knowledge about adverse events and their efficacy.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Vitamin D is a fat-soluble vitamin that plays a role not only in calcium homeostasis, but also in several other functions, including cell growth and immune functions, and is considered a neurosteroid. Vitamin D deficiency is highly prevalent worldwide and has been suggested to be associated with an increased risk of emotional disorders. Therefore, the association between vitamin D levels and psychophysiological disorders, such as depression, anxiety, and mood, has been investigated. To list these variables, a bibliographical literature research was conducted in the MEDLINE/PubMed, Web of Science, Scopus, Science Direct and PsycINFO databases, between November and December 2020, with no year limits of publication. The studies involved humans aged between 18 and 59 years without associated diseases. This review presents evidence of the main variables involved in this association, main tools used to verify these variables, and methods used to verify circulating vitamin D levels in populations. Most studies have indicated that the main psychophysiological variables involved with vitamin D levels are depression and anxiety followed by mood, and an association has been observed between increased serum vitamin D levels and reduction in symptoms of depression, anxiety, and mood, and there is a heterogeneity of methods for assessing vitamin D. More studies are clearly needed to improve our understanding of their role in modulating the psychophysiological aspects of vitamin D levels.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

RESUMO Objetivos: identificar os determinantes socioespaciais associados à incidência de infecção pelo vírus da imunodeficiência humana (HIV) descritos na literatura nacional e internacional. Métodos: revisão integrativa da literatura que incluiu artigos originais indexados nas bases de dados LILACS, PUBMED, CINAHL e Scopus, no recorte temporal de 2015 a 2019. A síntese dos artigos foi feita de forma descritiva em duas categorias segundo o tipo de abordagem. Resultados: foram selecionados 13 artigos, com predomínio de estudos ecológicos. Regiões com acesso deficiente aos serviços de saúde, alta densidade populacional, maiores índices de pobreza, intenso fluxo migratório, áreas fronteiriças, bem como precárias condições socioeconômicas e comportamentos de risco estão associados às maiores taxas de infecção. Considerações Finais: revela-se a influência das desigualdades regionais sobre a incidência de infecção pelo HIV, ratificando a relevância das técnicas de análise espacial para o mapeamento da distribuição de doenças e identificação de áreas de risco.

The synthesis of the articles was done in a descriptive manner in two categories according to the type of approach. Results: 13 articles were selected, with a predominance of ecological studies. Regions with deficient access to health services, high population density, higher rates of poverty, intense migratory flow, border areas, as well as precarious socioeconomic conditions, and risk behaviors are associated with higher rates of infection. Final Considerations: the influence of regional inequalities on the incidence of HIV infection is revealed, confirming the relevance of spatial analysis techniques for mapping the distribution of diseases and identifying risk areas.

Evidence shows that metabolic syndrome (MS) is associated with a greater risk of developing Parkinson's disease (PD) because of the increase in oxidative stress levels along with other factors such as neuroinflammation and mitochondrial dysfunction. However, because some studies have reported that MS is associated with a lower risk of PD, the relationship between MS and PD should be investigated. This study aimed to investigate the effect of MS on PD. Two authors searched five electronic databases, namely, MEDLINE, PubMed, Scopus, PsycINFO, Web of Science, and Science Direct, for relevant articles between September and October 2020. After screening the title and abstract of all articles, 34 articles were selected for full-text review. Finally, 11 articles meeting the eligibility criteria were included in the study. The quality of articles was critically evaluated using the Joanna Briggs Institute. Overall, we evaluated data from 23,586,349 individuals (including healthy individuals, with MS and PD) aged 30 years or more. In cohort studies, the follow-up period varied between 2 and 30 years. MS contributed considerably to the increase in the incidence of PD. In addition, obesity, a component of MS, alone can increase the probability of developing neurodegenerative diseases. However, despite few studies on MS and PD, changes in cognitive function and more rapid progression of PD disease has been documented in patients with MS using methods commonly used in research.

RESUMO Avaliar propostas de avaliação da educação é um passo relevante para apreciar o potencial dessas iniciativas em promover qualidade com equidade, tendo em vista garantir o cumprimento do dever do Estado na garantia do direito à educação. Com esse pressuposto, o artigo apresenta aspectos e dimensões para análise de propostas de avaliação de políticas de Educação Infantil no âmbito de sistemas e redes de ensino. As proposições resultam de levantamento e interpretação de literatura e se apoiam em legislação e normas vigentes no país, em diálogo com a literatura que trata de requisitos para a elaboração e implementação de avaliações. Os resultados do estudo ratificam a existência de parâmetros para subsidiar processos de avaliação da etapa, estando estes direcionados para as questões de acesso, insumos, processos e resultados, destacando-se a importância de considerar as especificidades da etapa, as finalidades da avaliação e a consequência dada aos seus resultados.
ABSTRACT Evaluating education evaluation proposals is a relevant step to consider the potential of these initiatives in promoting quality with equity, to ensure the fulfillment of the State's duty in guaranteeing the right to education. With this assumption, the article presents aspects and dimensions for the analysis of evaluation proposals regarding the Early Childhood Education policy, within the scope of education systems and networks. The propositions result from the survey and interpretation of literature and are supported by legislation and norms in force in the country, in dialogue with the literature that deals with requirements for the preparation and implementation of evaluations. The results of the study confirm the existence of parameters to support the evaluation processes of the stage, which are directed to issues of access, inputs, processes, and results, highlighting the importance of considering the specificities of the stage, the purposes of the evaluation and consequence given to their results.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

ABSTRACT Objective to examine the Brazilian and international scientific production for connections between bioethical principles and the health care provided to older adults at the end of their lives. Method between October and November 2019 this integrative review searched the Medline (Pubmed), Lilacs, and Scopus databases using the descriptors: “Palliative Care”, “Aged”, and “Bioethics” for the period 2014-2019. Results the following categories emerged from the selected articles: therapeutic conducted with regard to end-of-life care; decision making on end-of-life care; and challenges in end-of-life care. Conclusion of particular importance is for health professionals to uphold their commitment to older adults and their families, consider their subjectivities and preferences, and empower and equip them so that care is guided by bioethical principles in order to assure a dignified process of dying and death.
RESUMEN Objetivo examinar la producción científica brasileña e internacional en busca de conexiones entre los principios bioéticos y la atención médica brindada a los adultos mayores al final de sus vidas. Método entre octubre y noviembre de 2019, esta revisión integradora buscó en las bases de datos Medline (Pubmed), Lilacs y Scopus utilizando los descriptores: "Cuidados paliativos", "Envejecido" y "Bioética" para el período 2014-2019. Resultados las siguientes categorías surgieron de los artículos seleccionados: terapéutico realizado con respecto a la atención al final de la vida; toma de decisiones sobre la atención al final de la vida; y desafíos en la atención al final de la vida. Conclusión es de particular importancia que los profesionales de la salud mantengan su compromiso con los adultos mayores y sus familias, consideren sus subjetividades y preferencias, y los empoderen y equipen para que la atención se guíe por principios bioéticos para asegurar un proceso digno de muerte y muerte.
RESUMO Objetivo analisar a produção científica nacional e internacional, buscando as interfaces existentes entre os princípios bioéticos e os cuidados em saúde prestados ao fim da vida às pessoas idosas. Método revisão integrativa, cuja busca ocorreu entre outubro e novembro de 2019 nas bases de dados Medline via Pubmed, Lilacs e Scopus com os descritores: “Palliative Care", “Aged” e “Bioethics” de 2014-2019. Resultados a partir dos artigos selecionados, emergiram as seguintes categorias: condutas terapêuticas frente aos cuidados ao fim da vida; tomada de decisão nos cuidados ao fim da vida; e desafios nos cuidados ao fim da vida. Conclusão destaca-se a relevância dos profissionais manterem o compromisso com a pessoa idosa e sua família de forma a considerar suas subjetividades e preferências e os instrumentalizar para que os cuidados sejam pautados em princípios bioéticos, para assim proporcionar um processo de morte e de morrer com dignidade.
