ABSTRACT Introduction: Fabry disease (FD) is an inborn error of metabolism characterized by α-galactosidase A deficiency. The primary objective was to evaluate the genetic and phenotypic profile of Fabry disease in hemodialysis. Methods: Observational cohort study to determine the incidence of genetic variations and phenotypic changes for FD in hemodialysis patients in the Paraiba Valley and Eastern São Paulo. Genetic testing for the GLA gene was performed for men and women over 12 years of age at the hemodialysis clinics between January 2016 and December 2019 as a screening protocol. Results: The cases came from screening exams of the index case among patients with chronic kidney disease, resulting in 17 families and totaling 82 patients under study. The classification of the most prevalent variant was that of uncertain significance (54%), followed by the pathogenic variant (46%). Five patients in two families were described with two types of variants not previously described in the literature, with pathogenic behavior. Comparing the types of variants, the presence of a pathogenic variant was associated with higher levels of lysoGB3, lower values for alpha-GAL activity and higher frequency of symptoms related to FD. Conclusion: We characterized an extensive population of patients with FD variants with rich genetic, clinical and biomarker details. We believe that this study can help to better characterize the Brazilian population with FD and the most frequent types of variants.

RESUMO Introdução: A doença de Fabry (DF) é um erro inato do metabolismo caracterizado pela deficiência da enzima α-galactosidase A. O objetivo primário foi avaliar o perfil genético e fenotípico da doença de Fabry em hemodiálise. Métodos: Estudo de coorte observacional para determinar a incidência de variações genéticas e alterações fenotípicas para DF em pacientes em hemodiálise no Vale do Paraíba e Zona Leste de São Paulo. O teste genético para o gene GLA foi realizado para homens e mulheres em todos os pacientes das clínicas de hemodiálise maiores de 12 anos entre janeiro de 2016 a dezembro de 2019 como protocolo de rastreio. Resultados: Os casos foram provenientes de exames de triagem do caso índice entre pacientes portadores de doença renal crônica, resultando em 17 famílias e totalizando 82 pacientes em estudo. A classificação da variante mais prevalente foi a de significado incerto (54%), seguida da variante patogênica (46%). Foram descritos 5 pacientes em duas famílias com dois tipos de variantes ainda não previamente descritos na literatura com comportamento patogênico. Na comparação entre os tipos de variantes, a presença de variante patogênica foi associada a maiores níveis de lysoGB3, menores valores da atividade da alfa-GAL e maior frequência de sintomas relativos à DF. Conclusão: Caracterizamos uma extensa população de pacientes com variantes para DF com riqueza de detalhes de genética, clínica e de biomarcadores. Acreditamos que este estudo possa auxiliar na melhor caracterização da população brasileira com DF e nos tipos mais frequentes de variantes.

In higher education, reasonable accommodations are increasingly made for students with a wide range of disabilities. However, rigorous assessment is paramount to ensure these students are supported while preventing ineligible students from gaining unfair advantages. In this context, we sought to identify under which circumstances a university student should be allowed academic accommodation for attention-deficit/hyperactivity disorder (ADHD) and to outline an evidence-based policy for use in Brazil based on the global experience. We reviewed the literature to acquire information on what documents are commonly required by disability services before accommodations for ADHD are provided (including detection of malingering) and scrutinized the eligibility criteria of leading universities worldwide. Finally, renowned experts in the field and national stakeholders were consulted. Despite an exhaustive search, we found no international standard for the assessment of students with ADHD who request academic accommodation; even renowned institutions worldwide differ in their approaches to granting accommodations on the grounds of ADHD. Therefore, we propose a unified set of nationwide criteria for Brazilian universities, which could be generalized internationally. Abstract Objectives: The objective of the study was to verify whether (FFM), maturity status (MS) and chronological age (CA) are determinants of physical fitness performance, and to analyze FFM and physical performance aligned by CA and MS in children and adolescents. Methods: A descriptive correlational study was carried out in 863 schoolchildren. Weight, height, and waist circumference (WC) were evaluated. Body mass index (BMI), FFM, fat mass (FM), MS (Age at peak height velocity, APHV) were calculated. The physical tests of velocity 20 m, agility 5 m × 10rep, and horizontal jump (HJ) were evaluated. Results: The APHV was estimated in boys at 14.0 ± 0.36APHV and in girls at 11.96 ± 0.49APHV. The relationships between CA and APHV with FFM was r = 0.80 in boys and r = 0.44 to 0.45 in girls. The relationships between FFM and physical tests in boys were [HJ (r = 0.70), agility 5m × 10rep (r = -0.68), velocity (r = -0.61)] and in girls [HJ (r = 0.42), agility 5m × 10rep (r = -0.52), velocity (r = -0.20)]. The differences in FFM and physical fitness tests were more pronounced when aligned by APHV than by CA. Conclusion: It was verified that both FFM, CA, and APHV are determinants of physical fitness performance. In addition, the APHV should be introduced in physical education as a means of ranking physical performance among schoolchildren.

Abstract Hydrocotyle umbellata L., Araliaceae, is a species that is recommended in Ayurvedic medicine for its effects on the central nervous system, such as anxiolytic and memory-stimulant effects. Despite the medicinal potential of this species, its phytopharmaceutical and technological potential to produce standardized extracts has not been investigated. This study analyzes the influence of spray drying parameters on the contents of the chemical markers (total phenolic, total flavonoid, and hibalactone) and the functional properties of H. umbellata extract. The optimization of drying conditions was performed using a central composite design combined with response surface methodology and desirability function approach. The mathematical models fitted to experimental data indicated that all the evaluated drying parameters significantly influenced the chemical contents. The optimal conditions were: inlet temperature of 120 °C, feed flow rate of 4 mL min-1, and colloidal silicon dioxide:maltodextrin ratio of 16%:4%. Under these conditions, the powder samples had spherical particles and desirable physicochemical and functional properties, such as low water activity and moisture content, good product recovery, reconstitution, and flowability. Thus, spray drying might be a promising technique for processing standardized H. umbellata extracts.

Connaraceae is a plant family usually used in traditional medicine. Recently, we have reported the chemical composition and biological activity of 39 species around the world. This work highlights the application of these species to diabetes control, which has been reported at least to half of this family. In this line, there are several species of the Connaraceae without scientific evidence of traditional use, among these the four species study here. Thus, this work aims the chemical composition prospection of Connarus blanchetii, Connarus regnellii, Connarus suberosus and Rourea glazioui, as well as the identification of possible bioactivity. The leaves of these four species were collected, prepared and submitted to different extractive methods (aqueous decoction and infusion and ethanolic maceration), these being tested in a preliminary phytochemical investigation and subjected to antioxidant assay with 2,2-diphenyl-1-picrylhydrazyl and ferric reducing antioxidant power. Then, we selected the maceration extract to continue for fractions, with the ethyl acetate and n-butanol presenting the best results with antioxidant and anti-advanced glycation end products (AGEs) activity potential. Finally, the chemical composition was determinate by high-resolution mass spectrometry (HRMS) in combination with Global Natural Products Service (GNPS) data bank. As a result, this study indicated the presence of 29 phenolics compounds with reported activity as antioxidant and with potential of protein glycation, supported the diabetes activity of this plants. The n-butanolic fraction of R. glazioui showed compounds such as apigenin, kaempferol, quercetin, myricetin and chlorogenic acid, which have generated a half maximal inhibitory concentration (IC50) of 36.5 µg mL 1 for oxidative glycation inhibition, being highlighted as the most active.

Resumen. El tiburón sedoso Carcharhinus falciformis es una de las especies de tiburón más importantes en las capturas de la pesquería de pequeña escala de la costa mexicana del Pacífico. En el presente trabajo reportamos los principales componentes alimentarios de C. falciformis capturados por la flota pesquera de pequeña escala en Acapulco, Guerrero, México, de acuerdo con la talla (neonatos y juveniles) y el sexo (machos y hembras), así como la amplitud de su nicho y nivel trófico. Se recolectaron en total 165 estómagos de C. falciformis, de los cuales 57 presentaron contenido estomacal y 108 se encontraron vacíos. Se identificaron en total 11 componentes alimentarios: 4 crustáceos, 3 peces, 2 cefalópodos, 1 tortuga marina y 1 ave. De acuerdo con el índice de importancia relativa especifica de la presa (%IIREP), el grupo de los crustáceos representó el 77.17% de la dieta. El cangrejo Portunus xantusii fue el componente más importante de la dieta de neonatos, juveniles, machos y hembras. Las dietas entre neonatos y juveniles y entre machos y hembras no presentaron grandes diferencias (índice de Morisita-Horn: 0.86 y 0.80, respectivamente). El tiburón sedoso presentó una amplitud de nicho estrecha en todas las categorías, lo que indicó una estrategia de alimentación de tipo especialista (índice de Levin <0.60). De acuerdo con el modelo de Amundsen, C. falciformis mostró una preferencia por P. xantusii. El nivel trófico determinado para C. falciformis indicó que es un depredador secundario.

Abstract. The silky shark, Carcharhinus falciformis, is one of the most important shark species in the small-scale fishery landings on the Pacific coast of Mexico. In the present work, we report the main food components of C. falciformis caught by small-scale fisheries in Acapulco, Guerrero, Mexico, according to size (neonates and juveniles) and sex (males and females), as well as reporting the niche breadth and trophic level. A total of 165 stomachs of C. falciformis were collected, of which 57 contained food and 108 were empty. A total of 11 prey groups were identified: 4 crustaceans, 3 fish, 2 cephalopods, 1 sea turtle, and 1 bird. According to the prey-specific index of relative importance (%PSIRI), crustaceans accounted for 77.17% of the diet. The crab Portunus xantusii was the most important component of the diet of neonates, juveniles, males, and females. The diets did not show great differences between neonates and juveniles and between males and females (Morisita-Horn index: 0.86 and 0.80, respectively). The silky shark presented a narrow niche breadth in all categories, indicating a specialist-type feeding strategy (Levin’s index <0.60). According to Amundsen’s model, C. falciformis showed preference for P. xantusii. The trophic level determined for C. falciformis indicated that it is a secondary predator.

Abstract: We address some predictions derived from the biogeographic model proposed by Luis Borrero (1989-90) in order to analyze the earliest human occupations identified on the north coast of the Beagle Channel (8600-6500 cal. years BP). We examined the chronological, stratigraphic and archaeological information from the basal deposits of Imiwaia I, Binushmuka I and Túnel I to explore the formational processes and temporal structure of the archaeological contexts during the maximum extension of the Holocene marine transgression. Continuous human activity is inferred from 8600 cal. years BP on the southern coast of Tierra del Fuego. The structures of the earliest archaeological deposits indicate low-intensity and discrete occupations over time. The archaeological signal remains weak until the Holocene marine transgression reached its maximum level at approximately 7000 cal. years BP, when the formation of multicomponent archaeological deposits (shell middens) becomes visible in the region. Lastly, we discuss the implications of this information for the colonization of the Fuegian archipelago.

Resumen: Partimos de algunas predicciones derivadas del modelo biogeográfico propuesto por Luis Borrero (1989-90) para analizar las ocupaciones humanas más tempranas identificadas en la costa norte del Canal Beagle (8600-6500 años cal. AP). Examinamos la información cronológica, estratigráfica y la composición de los depósitos basales de Imiwaia I, Binushmuka I y Túnel I con el fin de explorar los procesos de formación y estructura temporal de los contextos arqueológicos en momentos previos al máximo alcance de la transgresión marina del Holoceno. Se observa una actividad humana continua en la costa sur de Tierra del Fuego desde hace al menos 8600 años cal. AP. Las estructuras de los depósitos más tempranos marcan ocupaciones con baja intensidad y discretas en el tiempo. La señal arqueológica se mantiene débil hasta que la transgresión marina del Holoceno alcanzó su máximo nivel en un momento cercano a los 7000 años cal. AP, cuando la formación de depósitos arqueológicos multicomponentes (concheros) comenzó a hacerse visible en la región. Por último, se discuten las implicaciones de esta información para el poblamiento del archipiélago fueguino.

Abstract Background: In laparoscopic surgery, image quality can be severely degraded by surgical smoke caused by the use of tissue dissection tools that reduce the visibility of the observed organs and tissues. Objective: Improve visibility in laparoscopic surgery by combining image processing techniques based on classical methods and artificial intelligence. Method: Development of a hybrid approach to eliminating the effects of surgical smoke, based on the combination of the dark channel prior (DCP) method and a pixel-to-pixel neural network architecture known as a generative adversarial network (GAN). Results: Experimental results have shown that the proposed method achieves better performance than individual DCP and GAN results in terms of restoration quality, obtaining (according to PSNR and SSIM index metrics) better results than some related state-of-the-art methods. Conclusions: The proposed approach decreases the risks and time of laparoscopic surgery because once the network is trained, the system can improve real-time visibility.

Resumen Antecedentes: Durante la cirugía laparoscópica, la calidad de la imagen puede verse gravemente degradada por el humo quirúrgico causado por el uso de herramientas de disección de tejidos que reducen la visibilidad de los órganos y tejidos. Objetivo: Mejorar la visibilidad en cirugía laparoscópica mediante la combinación de técnicas de procesamiento de imágenes basadas en técnicas clásicas e inteligencia artificial. Método: Desarrollo de un enfoque híbrido para la eliminación de los efectos del humo quirúrgico, basado en la combinación del método del principio del canal oscuro (DCP, dark channel prior) y una arquitectura de red neuronal píxel a píxel conocida como red antagónica generativa (GAN, generative adversial network). Resultados: Los resultados experimentales han demostrado que el método propuesto logra un mejor rendimiento que los resultados individuales de DCP y GAN en cuanto a calidad de la restauración, obteniendo (según las métricas de la proporción máxima de señal a ruido [PSNR, Peak Signal-to-Noise Ratio] y el índice de similitud estructural [SSIM, Structural Similarity Index]) mejores resultados que otros métodos relacionados. Conclusiones: El enfoque propuesto disminuye los riesgos y el tiempo de la cirugía laparoscópica, ya que una vez que la red está correctamente entrenada, el sistema puede mejorar la visibilidad en tiempo real.

Abstract: The worldwide pandemic of COVID-19 caused by SARS-CoV-2 has made it necessary to search for treatment alternatives. The WHO has recommended the FDA-approved drug Remdesivir targeting viral RNA polymerase. Additionally, natural compounds with antiviral properties have been computationally evaluated. However, these studies focus on using the AutoDock Vina (ADV) algorithm scoring function to predict candidates. We propose to evaluate the phytochemicals Piperina_ID_638024, EPGG_ID_65064, Curcumina_ID_969516, and Capsaicina_ID_1548943 against the RNA polymerase of SARS-CoV-2 (PDB_ID_6NUR), using Remdesivir_ID_121304016 as control, through computational, comparative and multivariate analysis of the scoring functions ADV, PLANTS, MolDock, Rerank and DockT considering the solubility of ligands and hydrophobicity of the cavities involved in the interactions, to increase the precision in predicting the best docking of natural compounds in front of COVID-19. We found that 4/5 of the scoring functions except ADV predicted that the most thermodynamically favorable docking occurs with Piperine, outperforming Remdesivir. We also observe that the scores of the PLANTS, ADV and DockT functions are affected by the solubility of the ligand and the hydrophobicity of cavities. Therefore, under the conditions of this study, we conclude by proposing the MolDock and Rerank algorithms for rapid screening and reorganization of couplings, respectively, when working with soluble ligands (Rp=0.70), regardless of their polarity, and targeting hydrophobic cavities (Rp=0.95 and Rp=0.90, respectively) of the SARS-CoV-2 RNA polymerase, especially for computational approaches in the context of drug research versus COVID-19.

Resumen: La pandemia mundial del COVID-19 causada por el SARS-CoV-2 ha hecho necesario buscar alternativas de tratamiento. La OMS ha recomendado el fármaco aprobado por la FDA Remdesivir dirigido a la RNA polimerasa viral. Adicionalmente, se han evaluado computacionalmente compuestos naturales con propiedades antivirales. Sin embargo, estos estudios se centran en el uso de la función de puntuación del algoritmo AutoDock Vina (ADV) para predecir los candidatos. Aquí proponemos evaluar los fitoquímicos Piperina_ID_638024, EPGG_ID_65064, Curcumina_ID_969516, y Capsaicina_ID_1548943 frente a la RNA polimerasa del SARS-CoV-2 (PDB_ID_6NUR), usando Remdesivir_ID_121304016 como control, mediante análisis computacional, comparativo y multivariado de las funciones de puntuación ADV, PLANTS, MolDock, Rerank y DockT considerando la solubilidad de ligandos e hidrofobicidad de las cavidades implicadas en las interacciones, para aumentar la precisión en la predicción de los mejores acoplamientos de los compuestos naturales frente al COVID-19. Encontramos que 4/5 de las funciones de puntuación exceptuando ADV predijeron el acoplamiento termodinámicamente más favorable con Piperina, superando a Remdesivir. También observamos que las calificaciones de PLANTS, ADV y DockT se afectan por la solubilidad del ligando e hidrofobicidad de cavidades. Bajo las condiciones de este estudio concluimos que los algoritmos MolDock y Rerank son más adecuados para el cribado rápido y la reorganización de acoplamientos, cuando se trabaje con ligandos solubles (Rp = 0.70 para ambos), indistintamente de su polaridad, y dirigidos a cavidades hidrofóbicas de la RNA polimerasa del SARS-CoV-2 (Rp = 0.95 y Rp = 0.90, respectivamente), especialmente para los enfoques computacionales en el contexto de la investigación de fármacos frente al COVID-19.

SUMMARY: Angiogenesis is the process by which new blood vessels are formed from other existing ones. A balance between proangiogenic factors and anti-angiogenic factors within the microenvironment must exist for the process to be carried out correctly. Similarly, the existence of natural chemicals such as polyphenols, which are capable of being acquired in the diet, induce these factors in the angiogenic process. Polyphenols were administered in the methylcellulose filters on the of chorioallantoic membrane of White Leghorn eggs, maintaining the normal posterior development of the fetus. 15 chicken fetuses were fixed in buffered formalin, obtaining the hearts to histological processing, performing histological, histochemical and immunohistochemical techniques. VEGF levels and the ability of the blood vessels growing under the stimulation of the polyphenols were evaluated. Immunoreactivity was quantified by Image J. The results indicate that caffeic acid and pinocembrin decreased microvascular density and VEGF expression in hearts stimulated with these polyphenols. Both the caffeic and pinocembrin acids play an inhibitory role in the physiological angiogenesis process in the chicken heart, which decrease the microvascular density and could act by modulating the signaling pathways mediated by the VEGFR or by modulating the availability of VEGF. The use of these polyphenols could be useful in studies of other tissues associated with pathological angiogenesis.

RESUMEN: La angiogénesis es el proceso por el cual se forman nuevos vasos sanguíneos a partir de otros ya existentes. Para que esto se lleve a cabo de forma correcta debe existir un balance entre los factores proangiogénicos y los factores antiangiogénicos dentro del microambiente tisular. Por otra parte, la existencia de productos químicos naturales como los polifenoles, que son capaces de adquirirse en la dieta, inducen a estos factores a intervenir en el proceso de angiogénesis. Se administraron los polifenoles en filtros de metilcelulosa sobre la membrana alantocoriónica de huevos White Leghorn, manteniendo el posterior desarrollo normal del feto. Se utilizaron 15 fetos de pollo fijados en formalina tamponada, a los cuales se extrajo el corazón. El procesamiento de las muestras de corazón se realizó a través de técnicas histológicas, histoquímicas e inmunohistoquímica. Finalmente se evaluó la presencia del VEGF y la capacidad de formar vasos sanguíneos bajo el tratamiento con los polifenoles. La inmunorreactividad fue cuantificada mediante Image J®. Los resultados indican que Ácido cafeico y Pinocembrina disminuyen la densidad microvascular y la expresión de VEGF en corazones de fetos de pollo tratados con estos polifenoles. Tanto el Ácido Cafeico como la Pinocembrina cumplen un rol inhibitorio en el proceso de angiogénesis fisiológica en corazón de pollo, pudiendo modular las vías de señalización mediadas por los VEGFR o modulando la disponibilidad de VEGF. Estos polifenoles podrían utilizarse para el estudio de otros tejidos asociados a angiogénesis patológica.

Resumen Este artículo se centra en la turbulencia política-constitucional que Donald J. Trump ha venido generando desde que fue elegido presidente de Estados Unidos. Por un lado, nos referimos a los principales litigios y problemas políticos como señales de la actual crisis constitucional en Estados Unidos y sus consecuencias a nivel nacional e internacional. Por otro lado, explicamos los indicadores de la crisis constitucional en dos niveles de análisis. El primero considera los ataques de Trump contra las libertades constitucionales de los estadounidenses, mientras que el segundo estudia el debilitamiento del derecho internacional consuetudinario provocado por las políticas de Trump. El artículo revela evidencia empírica de todos los asuntos mencionados anteriormente para explicar el incremento de litigios ante los tribunales de distrito y la propia Corte Suprema de Estados Unidos. Por consiguiente, este trabajo señala que el rol del poder judicial será un factor clave para detener los ataques de Trump en contra del Estado de Derecho en Estados Unidos.

Abstract This paper focuses on the constitutional-political turbulence that Donald J. Trump has been creating since he was elected president of the United States. On the one hand, we point out the mainstream of litigious and political issues as signals of an on-going constitutional crisis in the United States and its consequences, both in the national and international scenarios. On the other, we explain the indicators of the constitutional crisis within two levels of analysis. The first one considers Trump’s multiple attacks against American constitutional liberties, while the second one studies the cases of Trump’s policies against customary international law. The paper unveils empirical evidence of all the above-mentioned issues in order to explain the increasing constitutional litigation in District Courts and the Supreme Court of the United States. Therefore, the article puts the finger on the role of the judiciary as a key factor in blocking Trump’s policies and his pervasive attacks on the American rule of law.

Resumo Este artigo concentra-se na turbulência político-constitucional que Donald J. Trump vem gerando desde que foi eleito presidente dos Estados Unidos. Por um lado, nos referimos aos principais litígios e problemas políticos como sinais da atual crise constitucional nos Estados Unidos e suas consequências no nível nacional e internacional. Por outro lado, explicamos os indicadores da crise constitucional em dois níveis de análise. O primeiro considera os ataques de Trump às liberdades constitucionais dos americanos, enquanto o segundo estuda o enfraquecimento do direito internacional consuetudinário causado pelas políticas de Trump. O artigo revela evidências empíricas de todas as questões acima mencionadas para explicar o aumento de litígios perante os tribunais distritais e a própria Suprema Corte dos Estados Unidos. Portanto, este trabalho indica que o papel do judiciário será um fator-chave para deter os ataques de Trump contra o estado de direito nos Estados Unidos.

ABSTRACT Takotsubo syndrome was first described in Japan in late 1989 and early 1990 and featured a group of patients with transient apical dyskinesia and preservation of basal left-ventricular contractility, which resembled the vessel used by fishermen to catch octopus. It is usually seen in situations involving catecholamine release and its clinical picture mimics that of acute myocardial infarction. The case presented occurs, surprisingly, in the postoperative period of cardiovascular surgery and is probably the first one found in this circumstance in Cuba: perioperative ischemia after mitral valve replacement, which recovered within nearly 72 hours, although complete recovery occurred later. Angiographic images are shown.

RESUMEN El síndrome de Takotsubo, descrito por vez primera en Japón a fines de 1989 y principios de 1990, caracteriza un conjunto de pacientes con discinesia apical transitoria y conservación de la contractilidad hacia la base del ventrículo izquierdo, que recuerda la forma de la vasija usada por los pescadores para atrapar pulpos. Habitualmente se observa en situaciones que impliquen elevación de catecolaminas y su cuadro clínico es similar al de un infarto agudo de miocardio. El caso que se presenta ocurrió, sorprendentemente, en el postoperatorio de una cirugía cardiovascular y probablemente sea el primero encontrado en Cuba en esta circunstancia: isquemia perioperatoria tras reemplazo valvular mitral, que se recuperó en aproximadamente 72 horas, aunque su recuperación total fue más tardía. Se muestran las imágenes angiográficas.

Resumen Antecedentes: La dilatación neumática se considera la primera línea de tratamiento no quirúrgico para pacientes con acalasia esofágica. Es una técnica segura y eficaz y actúa gracias a la ruptura de las fibras musculares del esfínter esofágico inferior mediante la aplicación de fuerza radial. Objetivo: Evaluar la eficacia de la dilatación con balón neumático en pacientes con acalasia esofágica a tres años de seguimiento en un centro de tercer nivel. Materiales y métodos: Estudio retrospectivo que incluyó pacientes con acalasia esofágica que recibieron dilatación con balón neumático. El éxito clínico se evaluó con la escala de Eckardt pre-dilatación y post-dilatación a 3, 6 y 12 meses, respectivamente; analizados con prueba de Wilcoxon. Además, se evaluó el éxito técnico, eventos adversos, y necesidad de re-dilatación a 2 años de seguimiento. Los datos se obtuvieron del expediente clínico y contacto vía telefónica; y se analizaron en SPSSv21; p<0.05. Resultados: Se incluyeron 17 pacientes; tres fueron excluidos por antecedentes de miotomía quirúrgica. De los 14 pacientes analizados, la mediana de edad fue 49.5 (RIC 34-65) años; 11/14 (78.6%) fueron mujeres. Según el tipo de acalasia: 1/14 (7.1%) fue Tipo I; 12/14 (85.8%) Tipo II; y 1/14 (7.1%) Tipo III. La duración de los síntomas al diagnóstico fue 10.5 RIC 8.2-27 meses. El IRP pre y post dilatación fue 19.2 RIC 17.9-34.2 y 11.7 RIC 6.9-13.2, respectivamente. La mediana de PSI para la primera dilatación fue 8 RIC 7.7-9. La mediana de seguimiento fue 19 RIC 13.5-25 meses. Solo 2 pacientes requirieron segunda dilatación a 6 y 12 meses, respectivamente. El éxito técnico en la primera y segunda dilatación fue de 100%, respectivamente. El éxito clínico fue 100% a 3 meses, 92.8% a 6 meses, y 85.7% a 12 y 24 meses con un puntaje de Eckardt pre-dilatación de 11 RIC 8.7-12 puntos, y post-dilatación a 3, 6 y 12 meses de 3 (RIC 2-4, p 0.001), 4 (RIC 3-4, p 0.001), y 4 (RIC 4-5 p 0.001), respectivamente. No hubo efectos adversos significativos. Conclusiones: La dilatación endoscópica con balón neumático para el tratamiento de acalasia es un procedimiento eficaz y de bajo riesgo con tasas de éxito clínico superiores al 85% a mediano y largo plazo. Es una excelente alternativa cuando la miotomía quirúrgica o endoscópica está contraindicada.

Starch is an alternative for developing biodegradable films that reduces the problem of environmental pollution generated by synthetic plastics. Safe materials with this characteristics must looked after, with similar properties to those of common plastics and, rather recently, they should have additional functionality. Therefore, the objective of this study was to produce films with oxidized starches of banana and oats with added betalains. The experimental design was completely random with four treatments. A one way ANOVA was carried out and means comparison was assessed with the Tukey statistical test (p≤0.05). The oxidized banana starch films presented higher tensile strenght values (5.7 MPa) and elastic modulus (7.2 MPa), but its percentage of elongation was lower (25 %) compared with the films of oxidized oat starch (1.6 MPa in stress fracture, 0.2 MPa modulus of elasticity, and 127 % elongation). The addition of betalains increased the elongation percentage in the starch films without affecting other mechanical properties, and further increased the permeability to water vapor. The x-rays diffraction showed a possible interaction between the oxidized banana starch and the betalain. Chemical modification by oxidation of the starches conferred physicochemical and functional properties to the films for packaging materials and polymeric matrices use via the incorporation of betalains.

El almidón es una alternativa para elaborar películas biodegradables y reducir el problema de contaminación ambiental generada por los plásticos sintéticos. Para ello se buscan materiales inocuos, con propiedades similares a los plásticos comunes y, de manera reciente, que tengan una funcionalidad adicional. Por ello, el objetivo de este estudio fue elaborar películas con almidones oxidados de plátano y avena adicionadas con betalaínas. El diseño experimental fue completamente aleatorizado con cuatro tratamientos y se realizó ANDEVA de una vía con una prueba de comparación de medias de Tukey (p≤0.05). Las películas de almidón oxidado de plátano presentaron valores mayores de tensión a la fractura (5.7 MPa) y módulo de elasticidad (7.2 MPa), pero su porcentaje de elongación fue menor (25 %) en comparación con las películas de almidón oxidado de avena (1.6 MPa en tensión a la fractura, 0.2 MPa módulo de elasticidad, y 127 % de elongación). La adición de betalaínas aumentó el porcentaje de elongación en las películas de almidón sin afectar otras propiedades mecánicas, y además aumentó la permeabilidad al vapor de agua. La difracción de rayos X mostró una posible interacción entre el almidón oxidado de plátano y la betalaína. La modificación química por oxidación de los almidones de plátano y avena confirió propiedades fisicoquímicas y funcionales a las películas para usarse como materiales de empaque y como matrices poliméricas para la incorporación de betalaínas.
