The role of probiotics in the immune response and intestinal microbiota of children with celiac disease: a systematic review

RESUMO Objetivo: Avaliar mudanças na resposta imunológica (diminuição de citocinas pró-inflamatórias no sangue) e na microbiota fecal (principalmente Bacteroidetes e Firmicutes) após a administração de probióticos em crianças com doença celíaca que seguiam uma dieta livre de glúten. Fontes de dados: Foram utilizadas para esta revisão as bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Springer e Scientific Electronic Library Online (SciELO), com os descritores “celiac disease AND probiotics”. No fim da busca, foram recuperados 168 artigos, dos quais quatro foram incluídos na síntese qualitativa final. Os critérios de inclusão foram ensaios clínicos randomizados e população pediátrica (1–19 anos) e os critérios de exclusão, intervenções que não fossem com probióticos, estudos com pacientes portadores de outras doenças associadas à doença celíaca, ou que não preenchessem critérios para o diagnóstico. Todos os estudos eleitos foram publicados até setembro de 2020, sem restrição de idioma, com pacientes recebendo cepas de Bifidobacterium breve ou B. longum e em uma dieta livre de glúten. Síntese dos dados: Os estudos demonstram que a administração de probióticos associada a uma dieta livre de glúten pode aproximar a microbiota fecal desses pacientes às condições típicas de um indivíduo saudável ao restaurar a abundância de algumas comunidades microbianas que caracterizam uma condição tipicamente fisiológica. Além disso, a administração de probióticos pode reduzir as citocinas pró-inflamatórias (principalmente TNF-alfa). Conclusões: Apesar da correlação positiva entre probióticos e microbiota fecal/marcadores sorológicos nos pacientes pediátricos portadores de doença celíaca, ressalta-se a necessidade de estudos multicêntricos futuros que abranjam um maior número de pacientes e maior tempo de acompanhamento.%
ABSTRACT Objective: To evaluate changes in peripheral immunological response (decrease in blood proinflammatory cytokines) and fecal microbiota (especially Bacteroidetes and Firmicutes) after administration of probiotics in children with celiac disease on a gluten-free diet. Data source: The databases MEDLINE, LILACS, Springer and SciELO were used for this review, with the descriptors “celiac disease AND probiotics”. At the end of the search, 168 articles were retrieved, four of which were included in the final qualitative synthesis, having as inclusion criteria randomized clinical trials and pediatric population (1–19 years) and, as exclusion criteria, interventions other than probiotics, studies with patients with other diseases associated with celiac disease, or patients who did not meet the diagnostic criteria. All elected studies were published until September 2020, without language restriction, with patients receiving strains of Bifidobacterium breve or B. longum and on a gluten-free diet. Data synthesis: The studies show that the administration of probiotics along with a gluten-free diet, can approximate the fecal microbiota of celiac patients to typical conditions of healthy individuals, by restoring the abundance of some microbial communities that characterize the typical physiological condition. In addition, the administration of probiotics can reduce serum proinflammatory cytokines (mainly TNF-alpha). Conclusions: Despite the positive correlation between probiotics and fecal microbiota/serological markers in pediatric patients with celiac disease, we emphasize the need for future multicentric studies that should include a larger number of patients and a longer follow up period.%

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Radiomic analysis of MRI to Predict Sustained Complete Response after Radiofrequency Ablation in Patients with Hepatocellular Carcinoma - A Pilot Study

OBJECTIVES: To investigate whether quantitative textural features, extracted from pretreatment MRI, can predict sustained complete response to radiofrequency ablation (RFA) in patients with hepatocellular carcinoma (HCC). METHODS: In this IRB-approved study, patients were selected from a maintained six-year database of consecutive patients who underwent both pretreatment MRI imaging with a probable or definitive imaging diagnosis of HCC (LI-RADS 4 or 5) and loco-regional treatment with RFA. An experienced radiologist manually segmented the hepatic nodules in MRI arterial and equilibrium phases to obtain the volume of interest (VOI) for extraction of 107 quantitative textural features, including shape and first- and second-order features. Statistical analysis was performed to evaluate associations between textural features and complete response. RESULTS: The study consisted of 34 patients with 51 treated hepatic nodules. Sustained complete response was achieved by 6 patients (4 with single nodule and 2 with multiple nodules). Of the 107 features from the arterial and equilibrium phases, 20 (18%) and 25 (23%) achieved AUC >0.7, respectively. The three best performing features were found in the equilibrium phase: Dependence Non-Uniformity Normalized and Dependence Variance (both GLDM class, with AUC of 0.78 and 0.76, respectively) and Maximum Probability (GLCM class, AUC of 0.76). CONCLUSIONS: This pilot study demonstrates that a radiomic analysis of pre-treatment MRI might be useful in identifying patients with HCC who are most likely to have a sustained complete response to RFA. Second-order features (GLDM and GLCM) extracted from equilibrium phase obtained highest discriminatory performance.

ABSTRACT The Choosing Wisely Initiative aims to collect statements from medical societies all over the world on medical interventions that result in no benefit to patients, with the potential to cause harm. In this article we present the views of the Diagnostic Laboratory Group at the Brazilian Society of Infectious Diseases (SBI). Ten experts from SBI were asked to list 10 diagnostic tests that were perceived as unnecessary in the field of infectious diseases. After voting for the more relevant topics, a questionnaire was sent to all SBI members, in order to select for the most important items. A total of 482 votes were obtained, and the top 10 results are shown in this manuscript. The Choosing Wisely statements of SBI should facilitate clinical practice by optimizing the use of diagnostic resources in the field of infectious diseases.

RESUMO Introdução: No ano de 2015, foi identificado no Brasil um aumento da prevalência de microcefalia em vinte vezes. Esta malformação foi associada pelo Ministério da Saúde à transmissão vertical do Zika vírus (ZIKV). Método: Investigamos dez lactentes com diagnóstico clínico presumido de microcefalia relacionada à ZIKV, que apresentavam anormalidades oculares, nascidos entre maio e dezembro de 2015. Resultados: Sete mães (70,0%) relataram sintomas (mal-estar, rash e artralgia) durante a gestação, sendo seis (85,7%) no primeiro trimestre. No dia da avaliação nenhuma alteração ocular foi identificada nas mães e elas não relataram sintomas oculares durante a gestação. Sorologia foi negativa para toxoplasmose, rubéola, citomegalovírus, sífilis e vírus da imunodeficiência adquirida (HIV) em todos os lactentes. Os achados oculares incluíram alterações maculares (depósito pigmentar grosseiro e/ou atrofia coriorretiniana) em 15 olhos (75,0%) e alterações do nervo óptico (hipoplasia do disco com sinal do duplo anel, palidez e/ou aumento da escavação papilar) em nove olhos (45,0%). Conclusões: Os pacientes apresentaram segmento anterior normal e importantes e anormalidades maculares e do nervo óptico. Estudos futuros vão avaliar a importância destas alterações visuais.
ABSTRACT Purpose: In 2015, a twenty-fold increase in the prevalence of microcephaly in Brazil was reported, and the Ministry of Health associated this abnormal prevalence with the maternal-fetal Zika virus (ZIKV) transmission. Methods: We assessed the ophthalmological findings of ten mothers and their infants that had been clinically diagnosed with ZIKV-related microcephaly and presented ocular abnormalities, born from May to December 2015. Results: Seven mothers (70.0%) referred symptoms during pregnancy (malaise, rash and arthralgia), of which six (85.7%) were in the first trimester. At the time of exam, no ophthalmological abnormalities were identified in the mothers and they did not report ocular symptoms during pregnancy. Serology was negative in all infants for Toxoplasmosis, Rubella, Cytomegalovirus, Syphilis and Human Immunodeficiency Viruses. Ocular findings included macular alterations (gross pigment mottling and/or chorioretinal atrophy) in fifteen eyes (75.0%), and optic nerve abnormalities (hypoplasia with double-ring sign, pallor, and/or increased cup-to-disk ratio) in nine eyes (45.0%). Conclusions: Patients presented normal anterior segment and important macular and optic nerve abnormalities. Further studies will assess the visual significance of these alterations.

SUMMARY The aim of this study was to investigate the association of EBV and HPV with gingivitis and/or periodontitis according to the immunologic status. To this end, 74 oral biopsies from transplanted and non-transplanted individuals with the abovementioned oral manifestations were submitted to a screening by PCR for both viruses. According to the results, EBV was strongly associated with gingivitis and/or periodontitis in transplanted individuals (p = 0.011) but not HPV (p = 0.766). EBV-HPV co-detections did not enhance the presence of tissue injury as well. Although a causal relationship was not investigated in this study, the higher frequency of these two oncoviruses in lesion tissues must be investigated in follow-up studies, especially among immunocompromised individuals.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Foi avaliada neste estudo a presença de caspase-3-clivada no coração de ratos irradiados durante o declínio da função ventricular. Ratas Wistar foram irradiadas com uma dose única de radiação (15 Gy) diretamente no coração e as análises molecular, histológica e fisiológica foram realizadas treze meses pós a irradiação dos animais. As expressões de procolágeno tipo I, TGF-ß1 e caspase-3-clivada foram analisadas usando Western blotting. Alterações cardíacas estruturais e funcionais foram investigadas por meio de ecocardiografia e microscopia eletrônica. No grupo irradiado, os níveis de procolágeno tipo I, TGF-ß1 e caspase-3-clivada estão aumentados Alterações histológicas importantes (degeneração do tecido cardíaco e deposição de colágeno) e funcionais (fração de ejeção reduzida) foram observadas. Dados sugerem que o declínio da função cardíaca após a exposição à radiação ionizante está relacionado, em parte, com aumento do colágeno e da caspase-3-clivada.
The caspase-3-cleaved presence was evaluated in this study in the heart of irradiated rats, during the decline of ventricular function. Female Wistar rats were irradiated with a single dose of radiation (15 Gy) delivered directly to the heart and the molecular, histological and physiological evaluations were performed at thirteen months post-irradiation. The expressions of procollagen type I, TGF-ß1 and caspase-3-cleaved were analyzed using Western blotting. Cardiac structural and functional alterations were investigated by echocardiography and electron microscopy. In the irradiated group, the levels of procollagen type I, TGF-ß1 and caspase-3-cleaved are increased. Significant histological changes (degeneration of heart tissue and collagen deposition) and functional (reduced ejection fraction) were observed. Data suggest that the cardiac function decline after exposure to ionizing radiation is related, in part, to increased collagen and increased caspase-3-cleaved.

A sensitive spectrophotometric method was developed for sulphate determination in automotive ethanol fuel. The method based on the reaction of the analyte with barium-dibromosulphonazo(III) complex lead to a decrease in the magnitude of the absorbance signals monitored at 649 nm. No sample pretreatment is required and the proposed method allows sulphate determination in the 0.45 - 6.50 mg L-1 range with R.S.D. < 2% and limit of detection of 0.14 mg L-1. The method has been successfully applied for sulphate determination in automotive ethanol fuel and the results agreed with the reference chromatographic method.

Forage mass and structure of piatã grass deferred at different heights and variable periods

The objective of this study was to determine the deferment period and the most adequate initial sward height of Brachiaria brizantha cv. Piatã for deferment in the region of Viçosa, Minas Gerais. For so, the following characteristics were evaluated: masses and bulk density of forage and its morphological components, light interception by the sward, leaf area index, leaf angle, sward and extended tiller height and falling index of the deferred piatã grass. The three deferment periods assessed (65, 85 and 105 days) were randomized to the plots and the three sward heights (20, 30 and 40 cm), randomized in the subplots. The experimental design was of completely randomized blocks, with three replicates, in a subdivided plots arrangement. Masses of forage, green stem and dead tissue were higher in the piatã grass managed with deferment period of 105 days. The mass of green leaf blade was higher in the piatã grass deferred with 20 cm height associated to the deferment period of 85 days, which had, as consequence, greater bulk density of green leaf blade. The interception of light by the sward was greater in the piatã grass kept longer in deferment associated with the higher leaf area index, regardless of its initial height. The heights of the sward and extended tillers were greater in the piatã grass under periods of 85 and 105 days of deferment; however, the falling index was not affected by the factors studied. In the region of Viçosa, Minas Gerais, B. brizantha cv. Piatã fertilized with 75 kg/ha nitrogen and deferred with initial height of 20 cm and for a period of 85 days, from mid April, has adequate productivity and structure for animal production.

Caesalpinia ferrea Mart (Leguminosae) é uma planta medicinal utilizada principalmente no tratamento do diabetes, dentre outras propriedades terapêuticas, mas que também apresenta relatos de hepatotóxicos. Embora apresente em sua constituição substâncias capazes de interferirem na fertilidade, como flavonóides e cumarina, nenhum estudo foi ainda realizado para avaliar os efeitos adversos dessa planta no funcionamento do sistema reprodutor após tratamento de longa duração. Portanto, este trabalho foi desenvolvido com o objetivo de avaliar a utilização segura e os efeitos de C. ferrea nos órgãos vitais, no sistema reprodutor e na produção de espermatozóides de ratos Wistar submetidos a tratamento crônico. Animais imaturos e adultos foram tratados com o extrato aquoso de C. ferrea na dose de 300 mg/kg de peso corporal, administrado durante um ou dois ciclos espermatogênicos dessa espécie. Os órgãos reprodutores e vitais foram analisados e os espermatozóides foram coletados na secreção epididimária proveniente da cauda do epidídimo direito. A administração crônica de C. ferrea não alterou significativamente o peso corporal e nem o peso dos órgãos reprodutores e vitais. A produção de gametas também não foi afetada. Os dados sugerem que a utilização crônica de C. ferrea não interfere com o funcionamento normal do sistema reprodutor do rato Wistar.
Caesalpinia ferrea Mart (Leguminosae) is a medicinal plant used to treat diabetes, among other therapeutic properties, but which is also reported to have hepatotoxic effects. Although it contains substances such as flavonoids and coumarin, which are known to have antifertility activity, no studies have apparently been conducted to evaluate the potential adverse side effects of this plant on the function of the reproductive system after a chronic treatment. Therefore, this investigation was carried out to evaluate the effect and safety of the long-term exposure to C. ferrea on male Wistar rats' vital organs, reproductive system and sperm production. Adult and immature male rats were treated with an aqueous extract of C. ferrea at a dose level of 300 mg/kg of body weight, administered during one or two spermatogenic cycles of this species. The reproductive and vital organs were analyzed, and sperm was collected from the epididymal secretion of the right epididymis cauda. The long-term administration of C. ferrea did not significantly alter the body, vital and reproductive organs weights. Gamete production was not affected either. The chronic assessment of C. ferrea suggests that this plant does not affect the normal functioning of the Wistar rat reproductive system.

Hepatitis B is preventable through immunization and has a higher risk of transmission after a percutaneous accident when compared to hepatitis C and HIV. This study determines the vaccine coverage and hepatitis B serological situation among students of an undergraduate medical program. This is an epidemiological study with quantitative approach. A structured data collection instrument was used. Descriptive analysis and prevalence were computed. The prevalence of accidents involving biological material was 37.2%: 88.6% of the injured students reported they had completed the Hepatitis B vaccine scheme and only 47.5% were submitted to the corresponded serology. The probability of accidents with biological material among undergraduates and insufficient vaccine coverage reinforces the importance of providing guidance on immunization and evaluation of seroconversion for Hepatitis B since exposure to it is a risk factor associated with occupational transmission of Hepatitis B.
A hepatite B é passível de prevenção através de imunização e apresenta maior risco de transmissão, após acidente percutâneo, se comparada a hepatite C e ao HIV. Objetivou-se determinar a cobertura vacinal e a situação sorológica para hepatite B entre alunos de um curso de graduação em medicina. Trata-se de estudo epidemiológico de abordagem quantitativa. Utilizou-se instrumento de coleta de dados estruturado, realizado análise descritiva e cálculo de prevalência. A prevalência de acidentes envolvendo contato com material biológico foi de 37,2%; 88,6% dos alunos acidentados informaram ter completado o esquema de vacinação para hepatite B e apenas 47,5% realizaram a sorologia correspondente. A possibilidade de ocorrência de acidentes com material biológico entre graduandos e cobertura vacinal insuficiente reforça a importância da orientação sobre a imunização e a avaliação da soroconversão para hepatite B, uma vez que esta exposição constitui fator de risco sabidamente associado a transmissão ocupacional da hepatite B.

A nanotecnologia está relacionada às estruturas, propriedades e processos envolvendo materiais com dimensões numa escala nanométrica. O conceito de nanotecnologia em aplicações farmacêuticas, mais especificamente na liberação de fármacos, está baseado na produção de nanopartículas que contêm moléculas do fármaco a serem depositadas exclusivamente no órgão ou tecido alvo com a vantagem de serem inativas. Neste contexto, o presente trabalho busca estudar a utilização de uma organoargila, previamente modificada com octadecilamina, viscogel B8, como matriz de liberação controlada de fármacos por meio da reação de intercalação desta argila com PEG 6000 e MEG, comparando-se os resultados obtidos com um derivado previamente obtido da intercalação desta argila com PVP K-30. Estes materiais têm como diferencial a reunião de diferentes aspectos de funcionalidade, possibilitando se obter excipientes de liberação modificada de propriedades mistas.
Nanotechnology is related to structures, properties and processes involving materials in a nanometer dimension. This is widely used for pharmaceutical applications, more specifically in drugs' release. In this context, the present work studies the use of organophilic montmorillonite (OMMT), viscogel B8, as matrix of controlled release of drugs. New polymer-layered silicate nanocomposites (PLN) of MEG, PEG 6000 and OMMT were prepared and compared with PVP K-30-OMMT.
