ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

ABSTRACT Tree breeders are prioritizing the increase of the interspecific diversity in the breeding programs due to the pronounced environmental changes, and expansion of the forest frontier to less productive areas. In this scenario, there is a growing interest for species of the genus Corymbia due to their good growth, resistance to biotic and abiotic factors, and industrial properties. Also, Corymbia spp. can cross with each other, generating interspecific hybrids. However, there are great difficulties on defining the best crosses. Thus, the objectives of this study were to evaluate the genetic variability of 64 progenies of Corymbia maculata and 64 progenies of Corymbia torelliana and to use the GGE biplot method to point the most appropriated crosses to broad and specific environments in order to generate stable and productive interspecific hybrids of C. maculata and C. torelliana. The GGE biplot was an efficient method for evaluating the genetic variability between progenies, for visualizing the genotype by environment interaction pattern and for identifying the best crosses in order to generate interspecific hybrids of the species. By adopting a selection intensity of 10%, the recommended progenies to generate interspecific hybrids for the Aracruz municipality (ARA) were 10, 40, 49, 64, 20, 9 and 47 of C. maculata and 24, 56, 41, 64, 43, 31 and 12 of C. torelliana. For Três Lagoas municipality (TLA), the recommended progenies were 11, 17, 1, 48, 7, 63 and 21 of C. maculata and 42, 46, 61, 44, 10, 63 and 28 of C. torelliana.

Resumo As cidades brasileiras vêm crescendo de forma desordenada e sofrendo conturbados processos antropogênicos, o que transforma de forma severa a atmosfera local, influenciando na qualidade de vida da sociedade. Esta pesquisa tem por finalidade a análise da variação de temperatura através do uso de sensoriamento remoto, buscando identificar ilhas de calor e frescor, sua origem e a variação espaço-temporal, tendo como área de estudo o município de São Paulo, uma grande cidade com densidade demográfica de 7.398,26 hab/km2 e mais de 12.000.000 de habitantes. Optou-se por utilizar de imagens do satélite Landsat 5 e da metodologia do algoritmo SEBAL para cálculo de temperatura da superfície. Os resultados encontrados evidenciam que a variação de temperatura da superfície vem sofrendo aumento com o passar dos anos, o município de São Paulo, a presença de grandes zonas industriais, o crescimento urbano desordenado, a presença de impermeabilização do solo através de revestimento asfáltico, são alguns dos fatores que contribuem para o surgimento de ilhas de calor. Também ficou evidenciada a presença de zonas de frescor, boa parte delas em áreas arborizadas, o que colabora para a redução da temperatura e melhoramento da condição de vida da população.
Abstract Brazilian cities have been growing in a disorderly manner and undergoing a disturbed anthropogenic process, which severely transforms the local atmosphere, influencing the quality of life of society. The purpose of this research is to analyze the temperature variation through the use of remote sensing, seeking to identify heat and freshness islands, their origins and the space-behavioral variation, having as study area the Municipality of São Paulo, a large city with population density of 7,398.26 inhabitants/km2 and more than 12,000,000 inhabitants. We opted to use Landsat 5 satellite images and SEBAL algorithm methodology for surface temperature calculation. The results show that the surface temperature variation has been increasing over the years, the city of São Paulo, the presence of large industrial zones, the disordered urban growth, the presence of soil sealing through asphalt coating, are some of the factors that contribute to the emergence of heat islands. It was also evidenced the presence of freshness zones, most of them in wooded areas, which contributes to reducing the temperature and improving the living conditions of the population.

Resumen En la región norte de Mato Grosso (Brasil) la distribución de la precipitación afecta principalmente la actividad económica. La deficiencia de lluvias limita la producción agrícola y su exceso, el flujo de productos por vías secundarias. Estos obstáculos imponen la necesidad de un mayor conocimiento de la distribución de lluvias en regiones distantes del Estado, como Paranorte, en Juara-MT. Este estudio tuvo como objetivo identificar el comienzo y el final de la temporada de lluvias, así como la frecuencia e intensidad del verano, e investigar los eventos causales de estas temporadas y sus relaciones con los fenómenos ENOS (El Niño de Oscilación Sur). Para ello, se adoptaron métodos basados en la cantidad y la continuidad de las lluvias. Los resultados mostraron que en Paranorte la precipitación anual acumulada varía entre 2322 y 2720 mm. Los meses del año se dividen en un período seco (entre julio y agosto), dos meses de baja precipitación (mayo y septiembre - ≈ 80 mm) y dos más extensos con altas precipitaciones (entre enero y abril y entre octubre y diciembre, con 214 a 464 mm). Las sequías prolongadas ocurren con mayor frecuencia al comienzo (septiembre) y al final (mayo) de la temporada de lluvias. A su vez, los fenómenos ENOS influyen en la lluvia solo en primavera.
Abstract Rainfall predominantly influences economic activity in northern of Mato Grosso. The scarcity of rainfall limits agricultural production, and its excess hinders the flow of production on back roads. These obstacles impose the need for greater knowledge of rainfall in distant regions of the state, such as Paranorte, in Juara-MT. This study aimed to identify the beginning and end of the rainy season, as well as the frequency and duration of dry days, besides to investigate causal relationships of rainfall with ENSO (El Niño de Oscillation Sul) phenomena. Methods based on rainfall quantity and continuity were adopted. The results showed that in Paranorte, the accumulated annual precipitation varies between 2322 and 2720 mm. The months of the year are divided into a dry period (June, July and August), a period of low rainfall (May and September - ≈ 80 mm) and a period of high rainfall (October, November, December, January, February, March and April, with 214 to 464 mm). Prolonged droughts occur most often at the beginning (September) and end (May) of the rainy season. In turn, ENSO phenomena influence rainfall only in spring.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Abstract The Saanen goat breed has been widely explored in breeding programmes; however, there are few reports about the breed’s genetic and molecular composition. Thus, this study aimed to characterize the proteomic profile of spermatozoa from Saanen breeding goats. Five breeding animals with proven fertility were selected, the spermatozoa were collected, and the protein was extracted. Subsequently, the proteins were separated and analysed by two-dimensional electrophoresis and mass spectrometry; the proteins were then identified with the SwissProt database. A total of 31 proteins involved in reproduction were identified, including binding proteins on spermatozoa for fusion with the egg, acrosomal membrane proteins, metabolic enzymes, heat shock proteins, cytoskeletal proteins and spermatozoa motility proteins. The characterization of such proteins clarifies the molecular mechanisms of spermatogenesis and the modifications that ensure the success of fertilization.

ABSTRACT Traditionally, the treatment of overactive bladder syndrome has been based on the use of oral medications with the purpose of reestablishing the detrusor stability. The recent better understanding of the urothelial physiology fostered conceptual changes, and the oral anticholinergics – pillars of the overactive bladder pharmacotherapy – started to be not only recognized for their properties of inhibiting the detrusor contractile activity, but also their action on the bladder afference, and therefore, on the reduction of the symptoms that constitute the syndrome. Beta-adrenergic agonists, which were recently added to the list of drugs for the treatment of overactive bladder, still wait for a definitive positioning – as either a second-line therapy or an adjuvant to oral anticholinergics. Conservative treatment failure, whether due to unsatisfactory results or the presence of adverse side effects, define it as refractory overactive bladder. In this context, the intravesical injection of botulinum toxin type A emerged as an effective option for the existing gap between the primary measures and more complex procedures such as bladder augmentation. Sacral neuromodulation, described three decades ago, had its indication reinforced in this overactive bladder era. Likewise, the electric stimulation of the tibial nerve is now a minimally invasive alternative to treat those with refractory overactive bladder. The results of the systematic literature review on the oral pharmacological treatment and the treatment of refractory overactive bladder gave rise to this second part of the review article Overactive Bladder – 18 years, prepared during the 1st Latin-American Consultation on Overactive Bladder.

ABSTRACT Abstract: Overactive bladder syndrome is one of the lower urinary tract dysfunctions with the highest number of scientific publications over the past two decades. This shows the growing interest in better understanding this syndrome, which gathers symptoms of urinary urgency and increased daytime and nighttime voiding frequency, with or without urinary incontinence and results in a negative impact on the quality of life of approximately one out of six individuals – including both genders and almost all age groups. The possibility of establishing the diagnosis just from clinical data made patients' access to specialized care easier. Physiotherapy resources have been incorporated into the urological daily practice. A number of more selective antimuscarinic drugs with consequent lower adverse event rates were released. Recently, a new class of oral drugs, beta-adrenergic agonists has become part of the armamentarium for Overactive Bladder. Botulinum toxin injections in the bladder and sacral neuromodulation are routine modalities of treatment for refractory cases. During the 1st Latin-American Consultation on Overactive Bladder, a comprehensive review of the literature related to the evolution of the concept, epidemiology, diagnosis, and management was conducted. This text corresponds to the first part of the review Overactive Bladder 18-years.

RESUMO OBJETIVO Determinar a prevalência de síndrome metabólica e de seus componentes em adolescentes brasileiros. MÉTODOS Foram avaliados 37.504 adolescentes, participantes do Estudo de Riscos Cardiovasculares em Adolescentes (ERICA), estudo transversal de âmbito nacional e de base escolar. Os adolescentes, de 12 a 17 anos de idade, residiam em municípios com mais de 100 mil habitantes. A amostra foi estratificada e conglomerada em escolas e turmas. Os critérios da International Diabetes Federation foram utilizados para definir síndrome metabólica. Prevalências de síndrome metabólica foram estimadas segundo sexo, faixa etária, tipo de escola e estado nutricional. RESULTADOS Dos 37.504 adolescentes, 50,2% eram do sexo feminino; 54,3% tinham de 15 a 17 anos e 73,3% estudavam em escolas públicas. A prevalência nacional de síndrome metabólica foi 2,6% (IC95% 2,3-2,9), discretamente maior no sexo masculino e naqueles de 15 a 17 anos na maioria das macrorregiões. A prevalência foi a maior nos residentes na macrorregião Sul, nas adolescentes mais jovens e nos adolescentes mais velhos. A prevalência foi maior nas escolas públicas (2,8% [IC95% 2,4-3,2]) que nas escolas privadas (1,9% [IC95% 1,4-2,4]) e nos adolescentes obesos em comparação aos não obesos. As combinações de componentes mais frequentes, respondendo por 3/4 das combinações, foram: circunferência de cintura elevada, HDL-colesterol baixo e pressão arterial elevada, seguida de circunferência da cintura elevada, lipoproteína de alta densidade (HDL-c) baixo e triglicerídeos elevados, e wpor circunferência da cintura elevada, HDL baixa e triacilgliceróis e pressão arterial elevados. A HDL baixa foi o segundo componente mais frequente, mas a maior prevalência de síndrome metabólica (26,8%) foi observada na presença de triglicerídeos elevado. CONCLUSÕES O ERICA é o primeiro estudo nacional a apresentar prevalências de síndrome metabólica e descrever a participação dos seus componentes. Apesar de a prevalência da síndrome metabólica ter sido baixa, as altas prevalências de alguns componentes e de participação de outros na composição da síndrome torna importante o diagnóstico precoce de tais alterações, mesmo que não agrupadas na síndrome metabólica.
ABSTRACT OBJECTIVE To determine the prevalence of metabolic syndrome and its components in Brazilian adolescents. METHODS We evaluated 37,504 adolescents who were participants in the Study of Cardiovascular Risks in Adolescents (ERICA), a cross-sectional, school-based, national study. The adolescents, aged from 12 to 17 years, lived in cities with populations greater than 100,000 inhabitants. The sample was stratified and clustered into schools and classes. The criteria set out by the International Diabetes Federation were used to define metabolic syndrome. Prevalences of metabolic syndrome were estimated according to sex, age group, school type and nutritional status. RESULTS Of the 37,504 adolescents who were evaluated: 50.2% were female; 54.3% were aged from 15 to 17 years, and 73.3% were from public schools. The prevalence of metabolic syndrome was 2.6% (95%CI 2.3-2.9), slightly higher in males and in those aged from 15 to 17 years in most macro-regions. The prevalence was the highest in residents from the South macro-region, in the younger female adolescents and in the older male adolescents. The prevalence was higher in public schools (2.8% [95%CI 2.4-3.2]), when compared with private schools (1.9% [95%CI 1.4-2.4]) and higher in obese adolescents when compared with nonobese ones. The most common combinations of components, referring to 3/4 of combinations, were: enlarged waist circumference (WC), low HDL-cholesterol (HDL-c) and high blood pressure; followed by enlarged WC, low HDL-c and high triglycerides; and enlarged WC, low HDL-c, high triglycerides and blood pressure. Low HDL was the second most frequent component, but the highest prevalence of metabolic syndrome (26.8%) was observed in the presence of high triglycerides. CONCLUSIONS ERICA is the first Brazilian nation-wide study to present the prevalence of metabolic syndrome and describe the role of its components. Despite the prevalence of Metabolic Syndrome being low, the high prevalences of some components and participation of others in the syndrome composition shows the importance of early diagnosis of this changes, even if not grouped within the metabolic syndrome.

Considerou-se hipertensão arterial a média da pressão arterial sistólica ou diastólica maior ou igual ao percentil 95 da curva de referência. Foram estimadas prevalências e intervalos de confiança de 95% (IC95%) de hipertensão arterial e de obesidade, nacionais e nas macrorregiões do País, por sexo e grupo etário, assim como as frações de hipertensão atribuíveis à obesidade na população. RESULTADOS Foram avaliados 73.399 estudantes, 55,4% do sexo feminino, com média de idade 14,7 anos (DP = 1,6). A prevalência de hipertensão arterial foi 9,6% (IC95% 9,0-10,3); sendo as mais baixas observadas nas regiões Norte, 8,4% (IC95% 7,7-9,2) e Nordeste, 8,4% (IC95% 7,6-9,2) e a mais alta na região Sul, 12,5% (IC95% 11,0-14,2). A prevalência de obesidade foi 8,4% (IC95% 7,9-8,9), mais baixa na região Norte e mais alta na Sul. As prevalências de hipertensão arterial e obesidade foram maiores no sexo masculino. Adolescentes com obesidade tiveram prevalência de hipertensão arterial mais elevada, 28,4% (IC95% 25,5-31,2), do que aqueles com sobrepeso, 15,4% (IC95% 13,8-17,0), ou eutróficos, 6,3% (IC95% 5,6-7,0). A fração de hipertensão arterial atribuível à obesidade foi de 17,8%. Arterial hypertension was defined when the average systolic or diastolic blood pressure was greater than or equal to the 95th percentile of the reference curve. Prevalences and 95% confidence intervals (95%CI) of arterial hypertension and obesity, both on a national basis and in the macro-regions of Brazil, were estimated by sex and age group, as were the fractions of hypertension attributable to obesity in the population. RESULTS We evaluated 73,399 students, 55.4% female, with an average age of 14.7 years (SD = 1.6). The prevalence of hypertension was 9.6% (95%CI 9.0-10.3); with the lowest being in the North, 8.4% (95%CI 7.7-9.2) and Northeast regions, 8.4% (95%CI 7.6-9.2), and the highest being in the South, 12.5% (95%CI 11.0-14.2). The prevalence of obesity was 8.4% (95%CI 7.9-8.9), which was lower in the North region and higher in the South region. The prevalences of arterial hypertension and obesity were higher in males. Obese adolescents presented a higher prevalence of hypertension, 28.4% (95%CI 25.5-31.2), than overweight adolescents, 15.4% (95%CI 17.0-13.8), or eutrophic adolescents, 6.3% (95%CI 5.6-7.0). The fraction of hypertension attributable to obesity was 17.8%. CONCLUSIONS ERICA was the first nationally representative Brazilian study providing prevalence estimates of hypertension in adolescents. Regional and sex differences were observed. The study indicates that the control of obesity would lower the prevalence of hypertension among Brazilian adolescents by 1/5.

Prevalences and 95% confidence intervals (95%CI) of arterial hypertension and obesity, both on a national basis and in the macro-regions of Brazil, were estimated by sex and age group, as were the fractions of hypertension attributable to obesity in the population. RESULTS We evaluated 73,399 students, 55.4% female, with an average age of 14.7 years (SD = 1.6). The prevalence of hypertension was 9.6% (95%CI 9.0-10.3); with the lowest being in the North, 8.4% (95%CI 7.7-9.2) and Northeast regions, 8.4% (95%CI 7.6-9.2), and the highest being in the South, 12.5% (95%CI 11.0-14.2). The prevalence of obesity was 8.4% (95%CI 7.9-8.9), which was lower in the North region and higher in the South region. The prevalences of arterial hypertension and obesity were higher in males. Obese adolescents presented a higher prevalence of hypertension, 28.4% (95%CI 25.5-31.2), than overweight adolescents, 15.4% (95%CI 17.0-13.8), or eutrophic adolescents, 6.3% (95%CI 5.6-7.0). The fraction of hypertension attributable to obesity was 17.8%. CONCLUSIONS ERICA was the first nationally representative Brazilian study providing prevalence estimates of hypertension in adolescents. Regional and sex differences were observed. The study indicates that the control of obesity would lower the prevalence of hypertension among Brazilian adolescents by 1/5.

RESUMO OBJETIVO Determinar a prevalência de síndrome metabólica e de seus componentes em adolescentes brasileiros. MÉTODOS Foram avaliados 37.504 adolescentes, participantes do Estudo de Riscos Cardiovasculares em Adolescentes (ERICA), estudo transversal de âmbito nacional e de base escolar. Os adolescentes, de 12 a 17 anos de idade, residiam em municípios com mais de 100 mil habitantes. A amostra foi estratificada e conglomerada em escolas e turmas. Os critérios da International Diabetes Federation foram utilizados para definir síndrome metabólica. Prevalências de síndrome metabólica foram estimadas segundo sexo, faixa etária, tipo de escola e estado nutricional. RESULTADOS Dos 37.504 adolescentes, 50,2% eram do sexo feminino; 54,3% tinham de 15 a 17 anos e 73,3% estudavam em escolas públicas. A prevalência nacional de síndrome metabólica foi 2,6% (IC95% 2,3-2,9), discretamente maior no sexo masculino e naqueles de 15 a 17 anos na maioria das macrorregiões. A prevalência foi a maior nos residentes na macrorregião Sul, nas adolescentes mais jovens e nos adolescentes mais velhos. A prevalência foi maior nas escolas públicas (2,8% [IC95% 2,4-3,2]) que nas escolas privadas (1,9% [IC95% 1,4-2,4]) e nos adolescentes obesos em comparação aos não obesos. As combinações de componentes mais frequentes, respondendo por 3/4 das combinações, foram: circunferência de cintura elevada, HDL-colesterol baixo e pressão arterial elevada, seguida de circunferência da cintura elevada, lipoproteína de alta densidade (HDL-c) baixo e triglicerídeos elevados, e wpor circunferência da cintura elevada, HDL baixa e triacilgliceróis e pressão arterial elevados. A HDL baixa foi o segundo componente mais frequente, mas a maior prevalência de síndrome metabólica (26,8%) foi observada na presença de triglicerídeos elevado. CONCLUSÕES O ERICA é o primeiro estudo nacional a apresentar prevalências de síndrome metabólica e descrever a participação dos seus componentes. Apesar de a prevalência da síndrome metabólica ter sido baixa, as altas prevalências de alguns componentes e de participação de outros na composição da síndrome torna importante o diagnóstico precoce de tais alterações, mesmo que não agrupadas na síndrome metabólica.
ABSTRACT OBJECTIVE To determine the prevalence of metabolic syndrome and its components in Brazilian adolescents. METHODS We evaluated 37,504 adolescents who were participants in the Study of Cardiovascular Risks in Adolescents (ERICA), a cross-sectional, school-based, national study. The adolescents, aged from 12 to 17 years, lived in cities with populations greater than 100,000 inhabitants. The sample was stratified and clustered into schools and classes. The criteria set out by the International Diabetes Federation were used to define metabolic syndrome. Prevalences of metabolic syndrome were estimated according to sex, age group, school type and nutritional status. RESULTS Of the 37,504 adolescents who were evaluated: 50.2% were female; 54.3% were aged from 15 to 17 years, and 73.3% were from public schools. The prevalence of metabolic syndrome was 2.6% (95%CI 2.3-2.9), slightly higher in males and in those aged from 15 to 17 years in most macro-regions. The prevalence was the highest in residents from the South macro-region, in the younger female adolescents and in the older male adolescents. The prevalence was higher in public schools (2.8% [95%CI 2.4-3.2]), when compared with private schools (1.9% [95%CI 1.4-2.4]) and higher in obese adolescents when compared with nonobese ones. The most common combinations of components, referring to 3/4 of combinations, were: enlarged waist circumference (WC), low HDL-cholesterol (HDL-c) and high blood pressure; followed by enlarged WC, low HDL-c and high triglycerides; and enlarged WC, low HDL-c, high triglycerides and blood pressure. Low HDL was the second most frequent component, but the highest prevalence of metabolic syndrome (26.8%) was observed in the presence of high triglycerides. CONCLUSIONS ERICA is the first Brazilian nation-wide study to present the prevalence of metabolic syndrome and describe the role of its components. Despite the prevalence of Metabolic Syndrome being low, the high prevalences of some components and participation of others in the syndrome composition shows the importance of early diagnosis of this changes, even if not grouped within the metabolic syndrome.

Alignment of the tibial component in total knee arthroplasty procedures using an intramedullary or extramedullary guide: double-blind randomized prospective study

OBJETIVOS: Avaliar os resultados obtidos com o uso de guia intramedular ou extramedular para o corte tibial em artroplastias totais do joelho, com vistas a identificar sua acurácia e a superioridade de um em relação ao outro.MÉTODOS: Estudo prospectivo, randomizado, duplo cego de 41 artroplastias totais de joelho feitas entre agosto de 2011 e março de 2012. Foi medido o ângulo entre a base do componente tibial e o eixo mecânico da tíbia no período pós-operatório imediato por meio de radiografia em incidência anteroposterior da tíbia que englobou joelho e tornozelo.RESULTADOS: Não houve diferença demográfica entre os dois grupos avaliados. O alinhamento médio do componente tibial nos pacientes do grupo A (intramedular) foi de 90,3° (84°-97°). No grupo B (extramedular), foi de 88,5° (83°-94°).CONCLUSÃO: Não encontramos, em nosso estudo, diferença quanto à precisão ou acurácia de qualquer um dos guias. Alguns pacientes apresentam contraindicação, absoluta ou relativa, para o uso de um ou outro guia. Todavia, para os demais casos, não há superioridade de algum deles.
OBJECTIVES: To evaluate the results obtained through using an intramedullary or extramedullary guide for sectioning the tibia in total knee arthroplasty procedures, with a view to identifying the accuracy of these guides and whether one might be superior to the other.METHODS: This was a randomized double-blind prospective study on 41 total knee arthroplasty procedures performed between August 2011 and March 2012. The angle between the base of the tibial component and the mechanical axis of the tibia was measured during the immediate postoperative period by means of radiography in anteroposterior view on the tibia that encompassed the knee and ankle.RESULTS: There was no demographic difference between the two groups evaluated. The mean alignment of the tibial component in the patients of group A (intramedullary) was 90.3° (range: 84-97°). In group B (extramedullary), it was 88.5° (range: 83-94°).CONCLUSION: In our study, we did not find any difference regarding the precision or accuracy of either of the guides. Some patients present an absolute or relative contraindication against using one or other of the guides. However, for the other cases, neither of the guides was superior to the other one.

Experimental and numerical analysis of foundation pilings partially embedded in rock

Analisa-se o comportamento de quatro estacas-raiz (ϕ=0,41m) executadas na cidade de Foz do Iguaçu/PR, as quais foram submetidas a provas de carga com carregamento lento. Os resultados foram confrontados com aqueles obtidos por meio de modelagem numérica tridimensional pelo método dos elementos finitos, o qual permite simular o comportamento elásto-plástico do solo. O subsolo local apresenta variada estratigrafia, composto por uma camada superficial de solo residual, seguida por alteração de rocha e rocha sã em poucos metros de profundidade. Os parâmetros geotécnicos do maciço foram determinados por meio de correlações obtidas a partir de ensaios de campo, estimando-se os valores de coesão, ângulo de atrito, módulo de deformabilidade e resistência à compressão uniaxial das diferentes camadas do subsolo. As provas de cargas foram interrompidas quando alcançaram 3000 kN, apresentando deslocamentos inferiores a 5mm na carga de trabalho (1500 kN). Verificou-se que as estacas trabalharam por atrito lateral com tensões médias da ordem de 70 kPa, para o trecho superficial (solo residual), e superiores a 150 kPa, nos trechos em alteração de rocha. Os parâmetros geotécnicos estimados forneceram valores que se adequaram perfeitamente às análises numéricas. Nesse sentido, pôde-se verificar, através das análises realizadas e pelo modelo de transferência de carga, que seus comprimentos poderiam ser reduzidos, possibilitando otimizar o projeto geotécnico.
The behaviours of four foundation pilings (ϕ=0.41 m) constructed in Foz do Iguaçu, Paraná (PR), Brazil and subjected to slow loading tests were analysed. The results were compared with results from three-dimensional numerical modelling using the finite element method, which facilitates simulation of the elasto-plastic behaviour of soil. The local subsoil comprises varied stratigraphies; it is composed of a residual soil surface layer followed by weathered rock and bedrock, which are a few meters deep. The massif geotechnical parameters were determined through correlations obtained from field tests, whereby the values for cohesion, angle of friction, modulus of deformability and uniaxial compressive strength in the different subsoil layers were estimated. The load tests were interrupted at 3000 kN and displaced by less than 5 mm in the working load (1500 kN). The pilings were subjected to lateral friction work with an average stress of approximately 70 kPa for the surface portion (residual soil) and greater than 150 kPa for the weathered rock portions. The estimated geotechnical parameters provided values that were an exact match with the numerical analyses. Thus, given the analyses and load transfer method, the piling lengths can be reduced, which will facilitate the optimisation of the geotechnical design.
