RESUMO OBJETIVO Estimar a soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares de quatro a 14 anos de idade residentes no município de São Paulo, segundo variáveis clínicas, demográficas, epidemiológicas e sociais, durante o período de fechamento das escolas como medida de controle da covid-19. MÉTODOS Realizou-se um inquérito sorológico em setembro de 2020 com amostra aleatória estratificada por tipo de rede de ensino (pública municipal, pública estadual e privada). Foi coletada amostra de sangue venoso utilizando-se o teste de imunoensaio de fluxo lateral da fabricante Wondfo para detecção de anticorpos totais contra o vírus SARS-CoV-2. Aplicaram-se questionários semiestruturados para o levantamento de dados clínicos, demográficos, sociais e epidemiológicos. RESULTADOS A soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares foi de 16,6% (IC95% 15,4–17,8). O estudo encontrou soroprevalências mais elevadas na rede pública municipal (18,5%; IC95% 16,6–20,6) e estadual (16,2%; IC95% 14,4–18,2) em relação à rede privada (11,7; IC95% 10,0–13,7) e entre escolares da raça/cor preta e parda (18,4%; IC95% 16,8–20,2) e no estrato social mais vulnerável (18,5%; IC95% 16,9–20,2). A pesquisa identificou menores soroprevalências nos escolares que relataram seguir as medidas recomendadas de proteção contra a covid-19. CONCLUSÃO A soroprevalência de anticorpos contra o vírus SARS-CoV-2 atinge principalmente os escolares socialmente mais vulneráveis. Este estudo pode contribuir para embasar políticas públicas que reforcem a importância da suspensão das aulas presenciais e da necessidade de estratégias de medidas de proteção e acompanhamento do status sorológico daqueles que ainda não foram contemplados no calendário vacinal.

This study can contribute to support public policies that reinforce the importance of suspending face-to-face classes and developing strategies aimed at protective measures and monitoring of the serological status of those who have not yet been included in the vaccination schedule.

Abstract Hydrogels are used for wound treatment, as they may contain one or more active components and protect the wound bed. Papain is one of the active substances that have been used with this purpose, alongside urea. In this paper, carboxypolymethylene hydrogels containing papain (2% and 10% concentrations) and urea (5% concentration) were produced. Physical-chemical stability was performed at 0, 7, 15 and 30 days at 2-8ºC, 25ºC and 40ºC, as well as the rheological aspects and proteolytic activity of papain by gel electrophoresis. Clinical efficacy of the formulations in patients with lower limb ulcers was also evaluated in a prospective, single-center, randomized, double-blind and comparative clinical trial. The results showed 7-day stability for the formulations under 25ºC, in addition to approximately 100% and 15% of protein activity for 10% and 2% papain hydrogel, respectively. The rheological profile was non-Newtonian for the 10% papain hydrogel tested. There were no significant differences regarding the mean time for healing of the lesions, although 10% papain presented a better approach to be used in all types of tissue present in the wound bed.

Abstract The objective of this paper was to develop and evaluate two semi-solid pharmaceutical forms containing 0.1% tacrolimus: cream (CRT01) and gel (GLT01). For the evaluation of physicochemical stability, at times 0, 30, 60 and 90 days, at 23°C and at 40°C, High Performance Liquid Chromatography coupled with a Diode Array Detector (HPLC-DAD) was employed. This method was developed and validated for tacrolimus quantification. The occlusivity test and skin permeation assay were also performed, using an animal model (Wistar rats), and the CRT01 and GLT01 were compared to the 0.1% tacrolimus ointment (PFU01) obtained from the University Pharmacy, Federal University of Rio de Janeiro, Brazil. CRT01 and GLT01 presented a homogeneous aspect and consistency adequate for topical products, along with sensory characteristics above PFU01. They also presented adequate physicochemical stability for 90 days and a lower occlusive effect than PFU01 (p<0.05). CRT01 showed greater affinity for the skin when compared to PFU01 and GLT01, with low systemic absorption. The CRT01 semi-solid formulation was considered the most adequate one to treat patients with atopic dermatitis or other dermatologic inflammatory diseases, promoting rational use of tacrolimus.

BACKGROUND Malaria remains common among native Amazonians, challenging Brazil′s elimination efforts. OBJECTIVES We examined the epidemiology of malaria in riverine populations of the country′s main hotspot - the upper Juruá Valley in Acre state, close to the Brazil-Peru border, where Plasmodium vivax accounts for > 80% of cases. METHODS Participants (n = 262) from 10 villages along the Azul River were screened for malaria parasites by microscopy and genus-specific, cytochrome b (cytb) gene-based polymerase chain reaction. Positive samples were further tested with quantitative TaqMan assays targeting P. vivax- and P. falciparum-specific cytb domains. We used multiple logistic regression analysis to identify independent correlates of P. vivax infection. FINDINGS Microscopy detected only one P. vivax and two P. falciparum infections. TaqMan assays detected 33 P. vivax infections (prevalence, 11.1%), 78.1% of which asymptomatic, with a median parasitaemia of 34/mL. Increasing age, male sex and use of insecticide-treated bed nets were significant predictors of elevated P. vivax malaria risk. Children and adults were similarly likely to remain asymptomatic once infected. MAIN CONCLUSIONS Our findings are at odds with the hypothesis of age-related clinical immunity in native Amazonians. The low virulence of local parasites is suggested as an alternative explanation for subclinical infections in isolated populations.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

METHODS This is a population-based household survey conducted every 15 days, between June and September 2020, and January and February 2021. In total, the study comprised 11 phases. The presence of antibodies against SARS-CoV-2 was identified in venous blood using a lateral flow test, Wondfo Biotech. In the last phase, the researchers combined it with an immunoenzymatic test, Euroimmun. The participants also answered a semi-structured questionnaire on sociodemographic and economic factors, and on social distancing measures. Prevalence estimates and the 95% confidence interval were estimated according to regions, Human Development Index, sex, age group, ethnicity, education, income, and variables associated with risk or prevention of infection. To compare the frequencies among the categories of each variable, the chi-square test with Rao-Scott correction was used, considering a significance level of 5%. RESULTS In total, 23,397 individuals were interviewed and had their samples collected. The estimated prevalence of antibodies against SARS-CoV-2 ranged from 9.7% (95%CI: 7.9–11.8%) to 25.0% (95%CI: 21.7–28.7). The prevalence of individuals with antibodies against the virus was higher among black and brown people, people with lower schooling and income, and among residents of regions with lower Human Development Index. The lowest prevalences were associated with recommended measures of disease protection. The proportion of asymptomatic infection was 45.1%. CONCLUSION The estimated prevalence of the infection was lower than the cumulative incidence variation, except for the last phase of the study. The differences in prevalence estimates observed among subpopulations showed social inequality as a risk of infection. The lower prevalence observed among those who could follow prevention measures reinforce the need to maintain social distancing measures as a way to prevent SARS-CoV-2 infection.

Abstract Usually, machining employs cooling methods to avoid high temperatures. In most cases, the cutting fluid is applied with high flow and low pressures. This technique consumes a large amount of cutting fluid that causes serious problems in the environment and in the health of the worker. An alternative to the conventional cooling method is the Minimum Quantity of Lubricant. The MQL attempts to minimize the consumption of cutting fluid, which this study is about, whereby the reduction provided by this MQL was 99.99%. This technique consumes only the necessary quantity of cutting fluid to create a thin film of lubricant on the cutting surface grinding wheel. The MQL aerosol is injected with high pressure in the contact zone, efficiently lubricating the grinding process. However, the cooling effect of MQL is not enough to dissipate all heat produced during the grinding process. This study analyzes the improvement in the cooling effect caused by the addition of water in the MQL. The tests were done using different concentrations of cutting fluid in water, e.g. 1:0, 1:1, 1:3, 1:5 (oil:water). The experiment occurred on the external cylindrical plunge grinding with a vitrified cubic boron nitrite (CBN) grinding wheel, serving as the workpiece, the AISI 4340 quenched and tempered steel. The analyzed outputs were: surface roughness, roundness deviation, acoustic emission and vibration.. The results improved with the addition of water in the cutting fluid used in the MQL technique. The comparison with the MQL conditions showed that the best condition was the fluid with more water concentration.

No sistema plantio direto ocorre estratificação química no perfil do solo, incluindo o pH, sendo que níveis mais altos são formados a partir da superfície do solo em direção às camadas mais profundas. A acidez do subsolo é um importante fator limitante da produção agrícola. O uso de gesso tem sido indicado quando a acidez do subsolo apresenta limitação para o crescimento radicular, ou seja, quando o teor de cálcio (Ca) é baixo e/ou o nível de alumínio (Al) é tóxico em camadas do subsolo. Entretanto, existem muitas dúvidas sobre os métodos mais apropriados para estimar a necessidade de gesso. Esse estudo foi realizado com o objetivo de desenvolver modelos numéricos, por meio de técnicas de aprendizado de máquina, para a estimativa da necessidade de gesso em solos sob plantio direto. Análises computacionais foram realizadas em um conjunto de dados, aplicando-se o algoritmo M5'Rules, baseado em modelos de regressão. Esses dados corresponderam aos atributos químicos de solos coletados de experimentos sob plantio direto que receberam doses de gesso na superfície, ao longo de até oito anos após a aplicação, na região Sul do Brasil. Os resultados mostraram que os modelos numéricos gerados pelo algoritmo M5'Rules foram de grande utilidade para estimar as doses de gesso a serem empregadas em plantio direto. Os modelos mostraram que a saturação por Ca na capacidade de troca de cations efetiva (CTCe) foi um atributo mais importante do que a saturação por Al para estimar a necessidade de gesso em solos sob plantio direto.
Chemical stratification occurs under no-till systems, including pH, considering that higher levels are formed from the soil surface towards the deeper layers. The subsoil acidity is a limiting factor of the yield. Gypsum has been suggested when subsoil acidity limits the crops root growth, i.e., when the calcium (Ca) level is low and/or the aluminum (Al) level is toxic in the subsoil layers. However, there are doubts about the more efficient methods to estimate the gypsum requirement. This study was carried out to develop numerical models to estimate the gypsum requirement in soils under no-till system by the use of Machine Learning techniques. Computational analyses of the dataset were made applying the M5'Rules algorithm, based on regression models. The dataset comprised of soil chemical properties collected from experiments under no-till that received gypsum rates on the soil surface, throughout eight years after the application, in Southern Brazil. The results showed that the numerical models generated by rule induction M5'Rules algorithm were positively useful contributing for estimate the gypsum requirements under no-till. The models showed that Ca saturation in the effective cation exchange capacity (ECEC) was a more important attribute than Al saturation to estimate gypsum requirement in no-till soils.

Objetivo Elaborar recomendações para o diagnóstico, manejo e tratamento da nefrite lúpica no Brasil. Método Revisão extensa da literatura com seleção dos artigos com base na força de evidência científica e opinião dos membros da Comissão de Lúpus Eritematoso Sistêmico da Sociedade Brasileira de Reumatologia. Resultados e conclusões 1) A biópsia renal deve ser feita sempre que possível e houver indicação e quando não for possível, o tratamento deve ser orientado com base na inferência da clase histológica. 2) Devem ser implementados medidas e cuidados idealmente antes do início do tratamento, com ênfase na atenção ao risco de infecção. 3) Devem-se compartilhar riscos e benefícios do tratamento com pacientes e familiares. 4) O uso da hidroxicloroquina (preferencialmente) ou difosfato de cloroquina é recomendado para todos os pacientes (exceto contraindicação) durante as fases de indução e manutenção. 5) A avaliação da eficácia do tratamento deve ser feita com critérios objetivos de resposta (remissão completa/remissão parcial/refratariedade). 6) Os IECA e/ou BRA são recomendados como antiproteinúricos para todos os pacientes (exceto contraindicação). 7) A identificação de sinais clínicos e/ou laboratoriais sugestivos de GN laboratoriais sugestivos de glomerulonefrite proliferativa ou membranosa deve indicar início imediato de terapia específica incluindo corticosteroides e agente imunossupressor, mesmo que não seja possível comprovação histológica. 8) O tempo de uso dos imunossupressores deve ser no mínimo de 36 meses, mas eles podem ser mantidos por períodos mais longos. A sua suspensão só deve ser feita quando o paciente atingir e mantiver remissão completa sustentada. 9) Deve-se considerar nefrite lúpica refratária quando a remissão completa ou parcial não for alcançada após 12 meses de tratamento adequado, quando uma nova biópsia renal deve ser considerada para auxiliar na identificação da causa da refratariedade e decisão terapêutica.
Objective To develop recommendations for the diagnosis, management and treatment of lupus nephritis in Brazil. Method Extensive literature review with a selection of papers based on the strength of scientific evidence and opinion of the Commission on Systemic Lupus Erythematosus members, Brazilian Society of Rheumatology. Results and conclusions (1) Renal biopsy should be performed whenever possible and if this procedure is indicated; and, when the procedure is not possible, the treatment should be guided with the inference of histologic class. (2) Ideally, measures and precautions should be implemented before starting treatment, with emphasis on attention to the risk of infection. (3) Risks and benefits of treatment should be shared with the patient and his/her family. (4) The use of hydroxychloroquine (preferably) or chloroquine diphosphate is recommended for all patients (unless contraindicated) during induction and maintenance phases. (5) The evaluation of the effectiveness of treatment should be made with objective criteria of response (complete remission/partial remission/refractoriness). (6) Angiotensin-converting enzyme inhibitors and/or angiotensin receptor blockers are recommended as antiproteinuric agents for all patients (unless contraindicated). (7) The identification of clinical and/or laboratory signs suggestive of proliferative or membranous glomerulonephritis should indicate an immediate implementation of specific therapy, including corticosteroids and an immunosuppressive agent, even though histological confirmation is not possible. (8) Immunosuppressives must be used during at least 36 months, but these medications can be kept for longer periods. Its discontinuation should only be done when the patient could achieve and maintain a sustained and complete remission. (9) Lupus nephritis should be considered as refractory when a full or partial remission is not achieved after 12 months of an appropriate treatment, when a new renal biopsy should be considered to assist in identifying the cause of refractoriness and in the therapeutic decision.

Open Services Innovation: The Case of BT in the UK

Open innovation was conceptualised by Chesbrough (2003, p. 34) using cases from the ICT (Information and Communications Technologies) sector, more specifically equipment and component suppliers such as Lucent, IBM and Intel. Subsequently, the vast majority of the case studies in open innovation emphasised innovation processes in goods-based firms, not in service firms. Open systems innovation was well known in the telecommunications industry well before Chesbrough's conceptualisation of open innovation. However, subsequent research has not paid much attention on its adoption by incumbent telecommunications operators. This paper investigates how open innovation was adopted by the incumbent telecommunications operator BT in the UK, using the case study as the research method. BT used open innovation as a management injunction to systematise innovation under a common framework to leverage and integrate technology and knowledge in order to address customer needs, and to change the way of thinking about innovation within BT.

Estimativas do coeficiente de repetibilidade têm sido utilizadas no estudo de caracteres de várias espécies de palmeiras, auxiliando na definição do número e período adequado de avaliações dos genótipos para maior eficiência dos programas de melhoramento. Para macaúba, não há relatos de estudos dessa natureza. Nesse contexto, faz-se importante a determinação do coeficiente de repetibilidade das seguintes características biométricas dos frutos: peso do fruto, volume do fruto, diâmetro equatorial do fruto, peso da amêndoa, volume da amêndoa, diâmetro da amêndoa e espessura do endocarpo; e a determinação do número mínimo de avaliações para um eficiente processo de seleção e avaliação de subamostras de macaúba. O trabalho foi desenvolvido no Departamento de Fitotecnia da Universidade Federal de Viçosa, analisando-se 145 subamostras de macaúba, provenientes de coletas realizadas nos estados de Minas Gerais, São Paulo, Mato Grosso do Sul, Pará, Maranhão e Pernambuco. Houve diferença significativa para todas as características analisadas e observou-se concordância nas magnitudes dos coeficientes de repetibilidade de cada característica, obtidos pelos diferentes métodos. No entanto, os coeficientes de repetibilidade variam entre as oito características, de 0,68 a 0,99, observados para a característica espessura do endocarpo e volume da amêndoa, respectivamente. As estimativas dos coeficientes de determinação obtidas a partir das 10 repetições para as oito características e pelos quatro métodos foram superiores a 95%. A avaliação de quatro frutos é suficiente para compreensão das características estudadas das subamostras de macaúba com 90% de acurácia.
Estimates of repeatability coefficient have been used in character study of several species of palms, helping to define the number and appropriate period of assessment of genotypes to increase the efficiency of breeding programs. For macaw palm there are no reports of such studies. In this context it is important to determine the repeatability coefficient of the following biometric characteristics of fruits: fruit weight, fruit volume, equatorial diameter of the fruit, kernel weight, kernel volume, equatorial diameter of de kernel, and thickness of the endocarp, and the determination of the minimum number of ratings for an efficient selection process and evaluation of macaw palm subsamples. The study was conducted at the Department of Plant Science, Federal University of Viçosa, and 145 macaw palm subsamples were analyzed, from collections made in the states of Minas Gerais, São Paulo, Mato Grosso do Sul, Pará, Maranhão and Pernambuco. There were significant differences for all traits analyzed, and there was agreement on the magnitude of repeatability coefficients of each feature obtained by different methods. However, the repeatability coefficients ranging between eight characteristics, from 0.68 to 0.99, and were observed for feature endocarp thickness and volume of kernel, respectively. The coefficient of determination obtained from 10 repetitions for the eight features and the four methods were superior to 95%. The evaluation of four fruits is sufficient to assess the traits of macaw palm subsamples with 90% accuracy.

This work brings as its main contribution the study of the phenomenon of density currents in non-stratified reservoirs, with flows in steady regimes. Flows are analyzed for a wide range of Reynolds and Richardson numbers in the entrance of the reservoir. Based on this hypothesis, a hybrid numeric model is presented considering the Reynolds Transport's Equation - focusing on the conservation for volume, mass and momentum - with the intention of obtaining three-dimensional components of velocities, reduced acceleration of gravity and geometric characteristics of currents along the reservoir. It is possible to notice in the numeric simulations, mainly, the need of complementation of the model that refers to the inclusion of the entrainment coefficient and the analysis in the unsteady regime.
