Abstract In Brazil, 65% of the electric power production comes from hydroelectric power plants (HPPs). In some cases, these power plants divert the course of rivers, resulting in regions impacted by the absence of water, which are known as reduced fow sections (RFS). These sections are regulated by legislation and aim to maintain the minimum fow necessary to preserve the fauna, flora, fish, and human consumption in the region. Depending on the time of year, this minimum flow, also known as ecological flow (residual flow), can reach significant values, resulting in the interruption of power generation in these HPPs. This article presents a study case of two hydroelectric power plants located in different Brazilian states. The objective of this study is to explore the feasibility of implementing auxiliary generator groups that can operate uninterruptedly at different drop heights while ensuring the preservation of residual flow during low flow periods or taking advantage of the overflow flow by spillways during high flow periods. This auxiliary system would use the same set of structures and transmission systems as the main power plant, minimizing environmental impacts and implementation costs, allowing this solution to be implemented in HPPs that face this type of problem in Brazil. The results obtained indicate that HPPs with larger reservoirs have a greater ability to maintain residual flows with minimal generation loss and that run-of-the-river power plants are the most impacted by the maintenance of residual flow.

Abstract In the county of Pedra Lavrada - PB, various mineral occurrences of pegmatite are observed with prominence at Alto Dois Irmãos - PB, where quartz, feldspar and mica are mined. The understanding of mineral constituents is fundamental to define their industrial employment and to implement improvements in the beneficiation process. The proposal of this research is the detailed characterization and proof of that part of the ore (quartz) considered waste, presenting low poisoning (Fe2O3) and high silica (SiO2) content, to enable its industrial use. Besides that, to minimize the environmental impact through the reuse of this waste, and to determine the correct utilization of the milling equipment. For the pegmatite characterization, done were density determination tests, mineralogical identification by X-ray diffraction (XRD), chemical analysis by fluorescents X-rays and morphological analysis by scanning electron microscopy (SEM/EDS). Then tests were done for the Work Index (WI) calculous and consequently the empirical determination of the dimensions (LxD) of the pebble mill. The sterile presented a medium density of 2.66g/cm³, majority chemical composition of silicon and oxygen and quartz as the main mineralogical stage, indicating that this material has high purity. The. The Work Index (WI) was 15.9 kwh/t showing that this material can be considered resistant to breaking in the grinding process. The necessary energy to the milling was defined in 11.52 kwh/st, that corresponds to 57.28 HP. This power allowed the mill selection with diammeter and length of 1.83m, that provides the production maximization and the energy saving.

ABSTRACT Tamarind (Tamarindus indica L.) is a species with promising potential for commercial production; therefore, studies involving practices of conducting culture in the initial stage are essential. The present study aimed to evaluate different shading levels and substrate compositions in the seedling production of tamarind. The experimental design was completely randomized in a 4×4 factorial scheme, four levels of shading (0%, 18%, 30%, and 50%) and four substrates (pine bark-based substrate pure and mixed with 50% vermiculite, and sphagnum peat-based substrate pure and mixed with 50% vermiculite), with four replicates of four seedlings. Growth and biometric relationships were evaluated at 40 and 98 d after transplantation (DAT). The results reported that seedlings formed on the sphagnum peat-based substrate on pure or associated with vermiculite at 98 DAT had a height greater than 35 cm and a total 8.5 g DM. In contrast, seedlings produced in pine bark-based substrate did not reach 20 cm in height and had less than 3 g total DM. The condition of 30% and 50% shading promoted greater growth in height, reaching an average height greater than 40 cm, while in the full sun environment, seedlings showed an average of less than 35 cm; however, for the other variables, full sun environment did not differ from 30% shading, producing good quality seedlings. Thus, the cultivation of seedlings in an environment with 0% or 30% shading, associated with the sphagnum peat-based substrate pure or mixed with 50% vermiculite promoted the production of tamarind seedlings with vigorous growth. In the best environments, the best substrates increased, on average, 41.3% number of leaves, 80.5% shoot dry mass, 56.8% root dry mass, and 71.0% total dry mass. In the best substrates, the best environments increased, on average, 18.0% number of leaves, 20.7% shoot dry mass, 8.7% root dry mass, and 14.3% total dry mass.

Abstract Objective Evaluate autonomic function and low-grade inflammation and characterize the correlation between these variables in schoolchildren with obesity living in the Brazilian northeast region. Methods 84 children with obesity and 41 with normal weight were included in this cross-sectional study. Anthropometry, body composition, blood pressure (BP), inflammatory biomarkers, and heart rate variability (HRV) indexes were analyzed in children aged 7 to 11 years. Results children with obesity had increased systolic (p= 0.0017) and diastolic (p= 0.0131) BP and heart rate (p= 0.0022). The children with obesity displayed significantly lower SDNN, RMSSD, NN50, HF (ms), HF (nu), SD1, SD2, and higher LF (ms), LF (nu), LF/HF, SD1/SD2, DFA-α1, and DFA-α2, compared to normal weight. A lower and higher capacity for producing IL-10 (p= 0.039) and IL-2 (p= 0.009), respectively, were found in children with obesity compared to children with normal weight. Although IL-2, IL-4 and IL17A did not correlate with HRV parameters, IL-6 was positively correlated with SDNN, LF (ms) and SD2, TNF-α was positively correlated with LF/HF and SD1/SD2 ratio, and IFN-γ was positively correlated with SDNN, RMMSSD, NN50, LF (ms), HF (ms), SD1, and SD2. Conclusions The findings suggest that children with obesity have impaired autonomic function and systemic low-grade inflammation compared to children within the normal weight range, the inflammatory biomarkers were correlated with HRV parameters in schoolchildren living in the northeastern region of Brazil.

ABSTRACT Introduction: Anemia is frequent in patients undergoing replacement therapy for kidney failure. Anemia in the pre- and post-transplantation period might be related to kidney transplant outcomes. The current study therefore sought to assess the relationship between anemia, delayed allograft function (DGF), chronic kidney allograft dysfunction (CAD), and death from any cause following kidney transplantation from a deceased donor. Methods: This was a retrospective study with 206 kidney transplant patients of deceased donors. We analyzed deceased donors’ and kidney transplant patients’ demographic data. Moreover, we compared biochemical parameters, anemia status, and medicines between DGF and non-DGF groups. Afterward, we performed a multivariate analysis. We also evaluated outcomes, such as CAD within one year and death in ten years. Results: We observed a lower frequency of pre-transplant hemoglobin concentration (Hb) but higher frequency of donor-serum creatinine and red blood transfusion within one week after transplantation in the group with DGF. In addition, there was an independent association between Hb concentration before transplantation and DGF [OR 0.252, 95%CI: 0.159–0.401; p < 0.001]. There was also an association between Hb concentration after six months of kidney transplantation and both CAD [OR 0.798, 95% CI: 0.687–0.926; p = 0.003] and death from any cause. Conclusion: An association was found between pre-transplantation anemia and DGF and between anemia six months after transplantation and both CAD and death by any cause. Thus, anemia before or after transplantation affects the outcomes for patients who have undergone kidney transplantation from a deceased donor.

RESUMO Introdução: A anemia é frequente em pacientes submetidos à terapia substitutiva para insuficiência renal. A anemia nos períodos pré e pós-transplante pode estar relacionada aos desfechos do transplante renal. Portanto, o presente estudo buscou avaliar a relação entre anemia, função retardada do enxerto (FRE), disfunção crônica do enxerto renal (DCE) e óbito por qualquer causa após transplante renal de doador falecido. Métodos: Este foi um estudo retrospectivo com 206 pacientes transplantados renais de doadores falecidos. Analisamos dados demográficos de doadores falecidos e pacientes transplantados renais. Além disso, comparamos parâmetros bioquímicos, status de anemia e medicamentos entre os grupos FRE e não-FRE. Posteriormente, realizamos uma análise multivariada. Também avaliamos desfechos, como DCE em um ano e óbito em dez anos. Resultados: Observamos menor frequência de concentração de hemoglobina (Hb) pré-transplante, mas maior frequência de creatinina sérica do doador e transfusão de hemácias no período de uma semana após o transplante no grupo FRE. Além disso, houve associação independente entre a concentração de Hb antes do transplante e a FRE [OR 0,252; IC 95%: 0,159–0,401; p < 0,001]. Houve também associação entre a concentração de Hb após seis meses de transplante renal e ambos, DCE [OR 0,798; IC95%: 0,687–0,926; p = 0,003] e óbito por qualquer causa. Conclusão: Encontrou-se uma associação entre anemia pré-transplante e FRE e entre anemia seis meses após o transplante e ambos, DCE e óbito por qualquer causa. Assim, a anemia antes ou após o transplante afeta os desfechos de pacientes que foram submetidos a transplante renal de doador falecido.

Abstract Introduction: Chronic kidney disease is usually asymptomatic, and its diagnosis depends on laboratory tests, with emphasis on serum creatinine and proteinuria. Objective: To assess knowledge on the role of serum creatinine as a biomarker of kidney function in a sample of the Brazilian population. Method: Cross-sectional observational study conducted in São Paulo (SP, Brazil), in which a random adult population was interviewed. Results: A total of 1138 subjects were interviewed, with a median age of 36 years old (27–52); 55.1% were female. Regarding the “creatinine” biomarker, 40.6% stated they had never performed such a test. When asked about their knowledge on the usefulness of this exam, only 19.6% knew its function. The other responses were “I don’t know” (71.6%), evaluating heart function (0.9%) and liver function (7.8%). Of those who reported they had already taken a creatinine test, only 29.4% correctly identified the role of creatinine. When dividing the groups into “knows” and “does not know” the function of creatinine, a statistically significant difference (p < 0.05) was observed regarding level of education, female sex, being a healthcare student/worker, having ever measured creatinine, knowing someone with kidney disease and older age. In the multivariate analysis, the main variable related to knowing the creatinine role was having previously taken the test (OR 5.16; 95% CI 3.16–8.43, p < 0.001). Conclusion: There is a significant lack of knowledge about creatinine and its use in checkups. The results indicate that greater efforts are needed from healthcare professionals to raise awareness on the role of serum creatinine.

Resumo Introdução: A doença renal crônica costuma ser assintomática e seu diagnóstico depende da realização de exames laboratoriais, com destaque para a creatinina sérica e pesquisa de proteinúria. Objetivo: Avaliar em uma amostra da população brasileira o conhecimento sobre o papel da creatinina sérica como marcador de função renal. Método: Estudo observacional transversal realizado na cidade de São Paulo (SP, Brasil), em que foi entrevistada uma população adulta aleatória. Resultados: Foram entrevistados 1138 indivíduos, com idade mediana de 36 anos (27–52); 55,1% do sexo feminino. Com relação ao marcador “creatinina”, 40,6% afirmaram que nunca realizaram tal dosagem. Quando questionados quanto ao conhecimento sobre a utilidade desse exame, somente 19,6% sabiam a sua função. As outras respostas foram “não sei” (71,6%), avaliar o funcionamento do coração (0,9%) e fígado (7,8%). Dos que afirmaram já terem realizado o exame de creatinina, somente 29,4% acertaram a função da creatinina. Ao dividir os grupos em “sabe” e “não sabe” a função da creatinina, percebeu-se diferença estatisticamente significante (p < 0,05) em relação ao grau de escolaridade, sexo feminino, ser aluno/trabalhador da saúde, ter dosado creatinina alguma vez, conhecer alguém com doença renal e maior idade. Na análise multivariada, a principal variável relacionada com conhecer a função da creatinina foi ter realizado o exame anteriormente (OR 5,16; IC 95% 3,16–8,43, p < 0,001). Conclusão: Há grande desconhecimento sobre a creatinina e seu uso em check-ups. Os resultados indicam que é necessário maior esforço por parte dos profissionais de saúde para divulgar o papel da creatinina sérica.

ABSTRACT BACKGROUND: Considering the ability of the health and self-management in diabetes questionnaire (HASMID-10) to verify the impact of self-management on diabetes, we highlight its relevance to scientific research and clinical applicability. However, to date, no study has been conducted to scientifically support its use in other languages. OBJECTIVE: To translate, cross-culturally adapt, and validate the HASMID-10 into the Brazilian Portuguese. DESIGN AND SETTING: A translation, cross-cultural adaptation, and validation study conducted at Ceuma University. METHODS: Study was conducted in accordance with the Guidelines for the Process of Cross-Cultural Adaptation of Self-Report Measures and Consensus-based Standards for the Selection of Health Measurement Instruments. We included participants of both sexes diagnosed with diabetes, aged between 18 and 64 years, and without cognitive deficits or any other limitations that would prevent them from answering the questionnaire. We assessed participants using the problem areas in diabetes (PAID) scale and HASMID-10. We assessed reliability using a test-retest model with a 7-day interval between assessments. We used intraclass correlation coefficient (ICC), 95% confidence interval (CI), standard error of measurement (SEM), minimum detectable difference (MDD), Spearman correlation coefficient, and floor and ceiling effects. RESULTS: Sample comprised 116 participants, most of whom were women, overweight, non-practitioners of physical activity, and nonsmokers. We observed significant correlations (P = 0.006; rho = −0.256) between the HASMID-10 and PAID, adequate reliability (ICC = 0.780) and internal consistency (Cronbach's alpha = 0.796). No ceiling or floor effects were observed. CONCLUSION: HASMID-10 has adequate measurement properties and may be used for Brazilians.

ABSTRACT BACKGROUND: A bibliometric analysis covering only the production of original studies or considering world production until 2022 has yet to be conducted. The creation and advancement of vaccines have also influenced research priorities, demonstrating the need for a new approach to this subject. OBJECTIVES: To analyze worldwide scientific production related to coronavirus disease 2019 (COVID-19) and the aged and to describe what has already been produced. DESIGN AND SETTING: Bibliometric analysis with a quantitative approach. METHOD: The search terms “COVID-19,” “SARS-CoV-2,” “Aged,” and “Elderly” were used to retrieve articles from the Web of Science database. A total of 684 articles were included in the analysis. Data were imported into RStudio Desktop Software and linked to R Software. The Bibliometrix R package and VOSviewer software were used for analysis. RESULTS: Most articles were published in 2020. These were produced by 4,937 authors and published in 379 journals. The keyword most used by the authors was “COVID-19.” Publications from 77 countries were obtained. China had the highest article production ranking, and Spain collaborated the most. The articles addressed the implications of the pandemic on the aged, the relationship between vaccination in the aged, and the implications for the disease itself. CONCLUSION: Further research should be conducted, mainly concerning vaccines and vaccination of the aged, owing to the need for and importance of immunization in this risk group, including assessing the long-term effects of vaccines.

Abstract Introduction Scientific studies in Brazil grew around 10.7% compared to previous years. However, the level of quality of evidence has been decreasing. The aim in our study is to examine the meeting abstracts of the Brazilian congress of coloproctology and analyze the level of evidence in trends and variables. Methods A descriptive bibliometric study, working with secondary data to review scientific abstracts in the annals of the coloproctology congress from 2015 to 2019. Results A total of 1756 abstracts of the Brazilian Congress of Coloproctology were analyzed for 5 years (2015-2019). There was a higher trend of abstracts presented with lower levels of evidence (level of evidence 5: 52.3% and 3: 30%), being the majority composed of case reports (49.4%) and retrospective studies (30.4%). The last two years analyzed (2018: 55.2% and 2019: 59.3%) had a predominance above average of case reports. From 2017 to 2019 there was a significant decrease in the number of level 2 evidence studies (18.10%,11.80% and 5.50%), while the number of studies with level 5 evidence showed an increase (45.60%, 56.60% and 61.40%). Statistical analysis occurred in only 17%, with an important decrease for the last two years (2018: 13.6%; 2019: 12.1%). Conclusions Although the data of this study is from the Brazilian coloproctology point of view, they are important for the global scientific community, as they allow a quantitative evaluation of the relative contribution from the level of evidence of Brazilian coloproctology researchers to the scientific scenario.

Objectives: To summarize evidence-based cognitive-behavioral therapy (CBT) treatment and propose clinical interventions for adult patients with obsessive-compulsive disorder (OCD). Methods: The literature on CBT interventions for adult OCD, including BT and exposure and response prevention, was systematically reviewed to develop updated clinical guidelines for clinicians, providing comprehensive details about the necessary procedures for the CBT protocol. We searched the literature from 2013-2020 in five databases (PubMed, Cochrane, Embase, PsycINFO, and Lilacs) regarding study design, primary outcome measures, publication type, and language. Selected articles were assessed for quality with validated tools. Treatment recommendations were classified according to levels of evidence developed by the American College of Cardiology and the American Heart Association. Results: We examined 44 new studies used to update the 2013 American Psychiatric Association guidelines. High-quality evidence supports CBT with exposure and response prevention techniques as a first-line treatment for OCD. Protocols for Internet-delivered CBT have also proven efficacious for adults with OCD. Conclusion: High-quality scientific evidence supports the use of CBT with exposure and response prevention to treat adults with OCD.

Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

ABSTRACT The production of seedlings of forest species for reforestation of biomes is a practice that restores the conditions of the environment, but water and substrates that favor the growth of seedlings are required. However, in the semi-arid region, water is a limiting factor, requiring the use of alternative sources such as treated domestic sewage. In this context, the objective was to evaluate the growth of Anadenanthera colubrina L. seedlings produced in substrates (soil + bovine manure and soil + coconut fiber) fertilized with treated domestic sewage effluent diluted in public-supply water in proportions (100, 75, 50 and 25% of domestic effluent) and using water from the supply network as a control. The treatments were arranged in a completely randomized design, in a split-plot with three replicates. The treated domestic effluent (TDE) used in the experiment came from the domestic sewage treatment plant of the decanter-digester type of the Projeto de Assentamento Milagre Apodi/RN. The study was conducted in a seedling production nursery at UFERSA, with growth and vigor evaluations at 30, 60, 90 and 120 days after transplanting. The production of seedlings fertigated with domestic effluent caused differences in all growth variables, with improvement in phytomass production. Seedlings fertigated with a dilution of 50% TDE and 50% water supply showed better results when produced with substrate composed of soil and coconut fiber. The seedlings showed good quality, and those cultivated with 50% and 75% of wastewater showed better quality after 120 days of cultivation.

RESUMO A produção de mudas de espécies florestais para reflorestamento de biomas é uma prática que restaura as condições do meio, porém é necessário água e substratos que favoreçam o crescimento das mudas. Entretanto, na região semiárida a água é um fator limitante sendo necessário uso de fontes alternativas como os esgotos domésticos tratados. Neste sentido, objetivou-se avaliar o crescimento de mudas de angico produzidas em substratos (solo + esterco bovino e solo + fibra de coco) fertigadas com efluente de Esgoto Doméstico Tratado (EDT) e diluídos em água de abastecimento nas proporções (100, 75, 50 e 25% do EDT) e água de abastecimento (testemunha). Os tratamentos foram dispostos em delineamento inteiramente casualizado, em parcelas subdivididas com três repetições. O efluente EDT utilizado no experimento foi proveniente da estação de tratamento de esgoto doméstico do tipo decanto digestor do Projeto de Assentamento Milagre Apodi/RN. O estudo foi realizado em viveiro de produção de mudas da UFERSA, sendo realizadas avaliações de crescimento e vigor aos 30, 60, 90 e 120 dias após o transplantio. A produção de mudas fertigadas com efluente doméstico promoveu diferenças em todas as variáveis de crescimento com melhoria na produção de fitomassa. As mudas fertigadas com diluição de 50% do EDT e 50% água de abastecimento proporcionaram melhores resultados quando produzidas com o substrato composto por solo e fibra de coco. As mudas apresentaram-se como de boa qualidade e as cultivadas com 50% e 75% de água residuária mostraram-se com maior qualidade aos 120 dias de cultivo.

ABSTRACT Introduction The purpose of this retrospective bibliometric study was to assess the discrepancies between coloproctology surgery meeting abstracts and subsequent full-length manuscript publications. Methods Abstracts presented at the Brazilian Congress of Coloproctology Surgery from 2015 to 2019 were compared with matching manuscript publications. Discrepancies between the abstract and therefore the subsequent manuscript were categorized as major (changes within the purpose, methods, study design, sample size, statistical analysis, results, and conclusions) and minor (changes within the title, authorship, and number of female authors) variations. Results The conversion rate of abstracts in published manuscripts was 6,9% (121 abstracts). There were inconsistencies between the study title (66,1%), authorship (69,5%), study design (3,3%), sample size (39,2%), statistical analysis (24,8%), results (25,6%), and conclusions (12,4%) of manuscripts compared with their corresponding meeting abstracts. Conclusion As changes occur before manuscript publication of coloproctology surgery meeting abstracts, caution should be exercised in referencing abstracts or altering surgical practices based on abstracts content.

Abstract Objective The aim of this study was to analyze the effects of fluid overload related to mechanical ventilation, renal replacement therapy, and evolution to discharge or death in critically ill children. Methods A retrospective study in a Pediatric Intensive Care Unit for two years. Patients who required invasive ventilatory support and vasopressor and/or inotropic medications were considered critically ill. Results 70 patients were included. The mean age was 6.8 ± 6 years. There was a tolerable increase in fluid overload during hospitalization, with a median of 2.45% on the first day, 5.10% on the third day, and 8.39% on the tenth day. The median fluid overload on the third day among those patients in pressure support ventilation mode was 4.80% while the median of those who remained on controlled ventilation was 8.45% (p = 0.039). Statistical significance was observed in the correlations between fluid overload measurements on the first, third, and tenth days of hospitalization and the beginning of renal replacement therapy (p = 0.049) and between renal replacement therapy and death (p = 0.01). The median fluid overload was 7.50% in patients who died versus 4.90% in those who did not die on the third day of hospitalization (p = 0.064). There was no statistically significant association between death and the variables sex or age. Conclusions The fluid overload on the third day of hospitalization proved to be a determinant for the clinical outcomes of weaning from mechanical ventilation, initiation of renal replacement therapy, discharge from the intensive care unit, or death among these children.
