This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

ABSTRACT Brazil is currently the 4th world’s largest banana producer, producing around 7 million tons. In this scenario, several studies have been developed with a large amount of data, such as climatic, morphological, and nutritional data, in an attempt to improve these numbers even further. This study aims to classify banana ripening stages by artificial neural networks (ANN) as a function of plant physical, physicochemical, and biochemical parameters. The used ANN consisted of a three-layer feedforward backpropagation network, with eight neurons in the input layer (physical, physicochemical, and biochemical parameters), ten neurons in the intermediate layer, and two neurons in the output layer (classification of banana ripening stages). The results showed three configurations. ANN presented an excellent result for the training phase, with 100% accuracy in the sample classification for the three configurations. The validation and testing phases, that is, the classification of samples that were not part of the training, showed 91.6% and 94.4% accuracy in the first and second configurations, respectively, and 89.5% accuracy in the third configuration.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

ABSTRACT Objective: To evaluate incidence of subclinical and overt hyperthyroidism and hypothyroidism. Subjects and methods: The Brazilian Longitudinal Study of Adult Health (ELSA-Brasil) is a prospective cohort study of 15,105 civil servants, examined at baseline and over a 4-year follow-up. This analysis included 9,705 participants with normal thyroid function at baseline, follow-up information about thyroid function and with no report of using drugs that may interfere in the thyroid function. Thyroid function was defined by TSH/FT4 levels or routine use of thyroid hormones/anti-thyroid medications. Annual and cumulative (over 4-year) incidence rates were presented as percentages (95% Confidence Intervals). Results: The incidence of all overt and subclinical thyroid disease was 6.7% (1.73%/year): 0.19% for overt hyperthyroidism (0.048%/year), 0.54% for subclinical hyperthyroidism (0.14%/year), 1.98% for overt hypothyroidism (0.51%/year), and 3.99% for subclinical hypothyroidism (1.03%/year). The incidence of all thyroid diseases was higher in women, when compared to men, with a low women:men ratio (1.36). For Blacks the highest incidence was for overt hyperthyroidism, while for Whites, the highest incidence was for overt hypothyroidism. However, the highest incidence of overt hyperthyroidism was detected in Asian descendants. The presence of antithyroperoxidase antibodies at baseline was associated with higher incidence of overt thyroid diseases. Conclusion: These results showed a high incidence of hypothyroidism, which is compatible with a country with a more-than-adequate iodine intake. The low women:men ratio of the incidence of thyroid dysfunction highlights the importance of the diagnosis of thyroid diseases among men in Brazil.

This work presents some modifications in the post-synthetic processing for a classical arsenic reagent: AsI3. In comparison with the widely used analog, the trichloride, arsenic triiodide presents several advantages such as low toxicity, air stability, and low volatility. It was used as a synthetic precursor in the preparation of a variety of arsenic(III) derivatives like arsines, arsenites, and thioarsenites. Besides that, AsI3 was submitted to a diversity-oriented Grignard reaction in the preparation of a homologous series of trialkyl arsines ranging from AsC3H9 to AsC12H27. The series was analyzed by comprehensive two-dimensional gas chromatography coupled with time-of-flight mass spectrometry to provide a trialkyl arsines library that can be used for the direct analysis of natural samples.

Abstract For human/SARS-CoV-2 interactome genes ACE2, TMPRSS2 and BSG, there is a convincing evidence of association in Asians with influenza-induced SARS for TMPRSS2-rs2070788, tag-SNP of the eQTL rs383510. This case illustrates the importance of population genetics and of sequencing data in the design of genetic association studies in different human populations: the high linkage disequilibrium (LD) between rs2070788 and rs383510 is Asian-specific. Leveraging on a combination of genotyping and sequencing data for Native Americans (neglected in genetic studies), we show that while their frequencies of the Asian tag-SNP rs2070788 is, surprisingly, the highest worldwide, it is not in LD with the eQTL rs383510, that therefore, should be directly genotyped in genetic association studies of SARS in populations with Native American ancestry.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

The phytochemical investigation of the hexane and methanol extracts from the bark of Duguetia pycnastera Sandwith (Annonaceae) afforded seven known compounds, two asarone-derived phenylpropanoids and five isoquinoline-derived alkaloids. The asarones, γ-asarone (1-allyl-2,4,5-trimethoxybenzene) and 2,4,5-trimethoxy-styrene were isolated of the hexane extract while the aporphine alkaloids, O-methylmoschatoline, lysicamine, nornuciferidine, and guatterine N-oxide, and the benzyltetrahydroisoquinoline alkaloid, (S)-reticuline were isolated of the alkaloid fraction of the methanol extract. This is the first report of these compounds in D. pycnastera. γ-Asarone is being reported for the first time in the Annonaceae. Nornuciferidine is described for the second time in the Annonaceae while guatterine N-oxide is the third register. The structures of the isolated compounds were established by extensive analyses using 1D and 2D NMR spectroscopy in combination with MS. The cytotoxic activity of the isolated compounds (except for nornuciferidine) was evaluated against cancer and non-cancerous cell lines, in which lysicamine was the most active compound, mainly against HL-60, HepG2, and K562 with IC50 values of 24.40, 28.86 and 38.75 µmol L-1, respectively.

ABSTRACT Introduction Influenza is an important cause of morbimortality worldwide. Although people at the extremes of age have a greater risk of complications, influenza has been more frequently investigated in the elderly than in children, and inpatients than outpatients. Yearly vaccination with trivalent or quadrivalent vaccines is the main strategy to control influenza. Objectives Determine the clinical and molecular characteristics of influenza A and B infections in children and adolescents with influenza-like illness (ILI). Methods: A cohort of outpatient children and adolescents with ILI was followed for 20 months. Influenza was diagnosed with commercial multiplex PCR platforms. Results: 179 patients had 277 episodes of ILI, being 79 episodes of influenza A and 20 episodes of influenza B. Influenza A and B cases were mild and had similar presentation. Phylogenetic tree of influenza B viruses showed that 91.6% belonged to the B/Yamagata lineage, which is not included in trivalent vaccines. Conclusions: Influenza A and B are often detected in children and adolescents with ILI episodes, with similar and mild presentation in outpatients. The mismatch between the circulating influenza viruses and the trivalent vaccine offered in Brazil may have contributed to the high frequency of influenza A and B in this population.

RESUMO Objetivo: Avaliar o impacto da acuidade visual, danos no campo visual e outros fatores na qualidade de vida em pacientes brasileiros com glaucoma. Métodos: Este foi um estudo transversal prospectivo incluindo 49 pacientes com glaucoma selecionados com base na presença de defeitos por perimetria automatizada padrão reprodutíveis em pelo menos um olho no momento da avaliação. Um exame oftalmológico detalhado foi realizado em cada paciente. Todos os pacientes possuíam perimetria automatizada padrão reprodutível e preencheram um questionário NEI VFQ-25. As associações dos escores de qualidade de vida à acuidade visual melhor corrigida e à perda de campo visual dos melhores e piores olhos foram investigadas. Resultados: A média dos escores de qualidade de vida dos pacientes foi de 58,8 ± 18,7 unidades. Os maiores e menores valores médios (85,0 ± 24,2 e 37,5 ± 36,5 unidades) foram observados nas subescalas “Social Functioning Subscale” e “Driving Subscale”, respectivamente. Pacientes com glaucoma avançado (desvio médio <-12 dB) no pior olho tiveram escores de qualidade de vida significativamente menores (p=0,007). Houve correlação significativa entre escores de qualidade de vida e a acuidade visual dos olhos melhores e piores (r2=13%; p=0,010 e r2=32%; p<0,001; respectivamente). Houve também uma correlação significativa entre os escores de qualidade de vida e desvios médios da perimetria automatizada padrão dos olhos melhores e piores (r2=13%; p=0,023 e r2=47%; p<0,001; respectivamente). Em um modelo multivariado contendo dados socioeconômicos e de comorbidades, a qualidade de vida permaneceu significativamente relacionada ao desvio médio padrão da perimetria automatizada do olho melhor e pior (r2=23%; p=0,29 e r2=49%; p<0,001, respectivamente) bem como para a acuidade visual do olho melhor e pior (r2=18%; p= 0,017 e r2=40%; p<0,001, respectivamente). Conclusão: O desvio padrão da perimetria automatizada padrão e a acuidade visual dos olhos melhor e pior foram associados à menor qualidade de vida em pacientes brasileiros com glaucoma. A qualidade de vida foi em grande parte altamente associada ao desvio padrão da perimetria automatizada padrão do pior olho.
ABSTRACT Purpose: To evaluate the impact of visual acuity, visual field damage, and other factors on the quality of life in Brazilian patients with glaucoma. Methods: This cross-sectional prospective study involved 49 patients with glaucoma enrolled based on the presence of reproducible standard automated perimetry defects in at least one eye at the time of evaluation. A detailed ophthalmologic examination was performed on each patient. All patients had reproducible standard automated perimetry and completed an NEI VFQ-25 questionnaire. The associations of the quality of life scores to the best-corrected visual acuity and the visual field loss of the better and worse eyes were investigated. Results: The mean quality of life score of the patients was 58.8 ± 18.7 units. The highest and lowest mean values (85.0 ± 24.2 and 37.5 ± 36.5 units) were observed in the “Social Functioning Subscale” and “Driving Subscale,” respectively. Patients with advanced glaucoma (mean deviation <-12 dB) in the worse eye had significantly lower quality of life scores (p=0.007). There was a significant correlation between the quality of life scores and the visual acuity of the better and worse eyes (r2=13%, p=0.010 and r2=32%, p<0.001, respectively). There was also a significant correlation between the quality of life scores and standard automated perimetry mean deviation of the better and worse eyes (r2=13%, p=0.023 and r2=47%, p<0.001, respectively). In a multivariate model containing socioeconomic and comorbidity indices, quality of life remained significantly related to the standard automated perimetry mean deviation of the better and worse eyes (r2=23%, p=0.29 and r2=49%, p<0.001, respectively) as well as to the visual acuity of the better and worse eyes (r2=18%, p=0.017 and r2=40%, p<0.001, respectively). Conclusion: The standard automated perimetry mean deviation and the visual acuity of the better and worse eyes were associated with lower quality of life in Brazilian patients with glaucoma. Quality of life was mostly highly associated with the standard automated perimetry mean deviation of the worse eye.

Abstract This study aimed to optimize ultrasound-assisted palm oil-in-water nanoemulsions. The influence of hydrophilic-lipophilic balance (HLB), oil and surfactants concentrations, ultrasound intensity and processing time were investigated by means of two experimental designs. Desirability profiles were applied to detect the optimal conditions for preparing the nanoemulsion. Stable nanoemulsions with small droplet size and polydispersity index (PDI) were obtained at an HLB of 6.5, oil and surfactant concentrations of 2.8 wt%, ultrasound intensity of 200 W.cm-2 and processing time of 15 minutes. The nanoemulsion presented stability for 30 days at three different temperatures (4°C, 25°C and 40°C), which makes it suitable for application in the food and cosmetic industries. Ultrasound was shown to be a promising technology to produce nanoemulsions containing palm oil.

Bertholletia excelsa Bonpl. (Lecythidaceae) is a South American tree worldwide known for providing the Brazil nuts. In the Amazon Region, B. excelsa is found in monocultures, integrating agroforestries and providing raw materials for food and timber industries. Through the application of an integrative analysis based on high-performance liquid chromatography coupled with diode array detector and tandem mass spectrometry (HPLC-DAD-MS/MS) and nuclear magnetic resonance (NMR) techniques, the present study showed that B. excelsa bark biomass residues contain large quantities of ellagic acid (EA) and its derivatives. Qualitatively, five compounds were characterized for the first time in this species. Quantitations were carried out to determine the total amount of these compounds in outer and inner bark tissues. A total of 4.96 and 44.09 g of EA derivatives per kg of dry residues was determined for the outer and inner barks, respectively. Among the EA derivatives, eschweilenol C, ellagic acid and valoneic acid dilactone were the main compounds. These results pointed B. excelsa barks as a valuable biomass residue with potential to be source of health-promoting compounds. Therefore, a potential raw material as source of valuable bioactive phenolic compounds is described herein.

RESUMO Idosos apresentam risco elevado de desenvolverem deficiência de Vitamina D (VitD) devido à diminuição da ingestão e da síntese na pele. A maioria das ações biológicas da VitD é mediada pelo receptor da vitamina D (VDR), que está presente nos neurônios e células gliais do hipocampo, e no córtex e em núcleos subcorticais, áreas essenciais para a cognição. Sabe-se que polimorfismos do gene VDR podem diminuir a afinidade do VDR pela VitD. Objetivo: O presente estudo teve como objetivo investigar a influência dos níveis de VitD no declínio cognitivo em pacientes com demência devida à doença de Alzheimer (DA, n = 32) e comprometimento cognitivo leve (CCL, n = 15) em comparação a um grupo de idosos cognitivamente saudáveis (n = 24). Nós também avaliamos a associação entre polimorfimos no gene do receptor de VitD e as alterações cognitivas. Métodos: Quatro polimorfismos no gene VDR foram estudados, sendo BsmI, ApaI, FokI e TaqI, por PCR-RFLP. Os níveis séricos de 25-hidroxi vitamina D (25(OH)D) foram determinados por HPLC. Resultados: Não houve diferença significativa nos níveis de 25(OH)D ou nas frequências genotípicas / alélicas entre os grupos. Níveis deficientes de 25(OH)D foram mais frequentes nas mulheres. Os genótipos AA / AG do polimorfismo BsmI foram associados a níveis suficientes de 25(OH)D, enquanto o genótipo GG deste mesmo polimorfismo foi associado a níveis insuficientes no grupo com comprometimento cognitivo (em indivíduos com DA ou CCL). Conclusões: Os resultados obtidos não confirmam a relação entre redução dos níveis de VitD, polimorfismos no gene VDR e função cognitiva alterada nesta amostra. No entanto, os dados indicam que o polimorfismo BsmI no gene VDR está associado aos níveis de VitD em indivíduos com declínio cognitivo.
ABSTRACT Elderly people are at a high risk of developing vitamin D (VitD) deficiency due to both decreased intake and cutaneous synthesis. Most of the biological actions of VitD are mediated by the vitamin D receptor (VDR), which is present in neurons and glial cells of the hippocampus, and in the cortex and subcortical nuclei, essential areas for cognition. It is known that VDR gene polymorphisms may decrease the VDR affinity for VitD. Objective: The present study aimed to investigate the influence of VitD levels on cognitive decline in patients with dementia due to Alzheimer's disease (AD, n = 32) and mild cognitive impairment (MCI, n = 15) compared to cognitively healthy elderly (n = 24). We also evaluated the association of VDR gene polymorphisms with cognitive disturbance. Methods: Four polymorphisms on the VDR gene were studied, namely, BsmI, ApaI, FokI and TaqI, by polymerase chain reaction-restriction fragment length polymorphism. Serum levels of 25-hydroxy vitamin D (25(OH)D) were determined by high performance liquid chromatography. Results: No significant difference in 25(OH)D levels or genotypic/allelic frequencies was observed between the groups. Deficiency of 25(OH)D was more frequently observed in women. The AA/AG genotypes of the BsmI polymorphism was associated with sufficient 25(OH)D levels, while the GG genotype of this same polymorphism was associated to insufficient levels in the cognitively-impaired group (individuals with AD or MCI). Conclusions: The data obtained do not confirm the relationship between reductions of VitD levels, polymorphisms in the VDR gene, and altered cognitive function in this sample. However, the data indicate that BsmI polymorphism in the VDR gene is associated with the VitD levels in individuals with cognitive decline.

Abstract Duchenne Muscular Dystrophy (DMD) is the most common X-linked muscular disease affecting humans. The Golden Retriever Muscular Dystrophy model (GRMD) is considerthe most suitable for several studies. This assay aims to quantify lymphocyte subpopulations CD4, CD5, and CD8, and standardize, the serum electrophoretic profile, to understand their contribution to the pathologic process in normal Golden Retriever dogs (GR group) and dystrophic´s (GRMD group), through the umbilical cord blood, in dogs aged from 2 to 3 months (GR II and GRMD II), and in dogs over 1 year of age (GR III and GRMD III). No significant differences were observed between the CD8+ lymphocyte subpopulations of the groups studied. The CD4+ and CD5+ lymphocyte subpopulations were significantly higher in the GRMD III group compared to the GR III group. Twenty-two different proteins in the gel were identified. The serum concentrations of the proteins belonging to the GR I and GRMD I groups were significantly lower than those of the other groups. We show that expression of acute phase proteins are worst during the aging of the dogs. We hope to expand knowledge to better understand the GRMD model and the translational data.

RESUMO: Este estudo teve como objetivo analisar os casos de mastocitomas cutâneos em cães atendidos no Hospital Veterinário Governador Laudo Natel da Faculdade de Ciências Agrárias e Veterinárias (FCAV) Unesp Câmpus Jaboticabal, no período de 2005 a 2015, além de avaliar a ocorrência dessa neoplasia em relação à raça, sexo e idade; determinar as regiões corpóreas mais acometidas e as características neoplásicas observadas, assim como, os métodos de diagnóstico mais utilizados no setor, estabelecer os principais graus histológicos diagnosticados e os tratamentos preconizados, verificar alterações de tamanho em relação aos linfonodos, à taxa de recidiva, metástase e possíveis associações entre as variáveis consideradas. Foram avaliados 192 prontuários clínicos de pacientes e observou-se que os cães sem raça definida (SRD) (n=46, 23,96%) foram os mais acometidos, seguidos por cães da raça Boxer (n=40, 20,83%) e Pit bull (n=27, 14,06%). A maioria dos cães eram fêmeas (n=107, 55,7%), e com idade de 7 a 11 anos (n=120, 62,5%). Do total, 32 pacientes apresentaram (16,7%) tumores localizados em membro pélvico, seguidos por nódulos em membro torácico (10,4%, n=20). Sessenta e cinco animais possuíam nódulos ulcerados (33,9%), sendo 96 nódulos de consistência firme (50%). Setenta e seis animais apresentavam múltiplos nódulos (39,6%) e aqueles acima de três centímetros perfaziam 39,1% da amostra (n=75). O método mais utilizado para o diagnóstico foi a punção aspirativa por agulha fina associado à histopatologia (HT), totalizando 39,8% dos nódulos (n=66), seguido pela HT com 37,3% (n=62). Em relação às variáveis histológicas, 18 pacientes apresentaram grau I (11,8%), 112 com grau II (73,7%) e 22 com grau III (14,5%) de acordo com Patnaik et al. (1984). Segundo a classificação de Kiupel et al. (2011), 37 casos foram classificados como de alto grau (35,2%) e 68 como sendo de baixo grau (64,8%). Foi observada associação estatística quando se avaliou a presença de nódulos solitários/múltiplos e o grau histológico, segundo Patnaik et al. (1984) (p=0,008) e Kiupel et al. (2011) (p=0,004), sendo que nódulos com piores características prognósticas eram mais frequentes nos animais diagnosticados com múltiplos nódulos, independente da classificação histológica empregada. Também foi observada a associação entre a presença de nódulos múltiplos com estadio II ou IV (p<0,001) e com a presença de recidiva (p=0,001). Em relação à classificação histológica de Kiupel et al. (2011) foi possível observar associação estatística entre o grau histológico e o tamanho do tumor (p=0,001), a consistência (p=0,028) e a velocidade de crescimento do nódulo (p<0,001); aumento de linfonodo (p<0,001) e o tratamento empregado (p=0,002). Nódulos com tamanho superior a três centímetros (56,8%, n=21) e de crescimento rápido (52,9%, n=36%), eram mais frequentes nos tumores de alto grau. Aumento de linfonodo (80,6%, n=25) e tratamento quimioterápico (83,3%, n=5) também estavam associados a pacientes com alto grau histológico. Em relação à classificação de Patnaik et al. (1984), as variáveis raça e sexo diferiram proporcionalmente entre os grupos histológicos (p=0,008 e 0,040 respectivamente), sendo que o aumento do linfonodo e o crescimento rápido do tumor também tiveram associação com o grau histológico (p=0,002 e 0,001). Quase a metade dos pacientes (47,4%) foi submetidas ao tratamento cirúrgico e quimioterápico (n=91), sendo que em 31,3% (n=60) dos pacientes realizou-se apenas o procedimento cirúrgico. Sessenta e um animais (31,8%) apresentaram aumento de tamanho em linfonodos regionais (31,8%), sendo que 71,9% dos nódulos avaliados tiveram crescimento rápido. O tempo de evolução clínica do tumor foi menor ou igual a um ano, em 158 pacientes (82,3%), seguido por 25 pacientes (13%) com evolução de 1 a 2 anos. Foram observados 66 casos (40,5%) de recidiva e 16 (8,3%) de metástase à distância. Com este estudo, pode-se concluir que cães sem raça definida são os mais acometidos pelo mastocitoma cutâneo, na região de Jaboticabal, seguido por cães da raça Boxer, com idade variando entre 7 e 11 anos, não sendo observada predileção sexual; os nódulos encontrados eram predominantemente não ulcerados, solitários e menores que três centímetros; a maioria dos pacientes apresentou mastocitoma grau II, segundo Patnaik et al. (1984) e baixo grau segundo Kiupel et al. (2011); não se observou recidiva, em sua maioria, nem metástase em linfonodos e/ou a distância; o diagnóstico mais utilizado foi a punção aspirativa por agulha fina associada à histopatologia; a localização dos nódulos era mais prevalente nos membros pélvicos e torácicos, seguido por cabeça e pescoço. A consistência dos nódulos era de apresentação firme ou macia; e a metade dos pacientes foi submetida ao procedimento cirúrgico associado à quimioterapia antineoplásica.
ABSTRACT: This study analyzed the cases of cutaneous mastocytomas in dogs seen at the Governador Laudo Natel Veterinary Hospital of the College of Agrarian and Veterinary Sciences (FCAV), Unesp, in Jaboticabal, from 2005 to 2015. The objective was to determine the occurrence of this neoplasm regarding breed, sex, and age, the most affected body parts and observed neoplastic characteristics; the most widely used diagnosis methods, the main histological grades and recommended treatments, to evaluate size changes in relation to lymph nodes, recurrence rate, metastasis and to verify possible associations between the evaluated variables. The clinical files of 192 patients were evaluated. The most affected dogs were of mixed breed (n=46, 23.96%), followed by Boxer (n=40, 20.83%) and Pitbull (n=27, 14.06%). Most of the dogs were females (n=107, 55.7%), aged between 7 and 11 years old (n=120, 62.5%). Thirty-two patients had (16.7%) tumors located in the pelvic limb, followed by thoracic limbs (10.4% n=20). Sixty-five animals had ulcerated nodules (33.9%), of which 96 had firm consistency (50%). Seventy-six animals had multiple nodules (39.6%), and 39.1% of the sample (n=75) had nodules larger than three centimeters. The most used diagnosis method was fine needle aspiration together with histopathology (HT), totaling 39.8% of nodules (n=66), followed by HT in 37.3% of the patients (n=62). Regarding the histological variables, 18 patients had grade I (11.8%), 112 grade II (73.7%), and 22 grade III (14.5%) classified according to Patnaik et al. (1984). According to the classification of Kiupel et al. (2011) 37 cases were classified as high grade (35.2%) and 68 as low grade (64.8%). Statistical association was observed when the presence of solitary/multiple nodules and the histological grade were evaluated according to Patnaik et al. (1984) (P=0.008) and Kiupel et al. (2011) (P=0.004). The nodules with worse prognostic feature were more frequent in animals diagnosed with multiple nodules, regardless of the histological classification used. The presence of multiple nodules in stage II or IV (p<0.001) and recurrence of the disease (p=0.001) was also observed. Regarding the histological classification of Kiupel et al. (2011), there was a statistical association between histological grade and tumor size (p=0.001), consistency (p=0.028) and nodule growth rate (p<0.001), lymph node enlargement (p<0.001) and the treatment used (p=0.002). Nodules larger than three centimeters (56.8% n=21) and fast growing (52.9% n=36%) were more frequent in high-grade tumors. Lymph node enlargement (80.6% n=25) and chemotherapeutic treatment (83.3% n=5) were also associated with patients with this histological grade. According to the classification of Patnaik et al. (1984), breed and sex differed proportionally between the histological groups (p=0.008 and 0.040 respectively), and lymph node enlargement and rapid tumor growth were also associated with histological grade (p=0.002 and 0.001). Almost half of the patients (47.4%) underwent surgical and chemotherapeutic treatment (n=91) while only surgical procedure was performed in 31.3% (n=60) of the patients. Sixty-one animals (31.8%) had enlarged regional lymph nodes (31.8%), and 71.9% of the nodes evaluated had rapid growth. The clinical evolution time of the tumor was less than or equal to one year in 158 patients (82.3%), followed by 1 to 2 years in 25 patients (13%). There were 66 cases (40.5%) of recurrence and 16 (8.3%) of distant metastasis. With this study, it can be concluded that dogs with mixed breed are the most affected by cutaneous mastocytoma in Jaboticabal city, followed by Boxer dogs, with ages varying between 7 and 11 years, with no sexual predisposition observed. It was also concluded that nodules were predominantly non-ulcerated, solitary and smaller than 3 centimeters, that the majority of patients presented grade II mastocytoma according to Patnaik et al. (1984) and low grade according to Kiupel et al. (2011), not being observed in most of them recurrence, lymph node or distance metastasis. The most commonly used diagnosis was fine-needle aspiration associated with histopathology, and in relation to location, nodules were found predominantly in pelvic and thoracic limbs, followed by head and neck. The consistency of the nodules was both firm and soft, and half of the patients underwent the associated surgical procedure of antineoplastic chemotherapy.
