Abstract The iAtlantic Project has established an international collaborative strategy to improve mapping and characterization of deep and open ocean ecosystems in understudied regions of the Atlantic and evaluate their health. In December 2022, the first iAtlantic expedition in the South Atlantic set off to map and explore seafloor ecosystems in the Santos Basin slope (200-1,000 m depths) in collaboration with the Petrobras ‘Santos Basin - Regional Characterization Project.’ The 17-day ‘iAtlantic_BR10-Petrobras’ cruise was conducted on board the research vessel NPqHOc Vital de Oliveira (Brazilian Navy) and performed (a) water column structure characterization, (b) seafloor morphology mapping, (c) description of benthic habitats and communities by seafloor imagery and biological/ geological sampling, and (d) ex-situ experiments to assess the functioning of sedimentary ecosystems and their responses to climate-related environmental changes. This study describes the rationale behind the iAtlantic_BR10-Petrobras cruise science plan, reports its sampling strategy and methods, and summarizes its collected data and preliminary results.

Resumo Fundamento: Principal causa de morte em todo o mundo, as doenças cardiovasculares (DCV) e sua prevalência nos médicos cardiologistas são pouco conhecidas. Objetivos: Descrever os hábitos de vida e os fatores de risco cardiovascular e verificar a prevalência de diagnóstico, conhecimento e controle dos fatores de risco cardiovasculares (FRCV) de médicos cardiologistas associados e especialistas pela Sociedade Brasileira de Cardiologia. Métodos: Estudo multicêntrico nacional transversal que avaliou cardiologistas brasileiros por meio de questionário sobre hábitos de vida, doenças preexistentes, medicações em uso, medidas antropométricas, pressão arterial e dosagens de glicose e lípideos sanguíneos. Resultados: Foram avaliados 555 cardiologistas, 67,9% do sexo masculino, média de idade de 47,2±11,7 anos. A maioria era não tabagista (88,7%), fisicamente ativa (77,1%), consumia bebida alcóolica (78,2%), com circunferência abdominal normal (51,7%) e excesso de peso (56,1%). As prevalências de hipertensão arterial sistêmica (HAS), diabetes mellitus (DM) e dislipidemia (DLP) foram de 32,4%, 5,9% e 49,7%, respectivamente e, destes, apenas 57,2%, 45,5% e 49,6% sabiam ter as doenças. Conclusões: Os cardiologistas brasileiros participantes do estudo apresentaram prevalências significativas de HAS, DM e DLP, mas apenas a metade dos participantes sabia ser portador dessas condições e, entre eles, as taxas de controle eram baixas para HAS e DLP, apesar de os cardiologistas serem profissionais detentores de conhecimento diferenciado sobre esses FRCV. Os achados representam um alerta para a abordagem dos FRCV em cardiologistas brasileiros e estimulam a realização de estudos futuros.
Abstract Background: A major cause of death worldwide, cardiovascular diseases and their prevalence in cardiologists are little known. Objectives: To describe life habits and cardiovascular risk factors (CVRF) and to investigate the prevalence of diagnosis, awareness, and control of these CVRF among cardiologists members affiliated to and specialists from the Brazilian Society of Cardiology. Methods: National multicenter cross-sectional study to assess Brazilian cardiologists using a questionnaire on life habits, preexisting diseases, current medications, anthropometric measurements, blood pressure, and levels of glucose and lipids. Results: A total of 555 cardiologists were evaluated, of which 67.9% were male, with a mean age of 47.2±11.7 years. Most were non-smoker (88.7%) and physically active (77.1%), consumed alcohol (78.2%), had normal weight circumference (51.7%), and were overweight (56.1%). The prevalence of systemic arterial hypertension (SAH), diabetes mellitus (DM), and dyslipidemia (DLP) were 32.4%, 5.9%, and 49.7%, respectively, of which only 57.2%, 45.5%, and 49.6%, respectively, were aware of the diseases. Conclusions: The Brazilian cardiologists participating in the study had a high prevalence of SAH, DM and DLP, but only a half of participants were aware of these conditions and, among these, the rates of controlled disease were low for SAH and DLP, although cardiologists are professionals with great knowledge about these CVRF. These findings represent a warning sign for the approach of CVRF in Brazilian cardiologists and encourage the conduction of future studies.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Resumo Fundamento: Os efeitos da caminhada de longa distância sobre o sistema cardiovascular são pouco estudados. Objetivos: O objetivo geral deste estudo foi verificar esses efeitos sobre o comportamento da função diastólica e dos biomarcadores cardíacos CK-MB (massa), troponina T e NT-proBNP em atletas amadores. Método: Este estudo longitudinal realizado em 2015 avaliou os participantes nas 5 etapas seguintes: A0 (basal) antes de iniciar o percurso, e as demais, A1, A2, A3 e A4 ao final de cada dia, totalizando 244,7 km. Em todas as etapas foram dosados os biomarcadores NT-proBNP, CK-MB (massa) e troponina T. Realizou-se ecocardiograma para análise das ondas E, A e E'. Adotado p < 0,05 como significativo. Resultados: Foram avaliados 25 participantes com média de idade de 46 ± 10,5 anos e índice de massa corporal de 20,2 ± 2,3 kg/m2. Encontrou-se aumentos dos valores de NT-proBNP de A0 para A1, A2, A3 e A4 (p < 0,001), CK-MB (massa) de A0 para A2 (p < 0,001) e da onda E' de A0 para A1, A2, A3 e A4 (p < 0,001). Foram identificadas correlações entre os seguintes: CK-MB (massa) e troponina T (A1: r = 0,524, p = 0,010; A4: r = 0,413, p = 0,044); CK-MB (massa) e NT-proBNP (A4: r = 0,539, p = 0,006); e E/A e E' (A0: r = 0,603, p < 0,001; A1: r = 0,639, p < 0,001; A4: r = 0,593, p = 0,002); e correlação negativa entre CK-MB (massa) com E/A (A1: r = −0,428, p = 0,041). Conclusão: Os efeitos da atividade física intensa, prolongada e intercalada foram verificados a partir das variações significativas no comportamento da CK-MB (massa), NT-proBNP e E'. Apesar das alterações encontradas, não houve critérios sugestivos de dano ao miocárdio.
Abstract Background: The effects of long-distance walking on the cardiovascular system have been little studied. Objectives: The general objective of this study was to verify these effects on the behavior of diastolic function and the cardiac biomarkers CK-MB (mass), troponin T, and NT-proBNP, in amateur athletes. Method: This longitudinal study, conducted in 2015, evaluated participants during the following 5 stages: E0 (baseline) before starting the trajectory and the others, E1, E2, E3, and E4, at the end of each day, totaling 244.7 km. Increased values were found for NT-proBNP from E0 to E1, E2, E3, and E4 (p < 0.001), CK-MB (mass) from E0 to E2 (p < 0.001), and E' wave from E0 to E1, E2, E3, and E4 (p < 0.001). Positive correlations were identified between the following: CK-MB (mass) and troponin T (E1: r = 0.524, p = 0.010; E4: r = 0.413, p = 0.044); CK-MB (mass) and NT-proBNP (E4: r = 0.539, p = 0.006); and E/A and E' (E0: r = 0.603, p < 0.001; E1: r = 0.639, p < 0.001; E4: r = 0.593, p = 0.002). A negative correlation was found between CK-MB (mass) and E/A (E1: r = −0.428, p = 0.041). Conclusion: The effects of intense, prolonged, and interspersed physical activity were verified based on significant variations in the behavior of CK-MB (mass), NT-proBNP, and the E' wave. New diesters derived from phthaloylglycine (7a-7i) were synthesized and their structures characterized by infrared, 1H and 13C nuclear magnetic resonance (NMR) spectroscopy. The compounds were evaluated in an in silico study, which demonstrated positive features indicating a possible drug candidate. The diesters showed antifungal activity ranging from moderate to strong against strains of Candida. Compounds 7a, 7b, 7c, 7e and 7i had a moderate minimum inhibitory concentration (MIC) of 1024 µg mL−1 against all fungal strains, while 7h showed a very good MIC of 256 µg mL−1 against Candida albicans, Candida parapsilosis and Candida krusei and 64 µg mL−1 against Candida tropicalis. However, only 7h and 7i were able to inhibit bacterial growth of strains of Staphylococcus aureus, Staphylococcus epidermidis, Pseudomonas aeruginosa and Escherichia coli with an MIC of 1024 µg mL−1.

Resumo Fundamento Os efeitos da redução na ingestão do sal sobre a pressão arterial (PA) casual de hipertensos já foram amplamente estudados, entretanto essa análise ainda é escassa no contexto da redução exclusiva do sal de adição na rigidez arterial e em indivíduos normotensos e pré-hipertensos. Objetivo Avaliar os efeitos da redução progressiva na ingestão do sal de adição (de 6 para 4 g/dia) sobre os valores da pressão periférica e central, a rigidez arterial em normotensos, pré-hipertensos e hipertensos. Métodos Ensaio clínico, simples cego com 13 semanas de seguimento. Foram avaliados normotensos (≤130/85 mmHg), pré-hipertensos (≥130 e <139/≥85 e <90 mmHg) e hipertensos estágio 1 (≥140 e <160/≥90 e <100 mmHg). Utilizou-se medida casual e monitorização residencial da PA com aparelho automático OMRON 705CP, medida central da PA com Sphygmocor®, dosagem do sódio urinário de 24h (colhido no intervalo entre cada visita) e mensuração de sal de adição. Foi adotado nível de significância p<0,05 para todas as análises. Resultados Foram avaliados 55 participantes (18 normotensos; 15 pré-hipertensos; 22 hipertensos) com mediana 48 anos (IQ:39-54). Os grupos foram semelhantes em relação a idade e sexo. Não houve diferença entre medidas de PA e excreção de sódio antes e depois da intervenção. Os parâmetros de rigidez arterial também não sofreram alterações significativas. Conclusão A redução gradativa da ingestão de sal de adição num seguimento de 13 semanas não foi capaz de reduzir de maneira significativa os valores periféricos e centrais da PA. Abstract Background Although the effects of salt intake reduction on casual blood pressure have been extensively studied in hypertensive individuals, data on reductions of added salt on arterial stiffness in both normotensive and prehypertensive subjects are scarce. Objective To evaluate the effects of progressive reduction in added salt intake (from 6 grams to 4 grams per day) on peripheral and central blood pressure and arterial stiffness in normotensive, prehypertensive and hypertensive individuals. Methods This was a single-blinded clinical trial with 13 weeks of follow-up. Normotensive (≤ 130/85 mmHg), prehypertensive (≥ 130 e < 139/≥ 85 e < 90 mmHg) and stage 1 hypertensive individuals (< 139/≥ 85 and < 90 mmHg) were assessed. Casual blood pressure measurements and ambulatory blood pressure monitoring were performed using the automated OMRON 705CP device, and central blood pressure was measured using the Sphygmocor®. Twenty-four-hour urinary sodium excretion and the amounts of added salt consumed were measured. Statistically significance level was set at p < 0.05 for all analysis. Results A total of 55 participants (18 normotensive, 15 prehypertensive and 22 hypertensive), median age 48 years (IQR:39-54) were studied. The groups were not different in age or sex. No difference was observed in blood pressure or sodium excretion levels before and after the intervention. No significant changes in arterial stiffness parameters were observed. Conclusion The progressive reduction in added salt intake during a period of 13 weeks did not cause significant reductions in peripheral and central blood pressure. Abstract Candida is a human fungal pathogen that causes a wide range of diseases. Candida albicans is the main etiologic agent in these diseases; however, infections can be caused by non-albicans Candida species. Virulence factors such as biofilm production, which protect the fungus from host immunity and anti-fungal drugs, are important for the infection. Therefore, available antifungal drugs for candidiasis treatment are limited and the investigation of new and effective drugs is needed. Verapamil is a calcium channel blocker with an inhibitory effect on hyphae development, adhesion, and colonization of C. albicans. In this study, we investigated the effect of verapamil on cell viability and its antifungal and anti-biofilm activity in non-albicans Candida species. Verapamil was not toxic to keratinocyte cells; moreover, C. krusei, C. parapsilosis, and C. glabrata were susceptible to verapamil with a minimal inhibitory concentration (MIC) of 1250 μM; in addition, this drug displayed fungistatic effect at the evaluated concentrations. After treatment with verapamil, reduced viability, biomass, and mitochondrial activity were observed in biofilms of the non-albicans Candida species C. krusei, C. glabrata, and C. parapsilosis. These findings highlight the importance of the study of verapamil as an alternative treatment for infections caused by non-albicans Candida species.

Abstract Aim: To Verify the effects of hydrogymnastics on functional autonomy in elderly women. Method: In November 2018, a systematic review of the literature was performed in the databases MEDLINE, SciELO, LILACS, PEDro, Cochrane, Web of Science, SPORTDiscus, SCOPUS e CINAHL. Two independent researchers selected randomized clinical trials, which evaluated the effects of hydrogymnastics over healthy elderly women performance on functional tests performance on functional tests. From each study they selected the number of participants in both experimental and control groups (EG and CG), age, intervention protocol, mean and standard deviation before and after the intervention for the following variables: legs and biceps muscular strength, posterior muscle chain extensibility, and agility - all of which compose the functional autonomy on Rikli and Jones test. We analyzed the methodological quality and the risk of bias through the Jadad Scale and the Cochrane tool respectively. We performed the data analysis through the random effects model and the mean difference between CG and EG. The analysis of the publication bias was done with Egger Test. Results: We found a total of 887 studies in the aforementioned databases, and five randomized clinical trials were included in the present meta-analysis. In spite of the evidence level is very low, the hydrogymnastics promoted an increase in agility and leg muscle strength in elderly women.

RESUMO As características regionais das águas dos aquíferos aluvionais são diferentes, uma vez que dependem da geologia e do clima. Também pode haver variação local da qualidade da água disponível quando a fonte hídrica é superficial (rios e lagos) ou subterrânea em função da geologia. Objetivando investigar a qualidade da água subterrânea do aquífero aluvial na Sub-Bacia do Rio das Cobra (RN) para fins de construção de barragens subterrâneas, foram monitorados 5 poços amazonas, 4 no rio principal e 1 do afluente, em épocas de estiagem e chuvosa, durante 2 anos hidrológicos. Para caracterização hidroquímica do aquífero foram coletadas 15 amostras de água dos poços amazonas. A origem da recarga dos aquíferos e avaliação hidroquímica da água foram avaliadas utilizando-se a análise estatística multivariada. Os resultados da avaliação hidroquímica indicaram que os íons Na+ e Cl- são mais influentes na salinização da área estudada. Há riscos de agravamento dos problemas de salinidade e toxicidade no médio e baixo curso da Sub-Bacia do Rio Cobra.
ABSTRACT The regional characteristics of waters from alluvial aquifers are different, since they depend on geology and climate. Also, there may be local variation in the quality of the available water when the water source is superficial (rivers and lakes) or is underground due to geology. In order to investigate the groundwater quality of the alluvial aquifer in the Sub-Basin of the Cobra River (RN) for construction of underground dams, five wells, four in the main river and one in the tributary, were monitored in dry and rainy seasons, during two hydrological years. Fifteen water samples from ‘Amazonas’ wells were collected for hydrochemical assessment. Multivariate analysis were performed to evaluate the origin of aquifer recharge and water hydrochemical characteristics. Hydrochemical assessment results indicated that Na+ and Cl- ions have greater influence on the salinization of the studied area. There are risks of worsening salinization and toxicity problems in the middle and lower courses of the Cobra River Sub-Basin.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Diagnóstico imuno-histoquímico e caracterização anatomopatológica de Mycoplasma gallisepticum em galinhas de subsistência

A micoplasmose aviária é causada por bactérias da família Mycoplasmataceae. Mycoplasma gallisepticum (MG) é a espécie mais patogênica e que tem a maior importância econômica para a produção avícola. Este estudo teve por objetivo utilizar a técnica de imuno-histoquímica (IHQ) como método de diagnóstico da infecção por MG em aves. No presente relato são descritos dois surtos de micoplasmose por MG em galinhas de subsistência. Clinicamente as aves apresentaram prostração, hiporexia, dificuldade respiratória, secreção nasal e ocular. Na necropsia foram observados secreção serosa, edema e deposição de cáseo em conjuntiva (7/10) e seios nasais (4/10), sacos aéreos espessados com espuma e cáseo (6/10); traqueia difusamente avermelhada (4/10); pulmões com pontos esbranquiçados de 0,5cm (2/10); e saco pericárdico com deposição de fibrina (2/10). No exame histopatológico foram evidenciados traqueíte (10/10), sinusite (5/5) e conjuntivite (3/4) hiperplásica linfoplasmocitária aguda; broncopneumonia fibrinonecrótica (5/10); pericardite fibrinosa aguda (2/10); e aerossaculite fibrinonecrótica (1/1). No exame de IHQ anti-MG foi evidenciada marcação na superfície extracelular dos cílios e/ou topo do epitélio da traqueia (10/10), brônquios (5/10) e seios nasais (4/5). Em sete dos dez casos analisados foi detectada a presença de MG por PCR em tempo real realizado a partir de amostras de suabe traqueal. A técnica de IHQ anti-MG utilizada como método de diagnóstico apresentou boa concordância com os sinais clínicos, as lesões histopatológicas e os resultados de PCR em tempo real.
Avian mycoplasmosis is caused by bacteria from the Mycoplasmataceae family. Mycoplasma gallisepticum (MG) is the most pathogenic and economically significant species affecting poultry. The aim of this study was to use the immunohistochemistry technique (IHC) as a diagnostic method for the MG infection in poultry. In this report we described two outbreaks of mycoplasmosis caused by MG in free-range chickens. Clinical signs were characterized by prostration, decreased appetite, difficult breathing, nasal and ocular discharge. Necropsy findings were serous secretion in conjunctiva (7/10) and seioses (4/10), edema and caseous exudate; air sacs thickened with foam and caseous exudate (6/10); trachea diffusely reddish (4/10); lungs with 0.5 cm whitish spots (2/10); and pericardial sac with fibrin exudate (2/10). Histologically was observed a lymphoplasmacytic hyperplastic acute tracheitis (10/10), seiositis (5/5) and conjunctivitis (3/4); fibrinonecrotic bronchopneumonia (5/10); acute fibrinous pericarditis (2/10); and fibrinonecrotic aerosaculitis (1/1). IHC anti-MG stained in the extracellular surface of ciliated brush border and/or in the top of epithelium of trachea (10/10), bronchi (5/10) and sinuses (4/5). In seven out of ten cases it was possible to detect MG by real-time PCR from tracheal swabs. IHC anti-MG used as a diagnostic method showed good correlation with clinical signs, lesions and real-time PCR results.

Currently, algae and algae products are extensively applied in the pharmaceutical, cosmetic and food industries. Algae are the main organisms that take up and store heavy metals. Therefore, the use of compounds derived from algae by the pharmaceutical industry should be closely monitored for possible contamination. The pollution generated by heavy metals released by industrial and domestic sources causes serious changes in the aquatic ecosystem, resulting in a loss of biological diversity and a magnification and bioaccumulation of toxic agents in the food chain. Since algae are at the bottom of the aquatic food chain, they are the most important vector for transfer of pollution to upper levels of the trophic chain in aquatic environments. Moreover, microalgae are also used for the bioremediation of wastewater, a process that does not produce secondary pollution, that enables efficient recycling of nutrients and that generates biomass useful for the production of bioactive compounds and biofuel.

In this work we described the cultivation of Chlorella vulgaris in a photobioreactor to algal biomass production. The dried biomass was used as feedstock for biodiesel production, it presented 26% lipids and via sonocatalysis stage of the methodology resulted in 60% of fatty acid methyl esters (FAME). The FAME content was confirmed by Gas Chromatography (GC).

OBJECTIVE: To assess the effect of blood pressure (BP) control and other cardiovascular risk factors in patients with diabetes mellitus in a referral service for the treatment of hypertension. METHODS: A retrospective study where diabetic patients (at least 2 fasting glucose levels above 126 mg/dL, use of hypoglycemic agents or insulin, or both of these) were included. They were evaluated at the first appointment (M1) and at the last appointment (M2), regarding blood pressure, body mass index (BMI), use of hypertensive drugs, glycemia, total cholesterol (TC), creatinine, and potassium. RESULTS: Of 1,032 patients studied, 146 patients with a mean age of 61.6 years had diabetes, and 27 were men (18.5%). Mean follow-up was 5.5 years. BP values were 161.6 x 99.9 mmHg in M1 and 146.3 x 89.5 mmHg in M2. In M1, 10.4% of the patients did not use medications, 50.6% used just 1 drug, 30.8% used 2 drugs, and 8.2% used 3 or more drugs. In M2, these values were 10.9%, 39%, 39.7%, and 10.4%, respectively. Diuretics were the most commonly used medication, whereas angiotensin-converting enzyme inhibitors (ACE inhibitors) were those drugs which presented greater increase when comparing M1 to M2 (24.6% and 41.7%, respectively). Only 17,1% reached the recommended goal (BP<130x85 mmhg). The other cardiovascular risk factors did not change significantly. CONCLUSION: Our data reinforce the necessity of a more aggressive approach in the treatment of these patients, despite the social and economic difficulties in adhering to treatment.

A febre maculosa brasileira foi detectada pela primeira vez no Estado de São Paulo em 1929. No entanto, não há registro sistemático de casos neste Estado. Em 1985 ocorreram três casos desta doença no Município de Pedreira, situado na região de Campinas, que fica no nordeste do Estado de São Paulo, Brasil, correspondendo à 5ª Região Administrativa, e compreendendo 88 municípios. Alguns estudos foram realizados no Município de Pedreira, mas a falta de registro de casos foi um obstáculo encontrado. Com a finalidade de recuperar o histórico da doença, resolveu-se pesquisar e registrar as ocorrências de febre maculosa na região no período de 1985 a 2000 e analisar o seu comportamento. Foram recuperados todos os registros da doença nos diversos serviços de saúde pública. Observou-se uma ampliação da área de transmissão e a ocorrência de um aumento dos casos suspeitos a partir de 1996, ano em que a doença foi determinada como de notificação compulsória na região. Esta doença foi causa de óbito na maioria dos anos do período de estudo. Conclui-se que a febre maculosa está em ascensão na região e estudos bioecológicos complementares estão sendo desenvolvidos para melhor compreensão da epidemiologia dessa doença, que é mundialmente reconhecida como um problema emergente de saúde pública.
Brazilian spotted fever was detected for the first time in the State of São Paulo in 1929. However, there is no systematic reporting of the disease in the State. In 1985, three cases of the disease occurred in the municipality of Pedreira, located in the Campinas Region, belonging to the 5th Administrative Region, in the Northeast part of the State, including 88 municipalities. An investigation was conducted at the time, but the lack of case registry limited its scope. The present study was undertaken with the aim of recovering the history of the disease in the Region. Data recovered from several public health services for 1985-2000 were used to analyze incidence patterns. It was observed that the transmission area expanded and the number of suspected cases increased, especially after 1996, when mandatory reporting was established. Deaths due to spotted fever were observed in most of the years under study. The study concluded that spotted fever incidence is increasing in the Campinas Region. Complementary bio-ecological studies are currently under way to better understand the epidemiology of this disease, recognized worldwide as an emerging public health problem.
