Cisticercose do IV ventrículo. Considerações anátomo-clínicas e sôbre a terapêutica cirúrgica

The AA. present a general view of the anatomo-clinical problem of the neurocysticercosis. There are some consideration on the local and general reactions of the cysticercos on the central nervous system. They classified the clinical aspects on the following groups: convulsive, with or without mental disturbances, pseudo-tumoral, hypertensive, apopletic and ventricular. The ventricular group is characteristic and with the spinal fluid it is possible a clinical diagnosis. The surgical therapy is discussed based on 4 cases and the general conclusion as results is pessimistic. The AA. believe that the result will depend of an early diagnosis, before the local and general reaction appears, and of the rare possibility of an isolated cysticercus. The first case was a typical 4th. ventricle syndrome, but at operation was found cysticercus in the 4th. ventricle, plus arachnoiditis of posterior fossa; follow-up showed periods of intracranial hypertension and remission until death 20 months after operation. The 2nd. case showed an acute hypertension with sudden death and at necropsy only one cysticercus in the 4th. ventricle with intensive local reaction was found. The 3rd case with cysticercrdus in the 4th. ventricle, but mental signs due probably to a multiple cysticercosis, or meningoencephalitis, had improvement only of the hypertensive signs. The 4th. case - cysticercosis of 4th. ventricle plus arachnoiditis of the cisterna lateralis - had improvement of the hypertensive signs, but no possible follow-up.
