This study aimed to analyze the medication system in a specialized hospital in Rio de Janeiro. This is an evaluation study of a descriptive nature. We selected some of the key processes of the medication system: prescribing, dispensing, preparation and administration of medicines. In order to describe the system of medication were used for data collection: interviews, non-participant observation and analysis of medical records. We identified a large number of steps and involved professionals, and problems related to safety and the environment during the prescribing, dispensing, preparation and administration of medicines. Insecure systems can generate different consequences for patients and health institutions, in this sense is necessary to implement strategies aimed at changing the culture of detecting faults in order to help professionals in the prevention of errors through measures that ensure quality of procedures performed.
