OBJECTIVE: The transperitoneal approach to the abdominal aorta for surgery of either occlusive or aneurysmal disease is undoubtedly the most often used approach, although the retroperitoneal approach is frequently mentioned as being less prone to complications. The aim of this study was to compare both approaches regarding late complications. METHOD: Late results were retrospectively studied in 96 patients (81 cases of aortic aneurysm and 15 cases of obstructive disease). The retroperitoneal approach was used in 43 cases and the transperitoneal in 53. RESULTS: There were no significant differences in the occurrence of incisional hernia, abdominal bulging, and retrograde ejaculation. CONCLUSIONS: There were no differences between the two procedures regarding the parameters considered for late assessment.
