Polimorfismos nos genes VEGF e KDR no desenvolvimento da endometriose: revisão sistemática

Resumo

Objetivos:

revisar os trabalhos que utilizaram o delineamento caso-controle para verificar a associação de polimorfismos nos genes VEGF e KDR no desenvolvimento da endometriose.

Métodos:

revisão sistemática que pesquisou nas bases de dados PubMed, MEDLINE, BVS, SciELO os artigos publicados até o dia 1 de setembro de 2015, considerando os descritores: endometriosis and ("polymorphism" or "SNP" or "geneticpolymorphism") and ("VEGF" or "Vascular endothelial growth factor" or "VEGFR-2" or "Vascular endothelial growth factor-2" or "KDR" or "Kinase Insert Domain Receptor").

Resultados:

identificou-se 106 artigos, sendo 11 considerados elegíveis. Observou-se resultados discrepantes quanto aos polimorfismos no gene VEGF no desenvolvimento da endometriose, podendo ser explicados pelas diferenças metodológicas, pelo tamanho amostral, pelo tipo de controle elegível, pela utilização da estimativa de risco não ajustada e pela heterogeneidade das populações estudadas. Apenas um estudo investigou polimorfismos no gene KDR no desenvolvimento da endometriose, contudo não foi elegível nesta revisão.

Conclusões:

para evitar as discrepâncias dos resultados observados sugerimos que o grupo controle ideal deva ser formado por mulheres férteis e livres de doenças ginecológicas. Estudos multicêntricos com delineamento adequado, envolvendo diferentes populações, além da análise combinada de polimorfismos nos genes VEGF e KDR ainda são necessários para contribuir no entendimento desta doença que é um problema social, clínico e econômico.

Abstract

Objectives:

to review studies that used case-control design to verify the association of polymorphisms in VEGF and KDR genes in the development of endometriosis.

Methods:

the systematic review selected articles published until September 1, 2015 from PubMed, MEDLINE, BVS, SciELO databases, considering the following key words: endometriosis and ("polymorphism" or "SNP" or "genetic polymorphism") and ("VEGF" OR "Vascular endothelial growth factor" or "VEGFR-2" or "Vascular endothelial growth factor-2" or "KDR" or "Kinase Insert Domain Receptor").

Results:

106 articles were identified, only 11 were eligible. Discrepant results were observed regarding polymorphisms in VEGF gene in the development of endometriosis, which can be explained by methodological differences, sample size, eligible control type, using the unadjusted risk estimates and the heterogeneity of the studied population. Only one study investigated polymorphisms in KDR gene in the development of endometriosis, however it was ineligible for this review.

Conclusions:

to avoid discrepancy in the results, we suggest that the ideal control group should be formed by fertile women and free of gynecological diseases. Multicentric studies with adequate design, involving different population besides the combined analysis on polymorphisms in VEGF and KDR genes are still necessary to contribute in the understanding of this disease, which are social, clinical and economical problems.

Introdução

A endometriose é uma doença ginecológica, caracterizada pela presença de tecido endometrial funcional fora da cavidade uterina, representando um dos distúrbios ginecológicos benignos mais comuns.11 Giudice LC. O real perfil das pacientes com endometriose é impreciso, embora haja consenso de que ela esteja presente em pelo menos 10% das mulheres em idade reprodutiva,22 Giudice LC, kao LC. Endometriosis. As mulheres portadoras de endometriose podem ser assintomáticos (10,7%), no entanto, a maioria apresenta sintomas, em diferentes intensidades, sendo os principais: dismenorreia (52-97%), dor pélvica crônica (22-69%), infertilidade (25-59%), dispareunia de profundidade (44-71%) e sintomas intestinais (71%) e urinários cíclicos (60%).66 Fauconnier A, Staraci S, Roman Chh, Panel P, Descamps P. Várias são as teorias para explicar o surgimento da endometriose, entretanto sua etiologia ainda não está clara. A teoria mais aceita até hoje é a proposta por Sampson,1010 Sampson JA. Peritoneal endometriosis due to the menstrual dissemination of endometrial tissue into the peritoneal cavity. Am J Obstet Gynecol. 1927; 14: 422-69. que descreveu que o tecido endometrial, por fluxo menstrual retrógrado, retorna pelas tubas uterinas e adere à cavidade peritoneal. Para que isso ocorra, o processo de angiogênese, caracterizado pelo crescimento de novos vasos a partir de vasos pré-existentes, é essencial.1111 Rocha ALL, Reis FM, Taylor RN. Dentre os fatores pró-angiogênicos destaca-se o fator de crescimento edotelial vascular (VEGF), que assume um importante papel no desenvolvimento da endometriose.1111 Rocha ALL, Reis FM, Taylor RN. Em 2008, nosso grupo observou um aumento da distribuição de VEGF e seu receptor, VEGFR-2, em amostras de endometriose de ovário, bexiga e no reto sigmóide, quando comparadas ao seu controle, sendo que a maior distribuição de VEGF e VEGFR-2 foi observada na endometriose do reto-sigmóide, considerada a mais grave.1212 Machado DE, Abrão MS, Berardo PT, Takiya CM, Nasciutti LE. O VEGF é codificado por um gene com o mesmo nome, localizado no cromossomo 6p21.3, composto por oito éxons,1313 Vincenti V, Cassano C, Rocchi M, Persico G. Assignment of the vascular endothelial growth factor gene to human chromosome 6p21.3. Circulation. 1996; 93: 1493-5. enquanto que o seu receptor, VEGFR-2, é codificado pelo gene KDR (receptor do domínio de inserção da cinase), localizado no cromossomo 4q11-q12, sendo composto por trinta éxons.1414 Lu D, Kussie P, Pytowski B, Persaud K, Bohlen P, Witte L, Zhu Z. Ambos os genes são polimórficos, destacando-se no estudo da endometriose cinco polimorfismos de um único nucleotídeo (SNPs) no gene VEGF (rs699947, rs833061, rs1570360, rs2010963 e rs3025039) e três no gene KDR (rs1870377, rs2305948 e rs2071559), em virtude da alta frequência em que ocorrem nas diferentes populações, além de todos interferirem de forma significativa na atividade ou expressão da enzima.1515 Renner W, Kotschan S, Hoffmann C, Obermayer-Pietsch B, Pilger E. Recentemente, nosso grupo observou uma associação positiva para o SNP VEGF rs1570360 no desenvolvimento da endometriose (OR: 1,90; IC95%: 1,23 - 2,97) em mulheres brasileiras.88 Perini JA, Cardoso JV, Berardo PT, Vianna-Jorge R, Nasciutti LE, Bellodi-Privato M, Machado DE, Abrão MS. Entretanto, os resultados das análises ainda são controversos. Nesse contexto, o objetivo deste estudo foi revisar todos os trabalhos que utilizaram o delineamento de estudo do tipo caso-controle para investigar a magnitude de associação de SNPs dos genes VEGF e/ou KDR na susceptibilidade de desenvolver a endometriose, com o intuito de discutir as possíveis causas dos resultados controversos já descritos.

Métodos

Para identificar todos os estudos publicados até o dia 1 de setembro de 2015, sobre as associações entre SNPs de VEGF e/ou KDR e o desenvolvimento da endometriose foi realizada uma avaliação dos títulos e dos resumos (abstracts) dos estudos encontrados por dois pesquisadores, de forma independente e "cegada", na base de dados do PubMed, MEDLINE, BVS e SciELO, utilizando os seguintes descritores: ("endometriosis" OR "endometriose") AND ("polymorphism" OR "polimorfismo" OR "SNP" OR "genetic polymorphism" OR "polimorfismos genéticos") AND ("VEGF" OR "Vascular endotelial growth factor" OR "Fator de crescimento endotelial vascular" OR "VEGFR-2" OR "Vascular endotelial growth fator-2" OR "Fator de crescimento endotelial vascular-2" OR "KDR" OR "Kinase Insert Domain Receptor" OR "Receptor do domínio de inserção da cinase"). Os estudos foram selecionados seguindo os critérios de inclusão e exclusão, pré- selecionados, conforme descritos a seguir.

Como critério de inclusão para esta revisão foram selecionados estudos que utilizaram um desenho do tipo caso-controle e que avaliaram associações entre polimorfismos do gene VEGF e/ou KDR e o desenvolvimento da endometriose. Quanto ao período de publicação foram incluídos artigos publicados até setembro de 2015, sem restrição de idioma.

Os critérios de exclusão dos estudos foram: (i) ausência de uma população controle; (ii) dados de associações incompletos; (iii) dados em duplicata; (iv) meta-análise, revisão, cartas, comentários ou artigos editoriais; (v) aqueles que não analisaram SNPs no VEGF e/ou KDR; (vi) os que não incluíram pacientes com endometriose; (vii) e aqueles aos quais não tivemos acesso ao texto na íntegra.

Depois de realizada a estratégia de busca na base de dados do PubMed, MEDLINE, BVS e SciELO dos artigos que estudaram SNPs nos genes VEGF e/ou KDR com o desenvolvimento da endometriose, as seguintes informações foram extraídas: primeiro autor; ano de publicação; revista; país em que o estudo foi realizado; número de casos e controles; média de idade dos casos e controles com seus respectivos desvios de padrão e/ou intervalo; critérios de seleção/elegibilidade e exclusão dos casos de endometriose e dos controles; estadiamento da endometriose; fonte dos controle; tipo de SNP; técnica de genotipagem utilizada para identificação dos SNPs estudados; dados de frequência alélica e genotípica dos SNPs e dado estatístico da razão de chances (OR), com seus respectivos intervalos de confiança 95% (IC95%).

Avaliação da qualidade dos estudos incluídos

Dois revisores, independentemente, avaliaram a qualidade metodológica dos 19 estudos incluídos nesta revisão, utilizando os critérios do instrumento STROBE (Strengthening the Reporting of Observational Studies in Epidemiology).3838 Von EE. Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) statement: guidelines for reporting observational studies. Brit Med J. 2007; 355 (7624): 806-8. Vinte e dois itens de avaliação receberam uma pontuação de 0 a 1. Esses itens referem-se ao título e resumo do artigo (item 1), à introdução (itens 2 e 3), aos métodos (itens de 4 a 12), aos resultados (itens de 13 a 17), à discussão (itens de 18 a 21) e à informação sobre o financiamento (item 22). Depois da avaliação de todos os critérios, cada artigo recebeu uma nota de 0 a 22 de cada pesquisador. Para a pontuação final foi realizada uma média das duas notas, sendo transformada em percentual para melhor avaliar a qualidade dos artigos. Os revisores definiram, previamente, que os artigos que atingissem um percentual superior a 50% seriam considerados satisfatórios.

Resultados

A Figura 1 apresenta o fluxograma da seleção dos artigos para esta revisão. A estratégia de busca identificou 100 publicações para o gene VEGF e 6 publicações para o KDR, não tendo sido encontrado nenhum artigo que avaliasse simultaneamente a influência de SNPs em ambos os genes com o desenvolvimento da endometriose. Após a remoção de artigos duplicados, 34 foram selecionados para a leitura dos títulos e resumos. No entanto, foram excluídos 14 estudos: 5 porque eram meta-análises, 2 porque eram trabalhos de revisão, um porque era uma crítica a uma meta-análise publicada anteriormente, um porque explorou SNPs no gene IL-10, em vez de polimorfismos nos genes VEGF e/ou KDR, 4 porque estudaram pacientes com adenomiose e um porque avaliou mulheres com pterígio em vez de pacientes com endometriose. Depois da leitura completa dos 19 artigos elegíveis para esta revisão, foram avaliados os critérios de inclusão e exclusão dos casos e controles, sendo, ainda, excluídos 8 estudos: 5, que utilizaram casos de endometriose sem confirmação histopatológica da doença;2020 Bhanoori M, Arvind Babu K, Pavankumar Reddy NG, Lakshmi Rao K, Zondervan K, Deenadayal M, Kennedy S, Shivaji S. Na Tabela 3 são apresentadas as características dos SNPs investigados nos estudos elegíveis para esta revisão. Foram estudados 5 SNPs no gene VEGF, sendo genotipados pelas técnicas de PCR-RFLP (81,8%) e PCR em tempo real (18,2%). O SNP rs699947 foi avaliado em 2 (18,2%) estudos e a frequência do alelo VEGF -2578A variou entre 19-40% e 26-34% nos casos e controles, respectivamente, sendo que Liu et al.2727 Liu Q, Li Y, Zhao J, Sun DL, Duan YN, Wang N, Zhou RM, Kang S.. Esta revisão apresenta uma descrição detalhada dos trabalhos que utilizaram o delineamento de estudo do tipo caso-controle para investigar a magnitude de associação de SNPs do gene VEGF e/ou KDR na susceptibilidade do desenvolvimento da endometriose. A angiogênese, via sinalização do VEGF-KDR, é um evento importante para o desenvolvimento da doença,1111 Rocha ALL, Reis FM, Taylor RN. Angiogenisis and Endometriosis. O possível efeito do SNP rs7667298, localizado na região 5'-UTR do gene KDR, não está bem claro. No entanto, esse SNP está associado ao SNP rs2071559 (KDR-604T>C), localizado na região promotora. Os SNPs rs1531289 e rs7692791, localizados nos íntrons do gene KDR, ainda não foram estudados em relação aos seus efeitos funcionais.

O papel do VEGF-KDR na fisiopatologia da endometriose, assim como a influência do SNPs em modular os níveis e a atividade destas proteínas justificam o elevado número de estudos que buscaram descrever a magnitude de associação dos SNPs nos genes VEGF e KDR na susceptibilidade do desenvolvimento da endometriose.88 Perini JA, Cardoso JV, Berardo PT, Vianna-Jorge R, Nasciutti LE, Bellodi-Privato M, Machado DE, Abrão MS. Para o KDR, apenas um estudo do tipo caso-controle avaliou a magnitude de associação entre cinco SNPs (rs1531289, rs2305948, rs1870377, rs7692791 e rs7667298) e o desenvolvimento da endometriose em mulheres chinesas, observando uma associação negativa na presença do alelo 1192T (rs2305948).1717 Kang S, Shi YY, Li Y, Wang N, Lu YC, Zhou RM, Zhao XW. No entanto, este estudo foi excluído desta revisão, já que os controles incluídos realizaram apenas a ultrassonografia para diagnóstico negativo de endometriose e este método de imagem não descarta a presença de endometriose pélvica.4444 Menakaya U, Reid S, Infante F, Condous G. Além disso, até o presente momento, não existem dados na literatura que avaliem de forma combinada SNPs nos genes VEGF e KDR e o desenvolvimento da endometriose.

Os resultados controversos observados nos estudos do tipo caso-controle podem ser explicados pelo tamanho amostral, pelas diferenças metodológicas delineadas em cada estudo, principalmente, pelo tipo de controle elegível, pela diferença da frequência dos SNPs nas diferentes populações e pela utilização da medida de associação, razão de chances (OR) não ajustada, utilizada para avaliar a magnitude de associação de SNPs nos genes VEGF e KDR no desenvolvimento da endometriose. As técnicas de estratificação e regressão múltipla são dois métodos estatísticos, que podem ser utilizados para excluir possíveis variáveis de confusão e, consequentemente, produzir ORs ajustados.4646 Fuchs SC, Paim BS. Os estudos epidemiológicos analíticos observacionais do tipo caso-controle são aplicados para avaliar exposições entre grupos semelhantes, que apresentam (casos) ou não (controles) a doença.5050 Schulz KF, Grimes DA. Além disso, os estudos do tipo caso-controle podem contribuir para encontrar achados importantes e eficientes em termos de tempo relativamente curto com poucos recursos financeiros.4848 Rego MAV. Entretanto, estudos do tipo caso-controle tendem a ser mais suscetíveis a vieses que outros desenhos analíticos epidemiológicos e a forma como os indivíduos são recrutados para o estudo pode distorcer a estimativa da medida de efeito.5151 Rêgo MAV. A definição do grupo caso é um ponto crítico em um estudo caso-controle, sendo os critérios de diagnóstico e de elegibilidade, dois aspectos fundamentais para a seleção dos indivíduos. As principais limitações dos estudos do tipo caso-controle são: a falta de validade devido ao viés de seleção; o recrutamento de controles não representativos da população sob risco; erros de medidas de associação, em função dos diferentes vieses de lembrança entre casos e controles e as variáveis de confusão que falseiam uma associação causal.5050 Schulz KF, Grimes DA. Diferenças no padrão da frequência alélica podem ser observadas em estudos com grupos étnicos heterogêneos, assim como fatores ambientais contribuem para diferentes respostas fenotípicas.5959 Kennedy S: Genetics of endometriosis: a review of the positional cloning approaches. Além disso, deve-se levar em conta o tamanho amostral para evitar erros do tipo I, afirmando que o fator de risco está associado com a doença, mas, de fato, não está; ou erros do tipo II, afirmando que o fator de risco não está associado com a doença, mas, de fato, está.5050 Schulz KF, Grimes DA. Corroborando com esses achados, sugerimos também que o grupo controle ideal para realizar estudos do tipo caso-controle, que avaliam o perfil genético de mulheres com endometriose, deve ser formado por mulheres submetidas à laqueadura, já que é possível fazer a inspeção direta da cavidade pélvica e excluir a presença das lesões endometrióticas.

Este trabalho de revisão teve o intuito de descrever e discutir os critérios de inclusão/exclusão utilizados para definir o grupo de casos com endometriose e, principalmente, dos controles, com o intuito de verificar diferenças metodológicas que pudessem contribuir para as discrepâncias dos resultados encontrados na literatura. Os estudos analíticos observacionais do tipo caso-controle, quando são cuidadosamente delineados, podem fornecer informações relevantes para a identificação dos fatores de risco, especialmente no caso da endometriose, que é uma doença complexa e multifatorial. As análises epidemiológicas que exploram o perfil molecular das mulheres com endometriose podem contribuir para as investigações etiológicas e para construir um instrumento para as ações e intervenções no âmbito da saúde pública.

Em conclusão, sugerimos que estudos mais abrangentes, com delineamento metodológico correto, envolvendo diferentes populações, assim como a análise combinada de SNPs nos genes VEGF e/ou KDR, ainda são necessários em virtude do papel fundamental desses fatores no desenvolvimento da endometriose. Sugerimos que a seleção de um grupo controle de base hospitalar, sem histórico de infertilidade e distúrbios ginecológicos, como é o caso das mulheres submetidas ao procedimento de laque-adura, corresponde ao tipo de controle adequado para avaliar a magnitude de associação de SNPs nos genes VEGF e/ou KDR no desenvolvimento da endometriose.
