Pancreatite aguda e COVID-19: uma revisão integrativa da literatura

RESUMO

Os primeiros casos da doença COVID-19 foram identificados no final de 2019 na China, mas não foi necessário muito tempo para que se tornasse pandêmica. Acreditava-se, a princípio, que ela fosse restrita apenas a sintomas respiratórios, até que manifestações extrapulmonares fossem mundialmente relatadas. Quadros de pancreatite aguda concomitantes ao diagnóstico de infecção por SARS-CoV-2 vêm sendo observados em alguns pacientes, na ausência das etiologias mais comuns descritas na literatura. Postula-se que a presença do receptor viral ECA-2 no pâncreas seja responsável pelo dano celular direto e que o estado hiperinflamatório da COVID-19 favoreça o desenvolvimento da pancreatite por mecanismo imunomediado. Este estudo teve como objetivo analisar a correlação entre pancreatite aguda e a doença COVID-19 como um provável fator de causalidade. Realizou-se uma revisão integrativa da literatura, foram incluídos estudos publicados entre janeiro de 2020 e dezembro de 2022 que trouxessem dados acerca de pacientes diagnosticados com pancreatite aguda conforme a Classificação de Atlanta revisada com diagnóstico confirmado de COVID-19 no mesmo período. Um total de trinta estudos foram revisados. Aspectos demográficos, clínicos, laboratoriais e de imagem foram analisados e discutidos. Acredita-se que o SARS-CoV-2 foi o responsável pelo desenvolvimento de pancreatite aguda nestes pacientes, devido à ausência de demais fatores de risco precipitantes, bem como à estreita relação temporal entre ambos. Uma atenção deve ser dada às manifestações gastrointestinais em pacientes acometidos pela COVID-19.

ABSTRACT

The first cases of the COVID-19 disease were identified in late 2019 in China, but it didnt take long for it to become pandemic. At first, it was believed that it was restricted to respiratory symptoms only, until extrapulmonary manifestations were reported worldwide. Acute pancreatitis concomitant with the diagnosis of SARS-CoV-2 infection has been observed in some patients, in the absence of the most common etiologies described in the literature. It is postulated that the presence of the ECA-2 viral receptor in the pancreas is responsible for the direct cellular damage and that the hyperinflammatory state of COVID-19 favors the development of pancreatitis through an immune-mediated mechanism. This study aimed to analyze the correlation between acute pancreatitis and COVID-19 disease as a probable causality factor. An integrative literature review was carried out, including studies published between January 2020 and December 2022 that brought data on patients diagnosed with acute pancreatitis according to the revised Atlanta Classification with a confirmed diagnosis of COVID-19 in the same period. A total of thirty studies were reviewed. Demographic, clinical, laboratory and imaging aspects were analyzed and discussed. It is believed that SARS-CoV-2 was responsible for the development of acute pancreatitis in these patients, due to the absence of other precipitating risk factors, as well as the close temporal relationship between both. Attention should be given to gastrointestinal manifestations in patients affected by COVID-19.

INTRODUÇÃO

Assim como o relato deste último paciente, outros autores também observaram a ocorrência de pancreatite aguda em pacientes infectados pelo SARS-CoV-2 na ausência de fatores de risco conhecidos - colelitíase, alcoolismo, hipertrigliceridemia e outros99 Mederos MA, Reber HA, Girgis MD. Particularmente, os agentes infecciosos são responsáveis por cerca de 10% dos casos, dentre as causas virais nota-se o vírus coxsackie, hepatite B, citomegalovírus (CMV), vírus da imunodeficiência humana (HIV), herpes simples (HSV), caxumba e varicela-zoster1010 Rawla P, Bandaru SS, Vellipuram AR. Percebeu-se, por um estudo multicêntrico internacional de caráter prospectivo, que pacientes com pancreatite aguda e infecção por SARS-CoV-2 concomitantes, não só estão sujeitos a desfechos clínicos piores (aumento da gravidade da pancreatite, tempo de internação e falência de órgãos) como também apresentam um índice de mortalidade significativamente maior em 30 dias15.

OBJETIVO

Em virtude disso, nosso estudo tem como objetivo analisar a correlação entre pancreatite aguda e a doença COVID-19 como um provável fator de causalidade, na ausência das demais etiologias classicamente conhecidas..

MÉTODOS

Trata-se de uma Revisão Integrativa da Literatura executada em seis etapas: identificação do problema com elaboração de pergunta de norteadora, pesquisa de literatura, avaliação e análise criteriosa dos dados e, por fim, apresentação da revisão com seus resultados e limitações1616 Souza MT, Silva MD, Carvalho R. A busca foi realizada na base de dados PubMed e os descritores utilizados foram “Acute Pancreatitis” AND “adults” “COVID-19”, todos disponíveis no MeSH e DeCS.

Critérios de elegibilidade

Os critérios de inclusão foram: estudos inéditos prospectivos ou retrospectivos, relatos e séries de casos, publicados entre 1º de janeiro de 2020 a 31 de dezembro de 2022, de língua inglesa, que apresentassem casos e dados acerca da pancreatite aguda em adultos associada à COVID-19 confirmada laboratorialmente e/ou radiologicamente. Sendo o diagnóstico de pancreatite necessariamente conforme a Classificação de Atlanta revisada1717 Banks PA, Bollen TL, Dervenis C, et al. , com a presença de pelo menos 2 dos seguintes critérios: 1) dor abdominal consistente com pancreatite aguda (início agudo de dor epigástrica intensa e persistente, muitas vezes irradiando para as costas); 2) nível sérico de lipase ou amilase três ou mais vezes o limite superior do normal; 3) achados de imagem característicos de pancreatite aguda na tomografia computadorizada (TC), ressonância nuclear magnética (RNM) ou ultrassonografia (USG).

Foram excluídos: quadros de pancreatite aguda em crianças (<18 anos), estudos sem texto completo disponível ou pré-publicações e aqueles que não especificaram durante o relato ou metodologia o diagnóstico de pancreatite aguda baseado na Classificação de Atlanta. de pancreatite foram descartados, são eles: litíase biliar, histórico de abuso de álcool, hipertrigliceridemia (>1000mg/dL), hipercalcemia, infecções virais exceto o SARS-CoV-2, pacientes submetidos a procedimentos invasivos recentes como CPRE (colangiopancreatografia retrógrada endoscópica), pancreatite autoimune (níveis elevados de IgG4), pacientes com pancreatite aguda recorrente ou com história familiar que elevasse a suspeita de pancreatite hereditária, abuso de medicamentos, causas estruturais como pâncreas divisum, tumores ou lesões císticas próximas.

De início, foram identificados 94 estudos. Após a leitura dos títulos e resumos, excluiu-se 49 deles, restando 45 pré-selecionados para análise do texto na íntegra. Após a leitura integral, foram descartados 15 e a amostra final foi composta por 30 estudos eleitos para extração de dados e construção da revisão.

A Tabela 1 descreve o perfil demográfico geral (idade e sexo) de 31 pacientes dos relatos de caso e cartas, bem como sintomatologia, níveis das enzimas pancreáticas, presença ou não de achados característicos de pancreatite em exames de imagem e desfecho (recuperado - se alta hospitalar ou descrição de melhora clínica - ou óbito). A Tabela 2 sintetiza os principais resultados dos estudos maiores e a metodologia utilizada.

Um paciente em específico também recebeu diagnóstico de infecção por SARS-CoV-2 em um primeiro internamento, mas evoluiu com sonolência e foi reinternado dois dias após a alta; o diagnóstico de pancreatite aguda, por sua vez, deu-se por meio de exames laboratoriais e alterações em imagem, visto que não houve relato de sintomas gastrointestinais típicos36.

No 6º dia de internamento, a paciente gestante cursou com ruptura precoce de membranas e posteriormente evoluiu para trabalho de parto ativo. O paciente em que foi visualizado o pseudoaneurisma de artéria pancreaticoduodenal foi inicialmente tratado de forma conservadora. Entretanto, devido sua deterioração clínica, a embolização endovascular foi proposta como a melhor opção terapêutica, ele seria encaminhado para outro hospital para realizá-la até que um sangramento retal maciço ocorreu repentinamente. Apesar de todos os esforços para estabilizá-lo, foi a óbito por choque hipovolêmico. DISCUSSÃO

Foi possível resumir os dados atualmente disponíveis acerca do desenvolvimento de pancreatite aguda concomitante à COVID-19, descrevendo demografia, apresentação clínica, achados laboratoriais e de imagem, condutas e desfechos. Embora não haja evidências comprovadamente claras que a infecção por SARS-CoV-2 seja responsável por quadros de pancreatite aguda, os resultados obtidos a partir desta revisão corroboram com tal hipótese em virtude da ausência dos fatores de risco mais comuns, bem como estreita relação temporal entre ambos.

A incidência geral de pancreatite aguda é de 110 para 140 por 100.000 habitantes99 Mederos MA, Reber HA, Girgis MD. Acute Pancreatitis: A Review. JAMA. 2021;325:382-90. doi: 10.1001/jama.2020.20317.

. A maioria desses pacientes apresenta pancreatite aguda leve, que é autolimitada e geralmente se resolve em uma semana. . Poucos estudos trouxeram a estratificação de gravidade de seus pacientes, portanto, não achamos adequado inclui-la nos resultados.

As etiologias responsáveis por pancreatite aguda incluem: cálculos biliares e álcool principalmente, além de hipertrigliceridemia, hipercalcemia, causas iatrogênicas, infecções virais, bacterianas e fúngicas, fator hereditário, autoimune, medicamentoso e fatores anatômicos-estruturais99 Mederos MA, Reber HA, Girgis MD. Um dos estudos incluídos, realizado por Gubatan et al., comparou um grupo de pacientes com “pancreatite sem COVID-19” a outro com “pancreatite e COVID-19”, viu-se que a etiologia biliar e alcoólica foram as mais comuns no primeiro - semelhantemente à população em geral, enquanto que “pancreatite idiopática” curiosamente esteve presente em 69% dos casos no segundo grupo46.

Até o momento, postula-se que a lesão pancreática associada à COVID-19 seja oriunda de dois mecanismos fisiopatológicos principais: efeito citopático direto e resposta imune desregulada.

. Isso explicaria o tropismo viral pelo pâncreas e o mecanismo de lesão direta.

Uma outra explicação fisiopatológica acerca do desenvolvimento da pancreatite aguda, não excludente da anterior, refere-se ao estado hiperinflamatório em que pacientes acometidos pela COVID-19 estão inseridos. A resposta inflamatória sistêmica e exacerbada desencadeada pelo SARS-CoV-2 resulta na denominada tempestade de citocinas, com produção descontrolada de citocinas pró-inflamatórias como IL-6, TNF-α e quimiocinas, levando à falência de múltiplos órgãos, inclusive pancreática1111 Gupta V. Particularmente, o estudo de Gubatan et al. só considerou pancreatite aguda naqueles pacientes que preenchessem os três critérios, o que pode ter subestimado a taxa de pancreatite em sua amostra, já que o diagnóstico requer o mínimo de dois deles. . O estudo Pan et al. viu que 103 pacientes (50,5%) internados apresentaram um ou mais sintomas digestivos. Dentre eles, 97 desenvolveram sintomas respiratórios associados a sintomas digestivos e seis apresentaram apenas sintomas digestivos na ausência de sintomas respiratórios. A dor abdominal esteve presente na maioria dos relatos, infelizmente foi pouco detalhada por alguns no que se refere à localização, intensidade e irradiação. Dor abdominal, náuseas, vômitos e diarreia foram observados em quase 90% dos pacientes que tiveram pancreatite no estudo de Akarsu et al.4343 Akarsu C, Karabulut M, Aydin H, et al. Sabendo disso, é importante que as equipes de saúde estejam alertas às manifestações gastrointestinais dessa doença, pois podem ser indicativas de um acometimento multissistêmico, sobretudo pancreático, bem como reconheçam que um dos diagnósticos prováveis aos quadros abdominais agudos isolados pode ser uma infecção por SARS-CoV-2, mesmo na ausência de sintomas respiratórios associados.

Todavia, a dor abdominal típica pode não estar presente ou muito menos clara, como no caso do paciente sob efeito de sedação3939 Kurihara Y, Maruhashi T, Wada T, et al. O segundo critério diagnóstico consiste em atividade da lipase ou amilase sérica pelo menos três vezes maior que o limite superior da normalidade1717 Banks PA, Bollen TL, Dervenis C, et al. . Este foi um critério presente de forma majoritária dentre os pacientes. A lipase tem uma sensibilidade ligeiramente maior para detecção de pancreatite aguda, sua elevação ocorre mais cedo e dura mais tempo que a elevação da amilase4949 Greenberg JA, Hsu J, Bawazeer M, et al. Valores séricos normais de lipase são incomuns na prática clínica, contudo, a paciente relatada por Hatch-Vallier et al., por exemplo, possuía nível normal de lipase e seu diagnóstico foi obtido pela apresentação de dor epigástrica característica e achados radiológicos2727 Hatch-Vallier B, Jarodiya V, Hawa F, et al. Por outro lado, o aumento isolado de enzimas pancreáticas durante a COVID-19 é bastante questionável e tem sido discutido pela comunidade científica. Um estudo realizado por Wang et al. observou uma incidência de 17% de lesão pancreática entre 52 pacientes infectados pelo SARS-CoV-2; eles definiram “lesão pancreática” como qualquer anormalidade no nível de amilase ou lipase5050 Wang F, Wang H, Fan J, et al. Um exemplo claro foi visto em uma coorte de 110 pacientes diagnosticados com COVID-19, em que 24,5% deles apresentaram níveis aumentados de amilase e 16,4% níveis aumentados de lipase, mas apenas um paciente possuía níveis acima de três vezes o limite superior de normalidade e nenhum desenvolveu sinais clínicos ou radiológicos de pancreatite aguda5252 Pezzilli R, Centanni S, Mondoni M, et al. Sendo assim, ressaltamos que essa alteração laboratorial deve ser valorizada e serve como alerta durante o curso da pneumonia, mas não é, por si só, definidora de pancreatite aguda. Para isso, os demais critérios devem ser avaliados.

O terceiro e último está relacionado a achados condizentes com pancreatite aguda em tomografia computadorizada com contraste (mais comum), ressonância magnética ou ultrassonografia1717 Banks PA, Bollen TL, Dervenis C, et al. . É certo que a pancreatite pode ser diagnosticada em cerca de 80% dos pacientes com base apenas na presença de dor abdominal clássica e enzimas pancreáticas elevadas. Percebe-se que a coexistência de infecção por SARS-CoV-2 e pancreatite aguda podem ir além da provável causalidade, mas são fatores que se retroalimentam no que se refere à disfunção orgânica. Dessa forma, apesar de ainda não existirem dados robustos que defendam veementemente o SARS-CoV-2 como fator precipitante de quadros de pancreatite, as evidências acumuladas até então vêm favorecendo tal hipótese. As equipes de saúde devem estar atentas às repercussões gastrointestinais deste vírus, uma vez que a pancreatite aguda pode alternar de uma doença leve e autolimitada, que requer primordialmente medidas de suporte, a uma doença grave com complicações ameaçadoras à vida.

Aplicabilidade

Os resultados obtidos a partir dessa revisão propõe-se a advertir a comunidade médica sobre o provável acometimento pancreático decorrente do SARS-CoV-2, acrescentando evidências sobre seu cunho sistêmico e não apenas pulmonar. Tivemos o rigor de selecionar apenas estudos com o diagnóstico de pancreatite aguda conforme a Classificação de Atlanta revisada e não somente com alterações de enzimas pancreáticas. Além disso, excluímos relatos cujos pacientes apresentassem alguns dos principais fatores de risco conhecidos. Isso torna nossos resultados ainda mais confiáveis.

Limitações

Esta revisão possui inúmeras limitações. Devido à natureza do estudo, não é possível estabelecer uma relação causal entre a COVID-19 e o surgimento de pancreatite aguda. Uma revisão sistemática não foi realizada e, apesar da revisão integrativa permitir a inclusão de estudos de naturezas variadas1616 Souza MT, Silva MD, Carvalho R. Revisão integrativa: o que é e como fazer. Einstein. 2010;8:102-106. doi: 10.1590/s1679-45082010rw1134.

, isso abre caminho à heterogeneidade metodológica e estatística. A maioria das publicações ainda é restrita a relatos de caso e, infelizmente, a quantidade e qualidade de dados fornecidos acerca de história médica, investigação laboratorial e radiológica foi variável entre eles.

Logo, é necessário que estudos futuros sejam realizados a fim de investigarem de maneira mais minuciosa o mecanismo fisiopatológico associado. Uma atenção também deve ser dada ao início de sintomas gastrointestinais e seu intervalo de tempo em relação ao teste de COVID-19, bem como a achados laboratoriais e de imagem mais comuns. Recomendamos uma investigação mais abrangente dos fatores de risco predisponentes durante o atendimento destes pacientes e um melhor detalhamento de cada caso nas publicações vindouras.

CONCLUSÃO

Assim, os resultados provenientes desta revisão corroboram com a hipótese de que a infecção por SARS-CoV-2 possa ser responsável por quadros de pancreatite aguda em determinados pacientes, desde que afastadas as demais etiologias conhecidas. As justificativas fisiopatológicas incluem lesão pancreática por efeito citopático direto do vírus e por resposta imunomediada às custas do estado hiperinflamatório da pneumonia. Entretanto, dados mais robustos, capazes de estabelecer de fato uma relação causal entre ambos, devem ficar a cargo de estudos multicêntricos futuros.
