This paper describes the planning of the Home Assistance Program (H.A.P.) to be established at USP University Hospital. The object of the H.A.P. will be to give muitiprofesssional home care to patients with a pre-established profile, based on a humam holistic vision and considering the positive influence of the family environment. lt describes the criteria for accepting a patient in the program, presents a flowchart and the procedures for home care. It presents the nurse care proposal, the tools to be used in the interview and home visit, planning of humam resources and materials needed for rendering this care.
