A case of human loxoscelism occurred in São Paulo City was submitted to exchange transfusion procedure associated with specific heterologous serotherapy and urinary alkalization. The exchange transfusion procedure was propo sed to be employed in this case, because of intensive haemolysis associated with severity and imminent death risk.
