Avaliação da terapêutica, evolução e fatores de risco para ulcera de córnea em serviço universitário

Therapeutic analysis, evolution and risk factors for cornea ulcer in university hospital

RESUMO

Os autores estudaram 20 casos consecutivos de úlcera de córnea de origem bacteriana, atendidos no HC-Unicamp. Ressaltam a gravidade dos casos, todos com acuidade visual menor que 0,2 e somente 25% evoluindo sem complicações graves. Estudam o tratamento prévio instituído, em que nenhum paciente tinha exames laboratoriais nem usava colírio concentrado. Enfatizam que apesar de 60% terem sido examinados por oftalmologistas, 45% tinham conduta não convencional (20% sem medicação, 15% não sabendo o que usava, 5% com coIírio anestésico e 5% com adstringente ocular). Encontraram 55% dos pacientes usando antibiótico, sendo que, quatro com antibiótico tópico e os demais usavam antibióticos em associação com corticosteróide, antiviral e/ou antifúngico. Ressaltam ainda serem 90% homens e 40% trabalhadores rurais, sendo os fatores desencadeantes mais freqüentes: trauma (40%), alcoolismo (15%), herpes (10%), lente de contato (10%) e glaucoma (10%).

Concluem pela necessidade de medidas preventivas serem adotadas nos grupos de maior risco, associadas a melhor divulgação entre os oftalmologistas e clínicos gerais da conduta mais atualizada no tratamento de úlcera de córnea.

SUMMARY

Authors studied 20 cases of cornea ulcer in the Hospital das Clínicas da UNICAMP. They emphasize the importance of the cases all with visual acuity less than 0,2 and only 25% without future complications. They analize previous treatment with cases neither using fortified antibiotic eye drops nor having laboratory eye examination. They emphasize that though 60% had an ophthalmologic examination 45% had no convencional examination (20% without medication 15% not aware about the medication used, 5% with anaestesic eyedrop and 5% with adstringent). They found 55% of the patients were using antibiotics, 20% only one antibiotic, and the others using association of antibiotics with corticosteroids, anti-fungic or anti-viral. They found 90% were male and 40% were rural workers, and the risk factors more frequency: trauma (40%), alcoholism (15%), herpes (10%), contact lens (10%) and glaucoma (10%).

They finnaly call for the necessity of preventive measures for the risk group in addition to better divulgation between the ophthalmologists of the most up-to-date treatment for corneal ulcers.
