Descolamento exsudativo da retina sem sinais de retinopatia hipertensiva severa na toxemia da gestação⁺

Exudative retinal detachment without hipertensive retinopathyin toxemia of the pregnancy

RESUMO

A toxemia caracteriza-se por hipertensão durante o período gestacional associada à proteinúria e edema. A hipertensão arterial sistêmica pode ser crônica ou pode se iniciar durante a gestação, geralmente no terceiro trimestre.

São relatados, neste estudo, 7 casos de descolamento exsudativo de retina em pré-eclampsia severa, sendo 1 dos casos associado à hemorragia subretiniana. Todos evoluíram para melhora do descolamento após o parto e controle da hipertensão arterial sistêmica.

Todas as pacientes tiveram acuidade visual de 20/25 ou melhor em ambos os olhos, bem como ausência de alterações do campo visual após 3 meses de seguimento.

O estudo angiográfico demonstrou anormalidade da vascularização coroidéa sem sinais de comprometimento severo da vascularização retiniana, sugerindo o envolvimento da coriocapilar na fisiopatogenia do descolamento exsudativo da retina.

SUMMARY

The toxemia of pregnancy is characterized by high blood pressure, generalized edema and proteinurea. The high blood pressure may be chronic or may start during the pregnancy, generally in the last trimester.

We report 7 cases of exudative retinal detachment in severe pre-eclamptic state. One of this cases is associated with subretinal haemorrhage. All cases improved their retinal detachments after delivery and high blood pressure control.

All patients had 20/25 or better VA and no alteration of visual field after 3 months follow up.

The angiographic study demonstrated choroidal vasculature abnormalities, without severe hipertensive retinopathy. These findings suggest that the pathogenesis of exudative retinal detachment is probably due to choricapilaris alteration.
