World Health Organization and Panamerican Health Organization recomendations for the structuration of medical assistance systems, were pointed out. So two large systems, direct and indirect ones, were analysed, with profits and disprofits of both of them, for general medicine, specialized medicine, hospitalization, odontological and drug assistance and preventive activities.
