This paper discourses on an aspect little studied on medical attendance, i.e., the role of non-medical personnel in warranting necessary activities to perform professional services. The confluence of the organizational social interaction with the constraints of the processing of cognitive information causes "noise" in institutionalized environments. The limits of both attention and memory have always influenced the capacity of participants in understanding mutual communication. It is approached, then, the way by which elements of the organizational discourse and local artifacts, constrained by cognitive mechanisms, will help in shaping the creation and usage of both individual and collective representations.
