A new technique of posterior chamber IOL fixation: report of 22 cases

RESUMO

É descrita uma nova técnica de implante de L.I.O. de Câmara Posterior em pacientes sem suporte de cápsula posterior (Técnica de Fixação Escleral de Implante de Câmara Posterior).

Os autores descrevem e detalham a técnica em que fixam as alças da lente intra-ocular de câmara posterior nas posições 3:00 e 9:00 horas da escIera a 1,0 mm do limbo. O objetivo é posicionar as alças a nível de sulco ciliar.

Usando a abertura da técnica de facectomia convencional, realiza-se vitrectomia na região da câmara anterior e na cavidade vítrea anterior quando necessária.

Descreve-se um procedimento para prevenir a inclinação da L.I.O. em relação ao eixo visual, pelo reflexo da luz do microscópio sobre o corpo da lente intra-ocular.

São relatados 22 casos com acuidade visual inicial variando de projeção luminosa até dedos a 3 metros, sem correção óptica. Os resultados finais, após a cirurgia, variam entre 20/100 a 20/20 para longe e J4 até J1 para perto com correção óptica.

O segmento pós-cirúrgico deu-se entre 3 e 15 meses.

SUMMARY

A new technique of posterior chamber IOL implantation is reported in patients without posterior capsule support (Scleral Fixation Technique of Posterior Chamber IOL Implantation).

The authors describe and detail the technique that the IOL loops are fixed in the posterior chamber at 3:00 and 9:00 hours in the sclera 1,0 mm close to the limbus. The aim is to position the loops in the cilliary sulcus.

Using the conventional cataract extraction technique, vitrectomy performed in the anterior chamber and in the anterior vitreous cavity when necessary.

A procedure is descricribed to prevent the IOL tilting, using the microscope light reflex on the IOL body.

22 cases are reported with initial visual acuity ranging from light perception to counting fingers at 3 meters, without optic correction to final results after the surgery ranged from 20/100 to 20/20 and J4 to J1 with optical correction.

The post surgical follow-up were between 3 and 15 months.
